Abstract:
This article is a validation study of two national large-scale tests that measure the language proficiency of 11/12 year-old English learners in Norway and Slovenia. Following the example of Alderson and Banerjee (2008), the authors of the article have employed the EALTA guidelines for good practice to validate the tests, and to formulate major recommendationsfor improvement of both assessment instruments, where feasible (Alderson & Banerjee, 2008). The results of the validation study show that both national tests in English seem to fulfil most of the EALTA guidelines for good practice, although a few issues related to the testconstruct and test design procedures need to be re-assessed, and some changes may be required.

ABSTRACT Introduction: We applied three-dimensional gait analysis to assess the effects of enhanced active contraction of the transversus abdominis (EACTA) during walking. We sought to evaluate the effect of EACTA during walking in order to improve walking quality. Methods: Thirty college students were recruited and trained to perform EACTA during walking. We examined gait parameters under different conditions, including EACTA and habitual ACTA (natural walking with mild contraction of the feedforward mechanism of ACTA, HACTA) during walking using three-dimensional gait analysis. We compared differences in gait parameters under the two walking conditions using SPSS 16.0 statistical software. Results: The following gait parameters were significantly lower under EACTA conditions than under HACTA conditions (P < 0.05): stance phase, 59.151% ± 1.903% vs. 59.825% ± 1.495%; stride time, 1.104 s ± 0.080 s vs. 1.134 s ± 0.073 s:; stance time, 0.656 s ± 0.057 s vs. 0.678 s ± 0.053 s; and swing time, 0.447 s ± 0.028 s vs. 0.454 s ± 0.031 s, respectively. Gait parameters single support phase and mean velocity were significantly higher for EACTA than for HACTA conditions (both P < 0.05). Conclusions: Overall, the results revealed that EACTA during walking can improve gait. This method is simple, and EACTA training during walking to improve gait quality in daily life could provide a positive basis for people to strengthen the transverse abdominal muscle. ABSTRACT Enterotoxigenic Escherichia coli (ETEC) strains remain a formidable cause of diarrheal disease. To identify novel surface proteins of ETEC, we performed TnphoA mutagenesis of prototype ETEC strain H10407 and discovered a secreted protein not previously recognized in ETEC. DNA sequencing of the interrupted locus in mutant TnphoA.977 revealed a candidate 4,095-bp open reading frame without significant homology to commensal E. coli K-12 genomic DNA. Translation of this sequence revealed that it encoded a predicted peptide of 147.7 kDa that bears significant homology to members of the autotransporter family of bacterial virulence factors, particularly the serine protease autotransporters of the Enterobacteriaceae proteins. The gene identified in H10407, eatA (ETEC autotransporter A), encodes a potential serine protease motif (GDSGSP) in the secreted amino-terminal domain, and the predicted peptide shows more than 80% homology with SepA, a virulence protein secreted by Shigella flexneri. DNA hybridization and PCR demonstrated that eatA resides on the 92-kDa pCS1 virulence plasmid of H10407 and that it is present in multiple clinical ETEC strains. Immunoblots with antisera directed against a recombinant EatA passenger protein fragment identified a 110-kDa protein in supernatants purified from H10407 but not from the TnphoA.977 mutant or H10407-P, which lacks pCS1. EatA possesses serine protease activity that is abolished by mutations within a serine protease catalytic triad formed by residues H134, D162, and S267. Finally, interruption of the eatA gene retarded fluid accumulation in the rabbit ileal loop model, suggesting that this autotransporter contributes to the virulence of ETEC.

Abstract:
Health, Economics and health organization have the largest number of publications at all levels of government. However, direct measurement and dosed funding for each remains out of reach for the majority of the population. For them also remains unclear: what are the fundamental laws you need to follow to fulfill the doctor's orders. The movement towards monopoly narrows the health care market. Collective economic, social and urban projects do not reduce the pharmacy chain. The budget remains in surplus if mortality is not reduced. Medical statistics do not have the properties of a microscope. Therefore, for the population health is a social self-movement. Each industry has its own technology and economic costs, forming departmental lists of services and works at the expense of the state budget. The article on the Federal laws and regulations shows the relevance of updating health care to build a list of services and improve the methodology of budget planning on the basis of four health sectors R, N, Q and Z. It is established that the ratio of financing of the proposed industries (in multiplicities) is not : 10 : 100 : 1000 does not lead to the planned effects and possible efficiency evaluation. Conclusions and proposals of the article are necessary for the Concept of improving the efficiency of budget expenditures (adopted in January 2019).

