Full textMETODOLOGIA: Se han incluido 66 pacientes hipertensos esenciales de ambos sexos, de edades comprendidas entre 50-60 años, nunca tratados, y sin evidencia de enfermedad cardiovascular. Los pacientes con diabetes mellitus y/o ingesta enólica >30 grs/día, y aquéllos con una estenosis carotídea >50% fueron excluidos del estudio. Se realizaron las siguientes exploraciones: MAPA de 24 horas, ecocardiograma, y determinación del genotipo del gen de la enzima de conversión de la angiotensina (ECA) mediante PCR. A todos los pacientes se les realizó una resonancia magnética cerebral (RM) y se les clasificó en dos grupos, en función de la presencia o ausencia de LSB.
RESULTADOS: Veintisiete (40.9%) pacientes hipertensos mostraban LSB en la RM. Los pacientes con LSB tenían unas cifras de presión arterial (PA) sistólica, diastólica, y presión de pulso, significativamente mayores que los pacientes hipertensos sin LSB, tanto en la clínica como en la MAPA. La presencia de hipertrofia del ventrículo izquierdo (HVI) en pacientes hipertensos con LSB era significativamente mayor que en hipertensos sin LSB (88.4% versus 58.3%; P=0.01). De la misma manera, la presencia de una HVI concéntrica era significativamente mayor (P=0.002) en pacientes con LSB (54%) que en hipertensos sin LSB (11%). El riesgo relativo de HVI concéntrica para la presencia de LSB, independiente de los valores de PA, fue de 8.22 (95% IC: 2.06-32.78). En referencia al estudio genético, se objetivó que los pacientes con LSB presentaban con mayor frecuencia, en comparación con los pacientes sin LSB, tanto el alelo D (74% versus 51.4%; P=0.014) como el genotipo DD (64% versus 28.6%; P=0.022) del gen de la ECA.
CONCLUSIONES: La presencia de LSB en pacientes hipertensos asintomáticos de mediana edad es un hallazgo frecuente y está relacionado con la severidad de la elevación de la PA. La existencia de estas lesiones está asociada a la presencia de una HVI concéntrica. De la misma manera, y de forma independiente de las cifras de PA, la presencia del genotipo DD del gen de la ECA podría ser un factor predisponente para el desarrollo de LSB en pacientes con hipertensión arterial esencial.

Full textBackground: FPIES is a non-IgE-mediated food allergy, meeting precise diagnostic criteria. Treatment involves avoiding the offending food. There is no serum biomarker confirming the diagnosis or predicting the development of tolerance. Only the oral food challenge (OFC) regularly assesses allergenic tolerance. In most cases, children acquire spontaneous tolerance at school age. However, the severity of symptoms and the absence of specific IgE justify research into FPIES to gain a better understanding of the pathophysiology.Hypothesis: FPIES is associated with digestive dysbiosis, local digestive dysimmunity, chronic intestinal inflammation and increased intestinal permeability. Inflammation and permeability are exacerbated during an acute allergic reaction. Once tolerance is acquired, all these parameters return to normal. Objectives: To provide a longitudinal description of the faecal and salivary microbiota of paediatric patients with SEIPA, at the times of allergy and tolerance, as well as biomarkers of interest assessing inflammation, permeability and digestive dysimmunity, compared with a reference cohort. Materials and methods: Faecal and salivary samples were collected from children with FPIES followed in several AP-HP hospitals, after diagnosis on an elimination diet, and after acquiring tolerance, as well as from matched controls at the same ages (1:1 ratio). Additional stool samples were also collected before and after OFC. The composition of the microbiota was analysed by r16s RNA gene sequencing (V3-V4). The function of the faecal microbiota was estimated by measuring short-chain fatty acids (SCFA) (gas chromatography). Faecal calprotectin, eosinophil-derived neurotoxin (EDN), faecal secretory IgA, and faecal zonulin assessed inflammation, immunity and intestinal permeability (ELISA).Main results: From December 2019 to May 2023, 38 patients were included (mean age: 1.3 years). At inclusion, allergic patients had faecal and salivary dysbiosis, expressed as decreased alpha-diversity, different beta-diversity, several different bacterial genera, and increased relative abundance of acetate