This book has its origin in the research I did for my PhD studies in architecture, at that time it was not easy for me to look for the publication of the work, I was more interested in finishing my studies and continue with my life. Time and chance led me to publish part of that thesis in my first book, however that publication did not cover all the work done so far, so that’s what’s decided by the editors, leaving out the part of the analysis of the machines and taking only the explanations of hydraulic engineering, mestizo processes and treaties and their analysis, the book published in 2010, two years after obtaining the PhD, represented my debut feature Hydraulic Engineering in Mexico sixteenth century and despite the happiness that caused me that first publication had the feeling that there was something that I still need to do. Besides, I never found a clear explanation as to why only half of the thesis had been published, perhaps the number of pages, or the choice of a topic evaluated as being of greater interest, that question had been going around in my head for some time. Why had hydraulic machines been left out?

Tongan (Polynesian) shows ergative-absolutive (ERG-ABS) patterns in morphology as well as syntax, but the ERG-ABS pattern is not consistent throughout the language. Noun morphology shows a split between clitic pronouns and other types of nouns. In syntax, three phenomena show an ERG-ABS contrast: (a) relativization using the gap strategy is limited to ABS and ERG-relatives require resumption; (b) coordinate reduction applies only if the gap and the antecedent are in the same case, be it ABS or ERG; and (c) only ABS, but not ERG, can serve as the antecedent of the null SE anaphor. No single factor can account for all three of these phenomena and at least two of the three patterns are shown to be better viewed as PF-phenomena. The data suggest that syntactic ergativity should be understood as a construction-specific phenomenon rather than a language-specific property.

Joseph Toynbee, an English physician, believed that the only way to develop a rational strategy for the treatment of ear diseases was to dissect the temporal bones of patients with known inner ear diseases. In his book Diseases of the Ear (1860), Toynbee noted that not one dissection of a diseased ear had been done before 1800, although thousands of dissections had been done on most other organs of the body. The techniques for examination of the temporal bones developed by Joseph Toynbee had multiple technical problems. German otolaryngologist Karl Wittmaack solved these problems and revolutionized the study of temporal bone specimens. An otologist in London, Charles Skinner Hallpike, became interested in studying temporal bone pathology while visiting Wittmaack’s clinic in Germany. Hallpike went on to develop his own temporal bone laboratory in which he worked with neurosurgeon Hugh Cairns to describe the pathology of Ménière’s disease.

Like Joseph Toynbee, Harold Schuknecht believed that the only way to develop rational treatments for inner ear diseases was to understand the pathology of these diseases. Schuknecht used his human temporal bone studies to “refute the conceptual validity of several popular otologic therapies.” Probably the most controversial of these was the treatment of Ménière’s disease with endolymphatic shunt surgery. Schuknecht argued the concept that the endolymph sac can be drained to relieve endolymphatic hydrops is a pedantic notion at best. He studied numerous human temporal bone specimens of patients who had had shunts placed in the endolymphatic sac, and in every case the shunt devices were ensheathed in fibrous tissue. He debunked many other controversial treatments in otolaryngology. He argued that most cases of sudden deafness and acute vertigo (vestibular neuritis) were due to viral infections of the inner ear and eighth nerve, and that vascular treatments were inappropriate.

After his groundbreaking work in the mid-1860s, Josef Breuer continued to perform experiments on the inner ear balance receptors in animals. He studied the macules of fish, reptiles, and birds and noted that all these creatures had three macules arranged in the planes of the semicircular canals, perpendicular to one another. By contrast, mammals had only two macules located in the utricle (horizontal plane) and saccule (vertical plane), again perpendicular to each other. He developed the concept of “slip” to describe the movement of the otoconial membrane over the underlying sensory epithelium that occurred with linear displacement or gravity. He developed a mathematical model to hypothesize that in humans there was only one combination of responses from the two macules on each side for a single head position in space.

