On January 21, 2021, the Canadian Association for Security and Intelligence Studies (CASIS) Vancouver hosted its first digital roundtable event of the year, Radicalization and Violent Extremism in the Era of COVID-19. The presentation was conducted by guest speaker, Dr. Garth Davies, an Associate Professor in the School of Criminology at Simon Fraser University. He is also currently involved in developing data for evaluating programs for countering violent extremism. Dr. Davies’ presentation provided an overview of the changes that society has had to make in adapting to the COVID-19 pandemic and shared some of his research findings on radicalization and violent extremism online during the pandemic. The increase in working remotely and being on the Internet has possibly contributed to a larger dissemination of misinformation leading people to certain extremist sites and forums that may contribute to radicalization. Additionally, Dr. Davies answered questions submitted by the audience, which focused on online radicalization, online platforms used for recruiting by extremist groups, misinformation, and the Incel movement.

In Reply.— Dr Fisher criticizes the manner in which we have grouped our patients to compare survival between patients treated with ECMO and conventional therapy. We feel we have adequately addressed this criticism in our reply to Dr LeBlanc.1 Regarding the degree of illness of patients referred from other centers during the ECMO era, 75% required mechanical ventilation and 83% of ventilated neonates were hypotensive. This is compared to 57% and 38%, respectively, during the pre-ECMO era when very few patients were referred to our center from outside our region.

Tun Dr Mahathir Mohamad was seen as an influential leader when, for the second time in his political life, he was appointed as the Prime Minister of Malaysia at the age of 94 years old. Despite his age, he managed to garner strong support for his leadership when he took over the government following the country’s 14th general election (GE14) in May 2018. Nonetheless, little is known about what contributed to public confidence in the government during his leadership tenure. This paper uses leadership trait theory to discuss the factors influencing public confidence in the government’s capability in managing the country during Tun Dr Mahathir’s tenure as the prime minister. This study is part of a larger study that examines media use, political behavior, and well-being of industrial workers in Malaysia. A self-administered questionnaire was distributed to 475 industrial workers in Malaysia to gauge their perception of the factors contributing to their confidence in the government led by Tun Dr Mahathir. Overall, the findings revealed positive relationships between perception of the condition of the country, Tun Dr Mahathir’s leadership, and confidence in the government. However, no significant relationship was found between media use and public confidence in the government. Implications of leadership trait theory are also discussed. Homoeopathic Materia Medica (HMM) has evolved over the years with the advent of knowledge and new remedy provings. Different authors have tried to define symptomatology using their perspectives. However, as medical science evolved over time, so did the HMM. One needs to understand the changes and philosophy incorporated when writing the HMM. Dr. Cyrus Maxwell Boger, one of the stalwarts of homoeopathy, created Boger’s synoptic key (BSK) after integrating numerous concepts and his experiences. One needs to review the HMM before the first edition of the BSK to understand the concepts used by Dr. Boger to construct the BSK. This study was performed to understand the evolution of different concepts and philosophies of HMM and their influence on Boger.

Años después compartimos guardias como médicos de reciente graduación en distintas oportunidades.Nació el 19 de enero de 1942 en la Ciudad de La Plata donde cursó sus estudios primarios y secundarios. En 1967 se graduó como Doctor en Medicina y a partir de allí comenzó una brillante carrera como médico asistencial y docente. Al comienzo se inclinó por la Especialidad de Medicina Interna y se desempeñó como médico de la Sala IV de HIGA San Martín de La Plata hasta el año 1973. En ese mismo año fue designado Jefe de la Unidad Sanitaria Nº 16 de Berisso por concurso de Carrera Médico Hospitalaria.

Banyak desa yang menjadi bidikan KKN-DR di karenakan pelaksanaan di tempat tinggal masing-masing yang tentu saja mempunyai permasalahan dan budaya serta watak yang berbeda-beda. Adapun program-program yang dirancang oleh mahasiswa KKN-DR IAI Sunan Kalijogo Malang menggunakan metode Asset Based Community Development (ABCD) sebagai wadah pendekatan pemberdayaan berperan aktif baik masyarakat maupun mahasiswa dengan memunculkan potensi/kekuatan wilayah atau desa masing-masing. Berangkat dari metode Asset Based Community Development (ABCD) mahasiswa mempunyai 4 program yaitu, Bidang keagamaan (pembelajaran ubudiyah), Bidang Pendidikan (TPQ dan Bimbingan Belajar), Bidang Perekonomian dengan cara memajukan UMKM serta Bidang Lingkungan sehat mengajak pada disiplin protokol kesehatan secara offline maupun online melalui media seperti WhatsApp, Instagram, Google Meet dan Youtube. Seluruh program yang direncanakan mahasiswa KKN-DR IAI Sunan Kalijogo Malang 2021 telah dilaksanakan dengan baik, hal ini berkat dukungan dari seluruh anggota masyarakat desa beserta perangkatnya yang dengan penuh semangat serta antusias yang tinggi sehingga program yang dilaksanakn benar-benar bermanfaat dan dapat dirasakan tujuannya oleh kelompok sasaran yang direncanakan.

