Abstract:
Patients with diabetes require regular examination for eye disease, usually in primary care settings. Guidelines recommend patients with at least moderate non-proliferative diabetic retinopathy (NPDR) be referred to an ophthalmologist for treatment; however, poorly targeted referrals lead to access blocks. The quality of new referrals associated with diabetes to a public ophthalmology service in Sydney, New South Wales, Australia, were assessed for referral completeness and targeting. A cross-sectional audit of medical records for new patients referred to Westmead Hospital Eye Clinic in 2016 was completed. Completeness of medical and ophthalmic information in referrals and subsequent patient diagnosis and management in 2016–17 was recorded. Sub-analyses were conducted by primary care referrer type (GP or optometrist). In total, 151 new retinopathy referrals were received; 12% were sent directly to a treatment clinic. Information was incomplete for diabetes status (>60%), medical (>50%) and ophthalmic indicators (>70%), including visual acuity (>60%). GP referrals better recorded medical, and optometrists (37%) ophthalmic information, but information was still largely incomplete. Imaging was rarely included (retinal photos <1%; optical coherence tomography <3%). Median appointment wait-time was 124 days; 21% of patients received treatment (laser or anti-vascular endothelial growth factor) at this or the following encounter. Targeting referrals for ocular complication of diabetes to public hospitals needs improvement. Education, feedback and collaborative care mechanisms should be considered to improve screening and referral in primary care.

Abstract:
Ozone and fine particles (PM2.5) are the two main air pollutants of concern in the New South Wales Greater Metropolitan Region (NSW GMR) due to their contribution to poor air quality days in the region. This paper focuses on source contributions to ambient ozone concentrations for different parts of the NSW GMR, based on source emissions across the greater Sydney region. The observation-based Integrated Empirical Rate model (IER) was applied to delineate the different regions within the GMR based on the photochemical smog profile of each region. Ozone source contribution was then modelled using the CCAM-CTM (Cubic Conformal Atmospheric model-Chemical Transport model) modelling system and the latest air emission inventory for the greater Sydney region. Source contributions to ozone varied between regions, and also varied depending on the air quality metric applied (e.g., average or maximum ozone). Biogenic volatile organic compound (VOC) emissions were found to contribute significantly to median and maximum ozone concentration in North West Sydney during summer. After commercial and domestic sources, power generation was found to be the next largest anthropogenic source of maximum ozone concentrations in North West Sydney. However, in South West Sydney, beside commercial and domestic sources, on-road vehicles were predicted to be the most significant contributor to maximum ozone levels, followed by biogenic sources and power stations. The results provide information that policy makers can use to devise various options to control ozone levels in different parts of the NSW Greater Metropolitan Region.

Abstract:
An evaluation of the effect of sex re-assignment surgery on a group of patients attending a private clinic in Sydney, Australia. Fifty-seven patients who underwent full male-to-female sex re-assignment surgery between 1987 and 2000 completed a satisfaction survey. Several factors that might influence the extent of satisfaction with surgical outcome were explored, including age, work status, social life, and the appearance and function of the new genitalia. Patients reported significantly improved social and personal satisfaction following surgery, compared with five years previously. The study challenges outcomes from previously reported studies with regard to the age of patients at the time of surgery, and the finding that from the patient's perspective, there is no fundamental association between a successful surgical outcome and a satisfactory post-operative life experience.

As in many growing urban areas, the prevention of environmental damage as Sydney spreads westward into the Hawkesbury-Nepean River basin is a major challenge for planners, managers and the local community, We surveyed macroinvertebrates at 45 river and stream sites in April-June 1996, and reviewed data from other sources, in order to assess the issues involved in conservation of the lotic macro invertebrate fauna, Regional richness is high with 443 recorded species and morphospecies, Cluster analysis showed community pattems related mainly to waterway size (separating the Hawkesbury-Nepean River from tributary streams), geology (tributaries on shale or sandstone), tidal intrusion and urbanization (impoverished faunas in urban streams). The ability of genus richness of mites and major insect orders to reflect overall genus richness at a site was limited, and Diptera and Trichoptera appear to have the greatest value as biodiversity predictors. Urban expansion is the major threat to lotic macro invertebrate communities in the region, but agriculture, flow regulation, sand and gravel ex1raction and introduced species have probably also impacted on the fauna, Streams with high conservation value for macroinvertebrates include those few on the Cumberland Plain and surrounding slopes that retain substantial indigenous vegetation in relatively undeveloped catchments. The Hawkesbury-Nepean River sfill harbours a rich faunal community and is also important for conservation, The development of effective strategies for conservation assessment and management is problematic for several reasons, Some species in the region are known to be vulnerable, but the status of most cannot be assessed because of a lack of taxonomic and zoogeographic information, Most "biodiversity indicator" concepts are of dubious value for aquatic macroinvertebrate conservation. A multi-faceted management approach emphasizing subcatchment reserves, riparian restoration and the control of threatening processes is required.

This paper describes a service-based initiative to enhance capacity for evidence-based practice (EBP) in the South Western Sydney Area Health Service Division of Population Health. A working group planned an organisational response to a customised EBP needs assessment using the New South Wales Department of Health?s framework for capacity building focussing on five key action areas; organisational development, workforce development, resource allocation, leadership and partnerships. Innovative strategies to promote EBP were developed and implemented and on-site training programs that targeted specific groups of staff were conducted. Because there was commitment and leadership from senior staff for the initiative, a comprehensive approach to building capacity for EBP in population health was possible. Evidence of impact needs to be collected in the future.

Purpose: This study identifies the attitudes of participants in the root cause analysis (RCA) process and barriers to it?s implementation within one New South Wales area health service. Method: Employees and consumer representatives of the former South Western Sydney Area Health Service who participated in an RCA as either a team member or a team leader between December 2002 and October 2003 completed a self-administered survey. Results: Thirty seven of 39 eligible participants completed the survey (response rate 95%). The respondents identified formulation of causal statements, ensuring the causal statements met the ?rules of causality? outlined by New South Wales Health, and arranging times for interviews as most difficult. Team leader respondents (n = 7) ranked keeping the team focused, organising the first meeting within 7 days of the incident, and completing the RCA in three 2-hour meetings as barriers to the process. Conclusions: Training was valued by participants, however greater emphasis on the development of causal statements could be beneficial. Team leaders expressed difficulty in keeping the team focused and meeting the stipulated RCA timeframes, suggesting that additional support for RCA participants may be warranted.

Abstract:
A comprehensive evaluation of the performance of the coupled Conformal Cubic Atmospheric Model (CCAM) and Chemical Transport Model (CTM) (CCAM-CTM) for the New South Wales Greater Metropolitan Region (NSW GMR) was conducted based on modelling results for two periods coinciding with measurement campaigns undertaken during the Sydney Particle Study (SPS), namely the summer in 2011 (SPS1) and the autumn in 2012 (SPS2). The model performance was evaluated for fine particulate matter (PM2.5), ozone (O3) and nitrogen dioxide (NO2) against air quality data from the NSW Government’s air quality monitoring network, and PM2.5 components were compared with speciated PM measurements from the Sydney Particle Study’s Westmead sampling site. The model tends to overpredict PM2.5 with normalised mean bias (NMB) less than 20%, however, moderate underpredictions of the daily peak are found on high PM2.5 days. The PM2.5 predictions at all sites comply with performance criteria for mean fractional bias (MFB) of ±60%, but only PM2.5 predictions at Earlwood further comply with the performance goal for MFB of ±30% during both periods. The model generally captures the diurnal variations in ozone with a slight underestimation. The model also tends to underpredict daily maximum hourly ozone. Ozone predictions across regions in SPS1, as well as in Sydney East, Sydney Northwest and Illawarra regions in SPS2 comply with the benchmark of MFB of ±15%, however, none of the regions comply with the benchmark for mean fractional error (MFE) of 35%. The model reproduces the diurnal variations and magnitudes of NO2 well, with a slightly underestimating tendency across the regions. The MFE and normalised mean error (NME) for NO2 predictions fall well within the ranges inferred from other studies. Model results are within a factor of two of measured averages for sulphate, nitrate, sodium and organic matter, with elemental carbon, chloride, magnesium and ammonium being underpredicted. The overall performance of CCAM-CTM modelling system for the NSW GMR is comparable to similar model predictions by other regional airshed models documented in the literature. The performance of the modelling system is found to be variable according to benchmark criteria and depend on the location of the sites, as well as the time of the year. The benchmarking of CCAM-CTM modelling system supports the application of this model for air quality impact assessment and policy scenario modelling to inform air quality management in NSW.

We found that the prevalence of risks and special needs varied significantly by language groups in this study sample. Our results suggest that equity of access to quality care for this group of mothers and infants can be assured if models similar to the EOLO in SWSAHS are thoroughly planned and developed, and take account of cultural appropriateness for the population served.

Dust storms originating from Central Australia and western New South Wales frequently cause high particle concentrations at many sites across New South Wales, both inland and along the coast. This study focussed on a dust storm event in February 2019 which affected air quality across the state as detected at many ambient monitoring stations in the Department of Planning, Industry and Environment (DPIE) air quality monitoring network. The WRF-Chem (Weather Research and Forecast Model—Chemistry) model is used to study the formation, dispersion and transport of dust across the state of New South Wales (NSW, Australia). Wildfires also happened in northern NSW at the same time of the dust storm in February 2019, and their emissions are taken into account in the WRF-Chem model by using Fire Inventory from NCAR (FINN) as emission input. The model performance is evaluated and is shown to predict fairly accurate the PM2.5 and PM10 concentration as compared to observation. The predicted PM2.5 concentration over New South Wales during 5 days from 11 to 15 February 2019 is then used to estimate the impact of the February 2019 dust storm event on three health endpoints, namely mortality, respiratory and cardiac disease hospitalisation rates. The results show that even though as the daily average of PM2.5 over some parts of the state, especially in western and north western NSW near the centre of the dust storm and wild fires, are very high (over 900 µg/m3), the population exposure is low due to the sparse population. Generally, the health impact is similar in order of magnitude to that caused by biomass burning events from wildfires or from hazardous reduction burnings (HRBs) near populous centres such as in Sydney in May 2016. One notable difference is the higher respiratory disease hospitalisation for this dust event (161) compared to the fire event (24).

Abstract Background Transmitted human immunodeficiency virus type 1 (HIV-1) drug resistance (TDR) is an important contributor to antiretroviral treatment failure, and is associated with HIV-1 transmission among men who have sex with men (MSM), non-MSM clusters and individuals diagnosed with concurrent sexually transmissible infections (STI). Western Sydney has a culturally diverse population, with a high proportion of non-Australian-born individuals. This study describes the prevalence of TDR and non-B HIV-1 subtypes in a clinic-based population. Methods: A clinic database was examined for all newly diagnosed treatment-naïve HIV-1 patients and information on their HIV-1 resistance and subtype, demographics including country of birth and diagnosis of a bacterial sexually transmissible infection was collected. Results: Data were available from 74/79 individuals (62 cis-male, 16 cis-female and 1 transgender woman). Of the 74 genotypes, the prevalence of non-B subtypes and TDR was 43/74 (58%; 95%CI = 46.9–69.3) and 14/74 (19%; 95%CI = 10.0 to 27.8). It was also found that 30/79 (38%) had a concurrent bacterial STI. TDR was associated with subtype B infection (OR 3.53; 95%CI = 1.41–8.82; P = 0.007) and being born in Australia (OR 12.0; 95%CI = 2.45–58.86; P = 0.002). Conclusion: The relative prevalence of non-B HIV-1 subtypes and TDR is higher in Western Sydney than in the rest of Australia. TDR is associated with subtype B HIV-1 and being Australian born, suggesting ongoing local transmission. This highlights the diversity of the HIV epidemic locally and the need for interventions to prevent ongoing HIV transmission.

