Abstract:

Abstract The author presents a study of Bibliothèque National de France MS Latin 11269, a manuscript that he argues was associated with the court of Leonello d’Este and which represents an attempt to “fix” or “canonize” a vernacular work on a practical subject in erudite Latin poetry. The author reviews the life of Fiore dei Liberi and Leonello d’Este and discusses the author’s intentions in writing, how the manuscript shows clear signs of Estense associations, and examines the manuscript both in light of its codicological context and in light of humanist activity at the Estense court. He also presents the evidence for the book having been in the Estense library. Finally, he examines the place of the manuscript in the context of the later Italian tradition of fencing books. A complete concordance is presented in the appendix.

Beatrix Nápolyból indult Budára, s az útvonalat követve sorra vettük a nápolyi Castel Nuovo díszkapuját, a paloták reneszánsz ablaktípusát, Alberti építészeti kisugárzását, a nápolyi majolika padlók és a budaiak hasonlóságát. A ferrarai Castello Estense kis teraszkertje lehetett az egyik inspiráció a budai Cisterna Regia feletti függőkerthez. Még az esztergomi „menedék” tartózkodását is megvizsgáltuk. Az építőművészeti elemzés mellett Beatrix női személyiségének belefonódását sem hanyagoltuk el.

Summary The subject of my paper concerns the iconography of the mysterious relief at Modena (Galleria Museo e Medagliere Estense, inv. 2676) showing a young god in a cosmic egg. The paper is to review the state of research in modern scholarship since 1863, to discuss various attempts at its interpretation, and to propose my own working hypothesis, which links the Modena relief to the Orphic Rhapsodies and the Middle-Platonic passage transmitted by Porphyry of Tyre in his The Cave of the Nymphs 21–29.

In Italy during the Renaissance, an age that conjures up open systems of succession and ruthless dynastic struggle, the House of Este stood out for its singular succession practices. For almost one hundred and fifty years, illegitimately born Estense princes held office. Pope Pius II (r. 1458–64) marvelled:It is an extraordinary thing about that family that within our fathers' memory no legitimate heir has ever inherited the principate; the sons of their mistresses have been so much more fortunate than those of their wives. It is a circumstance contrary not only to Christian custom but to the law of almost all nations.

Modena, Biblioteca Estense, MS α.M.1.13 (ModD) is a collection of masses compiled in Ferrara around 1480 for the court of Ercole I d'Este. Although several scholars have argued that the changes in ModD were the result of composers reworking their own music or intervention by the scribe, the situation is far more complex. Through a careful examination of the textual and musical changes in ModD, this essay substantiates three hypotheses: that ModD had a separate and independent editor (Johannes Martini) apart from the scribe who copied it (Fra Filippo), and that both tampered with the music to a different extent; that the reasons behind the changes were in part politically and religiously motivated; and finally that several of the masses in ModD contain sections written by someone other than their original composer, a practice that may have been more common than we currently believe.

Proposito del presente lavoro è indagare la novità introdotta dal Regolamento estense del 1763 sull’amministrazione della giustizia feudale. Si tratta di una novità di non immediata percezione, posto che la riforma segnalava una tensione duplice e ambivalente. Da un lato, sembra che essa mirasse a razionalizzare i poteri esercitati da centri politici diversi; dall’altro, invece, che non intendesse pregiudicare quel medesimo sostrato di pluralità che ne costituiva il presupposto. Con l’obiettivo di risalire all’effettiva portata della nuova e contestata normativa, il contributo cercherà di tracciare un quadro delle intersezioni tra competenze feudali e ducali. Il reale spessore della feudalità del XVIII secolo, ricostruibile grazie al recupero dei diversi paradigmi nei quali la ‘sovranità’ del Duca si declinava, conduce a rappresentare la stessa più come riflesso, che come causa, di un più ampio ordine giuridico.

Abstract:

Durante el siglo XVI las élites nobiliarias tomaron como modelo de comportamiento al gentiluomo descrito por Baldassare de Castiglione en su Cortegiano. Según este ideal, la música jugaba un papel esencial en la formación de príncipes y cortesanos. El soberano humanista debía conjugar la doble faceta de intérprete y patrón. Paradójicamente y bajo este prisma, en las cortes italianas de Mantua y Ferrara la figura de la mujer adquirió una relevancia especial en dos ámbitos, por un lado en el intenso patronazgo musical ejercido por las princesas de las familias gobernantes, y por otro en la importancia de las cortesanas y profesionales como intérpretes. Todo ello posibilitó el desarrollo de un estilo de canto de gran refinamiento y de un virtuosismo extremo, explicitado en el concerto delle dame de la corte estense de Ferrara de finales del siglo XVI.

