Abstract:

The international concern about human rights, gender equality, domestic violence and the increase in the aging populations has brought elder abuse into the public focus. Elder abuse is a complex, multi-faceted health, social, criminal justice, international public health and human rights issue. The widely divergent and varying definitions is a controversial problem to understanding elder abuse. The elderly have been excluded from national gender-based programs on domestic violence and the abuse of women and children. In South Africa victims of elder abuse are often physically and or cognitively unable to speak for themselves which necessitates that the public be empowered and trained to become advocates for the aged. Lachs &
Pillemer ( 2004 : 1265 ) states that &ldquo
the physical and psychological impairement of elder persons could be predisposing factors for elder abuse &ldquo
. Statistical evidence on the incidence and prevalence rates of elder abuse is lacking as elderly persons are reluctant to identify care givers for fear of abandonment, retaliation and being left destitute ( Lachs &
Pillemer, 2004 : 1265 ). Despite the Bill of Rights as set out in the South African Constitution and the Older Persons Act, No. 13 of 2006 which was developed to deal with the empowerment and protection of elder persons and promote and maintain their status, rights, safety, security and well being the abuse of elder person continues to occur ( Older Persons Act, No. 13 of 2006 ). This qualitative research study is allied with the phenomenological approach in an attempt to understand elder person&rsquo
s perception, viewpoints and perspectives from their lived experiences and personal lives. Three (3) focus group discussions and eighteen (18) one-on-one interviews were conducted with elder persons living in the suburbs of Cape Town. Data from participants were audio-taped, transcribed verbatim before an inductive analysis lead to the emergence of broad themes and patterns. The main findings of the research study concluded that elderly persons were informed but not empowered about financial, emotional and verbal abuse. The abuse of the elder person in old age homes also featured prominently. The findings of the research study can be used to provide education and empower elder persons and the general public on specific aspects related to elder abuse which are : Financial, Emotional, Verbal and the abuse by staff at old age homes. These findings could be utilized by health and social welfare advocates and organisations who offer community educational and development programs to advocate against elder abuse.

Abstract:

This report presents findings from research in Ribáuè district, Nampula province, in northeast Mozambique. Our aim was to understand the living conditions of the impoverished elderly and identify strategies that may improve their situation. We conducted semi-structured interviews with elderly people, including some of the most vulnerable and some who receive income from the social assistance programme. To gain a wider context we met representatives of governmental and non-governmental organisations and referred to secondary sources.

We use the Sustainable Rural Livelihoods framework to present the capital assets (natural,social, financial, physical and human) of Ribáuè district and of its elderly inhabitants. We consider how access to assets is affected by political, economic and cultural factors in the community. We set the situation of the elderly against the development history of postindependence Mozambique, and international concern about poverty, the ageing population and their human rights.

We find that social assets in the form of human relationships, and natural assets in the form of land, are essential for elderly people’s livelihoods. Most of the elderly have very limited access to financial assets, apart from the few who receive social ssistance. This programme makes a very small but appreciated contribution to family welfare. However the programme’s reach has been limited geographically and eligibility regulations tend to obstruct access for the very poorest. In general the human capital of the rural and uneducated elderly is declining, as society increasingly values modern skills and knowledge. However the Ribáuè residents we met experience some benefit from infrastructure modernisation. Elderly people are receiving more attention, especially as carers of children.

We have identified existing strategies for improving elderly people’s livelihoods. We suggest ways to build on these, as well as possible new strategies. If development policies are both equitable and successful, increased employment and taxation should enable formal pension and social security systems to provide for the elderly in the long-term future. In the short to medium term, expansion of the social assistance programme must be prioritised, as it is vital to the most vulnerable. It is also important that people’s customary engagement in mutually supportive social networks is strengthened rather than undermined by development changes.

