Abstract:
The aim of this study was to examine whether scar imaging echocardiography with ultrasound multi-pulse scheme (eSCAR) can detect subclinical myocardial involvement in systemic lupus erythematosus (SLE). We consecutively recruited SLE patients and controls matched for age, sex, and cardiovascular risk factors. Participants with cardiac symptoms or a prior history of heart disease were excluded. All participants underwent eSCAR and speckle tracking echocardiography (STE) with global longitudinal strain (GLS) assessment. SLE patients were assessed for disease activity and were followed up for 12 months. Myocardial scars by eSCAR were observed in 19% of SLE patients, almost exclusively localized at the inferoseptal myocardial segments, and in none of the controls. GLS was significantly lower in most myocardial segments of SLE patients compared with the controls, especially in the inferoseptal segments. eSCAR-positive SLE patients received a higher cumulative and current dose of prednisone, and had significantly higher levels of anti-dsDNA antibodies (p = 0.037). eSCAR-positive patients were at higher risk of having SLE flares over follow-up (hazard ratio: 4.91; 95% CI 1.43–16.83; p = 0.0001). We identified inferoseptal myocardial scars by eSCAR in about one-fifth of SLE patients. Subclinical myocardial involvement was associated with glucocorticoid use and anti-dsDNA antibodies.

Abstract:
BackgroundScar imaging echocardiography with ultrasound multi-pulse scheme (eSCAR) is a novel technique allowing detection of myocardial scars. eSCAR has never been explored in patients with systemic sclerosis (SSc).ObjectivesThe aim of our study was to examine whether eSCAR may detect subclinical myocardial fibrosis in SSc.MethodsIn this cross-sectional study conducted between 11/2020 and 02/2021, consecutive SSc patients from the University of Verona underwent cardiovascular disease (CVD) screening procedures including standard transthoracic echocardiography (TTE) and myocardial scars detection through the eSCAR technique. We excluded patients with prior history of structural heart disease and pulmonary hypertension. To validate eSCAR findings, we assessed myocardial function through global longitudinal strain (GLS) analysis by speckle tracking echocardiography (STE). We also conducted a case-control study comparing eSCAR findings in randomly selected SSc patients and non-SSc controls matched 1:1 for age, sex and number of CVD risk factors. The primary outcome was the proportion of patients with myocardial scars. Principal component analysis (PCA) was used to determine main eSCAR patterns. Multivariable linear regression analysis was used to minimise confounding.ResultsScar imaging echocardiography revealed that 42/92 (44%) SSc patients had myocardial scars involving a median [25th-75th percentile] of 4 [1-7] segments. PCA identified that myocardial scars localised mostly at the inferior and septal segments (inferoseptal pattern; Figure 1). STE validation confirmed that GLS was significantly reduced in SSc patients with inferoseptal scars (Table 1). Otherwise, all standard echocardiography measures were normal in both groups. In multivariable regression analysis, prior digital ulcerations (B=0.41, 95% CI 0.008 to 0.809, p=0.045) and body mass index (B=-0.06, 95% CI -0.113 to -0.015, p=0.012) were both significantly and independently associated with inferoseptal myocardial scars. Abstract:
Background:Myocardial fibrosis is a severe complication of immune-mediated diseases, occurring in up to 30% of systemic lupus erythematosus (SLE) patients. Cardiovascular magnetic resonance imaging allows myocardial scar detection in SLE patients, but it is costly, time consuming, and unfit for patients with renal disease. Scar imaging echocardiography with ultrasound multi-pulse scheme (eSCAR) is a novel and promising technique that proved to be effective in detecting ischemic myocardial scars in patients with coronary artery disease (CAD).Objectives:To evaluate if the eSCAR technique is feasible and to better characterize SLE patients with cardiac involvement by eSCAR.Methods:We recruited consecutive patients with SLE classified according to the 2019 EULAR/ACR recommendations. Patients with diabetes mellitus, obesity, prior cardiovascular (CV) disease or anti-phospholipid (aPL) syndrome were excluded. Eligible participants underwent a thorough clinical assessment and a full echocardiography examination, including the eSCAR technique. Data on clinical variables were collected; disease activity was estimated by the SLE Disease Activity Index (SLEDAI) score. Lupus flare was defined as new/worse clinical signs and symptoms and/or lab measurements and a change/increase in treatment. Patients were compared according to the presence or absence of eSCAR. In this preliminary report, only descriptive analyses are provided. Continuous data are reported as median [25th; 75thpercentile].Results:We enrolled fifteen patients diagnosed with SLE (age 45 years [36; 47], disease duration 14 years [12; 20]), 13 (87%) were females. Median SLEDAI was 5 [2; 8]. The most frequent disease involvement included arthritis (73%), skin and mucous membranes (60%), lupus nephritis (47%) and cytopenias (47%). Patients had received treatment for lupus with 5 drugs [5; 8]. Cumulative prednisone dosage was 25 g [20; 44], whilst the current daily dosage of prednisone was 4 mg [0.0; 5.0]. Hypertension was present in 4 (27%) and hypercholesterolemia in 2 (13%) subjects; 4 patients (27%) were current or past smokers. The eSCAR technique was feasible in all participants with no adverse effects. Myocardial scars were detected in 2 patients (eSCAR-positive 13%; figure and table); eSCAR positive patients were females and had no history of cardiovascular involvement (including pericarditis); they had at least one relapse within the prior 12 months before enrollment; at least one cardiovascular risk factor was found in both patients (one was a smoker and the other one had hypertension); none received prior treatment with cyclophosphamide or rituximab; they had no renal involvement; arthritis and cytopenia were the prominent features of disease; anti-dsDNA titer was higher than eSCAR-negative patients.Conclusion:Echocardiography allowed detection of myocardial scars in patients with SLE. Our preliminary data show that eSCAR is feasible and well tolerated in a SLE population. ABSTRACT Fusarium species, a mold which causes disease mainly in plants has emerged as pathogen in immunocompromised patients. Fusarium is known to cause keratitis, onychomycosis, and endophthalmitis. Fusarium solani, is the most common isolate from clinical specimen. Here is a case, a 65-year-old male with type II diabetes mellitus since 10 years presented with a large ulcer on the left leg since 8 months following trauma. The fungal culture of the escar of the ulcer isolated a mold, Fusarium solani. The patient’s leg was amputated and was treated with amphotericin B. The patient was discharged on healing of the stump. This case gives emphasis on fungal culture in chronic diabetic ulcer.