Diabetic retinopathy (DR) is a leading cause of blindness worldwide and a costly complication of diabetes mellitus (DM). DR severity is associated with DM duration; thus, DR has become more devastating to individuals and healthcare systems owing to the aging population and the significantly increased human lifespan. Aging is an irreversible cellular state characterized by long-term stagnation of the cell cycle from excessive stress or damage. Furthermore, aging plays a critical role in developing age-related diseases, but its effects (direct or indirect) on DR development remain considerably understudied. Nonetheless, some studies have demonstrated that aging-related degeneration and DR development share common risk factors, which helps explain the increased prevalence of DR and visual impairment in the elderly population. This review aims to provide some conceptual insight into aging and DR development, two intertwined pathophysiological processes, and discusses potential therapeutic strategies for DR, including prevention and treatment, during this era of longevity.

Background: Although the number of therapeutic options for patients with relapsed/refractory multiple myeloma (RRMM) has increased substantially, it still remains incurable and there remains a significant unmet need for treatment options for highly refractory patients. In this single-center real-world retrospective analysis, we evaluated the efficacy and safety of VDPACE (bortezomib, dexamethasone, cisplatin, doxorubicin, cyclophosphamide, etoposide) as a salvage therapy in the era of daratumumab. Methods: We retrospectively analyzed 88 patients with RRMM who received at least 1 cycle of VDPACE, between 1/2016 till 6/2023 at the University of Kansas Health System in collaboration with the U.S Myeloma Innovations Research Collaborative (USMIRC). VDPACE consists of Bortezomib 1 mg/m2 subcutaneous on days 1, 4, 8 and, 11, doxorubicin 10 mg/m2 continuous infusion (CI) x 4 days, cisplatin 10 mg/m2 CI x 4 days, cyclophosphamide 400 mg/m2 CI x 4 days and etoposide 40 mg/m2 CI x 4 days. In this patient population, we identified two groups - 28 patients who were naïve to daratumumab (DN) and 60 patients who were refractory to daratumumab (DR). Descriptive analysis was performed on available data for patient characteristics, disease course and outcomes differentiating these two groups. Adverse events were graded based on the CTCAE v5.0 criteria. Responses to therapy including overall response rate (ORR), complete response or better (≥CR), and very good partial response (VGPR) were evaluated using the International Myeloma Working Group (IMWG) criteria. The Kaplan-Meier method was used for progression free survival (PFS) and overall survival (OS) assessment. Results: The median age for the entire patient population was 63 years (41-77). Median lines of therapies (LOT) in DN vs DR groups were 2 (1-10) vs 5 (1-10), respectively (Table 1). The ORR and ≥VGPR in DR vs DN was 53% vs 50% and 15% vs 29%, respectively. With a median follow-of 11.3 months (95% CI, 4.03-27), the median PFS for the entire patient population was 4 months (95% CI, 2.97-5.43) while the median OS was 11.1 months (95% CI, 6.97-14.9). However, the median OS for DN was 15.9 months (95% CI, 8.73-29.5) compared to 7.03 months ((95% CI, 6.27-13.57) p=0.06) for the DR group. See Figure 1. Treatment related toxicities were similar in both groups. The most common grade III/IV hematological toxicities were neutropenia (96% vs 93%, p=0.99), thrombocytopenia (93% vs 80% p=0.2), lymphopenia (89% vs 95%, p=0.3), and anemia (79% vs 73%, p=0.79). Transfusion of packed red blood cells (64% in DN vs 68% in DR group) and platelet (61% vs 80%) were common in both groups. Rates of neutropenic fever were similar in both groups; 46% in the DN vs 35% in the DR group, p=0.62, with pneumonia being the most common infection seen in 11% vs 5% of the patients, respectively. Sixty-day Rehospitalization rates in the two groups were 21% vs 50%. There was no treatment-related mortality reported with this regimen. The difference in OS after VD-PACE in DR and DN patients could be explained by higher no of prior LOT, presence of high risk cytogenetics, double refractory status, and limited options available for salvage. Conclusions: Our study shows that while the response rates with VDPACE were comparable in the two groups, there was a higher median OS in the DN. Hematologic adverse events were common in both groups, but the rate of re-hospitalization was higher in the DR group. Based on these results, VDPACE remains a safe and effective salvage therapy for patients with RRMM. For DR patients receiving VD-PACE, strong consideration should be given to clinical trials given poor long-term outcomes.

