Abstract:
The paper is focused on the impact of land use changes on water regime. First, an emphasis was given to what extent the main components of the water balance on the experimental catchment Všeminka (region Vsetínské Hills) were influenced. For this reason, the WBCM-5 model was implemented for the period of 10 years in a daily step with a particular reference to simulate the components of direct runoff and of subsurface water recharge. In the selected years of the period 1990–2000, the major changes were made in land use and also the significant fluctuation of rainfall-runoff regimes were observed (e.g. dry year 1992 and flood year 1997). After WBCM-5 parameter calibration it was found that some water balance components can change in relation to substantial land use changes even up to tens of percent in a balance-consideration, i.e. in daily, monthly and yearly or decade values, namely the components of interception and also of direct runoff and of subsurface water recharge. However, a different situation appears when investigating significant short-term rainfall-runoff processes. There were about seven real flood events analysed using the model KINFIL-2 (time step 0.5 hr) during the same period of about 10 years on the same catchment. Furthermore, some land use change positive or negative scenarios were also analysed there. As opposed to long-term water balance analyses, there was never achieved any greater differences in the hydrograph peak or volume than 10%. Summarising, it is always important to distinguish a possible land use change impact in either long-term balance or short-term runoff consideration, otherwise a misunderstanding might be easily made, as can often be found when commenting on the impact on floods in some mass media.

BackgroundWe sought to determine the extent and appropriateness of benzodiazepine use in an elderly community, by measuring prevalence and incidence of benzodiazepines and examining mental health status as a predictor of benzodiazepine use.MethodData were collected from two longitudinal studies of people from the same community, sampled in 1982–1983 and again in 1989–1991.ResultsBenzodiazepine prevalence did not decrease during the period under study, but there was a significant reduction in anxiolytic use. Prevalence of benzodiazepines in women is twice that in men, and incidence of hypnotics is slightly higher in women. Prevalence and incidence of hypnotics are strongly associated with increasing age. There were high proportions of long-term users (61 and 70%), and continued use was high (52%) among new users. A large proportion of benzodiazepine use was by those who were concurrently depressed. Similarly, anxiety predicted both current and subsequent use of hypnotics.ConclusionsMany older people still use benzodiazepines, contrary to official guidelines with regard to their mental health. Our findings add to the weight of opinion that persistent and long-term use should be discouraged.

Although the level of glycated hemoglobin (HbA1C) reflects chronic glycemic control, treatment-induced decreases in HbA1Cin patients who have established diabetes do not always predict beneficial clinical outcomes. Clinical outcomes are dramatically influenced by the history of previous glycemic control, the extent of current clinical complications, and the side effects of therapeutic agents. Rational approaches to the intensity of glycemic control in individual patients should take these factors into consideration, as well as in setting an appropriate goal for the HbA1Ctarget.

Diabetes and tuberculosis (TB) have existed for thousands of years. Today, the global burden of disease from diabetes and TB is huge and, in the case of diabetes, rapidly increasing. Recent systematic reviews show that diabetes is associated with an increased risk of TB, yet the potential public health and clinical importance of the association seems to be largely ignored. Irrespective of whether the association is causal or a result of co-morbid factors, in low-resource societies with a dual disease burden, can a common health system approach for diabetes and TB be adapted to address prevention and care? How and to what extent can this be done? Good-quality implementation research is urgently needed to create robust action plans to address this double burden.

This study analyzes the effects of several post-adoption behaviors (extent of use, routinization and infusion) on overall performance in using an Electronic Document Management System (EDMS). Furthermore, the authors test whether the routinization and infusion variables mediate the influence of the extent of use on overall performance. This research collects data from a survey answered by 2,175 employees (EDMS users) of Portuguese municipalities. The Partial Least Squares technique is applied to test the model. The results showed that routinization is directly predicted by the extent of use, whereas infusion is directly affected by the extent of use and also by routinization. Consequently, such post-adoptive behaviors are interrelated not only in a sequential process, but also in parallel, meaning that the infusion state of EDMS use is achieved from the evolutionary process of EDMS use and also directly from each of the prior stages of EDMS use. In addition, overall performance is directly influenced by routinization and infusion, signifying that the more employees use the EDMS to its fullest extent, the more likely it is for the overall performance to improve. Finally, an indirect effects analysis shows that routinization and infusion mediate the relationship between extent of use and overall performance.

The METRIC energy balance model uses an auto-selection approach for identifying hot (dry, bare soil) and cold (fully transpiring crop) anchor pixels for the internal calibration of the model. When an unmanned aerial system (UAS) is used for imagery, the small image size and the varying crop and soil water status of agricultural fields make the identification of reliable hot and cold pixels challenging. In this study, we used an experimental spearmint field under three irrigation levels (75%, 100%, and 125% of crop evapotranspiration, ETc). As a way of providing diverse field conditions, six different extents (Extent 1 to Extent 6) were selected from each day of the seven days of UAS imagery campaigns of the same field for generating UAS-based ETc maps using auto-selection of hot and cold anchor pixels for the internal calibration of the model. Extent 1 had the smallest coverage area of the field, including only plants that were irrigated at 75% of ETc, while the fields of view of the other extents increased to where the Extent 6 covered the spearmint field and all the surroundings including trees, a nearby water canal, irrigated grass, and irrigated and non-irrigated soil. The results showed that different sizes of extent resulted in the selection of variable hot (bare, but moist soil in small extents, and dry bare soil at the larger extents) and cold anchor pixels (crop under water stress at the small extents, and tree canopy or grass alongside the water canal at the larger extents). This variation resulted in significantly different ETc estimation for the same spearmint crop field, indicative of a potential limitation for the use auto-selection of hot and cold pixels when using the UAS-METRIC model.