Este escrito de investigación comprende de forma aproximada el modelamiento cinemático directo del robot equino de seis grados de libertad “6-DOF” desarrollado como producto de la investigación en curso titulada “Simulador equino para prácticas de tiro montado en la ESCAR”. El manipulador diseñado tiene fines educativos y será utilizado en las prácticas de tiro montado, como herramienta de apoyo didáctico en los cursos de carabineros de la policía nacional. Se presenta la propuesta de diseño del manipulador robótico adaptado a las necesidades educativas. Se utilizan actuadores eléctricos lineales de potencia que acoplan los eslabones de la cadena cinemática propuesta. Se aproxima la cinemática directa del robot, utilizando el algoritmo de Denavit-Hartenberg “DH” y se calcula el modelo dinámico del manipulador. Finalmente se valida el prototipo realizando pruebas de simulación del comportamiento del manipulador mediante la creación de segmentos de programa en Matlab® y se comparan con los resultados obtenidos mediante el control del mecanismo desde una aplicación de software desarrollada para tal fin en Microsoft Visual C# 2013®.

Abstract:
Introduction: Scrub typhus is public health problem in South East Asian Region and also it is of rising concern in India. Eastern part of Maharashtra state is highly vulnerable to the disease as it is under forest cover. This region is presently witnessing outbreak of scrub typhus. The present case control study aims to understand determinants of the scrub typhus in this geographical location. Material & Methods: Operational definition for scrub typhus cases used in present study was “Individuals who are positive for IgM Elisa of Scrub typhus”. Cases of scrub typhus reported to health system that comply with the operational definition were recruited. Individually matched controls were selected from the same village. Matching was done for age, sex, socioeconomic status and residence. Total 16 cases and 32 controls were enrolled and compared for socio-demographic, environmental and behavioral domains. Results: Clinical menifestations among cases were Fever, malaise, headache, escahar and rash. The most common sites of escar was groin 3 (42.8%), followed by suprascapular region 2 (28.6%), axilla 1(14.3%) and elbow 1 (14.3%). The risk factors for scrub typhus were vegetations around household within 3 feet range, length of grass blade more than 2 feet, routine work in shrub, engaging in forest related work and activities aroud lake with the odds 5.41 (1.13-25.83), 5.6 (1.24-25.49), 3.66 (1.04- 12.9) 4.2 (1.02-18.02) and 4.59 (1.25-16.77) respectively. Conclusion: Apart from other known risk factors, present study ascertained activities around lake, closeness of vegetations to the household (less than 3 feet) and length of grass blade (more than 2 feet) as new risk factors. These findings may be helpful in developing targeted intervention strategies to control scrub typhus.