Lack of function or malfunction of the vocal cords are not as common manifestations of neuromuscular disorders as dysarthria. It is typically seen in central diseases such as Parkinson disease. Certain muscle and nerve disorders affect the vocal cords, but in these cases, other features of these diseases make the diagnosis easy. Myasthenia gravis (MG) may present with intermittent hoarseness only early in the course of the disease. Consultation with an ear, nose, throat (ENT) specialist is recommended to characterize the type of cord pathology. Hysterical hoarseness and weakness are not unusual presentations to neuromuscular clinics. Unilateral vocal cord palsy is usually due to recurrent laryngeal nerve pathology.

Psychosis is one of the cardinal concepts of psychopathology (Jaspers), with an important descriptive use and frequent but unclear nosological connotations. Despite its central role in clinical psychiatry, it is only inadequately and vaguely addressed and articulated in the contemporary psychodiagnostic manuals. Typically, the descriptive use of this concept—as a ”break with reality”—is always infused with, and framed by pathogenetic hypotheses (e.g. ”weak ego-function” or ”brain disorder”). Because we are not in possession of any extraclinical index of psychosis, all definitions of”psychosis” and ”psychotic” remain on a vague, descriptive level and are often tautological. In particular, the attempts to define psychosis through the presence of delusions (or other ”psychotic symptoms”) only recapitulate the puzzle. This essay tries to identify a phenomenological commonality to such descriptions, examining the philosophical and clinical aspects of the concepts of”reality”, ”rationality” (theoretical and practical), ”reality testing”, ”intersubjectivity”, delusion, hallucination etc. It is concluded that ”psychosis” is a normative, context-sensitive, non-operationalizable concept, indicating a condition of ”radical irrationality”. This concept, although invaluable in clinical and legal work, is probably of only limited nosological (etiological) value.

The EU Capital Requirement Regulation (CRR) [8] and of EBA Regulatory Technical Standard for prudent valuation [15], published on Jan. 1st, 2014 and Jan. 28th, 2016, respectively, constitute the EU Prudent Valuation Framework. The CRR, art. 34, requires to Institutions a prudent valuation of positions measured at fair value and the deduction of the resulting Additional Valuation Adjustments (AVAs) from the Common Equity Tier One (CET1) capital. The art. 105 disciplines the AVAs intended to achieve an appropriate level of certainty in prudent value. The EBA RTS [15] allow two approaches to prudent valuation. The simplified approach, applicable by small financial institutions (with total absolute fair-valued assets and liabilities below EUR 15 billions), prescribes a very simple total AVA equal to 0.1% of the total fair value. The core approach, compulsory for institutions above the EUR 15 billion threshold, prescribes the calculation of 9 AVAs, referring to different sources of valuation uncertainty, as the excess of valuation adjustments required to achieve the prudent value with 90% level of confidence. Five out of nine AVAs include a 50% weight to take diversification into account and avoid double counting effects. Those positions for which a change in their fair value affects only partially the CET1 may be partially excluded from the AVAs calculation.

According to common sense, some but not all collections of objects are unified into larger wholes. For instance, a certain collection of pieces composes a person’s desk, but there is no object composed of that person’s left ear and the Eiffel Tower. Mereological idealism is the view that our conceptualizing activity is responsible for this unification: a collection of objects composes a whole if and only if those objects are co-apprehended by some mind under some concept. This chapter develops this view in detail and defends it against objections. Additionally, the chapter argues that mereological idealism is able to solve certain well-known problems faced by other theories of composition: the vagueness problem, the causal exclusion problem, and the problem of alternative conceptual schemes.

Berkeley’s contemporary Peter Browne objected that, on Berkeley’s philosophy of language, “Words may be Significant, tho’ they signify Nothing” with the result that “all Faith would terminate in the Ear.” The key problem for Berkeley is that even given an adequate theory of reference it is unclear, in the absence of ideas, how words can be put together into truth-evaluable propositions which are possible objects of belief. This chapter argues that Berkeley endorses a dispositional theory of assent (belief) which takes a believer to be a follower of certain rules. This theory of assent is combined with a radical holism about truth. Although true belief does involve a sort of fit with reality, this fit must be understood as involving feelings and actions and not only ideas.

