Abstract:
The landmark paper discussed in this chapter reviewed 51 epidemiological studies looking at the incidence and prevalence of both acute pain and chronic pain in children and adolescents. The paper divided the studies they reviewed into two broad categories: those which examined pain in non-clinical populations, such as school children or general population samples, and those which examined pain in clinical settings, such as inpatients, outpatients from specific clinics, or special populations. In the clinical studies assessed, back pain in athletes and overuse injury syndrome in musicians were also included as studies from special populations. The non-clinical studies included head, stomach/abdominal, and back pain, as well as further studies focusing on oral/dental and limb pain, dysmenorrhoea, and multiple pain problems. The authors concluded that the epidemiology of pain in children and adolescents remained relatively undocumented.

Hydrotherapy holds promise for certain sexual and pelvic problems: Water that hits the skin acts on the entire body, triggering the neuro-endocrine-immune system, the brain, the gut-brain, and the autonomic nervous system—the neuro-endocrine axis. Hormesis (regular application of small toxic events or stressors leading to adaption and invigoration) is the mechanism that balances physiological and biochemical processes, including sexuality. Water applications result in homeostasis (balancing of internal systems—such as temperature, electrolytes, and hormones) and invigoration (strengthening of biological functions) and influence diverse bodily functions and dysfunctions loosely related to sexuality and reproduction. Dysmenorrhea, functional infertility, pregnancy, sexuality after menopause, decreased libido, breast tenderness, pelvic pain syndromes, erectile dysfunction and urinary tract infections/irritated bladder are discussed. Cold shower, cold wash, barefoot walking, warm footbath, sitzbath, full bath, warm water bottle, sauna with cold-water immersion afterwards, and some variations of these are the discussed water applications here.

Abstract:
Women experience pain differently than men and suffer unique pain conditions. Pain syndromes frequently overlap in women and can be associated with co-occurring mental health disorders, addiction, and intimate partner violence as well as childhood sexual abuse. A discussion of extant concepts for understanding pain in women includes gender-role theory, exposure theory, and vulnerability theory. The chapter focuses predominantly on women of child-bearing age, and their unique risks and management requirements; consequently, contraceptive needs and fulfillment are reviewed for their place in the clinical management of pain and addiction. Unique to women, dysmenorrhea, chronic pelvic pain, endometriosis, fibromyalgia, interstitial cystitis, vulvodynia, and pregnancy may all be undervalued or overlooked in a non-gynecological setting. The higher susceptibility of women to osteoporotic disease and associated pain in older age constitutes another risk zone for pain mismanagement. A text box (30.1) describes the opioid neonatal abstinence syndrome, its diagnosis, and its management.