Background. Progressive interventions have recently improved programmatic outcomes in drug-resistant tuberculosis (DR-TB) care in South Africa (SA). Amidst these, a shorter regimen was introduced in 2017 with weak evidence, and has shown mixed results. Outcomes still fall short of national targets, and the coronavirus disease 2019 pandemic has undermined progress to date. Objectives. To describe the outcomes of participants treated for DR-TB using a shorter, compared with a longer, regimen in a deeply rural SA setting, and to explore other factors affecting these outcomes. Methods. This retrospective cohort study describes outcomes in short and long DR-TB treatment regimens, over 5 years, at two rural treatment sites in SA. Characteristics were analysed for outcome correlates using multivariable logistic regression models. Results. Of 282 treatment episodes, 62% were successful, with higher success in shorter (69%) compared with longer regimens (58%). Mortality was approximately 21% in both groups. Characteristics included high proportions of HIV co-infection (61%). Injectables (adjusted odds ratio (aOR) 3.00, 95% confidence interval (CI) 1.48 - 6.09), bedaquiline (aOR 3.16, 95% CI 1.36 - 7.35), increasing age (aOR 0.97, 95% CI 0.95 - 0.99) and HIV viraemia defined as final HIV-RNA viral load >1 000 copies/mL (aOR 0.16, 95% CI 0.07 - 0.37) were all significantly and independently associated with treatment success. Injectables (aOR 0.22, 95% CI 0.08 - 0.57), bedaquiline (aOR 0.05, 95% CI 0.01 - 0.19), increasing age (aOR 1.09, 95% CI 1.05 - 1.13), extra-pulmonary TB (aOR 8.15, 95% CI 1.62 - 41.03) and HIV viraemia (aOR 9.20, 95% CI 3.22 - 26.24) were all significantly and independently associated with mortality. Conclusion. In a rural context, treating DR-TB amid limited resources and a high burden of HIV co-infection, we found that after considering controls, a short regimen was no different to a longer regimen in terms of success or mortality. Therefore, by alleviating burdens on multiple stakeholders, a short regimen is likely to be favourable for rural patients, clinicians, and healthcare systems. Besides other previously described correlates of outcomes, HIV viraemia emerged as a novel marker for reliably predicting poor outcomes in DR-TB with HIV co-infection, and a pragmatic target for intervention.

This research aimed to determine the readiness of undergraduate students in online learning during the COVID-19 pandemic. This research was a qualitative research method using closed-ended questionnaires and interviews. Subject of this research were undergraduate students at University of Muhammadiyah Prof. Dr. Hamka. The results of this study show that students’ readiness were high in computer/internet self-efficacy, self-directed learning and motivation for learning, on the dimension of learner control and online communication self-efficacy students’ readiness were moderate. It can be concluded that undergraduate students at University of Muhammadiyah Prof. Dr. Hamka ready to face online learning. Furthermore, suggestions concerning the improvement of Online Learning Readiness were also discussed in this study.

This paper is a summary of the tradition of British travelers in Albania during the 19th century until the First World War. Referring to British history and British cultural traditions, these travelers are classified between two periods: from Georgian era to Edwardian era. British travelers began to visit Albania frequently, especially from the time of the rule of Ali Pasha Tepelena, through whose pasha’s territory traveled many British agents, missionaries, and adventurers, including the eminent poet Gordon Byron and his friend John C. Hobhouse, and Dr. Henry Holland. The first part of the paper deals with the main studies for travelogue literature, listing the authors and their studies according to the order and study approaches. As there are hundreds of books with travel notes from British travelers on Albania and Albanians, the second part of the article focuses only on some of the most famous British travelers, such as Edward Lear, Arthur Evans, Edith Durham, Henry N. Brailsford and Aubrey Herbert. The purpose of this paper is to make a chronological history of British travelers in Albania and historical literature on this literature genre of British travelers who traveled and describes Albania of late modernity.

The Lunenburg Lymphoma Biomarker Consortium (LLBC) evaluated the prognostic value of IHC biomarkers in a large series of patients with diffuse large B-cell lymphoma (DLBCL). Clinical data and tumor samples were retrieved from 12 studies from Europe and North America, with patients treated before or after the rituximab era. Using tissue microarrays from 1514 patients, IHC for BCL2, BCL6, CD5, CD10, MUM1, Ki67, and HLA-DR was performed and scored according to previously validated protocols. Optimal cut points predicting overall survival of patients treated in the rituximab era could only be determined for CD5 (P = .003) and Ki67 (P = .02), whereas such cut points for BCL2, BCL6, HLA-DR, and MUM1 could only be defined in patients not receiving rituximab. A prognostic model for patients treated in the rituximab era identified 4 risk groups using BCL2, Ki67, and International Prognostic Index (IPI) with improved discrimination of low-risk patients. Newly recognized correlations between specific biomarkers and IPI highlight the importance of carefully controlling for clinical and biologic factors in prognostic models. These data demonstrate that the IPI remains the best available index in patients with DLBCL treated with rituximab and chemotherapy.

