Darbo tikslas. Apžvelgti klinikinėje praktikoje gydytojams aktualią informaciją apie medicininę apšvitą ir jos poveikius bei praktines galimybes kontroliuoti diagnostinę apšvitą. Medžiaga ir metodai. Mokslinėse duomenų bazėse („PubMed“, „SienceDirect“ ir „EBSCO Publishing“) 2000–2015 metų skelbtų publikacijų apie jonizuojančios spinduliuotės poveikį, medicininę apšvitą ir diagnostinės apšvitos sukeliamą vėžio riziką apžvalga. Tarptautinio radiacinės saugos komiteto (TRSK) ir JAV Nacionalinės mokslų akademijos dokumentų apžvalga. Rezultatai ir išvados. Medicininė apšvita didėja dėl augančio rentgeno diagnostinių tyrimų skaičiaus. Didžiausią apšvitos dalį pacientai gauna atliekant kompiuterinės tomografijos tyrimus. Medicininė apšvita gali sukelti nulemtuosius ir atsitiktinius reiškinius. Publikuojami medicininės apšvitos sąlygoto vėžio rizikos tyrimų duomenys nėra vienareikšmiai. Nors vėžio rizika susijusi su gauta jonizuojančios spinduliuotės doze, bet kokio dydžio jonizuojančios spinduliuotės dozė vėžio riziką didina. Vaikų, jaunų asmenų, moterų, nėščiųjų ir genetinėmis ligomis sergančių pacientų jautrumas jonizuojančios spinduliuotės poveikiui yra didesnis. Atliekant su jonizuojančia spinduliuote susijusias diagnostines procedūras būtina remtis ALARA principais. Didžiausia radiologo įtaka mažinant diagnostinę apšvitą tiesiogiai susijusi su optimalių tyrimo protokolų sukūrimu ir pritaikymu.

Abstract:
Over the past seven years and since Archnet-IJAR has published its first issue in March 2007 the journal has managed to position itself on the map of web publishing in architecture and urbanism. Following a comprehensive process of academic review of articles and research work submitted to the journal, it has succeeded to secure its position in various databases. One of the unique features of the Archnet-IJAR was, and still is, the voluntary work nature of its editors and reviewers where no funds are involved at all in managing or reviewing or editing the submissions before publishing. Another unique feature is that the journal since the beginning was, and still is, made available for free without subscription fees or publishing fees. A third feature of Archnet-IJAR is the opportunity it offers to young academics, PhD scholars, reaching across the boundaries of cultures and regions.Key highlights in the past seven years are important to note. In qualitative and quantitative terms the journal has made significant achievements. In terms of quality the journal became part of important data and index bases including Avery index to Architectural Periodicals, EBSCO-Current Abstracts-Art and Architecture, INTUTE, Directory of Open Access Journals, Pro-Quest, Scopus-Elsevier and many university library databases. In terms of quantity 20 issues were published in 7 volumes (Figure 1). The 20 issues combined include 283 papers categorized in two categories: 237 original research articles and 46 book reviews and trigger articles. I should mention here that the issues produced by Archnet-IJAR cover almost every corner of the globe whether in terms of the context that they address or in terms of its authors’ affiliations and cultural backgrounds.

Huriah, Titih, and Vina Dwi Lestari. "School-based Smoking Prevention in Adolescents in Developing Countries: A Literature Review." Open Access Macedonian Journal of Medical Sciences 8, F (March 23, 2020): 84–89. http://dx.doi.org/10.3889/oamjms.2020.4336.

Full text
Abstract:
BACKGROUND: The incidence of diseases due to tobacco consumption has increased, especially in developing countries, where around 90% of smokers start consuming tobacco before the age of 18 years. School-based smoking prevention programs can prevent smoking among adolescents. This program is often implemented in developing countries because it is considered cost-effective, and the results are promising. At present, the school-based smoking prevention program is expanding with many methods. AIM: This study aims to describe the various types of school-based smoking prevention interventions in developing countries. METHODS: This research applied a literature review approach with the process of collecting data through the Science Direct, ProQuest, EBSCO, and NCBI databases. Search literature was by keyword “smoking AND prevention AND school-based AND adolescent AND randomized controlled trials or randomized controlled trials.” Inclusion criteria used for data collection were studies on school-based smoking prevention interventions, full text, and journal publishing from 2015 to 2019, using English. The research conducted in developing countries and prevention interventions was not only for smoking in the traditional way but also in the form of other than the consumption of tobacco such as cigars, smokeless tobacco, and hookah or shisha. Exclusion criteria in this review were publication articles not in the form of original publications such as letters to editors, only abstracts, and books. RESULTS: The literature search results found 594 journals with details of 99 Science direct journals, ProQuest 385 journals, NCBI 85 journals, and 25 EBSCO journals. The entire database belongs to the inclusion criteria and only seven articles meet the requirements. The results of the analysis revealed that interventions conducted in school-based smoking prevention programs for adolescents in developing countries included the application of the anti-smoking curriculum, behavior change intervention (BCI), and peer education. CONCLUSION: Interventions considered useful for preventing smoking in adolescents are the anti-smoking curriculum, BCI, and peer education.