Purpose A three-year research study, funded by Innovate UK, Consumer Models for Assisted Living (COMODAL) aimed to support the development of the consumer market for electronic assisted living technology (eALT) products and services, particularly for people aged 50-70, approaching older age and retirement themselves or with caring responsibilities for family or friends. The purpose of the COMODAL study was to gain a greater understanding of their needs and behaviours relating to the acquisition of eALT and develop sustainable consumer-led business models that might address these needs and support business development within a consumer market (Ward et al., 2016). The purpose of this paper is to present a follow up study to explore how the market may have changed since the publication of the research findings. Design/methodology/approach An online survey was used to collect both qualitative and quantitative data from individuals working in the supply and distribution of assisted living technologies in the UK regarding how their businesses had developed in the past two years. Findings The results showed that since the publication of the COMODAL research there have been changes in the way that the consumer market for eALT is being approached, not only with more direct marketing focused on consumer’s needs but also in direct partnerships with local authorities that offer greater choice with an improved range of products. Originality/value This is the first paper in the UK to follow up the impact of the original COMODAL research and explore its influence on the development of the consumer market for eALT.

Objective Magnetic resonance imaging (MRI) provides important information regarding tumors in the parapharyngeal space (PPS), revealing their origin, whether they are benign or malignant, and their relationships with surrounding structures. Methods Twelve tumors in the PPS were completely excised using an endoscopically assisted transoral approach (EATA). The MRI features were analyzed. Results Ten pleomorphic adenomas confirmed on postoperative pathological examination had the parotid pedicle sign. A fat space between the tumor and parotid gland may distinguish such a tumor from a tumor arising from a minor salivary gland in the prestyloid space and a tumor arising from the deep lobe of the parotid gland. Both the jugular vein and carotid artery were displaced posteriorly in all 10 cases of pleomorphic adenomas. The principal features of the two schwannomas confirmed on postoperative pathological examination were separation of the internal carotid artery and internal jugular vein and anteromedial displacement of the internal carotid artery, suggesting that the tumors originated in the poststyloid space. In this review, 95 tumors were excised by the EATA in the English-language literature. Conclusions MRI renders differential diagnosis possible. PPS tumors may be completely excised via an EATA guided by tumor features evident on preoperative MRI.

The 9th edition of the European Air Law Association (EALA) Munich Liability Seminar took place on the 19 May 2014. There were panels addressing current issues in private air law: passenger rights and the proposed revision of Regulation 261/2004; criminalization of aviation accidents and accident reporting; assessment of the Montreal Protocol 2014, amending the Tokyo Convention 1963, including the problem on unruly passengers. The seminar also included a presentation of reinsurance in the aviation sector from Munich Re. The seminar touched on space law with a panel consisting of two presentations on the topic of liability for damage caused by sub-orbital spacecraft. The seminar concluded with an in-house counsel panel of current trends in the procurement of legal services. On 19 May 2014 the Kempinski Hotel Airport Munich once again played home to the European Air Law Association (EALA) Munich Liability Seminar, this time back for its 9th edition. The seminar itself was preceded the previous night by a visit to the BMW Museum, which included a private tour of the exhibition. The following morning, the President of EALA, Prof. Dr P.M.J. Mendes de Leon, gave his welcoming address to the delegates, noting the international appeal of the seminar's programme, evidenced by the attendance of so many delegates from the United States. Dr Ehlers, reiterating the warm welcome to the delegates gave the floor to Dimitri de Bournonville, Partner at Kennedys Law Firm in Brussels, and moderator of the day's first panel.

La adopción de tecnología es un proceso complejo, que requiere analizarse de manera integral a través del tiempo. Esta investigación se realizó en 2014-2015, en dos comunidades indígenas del estado de Morelos: Barranca Honda y Cuentepec. El objetivo fue conocer el nivel de adopción y eficiencia en la cocción de alimentos con la Estufa Ahorradora de Leña “Tlecalli” (EALT). Se aplicaron 83 cuestionarios y 12 entrevistas (por hogar) complementadas con observación participante, registro fotográfico y videos. Los resultados son: un 85% a 95% continúan utilizando su EALT, con una eficiencia de más del 60% en ahorro de leña. La adopción se considera exitosa, después de más de siete años de haberse instalado, mejora el ambiente de sus hogares (libre de humo), así como su calidad y confort de vida respecto a la acción de cocinar.

ABSTRACTEnterotoxigenicEscherichia coli(ETEC) is a major cause of morbidity and mortality due to infectious diarrhea in developing countries for which there is presently no effective vaccine. A central challenge in ETEC vaccinology has been the identification of conserved surface antigens to formulate a broadly protective vaccine. Here, we demonstrate that EatA, an immunogenic secreted serine protease of ETEC, contributes to virulence by degrading MUC2, the major protein present in the small intestinal mucous layer, and that removal of this barrierin vitroaccelerates toxin access to the enterocyte surface. In addition, we demonstrate that vaccination with the recombinant secreted passenger domain of EatA (rEatAp) elicits high titers of antibody and is protective against intestinal infection with ETEC. These findings may have significant implications for development of both subunit and live-attenuated vaccines against ETEC and other enteric pathogens, includingShigella flexneri, that express similar proteins.