Este texto, escrito por Arturo Jauretche, recupera la figura del Dr. Carlos Alberto Alvarado (1904-1986) quien, en tiempos en que Ramón Carrillo era secretario de Salud Pública de la Nación, dirige una campaña que logra controlar el paludismo en el noroeste argentino con una reducción muy importante del número de casos, alcanzando una amplia repercusión mundial. En 1955, el Dr. Alvarado es separado de su cargo por la Revolución Libertadora, momento en que la Organización Panamericana de la Salud lo designa asesor regional y entre 1959 y 1964 lo nombran jefe del programa de erradicación de la malaria de la Organización Mundial de la Salud, en Ginebra, Suiza. A su regreso al país en 1966, organiza el Plan de Salud Rural, creando para ello la figura del "agente sanitario" y la "ronda sanitaria" anticipándose a conceptos que varios años después se plasmarían en la Atención Primaria de la Salud. El texto de Jauretche fue publicado originalmente en 1967 en el libro Los profetas del odio, por la editorial de Arturo Peña Lillo, editor de los grandes títulos del "pensamiento nacional" argentino. Su reedición en la sección "Memoria e historia" es un modo de rendir homenaje a una de las figuras indiscutibles de la Salud Pública argentina.

Abstract:
Tenaga pengajar (ustadz dan ustadzah) di pondok pesantren menghadapi tantangan dalam pembinaan santri. Perkembangan teknologi informasi pada era revolusi industri 4.0 telah berdampak pada pola pendidikan pondok pesantren. Oleh karena itu, diperlukan pelatihan Kepemimpinan Bertumbuh untuk meningkatkan kompetensi kepemimpinan para asatidz. Program pengabdian kepada masyarakat ini bermitra dengan Yayasan Al Asror Patemon Gunungpati Semarang. Metode yang digunakan adalah Seminar Pendidikan Strategi Kepemimpinan Bertumbuh dalam Menghadapi Tantangan Era Digital. Kegiatan ini juga bekerjasama dengan LP3 UNNES yang telah menjadikan Yayasan Al Asror sebagai mitra. Kegiatan pengabdian dilaksanakan di aula Madrasah Aliyah (MA) Al Asror, pada hari Sabtu, 8 Agustus 2020. Kegiatan diawali dengan penandatanganan dokumen rencana pelaksanaan kegiatan antara FE UNNES dan Yayasan Al Asror sebagai tindak lanjut Kerjasama (MOU) antara UNNES dan Yayasan. Narasumber kegiatan adalah Plt. Wakil Rektor Bidang Kerjasama dan Perencanaan, Dr. Hendi Pratama, MA. yang memaparkan materi mengenai “Menguatkan Reputasi Pribadi dan Lembaga Al Asror”. Narasumber kedua adalah Ketua LP3 UNNES, Dr. Ngabiyanto, M.Si. yang menyampaikan materi “Strategi Kepemimpinan Bertumbuh dalam Menghadapi Tantangan Era Digital”. Peserta kegiatan sangat antusias mengikuti kegiatan ini. Hal ini tampak pada partisipasi peserta saat narasumber mengajak dialog langsung dengan peserta. Peserta juga sangat antusias pada sesi tanya jawab. Teachers (ustadz and ustadzah) in boarding schools face challenges in the construction of students. The development of information technology in the era of industrial revolution 4.0 has had an impact on the pattern of boarding school education. Therefore, it is necessary to train “Kepemimpinan Bertumbuh” to improve the leadership competence of the asatidz. This community service program is partnered with Yayasan Al Asror Patemon Gunungpati Semarang. The method used is The Education Seminar of Growing Leadership Strategy in Facing the Challenges of the Digital Era. This activity also collaborates with LP3 UNNES which has made Al Asror Foundation as a partner. The service was held in madrasah Aliyah (MA) Al Asror hall, on Saturday, August 8, 2020. The activity began with the signing of a document on the implementation plan of activities between FE UNNES and Al Asror Foundation as a follow-up to the Cooperation (MOU) between UNNES and the Foundation. The speakers of the activity were Plt. Vice Rector for Cooperation and Planning, Dr. Hendi Pratama, MA. which presents material on "Strengthening the Personal Reputation and Institution of Al Asror". The second speaker was the Chairman of LP3 UNNES, Dr. Ngabiyanto, M.Si. who delivered the material "Leadership Strategy to Grow in Facing the Challenges of the Digital Era". Participants of the activity are very enthusiastic to participate in this activity. This is evident in the participation of participants when the speaker invites direct dialogue with the participants. Participants were also very enthusiastic at the question and answer session.
