Clinical supervision of students, interns, and postdoctoral trainees is an essential professional activity of clinical neuropsychologists. Clinical supervision is considered a specific competency by the American Psychological Association. Unfortunately, most practicing neuropsychologists receive very little training in the provision of supervision, most likely because the existing literature on supervision in clinical psychology pertains mostly to therapy supervision. Although there has been an increase in attention paid to this issue in neuropsychology over the past decade, there have been no published books on the provision of supervision specifically in clinical neuropsychology. This book fills that gap in the literature by providing a review of practical, ethical, legal, and theoretical considerations when providing supervision in clinical neuropsychology. The book is intended for practicing neuropsychologists who provide training and for advanced trainees who are learning how to conduct clinical supervision. The book includes appendices with useful forms for evaluating and designing effective supervision.

Traditional fears and misinformed concerns regarding youth resistance training have been replaced by scientific evidence that indicates regular participation in well-designed resistance-training programmes can be safe and effective for both children and adolescents. In addition to increasing muscular strength and power, regular participation in a structured resistance training-programme can increase bone mineral density, improve cardiovascular risk factors, fuel metabolic health, facilitate weight control, enhance psychosocial well-being, and prepare youth for the demands of daily physical activity and sport. An integrative approach to training, grounded in resistance exercise and motor skill development, can optimize children’s fitness potential and maximize their athletic performance while reducing the risk of sports-related injury. Qualified professionals are needed to plan, implement and progress developmentally appropriate resistance training to attain a level of muscular fitness that facilitates long-term physical development.

Almost fifty million people in the United States provide informal, non-professional supportive care to chronically ill and disabled family members. Survivors of critical illness often depend upon such care for their well-being and recovery. ICU survivors may need help with physical, psychological, and cognitive deficits, often involving many hours of care per day. Families play essential caregiving roles, supporting survivors’ diverse and profound needs, often with minimal outside help. Caregiving can be personally fulfilling, but many families suffer financially, physically, and emotionally as a result of caring for loved ones after critical illness. While some institutional programmes provide direct support to ICU survivors, little infrastructure exists to support the activity of informal family caregivers. Measures are needed to identify caregivers at risk and to develop and provide effective treatments both within and beyond the ICU. Research has begun to elucidate some of the ways in which providers of critical care can identify those at risk of long-term psychological or emotional hardship and suggests strategies to prevent some of the overwhelming sequelae of critical illness among families. More work is needed to provide a deeper and more nuanced understanding of risk factors and preventive strategies for caregiver burden following critical illness. This chapter reviews the burdens of caregiving and suggests ways to help ICU survivors’ families manage the daunting challenge of helping their loved ones recover from life-threatening illness.

The need for a new approach to cardiovascular disease prevention, both secondary and primary, that is different from traditional health service provision through hospital cardiac rehabilitation services and general practice is evident. The targets set in the cardiovascular prevention guidelines for modifiable cardiovascular risk factors-smoking, diet and physical activity, weight and its distribution, blood pressure, lipids, and diabetes-are not being adequately achieved for either coronary or other vascular patients or for those at high multifactorial risk of developing CVD. There is also evidence of an increasing disparity in levels of risk between different community groups, largely attributable to social determinants of health. Community-based prevention centres provide a novel approach to reducing cardiovascular risk, in which there is shared working between professionals and the public and a shared understanding of the barriers that individuals experience in their attempts to engage in effective measures for both secondary and primary prevention.

Radiation considerations are an integral part of the practice of nuclear cardiac imaging. Concern regarding radiation has increased in recent years, reflected in statements by many professional societies, and likely attributable both to rapid growth in use of nuclear cardiology as well as high doses received by some nuclear cardiology patients. The fundamental principles of medical radiological protection are justification (ensuring that the right test is performed for the right patient at the right time), optimization (ensuring that the test is performed in the right manner), and dose limitation, which while applicable to healthcare workers is not operative regarding patients. Three "As" facilitate and serve as an organizing principle for justification: awareness, appropriateness, and audit. Awareness incorporates knowledge of the benefits and risks of testing involving radiation and effective communication of these to the patient. Appropriateness in nuclear cardiology can be assessed using the American College of Cardiology's appropriateness criteria. Methods that have been demonstrated to improve appropriateness include using a collaborative learning model, a point-of-order decision support tool, and a multifaceted intervention including threatened loss of insurance coverage. A variety of strategies should be considered for optimization to ensure patient-centered imaging. These including strategic selection of both the protocol, e.g. selecting a stress-first protocol and performing stress-only imaging in patients without a high pre-test probability of abnormal findings on stress imaging, or using PET, and also the administered activity, e.g. by using weight-based dosing and/or software- or hardware-based advances in camera technology. Special considerations are required for pregnant, nursing, and pediatric patients.

Clinical governance appeared as a concept in the UK in the late 1990s following scandals in which patients were harmed as a consequence of health care failures. Further international research estimates that one in ten inpatients suffer harm as a result of their health care, leading to death in some cases. Clinical governance is a framework centred around domains of patient safety, clinical effectiveness, and patient experience, underpinned by effective teamwork, leadership, and communication. Its aim is to ensure consistent, reliable, high-quality care delivered by competent individuals in a safe environment. Understanding why things go wrong in health care is key to finding solutions to ensure patient safety. Health care is an increasingly high-risk activity at organizational, departmental, and individual levels, and hazard identification and management are important. Recent recognition of human factors as contributing to many adverse events has facilitated the exploration of how health care professionals function within the context of a high-pressure, unpredictable environment. Lessons from non-health care industries have changed focus from blaming individuals for errors to understanding systems and how they can promote or mitigate failure. The development of non-technical skills, such as teamwork, is vital, and a number of approaches to improving this element of human behaviour are described.

This book argues that the rampant hate-filled attacks against women online are best understood as patterned resistance to women’s political voice and visibility. This abuse and harassment coalesces into an often-unrecognized form of gender inequality that constrains women’s use of digital public spaces, much as the pervasive threat of sexual intimidation and violence constrain women’s freedom and comfort in physical public spaces. What’s more, the abuse exacerbates inequality among women, those from racial, ethnic, religious, and/or other minority groups, are disproportionately targeted. Drawing on in-depth interviews with women who have been on the receiving end of digital hate, Credible Threat shows that the onslaught of epithets and stereotypes, rape threats, and unsolicited commentary about their physical appearance and sexual desirability come at great professional, personal, and psychological costs for the women targeted—and also with underexplored societal level costs that demand attention. When effective, identity-based attacks undermine women’s contributions to public discourse, create a climate of self-censorship, and at times, push women out of digital publics altogether. Given the uneven distribution of toxicity, those women whose voices are already most underrepresented (e.g., women in male-dominated fields, those from historically undervalued groups) are particularly at risk. In the end, identity-based attacks online erode civil liberties, diminish public discourse, limit the knowledge we have to inform policy and electoral decision making, and teach all women that activism and public service are unappealing, high-risk endeavors to be avoided.