Two new species of deep-sea anglerfishes are described on the basis of specimens collected from off northeastern Taiwan. Oneirodes formosanus sp. nov., based on one adult female, differs from its congeners in having a deep caudal peduncle (15.4% SL) and esca with a single simple, elongate, unbranched, internally pigmented, anterior escal appendage; a simple, elongate, posterior escal appendage; an elongate terminal escal papilla; and no medial and lateral escal appendages. Gigantactis cheni sp. nov., based on three adult females, differs from its congeners in having a series of unpigmented filaments at base of illicium; a black terminal elongated esca bearing numerous dermal spinules; relatively more jaws teeth with the outtermost ones relatively short. A synopsis of Taiwanese species of the suborder Ceratioidei is provided.

Em solos sob plantio direto, o tráfego de máquinas pode imprimir ao solo estado de compactação restritivo ao rendimento de culturas. Este trabalho foi realizado na Universidade de Cruz Alta, no município de Cruz Alta (RS). O objetivo foi avaliar os efeitos de sistemas de manejo e estados de compactação do solo sob sistema plantio direto nas propriedades físicas e no rendimento de grãos de seis cultivares de soja. O experimento foi realizado em um Latossolo Vermelho distroférrico, com 427 g kg-1 de argila. Os tratamentos, estabelecidos em blocos ao acaso, com quatro repetições, constaram de sistemas de manejo e estados de compactação criados por um rolo compactador de 2 Mg, foram: plantio direto (PD) contínuo sem compactação adicional (PD-C0), PD com uma passada do rolo compactador (PD-C1), PD com três passadas do rolo compactador (PD-C2), PD com cinco passadas do rolo compactador (PD-C3) e PD escarificado sem preparo secundário (ESCAR). As propriedades físicas avaliadas foram resistência à penetração do solo (RP), densidade do solo (Ds), porosidade total (Pt), macroporosidade (Macro) e microporosidade (Micro). A compactação do solo causou menores valores de Ds, espaço poroso e RP do solo na camada de 0-0,10 m de profundidade. Houve relação direta entre a Ds, volume de Micro e a RP do solo e relação inversa entre a Ds e o volume de Macro, com o aumento do estado de compactação. Os cultivares de soja apresentaram comportamento semelhante para os diferentes estados de compactação do solo. A escarificação do solo em área manejada por oito anos sob sistema plantio direto não propiciou incremento no rendimento de grãos de soja. Quando os valores de RP foram de até 2,6 MPa, os de Ds de até 1,51 Mg m-3 e volume de Macro superiores a 0,10 dm³ dm-3, em condições de lavoura, não houve comprometimento do rendimento de grãos de soja.

A partir da compreensão conceitual desses construtos, propõe-se um novo modelo de bem-estar no trabalho. Objetivo: comprender la experiencia de descanso de las cuidadoras. Método: estudio cualitativo guiado por los procedimientos de la teoría fundamentada que forma parte de otro más amplio acerca del alivio del peso del cuidado en situaciones de vulnerabilidad. En el estudio participaron 17 cuidadoras de familiares con demencia avanzada que fueron seleccionas con propósito. Los datos se recolectaron por medio de 17 entrevistas semiestructuradas. El análisis de datos se hizo basado en los procedimientos de la teoría fundamentada. Resultados: las cuidadoras no descansan como antes lo hacían, tal experiencia es interpretada, a menudo es incompleta y depende del bienestar del familiar enfermo. La tranquilidad emerge en el estudio como el elemento central de esta nueva experiencia de descanso. Descansar ahora es una experiencia interpretada y que conlleva un trabajo de organización y emocional. Las cuidadoras crean y pactan condiciones que les proporcione tranquilidad para poder descansar. Conclusiones: el presente estudio profundiza en la experiencia de aliviar del peso del cuidado y destaca la connotación moral presente en el cuidado familiar. Conocer cómo descansan las cuidadoras y qué es lo que subjetivamente les proporciona descanso, no sólo contribuirá a que los profesionales de la salud lo promuevan y apoyen, sino también evitará culpabilizarlas por no querer o poder descansar. Los servicios de apoyo han de considerar el cambio que ha tenido para las cuidadoras la experiencia del descanso en sus vidas.