Since 1975, Social Security retirement benefits have been tied to the Consumer Price Index to adjust for inflation. The CPI measures price changes for a market basket of goods and services designed to replicate the average consumer's expenditures. The elderly, however, consume a market basket different from that of the typical person. In particular, the elderly tend to purchase more medical services than other consumers. Because the price of medical care increases more rapidly than other prices, the inflation rate experienced by the elderly is greater than the inflation rate for the general population, even when controlling for the upward quality bias in the medical care component of pricing data. However, given that this difference in inflation rates is less than the size of the total measurement error in the CPI, recipients of Social Security retirement benefits are actually overcompensated for increases in inflation. Over the course of a beneficiary's retirement, this overcompensation results in a total benefit that is 5.4 – 6.6% greater than what the total benefit would have been under an ideal inflation indexing scheme

Abstract:
Research available on elder abuse is limited. There continues to be a lack of uniformity in how to define and identify the problem, as well as how to intervene. One group which continues to be excluded from the process of gaining knowledge on the issue of elder abuse is the elderly themselves. As general practitioners are the primary source of health care for older people, their perspectives on elder mistreatment should be explored as well. The first objective of this study was to examine whether differences existed in the perceived severity of potentially abusive situations between three groups of older people and general practitioners. The second objective was to determine if gender differences existed in perceptions of severity of abuse. The sample consisted of 48 general practitioners, 40 independent elderly, 38 elderly caregivers and 36 elderly care-receivers. The participants' perceptions of elder abuse were assessed using a modified version of a questionnaire developed by Moon and Williams (1993). The questionnaire described 10 potential elder abuse scenarios which covered five categories of abuse: physical, psychological, sexual, financial and neglect. Participants were asked to indicate whether or not they perceived each situation to represent an example of elder abuse and, if they answered affirmatively, to rate the severity of the abusive behaviour and identify which aspect of the scenario they considered abusive. Data were analysed using split-plot analysis of variance, as well as contingency tables. The results suggested that significant differences existed in the perceptions of severity of elder abuse scenarios across groups and gender. General practitioners tended to view the scenarios as less severe examples of elder abuse than the older-aged groups. There were similarities within the elderly groups in that all groups perceived the sexual abuse scenarios as examples of more severe forms of abuse than the financial abuse scenarios. Within the elderly groups, caregivers generally perceived the scenarios as less abusive. With regard to gender differences, females generally perceived the sexual abuse scenarios as more severe than males. This was particularly so for female independent elders and female care-receivers. Both the symbolic interaction theory and social exchange theory were adopted to explain why there were differences in the perceived severity of the scenarios. It was argued that how the participants viewed the interactions between the characters in the scenarios, and whether they perceived the interactions as being rewarding or unrewarding for particular characters, would affect participants' perceptions of severity. To conclude, with such differences in views found, the development of effective assessment and intervention strategies will be difficult. However, both the public and professionals alike need to increase their understanding on the topic of elder abuse, lest the abuse continues.

Abstract:
Subarachnoid hemorrhage (SAH) is a disease with high risk of mortality and morbidity. Elderly patients have an even higher risk of poor outcome. The incidence of SAH increases with age and the elderly constitute a substantial and increasing proportion of the population. Thus, the management of elderly SAH patients is an imminent clinical challenge. Time trends in clinical management and outcome were investigated in 281 SAH patients aged ≥65 years admitted over an 18-year period. The volume of elderly patients, especially patients ≥70 years and patients in worse clinical condition increased over time. The proportion of patients with favorable outcome increased over time, without an increase in severely disabled patients. Technical results and clinical outcome of endovascular aneurysm treatment (EVT) was investigated in 62 elderly SAH patients. EVT can be performed in elderly SAH patients with high technical success, acceptable aneurysm occlusion degree, acceptable procedural complication rate, and fair outcome results. EVT was compared to neurosurgical clipping (NST) in 278 elderly SAH patients in the International Subarachnoid Aneurysm Trial. In good grade elderly SAH patients, EVT should probably be the favored treatment for internal carotid and posterior communicating artery aneurysms, while elderly patients with middle cerebral artery aneurysms appear to benefit from NST. Occurrence of secondary insults and their impact on clinical deterioration were studied in 99 patients with severe SAH. High intracranial pressure increased and high cerebral perfusion pressure decreased the risk of clinical deterioration. Elderly patients had less intracranial hypertension insults and more hypertensive, hypotensive and hypoxemic insults. Good outcome was achieved in 24% of elderly patients with severe SAH, and the proportion of severe disability was similar to that of younger patients. Patient age was not a significant predictor for vasospasm in 413 SAH patients when admission and treatment variables were adjusted for with multiple logistic regression.