Abstract:
This paper presents a summary of the key findings of the special issue of Atmosphere on Air Quality in New South Wales and discusses the implications of the work for policy makers and individuals. This special edition presents new air quality research in Australia undertaken by (or in association with) the Clean Air and Urban Landscapes hub, which is funded by the National Environmental Science Program on behalf of the Australian Government’s Department of the Environment and Energy. Air pollution in Australian cities is generally low, with typical concentrations of key pollutants at much lower levels than experienced in comparable cities in many other parts of the world. Australian cities do experience occasional exceedances in ozone and PM2.5 (above air pollution guidelines), as well as extreme pollution events, often as a result of bushfires, dust storms, or heatwaves. Even in the absence of extreme events, natural emissions play a significant role in influencing the Australian urban environment, due to the remoteness from large regional anthropogenic emission sources. By studying air quality in Australia, we can gain a greater understanding of the underlying atmospheric chemistry and health risks in less polluted atmospheric environments, and the health benefits of continued reduction in air pollution. These conditions may be representative of future air quality scenarios for parts of the Northern Hemisphere, as legislation and cleaner technologies reduce anthropogenic air pollution in European, American, and Asian cities. However, in many instances, current legislation regarding emissions in Australia is significantly more lax than in other developed countries, making Australia vulnerable to worsening air pollution in association with future population growth. The need to avoid complacency is highlighted by recent epidemiological research, reporting associations between air pollution and adverse health outcomes even at air pollutant concentrations that are lower than Australia’s national air quality standards. Improving air quality is expected to improve health outcomes at any pollution level, with specific benefits projected for reductions in long-term exposure to average PM2.5 concentrations.

The aim of this study is to compare the prevalence of chronic disease risk factors in humanitarian arrivals to Sydney, New South Wales (NSW) with the Australian Indigenous and non-Indigenous populations aged 35–44 years. Data on risk factors collected from 237 refugees presenting to the NSW Refugee Health Service (RHS) from January 2015 to August 2016 were retrospectively analysed and compared with data from the Australian Health Surveys, 2011–13 for the Indigenous and non-Indigenous Australian populations. This study found significantly higher levels of triglycerides (z=3; 95% CI, 0.16–0.26); hypertension (z=3.2; 95% CI, 0.17–0.29); and smoking (z=3.5; 95% CI, 0.27–0.33) in refugees compared with the general Australian population. The Indigenous population had significantly higher levels of triglycerides (z=4; 95% CI, 0.16–0.26); body mass indexes (BMIs) (z=3.3; 95% CI 0.58–0.72); and smoking (z=5.4; 95 CI 0.27–0.33) compared with refugees. Based on the study findings, screening for chronic disease risk factors from age 35 years may be warranted in all humanitarian arrivals to Australia, along with dietary and lifestyle advice.

The population dynamics of the common wallaroo or euro (Macropus robustus erubescens) were investigated in two adjacent sites in far western New South Wales. Wallaroo densities were generally higher in a site of high relief (South Ridge) than in one of low relief (South Sandstone); however, both sites exhibited large fluctuations in numbers (ranges of 2.23-18.31 per km*2 and 3.48-19.99 per km*2, respectively). The proportion of adult males relative to adult females was significantly higher in South Sandstone (c. 1.1 : 1) than in South Ridge (c. 0.4: 1), indicating a difference in habitat usage by the sexes. At both sites, fluctuations in overall density were best explained by changes in the density of adult females; however, the relative importance of changes in the numbers of other size-sex classes in determining density fluctuations differed between the two sites. Total density was significantly related to the previous rainfall regime in South Ridge but not in South Sandstone. Reproductive condition of females and survivorship of young were related to environmental conditions. Adult mortality ranged from 4.55 to 25.81% per year and adult survivorship was positively correlated with the abundance of grass. Evidence is presented to support the hypothesis that dispersal of subadults is predominantly a male phenomenon.

AbstractFaced with the paradox of a large global increase in conservation reserves and a simultaneous global decrease in actual effective protection for biodiversity, conservation scientists and policymakers are questioning established conservation theory and practice. I argue that the fundamental premises, the foundational myths, for Western-style conservation also need to be questioned. The statistics on Indigenous land claims, and conservation reserves, in Australia and more specifically the state of New South Wales (NSW), reveal a landscape of policy failure in both arenas. Focusing on Australia, I use spatial analysis and policy histories to demonstrate converging trajectories of land use priorities for conservation needs and Indigenous peoples' needs. This intersection, while generating much potential for conflict, also creates new political landscapes. A combination of spatial and cultural analyses can create a clear picture of new “operational landscapes”, and an understanding of the (sometimes) complementary values of different cultural groups negotiating about these landscapes. From the basis that environmental problems are fundamentally social problems, this paper contributes to explorations of new paradigms supporting new social-ecological relationships, and new relationships between Indigenous and non-Indigenous peoples.

Rates of drug use remain substantially higher among gay and bisexual men (GBM) and people living with HIV (PLHIV) in Sydney, New South Wales, Australia. The use of drugs to enhance sexual pleasure within cultures of Party and Play creates opportunities to discuss sexual health, mental health, consent and wellbeing. Community organisations with a history of HIV prevention, care, treatment are well-placed to respond. ACON’s (formerly the AIDS Council of New South Wales) multi-dimensional response to ‘chemsex’ includes: direct client services support for individuals seeking to manage or reduce their use; health promotion activities that support peer education; partnerships with research institutions to better understand cultures of chemsex; and policy submissions that call for drug use to be approached as a health, rather than a criminal, issue. Breast cancer is the most common cause of cancer amongst Australian women and the second most common cause of cancer mortality. Despite the proven effectiveness of early intervention, screening rates remain subpar across many regions in New South Wales (NSW). Screening rates are particularly low within the culturally and linguistically diverse (CALD) area of South Western Sydney (SWS). The objective of this study was to qualitatively explore barriers and facilitators to breast screening from the perspectives of CALD women from SWS. CALD women aged ≥40 who resided in SWS were invited to participate in a semi-structured interview to explore barriers and facilitators to breast cancer screening. Interviews were recorded, transcribed verbatim and analysed thematically to identify recurring patterns in the data. Sixteen women from CALD backgrounds participated. Women in this study reported absence of symptoms, fatalistic beliefs and embarrassment during the procedure to be the primary reasons for reluctance to screen. Lack of general practitioner (GP) endorsement, transport issues and pain associated with the procedure were also reported as additional barriers to screening. Common facilitators to screening included encouragement from family and friends, family history of cancer and media adverts. CALD women have distinctive barriers to mammography, which lead to poor breast screening participation rates. Opportunistic health promotion in this area is warranted and may lead to better health outcomes amongst this population.

Rainwater tanks are increasingly adopted in Australia to reduce potable water demand and are perceived to reduce the volume of stormwater discharge from developments. This paper investigates the water balance of rainwater tanks, in particular the possible impacts these tanks could have in controlling the stormwater discharge volume. The study collected water quantity data from two sites in the Hawkesbury City Council area, New South Wales, Australia and utilised the collected data in a simple water balance model to assess the effectiveness of rainwater tanks in reducing the stormwater discharge volume. The results indicate that a significant reduction in discharge volume from a lot scale development can be achieved if the rainwater tank is connected to multiple end-uses, but is minimal when using irrigation alone. In addition, the commonly used volumetric runoff coefficient of 0.9 was found to over-estimate the runoff from the roof areas and to thereby under-estimate the available volume within the rainwater tanks for retention or detention. Also, sole reliance on the water in the rainwater tanks can make the users aware of their water use pattern and water availability, resulting in significant reductions in water use as the supply dwindles, through self-imposed water restrictions.

In the aftermath of the Cronulla riots, schools were faced with the fallout of social conflict, including having to deal with widespread fear and confusion both in their local communities and among students. This was especially the case for schools in the Sutherland Shire and in the local government area (LGA) of Bankstown. Apart from the presence of many young people in the initial riot and the revenge raids, some schools, like churches, had been the target of attacks (Leys and Box, 2005: 1; Daily Telegraph, 2005: 5). Schools were also targeted as places to battle the consequences of cultural division: the then Prime Minister John Howard, in his Australia Day speech just over a month after the riots, complained that the teaching of Australian history in schools needed reform to properly foster the core values that would bind a nation together (Sydney Morning Herald, 2006). At all levels of government, a raft of programs designed to ease local tensions were introduced, many of which focused on young people or on schools (see Board of Studies New South Wales, 2007; Surf Life Saving NSW, 2006). This article outlines the contexts for understanding the role of schools: both in terms of the spatial dynamics of the ‘Shire’ and in terms of the changing nature of educational policy. It then focuses on a National Values Education Project (NVEP) involving five schools in south and south-western Sydney as a direct response to the Cronulla riot. It suggests that these contexts produce both a degree of cultural heterogeneity in young people’s social lives and a degree of segregation amongst young people in schooling which delimits ‘what is possible’ in terms of schooling responses to the Cronulla riot.

This paper reports a qualitative pilot study exploring primary care health practitioners’ perspectives on the management of insomnia following the extensive media coverage on the adverse effects of zolpidem in 2007–08. General practitioners and community pharmacists were recruited throughout metropolitan Sydney, New South Wales using a convenience sampling and snowballing technique. Demographic information was collected from each participant followed by a semistructured interview. In total 22 participants were interviewed, including eight general practitioners and 14 community pharmacists. Interview transcripts were analysed using ‘framework analysis’. Participants’ responses illuminated some of the key issues facing primary care practitioners in the management of insomnia. Practitioners perceived there to be an overreliance on pharmacotherapy among insomnia patients and inadequate support for directing patients to alternative treatment pathways if they require or prefer non-pharmacological management. Current prescribing trends appear to favour older benzodiazepines in new cases of insomnia whereas some successful sporadic users of zolpidem have continued to use zolpidem after the media coverage in 2007–08. The findings of this pilot study suggest the need to address the limitations in the management of insomnia within the current health care system, to revise and disseminate updated insomnia guidelines and to provide educational opportunities and resources to primary care practitioners concerning management options.

Many Australian adults with type 2 diabetes mellitus (T2DM) do not follow recommended self-management behaviours that could prevent or delay complications. This exploratory study aimed to investigate the factors influencing self-management of T2DM in general practice. Semi-structured qualitative interviews were conducted with patients with T2DM (n = 10) and their GPs (n = 4) and practice nurses (n = 3) in a low socioeconomic area of Sydney, New South Wales, Australia. The interviews were analysed thematically using the socio-ecological model as a framework for coding. Additional themes were derived inductively based on the explicitly stated meaning of the text. Factors influencing self-management occurred on four levels of the socio-ecological model: individual (e-health literacy, motivation, time constraints); interpersonal (family and friends, T2DM education, patient-provider relationship); organisational (affordability, multidisciplinary care); and community levels (culture, self-management resources). Multi-level strategies are needed to address this wide range of factors that are beyond the scope of single services or organisations. These could include tailoring health education and resources to e-health literacy and culture; attention to social networks and the patient–provider relationship; and facilitating access to affordable on-site allied health services.