A concepção de saúde inclui bem-estar como um conceito chave. Em decorrência, encontram-se na literatura diferentes proposições teóricas para bem-estar. Este artigo tem como objetivo apresentar duas visões tradicionais e uma concepção nova sobre bem-estar. Inicialmente, são revisadas as bases teóricas que sustentam o bem-estar subjetivo. As concepções sobre bem-estar psicológico, ancoradas nas teorias que desenharam os primórdios da psicologia positiva, são apresentadas na segunda seção. Na seqüência, o conceito de bem-estar no trabalho é formulado apontando-se seus componentes assentados em vínculos positivos com o trabalho e com a organização. Na seção que encerra o artigo, sugere-se uma articulação, com base nas proposições da psicologia positiva, no intuito de ampliar a compreensão de fatores que contribuem para promover uma existência mais saudável.

A new species of the deep-sea ceratioid anglerfish genus Himantolophus is described from a single specimen collected off the east coast of the northern Andaman and Nicobar Islands, India, a member of the Himantolophus albinares-group, based in part on the absence of anterior escal appendages. The new species differs from other members of this group in having a unique illicial and escal morphology with a simple basimedial and a pair of basilateral filaments on the esca. The new species is compared with previously recognized members of the H. albinares-group.

Este trabalho tem por objetivo compreender como se movimentam as discussões e o que aportam ao universo dos estudos sobre mal-estar docente. Recorre-se ao Banco de teses da Capes, contemplando um período de cinco anos (2015 a 2020) para produzir o levantamento. Amparado na perspectiva dos estudos, do tipo estado da arte ou revisão de literatura, buscou-se pela análise, selecionar produções que pudessem evidenciar contribuições à área de interesse. Lançamos mão da Análise de Conteúdo, preconizada por Bardin, para a realização do tratamento dos dados. O comprometimento da saúde dos docentes é um fator que emergiu com ênfase entre as pesquisas analisadas. Dentre os resultados dos estudos salienta-se de um lado, saúde ocupacional, sintomas mentais, depressão e medicalização e, de outro, a aposta em uma formação continuada que reconheça o trabalho intelectual do profissional como algo inerente ao trabalho. Apostando no diálogo onde a horizontalidade se impõe como um princípio formativo direcionando a escola para um espaço de exercício autoral de uma profissão.

This paper studies the SER / ESTAR alternation in Spanish. Following the logic behind Freeze's (1992) and Kayne's (1993) analysis of Benveniste's (1960) approach to HAVE, it is argued that ESTAR derives from SER plus the incorporation of an additional functional element, labeled X. It is this element (which we argue has a prepositional nature in different languages), and not some lexical property (a feature), that is responsible for the oft-noted aspectual (i.e. non-standing, perfective, stagelevel) flavor of ESTAR. Even though we focus on the SER / ESTAR cut, our analysis ultimately argues for the idea that the distinction between Individual Level and Stage Level predicates does not indicate a 'lexical' (meaning 'primitive,' 'intrinsic,' or 'inherent') property of predicates. In line with recent neoconstructionist approaches to the lexicon (cf. Borer 2005, Hale & Keyser 1993, 2002, Marantz 1997, Ramchand 2008, among others), we pursue the idea that properties (transitivity, lexical aspect, stativity, etc.) that were interpreted as lexical in projectionist and lexicalist models are actually the consequence of a specific syntax.