Abstract:

RACIONAL: As estenoses benignas de esôfago são complicações decorrentes de diversas causas. Possuem tratamentos similares, na maioria dos casos necessitando de dilatação endoscópica, no entanto a resposta terapêutica, tempo ideal de tratamento, assim como intervalo entre as sessões podem ser variáveis. OBJETIVO: Analisar, do ponto de vista endoscópico, as estenoses benignas de esôfago em 14 anos de experiência no Hospital Universitário Clementino Fraga Filho da Universidade Federal do Rio de Janeiro, RJ, avaliando etiologia, a extensão da estenose, o número de dilatações necessário para atingir resposta terapêutica satisfatória, assim como a relação entre a extensão da estenose e a resposta terapêutica. MÉTODO: Foram analisadas 2.568 dilatações endoscópicas com uso de velas de Savary-Gilliard em 236 pacientes, durante um período de 14 anos e 10 meses, até junho de 2007. RESULTADOS: A estenose péptica foi a causa mais freqüentemente encontrada, seguida pela estenose cáustica. As estenoses longas e cáusticas necessitaram de maior número de sessões para ausência de disfagia. Estenoses pépticas e curtas responderam melhor a número menor de sessões de dilatação. CONCLUSÃO: A estenose péptica foi a causa mais comum e respondeu bem à terapia endoscópica, em concordância com a literatura. As estenoses cáusticas foram as mais refratárias, principalmente as longas. Quanto maior foi a extensão da estenose, também maior foi o número de sessões necessárias. Estenoses curtas apresentaram boa evolução na maioria dos casos. O número de dilatações necessárias dependeu diretamente da causa e da extensão da estenose.

AbstractThe article is inspired by the discovery of new documents concerning the case of the heretic Fanino Fanini da Faenza, who in 1549–1550 was at the centre of a bitter jurisdictional conflict between the Holy See and the Estense State. Thanks to the new evidence collected, it is possible to outline the role played by the legate of Romagna, Cardinal Girolamo Recanati Capodiferro, who conducted negotiations with Duke Ercole II d’Este through the mediation of the Portuguese scholar António Ribeiro. The study then attempts to broaden the analysis, showing that Fanini’s story allows us to reflect on the positions taken by some members of the sacred college (in particular Marcello Cervini and Capodiferro himself) in the months preceding the death of Paul III, on their attitude towards the heretical emergency, and on the attempts of the Italian princes to impose their own control over the inquisitorial courts.

Abstract:

The precious collection of manuscript scores and printed libretti dating from the late seventeenth century and housed today in the Biblioteca Estense at Modena is ample proof that Duke Francesco II d'Este (1660–94) was one of Italy's most generous patrons of music. Indeed his library, which good fortune has preserved almost intact, is an indispensable resource for the study of oratorio, opera and instrumental music in northern Italy in the last quarter of the century. Its contents show him to have been a man of catholic and modern taste, acquiring and promoting works by living composers active in Rome, Florence, Bologna, Ferrara, Venice and, of course, Modena. On the evidence of the Este music collection, historians have been consistent in applauding the cultural achievements of Francesco II's reign and yet, surprisingly, there have been no detailed studies of the way in which the duke exercised his patronage.

Questo breve saggio intende analizzare il modo in cui, a distanza di poco più di un decennio dalla stesura e messinscena dell'Aminta, e consapevole degli schemi pastorali post-tassiani sperimentati da diversi drammaturghi, il Guarini sia in grado di effettuare, in un'ottica di colta ed oculata coscienza metamorfica, e in un ambiente controriformista decisamente fortificato persino nel contesto adulatorio della corte, non solo un mutamento ma una quasi totale riscrittura di alcune componenti costitutive, portanti, della favola boschereccia. Le modalità metamorfiche che investono la figura e la funzione del Satiro non ne sono che un sintomo. Di fronte ad una nuova realtà, tutta fatta di cangianti linee e colori, l'apportatore della crisi nelle selve guariniane presenta alcune lievi tracce di una certa patologia di corte dell'epoca, una sorta di paranoia da voyeur, quella che nella realtà aveva già fatto rinchiudere il maggiore e invidiato poeta estense in Sant'Anna, quella stessa che nella finzione della scena indurrà inesorabilmente l'alpestre e tragicomico personaggio alla disfatta.