Background Eleven percent of people living with HIV in Australia remain unaware of their diagnosis, and there are missed opportunities for HIV testing in priority settings in New South Wales. HIV testing remains low outside of sexual health clinics with the exception of antenatal settings where HIV testing is routine. To understand why HIV testing rates are low, we sought to identify health worker-related barriers to HIV testing. Methods: We conducted an anonymous online survey to health workers in Western Sydney Local Health District (WSLHD) in September 2019. Tick-box, Likert scale responses were analysed using Chi-square and Kruskal–Wallis statistical tests, and free text responses were analysed with thematic analysis. Results: Three percent (n = 420) of WSLHD’s estimated 14 000 health workers responded. These included 317 clinicians (171 nurses, 65 doctors, 56 allied health professionals (AHPs), 25 midwives, and 103 health workers in non-clinical roles). Health workers were from a variety of in-patient/out-patient settings. Many health workers (291/420, 69%; 95%CI = 64.9–73.7%) were unaware that HIV testing is offered in their areas; doctors (82%) and midwives (80%) were more aware than nurses (23%) and AHPs (11%) (P < 0.0001). Doctors (Likert score = 3.62; 3.45/5) and midwives (2.84; 2.76) were significantly more comfortable discussing and confidently offering HIV testing than nurses (2.42; 1.81) or AHPs (1.83; 0.91) (P < 0.0001 for both). The top five barriers to HIV testing were (1) procedural knowledge, (2) identification of at-risk patients, (3) HIV knowledge, (4) positive result management, and (5) privacy concerns. Free text responses highlighted perceived stigma, testing/result responsibilities and resource challenges as barriers to HIV testing. Conclusions: Clinicians working in priority settings and with priority populations require more education and support to increase targeted HIV testing.

This study investigated whether the implementation of a multicomponent intervention project could improve compliance with the New South Wales (NSW) Health smoke-free healthcare policy. Environmental interventions were implemented on the hospital grounds for a 12-week period. Compliance was measured by a pre- and postintervention observational count of smokers on the hospital grounds. After implementation, there was a significant 73% reduction in people smoking on the hospital grounds, indicating improved compliance with the NSW smoke-free healthcare policy through the use of a non-punitive multicomponent intervention. What is known about the topic?Lack of compliance with the NSW Health smoke-free healthcare policy was reflected by the large numbers of smokers outside the front of an inner-city hospital in Sydney, Australia. This issue had been raised by staff, patients and visitors as an ongoing problem requiring a solution. What does this paper add?Designing and implementing a non-punitive multicomponent intervention using modelling, operant and associative learning principles can successfully increase compliance with policies designed to stop smoking on hospital grounds. What are the implications for practitioners?Implementing non-punitive multicomponent interventions to improve compliance with the NSW Health smoke-free healthcare policy may increase patients’ acceptance of smoking cessation support when offered. Healthcare staff need to be equipped with the knowledge and confidence to offer this support.

Purpose – The purpose of this paper is to examine the framework, operation and prospects of two innovative planning tools – biobanking and biodiversity certification – to protect biodiversity in the context of urbanisation. Design/methodology/approach – Using the Australian State of New South Wales, and its capital Sydney, as a focus, the paper reviews the history and performance of the two planning tools selected for examination. Findings – Through its analysis, this paper supports the use of biobanking and biodiversity certification as tools at the disposal of planners which should be employed to augment the array of tools to manage the impacts on biodiversity of urban development. Indeed, these tools have the potential to increase the level of conserved biodiversity if used appropriately. Originality/value – Biobanking and biodiversity certification are, respectively, examples of market-based and strategic-based statutory planning mechanisms that can be used to protect biodiversity in an urban development context. Creative adaption of environmental planning laws is required to ensure effective implementation of these innovative planning tools. The relevant contemporary statutory enactments that have given effect to these mechanisms are examined in this paper.

Relocating a natural history collection is a daunting prospect. Underpinning successful relocation is getting the fundamentals right. From the moment the seed of an idea for a new facility is planted, a raft of detailed planning and preparation issues emerge. Meticulous planning and management is essential, from initial design through to the last specimen being housed in its new location. Herbaria are complex organisms each with a core collection of specimen sheets and associated infrastructure to house them; ancillary collections such as ‘spirit’ and ‘DNA’, a library, databasing, mounting, materials, imaging, loans and exchange, facilities for environmental control, biosecurity, space for staff, volunteers, research students, and class or public access and outreach. All these elements require careful consideration for relocation regardless of the size of the collection. Timelines for relocations from initial decisions to commencement of the move vary widely. Early involvement of core herbarium staff is critical to managing risks to the integrity of the collection during a move. Success of the operation can be gauged immediately after the move and again, much later, based on feedback on the operation of the facility and whether planned expansion will meet future needs. All these considerations are important and essentially the same, irrespective of distance of relocation or size of the collection. We will discuss the fundamental issues of herbarium relocation based on two recent case studies.The Western Australian Herbarium moved from its 1970s home to a modern, purpose-built, best practice facility incorporating innovative design features in 2011 with c. 800,000 specimens. The John T. Waterhouse Herbarium at UNSW Sydney (The University of New South Wales) moved c. 66,000 specimens in October 2017 from within a 1962 departmental building, to a modern, purpose-built facility, incorporating significant improvements, as part of a much larger relocation of its School. We will provide a guide to assist future relocations, both imminent (such as the N.C.W. Beadle Herbarium at the University of New England (>100,000 specmens), and the National Herbarium of New South Wales, >1,400,000 specimens) and for those yet to be considered. This will be a presentation on behalf of the Managers of Australasian Herbarium Collections (MAHC), a network of herbarium Collection Managers in Australia and New Zealand.

Background: Ensuring patients receive post-test discussion when collecting HIV test results is an integral component of the HIV testing process. New South Wales Health Department (NSW Health) policy recommends that all patients be given their HIV results in person. We assessed the number of patients who returned for HIV test results to Royal Prince Alfred Sexual Health Clinic in Sydney, Australia, and predictors of return. Methods: The files of 218 patients having consecutive HIV tests from the beginning of January to the end of April 2007 were manually reviewed. Non-consenting patients and those returning to the clinic for another reason were excluded. Multivariate logistic regression was used to determine factors associated with return for HIV results in person within 4 weeks of having the test. Results: Seventy-two of 159 patients (45%) returned for their HIV result within 4 weeks of testing. Independent predictors of return were male gender (P = 0.041), attending the outreach men-only (v. base) clinic (P = 0.017), first HIV test at the clinic (P = 0.002) and sex overseas in the past year (P = 0.048). Conclusion: Over one-half of patients did not collect their HIV results in person and thus did not receive any post-test discussion. The strongest predictor of return for HIV test results was having a first HIV test at the clinic. Current NSW Health policy is failing to achieve high levels of HIV post-test discussion. For many patients, giving results by telephone may be a more appropriate strategy to ensure HIV post-test discussion.

IntroductionResidential fires remain a significant global public health problem. It is recognised that the reported number of residential fires, fire-related injuries and deaths significantly underestimate the true number. Australian surveys show that around two-thirds of respondents who experience a residential fire are unwilling to call the fire service, and international studies highlight that many individuals who access medical treatment for fire-related injuries do not have an associated fire incident report. The objectives of this study are to quantify the incidence, health impacts, risk factors and economic costs of residential fires in New South Wales (NSW), Australia.Methods and analysisThe RESFIRE cohort will include all persons living at an NSW residential address which experienced a fire over the period 2005–2014. Nine data sources will be linked to provide a comprehensive picture of individual trajectories from fire event to first responder use (fire and ambulance services), emergency department presentations, hospital admissions, burn out-patient clinic use and death. These data will be used to describe the circumstances and characteristics of residential fires, provide a profile of fire-related injuries, examine trends over time, and explore the relationship between fire circumstance, emergency and health services utilisation, and health outcomes. Regression modelling, including multilevel modelling techniques, will be used to explore factors that impact on these relationships. Costing models will be constructed.Ethics and disseminationEthical approval for this study has been obtained from the NSW Population and Health Service Research Ethics Committee and Western Sydney University Human Research Ethics Committee. Inconsistencies can commonly be expected between the financial goals of rangeland grazing enterprises and public conservation goals such as maintenance of ground cover to reduce erosion. Where the State wishes to promote conservation outcomes, incentive schemes which reward these outcomes on privately managed grazing lands are an option. We describe one such scheme intended to achieve conservation outcomes and support the development of resilience in the complex adaptive (human–environmental) rangeland system through payments related to measured ground cover. A pilot program in western New South Wales has shown that the practical operation of such a program is uncomplicated and that while several theoretical issues could be further refined there is a rationale for extension of the program based on parameters and processes that are agreed by the participants. We suggest that development of such a scheme should be considered as part of the policy mix related to natural resource management and drought assistance.

This study explored the perceived healthcare system and process barriers and enablers experienced by GPs and Credentialled Diabetes Educators (CDEs) in Australian primary care, in the delivery of preventative and early intervention foot care to people with diabetes. A qualitative design with inductive analysis approach was utilised and reported according to the Consolidated Criteria for Reporting Qualitative Studies (COREQ). Semi-structured interviews were conducted with two GPs and 14 CDEs from rural, urban and metropolitan areas of Australia. Participants were from New South Wales, South Australia, Victoria, Western Australia, the Northern Territory and Queensland. Barriers to providing foot care constituted five broad themes: (1) lack of access to footcare specialists and services; (2) education and training insufficiencies; (3) human and physical resource limitations related to funding inadequacies; (4) poor care integration such as inadequate communication and feedback across services and disciplines, and ineffectual multidisciplinary care; and (5) deficient footcare processes and guidelines including ambiguous referral pathways. Enablers to foot care were found at opposing ends of the same spectra as the identified barriers or were related to engaging in mentorship programs and utilising standardised assessment tools. This is the first Australian study to obtain information from GPs and CDEs about the perceived barriers and enablers influencing preventative and early intervention diabetes-related foot care. Findings offer an opportunity for the development and translation of effective intervention strategies across health systems, policy, funding, curriculum and clinical practice, in order to improve outcomes for people with diabetes.

This paper describes the results of a census of newly registered clients attending community-based health services in a large health region on the south western outskirts of Sydney to enumerate the client base of these services and to investigate the reach to population groups. Two questionnaires, one for adults and the other for children were administered to all new clients of community, allied and dental health services. These questionnaires collected enhanced demographic information during a three month period that was compared with the resident population. 10,734 new clients were registered. These included 2,638 adults and 3,047 children who were attending community health services. Older adults were over-represented in the adult client population while adults accompanying children to services were largely aged 20-39 years. The data suggest good reach of services to ethnic minority groups when country of birth was considered (32.3% NESB versus 30.4% in the resident population). When language spoken at home was used those who spoke a language other that English at home were under-represented in these data (31.6%) compared to the resident population (39.7%). Adults attending with a child were more likely to be married and to have stayed at school until at least 17 years. The study was the first attempt in NSW to enumerate the client base of community-based health services and was important in demonstrating to staff the value of adequate systems to monitor the use and reach of services. Farm dams are a ubiquitous limnological feature of agricultural landscapes worldwide. While their primary function is to capture and store water, they also have disproportionally large effects on biodiversity and biogeochemical cycling, with important relevance to several Sustainable Development Goals (SDGs). However, the abundance and distribution of farm dams is unknown in most parts of the world. Therefore, we used artificial intelligence and remote sensing data to address this critical global information gap. Specifically, we trained a deep learning convolutional neural network (CNN) on high-definition satellite images to detect farm dams and carry out the first continental-scale assessment on density, distribution and historical trends. We found that in Australia there are 1.765 million farm dams that occupy an area larger than Rhode Island (4678 km2) and store over 20 times more water than Sydney Harbour (10,990 GL). The State of New South Wales recorded the highest number of farm dams (654,983; 37% of the total) and Victoria the highest overall density (1.73 dams km−2). We also estimated that 202,119 farm dams (11.5%) remain omitted from any maps, especially in South Australia, Western Australia and the Northern Territory. Three decades of historical records revealed an ongoing decrease in the construction rate of farm dams, from >3% per annum before 2000, to ~1% after 2000, to <0.05% after 2010—except in the Australian Capital Territory where rates have remained relatively high. We also found systematic trends in construction design: farm dams built in 2015 are on average 50% larger in surface area and contain 66% more water than those built in 1989. To facilitate sharing information on sustainable farm dam management with authorities, scientists, managers and local communities, we developed AusDams.org—a free interactive portal to visualise and generate statistics on the physical, environmental and ecological impacts of farm dams.

