1.1 The publication last year of the English Life Table No. 14 enables my earlier paper (Daw, 1982) to be extended to include the latest table. This paper applied Redington and Michaelson's rx test to E.L.T. Nos. 8 to 13 and the corresponding figures for E.L.T. No. 14 have now been calculated. The Government Actuary's Department kindly supplied me with the unpublished exposed to risk figures which were used.

Abstract:
In analogy to the definition of the halting problem K, an r.e. set A is called a diagonal iff there is a computable numbering ψ of the class of all partial recursive functions such that A = {i ∈ ω: ψi(i)↓} (in that case we say that A is the diagonal of ψ). This notion has been introduced in [10]. It captures all r.e. sets that can be constructed by diagonalization.It was shown that any nonrecursive r.e. T-degree contains a diagonal, that for any diagonal A there is an r.e. nonrecursive nondiagonal B ≤TA, and that there are r.e. degrees a such that any r.e. set from a is a diagonal.In §2 of the present paper we show that the property “A is a diagonal” is elementary lattice theoretic (e.l.t.). This result complements and generalizes previous results of Harrington and Lachlan, respectively. Harrington (see [16, XV. 1]) proved that the property of being the diagonal of some Gödelnumbering (i.e., of being creative) is e.l.t., and Lachlan [12] proved that the property of being a simple diagonal (i.e., of being simple and not hh-simple [10]) is e.l.t.In §§3 and 4 we study the position of diagonals and nondiagonals inside the lattice of r.e. sets. We concentrate on an important class of nondiagonals, generalizing the maximal and hemimaximal sets: the -maximal sets. Using the results from [6] we are able to classify the -maximal sets that can be obtained as halfs of splittings of hh-simple sets.

Abstract:
The most basic construction of an r.e. nonrecursive set—e.g. of the halting problem—proceeds by taking the diagonal of a recursive enumeration of all r.e. sets. We will answer the question of which r.e. sets can be constructed in this manner.If ψ is a computable numbering of some class of partial recursive functions, we define the diagonal of ψ to be the set Kψ ≔ {i ∈ ω ∣ ψi(i)↓}- It is well known that Kφ is creative if φ is a Gödelnumbering, and that for each creative set K there exists a Gödelnumbering φ such that K = Kφ. That is to say, the class of diagonals of Gödelnumberings is characterized as the class of creative sets. This class was shown to be elementary lattice theoretic (e.l.t.) by Harrington (see [So87, XV. 1.1]).We give a characterization of diagonals of arbitrary computable numberings of the class P1 of all partial recursive functions. To this end we introduce the notion of a semihyperhypersimple (shhs) set, which generalizes the notion of hyperhypersimplicity to nonsimple sets. It is shown that the diagonals of numberings of P1 are exactly the non-shhs sets. Then, properties of shhs sets are discussed. For example, for each nonrecursive r.e. set A there exists a nonrecursive shhs set B ≤TA, but not every r.e. T-degree contains a shhs set. These results build upon previous work by Downey and Stob [DSta].The question whether the property “shhs” is (elementary) lattice theoretic remains open. A positive answer would give both an analog of Harrington's result mentioned above, and a generalization of the well-known fact, due to Lachlan [La68], that hyperhypersimplicity is e.l.t. Therefore, we suspect that shhs sets turn out to be useful in the study of the lattice of r.e. sets.Previously, for several constructions from recursion theory the role of the underlying numbering of P1 was investigated; see Martin ([Ma66a] or [So87, V.4.1]) and Lachlan ([La75] or [Od89, III.9.2]) for Post's simple set, and Jockusch and Soare ([JS73]; cf. also [So87, XII.3.6, 3.7]) for Post's hypersimple set. However, only Gödelnumberings were considered. An explanation for the greater variety which arises when arbitrary numberings of P1 are admitted is provided by the fact that the index set of Gödelnumberings is less complex than the index set of all numberings of P1. The former is Σ1-complete; the latter is Π4-complete.