In its pages, unassuming four-voice secular settings are preceded by citations from Pliny and Isidore and surrounded by dazzling illuminations of birds, fruits and plants. If splendour is never amiss in gifts, one must wonder what the relationship was between the components of this intriguing artefact. In this article I investigate the gift value of the source by examining the musical repertory and style in conjunction with its other features. I propose that musical strambotti enabled fifteenth-century educated Italians to perform symbolic gestures associated with ancient music. The repertory was also perfectly congruous with the poetic and decorative choices made for the manuscript. The examination of this peculiar source helps shed more light on the musical strambotto, its uses and its implied cultural associations.

Švýcarské nakladatelství Urs Graf Verlag GmbH se sídlem v Dietikonu na předměstí Curychu vydalo v letech 1977 a 1995 faksimile dvou výjimečných katalánských kartografických děl 14. a 15. století. Prvním z nich byla Mappa mundi, jež vznikla mezi lety 1375 a 1381), druhým kruhová Mappa mundi Estense z období mezi 1450 a 1460. První dílo je označováno též jako Katalánský atlas 1375 a skládá se z několika tabulí zachycujících tehdy známý svět od pobřeží Atlantského oceánu až po východní pobřeží Asie. Hlavními prameny pro zpracování východní části Asie byly cestopisy Marka Pola, tzv. Jana Mandevila a Odorika z Pordenone. (Grosjean, ed., 1977) Druhým dílem je kruhová mapa o průměru 113 cm, zachycující kromě známého světa i země dosud neobjevené, ale předpokládané, s jejich přibližnou polohou. (Milano, ed., 1995) Obě díla jsou bohatě ilustrována a na své si přijdou i milovníci vlajek. Faksimile, reprodukující díla ve skutečné velikosti, doplňují podrobné studie a přepisy katalánských textů na mapách s jejich komentovanými překlady (v prvním případě do němčiny, ve druhém do italštiny a němčiny). Tento příspěvek podává přehled o obou těchto mapách s originálními texty a jejich překlady na základě nejnovější literatury.

Estenose de carótida pode ser assintomática ou sintomática, manifestando-se por ataque isquêmico transitório ou infarto cerebral. Quando assintomática evolui com risco de 10% ao ano de evento clinico vascular carebral. 0 principal exame para diagnóstico é o ultra-som doppler de carótidas, porém não se recomenda esse exame rotineiramente para a população geral, reservando-se para os casos de alto risco vascular. Nas últimas duas décadas têm sido realizados vários estudos randomizados para definição de condutas nas estenoses carotideas. Com base nesses estudos, recomenda-se endarterectomia para estenose sintomática 70% se não houver Contra-indicações ao procedimento, e também para estenoses assintomaticas 60%, porém considerando-se cautelosamente o risco cirúrgico do serviço. Caso não haja indicação de cirurgia, os pacientes devem submeter-se a tratamento clinico, que no caso de estenose assintomática constitui-se do controle dos fatores de risco aterotrombotico e nos casos de estenose sintomática, além do controle dos fatores de risco, utiliza-se droga antiagregante plaquetaria.

Introdução: Ainda, não existe método ideal para o diagnóstico diferencial entre estenoses biliares malignas e benignas. Este estudo visa a comparar a concentração de fibronectina biliar nos pacientes com e sem estenose biliar maligna. Métodos: Durante a pancreatocolangiografia retrógrada endoscópica (PCRE), foram coletadas amostras de bile de 50 pacientes com estenose biliar extra-hepática maligna (40), benigna (10) e de 10 doentes sem estenose biliar (grupo controle). A fibronectina total na bile foi determinada por ensaio imunoenzimático. A concentração sérica de bilirrubina direta, fosfatase alcalina, gama glutamiltransferase, aspartato aminotransferase e alanina aminotransferase foi determinada nos pacientes com estenose biliar antes da PCRE. O diagnóstico final foi definido por cirurgia, biópsia ou acompanhamento clínico. Resultados: Os pacientes com neoplasia maligna eram significativamente mais idosos (p= 0,02) e apresentaram níveis mais elevados dos testes bioquímicos relacionados à colestase (p< 0,01). Não houve diferença significativa na concentração de fibronectina biliar entre os pacientes com estenose maligna (694,2 ± 823,5 ng/ml), benigna (828,9 ± 925ng/ml) e grupo controle (466,5 ± 621,5 ng/ml), ou entre os pacientes com (721,2 ± 836,6 ng/ml) e sem estenose (466,5 ± 621,5 ng/ml). Conclusões: As médias de idade e de resultados de exames laboratoriais relacionados à colestase foram mais elevadas nos pacientes com neoplasia maligna. A dosagem apenas da fibronectina total na bile não foi eficaz para diagnóstico diferencial das estenoses biliares.