compared with matched controls. Allergic patients did not have intestinal inflammation on elimination diet (calprotectin, EDN), but only acutely and transiently after an allergic reaction. Levels of faecal secretory IgA were higher in allergic patients than in controls or tolerant patients. Zonulin was similar between patients and controls. After acquiring tolerance, in the 22 patients becoming tolerant, faecal and salivary microbiota partially approximated those of controls. Secretory AGCC and IgA levels normalised (increase in relative butyrate, decrease in IgA). Conclusions: According to the results of this cohort, and to previous data in the scientific literature, children with FPIES have a gut dysbiosis before the onset of allergy, gut and salivary dysbiosis during allergy until the acquisition of tolerance, and then a return of a microbiota close to that of healthy children. The local adaptive immune response is exaggerated during the disease, with an increase in faecal secretory IgA in allergic individuals compared with controls, and normalisation after tolerance. The exact role of this IgA is not clearly identified

Full textThe Extracellular Matrix (ECM) constitutes a fibrillar platform that integrates the action of chemical and mechanical cues from the environment. Fibronectin (FN), a major component of the ECM, is at the center of this bioregulatory stage, modulating numerous biological procedures such as cell adhesion and motility, cell proliferation and differentiation, as well as ECM deposition and structure. FN is found in two forms: plasma FN (pFN) and cellular FN (cFN). cFN differs from pFN by the presence of alternatively spliced Extra Domains, namely EDB and EDA. Each of these alternatively spliced regions is encoded by a single exon, the regulation of which is strictly regulated and limited to embryonic tissues, as well as pathophysiological conditions such as wound healing, inflammation, and cancer. The term “oncofetal” was coined in order to describe FN isoforms harboring either of the Extra Domains, that are normally absent in adult tissues.How the presence of EDA and EDB regulates FN assembly and how the assembled variants regulate cell behavior is largely unknown. Single Extra Domain-targeted deletion studies in the mouse have revealed roles for EDA in the morphogenesis of lymphatic valves, atherosclerosis, and wound healing/fibrosis. Mechanistically, the inclusion of EDA expands the repertoire of cellular FN receptors (α4β1, α7β1 integrins and TLR4). To date, no ligand has been reported for EDB. Rather, its presence has been postulated to alter the conformation of the cell-binding domain of FN and to facilitate integrin-driven fibrillar assembly by mechanisms that remain to be established.The aim of this dissertation is to provide more insight regarding the roles of the FN Extra Domains EDA and EDB, and more specifically:1) To investigate how the presence of alternatively spliced EDA and EDB domains affect the fibrillar assembly of FN at the surface of assembly-competent cells, and2) To determining how the presence of EDB and EDA influences the biochemical, physical, and functional properties of the matrix that in turn affect the behavior of cells attached to it.Here we present a biological toolset composed of : i) lentiviral vectors harboring the full-length coding sequence of human cFN containing one, both, or none of the alternatively-spliced Extra Domains, ii) purified recombinant FN variants, iii) FN-null mouse embryo fibroblasts (MEFs) and iv) FN variant-expressing MEFs. These unique tools were used to study variant-specific assembly by fibroblasts and the effects of homogeneous cFN networks on cell behavior.Utilizing an unbiased computational approach, we document that the presence of the Extra Domains results in a distinct pattern of FN deposition and assembly that in turn influences both early and late signaling events that control cell proliferation as well as cell contractility. Finally, we present a series of novel data that point towards a FN-impacted control of cell energetics.We conclude that FN Extra Domains are responsible for the fine-tuning of the extent of several cellular processes that can reflect strict regulation in pathophysiological procedures such as tissue repair, fibrosis, and tumor progression