Women in Greek epic are treated as objects, as commodities to be exchanged in marriage or as the spoils of warfare. However, women also use objects to negotiate their own agency, subverting the male viewpoint by using the very form they themselves are thought by men to embody. Female objects in Homer can be symbolically significant and powerfully characterizing. They can be tools of recognition and identification. They can pause narrative and be used agonistically. They can send messages and be vessels for memory. This book brings together Gender Theory and the burgeoning field of New Materialisms, combining an approach predicated on the idea of the woman as object with one which questions the very distinction between subject and object. This productive tension leads us to decentre the male subject—and to put centre stage not only the woman as object but also the agency of women and objects. Homeric women are shown to be not only objectified but also well-versed users of objects. This is something that Homer portrays clearly, that Odysseus understands—but that has often escaped many other men, from Odysseus’ alter ego Aethon in Odyssey 19 to modern experts on Homeric epic.

Abstract:
In Pécs due to the continuous development of the town throughout many centuries, there are only few sites where the remains of the Roman town can be excavated and even fewer remains come to light undisturbed. Some archaeological monuments have already been disappeared for good, others are situated under streets and houses unavailable for any archaeological research. In many cases it is not easy to follow what happened to the finds and remains. The mosaic floor that was found in 1841 during the building of a house at 6 Káptalan Street and a Roman burial chamber, observed during canalization in 1927 belong to this category. Some researchers still consider these as they were parts of the same early Christian building. Some authors made mistakes or misinterpreted the sources, because they relied on old, short and inaccurate descriptions of the finds. Either they were not aware of the sources or did not read them carefully. Therefore there are many different opinions in the publications often contradicting each other. Also, the lack of drawings and documentation added to the difficulties of later research, just like the inaccurate observations in some previous publications. In 2011 when we made the illustrations to Pécs Története (History of Pécs town), Volume I, it became evident, that the reconstruction of the mosaics, published in 1984, and the interpretation of the archaeological remains found in 1927 were inaccurate. These Roman remains did not come to light in the same time yet they are connected to each other in so many ways. We have started our research with the examination of both the written sources and the actual archaeological material. Later we have researched the different opinions of earlier publications and only after that were we in the position - with the help of few survived drawings and photos - to re-evaluate the remains and to make an attemp to reconstruct what they might have looked like in Roman times. Our recent research on the finds from 1841 and 1927 started in 2011, when we made a presentation about them for the archaeology PhD students of the University of Pécs for the first time. In accordance with the topic of the semester first we gave an account of our ongoing research regarding the actual size of the mosaics and the location and identification of Burial Chamber X that came to light in 1927. We have also presented the new, amended reconstruction of the mosaics and some of the corrections we have made. We have published our finished research about the building and the mosaics on the 10th of December 2013 at the Morsa Archaelogica Conference in Pécs. We hope that this article helps to answer the questions that emerged or were left unanswered so far about the mosaics and the archaeological remains found in 1927.

Joint aspiration and microscopic examination of the aspirated synovial fluid remains the gold standard for the diagnosis of calcium pyrophosphate crystal deposition (CPPD). If synovial fluid aspiration is not feasible, plain radiography and/or ultrasound scanning may be used to detect chondrocalcinosis (CC) which predominantly occurs due to calcium pyrophosphate (CPP) crystals, and this can be used as a diagnostic surrogate for CPPD as suggested by the EULAR Task Force. Acute CPP crystal arthritis often associates with a brisk acute phase response (elevated C-reactive protein (CRP) and/or erythrocyte sedimentation rate (ESR), plasma viscosity) and neutrophilia. A mildly raised CRP and/or ESR may be present in chronic CPP crystal inflammatory arthritis. On the contrary, asymptomatic CC, or CPPD with osteoarthritis does not cause raised acute phase reactants. As CPPD most commonly occurs due to increasing age and osteoarthritis, investigations to screen for underlying metabolic abnormalities should be carried out in those with early-onset CPPD (under 55 years), or in those with florid polyarticular CC. As hyperparathyroidism gets more common with ageing its presence should be specifically sought in all age groups. Tests for other predisposing metabolic conditions should only be carried out in the presence of specific clinical features. Genotyping for mutations, especially in the ANKH gene, may be warranted in those with a family history of premature CPPD and no evidence of inherited metabolic predisposition, but such testing is unavailable to most clinicians.

