Full textMultiobjective linear programming methods with continuous variables date from 1960. It's only in 1973 that the indivisibility was taken into account in multiobjective optimization. This work is a contribution to the multiobjective integer linear problems and methods. The first part is about single and multiobjective problems with continuous variables. It includes the separation theorem used as a multiobjective case, and the theorem of the equivalence between Pareto's optimum and multiparametric program. This equivalence is a source of continuous multiobjective methods when the decision maker's preference weightings are made a priori, a posteriori or when they are progressive. These methods are used in general cases. The second part deals with multiobjective integer linear programming in theories and algorithms. The preceding equivalence theorem has been adapted to integer case in this part. Integer methods are subject of the third part; a greater place is given to a priori weightings, with integer goal programming and progressive weightings with integer interactive methods by preference revelation, because of their adaptability. Progressive preference weightings are emphasized so as to limit value judgement on objectives. So, methods are presented depending on whether variables describe n, 0-1 or they are mixed. Integer interactive methods with preferential vector have been suggested. They optimize all the objectives simultanously, respecting integer Pareto's optimum. Concerning mixed variables, the methods used were binary decomposition and median analysis

Full textThis research applies the Communicative Action Theory to the problematic of leading organizational change, impulses by politics, in small public health care organizations. More precisely we focus on the impact of the "35 hours", a reform we consider as a management tool. That law can be seen as a double injunction for organizational change and for a participative approach which seemed to be inspired by Communicative Action Theory. We follow a instrumental and a comprehensive approach of our research object. The empirical data have been collected using the " Appui-Conseil", a specific appliance associated to the law, as real Interventions-Researches. Results related to the instrumental part of our research point out that the organizational goals have been reached. The comprehensive part shows that the "habermasian" approach of change has been followed in various ways : strongly within the health care organizations; weakly for the relationships between these organizations and their administrative supervision. We envisage the possibility of an evolution of organizational structure to a "innovative" model which can be mentioned as an "communicative hospital". This scenario depends on the mastery of several risks, "the professionalization of team-project" is one example. Contributions of this research is first at a theoretical level by building an organizational change approach inspired by Communicative Action. Methodological contributions consist on our specific use of the "Appui-Conseil" appliance, which point out the possibility of what can be called as a "Intervention-Research at a large scale". There is also practical contributions of that "habermasian translation" which can be seen as a new way of leading change. The efficiency of this new approach is strongly related to the definition of the stakeholder affected by a change

Full textFirms become aware of the importance of the immaterial capital owned by their employees which corresponds to their experience and accumulated knowledge about the firm activities. This can be achieved by adopting a process of knowledge capitalizing. This process aims at identifying, preserving and enhancing the knowledge of the organization. However, considering the large amount of knowledge to be preserved and the emergence of a new type of organizations focused on processes, the health care organization must identify the sensitive organization’s processes which mobilize some knowledge that should be targeted by capitalization. In this dissertation we propose a Core Ontology of Organization’s Processes, entitled COOP and a Methodology for Identifying Sensitive Organization’s Processes for knowledge localization, entitled SOPIM. The proposed methodology is conducted in the association of protection of motors disabled of Sfax –Tunisia (ASHMS). A Decision Support System for identifying sensitive Organization’s Processes entitled OP-DSS is also implemented. OP-DSS is an implementation of the phases-based methodology COOP provides some rigorous definitions of concepts and a classification of the types of the organization’s processes. This ontology helped us in characterizing the organization’s processes and in defining the set of criteria for evaluating these processes. SOPIM is composed of two phases. The first one aims at constructing learning devoted to infer the preference model of the decision makers. The second concerns the exploitation of the preference model (decision rules) to classify the “Potential sensitive organization’s processes”

In this context, it is then necessary to wonder about a particular question that is the management of the penal risk by the establishments of health. This problem is essential because the protection of the health and the conservation of the physical integrity of the patient, but also the staffs and every outside person, are at the heart of their activity. Their objective is then to look after by facing a large number of constraints, sanitary but also economic, by avoiding the realization of the penal risk, and same by maintaining the level of the latter at least and so avoid the commitment of any penal responsibility

Full textLa gestion des équipements médicaux dans les centres de santé est cruciale, en particulier dans les services d'urgence. Cependant, en raison de la nature dynamique de ces environnements, ces équipements peuvent être indisponibles pour des raisons variées dans différentes situations. Les outils de gestion existants dépendent d'interventions manuelles, ce qui alourdit la charge de travail des professionnels de santé concernés et ne suit pas l'utilisation continue des équipements. Cette thèse propose une approche générique de solution pour gérer la localisation et l'utilisation des équipements médicaux tout en minimisant les interactions manuelles. Un méta-modèle a été développé pour permettre la création d'applications IoT spécialisées, adaptées aux besoins spécifiques de centres de santé de différents types. La recherche a commencé par une étude de la littérature sur des thèmes fondamentaux, suivie de la conception du méta-modèle, ayant fait l'objet d'évolutions de manière itérative. Des entretiens avec des professionnels de santé ont permis d'intégrer des informations sur le contexte médical. La faisabilité du méta-modèle a été testée par deux applications IoT dans des études expérimentales simulant des hôpitaux simples. La principale contribution de ce travail est la création d'un méta-modèle efficace pour développer des applications IoT adaptées à la gestion des équipements médicaux, avec un potentiel d'amélioration continue pour divers centres de santé

Full textThe restructuring of complex organizations, such as those of health facilities, is strongly influenced by recent economic, technological and regulatory changes. To understand the impact of these restructuring movements, one must analyze the changes affecting the various hierarchical levels of the healthcare organization. Under the French socio-economic context, healthcare institutions are currently undergoing multiple strategic and operational transformations, which requires professionals to adapt their skills and capabilities accordingly. In fact, the impact of medical, technical and digital innovations is driving the concerned structures to emphasize value in their adaptations. For a successful implementation, it is necessary that concerned parties intervene. These latters are then encouraged to focus on updating the organization's strategy, improving human resources development and adjusting necessary patterns and processes. Therefore, this research relies on analyzing organizational performance from a strategic alignment perspective. We aim to examine the managerial expectations of a study sample of managers, professionals, and staff in a French hospital which are undergoing full reconstruction. The results of this qualitative study show the difficulty of managers to implement a successful patient pathway process. It also shows that technological transformations can be used to ensure a very high organizational performance allowing people to take advantage of various intelligent services
