Abstract:
Carcinoma of the external auditory canal (EAC) is known to be very rare with a rate of 1-6 per million people. It affects mostly people over the age of 50 regardless of gender. The most common type of carcinoma of EAC is squamous cell carcinoma (SCC). The aim of this paper is to present the case of a 56-year-old woman with carcinoma of EAC. She was admitted to otolaryngology clinic due to ear buzzing lasting for five months, periodic otorrhea and hearing deterioration in her right ear. In otoscopic examination a tumor in the right ear meatus was identified. It filled almost the entire canal. Biopsy confirmed the presence of squamous-cell cancer. Tumor advancement was diagnosed using computer tomography of the temporal bones, magnetic resonance imaging of the head and ultrasonography of the neck. Imaging examinations indicated T3 N0 stage. Doctors decided to perform lateral petrosectomy with elective neck dissection. During the operation, the results of histopathological examination of frozen specimens from marginal tissues revealed an invasion of the anterior wall of the ear canal, capsule of temporomandibular joint and parotid gland. The resection eventually included also parotid gland and temporo-mandibular joint with the caput of the mandible. After recuperation, the patient had complementary radiotherapy. The presented case shows that precise imaging is necessary before surgical treatment, but unfortunately the invasion of cancer may not be estimated properly. Based on literature, the best prognosis concerns wide resection with complementary radiotherapy, although the schedule of treatment is not determined.

Abstract:
Tinnitus produced by repetitive contraction of the middle-ear muscles is a rare condition. We present an interesting case of bilateral middle-ear myoclonus causing incapacitating tinnitus in a patient with multiple sclerosis. Otological examination demonstrated rhythmic involuntary movement of the tympanic membrane. These movements correlated with a rhythmic ‘rushing wind’ noise perceived by the patient. Oropharyngeal examination showed no evidence of palatal myoclonus. Impedance audiometry confirmed rhythmic change in the middle-ear volume. Medical management was unsuccessful. The patient’s tinnitus was subsequently cured with bilateral sectioning of the tensor tympani and stapedial tendons.

Abstract:
A two years old male cat domestic long hair was brought to the Tabby Pet Care clinic with the owner's complaint that the cat often scratches his right and left ears. The results of clinical examination showed scratching marks on the pinna, blackish yellow, wet, and smelly ear serumen on the ear canal. Investigations include microscopic examination of ear swabs and cerumen swab cytology. Ear swab examination results observed the presence of Otodectes cynotis and Otodectes cynotis eggs. The results of cytological examination of cerumen swabs using the rapid stain method were observed to have bacterial infections in the form of bacilli, coccus, and the presence of neutrophil infiltration. Based on clinical symptoms and the results of investigations, the patient was diagnosed with external otitis with a fausta prognosis. Treatment of otitis externa is carried out with topical therapy Ilium® ear drop. Therapy is carried out for seven days. On the eighth day the patient was declared cured and allowed to go home.

Abstract:
Otoacariasis is the ear canal infestation by the arthropod Subclass: Acari, usually ticks or mites. It is a common complaint seen in animals. However, there are relatively few documented cases of human otoacariasis. In August 2019, a physically healthy 53-year-old Cypriot man visited Near East University hospital complaining of a blockage, partial deafness, and minor pruritis in his left ear. During the examination of the patient, a mite infestation of his left external auditory canal (EAC) was determined. The mites were removed from the EAC and sent to the laboratory for identification. The mite samples were viewed under Olympus-CX31 light microscopy and identified as Dermatophagoides pteronyssinus. The patient was treated with Permethrin (cream), Xylocaine spray, dilute acetic acid (vinegar), and three ear canal aspirations. After three days, the patient’s ear was re-examined, and the number of mites had greatly decreased within the EAC. Follow up consultations confirmed the eradication of the mite infestation. This is the first case report of human otoacariasis from Cyprus.