Chapter 6 starts with the accidental silencing of the statue in the early third century CE, and jumps ahead to its rediscovery in Europe. In the mid-eighteenth century, travelers reported seeing a huge statue with poems etched on its surface. Later, Napoleon’s surveyors brought back drawings scribbled down in their free time. The nineteenth century saw a craze for all things Egyptian: Hegel mentioned the colossus; Keats and Wordsworth turned Memnon into a Romantic hero. Memnon functioned as an alter ego for the poet himself, broken in body yet still striving to sing in the harsh environment of the real world. Just as he had in the imperial period, Memnon also represented something strange and inexplicable. The striking voice of Shelley’s “Ozymandias” is also heard only in the context of fragmentation and decay. The status of these statues as fragments, as colossal wrecks, allows for the magic of the voice.

The seventh chapter describes the annus mirabilis of the Marlborough-Godolphin partnership. With Italy and Spain the priorities, Marlborough is given discretionary instructions to march to save Turin. Instead he uses these to induce the Dutch to let him give battle in Flanders. His second great victory at Ramillies wins the Spanish Netherlands back from France and Marlborough is offered the governorship. With success in Spain and Italy, complete success seems in sight. But the Habsburgs lose their opportunity in Spain, the Dutch deny Marlborough the governorship, and the Whigs demand cabinet office for Marlborough’s son-in-law Sunderland, whom the queen dislikes. Godolphin says that if Marlborough will not return and support him he must resign. Believing that they need only one more year to achieve their objectives, Marlborough agrees, but also helps to rebalance the parties. The Whigs are discontented again and Harley now has the queen’s ear.

The first phase of Sartre’s philosophical publications displays an apparent ambivalence toward Husserl’s transcendental turn. Sartre accepts both major aspects of that turn, the phenomenological reduction and the use of transcendental argumentation. Yet his rejection of the transcendental ego that Husserl derives from this transcendental turn overlooks an obvious transcendental argument in favor of it. His books on emotion and imagination, moreover, make only very brief comments about the transcendental constitution of the world of experience. In each case, these appear at the end of the book and can seem to contradict the book’s central analysis. Fabry disease is a rare X-linked lysosomal storage disorder in which deficiency of alpha-galactosidase A leads to accumulation of substrate, mostly globotriaosylceramide (Gb3), which causes a progressive, multiorgan disease affecting predominantly the kidneys, skin, heart, and nervous system. Painful peripheral (‘acral’) neuropathy is characteristic. Proteinuria and estimated glomerular filtration rate (eGFR) are strongly associated with risk of progression, but this may be reduced by treatment with angiotensin-converting enzyme inhibitors as well as by enzyme replacement therapy (ERT). ERT was approved in 2001; it improves pain and other neuropathic symptoms, and well-being, and has been proven to clear deposits of Gb3 from tissues, at variable speeds. There is limited randomized controlled trial data but protective effects have been proven for renal outcomes, death, and better outcomes in some other organ systems. Renal function may be protected if ERT is commenced before there is heavy proteinuria or substantial loss of GFR. It is recommended to start ERT as soon as the diagnosis is made in those with very low or absent enzyme. For those with intermediate levels it is recommended to commence treatment only when signs or symptoms appear. Proteinuria and eGFR give most information from a renal point of view, but renal biopsy is also useful for confirming the renal diagnosis and staging the disease as well as monitoring progress in selected cases. Management should include regular screening for complications including myocardial and neurological assessments. It is likely that registries will show progressive rises in median survival with this condition.

This final chapter applies the analysis of misogyny to the 2016 presidential election, in which Hillary Clinton was defeated by Donald Trump, despite the latter being vastly underqualified and temperamentally and morally unsuited to the position. There was also a great deal of misogyny directed toward Clinton not only by Trump and others on the right but also from left-wing sources. It is argued that much of this misogyny and even the outcome were to some extent predictable, on the basis of evidence of misogynistic biases against women who compete for male-dominated leadership positions. Research in social psychology shows that, when a woman cannot be judged less competent than her male counterpart in such contexts, many people will hold that, although they are equally competent, she is less likable than he is. Women are just as likely as men to reject high-achieving women in this manner, due to ego-protective mechanisms.