Abstract:
The aim of this thesis was to explore the features of asymptomatic bacteriuria (ASB) in elderly people living in the community, and to seek diagnostic tools to discriminate between ASB and symptomatic urinary tract infection (UTI). All men and women aged 80 and over living in an urban district of Falun, Sweden, were invited to participate. Urine samples were obtained together with information on symptoms and on health indicators. The same cohort was surveyed again after 6 and 18 months. Urinary cytokines were analysed in 16 patients with UTI, in 24 subjects with ASB and in 20 negative controls. ASB occurred at baseline in 19.0% of women and 9.4% of men, and was found at least once in 36.9% of women and in 20.2% of men. ASB in women was associated with reduced mobility and urge urinary incontinence. Of those with ASB at baseline, 60% still had bacteriuria at 6 and 18 months, but among those with repeated findings of ASB with E. coli, only 40% had the same bacterial strain after 18 months. In women, the risk of developing a UTI within 24 months was higher among those with ASB at baseline than in those without. Urinary levels of cytokines (CXCL1, CXCL8 and IL-6) and leukocyte esterase were higher in patients with UTI than in patients with ASB. There is convincing evidence that ASB is harmless and should not be treated with antibiotics. However, such treatment is still often given, thereby unnecessarily contributing to the increased number of bacteria resistant to common antibiotics. This study confirms the high prevalence of ASB in elderly people living in the community. In order not to be misled by a urinary test showing bacteria, it is important to restrict urinary testing for bacteria to patients where there is a suspicion of UTI. In elderly patients with diffuse symptoms or in patients who are unable to communicate their symptoms, further diagnostic help could possibly be obtained by evaluating the urinary levels of leukocyte esterase and/or IL-6.

Abstract:
The initial aims of this investigation were to develop a reliable assay system for measuring serum 1,Z5-dihydroxyvitamin D [1,Z5(OH)ZD] concentrations and to establish a normal range in young healthy adults. Compared with young healthy individuals, the elderly population are indeed vitamin D deficient. Vitamin D deficiency was also demonstrated in a group of elderly osteomalacic patients. Slight improvements in osteomalacia was achieved by one month of treatment with vitamin D3' or alphacalcidiol with or without calcium supplements. The improvements were small and occured slowly. A significant increase in the strength of bone seems unlikely to occur in the short term. On the present evidence the combination of alphacalcidiol and calcium supplements seems no better than vitamin D3 or alphacalcidiol alone although it may require closer monitoring to avoid hypercalcaemia. In a group of elderly patients with osteoporosis and femoral neck fracture (FNF), serum osteocalcin concentrations rose significantly in the first week after fracture fixation. The change in osteocalcin correlated well (p < 0.001) with the change in serum 1,Z5(OH)ZD concentration. Histomorphometric measurements of the extent of osteoid correlated better with osteocalcin than alkaline phosphatase. Serum concentrations of 1,Z5(OH)ZD were also reduced in elderly patients with FNF irrespective of the presence of osteomalacia and therefore cannot be used as a screening test for osteomalacia in this patient group. Reduction of 1,Z5(OH)ZD was not due to a reduction in vitamin D binding protein. It is suggested that the low rate of bone turnover in these elderly patients reduces the requirement of vitamin D. Of the ten elderly patients who had underwent laryngo pharyngeal surgery all developed hypocalcaemia. This immediate post-operative decrease, due to a rapid reduction in circulating PTH concentrations, lead to an overall increase in urinary calcium excretion. Serum concentration of 1,25(OH)2D also fell postoperatively thus potentiating the hypocalcaemic state in these patients. Thus, it is important to give parenteral feeding supplemented with calcium and vitamin D, preferably alphacalcidiol. If delayed then profound as well as prolonged hypocalcaemia can occur. The human osteosarcoma cell 20S metabolised 25(OH)D3 in a substrate concentration and time dependent manner to produce products which were secreted into the extracellular medium. These products eluted from HPLC with a retention time coincident with 24,25(OH)2D3 and exhibited an UV absorption spectrum characteristic of a vitamin D sterol. Mass spectroscopy analysis indicated at least two products were synthesised by the cells. One was identical to 24,25(OH)2D3; the other appeared to be an unsaturated trihydroxylated derivative of vitamin D3.