Bertoldo, Bertoldino e Cacasenno was the first comic opera, which opened the Venetian era in the history of opera buffa. Contemporary dictionaries name Carlo Goldoni (text) and Vincenzo Legrenzio Ciampi (music) as its authors. The detailed review of the manuscript (preserved in the Estense Library in Modena) undertaken in the article for the first time, reveals the original “pasticcio” nature of this opera. An analysis of the handwriting and various markings shows that the score contains musical material representing two different productions of the opera: the Venice premiere in 1749 and a performance in London in 1755. Ciampi, who wrote about one third of all the musical numbers for the Venetian premiere of the opera in 1749, was apparently joined in his work by other composers. For the London production in 1755 Ciampi added a newly composed portion of musical numbers, however, still almost a quarter of the entire score remained consisting of insertions of music by other composers. The surviving music also makes it possible to establish exactly what material from the Venetian production was used in the parodies on this opera composed in Paris: Bertholde à la ville by Louis Anseaume (1754) and Le Caprice Amoureux, ou, Ninette à la cour by Charles-Simon Favart (1755). The study of Bertoldo, Bertoldino e Cacasenno expands our understanding of the practices of opera buffa at the early stage of its history.

Abstract:

OBJETIVO: Pesquisar a eficácia da Submucosa de lntestino Delgado (SID) porcina na correção de estenoses esofágicas cervicais em cães. MÉTODOS: Para produzir estenose, 12 animais foram submetidos a ressecções de porção elíptica de 3,5X2,0 cm, na parede anterior do esôfago cervical, suturado por pontos de fio de algodão. O processo evolui por 90 dias, atingindo a estenose desejada e comprovada por esofagograma. Na seqüência, a lesão cicatricial produzida foi ressecada e substituída por enxerto de SID. Transcorridos 2 meses os animais foram submetidos a novo esofagograma. Aferiu-se então a largura esofágica (nas porções proximal e média do esôfago cervical) após a realização da estenose e pós-correção. Os animais foram submetidos a eutanásia, ao 60º dia de pós-correção, e à necropsia os esôfagos foram retirados e enviados ao laboratório de Anatomia Patológica. RESULTADOS: Não houve fístula ou infecção. Ocorreram reepitelização completa da mucosa, discreta reação infamatória e neovascularização moderada. A luz esofágica foi ampliada em 70% dos animais (43% ± 13% em média) (p = 0,2135). A medida da porção proximal, passou de 0,76cm para 0,95cm em média (p=0,02). Não houve alteração significativa em relação a porção medial. CONCLUSÃO: A SID demonstrou ser, no cão, enxerto eficaz para correção de estenoses esofágicas, integrando-se nitidamente à sua parede e substituindo-a de forma adequada.

Contexto: A arteriografia com contraste (AC) tem sido o exame tradicional de avaliação de pacientes com suspeita de doença oclusiva aorto-ilíaca (DOAI). Recentemente, métodos menos invasivos, como a eco-Doppler, têm sido usados com a mesma finalidade. Objetivo: Comparar prospectivamente a eco-Doppler com a AC e eventual manometria arterial direta (MAD) na avaliação pré-operatória de pacientes com suspeita de DOAI. Métodos: Foram submetidos a eco-Doppler e a AC 125 pacientes internados para tratamento de doença arterial oclusiva dos membros inferiores, avaliando comparativamente 552 segmentos da aorta infrarrenal e das artérias ilíacas comum e externa. As lesões encontradas foram classificadas em cinco categorias: 1) normal e estenose leve (0 a 19%); 2) estenose moderada (20 a 49%); 3) estenose significativa (50 a 79%); 4) estenose crítica (80 a 99%); e 5) oclusão total. A MAD foi usada em 19 segmentos de 15 pacientes para classificar lesões limítrofes entre duas categorias. Foram calculados índices de validade (sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e acurácia) para distinguir lesões hemodinamicamente significativas de não-significativas e para distinguir estenoses críticas de oclusões. O padrão-ouro foi AC, complementado pela MAD. Foram também calculados coeficientes de correlação kappa entre arteriografias e eco-Doppler para o conjunto dos segmentos aorto-ilíacos. Resultados: Lesões clinicamente relevantes (estenoses de 50 a 99% e oclusões totais) foram observadas na eco-Doppler em 163 segmentos (29,5%) e na AC em 158 segmentos (28,6%). A eco-Doppler mostrou altos índices de validade para distinguir lesões hemodinamicamente significativas de lesões não-significativas em todos os segmentos (acurácia = 92%; kappa = 0,81) e para diferenciar estenoses críticas de oclusões (acurácia = 86%; kappa = 0,73). Os índices de correlação entre os resultados das eco-Doppler e das AC foram ótimos em todos os segmentos aorto-ilíacos. Conclusão: A eco-Doppler apresenta elevados índices de validade e ótimos coeficientes de correlação com a AC na avaliação de pacientes com suspeita de DOAI.