This study aims to investigate the extent at which undergraduate students use the internet for reading. The study specifically, (i) Examine the extent at which undergraduate students use the internet for reading, (ii) Investigate the difference on the extent of the use of internet for reading of male and female undergraduate students, (iii) Examine the difference of extent of the internet usage on reading of undergraduate students based on areas of specialization. The study was a descriptive research of survey type. A structured questionnaire designed by the researcher was used in data collection. A simple random sampling technique was used to select 150 respondents that were involved in this study from the University of Ilorin, Ilorin, Nigeria. Frequency count, mean and percentages were used to answer the research questions. Hypothesis one was tested with independent t-test while hypothesis two was tested using Analysis of Variance (ANOVA). All hypothesis were tested at 0.05 level of significance. The findings established that Undergraduates always use the internet for reading online news, E books, Email, Health information, Jokes, Comic strips, Fashion, Food/Nutrition and Sales information. Also, there was no significant difference between the extent of the internet usage on reading of undergraduate students based on gender and areas of specialization. It was however recommended that there is a need for extensive training program organized at regular interval so that all categories of users can improve their efficiency in the use of the internet.

Background: Medical students are more susceptible to stress associated with long working hours, arduous studies, and living alone away from the home, which may affect their psychological well-being. They may indulge in the use of internet and pornography to ameliorate their stress. Internet is one of the main sources of pornographic content as it is easily available, affordable, and accessible. Objective: The current study seeks to quantitatively estimate the extent of internet addiction and pornography use among medical students in India, and its effect on their general well-being. Methods: We conducted a cross-sectional study in a medical college in North India involving undergraduate (including interns) and postgraduate medical students. Those using internet for porn use were assessed using a semi-structured sociodemographic questionnaire, General Health Questionnaire 30, WHO Quality of Life Index (WHOQOL)-BREF, and Internet Addiction Test (IAT). Results: A total of 393 medical students who agreed on their pornography internet usage were recruited for the study. Problematic pornography internet usage (moderate and 0severe IAT scores) was seen in 11% (80) of the participants. Male and married students were more likely to be problematic users. The mean duration of pornography use was 8.02±6.32 h/week. The time spent on pornography use positively correlated with IAT and General Health Questionnaire scores, whereas it negatively correlated with WHOQOL-BREF scores. Conclusions: Internet pornography use is significantly prevalent amongst medical students in India. As the duration of pornography use increases, general health measures and quality of life decrease, which may impact their well-being and academic performance.

Off-label drug use among hospitalized children: identifying extent and natureObjective of the study was to determine the extent and nature of medicinal products used off-label in hospitalized children and to highlight the therapeutic areas with the highest need. Over a 4 week period, data about prescribed drugs to patients under 18 years of age at two paediatric wards in Bratislava (Unit for Infants and Unit for Older Children) were recorded and the drug-licensing status according to valid Summary of Product Characteristics (SPC) was identified. In total, 206 medicinal products containing 70 different generic substances were given to 49 paediatric patients at the Unit for Infants. Of these, 22% were evaluated as off-label. The highest rates were established among the drugs used for diseases of respiratory system and diseases of digestive system. The 68 children had been given at the Unit for Older Children 267 medicinal products resulting in 97 generic substances. In 15.7% of cases medicines were used in off-label manner, mainly due to unapproved paediatric indication and age. This pilot study provided a preliminary survey on drugs administered off-label to paediatric patients in Slovak Republic.

Background Surgical resection is the key to management of thyroid cancer, but determining the optimal surgical procedure for individual cases has been controversial. Methods The author reviews several large data bases that allow examination of prognostic criteria for long-term outcomes. Results Patients can be classified into good- or poor-risk groups that assist defining the optimal surgical procedure. Routine use of total thyroidectomy in all patients with thyroid cancer is best avoided; however, patients with medullary cancer generally need total thyroidectomy. Conclusions The definition of risk groups has clarified the options regarding choice of primary surgical therapy for differentiated thyroid cancer.

