Small biogenic changes in voltage such as action potentials in neurons can be monitored using extracellular single unit recording techniques. This technique allows for investigation of neuronal electrical activity in a manner that is not disruptive to the cell membrane, and individual neurons can be recorded from for extended periods of time. This chapter discusses the basic requirements for an extracellular recording setup, including different types of electrodes, apparatus for controlling electrode position and placement, recording equipment, signal output, data analysis, and the histological confirmation of recording sites usually required for in vivo recordings. A more advanced extracellular recording technique using piggy-back style multibarrel electrodes that allows for localized pharmacological manipulation of neuronal properties is described in detail. Strategies for successful signal isolation, troubleshooting advice such as noise reduction, and suggestions for general laboratory equipment are also discussed.

The relicts of medieval knights’ manor houses in Poland today are so called “grodziska stożkowate” (motte) - the anonymous hills having in themselves remnants of wooden buildings, exceptionally made of stone or brick and numerous tiny artifacts being the trace of the past household equipment. Unlike to the castles they are not so often visited but more often destroyed. The book presents the image of medieval knights’ manor houses, which we know due to archaeological excavations carried on for half a century. Maternal obesity is the most common pre-existing morbidity in pregnant women in the United Kingdom. Obesity is associated with increased risk of maternal and offspring morbidity and mortality. Increased maternal morbidity is multifactorial. There is an increased incidence of coexisting medical conditions. Adverse physiological changes related to obesity also contribute to risk. In addition to this, there is an increased risk of many complications developing de novo during pregnancy. There are many practical and technical challenges for the multidisciplinary team that must be addressed in order to care for the morbidly obese parturient effectively. Many items of equipment designed for use with the morbidly obese will need to be available. Due to the complexity of their care and increased risks, all women with a body mass index over 40 kg/m2 should be seen prior to labour and delivery by an anaesthetist. This allows for timely planning of their care, involvement of appropriate personnel and equipment, and expectation management. The use of neuraxial analgesia and anaesthesia, whilst prone to increased technical difficulties and failure rates, has significant advantages for many morbidly obese parturients. There are many increased risks associated with general anaesthesia in the morbidly obese, but this may be the only option for operative delivery in some super morbidly obese parturients who cannot tolerate a tilted supine position. The care of the morbidly obese parturient is truly multidisciplinary which should be coordinated by a named consultant obstetrician.

Abstract:
Skeletal muscle weakness affecting the respiratory and peripheral muscles is common in critically ill patients and can lead to difficulties in weaning, prolonged ICU admission, and significant morbidity in survivors. A number of techniques can be used to assess muscle strength. In the peripheral muscles, volitional techniques employing scoring systems or portable hand dynamometers are relatively simple and quick to use, requiring little or no specialist equipment. Such techniques can, however, only be applied to conscious and cooperative patients, preventing assessment of muscle weakness in many ICU patients. The volitional requirement also limits the ability to distinguish poor motivation and impaired cognition from true loss of muscle function. Non-volitional techniques involving motor nerve stimulation provide measures of muscle force production in non-cooperative patients but require specialist equipment. Normative data for comparative purposes are limited. Also, it is not clear which peripheral muscle best reflects generalized muscle weakness. Measurements of maximal inspiratory and expiratory pressures are widely used to assess respiratory muscle strength in ICU patients and are applicable to patients who can make some respiratory effort. As with all tests requiring patient cooperation, reliability is limited. Phrenic nerve stimulation allows direct, non-volitional assessment of diaphragm and phrenic nerve function, and normative values for comparative purposes are available. Magnetic phrenic nerve stimulation is well tolerated, can be performed in the presence of vascular catheters, and is used to document respiratory muscle weakness and track progression in critically ill patients.