OBJETIVO: Relacionar a influência dos dois tipos de anastomoses arteriais empregadas (término-terminal e término-lateral), da idade do receptor, do rim de doador vivo ou cadáver e do uso de remendo aórtico ("patch") quanto à presença ou não de estenose da artéria renal e, avaliar o tratamento destas estenoses. MÉTODO: Estudo retrospectivo de 278 pacientes submetidos a transplante renal no Hospital Universitário Clementino Fraga Filho (HUCFF), de maio de 1989 a maio de 2001. RESULTADOS: Não se mostrou relação entre as variáveis estudadas com a presença ou não de estenose da artéria renal. A estenose da artéria renal foi detectada em oito casos, que foram tratados, com sucesso, mediante angioplastia transluminal percutânea (ATP), com implante primário de "stent" (n = 7) ou sem implante de "stent" (n = 1). CONCLUSÕES: As variáveis estudadas não influenciaram no aparecimento da estenose da artéria renal e o tratamento endovascular foi efetivo na correção destas lesões estenóticas.

RACIONAL: As principais causas de estenose biliar maligna são câncer de pâncreas e colangiocarcinoma. A definição do prognóstico dos pacientes no momento da pancreatocolangiografia retrógrada endoscópica é importante na escolha da conduta mais adequada. OBJETIVO: Avaliar a importância do escovado endoscópico e da bilirrubinemia na determinação da sobrevida dos pacientes com estenose biliar maligna. MÉTODOS: Os pacientes com estenose biliar diagnosticados durante pancreatocolangiografia retrógrada endoscópica foram submetidos a duplo escovado. Amostras de sangue de todos eles foram obtidas para dosagem das bilirrubinas. Os pacientes foram acompanhados para determinar o diagnóstico final e a sobrevida. RESULTADOS: Diagnóstico final de doença maligna foi obtido em 40 pacientes de um total de 50 casos de estenose biliar. Os níveis séricos elevados das bilirrubinas ou a citologia por escovado positiva para malignidade estava relacionada a menor sobrevida. CONCLUSÃO: Os dados desta pesquisa demonstram a possibilidade de determinar o prognóstico em casos de estenoses biliares malignas através do resultado do escovado endoscópico ou da bilirrubinemia.

OBJETIVO: Analisar os desfechos dos pacientes submetidos ao reparo de estenose congênita de traqueia. MÉTODOS: Análise retrospectiva dos pacientes com estenose traqueal congênita tratados no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo entre 2001 e 2007. RESULTADOS: Seis meninos e uma menina (idade ao diagnóstico entre 28 dias e 3 anos) foram incluídos. Cinco pacientes apresentavam malformações intracardíacas e/ou de grandes vasos associadas. A extensão das estenoses foi curta em três pacientes, média em um e longa em três. As técnicas utilizadas foram traqueoplastia com enxerto de pericárdio em três pacientes, ressecção e anastomose em dois, traqueoplastia em bisel em um e correção de anel vascular em um. Um paciente morreu no intraoperatório por hipóxia e instabilidade hemodinâmica e outro no 11º dia pós-operatório por choque séptico. Outras complicações observadas foram pneumonia, arritmia, estenose na anastomose e estenose residual, malácia e formação de granulomas. O tempo médio de seguimento pós-operatório foi de 31 meses; quatro pacientes ficaram livres da doença e um necessitou de tubo T para manter a via aérea pérvia. CONCLUSÕES: A estenose congênita de traqueia é uma doença curável. Entretanto, seu reparo é complexo e está associado a taxas de morbidade e mortalidade significativas.