<p><strong>Abstract.</strong> Australia’s cities face significant social, economic and environmental challenges, driven by population growth and rapid urbanisation. The pressure to increase housing availability will lead to greater levels of high-density and medium-density stock. However, there is enormous political and community pushback against this. One way to address this challenge is to encourage medium-density living solutions through “precinct” scale development. Precinct-scale development has the potential to include additional hard and soft infrastructure that may offset the perceived negativities of higher densities. As part of Australian research into precinct-scale development, and as part of our broader Smart Cities approach, or more specifically City Analytics approach, new digital planning tools &amp;ndash; Envision and ESP &amp;ndash; have been developed to support scenario planning and design needs. They utilise a data-driven and scenario planning approach underpinned by Geographic Information System (GIS) functionality.</p><p>We focus on a case study in the City of Blacktown, Western Sydney, New South Wales, Australia. By 2036 Blacktown is forecast to grow to approximately 500,000 people (an increase of over 30<span class="thinspace"></span>%) and 180,000 dwellings. Most new dwellings will be delivered through urban infill. The Blacktown master plan promotes higher density housing, mixed employment uses and continued improvements to the public domain. Our study provides a unique opportunity to implement this broad strategy within a specific case and location. Specifically, this paper provides information on how these digital planning tools supported Blacktown planners in identifying, co-designing and implementing a new approach for precinct level planning. It also presents the results of an evaluation of digital-planning tools in the context of the Blacktown case study.</p>

Abstract Background Patient-delivered partner therapy (PDPT) is a method for an index patient to give treatment for genital chlamydia to their sexual partner(s) directly. In Australia, PDPT is considered suitable for heterosexual partners of men and women, but is not uniformly endorsed. We explored the policy environment for PDPT in Australia and considered how PDPT might become a routine option. Methods: Structured interviews were conducted with 10 key informants (KIs) representing six of eight Australian jurisdictions and documents relevant to PDPT were appraised. Interview transcripts and documents were analysed together, drawing on KIs’ understanding of their jurisdiction to explore our research topics, namely the current context for PDPT, challenges, and actions needed for PDPT to become routine. Results: PDPT was allowable in three jurisdictions (Victoria, New South Wales, Northern Territory) where State governments have formally supported PDPT. In three jurisdictions (Western Australia, Australian Capital Territory, Tasmania), KIs viewed PDPT as potentially allowable under relevant prescribing regulations; however, no guidance was available. Concern about antimicrobial stewardship precluded PDPT inclusion in the South Australian strategy. For Queensland, KIs viewed PDPT as not allowable under current prescribing regulations and, although a Medicine and Poisons Act was passed in 2019, it is unclear if PDPT will be possible under new regulations. Clarifying the doctor–partner treating relationship and clinical guidance within a care standard were viewed as crucial for PDPT uptake, irrespective of regulatory contexts. Conclusion: Endorsement and guidance are essential so doctors can confidently and routinely offer PDPT in respect to professional standards and regulatory requirements.

Young people with substance misuse issues are at risk of harm from significant negative health and life events. Contemporary research notes both a historical failure to recognize the unique needs of adolescents, and the ongoing need for dedicated adolescent treatment programs and outcome measures. It is concerning that there is so little literature assessing the quality, availability, and effectiveness of adolescent-focused treatment programs, and no adolescent-specific measurement tools centered on a young person's progress in residential treatment. This article reports on the process of developing a qualitative approach to mapping progress in treatment over time. The research seeks to develop an approach that captures, at three points in time and from multiple viewpoints, the progress of young people in four residential rehabilitation services located in New South Wales and Western Australia, across several dimensions of the personal and social aspects of life. Our aim is to develop an approach that is accessible to the alcohol and other drug workforce, and that informs the development of a psychometrically robust quantitative measure of progress that is meaningful and useful both to practitioners and to the young people themselves.

Currently, there are few robustly evaluated social and emotional wellbeing (SEWB) measures available for use with Aboriginal youth in research, policy, and practice. As such, this study used a Rasch measurement approach to examine the psychometric properties of Strong Souls, a 25-item self-reported SEWB instrument, created for use with Aboriginal youth in the Northern Territory. Our sample (N = 154) included youth (15–25 years old) living on Whadjuk (metropolitan Western Australia; N = 91) and Kamilaroi countries (rural New South Wales; N = 63). Using Rasch modelling techniques, evidence for multidimensionality in the scale was observed, resulting in subsequent analyses conducted separately on two subscales: Psychological Distress and Resilience. The Resilience subscale did not meet the Rasch model assumptions, with poor person and item separation and reliability indexes suggesting the scale was not reliably differentiating between participants’ Resilience scores. The Psychological Distress subscale had mixed separation and reliability index results, with good construct validity implied but poorer ability to target the distress of participants. Our findings provide novel evidence demonstrating the functioning of Strong Souls in a contemporary sample of Aboriginal youth, suggesting further modifications of the instrument are required before it can be used with confidence as a reliable measure in this population group.

Abstract. Regional reanalyses provide a dynamically consistent recreation of past weather observations at scales useful for local-scale environmental applications. The development of convection-permitting models (CPMs) in numerical weather prediction has facilitated the creation of kilometre-scale (1–4 km) regional reanalysis and climate projections. The Bureau of Meteorology Atmospheric high-resolution Regional Reanalysis for Australia (BARRA) also aims to realize the benefits of these high-resolution models over Australian sub-regions for applications such as fire danger research by nesting them in BARRA's 12 km regional reanalysis (BARRA-R). Four midlatitude sub-regions are centred on Perth in Western Australia, Adelaide in South Australia, Sydney in New South Wales (NSW), and Tasmania. The resulting 29-year 1.5 km downscaled reanalyses (BARRA-C) are assessed for their added skill over BARRA-R and global reanalyses for near-surface parameters (temperature, wind, and precipitation) at observation locations and against independent 5 km gridded analyses. BARRA-C demonstrates better agreement with point observations for temperature and wind, particularly in topographically complex regions and coastal regions. BARRA-C also improves upon BARRA-R in terms of the intensity and timing of precipitation during the thunderstorm seasons in NSW and spatial patterns of sub-daily rain fields during storm events. BARRA-C reflects known issues of CPMs: overestimation of heavy rain rates and rain cells, as well as underestimation of light rain occurrence. As a hindcast-only system, BARRA-C largely inherits the domain-averaged bias pattern from BARRA-R but does produce different climatological extremes for temperature and precipitation. An added-value analysis of temperature and precipitation extremes shows that BARRA-C provides additional skill over BARRA-R when compared to gridded observations. The spatial patterns of BARRA-C warm temperature extremes and wet precipitation extremes are more highly correlated with observations. BARRA-C adds value in the representation of the spatial pattern of cold extremes over coastal regions but remains biased in terms of magnitude.

The study compares the effects of biosolids compost (BC) and inorganic fertiliser (IF) on the nutrient status of a clay loam at Rydalmere in Western Sydney, and an earthy sand at Somersby, 85 km north of Sydney. The soils represent the textural range used for vegetable production in the Sydney Basin of New South Wales. The soils had been under permanent sod (Rydalmere) or native vegetation (Somersby) for at 40 years prior to the experiment. There were 3 treatments during February 1994: IF, fallow (F), and BC. The same plots were treated again during September 1994 to give the combinations: IF followed by IF (IF + IF); F by BC (F + BC); BC by BC (BC + BC). The 4 replicates of each treatment were randomised in blocks. Treatments were incorporated into raised beds to a depth of 15 cm and vegetables were planted in March and again in October 1994. The total amounts of C and N initially present in the soil in the beds (0-15 cm) at Rydalmere were at least 3 times greater than at Somersby and the ratio was greater for S, P, Ca, Mg, K, and Na. By April 1995, the quantities of the 8 monitored nutrients had decreased at both sites for the IF + IF treatment; increased at Somersby, but not at Rydalmere for F + BC; and increased at both sites for the BC + BC treatment. The changes caused by the BC + BC treatment were greater in relative terms at Somersby, because of the initial difference in fertility between the 2 soils. For example, the C and N content increased by an approximate factor of 2 at Somersby, and by greater factors for the other elements. BC + BC was also the only treatment to increase other indices of fertility, such as effective cation exchange capacity (ECEC) and Bray-P, in the surface 15 cm. The treatments did not affect pH in the soil profile (0-50 cm) and increases in EC were ephemeral. The nutrient composition of the 20-30 cm zone was affected at both sites. By the end of the experiment the concentration of C, N, P, Ca, and Bray-P increased and Mg decreased. For C and N between-treatment differences were small, indicating that the observed effects were mostly caused by leaching of the products of mineralisation of soil organic matter. Treatment effects were larger for P, Ca, Mg, and Bray-P, and the final concentrations for the F + BC and BC + BC treatments exceeded those for the IF + IF treatment. At Somersby, both BC treatments increased ECEC, consistent with the importance of leaching as a process of nutrient loss at this site. Consequently, intensive vegetable cropping may pose serious environmental risks particularly on sandy sites.

