Abstract:
Neuro-palliative care is an important resource for patients and families confronting severe brain injury. Although many clinicians equate brain injury with certain death or futility, survivors have substantial needs that might be met by palliative care expertise. This chapter suggests that the boundaries of palliative medicine include those with severe brain injury, most notably those in the minimally conscious state, and that with this nosological expansion practitioners of palliative care reflect carefully on often nihilistic attitudes directed towards patients with disorders of consciousness. This chapter establishes how to better meet the needs of these patients and their surrogates, reviewing definitional criteria for the vegetative and minimally conscious states, highlighting advances in diagnostic and therapeutic interventions (such as neuroimaging, drugs, and deep brain stimulation) and considering what neuroprosthetic devices tell us of the capacity of patients to experience-and functionally communicate-pain, distress, and suffering.

Abstract:
This chapter aims to provide the psychiatrist and ethicist with an understanding of reformational Christian ethics as it pertains to the faith of the believing patient in a psychiatry context. We introduce three reformational principles and apply them cursorily to diagnostic and therapeutic issues in clinical practice, followed by a more detailed application to the topic of suffering owing to mental disorder. Understanding reformational ethics may aid the psychiatrist towards a better relationship with the believing patient and equip him for engagement on issues of guilt, remorse, whether being punished by God, and whether mental disorder results from failure in faith. Clarity on these issues may bring consolation to the believing patient. That applies also to the issue of suffering owing to mental disorder, for which we present a scholastic reformational exegesis of Colossians 1:24, exemplifying the premises and methods for examining issues of reformational Christian faith and mental disorder.

Abstract:
Cystic echinococcosis (CE)\cystic hydatid disease is one of the most widespread and important global helminth zoonoses. The parasite Echinococcus granulosus is maintained in a wide spectrum of intermediate hosts, including sheep, goats, camels, cattle, pigs and equines. A number of wild intermediate hosts occur, including cervids in the northern part of the North American continent and Eurasia, marsupials in Australia and wild herbivores in East and southern Africa. The application of a range of molecular techniques to the characterization of the parasite has confirmed the existence of mostly host-adapted strains and genotypes of the parasite and several new species have been proposed. The ubiquitous domestic dog serves as the most important definitive host for the transmission of the parasite throughout its wide geographical range.A wide range of diagnostic techniques, including necropsy, arecoline purgation, coproantigen ELISA and DNA based tests are available for detecting E. granulosus infection in the definitive host. In intermediate animal hosts, diagnosis at post mortem still remains the most reliable option. In humans, imaging techniques including ultrasound, nuclear magnetic resonance (NMR) or computer aided tomography (CAT-scan provide not only a method of diagnosis but also reveal important clinical information on the location, condition, number and size of the hydatid cysts in man. Of these ultrasound is the most widely used diagnostic technique and is the only imaging technique for screening of populations in rural areas, where the disease is most common. A classification system has been developed which can be used to assess the likely development of a cyst and hence guide the clinician in treatment options for the patient. Treatment relies on surgery and/or percutaneous interventions, especially ‘Puncture, Aspiration, Injection, Re-aspiration’ (PAIR) and/or antiparasitic treatment with albendazole (and alternatively mebendazole).CE is largely a preventable disease. Successful elimination programmes have focused on frequent periodic treatments of dogs with anthelmintics and the control of slaughter of domestic livestock. In many regions elimination or even control remains a problem as the parasite is endemic over vast areas of low income countries where there may be limited resources for control. In some areas, such as former communist administered countries, the parasite is resurgent. New tools are becoming available to control the parasite, including a highly effective vaccine in sheep which prevents the infection in sheep and breaks the transmission cycle. In addition cost effective methods are being developed which may be appropriate in low income countries where financial resources are not available for intensive control programmes that have been successful in high income countries.

Abstract:
This book is for the non-specialist who may see problems in the head and neck. The aim is to help the reader develop a targeted approach in assessment and management. Such patients may be seen in the emergency department, in general practice, or on the ward. Because true ‘emergencies’ in the head and neck (i.e. a life- or sight-threatening condition) are few in number, a more broadly defined remit has been used, to cover urgent and potentially worrying problems which may present acutely. Generally speaking, patients do not present with a ready-made diagnosis, but rather with either a symptom located to an anatomical region (e.g. toothache, lump, or headache), or an obvious problem (e.g. nose bleed or injury). This is the starting point in each of the anatomically based chapters (‘Common presentations’ and ‘Common problems and their causes’). For each symptom there are a number of possible causes and these are listed. The next section in each chapter (‘Useful questions and what to look for’) lists the important diagnostic elements in relation to each symptom. The aim is to equip the reader with the necessary knowledge to enable them to quickly and accurately triage and diagnose a symptom or clinical problem. The remainder of each chapter details how to examine each site, useful investigations, and some notes on the conditions. Management and referral is also covered. This book is particularly helpful for those unfamiliar with ‘acute conditions’ of the head and neck and for those preparing for clinical examinations.