This thesis seeks to answer how urban design in an established town can be adapted to accommodate an aging population. It uses the town of Brookline, Massachusetts, as an example of a community making active efforts to improve its design and services to meet the needs of its elderly citizens. Among the many challenges seniors face in Brookline are barriers to mobility, need for activity and company, threats to physical safety, and limited range of travel. Through careful planning and design, local governments can make physical improvements to the public environment to allow seniors easy access to all parts of the town or city, and these improvements benefit people of all ages as well. However, institutions like the Senior Center in Brookline are essential for providing a high quality of life, by hosting recreational and educational activities, organizing services and outreach to isolated seniors, lobbying for appropriate representation in local policymaking, and increasing awareness of elderly issues. A combination of infrastructure improvements, services, and long-range planning can overcome the obstacles of cost, ignorance, and poor design to make the public environment accessible to all ages and abilities.

Abstract:
Efforts to predict successful grief resolution in adults have suggested that older adults may experience grief differently than younger adults. In addition, age, coping style, perceived control, and a social support system have also been identified as possible mediating factors in grief resolution. This study explored the effect of age, coping style, and perceived control on grief resolution in 48 independent living residents (aged 65-86) of a Northern California community who had experienced the loss of a spouse. All were members of a widowed person's support organization. It was hypothesized that successful grief resolution would be positively related to perceived external control and to an avoidance coping style. Grief resolution was measured by the Grief Resolution Index (Remondet & Hansson, 1987) and the Beck Depression Inventory (Beck, 1978). The degree of grief resolution was analyzed using 2 x 2 x 2 (Age x Locus of Control x Coping Style) ANOVA on each measure of grief resolution. The hypotheses were not supported.