One of the first challenges faced by new Australian Prime Minister, Kevin Rudd, was what to do with the former government’s controversial citizenship test. While a quick evaluation of the test shows that 93 percent of those who have sat it ‘passed’ (Hoare), most media controversy has focussed less on the validity of such a test than whether questions relating to Australian cricketing legend, Don Bradman, are appropriate (Hawley). While the citizenship test seems nothing more that a crude and populist measure imposed by the former Howard government in its ongoing nationalistic agenda, which included paying schools to raise the Australian flag (“PM Unfurls Flag”), its imposition seems a timely reminder of the challenge of understanding citizenship today. For as the demographic structures around us continue to change, so must our understandings of ‘citizenship’. More importantly, this fluid understanding of citizenship is not limited to academics, and policy-makers, but new technologies, the processes of globalisation including a globalised media, changing demographic patterns including migration, as well as environmental challenges that place pressure on limited resources is altering the citizens understanding of their own role as well as those around them. This paper aims to sketch out a proposed new research agenda that seeks to investigate this fluid and heterogenous nature of citizenship. The focus of the research has so far been Sydney and is enveloped by a broader aim of promoting an increased level of citizen engagement both within formal and informal political structures. I begin by sketching the complex nature of Sydney before presenting some initial research findings. Sydney – A Complex City The so-called ‘emerald city’ of Sydney has been described in many ways: from a ‘global’ city (Fagan, Dowling and Longdale 1) to an ‘angry’ city (Price 16). Sarah Price’s investigative article included research from the University of Western Sydney’s Centre of Culture Research, the Bureau of Crime Statistics and interviews with Tony Grabs, the director of trauma at St Vincent’s Hospital in inner city Darlinghurst. Price found that both injuries from alcohol and drug-related violence had risen dramatically over the last few years and seemed to be driven by increasing frustrations of a city that is perceived to be lacking appropriate infrastructure and rising levels of personal and household debt. Sydney’s famous harbour and postcard landmarks are surrounded by places of controversy and poverty, with residents of very backgrounds living in close proximity: often harmoniously and sometimes less so. According to recent research by Griffith University’s Urban Research Program, the city is becoming increasingly polarised, with the wealthiest enjoying high levels of access to amenities while other sections of the population experiencing increasing deprivation (Frew 7). Sydney is often segmented into different regions: the growth corridors of the western suburbs which include the ‘Aspirational class’; the affluent eastern suburb; the southern beachside suburbs surrounding Cronulla affectionately known by local residents as ‘the Shire’, and so on. This, however, hides that fact that these areas are themselves complex and heterogenous in character (Frew 7). As a result, the many clichés associated with such segments lead to an over simplification of regional characteristics. The ‘growth corridors’ of Western Sydney, for example, have, in recent times, become a focal point of political and social commentary. From the rise of the ‘Aspirational’ voter (Anderson), seen to be a key ‘powerbroker’ in federal and state politics, to growing levels of disenfranchised young people, this region is multifaceted and should not be simplified. These areas often see large-scale, private housing estates; what Brendan Gleeson describes as ‘privatopias’, situated next to rising levels of homelessness (“What’s Driving”): a powerful and concerning image that should not escape our attention. (Chamberlain and Mackenzie pay due attention to the issue in Homeless Careers.) It is also home to a growing immigrant population who often arrive as business migrants and as well as a rising refugee population traumatised by war and displacement (Collins 1). These growth corridors then, seem to simultaneously capture both the ambitions and the fears of Sydney. That is, they are seen as both areas of potential economic boom as well as social stress and potential conflict (Gleeson 89). One way to comprehend the complexity associated with such diversity and change is to reflect on the proximity of the twin suburbs of Macquarie Links and Macquarie Fields situated in Sydney’s south-western suburbs. Separated by the clichéd ‘railway tracks’, one is home to the growing Aspirational class while the other continues to be plagued by the stigma of being, what David Burchell describes as, a ‘dysfunctional dumping ground’ whose plight became national headlines during the riots in 2005. The riots were sparked after a police chase involving a stolen car led to a crash and the death of a 17 year-old and 19 year-old passengers. Residents blamed police for the deaths and the subsequent riots lasted for four nights – involving 150 teenagers clashing with New South Wales Police. The dysfunction, Burchell notes is seen in crime statistics that include 114 stolen cars, 227 burglaries, 457 cases of property damage and 279 assaults – all in 2005 alone. Interestingly, both these populations are surrounded by exclusionary boundaries: one because of the financial demands to enter the ‘Links’ estate, and the other because of the self-imposed exclusion. Such disparities not only provide challenges for policy makers generally, but also have important implications on the attitudes that citizens’ experience towards their relationship with each other as well as the civic institutions that are meant to represent them. This is particular the case if civic institutions are seen to either neglect or favour certain groups. This, in part, has given rise to what I describe here as a ‘citizenship surplus’ as well as a ‘citizenship deficit’. Research Agenda: Investigating Citizenship Surpluses and Deficits This changing city has meant that there has also been a change in the way that different groups interact with, and perceive, civic bodies. As noted, my initial research shows that this has led to the emergence of both citizenship surpluses and deficits. Though the concept of a ‘citizen deficits and surpluses’ have not emerged within the broader literature, there is a wide range of literature that discusses how some sections of the population lack of access to democratic processes. There are three broad areas of research that have emerged relevant here: citizenship and young people (see Arvanitakis; Dee); citizenship and globalisation (see Della Porta; Pusey); and citizenship and immigration (see Baldassar et al.; Gow). While a discussion of each of these research areas is beyond the scope of this paper, a regular theme is the emergence of a ‘democratic deficit’ (Chari et al. 422). Dee, for example, looks at how there exist unequal relationships between local and central governments, young people, communities and property developers in relation to space. Dee argues that this shapes social policy in a range of settings and contexts including their relationship with broader civic institutions and understandings of citizenship. Dee finds that claims for land use that involve young people rarely succeed and there is limited, if any, recourse to civic institutions. As such, we see a democratic deficit emerge because the various civic institutions involved fail in meeting their obligations to citizens. In addition, a great deal of work has emerged that investigates attempts to re-engage citizens through mechanisms to promote citizenship education and a more active citizenship which has also been accompanied by government programs with the same goals (See for example the Western Australian government’s ‘Citizenscape’ program ). For example Hahn (231) undertakes a comparative study of civic education in six countries (including Australia) and the policies and practices with respect to citizenship education and how to promote citizen activism. The results are positive, though the research was undertaken before the tumultuous events of the terrorist attacks in New York, the emergence of the ‘war on terror’ and the rise of ‘Muslim-phobia’. A gap rises, however, within the Australian literature when we consider both the fluid and heterogenous nature of citizenship. That is, how do we understand the relationship between these diverse groups living within such proximity to each other overlayed by changing migration patterns, ongoing globalised processes and changing political environments as well as their relations to civic institutions? Further, how does this influence the way individuals perceive their rights, expectations and responsibilities to the state? Given this, I believe that there is a need to understand citizenship as a fluid and heterogenous phenomenon that can be in surplus, deficit, progressive and reactionary. When discussing citizenship I am interested in how people perceive both their rights and responsibilities to civic institutions as well as to the residents around them. A second, obviously related, area of interest is ‘civic engagement’: that is, “the activities of people in the various organisations and associations that make up what scholars call ‘civil society’” (Portney and Leary 4). Before describing these categories in more detail, I would like to briefly outline the methodological processes employed thus far. Much of the research to this point is based on a combination of established literature, my informal discussions with citizen groups and my observations as ‘an activist.’ That is, over the last few years I have worked with a broad cross section of community-based organisations as well as specific individuals that have attempted to confront perceived injustices. I have undertaken this work as both an activist – with organisations such as Aid/Watch and Oxfam Australia – as well as an academic invited to share my research. This work has involved designing and implementing policy and advocacy strategies including media and public education programs. All interactions begin with a detailed discussion of the aims, resources, abilities and knowledge of the groups involved, followed by workshopping campaigning strategies. This has led to the publication of an ‘activist handbook’ titled ‘From Sitting on the Couch to Changing the World’, which is used to both draft the campaign aims as well as design a systematic strategy. (The booklet, which is currently being re-drafted, is published by Oxfam Australia and registered under a creative commons licence. For those interested, copies are available by emailing j.arvanitakis (at) uws.edu.au.) Much research is also sourced from direct feedback given by participants in reviewing the workshops and strategies The aim of tis paper then, is to sketch out the initial findings as well as an agenda for more formalised research. The initial findings have identified the heterogenous nature of citizenship that I have separated into four ‘citizenship spaces.’ The term space is used because these are not stable groupings as many quickly move between the areas identified as both the structures and personal situations change. 1. Marginalisation and Citizenship Deficit The first category is a citizenship deficit brought on by a sense of marginalisation. This is determined by a belief that it is pointless to interact with civic institutions, as the result is always the same: people’s opinions and needs will be ignored. Or in the case of residents from areas such as Macquarie Fields, the relationship with civic institutions, including police, is antagonistic and best avoided (White par. 21). This means that there is no connection between the population and the civic institutions around them – there is no loyalty or belief that efforts to be involved in political and civic processes will be rewarded. Here groups sense that they do not have access to political avenues to be heard, represented or demand change. This is leading to an experience of disconnection from political processes. The result is both a sense of disengagement and disempowerment. One example here emerged in discussions with protesters around the proposed development of the former Australian Defence Industry (ADI) site in St Marys, an outer-western suburb of Sydney. The development, which was largely approved, was for a large-scale housing estate proposed on sensitive bushlands in a locality that resident’s note is under-serviced in terms of public space. (For details of these discussions, see http://www.adisite.org/.) Residents often took the attitude that whatever the desire of the local community, the development would go ahead regardless. Those who worked at information booths during the resident protests informed me that the attitude was one best summarised by: “Why bother, we always get stuffed around any way.” This was confirmed by my own discussions with local residents – even those who joined the resident action group. 2. Privatisation and Citizenship Deficit This citizenship deficit not only applies to the marginalised, however, for there are also much wealthier populations who also appear to experience a deficit that results from a lack of access to civic institutions. This tends to leads to a privatisation of decision-making and withdrawal from the public arena as well as democratic processes. Consequently, the residents in the pockets of wealth may not be acting as citizens but more like consumers – asserting themselves in terms of Castells’s ‘collective consumption’ (par. 25). This citizenship deficit is brought on by ongoing privatisation. That is, there is a belief that civic institutions (including government bodies) are unable or at least unwilling to service the local community. As a result there is a tendency to turn to private suppliers and believe that individualisation is the best way to manage the community. The result is that citizens feel no connection to the civic institutions around them, not because there is no desire, but there are no services. This group of citizens has often been described as the ‘Aspirationals’ and are most often found in the growth corridors of Sydney. There is no reason to believe that this group is this way because of choice – but rather a failure by government authorities to service their needs. This is confirmed by research undertaken as early as 1990 which found that the residents now labelled Aspirational, were demanding access to public infrastructure services including public schools, but have been neglected by different levels of government. (This was clearly stated by NSW Labor MP for Liverpool, Paul Lynch, who argued for such services as a way to ensure a functioning community particularly for Western Sydney; NSWPD 2001.) As a result there is a reliance on private schools, neighbourhoods, transport and so on. Any ‘why bother’ attitude is thus driven by a lack of evidence that civic institutions can or are not willing to meet their needs. There is a strong sense of local community – but this localisation limited to others in the same geographical location and similar lifestyle. 3. Citizenship Surplus – Empowered Not Engaged The third space of citizenship is based on a ‘surplus’ even if there is limited or no political engagement. This group has quite a lot in common with the ‘Aspirationals’ but may come from areas that are higher serviced by civic institutions: the choice not to engage is therefore voluntary. There is a strong push for self-sufficiency – believing that their social capital, wealth and status mean that they do not require the services of civic institutions. While not antagonistic towards such institutions, there is often a belief is that the services provided by the private sector are ultimately superior to public ones. Consequently, they feel empowered through their social background but are not engaged with civic institutions or the political process. Despite this, my initial research findings show that this group has a strong connection to decision-makers – both politicians and bureaucrats. This lack of engagement changes if there is a perceived injustice to their quality of life or their values system – and hence should not be dismissed as NIMBYs (not in my backyard). They believe they have the resources to mobilise and demand change. I believe that we see this group materialise in mobilisations around proposed developments that threaten the perceived quality of life of the local environment. One example brought to my attention was the rapid response of local residents to the proposed White City development near Sydney’s eastern suburbs that was to see tennis courts and public space replaced by residential and commercial buildings (Nicholls). As one resident informed me, she had never seen any political engagement by local residents previously – an engagement that was accompanied by a belief that the development would be stopped as well as a mobilisation of some impressive resources. Such mobilisations also occur when there is a perceived injustice. Examples of this group can be found in what Hugh Mackay (13) describes as ‘doctor’s wives’ (a term that I am not wholly comfortable with). Here we see the emergence of ‘Chilout’: Children out of Detention. This was an organisation whose membership was described to me as ‘north shore professionals’, drew heavily on those who believed the forced incarceration of young refugee children was an affront to their values system. 4. Insurgent Citizenship – Empowered and Engaged The final space is the insurgent citizen: that is, the citizen who is both engaged and empowered. This is a term borrowed from South Africa and the USA (Holston 1) – and it should be seen as having two, almost diametrically opposed, sides: progressive and reactionary. This group may not have access to a great deal of financial resources, but has high social capital and both a willingness and ability to engage in political processes. Consequently, there is a sense of empowerment and engagement with civic institutions. There is also a strong push for self-sufficiency – but this is encased in a belief that civic institutions have a responsibility to provide services to the public, and that some services are naturally better provided by the public sector. Despite this, there is often an antagonistic relationship with such institutions. From the progressive perspective, we see ‘activists’ promoting social justice issues (including students, academics, unionists and so on). Organisations such as A Just Australia are strongly supported by various student organisations, unions and other social justice and activist groups. From a reactionary perspective, we see the emergence of groups that take an anti-immigration stance (such as ‘anti-immigration’ groups including Australia First that draw both activists and have an established political party). One way to understand the relationship between these groups is through the engagement/empowered typology below. While a detailed discussion of the limitations of typologies is beyond the scope of this paper, it is important to acknowledge that any typology is a simplification and generalisation of the arguments presented. Likewise, it is unlikely that any typology has the ability to cover all cases and situations. This typology can, however, be used to underscore the relational nature of citizenship. The purpose here is to highlight that there are relationships between the different citizenship spaces and individuals can move between groups and each cluster has significant internal variation. Key here is that this can frame future studies. Conclusion and Next Steps There is little doubt there is a relationship between attitudes to citizenship and the health of a democracy. In Australia, democracy is robust in some ways, but many feel disempowered, disengaged and some feel both – often believing they are remote from the workings of civic institutions. It would appear that for many, interest in the process of (formal) government is at an all-time low as reflected in declining membership of political parties (Jaensch et al. 58). Democracy is not a ‘once for ever’ achievement – it needs to be protected and promoted. To do this, we must ensure that there are avenues for representation for all. This point also highlights one of the fundamental flaws of the aforementioned citizenship test. According to the Department of Immigration and Citizenship, the test is designed to: help migrants integrate and maximise the opportunities available to them in Australia, and enable their full participation in the Australian community as citizens. (par. 4) Those designing the test have assumed that citizenship is both stable and, once achieved, automatically ensures representation. This paper directly challenges these assumptions and offers an alternative research agenda with the ultimate aim of promoting high levels of engagement and empowerment. Abstract:
Introduction One of the first challenges faced by new Australian Prime Minister, Kevin Rudd, was what to do with the former government’s controversial citizenship test. While a quick evaluation of the test shows that 93 percent of those who have sat it ‘passed’ (Hoare), most media controversy has focussed less on the validity of such a test than whether questions relating to Australian cricketing legend, Don Bradman, are appropriate (Hawley). While the citizenship test seems nothing more that a crude and populist measure imposed by the former Howard government in its ongoing nationalistic agenda, which included paying schools to raise the Australian flag (“PM Unfurls Flag”), its imposition seems a timely reminder of the challenge of understanding citizenship today. For as the demographic structures around us continue to change, so must our understandings of ‘citizenship’. More importantly, this fluid understanding of citizenship is not limited to academics, and policy-makers, but new technologies, the processes of globalisation including a globalised media, changing demographic patterns including migration, as well as environmental challenges that place pressure on limited resources is altering the citizens understanding of their own role as well as those around them. This paper aims to sketch out a proposed new research agenda that seeks to investigate this fluid and heterogenous nature of citizenship. The focus of the research has so far been Sydney and is enveloped by a broader aim of promoting an increased level of citizen engagement both within formal and informal political structures. I begin by sketching the complex nature of Sydney before presenting some initial research findings. Sydney – A Complex City The so-called ‘emerald city’ of Sydney has been described in many ways: from a ‘global’ city (Fagan, Dowling and Longdale 1) to an ‘angry’ city (Price 16). Sarah Price’s investigative article included research from the University of Western Sydney’s Centre of Culture Research, the Bureau of Crime Statistics and interviews with Tony Grabs, the director of trauma at St Vincent’s Hospital in inner city Darlinghurst. Price found that both injuries from alcohol and drug-related violence had risen dramatically over the last few years and seemed to be driven by increasing frustrations of a city that is perceived to be lacking appropriate infrastructure and rising levels of personal and household debt. Sydney’s famous harbour and postcard landmarks are surrounded by places of controversy and poverty, with residents of very backgrounds living in close proximity: often harmoniously and sometimes less so. According to recent research by Griffith University’s Urban Research Program, the city is becoming increasingly polarised, with the wealthiest enjoying high levels of access to amenities while other sections of the population experiencing increasing deprivation (Frew 7). Sydney, is often segmented into different regions: the growth corridors of the western suburbs which include the ‘Aspirational class’; the affluent eastern suburb; the southern beachside suburbs surrounding Cronulla affectionately known by local residents as ‘the Shire’, and so on. This, however, hides that fact that these areas are themselves complex and heterogenous in character (Frew 7). As a result, the many clichés associated with such segments lead to an over simplification of regional characteristics. The ‘growth corridors’ of Western Sydney, for example, have, in recent times, become a focal point of political and social commentary. From the rise of the ‘Aspirational’ voter (Anderson), seen to be a key ‘powerbroker’ in federal and state politics, to growing levels of disenfranchised young people, this region is multifaceted and should not be simplified. These areas often see large-scale, private housing estates; what Brendan Gleeson describes as ‘privatopias’, situated next to rising levels of homelessness (“What’s Driving”): a powerful and concerning image that should not escape our attention. (Chamberlain and Mackenzie pay due attention to the issue in Homeless Careers.) It is also home to a growing immigrant population who often arrive as business migrants and as well as a rising refugee population traumatised by war and displacement (Collins 1). These growth corridors then, seem to simultaneously capture both the ambitions and the fears of Sydney. That is, they are seen as both areas of potential economic boom as well as social stress and potential conflict (Gleeson 89). One way to comprehend the complexity associated with such diversity and change is to reflect on the proximity of the twin suburbs of Macquarie Links and Macquarie Fields situated in Sydney’s south-western suburbs. Separated by the clichéd ‘railway tracks’, one is home to the growing Aspirational class while the other continues to be plagued by the stigma of being, what David Burchell describes as, a ‘dysfunctional dumping ground’ whose plight became national headlines during the riots in 2005. The riots were sparked after a police chase involving a stolen car led to a crash and the death of a 17 year-old and 19 year-old passengers. Residents blamed police for the deaths and the subsequent riots lasted for four nights – involving 150 teenagers clashing with New South Wales Police. The dysfunction, Burchell notes is seen in crime statistics that include 114 stolen cars, 227 burglaries, 457 cases of property damage and 279 assaults – all in 2005 alone. Interestingly, both these populations are surrounded by exclusionary boundaries: one because of the financial demands to enter the ‘Links’ estate, and the other because of the self-imposed exclusion. Such disparities not only provide challenges for policy makers generally, but also have important implications on the attitudes that citizens’ experience towards their relationship with each other as well as the civic institutions that are meant to represent them. This is particular the case if civic institutions are seen to either neglect or favour certain groups. This, in part, has given rise to what I describe here as a ‘citizenship surplus’ as well as a ‘citizenship deficit’. Research Agenda: Investigating Citizenship Surpluses and Deficits This changing city has meant that there has also been a change in the way that different groups interact with, and perceive, civic bodies. As noted, my initial research shows that this has led to the emergence of both citizenship surpluses and deficits. Though the concept of a ‘citizen deficits and surpluses’ have not emerged within the broader literature, there is a wide range of literature that discusses how some sections of the population lack of access to democratic processes. There are three broad areas of research that have emerged relevant here: citizenship and young people (see Arvanitakis; Dee); citizenship and globalisation (see Della Porta; Pusey); and citizenship and immigration (see Baldassar et al.; Gow). While a discussion of each of these research areas is beyond the scope of this paper, a regular theme is the emergence of a ‘democratic deficit’ (Chari et al. 422). Dee, for example, looks at how there exist unequal relationships between local and central governments, young people, communities and property developers in relation to space. Dee argues that this shapes social policy in a range of settings and contexts including their relationship with broader civic institutions and understandings of citizenship. Dee finds that claims for land use that involve young people rarely succeed and there is limited, if any, recourse to civic institutions. As such, we see a democratic deficit emerge because the various civic institutions involved fail in meeting their obligations to citizens. In addition, a great deal of work has emerged that investigates attempts to re-engage citizens through mechanisms to promote citizenship education and a more active citizenship which has also been accompanied by government programs with the same goals (See for example the Western Australian government’s ‘Citizenscape’ program ). For example Hahn (231) undertakes a comparative study of civic education in six countries (including Australia) and the policies and practices with respect to citizenship education and how to promote citizen activism. The results are positive, though the research was undertaken before the tumultuous events of the terrorist attacks in New York, the emergence of the ‘war on terror’ and the rise of ‘Muslim-phobia’. A gap rises, however, within the Australian literature when we consider both the fluid and heterogenous nature of citizenship. That is, how do we understand the relationship between these diverse groups living within such proximity to each other overlayed by changing migration patterns, ongoing globalised processes and changing political environments as well as their relations to civic institutions? Further, how does this influence the way individuals perceive their rights, expectations and responsibilities to the state? Given this, I believe that there is a need to understand citizenship as a fluid and heterogenous phenomenon that can be in surplus, deficit, progressive and reactionary. When discussing citizenship I am interested in how people perceive both their rights and responsibilities to civic institutions as well as to the residents around them. A second, obviously related, area of interest is ‘civic engagement’: that is, “the activities of people in the various organisations and associations that make up what scholars call ‘civil society’” (Portney and Leary 4). Before describing these categories in more detail, I would like to briefly outline the methodological processes employed thus far. Much of the research to this point is based on a combination of established literature, my informal discussions with citizen groups and my observations as ‘an activist.’ That is, over the last few years I have worked with a broad cross section of community-based organisations as well as specific individuals that have attempted to confront perceived injustices. I have undertaken this work as both an activist – with organisations such as Aid/Watch and Oxfam Australia – as well as an academic invited to share my research. This work has involved designing and implementing policy and advocacy strategies including media and public education programs. All interactions begin with a detailed discussion of the aims, resources, abilities and knowledge of the groups involved, followed by workshopping campaigning strategies. This has led to the publication of an ‘activist handbook’ titled ‘From Sitting on the Couch to Changing the World’, which is used to both draft the campaign aims as well as design a systematic strategy. (The booklet, which is currently being re-drafted, is published by Oxfam Australia and registered under a creative commons licence. Much research is also sourced from direct feedback given by participants in reviewing the workshops and strategies The aim of tis paper then, is to sketch out the initial findings as well as an agenda for more formalised research. There is also a strong push for self-sufficiency – but this is encased in a belief that civic institutions have a responsibility to provide services to the public, and that some services are naturally better provided by the public sector. Despite this, there is often an antagonistic relationship with such institutions. From the progressive perspective, we see ‘activists’ promoting social justice issues (including students, academics, unionists and so on). Organisations such as A Just Australia are strongly supported by various student organisations, unions and other social justice and activist groups. From a reactionary perspective, we see the emergence of groups that take an anti-immigration stance (such as ‘anti-immigration’ groups including Australia First that draw both activists and have an established political party). (Information regarding ‘anti-refugee activists’ can be found at http://ausfirst.alphalink.com.au/ while the official website for the Australia First political part is at http://www.australiafirstparty.com.au/cms/.) One way to understand the relationship between these groups is through the engagement/empowered typology below. While a detailed discussion of the limitations of typologies is beyond the scope of this paper, it is important to acknowledge that any typology is a simplification and generalisation of the arguments presented. Likewise, it is unlikely that any typology has the ability to cover all cases and situations. This typology can, however, be used to underscore the relational nature of citizenship. The purpose here is to highlight that there are relationships between the different citizenship spaces and individuals can move between groups and each cluster has significant internal variation. Key here is that this can frame future studies. Conclusion and Next Steps There is little doubt there is a relationship between attitudes to citizenship and the health of a democracy. In Australia, democracy is robust in some ways, but many feel disempowered, disengaged and some feel both – often believing they are remote from the workings of civic institutions. It would appear that for many, interest in the process of (formal) government is at an all-time low as reflected in declining membership of political parties (Jaensch et al. 58). Democracy is not a ‘once for ever’ achievement – it needs to be protected and promoted. To do this, we must ensure that there are avenues for representation for all. This point also highlights one of the fundamental flaws of the aforementioned citizenship test. According to the Department of Immigration and Citizenship, the test is designed to: help migrants integrate and maximise the opportunities available to them in Australia, and enable their full participation in the Australian community as citizens. (par. 4) Those designing the test have assumed that citizenship is both stable and, once achieved, automatically ensures representation. This paper directly challenges these assumptions and offers an alternative research agenda with the ultimate aim of promoting high levels of engagement and empowerment. Abstract:
IntroductionIn 1802 George Howe (1769-1821), the recently appointed Government Printer, published Australia’s first book. The following year he established Australia’s first newspaper; an enterprise that ran counter to all the environmental factors of the day, including: 1) issues of logistics and a lack of appropriate equipment and basic materials to produce a regularly issued newspaper; 2) issues resulting from the very close supervision of production and the routine censorship by the Governor; and 3) issues associated with the colony’s primary purposes as a military outpost and as a penal settlement, creating conflicts between very different readerships. The Sydney Gazette was, critically for Howe, the only newspaper in the infant city for over two decades. Alternative voices would not enter the field of printed media until the 1820s and 1830s. This article briefly explores the birth of an Australian industry and looks at how a very modest newspaper overcame a range of serious challenges to ignite imaginations and lay a foundation for media empires.Government Printer The first book published in Australia was the New South Wales General Standing Orders and General Orders (1802), authorised by Governor Philip Gidley King for the purposes of providing a convenient, single-volume compilation of all Government Orders, issued in New South Wales, between 1791 and 1802. (As the Australian character has been described as “egalitarian, anti-authoritarian and irreverent” [D. Jones 690], it is fascinating that the nation’s first published book was a set of rules.) Prescribing law, order and regulation for the colony the index reveals the desires of those charged with the colony’s care and development, to contain various types of activities. The rules for convicts were, predictably, many. There were also multiple orders surrounding administration, animal husbandry as well as food stuffs and other stores. Some of the most striking headings in the index relate to crime. For example, in addition to headings pertaining to courts there are also headings for a broad range of offences from: “BAD Characters” to “OFFENSIVE Weapons – Again[s]t concealing” (i-xii). The young colony, still in its teenage years, was, for the short-term, very much working on survival and for the long-term developing ambitious plans for expansion and trade. It was clear though, through this volume, that there was no forgetting the colony of New South Wales was first, and foremost, a penal settlement which also served as a military outpost. Clear, too, was the fact that not all of those who were shipped out to the new colony were prepared to abandon their criminal careers which “did not necessarily stop with transportation” (Foyster 10). Containment and recidivism were matters of constant concern for the colony’s authorities. Colonial priorities could be seen in the fact that, when “Governor Arthur Phillip brought the first convicts (548 males and 188 females) to Port Jackson on 26 January 1788, he also brought a small press for printing orders, rules, and regulations” (Goff 103). The device lay dormant on arrival, a result of more immediate concerns to feed and house all those who made up the First Fleet. It would be several years before the press was pushed into sporadic service by the convict George Hughes for printing miscellaneous items including broadsides and playbills as well as for Government Orders (“Hughes, George” online). It was another convict (another man named George), convicted at the Warwick Assizes on March 1799 (Ferguson vi) then imprisoned and ultimately transported for shoplifting (Robb 15), who would transform the small hand press into an industry. Once under the hand of George Howe, who had served as a printer with several London newspapers including The Times (Sydney Gazette, “Never” 2) – the printing press was put to much more regular use. In these very humble circumstances, Australia’s great media tradition was born. Howe, as the Government Printer, transformed the press from a device dedicated to ephemera as well as various administrative matters into a crucial piece of equipment that produced the new colony’s first newspaper. Logistical Challenges Governor King, in the year following the appearance of the Standing Orders, authorised the publishing of Australia’s first newspaper, The Sydney Gazette and New South Wales Advertiser. The publication history of The Sydney Gazette, in a reflection of some of the challenges faced by the printer, is erratic. First published on a Saturday from 5 March 1803, it quickly changed to a Sunday paper from 10 April 1803. Interestingly, Sunday “was not an approved day for the publication of newspapers, and although some English publishers had been doing so since about 1789, Sunday papers were generally frowned upon” (Robb 58). Yet, as argued by Howe a Sunday print run allowed for the inclusion of “the whole of the Ship News, and other Incidental Matter, for the preceeding week” (Sydney Gazette, “To the Public” 1).The Sydney Gazette and New South Wales Advertiser Vol. 1, No. 1, 5 March 1803 (Front Page)Call Number DL F8/50, Digital ID a345001, State Library of New South WalesPublished weekly until 1825, then bi-weekly until 1827 before coming out tri-weekly until 20 October 1842 (Holden 14) there were some notable pauses in production. These included one in 1807 (Issue 214, 19 April-Issue 215, 7 June) and one in 1808-1809 (Issue 227, 30 August-Issue 228, 15 May) due to a lack of paper, with the latter pause coinciding with the Rum Rebellion and the end of William Bligh’s term as Governor of New South Wales (see: Karskens 186-88; Mundle 323-37). There was, too, a brief attempt at publishing as a daily from 1 January 1827 which lasted only until 10 February of that year when the title began to appear tri-weekly (Kirkpatrick online; Holden 14). There would be other pauses, including one of two weeks, shortly before the final issue was produced on 20 October 1842. There were many problems that beset The Sydney Gazette with paper shortages being especially challenging. Howe regularly advertised for: “any quantity” of Spanish paper (e.g.: Sydney Gazette, “Wanted to Purchase” 4) and needing to be satisfied “with a variety of size and colour” (P.M. Jones 39). In addition, the procurement of ink was so difficult in the colony, that Howe often resorted to making his own out of “charcoal, gum and shark oil” (P.M. Jones 39).The work itself was physically demanding and papers printed during this period, by hand, required a great deal of effort with approximately “250 sheets per hour … [the maximum] produced by a printer and his assistant” (Robb 8). The printing press itself was inadequate and the subject of occasional repairs (Sydney Gazette, “We Have” 2). Type was also a difficulty. As Gwenda Robb explains, traditionally six sets of an alphabet were supplied to a printer with extras for ‘a’, ‘e’, ‘r’ and ‘t’ as well as ‘s’. Without ample type Howe was required to improvise as can be seen in using a double ‘v’ to create a ‘w’ and an inverted ‘V’ to represent a capital ‘A’ (50, 106). These quirky work arounds, combined with the use of the long-form ‘s’ (‘∫’) for almost a full decade, can make The Sydney Gazette a difficult publication for modern readers to consume. Howe also “carried the financial burden” of the paper, dependent, as were London papers of the late eighteenth century, on advertising (Robb 68, 8). Howe also relied upon subscriptions for survival, with the collection of payments often difficult as seen in some subscribers being two years, or more, in arrears (e.g.: Sydney Gazette, “Sydney Gazette” 1; Ferguson viii; P.M. Jones 38). Governor Lachlan Macquarie granted Howe an annual salary, in 1811, of £60 (Byrnes 557-559) offering some relief, and stability, for the beleaguered printer.Gubernatorial Supervision Governor King wrote to Lord Hobart (then Secretary of State for War and the Colonies), on 9 May 1803: it being desirable that the settlers and inhabitants at large should be benefitted by useful information being dispersed among them, I considered that a weekly publication would greatly facilitate that design, for which purpose I gave permission to an ingenious man, who manages the Government printing press, to collect materials weekly, which, being inspected by an officer, is published in the form of a weekly newspaper, copies of which, as far as they have been published, I have the honor to enclose. (85)In the same letter, King wrote: “to the list of wants I have added a new fount of letters which may be procured for eight or ten pounds, sufficient for our purpose, if approved of” (85). King’s motivations were not purely altruistic. The population of the colony was growing in Sydney Cove and in the outlying districts, thus: “there was an increasing administrative need for information to be disseminated in a more accessible form than the printed handbills of government orders” (Robb 49). There was, however, a need for the administration to maintain control and the words “Published By Authority”, appearing on the paper’s masthead, were a constant reminder to the printer that The Sydney Gazette was “under the censorship of the Secretary to the Governor, who examined all proofs” (Ferguson viii). The high level of supervision, worked in concert with the logistical difficulties described above, ensured the newspaper was a source of great strain and stress. All for the meagre reward of “6d per copy” (Ferguson viii). This does not diminish Howe’s achievement in establishing a newspaper, an accomplishment outlined, with some pride, in an address printed on the first page of the first issue:innumerable as the Obstacles were which threatened to oppose our Undertaking, yet we are happy to affirm that they were not insurmountable, however difficult the task before us.The utility of a PAPER in the COLONY, as it must open a source of solid information, will, we hope, be universally felt and acknowledged. (Sydney Gazette, “Address” 1)Howe carefully kept his word and he “wrote nothing like a signature editorial column, nor did he venture his personal opinions, conscious always of the powers of colonial officials” (Robb 72). An approach to reportage he passed to his eldest son and long-term assistant, Robert (1795-1829), who later claimed The Sydney Gazette “reconciled in one sheet the merits of the London Gazette in upholding the Government and the London Times in defending the people” (Walker 10). The censorship imposed on The Sydney Gazette, by the Governor, was lifted in 1824 (P.M. Jones 40), when the Australian was first published without permission: Governor Thomas Brisbane did not intervene in the new enterprise. The appearance of unauthorised competition allowed Robert Howe to lobby for the removal of all censorship restrictions on The Sydney Gazette, though he was careful to cite “greater dispatch and earlier publication, not greater freedom of expression, as the expected benefit” (Walker 6). The sudden freedom was celebrated, and still appreciated many years after it was given:the Freedom of the Press has now been in existence amongst us on the verge of four years. In October 1824, we addressed a letter to the Colonial Government, fervently entreating that those shackles, under which the Press had long laboured, might be removed. Our prayer was attended to, and the Sydney Gazette, feeling itself suddenly introduced to a new state of existence, demonstrated to the Colonists the capabilities that ever must flow from the spontaneous exertions of Constitutional Liberty. (Sydney Gazette, “Freedom” 2)Early Readerships From the outset, George Howe presented a professional publication. The Sydney Gazette was formatted into three columns with the front page displaying a formal masthead featuring a scene of Sydney and the motto “Thus We Hope to Prosper”. Gwenda Robb argues the woodcut, the first produced in the colony, was carved by John W. Lewin who “had plenty of engraving skills” and had “returned to Sydney [from a voyage to Tahiti] in December 1802” (51) while Roger Butler has suggested that “circumstances point to John Austin who arrived in Sydney in 1800” as being the engraver (91). The printed text was as vital as the visual supports and every effort was made to present full accounts of colonial activities. “As well as shipping and court news, there were agricultural reports, religious homilies, literary extracts and even original poetry written by Howe himself” (Blair 450). These items, of course, sitting alongside key Government communications including General Orders and Proclamations.Howe’s language has been referred to as “florid” (Robb 52), “authoritative and yet filled with deference for all authority, pompous in a stiff, affected eighteenth century fashion” (Green 10) and so “some of Howe’s readers found the Sydney Gazette rather dull” (Blair 450). Regardless of any feelings towards authorial style, circulation – without an alternative – steadily increased with the first print run in 1802 being around 100 copies but by “the early 1820s, the newspaper’s production had grown to 300 or 400 copies” (Blair 450).In a reflection of the increasing sophistication of the Sydney-based reader, George Howe, and Robert Howe, would also publish some significant, stand-alone, texts. These included several firsts: the first natural history book printed in the colony, Birds of New South Wales with their Natural History (1813) by John W. Lewin (praised as a text “printed with an elegant and classical simplicity which makes it the highest typographical achievement of George Howe” [Wantrup 278]); the first collection of poetry published in the colony First Fruits of Australian Poetry (1819) by Barron Field; the first collection of poetry written by a Australian-born author, Wild Notes from the Lyre of a Native Minstrel (1826) by Charles Tompson; and the first children’s book A Mother’s Offering to Her Children: By a Lady, Long Resident in New South Wales (1841) by Charlotte Barton. The small concern also published mundane items such as almanacs and receipt books for the Bank of New South Wales (Robb 63, 72). All against the backdrop of printing a newspaper.New Voices The Sydney Gazette was Australia’s first newspaper and, critically for Howe, the only newspaper for over two decades. (A second paper appeared in 1810 but the Derwent Star and Van Diemen’s Land Intelligencer, which only managed twelve issues, presented no threat to The Sydney Gazette.) No genuine, local rival entered the field until 1824, when the Australian was founded by barristers William Charles Wentworth and Robert Wardell. The Monitor debuted in 1826, followed the Sydney Herald in 1831 and the Colonist in 1835 (P.M. Jones 38). It was the second title, the Australian, with a policy that asserted articles to be: “Independent, yet consistent – free, yet not licentious – equally unmoved by favours and by fear” (Walker 6), radically changed the newspaper landscape. The new paper made “a strong point of its independence from government control” triggering a period in which colonial newspapers “became enmeshed with local politics” (Blair 451). This new age of opinion reflected how fast the colony was evolving from an antipodean gaol into a complex society. Also, two papers, without censorship restrictions, without registration, stamp duties or advertisement duties meant, as pointed out by R.B. Walker, that “in point of law the Press in the remote gaol of exile was now freer than in the country of origin” (6). An outcome George Howe could not have predicted as he made the long journey, as a convict, to New South Wales. Of the early competitors, the only one that survives is the Sydney Herald (The Sydney Morning Herald from 1842), which – founded by immigrants Alfred Stephens, Frederick Stokes and William McGarvie – claims the title of Australia’s oldest continuously published newspaper (Isaacs and Kirkpatrick 4-5). That such a small population, with so many pressing issues, factions and political machinations, could support a first newspaper, then competitors, is a testament to the high regard, with which newspaper reportage was held. Another intruder would be The Government Gazette. Containing only orders and notices in the style of the London Gazette (McLeay 1), lacking any news items or private advertisements (Walker 19), it was first issued on 7 March 1832 (and continues, in an online format, today). Of course, Government orders and other notices had news value and newspaper proprietors could bid for exclusive rights to produce these notices until a new Government Printer was appointed in 1841 (Walker 20).Conclusion George Howe, an advocate of “reason and common sense” died in 1821 placing The Sydney Gazette in the hands of his son who “fostered religion” (Byrnes 557-559). Robert Howe, served as editor, experiencing firsthand the perils and stresses of publishing, until he drowned in a boating accident in Sydney Harbour, in 1829 leaving the paper to his widow Ann Howe (Blair 450-51). The newspaper would become increasingly political leading to controversy and financial instability; after more changes in ownership and in editorial responsibility, The Sydney Gazette, after almost four decades of delivering the news – as a sole voice and then as one of several alternative voices – ceased publication in 1842. During a life littered with personal tragedy, George Howe laid the foundation stone for Australia’s media empires. His efforts, in extraordinary circumstances and against all environmental indicators, serve as inspiration to newspapers editors, proprietors and readers across the country. He established the Australian press, an institution that has been described asa profession, an art, a craft, a business, a quasi-public, privately owned institution. It is full of grandeurs and faults, sublimities and pettinesses. It is courageous and timid. It is fallible. It is indispensable to the successful on-going of a free people. (Holden 15)George Howe also created an artefact of great beauty. The attributes of The Sydney Gazette are listed, in a perfunctory manner, in most discussions of the newspaper’s history. The size of the paper. The number of columns. The masthead. The changes seen across 4,503 issues. Yet, consistently overlooked, is how, as an object, the newspaper is an exquisite example of the printed word. There is a physicality to the paper that is in sharp contrast to contemporary examples of broadsides, tabloids and online publications. Concurrently fragile and robust: its translucent sheets and mottled print revealing, starkly, the problems with paper and ink; yet it survives, in several collections, over two centuries since the first issue was produced. The elegant layout, the glow of the paper, the subtle crackling sound as the pages are turned. The Sydney Gazette and New South Wales Advertiser is an astonishing example of innovation and perseverance. It provides essential insights into Australia’s colonial era. Purpose This paper aims to examine the planning policy and legal framework governing the creation and operation of urban agriculture in Sydney, Australia’s global city. All levels of urban agriculture are considered – from domestic and small community gardens to large agribusiness – as all make an important contribution to agricultural production in an urban context. Design/methodology/approach Using the Australian State of New South Wales and its capital Sydney, as a focus, the study examines the recent trend of the recognition and re-establishment of agriculture as a desired land use in cities. Three examples are selected for closer scrutiny – Horsley Park Urban Agriculture Precinct, located in the Western Sydney Parklands; City of Sydney’s City Farm, located in the inner suburb of St Peters; and the Western Sydney Aerotropolis Agribusiness Precinct, located at a new airport on the fringe of Sydney. Findings As more city-dwellers embrace urban food production and as city authorities seek to encourage and facilitate farming activities, it is clear that regulatory structures which allow it to happen should be incorporated into urban planning legislation at (in the Australian context) state government level. If cities want to encourage urban agriculture, planning legislation needs to be part of the broader legal framework for enabling it to germinate and thrive. Originality/value This paper explores the emergence of two new types of urban agriculture: first, the multi-functional, small-scale urban farming operation, situated conceptually between a community garden and a full-scale commercial agricultural enterprise, and located spatially in the midst of built-up urban form; and second, the intensive, high tech export-oriented model exemplified by the Aerotropolis Agribusiness Precinct.