Abstract:
O bambu é uma excelente alternativa sustentável, de elevada resistência, com baixo peso específico e renovável. Graças a um tratamento natural eficiente descoberto recentemente, utilizando Tanino, a durabilidade conferida permite a sua utilização em várias aplicações na construção civil. O objetivo do trabalho foi obter um método para execução de tesouras mais eficientes estruturalmente e verificar a sua viabilidade prática em coberturas de edificações. Para isto, foram executadas 6 tesouras de bambu, da espécie Dendrocalamus giganteus, reforçadas com graute e abraçadeiras metálicas. As tesouras foram posicionadas em um pórtico em que uma carga concentrada foi aplicada no ponto superior da tesoura até a ruptura, gerando dados de deslocamentos e cargas últimas. Desta forma, analisou-se o comportamento de cada tesoura quando submetida à esforços. Os resultados comprovaram que os reforços, além da utilização de barras roscadas nas uniões das barras das tesouras, agregam um significativo acréscimo de resistência mecânica para a referida estrutura. Além disso, os valores obtidos das cargas e deslocamentos nos ensaios realizados permitiram concluir que as tesouras podem ser usadas com segurança em coberturas de edificações, pois atendem aos critérios do Estado Limite de Serviço (E.L.S.) e do Estado Limite Último (E.L.U.).

IntroductionUtilization of E.C.T. has diminished since the middle of 20th century but, inspite of widespread use of pharmacological agents for treatment of major psychiatric disorders, E.C.T. is still treatment of choice for patients with severe suicidal symptoms, catatonia and diseases resistant to various pharmacological agents. But inspite of its excellent effect in various psychiatric disorders, acceptance of E.C.T. is very poor in society.ObjectiveObjective of this study was to know the attitude of attendants for E.C.T. and acceptance after detailed description of procedure and discussing merits and demerits of E.C.T.AimsAs still it is the best treatment for certain psychiatric disorders, so to increase the awareness about its benefits and try to remove the negativity regarding E.C.T. in society.MethodIn this study, 61 patients were randomly selected who had indication for E.C.T. without any discrimination of male and female. Their attendants were categorized by their socioeconomic status. All the attendants were asked open questions regarding their opinion for E.C.T. before taking consent.ResultSixty patients were selected who had indication for E.C.T. and their attendants were asked to give their opinion for E.C.T. Amongst them 9.84% were from upper socioeconomic status, 68.86% were from middle socioeconomic status and 21.31% were from lower socioeconomic status. Only 11.48% had a positive attitude, 75.40% had a negative attitude and 16.39% had no comments.ConclusionAfter developing good rapport and confidence with attendants and detailed explanation of E.C.T. procedure, its merits and demerits 75.40% had given a positive attitude, 22.95% had given negative attitude and still 1.64% had no comments.Disclosure of interestThe author has not supplied his declaration of competing interest.

This study evaluated in vitro the cytotoxicity of four root canal sealers (Topseal, EndoRez, TubliSeal and Kerr Pulp Canal Sealer E.W.T.) and their effects on reactive oxygen/nitrogen intermediate induction by mouse peritoneal macrophages. Thioglycollate-induced cells were obtained from Swiss mice by peritoneal lavage with 5 mL 10 mM phosphate-buffered saline, washed twice and resuspended (106 cells/mL) in appropriate medium for each test. Cytotoxicity was determined by the presence of hydrogen peroxide (H2O2) and nitric oxide (NO) by the peroxidase-dependent oxidation of phenol red and Griess reaction, respectively. Sealer suspensions were obtained in two different concentrations from each material: 18 mg/mL and 9 mg/mL, established according to compatibility parameters following MTT assay. Comparing the sealers, H2O2 release at concentrations of 9 mg/mL and 18 mg/mL was similar: Topseal > positive control (medium + cells + 5 mg/mL zimozan solution) > EndoRez > TubliSeal > Kerr Pulp E.W.T. > negative control (medium + cells). NO release at concentration of 9 mg/mL was: positive control (medium + cells + 10 µg/mL LPS solution) > Topseal > Kerr Pulp E.W.T. > TubliSeal = EndoRez > negative control (medium + cells); at concentration of 18 mg/mL was: positive control > Topseal > Kerr Pulp E.W.T > TubliSeal > EndoRez > negative control. Based on the results, it may be concluded that Topseal presented the highest cytotoxicity among the tested sealers, releasing higher concentrations of NO and H2O2 in macrophage culture.

AbstractFibreoptic endoscopy has only recently become used as a diagnostic tool for the E.N.T. surgeon but it has not been widely used in the investigation of pharyngeal disease. During a two-year period at Warwick Hospital 53 patients have undergone flexible fibreoptic endoscopy using a gastroscope, with visualization of the pharynx. larynx and upper G.I. tract as far as the duodenum. This report describes the technique employed, details the endoscopic findings in 53 patients, and discusses the indications for the technique and the advantages that it holds for the E.N.T. surgeon in the diagnosis and management of pharyngeal disease.

