Abstract:
Abstract Groundwater for potable uses containing high fluoride is a worldwide threat to human health. Electrocoagulation is a sustainable approach for decentralized treatment. This study aims to analyse the economic and environmental impacts of electrocoagulation applied at different scale in rural area of northern China by life cycle cost (LCC) and life cycle assessment (LCA) methods. Decentralized treatment for defluoridization in rural Fangshan District, Beijing, where the groundwater quality is typical in northern China, is surveyed as a case study. Three scenarios were evaluated. In scenario 1 (S1) all domestic water is electrocoagulated at community scale without dual water supply system. In scenario 2 (S2) only water for potable uses is electrocoagulated at community scale and raw water and treated water are distributed via dual water pipeline systems. In scenario 3 (S3) water for potable uses is electrocoagulated at household scale. Life cycle assessment is performed with openLCA software and Ecoinvent database. S1 has the highest impacts for eutrophication and global warming. S2 has the lowest impact for toxicity but highest impact for ozone layer depletion. S3 has the lowest impact for global warming and acidification but highest impact for human toxicity. S2 is the costliest while S3 is the most economical. In this case, life cycle cost and global warming impact are the most restricted factors so electrocoagulation at household scale is recommended.

Variable flux memory machines (VFMMs) with series hybrid magnets using both low coercive force (LCF) and high coercive force (HCF) permanent magnets (PMs) have been recognized as a viable candidate for wide-speed-range industrial applications due to the advantages of high torque density and wide speed range. Nevertheless, the adverse effects of LCF PM on the HCF PM in series-type VFMM under different magnetization states (MSs) are still unreported. In this paper, the flux barrier effect (FBE) of the LCF PM existing in series hybrid magnet VFMM is first revealed, and its causes are discussed as well as analyzed in depth on the basis of the equivalent magnetic circuit method and finite-element (FE) analyses. A topology of VFMM with dual-layer PMs is further developed to alleviate the FBE induced from LCF PMs. It can be found that the FBE can be effectively suppressed by employing the dual-layer (DL) PM arrangement and additional leakage flux paths. A prototype of the proposed design is built, and the theoretical and FE results are experimentally verified.

Object: By retrospectively analyzing the energy spectrum of squamous cell carcinoma, adenocarcinoma, small cell lung cancer (SCLC), and pulmonary metastases that underwent dual-layer detector spectral computed tomography (DLCT) 3-phase scan of the chest, we explored the value of a multiparameter energy spectrum in the assessment of pathological types of lung tumors. Methods: Cases of squamous cell carcinoma ( n = 20), adenocarcinoma ( n = 24), SCLC ( n = 26), and metastases ( n = 14) were collected. Then the largest cross-sectional area (LCA) of the lesion, computed tomography (CT) values in the plain scan phase, arterial and venous phases (HU, HUa, and HUv), iodine concentration, and effective atomic number in the arterial and venous phases (ICa, ICv, Zeff[a], and Zeff[v]) were measured and compared among the nonsmall cell lung cancer (NSCLC), SCLC and metastases, and other 3 groups of SCLC, squamous cell carcinoma, and adenocarcinoma. Results: Only the LCA is statistically different among SCLC, NSCLC, and metastases ( P < .05). And the treated subgroup analysis did not show significant differences among the groups. However, the untreated subgroup analysis showed that there was a significant difference between NSCLC and metastases in LCA, SCLC and metastases in ICa, NSCLC and SCLC in HUv, NSCLC and SCLC in Zeff(v) ( P < .05). Conclusion: The energy spectrum parameters of DLCT have a certain clinical value in distinguishing NSCLC from SCLC in the Zeff(v) and distinguishing SCLC from metastases in the ICa.

The antenna based on the L-shaped monopole antenna and single-fed coplanar waveguide structure is proposed to generate two mutually perpendicular components of the electric field and exhibit circular polarization. The additional I-type Stub is attached on the left side of the ground to provide the balance between both horizontal and vertical current distributions . Two rectangular elements are added to improve impedance matching and enhance the circular polarization within two designed passbands. Besides, the dual-band mm-wave filter is integrated with the antenna to suppress the noise. The entire structure is implemented on the single layer LCP substrate. The simulation results show that the first and second operation bands from 26.45-27.74 GHz and 36.73-38.48 GHz have greater than 10-dB return loss. The Axial Ratio (AR) bandwidth of less than 3-dB for both operation bands are 26.89-27.38GHz and 36.26-42.42GHz respectively. It is expected to be applied in the fifth generation of mobile communication equipment in the future.</p> <p>&nbsp;</p>