ABSTRACT A 32-year-old female presented with a history of otorrhea and hearing loss in her right ear since 2 years. On examination, external auditory canal was found to be filled with polypoidal tissue along with mucopurulent discharge. Audiometry showed mixed hearing loss of 50, 55, and 60 dB with air-bone gap of 30, 35, and 40 dB at frequencies of 0.5, 1, and 2 kHz respectively. She was operated for the same, and biopsy was sent for histopathological examination, which showed granulomatous inflammation. Abstract:
We report two cases of carcinoma in situ in the external auditory canal (EAC), presenting with symptoms such as pain, long-term itching of the ear, easy contact bleeding, canal otorrhea and hearing loss. Otoscopic examination revealed granulation tissue and a greyish-black tumour with irregular surface. The first patient had previously been diagnosed with otitis externa with persistent ear itching for the past three years. The second patient had received tympanoplasty for treatment of chronic otitis media on the right ear ten years ago. The first case was treated with wide excision, whereas the second patient received resection of the skin of the EAC together with its adjacent soft tissue, followed by skin grafting. No tumour recurrence was noted in the fourth and third post-operative year for the first and second patient respectively. We suggest that EAC carcinoma can be detected early and treated.

An 11-year-old, 18-kg, neutered male standard schnauzer was presented for evaluation of recurrent otitis externa with para-aural swelling and fistulation of the right external ear canal of 6 months’ duration. Otoscopic examination was impossible because of the severe stenosis of the ear canal. Right para-aural ultrasound examination and ultrasound-guided fine-needle aspiration of a mass-like lesion were performed. Cytology was suggestive of a follicular cyst. Magnetic resonance imaging revealed severe ear canal stenosis with a heterogeneous mass in the horizontal portion of the ear canal and associated otitis media. Total ear canal ablation with lateral bulla osteotomy was performed. Histopathological diagnosis was chronic otitis externa associated with multiple follicular cysts confined to the ear canal. Surgical treatment proved curative. This is the first report of multiple follicular cysts originating from the ear canal in a dog.

Abstract Background During audiologic evaluations, an examination of interaural asymmetry is commonly evaluated. To date, however, interaural differences for tympanometric indices have not been reported for adults. Without documented tympanometric right-left differences, one cannot determine if asymmetries are normal or are indicative of significant clinical asymmetries. Purpose The purpose of this study was to investigate interaural asymmetries in peak compensated static acoustic admittance (Ytm), equivalent ear canal volume (Vea), tympanometric peak pressure (TPP), and tympanometric width (TW) in normal adults. Research Design Descriptive, correlational, and inferential measures designs were employed. Study Sample Participants were 188 otologically normal Caucasian young adults (i.e., 77 females and 111 males; M = 23.0 years, standard deviation [SD] = 2.7, range = 18–30 years). Data Collection and Analyses Ytm, Vea, TPP, and TW measures were obtained bilaterally from an admittance tympanogram using a 226 Hz probe tone. Separate two-factor mixed measures analysis of variance examined the effect of ear and sex for each tympanometric index. Correlation analyses examined the association between right and left ear indices. Interaural differences in tympanometric indices were examined with independent t-tests. Critical differences, for deciding if two tympanometric indices are different between ears, were computed from the standard deviations of the right–left ear difference for statistical confidence levels of 85, 90, 95, and 99%. Results The effect of ear was not statistically significant (p > 0.05) on any tympanometric index. Females had significantly lower Ytm and smaller Vea measures than males (p < 0.05). Correlations between all right and left tympanometric indices were positive and statistically significant (p < 0.05). There were no statistically significant differences in interaural differences for any of the tympanometric indices as a function of sex (p > 0.05). Critical differences, for confidence levels for 85% to 99% confidence ranged from ± 0.20–0.36 mmhos for Ytm, ± 0.23–0.41 cm3 for Vea, ± 11.1–19.8 daPa for TPP, and ± 27.2–48.7 daPa for TW. Conclusion A clinician can use these critical differences to determine if tympanometric index interaural differences in ears of young normal Caucasian adults are statistically significant.