Abstract:

Regular exercises can improve walking distance and reduce physical impairment in patients with intermittent claudication. The objective of the research is to summarise and analyse the existing proof about the comparative effectiveness of Nordic walking and traditional walking in the improvement of health indicators in patients with intermittent claudication. Methods: The following online databases were used as sources for data collection: Ebsco, Sience Direct, Clinical Key, ProQuest, Scopus. The selection of works of research was performed on the basis of key words “Nordic walking”, “intermittent claudication”, “walking with poles”, “Nordic walking and walking and intermittent claudication” and the year of publishing (that is, from 2005 till 2015). Inclusion criteria met four works of research that were included in the critical analysis. Results and conclusions: There is no proof about the superiority of the Nordic walking programme over traditional walking (and there exist indications about the superiority of the traditional walking programme) in the improvement of helath indicators, but patients with intermittent claudication feel less discomfort during Nordic walking and they can cover a longer distance by using poles.

Abstract:
Psychology & Behavioral Sciences Collection is a primarily full-text journal database published by EBSCO Publishing Inc. for use by mental health practitioners, behavioral science researchers, and students at the undergraduate and graduate levels. Holdings include nearly 600 titles of which the vast majority are peer-reviewed and/or full-text. This database is touted for offering 290 full-text journals which are indexed in APA PsycInfo and which have the capability of being linked to results in PsycInfo for subscribing institutions. The EBSCOhost platform is intuitive and user-friendly with a variety of authentication methods and compatible browsers. In comparison to such competitive products as APA PsycArticles and ProQuest Psychology Database, Psychology & Behavioral Sciences Collection offers sufficient unique content and number of full-text journals indexed in PsycInfo to justify its purchase either in addition to these products or as a standalone database. All three products are cross-searchable with PsycInfo with a subscription. Those institutions seeking more extensive historical content, greater depth of coverage of a range of subjects in the behavioral sciences, access to APA published journals or the specialized Thesaurus of Psychologist Index Terms, or who prefer the ProQuest or APA PsycNet platform may consider PsycArticles and/or Psychology Database as alternative subscription options.

Abstract:
This study identified nearly 700 English-language refereed journals in education and psychology that were founded in 2000–2009. Part one discusses the publishers, format, open-access availability, and current status of these publications. Titles were then searched against coverage lists of EBSCO Academic Search Complete, Gale Academic OneFile, ProQuest Central, ERIC, PsycINFO, Web of Science, DOAJ, Google Scholar, WorldCAT, and the library catalogs of the “Big Ten” universities to determine whether databases and libraries include these new publications. Subscription database coverage was poor, ranging from 8.8 percent (ProQuest Central) to 42.0 percent (PsycINFO). Psychology materials were heavily favored over education items in several databases. Although some library catalogs provide better title-level coverage, they are unable to search individual articles. Google Scholar only indexed the publishers’ versions of the journals in 143 (58.0%) of 247 cases examined. Significant differences in database coverage and library holdings were found when comparing publications of major corporations (Elsevier, Routledge/Taylor & Francis, Sage, Springer, and Wiley) against periodicals produced by smaller companies, colleges/universities, and scholarly/professional organizations. This article also describes a “ubiquity index” devised by the author to identify approximately 70 “journals of the decade” based on database coverage and library holdings. The study provides much cause for concern about the comprehensiveness and currency of existing discovery tools. It also offers evidence that the relationship between libraries and publishing conglomerates deserves further examination.