Abstract Background Health systems around the world have been forced to make choices about how to prioritize care, manage infection control and maintain reserve capacity for future disease outbreaks. Primary healthcare has moved into the front line as COVID-19 testing transitions from hospitals to multiple providers, where tracking testing behaviours can be fragmented and delayed. Pooled general practice data are a valuable resource which can be used to inform population and individual care decision-making. This project aims to examine the feasibility of using near real-time electronic general practice data to promote effective care and best-practice policy. Methods The project will utilize a design thinking approach involving all collaborators (primary health networks [PHNs], general practices, consumer groups, researchers, and digital health developers, pathology professionals) to enhance the development of meaningful and translational project outcomes. The project will be based on a series of observational studies utilizing near real-time electronic general practice data from a secure and comprehensive digital health platform [POpulation Level Analysis and Reporting (POLAR) general practice data warehouse]. The study will be carried out over 1.5 years (July 2020–December 2021) using data from over 450 general practices within three Victorian PHNs and Gippsland PHN, Eastern Melbourne PHN and South Eastern Melbourne PHN, supplemented by data from consenting general practices from two PHNs in New South Wales, Central and Eastern Sydney PHN and South Western Sydney PHN. Discussion The project will be developed using a design thinking approach, leading to the building of a meaningful near real-time COVID-19 geospatial reporting framework and dashboard for decision-makers at community, state and nationwide levels, to identify and monitor emerging trends and the impact of interventions/policy decisions. This will integrate timely evidence about the impact of the COVID-19 pandemic related to its diagnosis and treatment, and its impact across clinical, population and general practice levels.