Adolescent suicidal behaviors and substance use are disturbingly common. Research suggests overlap of some of the etiological mechanisms for both adolescent suicidal behavior and substance use, yet clear understanding of the complex relations between these behaviors and their causal underpinnings is lacking. A growing body of evidence and a diathesis model (Mann et al. 1999; Mann, 2003) highlight the importance of impulse control as a proximal risk factor for adolescent suicidal and substance use behaviors. This literature review extends current theory on the relationships between adolescent suicidal behavior and substance use by: (1) examining how, when, and to what extent adolescent development is affected by poor impulse control, stressful life events, substance use behavior, and biological factors; (2) presenting proposed causal mechanisms by which these risk factors interact to increase risk for suicidal behaviors and substance use; and (3) proposing specific new hypotheses to extend the diathesis model to adolescents at risk for suicide and substance use. More specifically, new hypotheses are presented that predict bidirectional relationships between stressful life events and genetic markers of 5-HT dysregulation; substance use behavior and impulsivity; and substance use behavior and suicide attempts. The importance of distinguishing between different developmental trajectories of suicidal and substance use behaviors, and the effects of specific risk and protective mechanisms are discussed. Use of new statistical approaches that provide for the comparison of latent growth curves and latent class models is recommended to identify differences in developmental trajectories of suicidal behavior and substance use. Knowledge gained from these prospective longitudinal methods should lead to greater understanding on the timing, duration, and extent to which specific risk and protective factors influence the outcomes of suicidal behavior and substance use. In turn, findings from these studies should inform researchers who conduct future treatment and prevention studies.

Background:Ultrasound reveals thyroid abnormalities in 18% of children and adolescents, of which, 22% are malignant. This creates a dilemma for practitioners who must distinguish lesions that require removal (high risk for malignancy) from lesions that can be observed (low risk for malignancy). Furthermore, treatment of children with differentiated thyroid cancer (DTC) is evolving. Previous treatments were based on adult protocols prescribing total thyroidectomy, lymph node dissection, and radioactive iodine (RAI) ablation for children with DTC, regardless of disease extent. This achieved excellent disease-free survival but high, and potentially avoidable, surgical and medical complications including an increase in secondary non-thyroid malignancies.Methods:This manuscript is a synopsis of cases presented during a symposium at the 2015 Pediatric Endocrine Society meeting (San Diego, CA) with recommendations based on the American Thyroid Association (ATA) management guidelines for children and adolescents with thyroid nodules and DTC.Results:The cases were selected to demonstrate application of the guidelines across a variety of pediatric patients with DTC highlighting key points of the ATA guidelines. The cases will assist practitioners in learning how to apply these guidelines to patient management.Conclusion:Treatment of children with thyroid nodules and DTC is evolving. Current guidelines emphasize the importance of surgery by experienced teams and deferral of RAI ablation for low-risk patients.

The article discusses a study conducted at Aligarh Muslim University to identify the awareness and use of Web 2.0 Tools by visually impaired students. The survey technique was employed in which structured interview method was followed to collect data from 83 visually impaired students and analysed using SPSS (Version 23). Findings of the study reveal that these students were aware about Web 2.0 Tools. Among these, 32.5% students possessed membership of these tools for more than 3 years and majority were using WhatsApp (89.2%), YouTube (87.9%), and Facebook (85.5%) on daily basis. These tools were used by students for different purposes, for instance 91.6% students utilized these tools for academic purposes, 84.3% for seeking job opportunities, and 81.9% for sharing knowledge. It also came to light that the major problem faced by visually impaired users was in the form of compatibility issues with screen readers (85.5%) and non-availability of JAWS software in all languages (66.3%). Further, the study also put forth certain suggestions for enhancing the use of Web 2.0 Tools among visually impaired students.

In 2017, New Zealand’s revised curriculum for early childhood education, Te Whāriki, expanded reference to the use of technology for teaching and learning to include digital media and related devices. This article reports findings from a doctoral study about tablet computer use among New Zealand’s four major early childhood service types: education and care centres, home-based services, kindergartens, and playcentres. Data were gathered in 2017, initially through a national survey, followed by a collective case study. Seven services participated in the collective case study which was designed to explain the results of the survey. Descriptive statistics and inferential statistics were used to analyse survey data while cross-case analysis was used to identify themes from the responses from each service in the collective case study. The results are presented according to two categories of respondents, services who classified themselves as non-users and services who were using tablet computers for teaching and learning at the time of the survey. The national survey results revealed that more than half of the services did not use tablets. Non-users’ reasons for not using tablet computers are discussed considering findings from both quantitative and qualitative phases of the study. Services who used tablets did so for a variety of reasons, including for documentation and assessment, to support children’s learning and teaching work. Qualitative data regarding policies or guidelines for staff about the use of and access to digital media, teachers’ and educators’ learning for how to use touchscreen tablets for teaching and learning, as well as services’ preferences on the facilitation of children’s tablet use are also presented. An important issue uncovered in this study was the use of personal tablets within ECE services. Among non-users, teachers and educators from more than half of home-based services and playcentres used their personally owned tablet computers, raising concerns about cybersafety and screen time. Many user services did not have formal guidelines or policies regarding tablet use. The data suggest that some services relied on the use of teachers’ and educators’ personally owned tablets. Implications arising from the findings of this study are explored, including the relevance of using digital technology for supporting distance learning and learning at home as a result of the global Covid-19 pandemic.
