SamenvattingAan de hand van literatuurgegevens wordt een overzicht gegeven van de rol van biologische factoren bij het tot stand komen van de drie belangrijkste psychiatrische stoornissen in het kraambed: ‘maternity blues’, postpartumdepressie en kraambedpsychose. Voorde blues en de postpartumdepressie worden vooral stoornissen in de stofwisseling van L-tryptofaan, progesteron en de endorfines beschreven maar zonder dat een duidelijk biopsychofysiologische verklaring van de ziektebeelden kan worden gegeven. Voorde kraambedpsychose wordt vooral aan dopamine en beta-enkefaline aandacht besteed.Het blijkt dat over de bio-etiologie van deze aandoeningen vooralsnog geen consensus bestaat.

Abstract:
Background: Neutrophil activation is a mandatory stage and a sensitive marker of systemic inflammatory response. The development of this condition is associated with subsequent multiple organ failure which is the main indication for the patients stay in the intensive care unit. The search for drugs that could prevent the development of systemic inflammatory response and reduce mortality remains an urgent task of anesthesiology/resuscitation.Aim: To study the anti-inflammatory effect of dalargin, a synthetic analogue of lei-enkephalin, on human neutrophils in vitro.Materials and methods: The study was performed on blood neutrophils isolated from 5 healthy donors. A proportion of neutrophils were activated by 10 mkM formil-Met-Leu-Pro (fMLP) and 100 ng/mL lipopolysaccharide (LPS) with subsequent assessment of their activity by fluorescent antibodies to the degranulation markers CD11b and CD66b. Thereafter intact and activated neutrophils were treated with dalargin solution at concentrations of 50 and 100 mcg/mL.Results: Dalargin at 100 mcg/mL reduced the expression of CD11b molecules on the surface of intact neutrophils by 5.5-fold (p=0.008). On the contrary, LPS at a dose of 100 ng/mL increased the expression of the same molecules by 46% (p=0.08). The addition of dalargin at 50 mcg/mL to LPS-activated neutrophils reduced the expression of CD11b molecules (p=0.016). The addition of dalargin at 50 mcg/mL to fMLP-activated neutrophils significantly (p=0.008) reduced the expression of CD11b molecules and reversed their expression virtually to the level of the control. The addition of dalargin at 100 mcg/mL to neutrophils activated by fMLP at 10 mkM reduced the expression of CD11b on their surface to a level below the control by 23% (p=0.08).Conclusion: Dalargin at the studied concentrations has an anti-inflammatory effect on both intact and pre-activated bacterial components of neutrophils, thus inhibiting the process of activation and degranulation in a dose-dependent manner.

Fruktuasi leukosit dalam jumlah besar pada setiap individu terjadi pada kondisi stres, aktivitas fisiologi yang tinggi, gizi dan umur. Jumlah leukosit yang menyimpang dari keadaan normal mempunyai arti klinik penting untuk evaluasi proses penyakit. Al-Hijamah bekerja dengan cara merangsang dan mengaktifkan sistem kekebalan tubuh, pengeluaran enkefalin, pelepasan neurotransmitter, penyempitsn dan pelebaran pembuluh darah dan the gates control theory pada system saraf pusat (yang berfungsi mengurangi sensasi nyeri). Tujuan penelitian adalah Untuk mengetahui pengaruh Al-Hijamah terhadap perubahan kadar leukosit. Jenis penelitian ini bersifat eksperimental uji klinis tanpa pembanding (Pre and Post test without control group design). Penelitian ini di lakukan di Klinik Bekam Ibnu Sina Palembang. Pemeriksaan darah di lakukan di Balai Besar Laboratorium Kesehatan (BBLK) daerah Sumatera Selatan. Waktu penelitian selama 14 hari. Besar sampel sebanyak 30 orang. Data karakteristik sampel dan hasil pengukuran dinilai homogenitas mengunakan uji Shafiro Wilk. Analisis deskripsi untuk mengetahui nilai rerata dan standar deviasi. Terjadi peningkatan yang signifikan nilai rerata leukosit. Al-Hijamah berpengaruh terhadap peningkatan kadar leukosit.

