Abstract:
Potentially violent patients need immediate attention and evaluation to determine their risk of imminent violence. A past history of violence is the best predictor of future violent behavior, and individuals who have committed violent acts in the past and have been arrested for assaultive behavior represent the highest risk; people who carry weapons or have access to weapons are of relatively high risk. Individuals with violent impulses who are either intoxicated or are in withdrawal have the most extreme risk for imminent violence. The treatment of acute aggression or agitation involves the judicious use of sedative-anxiolytics or low doses of second-generation antipsychotics. SSRIs have been used to treat aggressive, impulsive, and violent symptoms, particularly in individuals with head injuries, and lithium carbonate can reduce impulsive aggression to extremely low levels in some aggressive patients. Two Tarasoff decisions have become national standards for clinical practice regarding “duty to warn” and “duty to protect” all potential victims of life-threatening danger from a homicidal patient.

Abstract:
HIV and AIDS, perhaps more than any other illness during any other time, contributed to the end of a “collusion of silence” and parentalism that characterized medicine prior to the era of civil rights and patient rights. The bioethical aspects of both healthcare delivery and care at the end of life changed dramatically during the beginning of the AIDS epidemic when young persons and their dedicated teams of caregivers were faced with a new infectious illness associated with both sexual transmission and intravenous drug use. This chapter delineates the ethical aspects of HIV and exceptionalism, screening, routine testing, informed consent, advance care planning through advance directives, confidentiality, criminalization, the duty to warn, and pre- and post-exposure prophylaxis (PrEP and PEP).

Abstract:
This chapter focuses on the antebellum era, a critical period of transformation and dissension. Concentrating on the prominent minister George F. White, it reveals how White's opposition to abolitionists such as Lucretia Mott was rooted in his experience as a supporter of Elias Hicks, whose controversial teachings helped fracture American Quakerism in the 1820s. In the 1840s, White was the most controversial, polarizing figure in Hicksite Quakerism. He felt it his duty to use his unquestioned talents to warn Friends, in the most forceful terms, against participating in antislavery, temperance, nonresistance, and other reform movements that many saw as advancing Quaker testimonies. The controversy over his crusade against reform movements would ultimately help fracture every Hicksite yearly meeting except Baltimore and change the course of Hicksite Quakerism.

Key requirements of federal laws and regulations are outlined in this chapter; the authors emphasize the need for an understanding of local and regional laws and precedents, which goes beyond the scope of the chapter, and a separate text box is provided with resources. Also provided is a text box analyzing 2017 changes to the federal Rule 42, CFR, relating to confidentiality and re-disclosure of information specific to treatment of substance use disorders. Areas of chapter focus include: HIPAA, confidentiality, general medical recordkeeping principles and obligations, federal audits (DATA 2000 and buprenorphine), medication storage, reporting requirements and the duty to warn, reports to employers, and the management of an office closure.

The prewar history of the Japanese intelligence community demonstrates how having power over much, but insight into little can have devastating consequences. Its postwar history—one of limited Japanese power despite growing insight—has also been problematic for national security. This book dissects the fascinating history of the intelligence community in Japan. Looking at the impact of shifts in the strategic environment, technological change, and past failures, it probes the reasons why Japan has endured such a roller-coaster ride when it comes to intelligence gathering and analysis, and concludes that the ups and downs of the past century—combined with growing uncertainties in the regional security environment—have convinced Japanese leaders of the critical importance of striking balance between power and insight. Using examples of excessive hubris and debilitating bureaucratic competition before the Asia-Pacific War, the unavoidable dependence on U.S. assets and popular sensitivity to security issues after World War II, and the tardy adoption of image-processing and cyber technologies, the book highlights the century-long history of Japan's struggles to develop a fully functioning and effective intelligence capability, and makes clear that Japanese leaders have begun to reinvent their nation's intelligence community.