Abstract Issue Airports provide infrastructure for an industry situated between economic development and environmental detriment. In Western Sydney, a new international airport is being built in an area of sustained population growth. To proactively address the transformation created by the airport, a research hub in South Western Sydney began in 2015 to prepare a health promotion and protection agenda surrounding the airport. Description The concept of an airport that proactively addresses social determinants of health (equity) was introduced, and has been met with in-principle positive support from aviation-related stakeholders. However, follow through with demonstrated commitment has been limited. An exploration of the policy domain of the Western Sydney airport region was undertaken to identify the organisations involved and the control they exert over key determinants of health (equity) and their interrelationship. The intent was to gauge urban health professionals' ability to influence change within the airport planning and development process. Results The analysis shows that power within airport planning is spread widely vertically and horizontally. Despite having a centralised planning organisation, decision making authority is dispersed across multiple levels of public and private governance within the wider airport region. Organisations are bound by (or defer to) higher level regulatory guidelines or legislation for technical aspects of airport planning. Those with more flexible influence lack incentive to engage with broader public health activities. Lessons Lobbying for better public health outcomes has bypassed the complex planning system within airport development settings. Opportunities exist to influence planning through formal and non-formal structures for specific “wins”. However, thinking outside a health box may provide the biggest opportunity for change yet, as shown by the successful legal blocking of the Heathrow runway expansion by climate change activists. Key messages Airports as major urban infrastructure sit under complex organisational structures both in their development and management. Public health responses need to be more targeted and innovative in advocacy and engagement.