Abstract:
ABSTRACT Background Because residents are often on the frontlines of patient care and are likely to witness adverse events firsthand, it is critical they report patient safety events. They may, however, be underreporting. Objective We examined the current literature to identify strategies to increase patient safety event reporting by residents. Methods We used CINAHL (EBSCO Information Services, Ipswich, MA), EMBASE (Elsevier, Amsterdam, the Netherlands), PsycINFO (APA Publishing, Washington, DC), and PubMed (National Center for Biotechnology Information, Bethesda, MD) databases. The search was limited to English-language articles published in peer-reviewed journals through March 2020. Key terms included “residents, trainees, fellows, interns, graduate medical education, house staff, event reporting, patient safety reporting, incident reporting, adverse event, and medical error.” To organize findings, we adapted a published framework of strategies for encouraging self-protective behavior. Results We identified 68 articles that described strategies used to increase event reporting. The most sustainable interventions used a combination of 3 of the 5 strategies: behavior modeling, surveys and messaging, and required limited financial support. The survey creates awareness; the behavior modeling is critical for educational purposes, and the reminders help to reinforce the new behavior and embed it into routine patient care activities. We noted a dearth of studies involving trainees in root cause analysis following submission of event reports. Conclusions The most successful sustainable interventions were those that combined strategies that minimized time for busy physicians, incorporated accessible event reporting in already existing medical records, and became part of a normal workflow in patient care.

Abstract:
This past year Advances in Social Work hit its 20th anniversary! We are proud of our legacy of being one of the first, if not the first, open access journals in social work. Looking back over our first 20 years, it is evident that we ramped up our productivity as a scholarly journal over time. In our first decade (2000-2009), we published 142 papers and offered 3 special issues. In our second decade (2010-2019), we more than doubled the number of papers published to 307 and tripled the number of special issues (n=10). Our first decade relied on the efforts of three consecutive editors (Cournoyer, Daley, Barton) and two guest editors (Adamek, Vernon). Our second decade saw the addition of an Assistant Editor (Valerie Decker), an open access technical expert (Ted Polley), a Statistical Consultant (Jieru Bai), and the contributions of 16 guest editors. We grew from 33 reviewers evaluating manuscripts in 2000 to 189 individuals from over 100 universities and institutions in 7 countries, 1 territory, and 43 states serving as reviewers in 2019. The work of Advances in Social Work is ably guided by our Editorial Board. This fall we are pleased to welcome two new editorial board members: Dr. Lauri Goldkind and Dr. Lisa Zerden from the University of North Carolina at Chapel Hill. We are looking forward to their contributions as we head into our third decade. In this issue of Advances in Social Work we are pleased to present 10 empirical papers authored by 35 scholars who are geographically dispersed across 15 states in the U.S., Washington, DC, and Canada. Four papers touch upon various aspects of violence prevention or intervention, two papers address social work management issues, three papers focus on diversity and/or advocacy for particular populations, and the final paper shares an efficacious approach for teaching practice skills to online students. While celebrating our first 20 years, we continue to look to the future. We are grateful for the ongoing support of Dean Tamara Davis as we endeavor to document and share emerging knowledge in the field of social work in a fully open access format. We are fully indexed in SocINDEX with Fulltext (EBSCO), Social Work Abstracts (EBSCO), and Social Services Abstracts (ProQuest) and were recently accepted into Scopus. As we forge ahead into our third decade, we commit to publishing the latest works from social work scholars around the world, addressing contemporary issues of the utmost importance to the communities we all serve. We look forward to bringing you upcoming issues of Advances in Social Work highlighting interprofessional practice and education (Summer 2020), gender inequity in the workforce (Fall 2020), and anti-racist education (Spring 2021). We welcome your suggestions for special issue topics that will help to advance social work and our causes around the globe.