Experiments in Art and Technology (E.A.T.) is an organization co-founded in 1966 by artists Robert Rauschenberg and Robert Whitman, and engineers Billy Klüver and Fred Waldhauer, in order to support collaboration between artists and engineers. The E.A.T. datascape is a digital instrument for analyzing the digitized traces left by its members via many available resources. Its aim is to study as closely as possible the complexity of collaborative interdisciplinary works. The E.A.T. datascape methodology makes it possible, by means of an anthropological action-centred approach, to go beyond the distinction between art history and art sociology and to renew the social history of art by challenging the notion of authorship and by describing the work as constituted by the intersection between heterogeneous trajectories, rather than an object within a context that would influence it, or constitute its environment. In other words, it allows us to reflect on what digital design does, in turn, to the social history of art, and to put forward hypotheses about what a digital social history of art might be or could offer to the study of complex, interdisciplinary projects that are multiplying in the contemporary art world.

Objective: To ascertain the role of Exercise Tolerance Test as ascreening tool for coronary artery disease, in asymptomatic men who had one or more risk factors for developingischemic heart disease. Design: A descriptive study. Place and Time Period: Department of Medicine PAF HospitalMushaf Sarghoda from March 2004 to March 2005. Patients and Methods: A total no of 100 asymptomatic men age25-50 years with mean age of 38 years were included in the study. They were divided in to five groups according tonumber of risk factors they had for developing CAD. Following risk factors were included in the study (hypertension,diabetes ,smoking, obesity, hyper-lipidemia, family history of CAD.). All of them underwent E.T.T over treadmill usingmodified Bruce protocol. Test was considered positive if 2mm or more ST segment depression was noted .those testedpositive had coronary angiogram done to confirmed IHD. Results: Seven out of 100 tested positive for IHD on E.T.T.Those tested positive underwent coronary angiography 6 out of seven showed significant narrowing of coronaryarteries and 1 had normal angiogram. These results showed that E.T.T has a 76% sensitivity and 87% Specificity.Conclusion: E.T.T is an important, reliable and cost effective test to detect latent CAD in asymptomatic persons whohad multiple risk factors for developing CAD.

Se describen las actividades orientadas a la extensión de las interfaces de hardware y software de un kit de asistencia desarrollado por la corporación INTEL para que personas que sufren de alguna discapacidad motriz derivada de la E.L.A. (Esclerosis Lateral Amiotrófica) puedan operar un equipo de cómputo de forma autónoma. El objetivo de este trabajo es el de incrementar el espectro de usuarios finales de esta plataforma al partir del grupo original de usuarios con E.L.A. hacia un conjunto más general e incluir personas con niveles de discapacidad motriz de grado medio/severo derivados de trastornos físicos periféricos y/o neurológicos. Se introducen los antecedentes, descripción general de la plataforma INTEL-ACAT, objetivos y metodología a desarrollar, se definen los perfiles de los usuarios que serán anexados y finalmente se detallan las características de las interfaces de hardware/software a desarrollar que permitirán a estos nuevos perfiles operar una computadora de forma independiente.

Tuberculosis is one of the biggest health challenge, the world is facing. Cervical lymphadenitis is a common manifestation of mycobacterial infections encountered in otorhinolaryngologic practices. It may be the manifestation of a systemic tuberculous disease or a unique clinical entity localized to neck. It remains a diagnostic and therapeutic challenge because it minimizes other pathological processes and eyelids in consistent physical and lab findings. A high index of suspicion is needed for the diagnosis of mycobact cervical lymphadenitis and should be considered in the differential diagnosis of a cervical mass especially in endemic areas. Introduction . The aim of this study was to determine the frequency and management of complications of AOM and COM over a period of 15 years in E.N.T Clinic “Iuliu Haţieganu”, University of Medicine and Pharmacy Cluj-Napoca, and to discuss the new concept in their treatment. Methods . Between January 2001 to December 2015, patients admitted to E.N.T. Department, “Iuliu Haţieganu” University of Medicine and Pharmacy Cluj-Napoca, with the diagnosis of otitis media and an associated intratemporal or intracranian complication were analyzed retrospectively. The overall incidence of all complications and of each complication individually was determined. Results . A total of 930 patients were diagnosed with otitis media. Complications of otitis media were diagnosed in 35 patients; thus, the incidence of complications was 3.37%. We identified 27 (2.9%) intratemporal complications and 8 (0.86%) intracranial complications. The most frequent complication was labyrinthine fistula in 13 (1.39%) patients. Conclusion . The incidence of otitis complications remains significant in our department. Chronic otitis media with cholesteatoma is the most frequent etiology of complications. Labyrinthine fistula is the most common complication of otitis media.