Abstract:
Asthma is a chronic disease of the airways that can affect all ages. It is usually undertreated in the elderly population, resulting in complications and increased severity for the patient. Different phenotypes may imply different treatments and therefore it becomes important to correctly determine which type of asthma the patient is suffering from. Biomarkers of asthma have been clinically studied in order to help discriminate among the different phenotypes of the disease, cytokines being among the most promising. Our main goal was to measure six serum cytokines, including IL-1β, IL-6, IL-8, IL-10, IL-12p70 and TNFα, in an elderly population with asthma. The population of this study included 16 elderly patients with allergic asthma, 20 elderly patients with non-allergic asthma, 9 elderly control individuals, as well as 11 young adult control individuals. The serum of the experimental subjects was quantified for the six aforementioned cytokines by flow cytometry, using the Cytometric Bead Array (CBA) Human Inflammatory Cytokines Kit (BD Biosciences). The results showed that the measured levels of TNF-α, IL-10 and IL-8 were increased in the non-allergic asthmatics group, although with no statistical significance. IL-6 was significantly increased in the elderly non-allergic asthmatic group, when compared to both control groups and when compared to the young adult controls alone, but no significant differences were found when comparing to the elderly control group separately. IL-1β median levels were shown to be elevated in the young adults control group, but also without a significant statistical difference. IL-12p70 was not detected in any subject. In conclusion, we could not define a pattern of changes in the measured cytokine levels related to phenotype differences or aging, although it is possible to notice a trend for increased levels of severity cytokines (TNF-α, IL-6 and IL-8) in non-allergic asthmatic elderly individuals, suggesting that this phenotype of asthma is indeed more severe than its counterpart. So far, this is the only study that measured the aforementioned serum inflammatory cytokines in an elderly population with asthma.
A asma é uma doença crónica das vias aéreas que afeta todas as idades e é incorretamente tratada ou sub-diagnosticada na população idosa, resultando em complicações para o doente e numa diminuição da qualidade de vida. Diferentes fenótipos podem requerer diferentes tratamentos e, portanto, é necessário diagnosticar corretamente qual o tipo de asma de que o doente sofre. Apesar de grande parte do tratamento e pesquisa se concentrarem na asma alérgica, existem várias outras formas da doença que requerem atenção, tal como a asma neutrofílica ou asma não-alérgica, que afeta normalmente os doentes mais velhos. A doença pulmonar obstrutiva crónica (DPOC), também muito comum em idosos, pode prejudicar ainda mais o diagnóstico da asma, devido às semelhanças entre as duas doenças. Biomarcadores da asma estão a ser estudados clinicamente com vista a aumentar o poder discriminativo entre os diferentes fenótipos da doença, estando as citocinas dentro dos mais promissores. Assim sendo, o nosso principal objetivo foi medir seis citocinas, nomeadamente a IL-1β, IL-6, IL-8, IL-10, IL-12p70 e TNF-α, no sangue periférico de uma população de idosos asmáticos. Pretendíamos também usar a citometria de fluxo para quantificar as citocinas, de modo a podermos comparar facilmente os resultados e estabelecer padrões citocínicos. Também tínhamos como objectivo avaliar alterações nos padrões citocínicos relacionadas com a idade, usando um grupo controlo de adultos jovens. Por fim, pretendíamos investigar possíveis alterações nos padrões citocínicos relacionados com cada fenótipo da asma (alérgica ou não-alérgica). A população deste estudo foi constituída por 16 idosos com asma alérgica, 20 idosos com asma não-alérgica, 9 idosos não asmáticos para controlo, assim como 11 adultos jovens não asmáticos para controlo, recrutados através de uma base de dados. Os indivíduos foram inseridos nos respectivos grupos pelos resultados de testes cutâneos, medição da IgE total e específica, e pelas respostas dadas a um questionário. As citocinas previamente referidas foram quantificadas no soro dos indivíduos por citometria de fluxo, usando o Cytometric Bead Array (CBA) Human Inflammatory Cytokines Kit (BD Biosciences). Os resultados obtidos mostraram que o TNF-α, a IL-10 e a IL-8 se encontravam elevadas no grupo dos asmáticos não alérgicos, apesar da diferença não ser significativa. A IL-6 encontrava-se significativamente elevada no grupo dos idosos com asma não-alérgica, quando comparada com ambos os grupos de controlo e quando comparada com o grupo dos adultos jovens individualmente, embora não tenham sido encontradas diferenças significativas quando comparada com o grupo de controlo dos idosos, separadamente. Os níveis de IL-1β encontravam-se elevados no grupo dos adultos jovens, porém sem significado estatístico. A IL8 encontrava-se geralmente mais elevada nos idosos. A IL12p70 não foi detectada em nenhum indivíduo. Concluindo, medimos as citocinas previamente referidas no sangue periférico de uma população idosa, cumprindo assim o nosso objectivo principal. Porém, não foi possível definir um padrão de altercações a nível das citocinas relacionado com diferenças entre os fenótipos ou relacionadas com o envelhecimento, embora tenha sido possível notar uma tendência para um aumento das citocinas ligadas à severidade (TNF-α, IL-6 e IL-8) no grupo dos idosos com asma não-alérgica, o que nos leva a concluir que este fenótipo da asma é, de facto, mais severo, tal como sugere a bibliografia. O principal ponto forte é o facto deste ser o primeiro estudo a medir estas seis citocinas no soro de idosos com asma. Uma limitação deste estudo centra-se no facto dos voluntários terem sido classificados como asmáticos através das respostas a um questionário, não tendo, portanto, nenhuma confirmação clínica da existência da patologia. Isto pode ter levado à inclusão de voluntários nos grupos errados. Para obtenção de melhores resultados os voluntários deveriam ser recrutados após o diagnóstico efetuado por um especialista e submetidos a testes complementares, como por exemplo a espirometria, de modo a garantir a correta inserção dos indivíduos nos grupos. Outros dos marcadores referenciados neste trabalho, como a proteína catiónica dos eosinófilos (ECP) e a YKL-40, também aparentam ter algum potencial para a monitorização e diagnóstico da asma, devendo ser investigados.

With the emergence of natural user interfaces (NUl) technologies; which introduces new ways of interacting with computers, the way humans think about using computers has changed. Computer games have now a new dimension; where the player can perform a wide range of movements, ranging from simple hand gestures to moving the whole body or simply changing body position. This project aims to evaluate new ways of motivating older adults to move and perform exercises by encouraging them to play computer games that are fun. For gaming to work with older adults. the perceived affordancet or opportunity for action, of the game needs to give the player a meaningful indication about what actions are possible. and what they should do next. This project focuses on how to design games that adapts to individual player's affordancest where the level of difficulty of each level in the game will be able to accommodate an individual user's physical abilities. The research also investigates usability issues associated with designing digital games for older adults. An iterative testing methodology was applied to create several game prototypes to stipulate a set of design heuristics that will serve as a shared language between designers and developers. This project also Investigates how the player performance can be analysed when playing Kinect games, in such a way to enable older people to win against younger. A new novel algorithm called Best Hit Algorithm is implemented. The algorithm is tested with both older and younger adults to prove its validity; it provides a fair challenge mechanism where each player performance is measured according to hislher best performance. This is contrary to the current implementations of Kinect games where the winner usually is the player who has higher physical capabilities (e.g. faster response speed).