Abstract:
Acupuncture clinical practice guidelines are authoritative medical recommendations developed by evaluating and integrating acupuncture-related evidence. However, their synthesis and dissemination are not integrated, and clinical practitioners require more credible effective evidence. The study aim was to systematically review problems disseminating acupuncture clinical practice guidelines to clinical practitioners, to facilitate evidence dissemination. This systematic review included searches of PubMed, EBSCO, Web of Science, and four major Chinese electronic databases (CNKI, VIP, Wanfang Database, and SinoMed) from inception to October 26, 2021. Two independent reviewers screened the literature, extracted information, and evaluated the quality of included studies. A systematic review was subsequently performed. Eleven studies were reviewed: nine (81.8%) cross-sectional surveys and two (18.2%) systematic reviews. The evaluated clinical practice guidelines differed across studies; seven studies (63.6%) evaluated guidelines for a specific disease, one (9.1%) evaluated guidelines for acupuncture therapies (e.g., moxibustion and fire acupuncture), one (9.1%) evaluated US acupuncture guidelines and recommendations, and two (18.2%) did not describe the guideline content. The included studies used different evaluation indicators. Guideline dissemination problems included lack of guideline standardization, unclear target population, mismatch between guidelines and application environment, lack of reliable health economics evaluation, poor quality content of the recommendations, lack of linkage between recommendations and evidence, and disassociation of recommendations from clinical practice et al. The development and publishing of credible acupuncture clinical practice guidelines is urgently needed to improve the usability of guidelines and standardize and disseminate tools for analysing information to clinical practitioners and to help the domestic and international acupuncture community to apply evidence to practice. Recommendations for promoting the dissemination of acupuncture clinical practice guidelines are to define clinical events suitable for the target population, to develop recommendations relevant to clinical practice, to improve the evidence evaluation index system, and to further standardize the method and process of formulating guidelines.

Abstract:
Dear Sir, From the beginning of life on this planet research and writing remained one of the most important aspect for getting more attentions and values. But, with the pas- sage of times different methods has been introduced to share the knowledge and information i.e. from hand writing to hard publication and then to online publication. Dear readers, it was a story of 2015 when the honorable Dr. Phuc Van Pham (Founder and Editor-in-Chief) came with an idea and started the journal with name Biomedical Research and Therapy is the official journal of the Laboratory of Stem Cell Research and Application (SCL) (now as Stem Cell Institute, VNUHCM University of Science, Ho Chi Minh city, Viet Nam). After launching the journal, it was thinking that the journal will not exist for long time, be- cause it was a hard journey with limited resources. However, the journal is entered in to their 5th volume in 2018. Due to the hard working, creative ideas, team working and non-stoppable efforts of Dr. Pham and his team proofed that Biomedical Research and Therapy is one the emerging journal in the field of biomedical sciences. The journal is now indexed and abstracted by world renowned databases including Web of Science, Scopus, Embase, Index Copernicus, Ebsco, and Google Scholar. In a very short time of period the journal become more popular in the Asian countries. Researchers from different parts of the world is publishing their work in Biomedical Research and Therapy. Abstract:
AbstractEven in as small a country as R. Macedonia with limited resources allocated for science, there are many journals trying to establish good editorial practices and policies in publishing the scientific work achieved. Among the currently existing medical journals Macedonian Medical Review (MMR), ISSN 0025-1097, deserves to be elaborated as the oldest journal with continuous publication since its first appearance as the journal of the Macedonian Medical Association (MMA). Since its first issue, published in 1946, there has been an opus of some 4500 peer-reviewed published papers in more than 210 issues and some 80 supplements from various congresses and meetings. In this regard, great respect should be paid not only to the editorial boards, but also to the collaborators who have contributed to its successful continuity in all previous years. In line with the needs for further development of the journal and possibilities for access to world databases, the Editorial Board of MMR has made every effort to improve and modernize its work as well as the technical quality of the journal. Hence, MMA has signed a contract with De Gruyter Open as leading publisher of Open Access academic content for further improvement and promotion of the journal and facilitation of the Medline application, so we do hope for the further success of the journal.BANTAO Journal is published on behalf of the Balkan Cities Association of Nephrology, Dialysis, Transplantation and Artificial Organs (BANTAO), ISSN 1312-2517. The first issue was published in 2003, ten years after BANTAO was born. Its appearance was an extremely important event in the existence of BANTAO. The first official editor of the journal was Dimitar Nenov, Varna (2003-2005), followed by Ali Basci (Izmir, Turkey) and Goce Spasovski (Skopje, Macedonia) as editor-in-chief since 2009. Over the years, the Journal has been included in the EBSCO, DOAJ and SCOPUS/SCIMAGO databases. The journal is published biannually. Until now, 345 papers have been published in the past 11 years, in 21 regular issues and 3 supplements. It may be said that the journal is the “glue” between the nephrologists from the Balkan cities, reflecting the high quality research and scientific potential of Balkan nephrologists. The entire process of submitting and reviewing the manuscripts is electronically done and after their acceptance they are freely available (open access journal) on the website of the association and the journal: www.bantao.org.In this regard, the current President of BANTAO has already signed a contract with De Gruyter Open as leading publisher of Open Access academic content for further improvement and promotion of the journal and Medline application for the further success of the journal.