Simultaneous involvement of lungs, larynx and middle ear by tuberculosis is very rare. We present a rare case of simultaneous involvement of larynx and ear in a patient with pulmonary tuberculosis. A 17 years old male presented with fever and cough for 8 months, bilateral deafness for 5 months and hoarseness of voice for 3 months. The patient gave history of anorexia and progressive weight loss and history of close contact with tuberculosis patient. There were coarse crepitations on auscultation of both lungs. Indirect laryngoscopy revealed diffuse nodular swelling as well as thickening of the vocal cords. Examination of both ears revealed pale-yellow granulation tissue. ESR was 94 mm in 1st hour and chest radiograph showed diffuse patchy opacities extending over both lung fields. The results of two sputum examinations showed numerous acid-fast bacilli Histopathologic examination of punch biopsy from right and left ear lesions and biopsy tissue from laryngeal lesion taken by endoscopic instrument revealed granulomatous inflammation histologically consistent with tuberculosis.The patient responded well and promptly to the antituberculous therapy. The aim of this article is to create an awareness of Ear Nose Throat tuberculosis and to consider tuberculosis in the differential diagnosis of ear and laryngeal diseases and to emphasize the need for prompt recognition and treatment. Canine otoacariasis, or otodectic mange, is a common parasitic disorder of dogs' ear canals caused by the mite Otodectes cynotis. Infestation can be detected through diverse protocols of varying sensitivity. We evaluated the use of video otoscopy in comparison with conventional otoscopy and cerumen examination under a microscope for diagnosingO. cynotis in dogs. Thirty-five dogs were evaluated bilaterally for the presence of ear mites, using a veterinary otoscope (Gowlands®), a video otoscope (Welch Allyn®) and the gold-standard technique of examination of swab-collected cerumen under a microscope. Each ear was considered to represent one sample, and 69 ears were examined, since one dog presented with one completely stenotic ear canal. Ear mites were diagnosed in 59.42% (41/69) through video otoscopy. The same 41 infested ear canals were detected by means of cerumen examination under a microscope, whereas conventional otoscopy was able to diagnose mites in only 39.13% (27/69). This difference was statistically significant (p < 0.001). Video otoscopy proved to be superior to conventional otoscopy, and equivalent to the gold standard for detection of O. cynotis in canine ear canals, and should be recommended for controlled trials on drug efficacy for treatment of canine otoacariasis.

Objectives: The Ear, Nose, and Throat Department of Samoa is developing a school outreach service, and the aim of this study is to determine the pattern of ear disease among students attending a school for people with disabilities in Samoa. Methods: The study design is a prospective study of a 2-day outreach visit to a school for people with disabilities by the Samoan Ear, Nose, and Throat team who recorded their clinical findings (N = 28). Two Ear, Nose, and Throat Specialists performed ear examinations for each student and recorded their diagnosis and recommendations as per routine Ear, Nose, and Throat Clinic protocols on a purposefully designed form based on the World Health Organization Ear and Hearing Survey. This information was entered into an Excel spreadsheet for descriptive statistical analysis. Results: Results showed that 39% (n = 11) of students passed their initial ear examination, while 61% (n = 17) of students presented with at least one ear condition requiring Ear, Nose, and Throat intervention. The most common pathology was impacted wax (n = 15 ears). Following the Ear, Nose, and Throat Clinic outreach visit, 21 students (75%) no longer required Ear, Nose, and Throat or medical services, five students (18%) required a medical prescription for the treatment of ear disease, and two students (7%) were referred to the Ear, Nose, and Throat Department for further review. Conclusion: Students attending schools for people with disabilities are a priority population for the Ear, Nose, and Throat Clinical service outreach programme.