Typically epidemic typhus occurred during instances in which humans were forced to live in crowded, cold and unhygienic conditions (eg aboard ships, in jails, and during military operations). Until recently man was considered to be the only reservoir for R.prowazekii, but in 1975 a new sylvatic cycle involving the flying squirrel and its ectoparasites was discovered in the eastern United States.Murine typhys occurs throughout the world. Its epidemiology is primarily linked to the distribution of rats and the rat flea, Xenopsylla cheopis. However, recently both a new reservoir (opossums in southern California) and a new potential vector (the cat flea) have been discovered.Control or avoidance of the vectors are the cornerstones of strategies to prevent morbidity and mortality.

This chapter explores how courts in the United Kingdom have used and interpreted the Convention on the Rights of Persons with Disabilities (CRPD) by analysing the seventy-five cases mentioning the CRPD prior to June 2016. These cases are unevenly spread—in geography and in subject matter. In a significant number of these cases, civil society organisations and equality bodies supported disabled litigants (eg through third party interventions). The Public Sector Equality Duty has been construed as giving judges very little power to use the CRPD to hold public sector bodies to account. The CRPD was used as an interpretive aid only in connection with understanding how ECHR and EU law should be understood in the domestic context—suggesting that, were ECHR and EU law no longer to be part of United Kingdom law, the CRPD would play a greatly diminished role in guiding case law in the United Kingdom.

Functionalism in the theory of mind requires an account of function that has a normative component—mere conditional connection (whether indicative or sub-junctive) is not enough. For instance, a component of a computing system isn’t an adder just in case its output is always or would always be the sum of the inputs, since any computing system in a world with as much indeterminism as ours can err or malfunction. Two general reductions of normative language have been proposed that one might wish to apply to the problem of defining proper function: the evolutionary reduction (Wright, Millikan) and the agential reduction (Plantinga). We argue that whatever the merits of the reductions in other contexts, a functionalist theory of mind that defines proper function in either of these ways must fail. The argument proceeds by first showing the agential reduction is viciously circular in the context of a functionalist theory of agency. Second, if functionalism about mind is true and proper function is reducible evolutionarily, then it is possible to have a situation in which the presence or absence of mental properties depends in an implausibly spooky, acausal way on remote facts. It is plausible that the only currently avail-able way for the functionalist to meet these challenges is to accept irreducible end-directed causal powers of minds and/or their functional parts, in accordance with a broadly Aristotelian tradition.

Closing Death’s Door explores innovative legal strategies to address the challenge of medical error. In the United States today, several hundred thousand patients die in hospitals each year because of errors in medical treatment—the nation’s third leading cause of death. The legal mechanism designed to deal with this epidemic of injury and death is the medical malpractice system. It has failed to stem the tide of iatrogenic harm. Among the reasons are the costliness of the malpractice system, its availability to only a minuscule percentage of those harmed, and decades of “tort reform” efforts that have effectively extinguished the system for all but the most egregious claims. In 1999, in To Err Is Human, the Institute of Medicine (now the National Academy of Medicine) sounded an alarm about the toll taken by medical error. Its proposed solution—a set of reporting systems to document problems and generate data on which solutions might be based—has been a failure. The time has come for a fresh look at what the law might do to contribute to patient safety. To begin a conversation about legal innovations designed to spur healthcare system improvements directed at reducing harmful medical errors, this book explores a number of possible steps, including well-designed economic incentives to stimulate greater investment in safety; promotion of systems approaches to safer delivery of care; government regulation and surveillance in especially risky treatment contexts; and encouragement of a range of technological improvements, especially involving information technology.