Abstract:
Health tourism is when patients travel to another state (or within a country) to improve or restore health (Hudson and Li, 2012). Recent studies show that health care spending is on the rise, averaging around 9 percent of GDP in OECD countries, and that the right balance needs to be struck between spending on disease prevention and treatment. Assessing the problem field of the health tourism industry and the current situation, the following key problems are identified: lack of communication and support between health tourism policy makers and health tourism organizations; lack of a culture of cooperation and organizational partnership between the public and private sectors; lack of strategic planning; gaps in the targeted distribution of responsibilities between actors in the health tourism network; resistance of the public and private sector to change. Thus, the article solves the problem – how to ensure the effective application of the principles of organizational wisdom in the health tourism industry? According to the authors, it is expedient for the organizations involved in this complex field of tourism to base their activities on the principles of organizational wisdom. The aim of the article is to propose the application of the principles of organizational wisdom in the health tourism industry. For the systematic review, scientific articles were searched in databases: Cambridge Journals Online; EBSCO; Emerald Publishing; SAGE Journals Online; Science direct; Taylor & Francis. The scientific articles included in the systematic review cover the period 1999-2020. The following methods are used to present the results of the research: comparative analysis, synthesis, graphical representation and generalization. The study identified the following essential principles of organizational wisdom that are appropriate to adapt in the health tourism industry: transformational leadership; organizational culture, management structure and teamwork; recognition of environmental change and rapid and effective response to it; systemic thinking; interaction between different organizations that can lead to high-quality solutions (innovation); the ability to accumulate organizational memory to compare past and present situations and decisions made. The authors in the article substantiate the opinion that the sustainability of the application of these principles depends on the mechanism of organizational partnership in organizing / creating health tourism cooperative. Such an organization has: a balanced structure and clearly defined roles and responsibilities of participants; long-term commitments are matched by specific short-term objectives that can be measured; periodic evaluation of the effectiveness of the role played by each partner; accurate and effective communication between partners and all stakeholders.