Abstract Background Aboriginal people face challenges on several fronts when it comes to the health and wellbeing of their community, compared to the rest of the Australian population. This is no different in urban areas such as Australia’s largest urban Aboriginal community located in Blacktown, NSW, where sexually transmitted infections (STIs) remain an issue of concern. Across Australia, rates of infectious syphilis, human immunodeficiency virus (HIV), and hepatitis C infection have increased by 400, 260, and 15% respectively while gonorrhoea decreased 12% in the 5-year period from 2013 to 2017. This study explores how to address the barriers that prevent young Aboriginal people under 30 years of age from accessing STI treatment through Government Sexual Health Services. Methods This qualitative study will use purposeful sampling to recruit 20 male and 20 female health consumers, 10 Aboriginal elders and 10 sexual health clinicians. This recruitment will be undertaken with the assistance of the local Government Health Services and local Aboriginal organisations. One-on-one semi-structured interviews will be undertaken by someone of the same gender in order to address cultural preferences. Data will be entered into NVivo and thematically analysed. Discussion This study will seek to add to the literature that explores why young Aboriginal people do not access sexual health services. This study seeks to understand the experience of clinicians, Aboriginal elders and Aboriginal young people to provide practical policy and clinical redesign evidence that can be used to improve the experience and cultural safety of sexual health services in urban areas of Australia. The results of the qualitative research will be disseminated with the assistance of participating local Aboriginal organisations, and the findings will be published through peer-reviewed scientific journals and conference presentations. Trial registration The study is approved by the Western Sydney Local Health District Human Research Ethics Committee (HREC/16/WMEAD/449) and the New South Wales Aboriginal Health and Medical Research Council’s Human Research Ethics Committee (1220/16).