Studies have indicated that vitamin B6 status decreases with age. However, little is known about vitamin B6 status of elderly people in Taiwan. The purpose of this study was to assess vitamin B6 status of elderly Taiwanese and to examine the effect of protein on various indices of vitamin B6 status in the elderly. Thirty-nine men (mean age = 69.9 ± 4.2 years) and 55 women (mean age = 69.5 ± 3.9 years) completed a 24-h diet recall. The mean total vitamin B6 intake (men: 1.7 ± 0.9 g/day; women: 1.6 ± 1.2 g/day) was higher than the 1998 US Dietary Reference Intakes (DRI) and the current Taiwan Recommended Daily Nutrient Allowance (RDNA). Dietary energy and protein intakes were not related to any vitamin B6 status parameters in any sex groups and the pooled group. Vitamin B6 intake correlated only with erythrocyte aspartate transaminase activity coefficient (EAST-AC) in the pooled (r = –0.214, p < 0.05) group. There were no significant differences in plasma pyridoxal 5’-phosphate (PLP), erythrocyte alanine transaminase activity coefficient (EALT-AC), and EAST-AC between sex groups. Although elderly subjects had adequate mean plasma PLP concentrations, 59% of men and 55% of women had plasma PLP concentrations lower than a cutoff of 20 nmol/L. The mean EALT-AC was < 1.25 in two groups with adequate vitamin B6 status. However, 23% of men and 18% of women had EALT-AC values > 1.25. The mean EAST-AC value of subjects was higher than the suggested value (< 1.8) for inadequate vitamin B6 status. The incidence of biochemical vitamin B6 deficiency in our elderly is probably more relevant from other causes than from dietary intake of protein and vitamin B6.

Food security remains a major issue for developing countries. Reducing arable land abandonment (ALA) is crucial to ensuring food security. In China, the ‘decline in both quantity and quality’ of arable land resources, especially in major grain-producing areas, has become increasingly serious. This study uses fuzzy set qualitative comparative analysis (fsQCA) to explore the core conditions and combinations of paths leading to explicit and implicit abandonment using 30 typical cases in the main grain-producing areas of Hubei Province. The results show that (1) three combined pathways lead to explicit ALA (EALA) and that two pathways lead to implicit ALA (IALA); (2) laborer health (LH) is the core condition leading to EALA; and (3) LH, agricultural laborer (AL), per capita income (PCI) and social relationships (SRs) are the core conditions leading to IALA. To effectively alleviate ALA, the government should improve production conditions, pay attention to laborer health issues, improve agricultural returns and strengthen food security publicity and guidance, thereby promoting the rational use of arable land in these areas. The findings in this study link the changes in arable land use and provide a reference for other developing countries in ensuring food security.

ObjectivesEvaluation of concepts and reactions on a public online forum concerning depression.MethodsRevision of 100 consecutive entries posted using an adapted Collazzi method.ResultsComments were divided according to their content and the main subjects found were symptomatology, suicide, therapeutic advice and concerns about medication, psychotherapy and alternative therapies. The effect of a support group was found to be present based on the comments to each individual post by diminishing loneliness and deconstructing desperation. Non-therapeutic opinions were expressed and there were high levels os expressed emotion.ConclusionThe absence of a ealth professional as group moderator may lead to conflict and misleading remarks.

The vitamin B6 status of high school students and its relationship with dietary intake were investigated in this cross-sectional study by face-to-face interview. A total of 157 healthy students aged 16–18 years (eighty-three boys and seventy-four girls) were randomly recruited from two out of nineteen senior high schools in Tainan, Taiwan. Vitamin B6 intakes were calculated from three 24-h dietary records. Direct and indirect vitamin B6 status indicators were measured in plasma, erythrocytes and urine. The anthropometric data, being similar to those of the first Nutrition and Health Survey in Taiwan (1993–1996), showed the normal growth and development of these students. All students except one girl (28·7 nmol/l) had plasma pyridoxal-5′-phosphate (PLP) levels >35 nmol/l, indicating an adequate vitamin B6 status. The mean dietary vitamin B6 intakes of boys and girls were 1·04 (sd 0·29) and 0·96 (sd 0·27) mg/d, respectively. Vitamin B6 status indicators, including plasma PLP, erythrocyte alanine aminotransferase activity coefficient (EALT-AC), aspartate aminotransferase activity coefficient (EAST-AC) and urinary 4-pyridoxic acid (4-PA), were correlated with vitamin B6 intake. Students with adequate values of plasma PLP (>35 nmol/l), EALT-AC ( < 1·25), EAST-AC ( < 1·8) and urinary 4-PA (>3·0 μmol/d) had median intakes of 1·08 and 1·01 mg/d, respectively, for boys and girls. This study suggests that vitamin B6 requirements for boys and girls aged 16–18 years were approximately 1·1 and 1·0 mg/d, respectively.