Abstract:
Medication therapy is the most prevalent and critical intervention of health delivery and the source of most errors in healthcare. Medication errors and associated adverse drug events (ADE) have serious health and economic ramifications, and in elderly patients ADE are the leading cause of morbidity and mortality. Medication reconciliation is the process of evaluating current medication treatment to manage the risk and optimize the outcomes of medication treatment by detecting, solving, and preventing ADEs. This education project answered the question whether education provided to long term care staff would improve knowledge of medication reconciliation and be retained over time. The education program was developed through results of a literature search to identify evidence-based standards for medication reconciliation. The guiding theory for program was Kurt Lewin's theory of planned change. The test was developed on the medication reconciliation content and arrangements made for each of the 30 participants who were RNs, LPNs, and CMAs to take the test before and after the education program and again at 30 and 45 days. Results showed statistically significant improvement (p < 0.05) with knowledge of medication reconciliation retained at 30- and 45-days post intervention. Positive social change is possible as nurses and CMAs in the long-term care facility use the knowledge of medication reconciliation to improve patient medication safety for the long-term care residences in the facility. Through appropriate reconciliation, medication errors and ADEs can be reduced or prevented and patient outcomes improved.

How do social media and internet usage look like in the eyes of ageing people, specifically those at the age of 80 years or above that live independently? What technologies are most interesting to them and why? What do they struggle with that could lead them to avoid specific technologies or to prefer one technology more than others? The reason of the study was to get an insight of elderlies attitudes, regarding technologies in general and social media specifically, to know what hinders those seniors to be excluded of the recent technologies and to not be plugged into cyberspace. The specific questions that the research wanted to answer were “What are the actual barriers for internet use among the elderly?” and “Also, what could be done to address these barriers?”. The study was taking place in one retirement house in Skellefteå with 4 women at the desired age group and was carried out using semi-structured interviews mixed with focus groups as the method for data gathering. This research will be assisting an EU funded project aimed to start in the summer of 2017 named PLACE-EE and is planned to be a platform for ageing community engagement. The results showed that more technical and human support were required in a local environment. The elderly simply did not have the drive to personally engage themselves actively in technology to use the internet or social media among other things. One of the suggestions made to solve this would be to integrate the technology into their daily routines where they are socially active, like the community centers that they valued quite a lot.

The focus of this study is to investigate the reasons of less use of mobile phone in elderly life and what other functionalities elderly want in coming mobile phones. The results of this study would be supportive in understanding the problems of diabetes type 2 patients in using mobile phone. This study would also be helpful for the manufacturers in developing the mobile phones according to the needs of elder persons. The analysis of this research work depends on three study stages. It starts from the literature review, in which we learned the effects of diabetes on elder persons. It also includes the usability features of mobile phone which necessary for implementing the future functionalities with elderly perspective. On the basis of literature review we designed the questionnaire and prepare the interviews. Interviews were conducted with two health care professionals and questionnaires with the citizens of county of Blekinge. We believe that our analysis will be helpful for both elderly and mobile phone manufacturing companies and they will also be supportive in increasing the quality of life of elderly.

A descriptive study was done to determine the difference in life satisfaction of elderly females who were overweight as compared to elderly females of ideal weight. The sample consisted of 30 females, aged 60 to 80, who were living in the community. No differences in life satisfaction scores were found between the overweight and ideal weight elderly females. The elderly females who were identified as being overweight had a mean life satisfaction score of 14.8 with a standard deviation of 2.45. Elderly females of ideal weight had a mean life satisfaction score of 15.9 with a standard deviation of 2.12. Major differences in summated results of selected demographic variables were not found for the elderly females who were overweight as compared to the elderly females of ideal weight. Findings from the study and implications for nursing are discussed. Recommendations for further research are included.
