Abstract:
This chapter examines food prices from 1900 to 2015. Despite growing populations, rising incomes, new technologies, globalization, and the emergence of commodities as an asset class, no trends are evident in food price levels or volatility. Still, food prices have averaged higher since 2010, harming the poor and raising fears that agricultural productivity growth has slowed. Consistently since 1900, food prices have been more volatile than the prices of manufactured goods and most other commodity groups. This relation drives terms-of-trade volatility, which slows economic growth. At the farm level, price volatility impedes investment and technology adoption, and encourages low-income livelihood strategies. Past policies to manage food prices have not worked and governments have shifted to policies aimed at mitigating the consequences of high and volatile food prices. Extending the reach of risk markets, warehouse receipt systems, index insurance, and contract farming can be useful policy components.

Abstract:
This book analyzes gendered forms of language use in several different gay male subcultures. The subcultures considered include drag queens, radical faeries, bears, circuit boys, barebackers, and leathermen. The chapters include ethnographic-based studies of language use in each of these subcultures, giving special attention to the ways in which linguistic patterns index forms of masculinity and femininity. In each case, speakers combine linguistic forms in ways that challenge normative assumptions about gender and sexuality. In an extension of prior work, Barrett discusses the intersections of race, gender, and social class in performances by African American drag queens in the 1990s. An analysis of sacred music among radical faeries considers the ways in which expressions of gender are embedded in a broader neo-pagan religious identity. The formation of bear as an identity category (for heavyset and hairy men) in the late 1980s involve the appropriation of linguistic stereotypes of rural Southern masculinity. Among regular attendees of circuit parties (similar to raves), language serves to differentiate gay and straight forms of masculinity. In the early 2000s, barebackers (gay men who eschew condoms) used language to position themselves as rational risk takers with a natural innate desire for semen. For participants in the International Mr. Leather contest, a disciplined, militaristic masculinity links expressions of patriotism with BDSM sexual practice. In all of these groups, the construction of gendered identity involves combining linguistic forms that would usually not co-occur. These unexpected combinations serve as the foundation for the emergence of unique subcultural expressions of gay male identity.

Abstract:
Mechanism, patient presentation, and physical exam are key elements to accurate and prompt diagnosis of thoracic injury. A high proportion of poly-trauma victims suffer thoracic injury which is associated with mortality of with 25-50%. Initial management is guided by ATLS principles of Airway, Breathing, and Circulation. The FAST (focused assessment with sonography for trauma) exam plays a key role in the initial evaluation of thoracic trauma. Most injuries to the thoracic cavity may be managed non-operatively or with bedside procedures with the caveat that patients with thoracic injury require close attention and monitoring in the ICU for potential deterioration. A high index of suspicion is paramount to successful patient care. When operative intervention is required it is often emergent.

Abstract:
Acute cyanide poisoning poses vital diagnostic and therapeutic challenges for emergency physicians and intensivists. While it presents certain unique clinical features, cyanide poisoning may be confused with other entities. Definitive, contemporaneous diagnosis at the bedside is impossible in most hospitals. A thorough anamnesis, rapid physical assessment, and evaluation of key laboratory indicators often point the clinician in the right direction. Smoke inhalation from structure fires represents the most frequent source of cyanide poisoning. Symptom onset may be gradual in the case of skin exposures to cyanide or ingestion of compounds that are metabolized to cyanide. However, acute cyanide poisoning presents as a syndrome of rapidly evolving and deteriorating vital signs, profound neurological and cardiovascular dysfunction, and if therapeutic interventions are not timely and adapted, death. There is little time for diagnostic testing: one must act! The sine qua non of treatment is excellent supportive care, with aggressive airway management, support of blood pressure, and correction of acidosis. Treatment of acidosis is particularly relevant in the case of cyanide. Rapid administration of specific cyanide antidotes may be lifesaving. While geographic variations exist in antidote availability, most commercially available antidotes have been demonstrated to be effective. Hydroxocobalamin and sodium thiosulphate, both safe in the setting of smoke inhalation, offer the highest therapeutic index, a critical consideration when the diagnosis is uncertain.