Las presentes reflexiones retoman algunos desarrollos de la investigación en curso que realizamos en el marco de la Secretaria de Ciencia y Técnica de la Universidad Nacional de Córdoba, Argentina, titulada “Urbanismo estratégico y conflictos: experiencias de disputas urbanas en Córdoba Capital (2016-2017)”. Hemos indagado en diversos barrios: Alberdi, San Vicente, Güemes, San Martín, El Tropezón, donde ciertos actores luchan por permanecer en el espacio urbano del que han sido/vienen siendo desplazados, realizando demandas de subjetividad e identidad que se traman en situaciones heterogéneas de conflicto frente al avasallamiento del desarrollismo inmobiliario y de los gobiernos municipal y provincial. Ante esto, en Alberdi, la Multisectorial; en San Vicente, la Red de Vecinos, los vecinos del asentamiento El Tropezón, de la ex- cárcel de San Martín y los artesanos de Güemes, se constituyen como actores sociales en estos barrios pericentrales. En esta presentación retomamos la idea de M. de Certeau para dar cuenta de algunas tácticas que actualizan los miembros de estos colectivos y las interrogamos en cuanto a los horizontes de cambio social que traman y expresan. El objetivo es dar cuenta de algunos núcleos centrales de sentido en tanto organizadores de la acción con otros, en el espacio/temporalidad actual de la ciudad cordobesa, en vistas a pensar —junto a J.M. Barbero— sobre algunas manifestaciones en el presente del “estar juntos”, construyendo significados sobre el barrio/la ciudad.

e16027 Background: The pivotal goal of esophageal cancer (ESCA) screening is to identify early-stage cancer or precancerous lesions when curable treatments are available. Methylation of circulating-free DNA (cfDNA) has shown promising results in the early detection of multiple tumors recently. Here we conducted a prospective study to investigate the performance of cfDNA methylation in the early detection of ESCA. Methods: Specific methylation markers for ESCA were identified and optimized based on 24 esophageal tumor and its corresponding adjacent tissues. Age-matched participants with ESCA (n = 136), benign esophageal disease (n = 21) and healthy controls (n = 126) were randomized into the training and testing sets to develop an early-detection classifier. Results: In total, 921 differentiated methylation blocks (DMBs) between tumor and adjacent tissues were identified, of which 679 (73.7%) showed higher methylation level and 242 (26.3%) had lower methylation level in tumor tissues. In the training set, the specificity of the model built with these DMBs was 94.1% (95% CI, 85.5%−98.4%) and the sensitivity was 86.0% (95% CI, 72.2%−94.8%). The sensitives increased with stages, which were 77.8% (95% CI, 39.8%−97.2%), 90.9% (95% CI, 58.7%−99.8%), 83.3% (95% CI, 51.7%−97.9%) and 100.0% (95%CI, 63.1%−100.0%) for stage I-IV, respectively. Similar results were observed in the testing set with area under curve (AUC) of 0.932 (95% CI, 0.887−0.977). Conclusions: The cfDNA methylation profiles distinguished ESCAs from healthy individuals and benign esophageal diseases with promising sensitivity and specificity. Consideration of the potential value of early detection in ESCAs, further evaluation in larger prospective studies is warranted.

Inoculation of the bacterial cells of microbial seed coating agents (SCAs) into the environment may result in limited survival and colonization. Therefore, the application efficacy of an encapsulated microbial seed coating agent (ESCA) was investigated on potted cotton plants; the agent was prepared with polyvinyl alcohol, sodium dodecyl sulfate, bentonite, and microencapsulatedBacillus subtilisSL-13. Scanning electron micrography revealed that the microcapsules were attached to ESCA membranes. The ESCA film was uniform, bubble-free, and easy to peel. The bacterial contents of seeds coated with each ESCA treatment reached 106 cfu/seed. Results indicated that the germination rate of cotton seeds treated with ESCA4(1.0% (w/v) sodium alginate, 4.0% polyvinyl alcohol, 1.0% sodium dodecyl sulfate, 0.6% acacia, 0.5% bentonite, and 10% (v/v) microcapsules) increased by 28.74%. Other growth factors of the cotton seedlings, such as plant height, root length, whole plant fresh weight, and whole plant dry weight, increased by 52.70%, 25.13%, 46.47%, and 33.21%, respectively. Further analysis demonstrated that the peroxidase and superoxide dismutase activities of cotton seedlings improved, whereas their malondialdehyde contents decreased. Therefore, the ESCA can efficiently improve seed germination, root length, and growth. The proposed ESCA exhibits great potential as an alternative to traditional SCA in future agricultural applications.