Abstract:
“Those with access to these resources — students, librarians, scientists — you have been given a privilege. You get to feed at this banquet of knowledge while the rest of the world is locked out. But you need not — indeed, morally, you cannot — keep this privilege for yourselves. You have a duty to share it with the world.” - Aaron Swartz1 (who killed himself at the age of 26, facing a felony conviction and prison sentence for downloading millions of academic journal articles) The Philippine Journal of Otolaryngology Head and Neck Surgery was accepted into the Directory of Open Access Journals (DOAJ) on October 9, 2019. The DOAJ is “a community-curated online directory that indexes and provides access to high quality, open access, peer-reviewed journals”2 and is often cited as a source of quality open access journals in research and scholarly publishing circles that has been considered a sort of “whitelist” as opposed to the now-defunct Beall’s (black) Lists.3 As of this writing, the DOAJ includes 13,912 journals with 10,983 searchable at article level, from 130 countries with a total of 4,410,788 articles.2 Our article metadata is automatically supplied to, and all our articles are searchable on DOAJ. Because it is OpenURL compliant, once an article is on DOAJ, it is automatically harvestable. This is important for increasing the visibility of our journal, as there are more than 900,000 page views and 300,000 unique visitors a month to DOAJ from all over the world.2 Moreover, many aggregators, databases, libraries, publishers and search portals (e.g. Scopus, Serial Solutions and EBSCO) collect DOAJ free metadata and include it in their products. The DOAJ is also Open Archives Initiative (OAI) compliant, and once an article is in DOAJ, it is automatically linkable.4 Being indexed in DOAJ affirms that we are a legitimate open access journal, and enhances our compliance with Plan S.5 The Plan S initiative for Open Access publishing launched in September 2018 requires that from 2021, “all scholarly publications on the results from research funded by public or private grants provided by national, regional, and international research councils and funding bodies, must be published in Open Access Journals, on Open Access Platforms, or made immediately available through Open Access Repositories without embargo.”5 Such open access journals must be listed in DOAJ and identified as Plan S compliant. There are mixed reactions to Plan S. A recent editorial observes that subscription and hybrid journals (including such major highly-reputable journals as the New England Journal of Medicine, JAMA, Science and Nature) will be excluded,6 quoting the COAlition S argument that “there is no valid reason to maintain any kind of subscription-based business model for scientific publishing in the digital world.”5 As Gee and Talley put it, “will the rise of open access journals spell the end of the subscription model?”6 If full open access will be unsustainable for such a leading hybrid medical journal as the Medical Journal of Australia,6 what will happen to the many smaller, low- and middle-income country (southern) journals that cannot sustain a fully open-access model? For instance, challenges facing Philippine journals have been previously described.7 According to Tecson-Mendoza, “these challenges relate to (1) the proliferation of journals and related problems, such as competition for papers and sub-par journals; (2) journal funding and operation; (3) getting listed or accredited in major citation databases; (4) competition for papers; (5) reaching a wider and bigger readership and paper contribution from outside the country; and (6) meeting international standards for academic journal publications.”7 Her 2015 study listed 777 Philippine scholarly journals, of which eight were listed in both the (then) Thomson Reuters (TR) and Scopus master lists, while an additional eight were listed in TR alone and a further twelve were listed in Scopus alone.7 To date, there are 11,207 confirmed Philippine periodicals listed on the International Standard Serial Number (ISSN) Portal,8 but these include non-scientific and non-scholarly publications like magazines, newsletters, song hits, and annual reports. What does the future have in store for small scientific publications from the global south? I previously shared my insights from the Asia Pacific Association of Medical Journal Editors (APAME) 2019 Convention (http://apame2019.whocc.org.cn) on the World Association of Medical Editors (WAME) Newsletter, a private Listserve for WAME members only.9 These reflections on transformation pressures journals are experiencing were the subject of long and meaningful conversations with the editor of the Philippine Journal of Pathology, Dr. Amado Tandoc III during the APAME 2019 Convention in Xi’an China from September 3-5, 2019. Here are three main points: the real need for and possibility of joining forces- for instance, the Journal of the ASEAN Federation of Endocrinology Societies (JAFES) currently based in the Philippines has fully absorbed previous national endocrinology journals of Malaysia and the Philippines, which have ceased to exist. While this merger has resulted in a much stronger regional journal, it would be worthwhile to consider featuring the logos and linking the archives of the discontinued journals on the JAFES website. Should the Philippine Journal of Otolaryngology Head and Neck Surgery consider exploring a similar model for the ASEAN Otorhinolaryngological – Head and Neck Federation? Or should individual specialty journals in the Philippines merge under a unified Philippine Medical Association Journal or the National Health Science Journal Acta Medica Philippina? Such mergers would dramatically increase the pool of authors, reviewers and editors and provide a sufficient number of higher-quality articles to publish monthly (or even fortnightly) and ensure indexing in MEDLINE (PubMed). the migration from cover-to-cover traditional journals (contents, editorial, sections, etc.) to publishing platforms (e.g. should learned Philippine societies and institutions consider establishing a single platform instead of trying to sustain their individual journals)? Although many scholarly Philippine journals have a long and respectable history, a majority were established after 2000,7 possibly reflecting compliance with requirements of the Commission on Higher Education (CHED) for increased research publications. Many universities, constituent colleges, hospitals, and even academic and clinical departments strove to start their own journals. The resulting journal population explosion could hardly be sustained by the same pool of contributors and reviewers. In our field for example, faculty members of departments of otorhinolaryngology who submitted papers to their departmental journals were unaware that simultaneously submitting these manuscripts to their hospital and/or university journals was a form of misconduct. Moreover, they were not happy when our specialty journal refused to publish their papers as this would constitute duplicate publication. The problem stemmed from their being required to submit papers for publication in department, hospital and/or university journals instead of crediting their submissions to our pre-existing specialty journal. This escalated the tension on all sides, to the detriment of the new journals (some department journals ceased publication after one or two issues) and authors (whose articles in these defunct journals are effectively lost). The older specialty journals are also suffering from the increased number of players with many failing to publish their usual number of issues or to publish them on time. But how many (if any at all) of these journals (especially specialty journals) would agree to yield to a merger with others (necessitating the end of their individual journal)? Would a common platform (rather than a common journal) provide a solution? more radically, the individual journal as we know it today (including the big northern journals) will cease to exist- as individual OA articles (including preprints) and open (including post-publication) review become freely available and accessible to all. However proud editors may be of the journals they design and develop from cover to cover, with all the special sections and touches that make their “babies” unique, readers access and download individual articles rather than entire journals. A similar fate befell the music industry a decade ago. From the heyday of vinyl (33 and 78 rpm long-playing albums and 45 rpm singles) and 8-tracks, to cassettes, then compact disks (CD’s) and videos, the US recorded music industry was down 63% in 2009 from its peak in the late 70’s, and down 45% from where it was in 1973.10 In 2011, DeGusta observed that “somewhat unsurprisingly, the recording industry makes almost all their money from full-length albums” but “equally unsurprising, no one is buying full albums anymore,” concluding that “digital really does appear to have brought about the era of the single.10 As McDowell opines, “In the end, the digital transforms not only the ability to disrupt standard publishing practices but instead it has already disrupted and continues to break these practices open for consideration and transformation.”11 Where to then, scientific journals? Without endorsing either, will Sci-Hub (https://sci-hub.se) be to scholarly publishing what Spotify (https://www.spotify.com) is to the music industry? A sobering thought that behooves action.