Capillaria species are members of the superfamily Trichinelloidae. These worms have a filamentous thin anterior end and a slightly thicker oesophagus which is surrounded by glandular cells or stichocytes. This oesophageal pattern is called stichosomal oesophagus. Capillaria species are parasites which are found in many vertebrate animals. More than two hundred species have been reported in several vertebrate species, including fish, amphibians, reptiles, birds, and mammals (Cross 1992; Chitwood et al. 1968), but only three species infect humans. These are Capillaria hepatica , C. aerophila and C. philippinensis (McCarthy and Moore 2000). Of these intestinal capillariosis, a fish-borne parasitic zoonosis caused by C. philippinensis , is the most important. Humans acquire the parasite, C. philippinensis, by eating uncooked or raw freshwater fish (Cross and Basaca-Sevilla 1991). The disease is endemic mainly in Philippines and Thailand where there are many reported fatalities.Although C . hepatica is found in rodents worldwide, only a few cases of hepatic capillariosis have been reported in humans from Europe, Asia, Africa, North and South America. The infection is acquired by the ingestion of embryonated eggs from the soil. Female worms deposit eggs in the liver tissue and granulomas develop around the egg. The eggs are released after the rodent is eaten and the liver digested. Eggs pass in the faeces and are deposited in the soil where they embryonate. Avoidance of contaminated soil would prevent human infection and destruction of rodents would control animal infections.Only 12 cases of human infection caused by Capillaria aerophila have been reported, the majority from Russia. The parasite is found within tissue of the respiratory passages of canines and felines worldwide.Anatrichosoma cutaneum (Nematoda, Trichosomoididae), also included in this chapter, is primarily a subcutaneous parasite of monkeys, but there are two reports of cutaneous infections in humans resulting in serpiginous lesions in the skin of the soles, palms, and nasal passages. In addition there is a further suspected case isolated from a breast nodule and a possible case of mucosal lesions in the mouth reported. Whole monkey colonies can be infected with this parasite and control is difficult.

Inherited metabolic diseases (IMDs) are rare genetic disorders: clinically heterogeneous, and they can present at any age. With the expanded newborn screening panels, many of the IMDs have been successfully screened. Early diagnosis and treatment of these conditions have led to improved neurological outcomes and overall survival of these individuals, and now many of them are reaching childbearing age. Despite treatment, the potential presence of preexisting organ involvement may not only impact their fertility potentials but also may impose a higher risk of adverse maternal and fetal outcomes. Pregnancy leads to an extra strain on maternal metabolism; this may result in the manifestation of symptoms of a previously unknown disease or a progression of a known disease. This chapter will address the possible complications of some inherited disorders of metabolism that are associated with maternal or fetal neurological manifestations such as disorders of energy metabolism (eg, mitochondrial disorders, adult onset urea cycle disorders, ornithine transcarbamylase (OTC) deficiency, amino acidopathies, phenylketonuria (PKU), and impaired fatty acid oxidation disorders). We will provide special emphasis on the available potential treatments and plan of care during pregnancy and postpartum periods.

While the Treaty on European Union explicitly allows a Member State to withdraw from the Union, no country thus far had ever decided to secede from what is arguably the most successful experiment in regional integration in history. Brexit, therefore, calls into question consolidated assumptions on the finality of the EU, and simultaneously opens new challenges—not only in the institutional fabric of Europe, but also in the UK constitutional settlement, eg in Northern Ireland and Scotland. This book provides a first comprehensive analysis of the challenges posed by Brexit, their causes, and their consequences. This is a book about living solid organ donors as patients in their own right. This book is premised on the supposition that the field of living donor organ transplantation is ethical, even if some specific applications are not, eg, pre-mortem organ procurement of an imminently dying patient. When Joseph Murray performed the first successful living kidney donor transplant in 1954, he thought this would be a temporary stopgap. Today, however, the goal of adequate organ supply without living donors remains elusive. If anything, the supply:demand ratio is worse. In this book, a five-principle living donor ethics framework is developed and used to examine the ethical issues raised by living donor selection demographics, innovative attempts to increase living organ donation, and living donor decision-making and risk thresholds. This ethics framework uses the three principles of the Belmont Report modified to organ transplantation (respect for persons, beneficence, and justice) supplemented by the principles of vulnerability and of special relationships creating special obligations. The approach requires that the transplant community fully embraces living organ donors (and prospective living organ donors) as patients to whom special obligations are owed. Only when living organ donors are regarded as patients in their own right and have a living donor advocate team dedicated to their well-being can the moral boundaries of living solid organ donation be determined and realized. This book provides theoretical arguments and practice guidelines, complemented by case studies, to ensure that living donors are given the full respect and care they deserve.