The influence of premedication and intraoperative narcotic administration on the incidence of vomiting after E.N.T. surgery was studied over a six-month period in 596 children. A high incidence of vomiting was found after intramuscular papaveretum and hyoscine and after oral diazepam. Oral trimeprazine tartrate was associated with a low incidence of vomiting and was found to have an anti-emetic effect when an opiate analgesic was administered after induction of anaesthesia.

AbstractThe study reviewed the case histories of 14 young aphonies. A questionnaire was completed by the five speech therapists involved with these cases. The patients were all initially examined by E.N.T. specialists and then treated by speech therapy. All the patients were ‘cured’ by speech therapy, that is the voice returned to its premorbid state. This study looks at common characteristics of presentation, different approaches to management, and the patterns of voice return.

We discuss two patients, sisters, whose father and mother are cousins, who have a mild genetic mutation, but severe systemic and moderate CNS disease, the treatment using Enzyme Replacement Therapy and the good response for it. CLINICAL PRESENTATION: – J.O.N, female, eleven years old, diagnosed in february 2003, presenting anemia, severe hepatosplenomegaly, bone disease, growth retardation, mild cognitive deficit, strabismus, important gait disturbance. – Peripheral blood: red cels=3,53, hemoglobin=7,9, hematocrit 25,7%, leucocytes=3.800, platelets=36.000 – Beta glucosidase level = 1,34 nmol/h/mg protein (n= 10 – 45) – Chitotriosidase level = 58.582 nmol/h/mg protein (n= 8,85 – 132) – Mutation: L444P/G377S – S.L.N, female, four years old, diagnosed in February 2003, presenting anemia, moderate hepatosplenomegaly, bone disease, growth retardation, mild cognitive deficit, strabismus, gait disturbance. – Pheriferal blood: red cels=4,22, hemoglobin=9,1, hematocrit=27,2% – Leucocytes=6.200, platelets=54.000 – Beta glucosidase level = 0,67 nmol/h/mg protein (n= 10 – 45) – Chitotriosidase level = 30.002 nmol/h/mg protein (n= 8,85 – 132) – Mutation: L444P/G377S TREATMENT AND RESULTS: Both patients started the enzyme replacement therapy using imiglucerase, 60 UI/KG every other week in june 2003. Fastly we observed the hepatosplenomegaly reduction, increase in pheriferal blood cells, the gait disturbance was disappearing (patient S.L.N is walking by herself and J.O.N is walking using walking-stick or when someone help her), the bone disease and the cognitive deficit are less marked and they are growing. COMMENTS: – The mutation G377S are expected to cause a type 1 phenotype, even in compound heterozigosity with a second mutation (in these cases L444P) or a null allele; in these cases we observed a Gaucher Disease type III; – The mild mutation G377S causes a high residual activity of Beta-glucosidase enzyme (17,6%), but in our experience, it was not, just by itself, a CNS protector; – We observed a very good response to Enzyme Replacement Therapy in the systemic disease using Imiglucerase 60 UI/Kg every other week. We could see a increase in neurological functions too. May be a mild mutation G377S, due the residual Beta glucosidase activity “helped” the ERT in these cases. – The genotyping Gaucher Disease patients may be critical to improve genotype-phenotype correlations, to offer genetic counseling to carrier couples and in some cases even for therapeutic decision making. Seventy nine children travelled 70 miles with their father or mother to P. A. H. from Mid Glamorgan where the E.N.T. waiting list delay for tonsillectomy was some two years.Most of the families who were offered the travel arrangements agreed and from the results of the questionnaires answered by the first 50 families, it was shown to be a successful exercise as both the children and parents were very happy with the arrangements and there were no complications.

(Experiments in Art & Technology) for Expo '70 in Osaka, Japan. The Pavilion is being returned to contemporary artists and audiences in order to extend the experience of the original to the Internet.
