Abstract:
Over the past 10 to 15 years, Interstate pavements in northern Florida have experienced a significant number of failures, primarily due to rutting. It was believed that the present fine-graded, 50-blow Marshall-designed mixes were inadequate to withstand current loading conditions. The newly developed Superpave system represented an opportunity to address several of Florida’s asphalt pavement problems. Thus, the Florida Department of Transportation made a concerted effort to implement Superpave technology in 1996. During this period, eight projects were changed from the traditional Marshall mix designs to Superpave. Although the new procedure offers potential for improved pavement performance, there has been very little experience nationally with its field application. Florida’s early experiences with the field implementation of Superpave are documented.

Background: Atazanavir (ATV) is a newly approved protease inhibitor following successful clinical trials in naive and treatment-experienced patients. We describe early experience with ATV in treatment-experienced patients attending a single ambulatory care clinic in Sydney. Methods: Patients commencing ATV between February 2003 and May 2004 in an expanded access program were identified from the clinic pharmacy’s database. Data were retrospectively collected from patients’ medical records. Results: Data from 30 patients were analysed. Reasons for commencing ATV were: virological failure in six patients (20%); toxicity to previous regimen in 13 patients (43%); simplification strategy in two patients (7%); and recommencing therapy in nine patients (30%) following treatment interruption. Six patients (20%) discontinued ATV. One patient discontinued ATV due to virological failure, two patients discontinued due to toxicity to concomitant antiretrovirals, two as a result of the patient’s choice and one as a result of the physician’s decision. Eighteen patients commenced ATV in combination therapy with a detectable viral load (VL). From a baseline VL of 4.3 ± 1.1 log10 copies mL–1, 15 (83%) had >1.0 log decrease in VL with 11 (61%) achieving viral suppression (<50 copies mL–1). Three (16%) failures were recorded in this group. Twelve subjects commenced ATV with an undetectable VL. One failure was recorded in this group. Bilirubin increased by 22.7 μmol L–1 (P < 0.001), with significant decreases in cholesterol (1.4 mmol L–1, P = 0.01) and triglycerides (1.5 mmol L–1, P = 0.01) in 12 patients on ritonavir-boosted ATV. Conclusion: This audit found ATV to be safe, well tolerated and had good potency in treatment-experienced patients. However caution should be exercised in switching to ATV in heavily pre-treated patients.

Intraoperative autotransfusion of whole blood was performed using the Solcotrans device in 45 patients undergoing major vascular surgery or other procedures in which there was a clean operative field. Overall, 60% of the blood shed during these operations was reinfused. Approximately 50% of patients avoided the potential complications of homologous transfusion. There were no complications attributable to the use of this device although in a sample of 13 patients, 30% of the erythrocytes were haemolysed and there were other minor haematological abnormalities. In conclusion this device provides a simple method of autotransfusing 2 or 3 litres of blood aspirated from an uncontaminated operative field.

Objectives: To present our initial experience with laparoscopic pyeloplasty and to evaluate the safety and short-term outcome of this technique.Methods: Five patients underwent laparoscopic dismembered pyeloplasty for the management of ureteropelvic junction obstruction (UPJO) at Chittagong between July’2007 to Mach’2009. Patient age at surgery was 8–22 years. There were two boys and three girls. All had unilateral UPJO with a normal contralateral kidney. We used 5 mm instruments for grasping, blunt dissection, incising and suturing to facilitate safe and precise surgery. The outcome was measured by the operative time, perioperative complications and resolution of obstruction and symptoms.Results: Mean operative time was 195 minutes (range 175–220 min). No major perioperative complications occurred in any cases. Overall, successful resolution of UPJO was observed in all the five cases evident by renogram.Conclusions: Laparoscopic pyeloplasty represents a safe and effective option in the surgical treatment of UPJO.DOI: http://dx.doi.org/10.3329/jpsb.v1i1.19448

Australia in 1902 was a fledgling colony in its second year of Federation with a population of around 3.7 million. European settlement had been largely confined to the coastal margins of this enormous land mass, although some bold adventurers in search of gold and farmland had struggled their way into the interior. Horsham, situated 300 km northwest of Melbourne in the state of Victoria, was founded in June 1849. By 1902 the town, with a population of around 2500, had grown to boast a hospital, two doctors, a pharmacist and a dentist. It was at the Horsham Hospital on January 7, 1902 that Dr Robert Ritchie performed Australia's first recorded spinal anaesthetic. Ritchie performed a lumbar puncture at the L3–4 level, injected 2 ml of 2% cocaine solution and waited for a total of 20 minutes before realising that the sensation the patient was feeling when he pinched him was pressure, not pain. The 78-year-old man with a gangrenous right leg, prostatic obstruction and congestive cardiac failure was laid supine, and had his right leg amputated through the thigh while being administered brandy and water. Strychnine injections were administered four hourly postoperatively. The adoption of the technique of spinal anaesthesia spread quickly in Australia despite communication difficulties at that time.

This paper attempts to describe links between the transactional or dialectical and the mechanistic models of development and to provide a rationale for why both types are useful. Examples of data sets, deriving principally from the early experience literature, are provided which conform to both, and it is concluded that the applicability of the model for describing behavioral development is strongly influenced by where the individual stands on three empirical dimensions: (1) the relative power of the environments that the individual is exposed to; (2) the plasticity of the individual's behavior; and (3) the deviation of the individual from developmental norms. These factors crucially affect the degree of reciprocity found in organism-environment interactions, and the results suggest that differences between developmental models are reconcilable.

Abstract:
Josefina Fierro de Bright served as a political and social activist in the 1930s and 1940s through her participation in the Mexican Defense Committee, El Congreso (the National Congress of Spanish-Speaking Peoples), and the Sleepy Lagoon Defense Committee, as well as her important efforts to end the violent attacks on ethnic Mexicans in Los Angeles during the Zoot Suit Riots. Fierro participated in organizations focused on human, civil, women’s, and labor rights. She contributed to a cross-cultural “politics of opposition” determined to create a world where true equality might flourish. She used American nationalist and transnationalist approaches. In the United States, Fierro networked with activists, celebrities, and political leaders who supported many of the same causes that she did. Her transnational approach materialized in the form of collaboration with the Mexican consulate, which also sought to secure the human rights of ethnic Mexicans living in the United States during a time of strong anti-Mexican sentiment. In order to understand why and how Fierro emerged as a leader willing to challenge the racism undergirding the segregation and mistreatment of ethnic Mexicans in California in the 1930s and 1940s, this study examines her family’s history of social activism, the fluid sociocultural environment of an American Left in which women played central roles, and her bold and charismatic leadership style.