ABSTRAK Kesehatan telinga yang tidak dijaga dengan baik, dapat menyebabkan gangguan pendengaran. Gangguan pendengaran pada anak dapat menyebabkan sulit berkomunikasi dan menerima pelajaran sehingga prestasi menurun. Kegiatan ini bertujuan memberikan pengetahuan mengenai menjaga kesehatan telinga dan melakukan pemeriksaan telinga. Kegiatan dilakukan di Panti Asuhan Baiturrahmah yang berlokasi di Desa Rimbo Panjang, Kecamatan Tambang, Kabupaten Kampar, Riau. Kegiatan diawali dengan memberikan penyuluhan mengenai menjaga kesehatan telinga dilanjutkan dengan pemeriksaan telinga. Kegiatan diikuti oleh 42 orang anak. Sebelum dan sesudah penyuluhan diberikan pre dan post test dengan hasil rata-rata 46,67 dan 70,48, terdapat peningkatan pengetahuan yang signifikan pada anak mengenai kesehatan telinga. Dari hasil pemeriksaan didapatkan 15 orang anak dengan serumen baik pada satu atau kedua telinga diantaranya 3 orang berupa serumen prop, dan 1 orang terdapat benda asing semut di liang telinga. Melalui kegiatan ini terdapat peningkatan pengetahuan dari anak-anak penghuni panti asuhan mengenai cara menjaga kesehatan telinga dan memiliki telinga yang lebih bersih dan sehat setelah dilakukan pemeriksaan. Kata kunci: panti asuhan, pendengaran, serumen, telinga ABSTRACT Ear health if not maintained properly, can cause hearing loss. Hearing loss in children can cause difficulties in communicating and receiving lessons so that achievement decrease. The aims of this activity was to provide knowledge about ear health and conduct ear examinations. The activity was carried out at the Baiturrahmah Orphanage located in Rimbo Panjang Village, Tambang District, Kampar Regency, Riau. The activity began with providing counseling on how to maintain ear health followed by ear examination. This activity was attended by 42 children living in the orphanage. Before and after being given education we provided pre and post test with the average result 46.67 and 70.48, there was a significant increase in children's knowledge about ear health. From the results of the examination, it was found that 15 children had cerumen in one or both ears, including 3 in the form of cerumen prop, and 1 person with a foreign body ants in the ear canal. Through this activity there was an increase in the knowledge of the children living in the orphanage about how to maintain healthy ears and have cleaner and healthier ears after the examination. AbstractMiddle ear actinomycosis is an atypical and exceedingly rare anaerobic infection. The authors report a case of a 7-year-old girl with persistent right side otalgia, conductive hearing loss, and headache. Otoscopic findings included a thickened, intact, and bulging tympanic membrane. Computed tomography imaging revealed soft tissue density filling the middle ear with areas of bone erosion. Typical sulfur granules were found on surgical exploration. Actinomycosis was diagnosed by histopathological examination. Penicillin was prescribed for 5 weeks followed by oral amoxicillin for 6 months. Recovery was uneventful and with a 2-year follow-up, no recurrence or complications were observed.

Otalgia, or ear pain, is a common presentation in general practice. It can be broadly divided into primary and secondary otalgia. Primary otalgia is pain that arises from pathology of the external, middle and inner ear. Secondary otalgia, or referred otalgia, is pain that arises from pathology outside the ear. There are many causes of secondary otalgia, due to the complex sensory innervation of the ear. History and physical examination are vital in determining the aetiology. This clinical review discusses the possible causes of ear pain, and provides a systematic approach to assessment and management, including when to refer to specialists.