Abstract:
The article presents the facts of the stress-limiting and healing effects of a synthetic analogue of leucine-enkefalin dalargin (D), the analysis of literature data of the modulating effects of D on the activity of sympathetic-adrenal system (SAS) in various extraordinary influences. D prevents: hyperactivity of SAS, associated development of dystrophic-destructive changes in the organs (heart, gastric mucosa, duodenum) and the exhaustion of catecholamine’s (CA) depot in the sympathetic endings. It is noted that similar protective effects against the development of neurogenic lesions in the organs (heart, gastric mucosa, liver and pancreas) have pharmacological agents with antiadrenergic component of actions (gangliolitiks and sympatholitiks, alpha - and beta-adrenoblokers). In the article mapped the healing effects of D in relation to the gastric mucosa destructions, skin incisional wounds, bone fractures with the ability of the sympathetic nervous system to stimulate reparative regeneration in various tissues (cornea. gastric mucosa, myocardium, liver, sympathetic reinnervation). It is concluded that the stress-limiting and healing effects of D have a noradrenergic component associated with modulating effect of D on the activity of SAS. In the conditions of extraordinary influences D suppresses SAS hyperactivity, keeping the reserves of CA in the sympathetic endings. In the implementation of reparative regeneration of tissues CA, and primarily sympathetic mediator noradrenalin, promote healing of damaged organs and tissues.

Enkefalinazy, należące do rodziny metaloproteaz zależnych od jonów cynku pełnią ważną rolę w regulacji aktywności endogennego układu opioidowego (ang. endogenous opioid system, EOS). Inhibitory enkefalinaz pozwalają na zwiększenie stężenia wybranych endogennych peptydów opioidowych, co znalazło zastosowanie w lecznictwie. Pierwszym lekiem opartym na tym mechanizmie działania był racekadotryl, wykazujący działanie przeciwbiegunkowe. W przeciwieństwie do innych leków wpływających na aktywność EOS, stosowanie racekadotrylu wiąże się z niższym prawdopodobieństwem wystąpienia efektów ubocznych, co może być kluczowym kryterium przy doborze leczenia u pacjentów z przewlekłymi chorobami jelit, którym towarzyszy biegunka. Dominujący udział EOS w modulacji informacji nocyceptywnej stwarza również perspektywy do zastosowania inhibitorów enkefalinaz w leczeniu bólu. W pracy omówiono zastosowanie inhibitorów enkefalinaz w chorobach układu pokarmowego, takich jak zespół jelita drażliwego oraz nieswoiste choroby zapalne jelit.

Abstract : Intervention Of Nonpharmacological And Pharmacological, Coronary Heart Disease, Pain Intensity. Chest pain in the cardiovascular system is one of the main complaints which often complained patients to ask for medical help. The pain is different from one individual to another based on the pain threshold and tolerance release of endorphins enkefalin which incurred hifofisis into the circulation. The chest pain which felt by the patients tend to require more urgent attention than the other symptoms. Objectives of the study was to determine the intervention effect of pharmacologi and nonpharmacologi to pain intensity decrease in coronary heart disease patients. This type of research was experimental, research design using quasi experiment. Sampling using total of sampling method. Analysis of data used was t dependent test. Results was show that there was a significant effect between intensity of pain before intervention to the intensity of pain after intervention. Conclusion, Intervention of nonpharmacologi and pharmacologi provide a positive and significant effect on pain intensity decrease.

ABSTRAK Latar Belakang : Upaya dilakukan untuk menurunkan rasa nyeri pada ibu pos sectio caesarea salah satunya menggunakan teknik Abdominal Breathing. Teknik ini di percaya mampu merangsang tubuh untuk melepaskan opoid endogen yaitu endofin dan enkefalin yang dapat mengurangi rasa nyeri dan teknik ini mudah dilakukan tanpa menggunakan alat relaksasi. Sebagai pusat rujukan di kota metro Rumah Sakit Ahmad Yani Metro Tahun 2017 persalinan SC sebanyak 181 (47%) dari 404 persalinan.Tujuan : Mengetahui ada pengaruh tekhnik abdominal breathing terhadap penurunan skala nyeri ibu post SC di RSUD Jendral Ahmad Yani Metro .Metode : Jenis penelitian kuantitatif dengan desain Purposive Sampling rancangan penelitian cross sectional. Populasi dalam penelitian ini adalah seluruh ibu sectio caesarea berjumlah 30 orang, dengan teknik sampling Purposive Sampling. Data diambil dengan lembar observasi. Analisa data univariat dan bivariat uji t (t-test).Hasil : dari hasil analisis data terhadap 30 responden Rata-rata nyeri post SC sebelum diberikan Abdominal breathing adalah 6,47 dengan standar deviasi 0,507. Rata-rata nyeri post SC setelah diberikan Abdominal breathing adalah 4,33 dengan standar deviasi 0,802. Ada pengaruh tekhnik Abdominal breathing terhadap nyeri post SC (t-test > t hitung, 17,147> 1.725, p–value < 0,05). Dengan penurunan sebesar 2,133.Kesimpulan :Ada pengaruh teknik abdominal breathing terhadap penurunan skala nyeri ibu post section caesaria di RSUD. Jendral Ahmad Yani Metro.