Abstract:
Background:Individuals with Sjögren’s syndrome (SS) experience significantly higher levels of sexual dysfunction and sexual distress than healthy controls (van Nimwegen et al., 2015). Identifying associated factors may help to identify a subgroup of patients with SS who may benefit from early intervention to maintain sexual wellbeing and avoid unnecessary sexual disruption. Objectives:To explore and map the salient symptoms and factors that influence alterations in sexual functioning and intimate relationships for people with SS.Methods:The protocol for this review was registered with the Open Science Framework prior to commencement of the searches. The peer-reviewed search strings were used to search the following databases from inception to June 2019: Cochrane Library, CINAHL [EBSCO], MEDLINE [ProQuest], PUBMED [MEDLINE], ScienceDirect, Scopus and Web of Science. Grey literature was searched for on academic databases, topic-specific repositories, and Google Scholar. Databases were searched using key terms corresponding to sexual functioning and intimate relationships. Studies were included if their participant sample was comprised of adults aged ≥18 years, with a diagnosis of primary or secondary SS. Studies were not excluded based on source type, methodology or design. To qualify for inclusion, studies needed to have been peer-reviewed and available in English. Retrieved articles were then screened against the inclusion/exclusion criteria by two reviewers. Hand-searching was conducted on the reference lists of included articles, as well as the three most prevalent publishing journals until saturation had been achieved. Results:The search strategy returned 3527 unique citations. After screening processes were completed, only 19 articles met the inclusion criteria. Studies were predominately conducted in European countries (79%), within the last decade (68%; 2010-2019), and were mainly quantitative (n = 17; 89.5%), case-controlled (88.3%), and cross-sectional (100%) in nature. In total there were 1281 patients, 47% (n = 605) were patients with primary SS and the remaining 53% (n = 676) were patients with secondary SS. Both patient groups were predominately comprised of females (n = 600; 99% and n = 673; 99.5%, respectively), with a combined mean age of 50.82 years (M ranges = 35 – 62.82 years). An amalgamation of results from 17 studies, found that women with SS who score higher on the ESSPRI scale (total score and the subdomains of pain, fatigue and dryness) were more likely to experience significantly greater levels of vaginal dryness, sexual dysfunction and sexual distress. Moreover, women with SS who present with clinical levels of anxiety or depression were also more likely to experience disruptions in their sexual functioning and appraise their sexual life more negatively. Furthermore, patients who report greater severity of oral or ocular dryness, or dyspareunia may experience vaginal dryness, which may have ramifications on sexual functioning. Women of all ages are at risk of experiencing sexual dysfunctions, however, younger women (≤50 years) may experience more burdensome disruptions than older women. Finally, women who do not use lubrication products during sexual activity may be impacted further.Figure 1.Factors significantly associated with sexual dysfunction and sexual distress for patients with SS.Conclusion:Younger women (≤50) with SS who present with more severe symptoms of fatigue, pain, and oral or ocular dryness, or with clinical levels of anxiety or depression, may be at increased risk of experiencing sexual dysfunction and sexual distress. Healthcare professionals should be aware of these potential risk factors and initiate conversations around sexuality as and when a potentially ‘at risk’ individual is identified.References:[1]van Nimwegen, J.