Abstract:
Diseases of ear, nose, and throat (ENT) are common diseases in the world. The diseases are essential due to morbidities, which affect the physiological functions in the head and neck region. Cerumen impaction is the most common finding disease of ENT in children. The community service was done to find the distribution of ear, nose, and throat examination results of students in Letjen Jamin Ginting Junior High School, Berastagi, especially cerumen impaction which then were treated with cerumen removal. The community service also aimed to increase student's knowledge about ENT disease. The ENT examination was done to 188 students. The students with cerumen impaction were treated with cerumen removal. The students also educated about ENT disease to improve knowledge about ENT disease. Among 188 students, the number of male students was more than female with 98 students (52.1%), where 76 students (40.4%) were 14 years old. 150 students (79.8%) did not have any complaint about their ear, nose, and throat. Ear fullness was a common chief complaint from the students with 15 students (8.0%). Ear disease was a frequent disease with 44 students (47.3%) and cerumen impaction as the most common findings with 43 students (22.9%). In ENT disease education, the evaluation was done with the highest result was get by 8 students with 80% corrected answers. Ear disease had high prevalence with cerumen impaction as the most common findings of ENT examination.

<p class="abstract">The malignancies of tympanomastoid region are very rare entity. These tumours are more common in elderly persons in 6th and 7th decade. Males are more commonly affected than females. Chronically discharging ears are considered as risk factor which may be due to metaplasia in the middle ear mucosa following prolonged chronic infection. The most common symptoms are long standing blood tinged ear discharge, severe nocturnal pain, rapidly growing polypoidal or granulomatous mass in EAC or middle ear, peripheral facial palsy and painless ulceration over pinna or EAC. CT scan for bony erosion and MRI for soft tissue involvement and neural invasion are investigations of choice. Tissue biopsy is must for histopathological examination and confirmation of diagnosis. Surgery and chemo-radiotherapy are the mainstay of treatment. Surgery includes wide local excision, lateral temporal bone resection, subtotal temporal bone resection and total temporal bone resection.</p><p class="abstract"> </p>

We describe a patient with nasopharyngeal lymphoma who was admitted for chemotherapy. At admission, otoscopic examination revealed a large polyp occluding the left external ear canal. The polyp was removed, and histopathologic examination showed the same tumor that was found previously in the nasopharynx. Magnetic resonance imaging showed an infiltrative process that involved the left side of the nasopharynx and extended toward the left middle and external ear. To our knowledge, this is the first case report of a direct extension of nasopharyngeal lymphoma into the middle and external ear.

They are usually confused with exostosis which are multiple, bilateral, smooth-bordered, broad-based lesions without deep extension. Extrapulmonary small cell carcinoma (EPSCC) affecting the external auditory canal (EAC) is uncommon. We herein report a case involving a 56-year-old man with EPSCC of the EAC who had a 48-year history of recurrent purulent discharge in both ears and a 20-day history of right ear pain and hemorrhage followed by incomplete right eyelid closure and an askew mouth. He underwent surgical removal of middle ear granulation tissue, residual ossicles, and a right EAC mass. Postoperatively, pathomorphological examination combined with immunohistochemical staining supported a diagnosis of small cell carcinoma. Radiation therapy at a dose of 60.06 Gy in 33 daily fractions was completed 1 month after surgery, and synchronous etoposide–cisplatin regimen chemotherapy was performed for two cycles and four sequential cycles. One year postoperatively, magnetic resonance imaging showed no tumor in the ear; however, computed tomography showed multiple liver space-occupying lesions that were considered to indicate liver metastasis. Further chemotherapy was performed, but the patient died 15 months postoperatively. This case indicates that timely and accurate chemoradiotherapy is likely the most reasonable approach to EPSCC of the EAC given the aggressiveness of this tumor.

Introduction. Tuberculous otitis is a diagnostic problem due to the difficulty to obtain microbiological, histomorphological and cytological confirmation of the disease. Objective. Our objective was to compare clinical and radiological characteristic and development of otogenic complications in patients with tuberculous otitis and otitis with cholesteatoma as the most destructive form of chronic nonspecific otitis in the purpose of establishing the diagnostic criteria for tuberculous otitis. Methods. Medical records of 12 patients with tuberculous otitis and 163 patients with cholesteatoma treated at the Institute of Otorhinolaryngology and Maxillofacial Surgery in Belgrade during the eight-year period were analyzed. All of the patients underwent otomicroscopic, audiological and radiological examination of the thorax and temporal bone, microbiological examination of the secretion and histomorphological examination of the tissue taken during middle ear surgery. Statistical analysis was done using ?2 test with Yates correction. Results. Otogenic complication as facial palsy and sensorineural hearing loss were more frequent in tuberculous otitis patients, than in cholesteatoma. Also, fistulas of the labyrinth and facial canal bone destruction were also more frequent in tuberculous otitis than in cholesteatoma. A larger extent of temporal bone destruction was noticed on CT scans of the temporal bone in half of the patents with tuberculous otitis. Coexistence with miliary pulmonary tuberculosis was detected in one third of the patients. There were no microbiological or histomorphological confirmations of the disease, except in one case with positive Ziehl-Neelsen staining. Conclusion. Tuberculous otitis media should be considered in patients with serious otogenic complications and with shorter duration of ear discharge, and in association with diagnosed miliary pulmonary tuberculosis and extensive temporal bone destruction. Polymerase chain reaction still is not reliable for diagnosis.

Concurrent otitis media, based on radiographic findings, was diagnosed in all three dogs. Disruptions of the external ear canals were confirmed at surgery, and the dogs each responded to total ear canal ablation and lateral bulla osteotomy.

The high number of patients who come for ear therapy with massage skills, the P3T Center intends to carry out an observational screening of ear therapy. This screening aims to find out how the diagnostic method and application of ear therapy with massage skills and the benefits of ear therapy to overcome the problem of hearing loss (hearing impairment). This screening design uses an observational method in which the researcher makes observations and interviews with respondents. Data collection techniques were carried out by looking at the results of the examination of respondents' hearing tests and the results of interviews with respondents. Patient therapy results in 18 people (60%) no progress or the same as the original condition. this shows that therapy can be said to be ineffective in curing ear disorders. The results of the audiometry examination of the right and left ear stated that none of the patients was normal. Even though there are patients who claim to be able to hear loud sounds in the questionnaire, it is not supported by audiometry results. So it can be concluded that ear therapy with massage skills has no impact on patients suffering from hearing loss.

The aim of this study was to develop an examination protocol for magnetic resonance imaging, in order to display diagnostically important information of the canine middle and inner ear. To ensure that this protocol could also be used as a basis for determining pathological changes, the anatomical structures of the ear were presented in detail. To minimise stress through anaesthesia in live animals, preliminary examinations were carried out on four dog cadavers. During these initial examinations, three-dimensional (3D) sequences proved to be superior to two-dimensional ones. Therefore, only 3D sequences were applied for the main examinations performed on six clinically healthy Beagles. The anonymised MR images were rated by three experienced reviewers using a five-point scale. The most valuable sequence was a T2-weighted CISS sequence (TR = 16.7 ms, TE = 8.08 ms). This sequence proved to be most suitable for illustrating the inner ear structures and enabled good tissue contrasts. The sequence ranked second best was also a T2-weighted DESS sequence (TR = 19 ms, TE = 6 ms), allowing the imaging of the tympanic cavity and enabling 3D reconstruction due to its isotropic voxels. Due to low contrast and strong noise, the other sequences (TSE, FISP, MP RAGE) were not suitable for anatomical illustration of the middle and inner ear.

Chronic suppurative otitis media (CSOM), according to statistics, is one of the most common diseases in otorhinolaryngological practice, accounting for, on average, from 5.8 to 20.7% of all ENT pathology. The incidence of CSOM in the Republic of Uzbekistan is up to 6.5%. The pathological process in CSOM leads to the destruction of the bone structures of the middle ear and progressive hearing loss, and in some cases - to an increase in vestibular symptoms. Purpose of the study: vestibular disorders in chronic purulent otitis media using modern diagnostic methods. Materials and methods: In order to study the features of cochleovestibular disorders in CSOM, in the Department of Congenital and Acquired ENT Diseases at the Republican Specialized Scientific and Practical Medical Center of Pediatrics of the Ministry of Health of the Republic of Uzbekistan were examined 13 patients with CSOM with labial complications . All patients underwent a set of standard examinations: ENT examination, otoscopy with video recording, stato coordination tests, complex vestibulometry using modern computer video nystagmography, a set of audiometric tests, computed tomography of the temporal bones. Results: 38.4% of patients complained of dizziness, but when using the method of active survey complaints of dizziness were detected in 61.5% of cases. The analysis of the results of stato-coordination tests in 30.7% of patients revealed a deviation towards the affected ear, in 61.5% of cases - spontaneous nystagmus. Conclusions: the results allow us to conclude that the main complaint of patients with CSOM are of hearing loss (100%), discharge from the ear cavity (100%), complaints of vestibular manifestations were present in 30 to 55% of subjects. Detection of mixed deafness in 53.8% of patients during audiological examination indicates involvement in the process of the inner ear, hence all patients with CSOM, regardless of the duration and stage of the inflammatory process, should conduct a comprehensive vestibulometric examination to determine vestibular status, the degree of involvement of inner ear structures , prognosis, choice of treatment tactics and prevention of complications.

Intracranial subdural empyema (ISE) is an uncommon condition previously associated with almost 100% morbidity and mortality. Since the introduction of antibiotics and advancements in diagnosis the complication rates have significantly improved. We report an unusual case of a 32-year-old Aboriginal male diagnosed with ISE. On closer inspection the ISE was found to be a complication of otitis media with a cotton bud lodged in the external acoustic meatus. The report provides a literature review on the relationships of ISE, otitis media and foreign bodies. We conclude that although rare, all patients with suspected ISE should undergo an ear examination as it is at no cost to the patient or health service but may be the difference between life and death.

To present a case referred to our clinic with severe right ear pain but without any abnormal finding during otological examination and diagnosed as myocardial infarction and also to draw attention to otalgia which can occur secondary to myocardial infarction.Case Report. An 87-year-old female admitted with right ear pain lasting for nearly 12 hours and sweating on the head and neck region. On otolaryngologic examination, any pathological finding was not encountered. Her electrocardiogram revealed findings consistent with myocardial infarction. Her troponin values were 0.175 ng/L at 1 hour, and 0.574 ng/L at 3 hours. The patient was diagnosed as non-ST MI, and her required initial therapies were performed. On cardiac angiography, very severe coronary artery stenosis was detected, and surgical treatment was recommended for the patient. The patient who rejected surgical treatment was discharged with prescription of medical treatment.Conclusion. Especially in elderly patients with complaints of ear pain but without any abnormal finding on otoscopic examination, cardiac pathologies should be conceived.

Giant cell tumor (GCT) is classified as a benign bone tumor, and it is frequently identified at the epiphysis of long bones and relatively rare in the temporal bone. For orthopedists expert at recognizing bone and soft tissue tumors, the diagnosis of GCT is relatively easy; however, since head and neck surgeons experience few cases of GCT, it may be difficult to diagnose when it occurs in the temporal bone. A 32-year-old man complained of left hearing loss, aural fullness, and tinnitus. Examination of the ear revealed a bulging tumor. Audiologic examination demonstrated conductive hearing loss of the left ear. Computer tomograph of the temporal bone showed a soft-tissue-density specification indicating bone destruction at the left temporal bone. The tumor invaded the skull base. Imaging examinations using magnetic resonance imaging revealed a nonhomogenous isosignal intensity area on T1 at the left temporal bone. After intravenous gadolinium, the mass showed unequal enhancement. This patient subsequently underwent surgery to remove the lesion using transmastoid and middle fossa approach. Pathological examinations from specimens of the tumor revealed characteristic of GCT. No clinical or radiological evidence of tumor recurrence was detected for 4 years.