Abstract:
Anterior STEMI (ST-segment elevation myocardial infarction) is associated with the worst prognosis of all infarction locations. We report the case of a 37-year-old male patient who presented for two hours of severe chest pain and was diagnosed with Killip I anterior STEMI in the emergency room. The emergency coronary angiogram revealed acute thrombotic ostial LAD (left anterior descending artery) occlusion and acute thrombotic ostial ramus intermedius (RI) near-occlusion. Thrombus aspiration for the LAD occlusion was performed and a large thrombus was extracted, followed by the artery’s reperfusion. However, we noticed that there was a large diagonal branch providing septal perforating arteries and that there was a distal LAD occlusion. We implanted a drug-eluting stent on the site of the proximal LAD lesion, but we could not obtain any flow in the distal occluded LAD. The patient underwent dual antiplatelet and unfractionated heparin treatment, and, 8 days later, we performed another coronary angiogram. To our surprise, there was very few residual thrombi in the previously occluded LAD segment, and there was no more thrombus in the RI. We noticed TIMI 3 flow in all coronary arteries and an increase in the patient’s left ventricular ejection fraction was also recorded.

Abstract:
The paper presents results of the investigation of the AlCrN and AlCrN + CrCN coatings, deposited by arc evaporation method on the austenitic steel substrate. Topography studies performed with the use of AFM showed that the roughness value was 24 nm for AlCrN and 14 nm for CrCN. Chemical analysis carried out with energy dispersive X-rays spectroscopy confirmed the chemical composition of the coatings. Transmission electron microscopy (TEM) investigations showed a column structure of AlCrN and CrCN layers with a width in the range of 10–200 nm. Tribological properties analyzed using a scratch test and ball-on-disc method showed a good adhesion of the coatings to the substrate (LC2 is 40 and 46 N for AlCrN and AlCrN + CrCN, respectively) and its high wear resistance; the use of an additional CrCN layers caused a decrease in the friction coefficient by 19%. Structure modification of the AlCrN + CrCN coating system and the related improvement in its mechanical and tribological properties allowed increasing the lifetime of the coated elements. The original approach was to produce a dual-layer coating, consisting of an internal, hard AlCrN layer and an external layer of CrCN, providing good tribological properties, as well as an appropriate forming of the transition zone between the layers.

Fluorescence decays of jet-cooled pyrazine excited at various positions of rovibronic transitions within the 0–0 band were measured by the use of a pulsed dye laser pumped by an excimer or a nitrogen laser. The experimental data thus obtained on the number (N) of the triplet states coupled to the initially excited singlet rovibronic state were analyzed and interpreted by the aid of computer simulation using an asymmetric rotor program. The results indicate that the extraordinarily great variation of N with excitation energy and unreasonably small values of N hitherto reported in the literature can be understood in terms of the conventional theory based on the singlet–triplet mixed state model which has been developed for elucidating the mechanism of the occurrence of dual fluorescence in intermediate case molecules.

Background: Thrombotic occlusion of the coronary artery, which succeeds the atherosclerotic plaquerupture or erosion, gives rise to a major portion of acute myocardial infarction (AMI) incidences.Nevertheless, coronary embolism is gaining increasing recognition as another important factor contributingto AMI. Case presentation: A 72-year-old woman with atrial fibrillation (AF) and diabetes mellitus histories,presented with chest pain radiating to the left arm and shoulder that began 6 hours prior to admission.Electrocardiogram revealed AF plus ST-segment elevation in the anterior leads.Intervention: Patient was first treated with anti-platelet agents (aspirin plus ticagrelor) and atorvastatin.Emergency coronary angiography depicted multi-site coronary embolization of the left circumflex artery(LCX) and the left anterior descending artery (LAD). Blood flow was not restored after intracoronaryinjection of 600 ug tirofiban. 40 mg recombinant human prourokinase was then administered via aspirationthrombectomy catheter. Outcome: Two weeks later, coronary angiography showed no residual obstructive lesion in the LCX andLAD with TIMI (thrombolysis in myocardial infarction) 3 flow. Conclusion: Primary percutaneous coronary intervention is the most effective measure. In the case offailed blood flow restoration, thrombolytic treatment in both intravenous and intracoronary route should beconsidered.

Conventional coherent pulse sequences such as photon echoes measure only highly averaged relaxation rates in complex multilevel systems, such as molecules undergoing state-changing collisions. Pulse frequency, phase, and shape control lets us generate sequences which give a more detailed understanding of the dynamics. Results of dual frequency, crafted shape sequences on I2 are presented which show that the "coherence dephasing" time T2 is primarily due to population redistribution (energy changing collisions) in the electronically excited state, and that the electronic ground state has a much smaller cross section for such collisions. Quantitative analysis is only possible with modified laser pulse shapes which excite a single velocity component, and requires pulse sequences which correct for the hyperfine dependence of predissociation. Since this correction is rarely made in previously reported coherent transient measurements, literature values of T2 and T1 may not be reliable in the zero pressure limit.

Abstract Introduction Recently, targeted marketing has encouraged teen e-cigarette vaping. Although e-cigarettes are often presented as a safe alternative to conventional tobacco, their toxicity is unclear. In adults, we have previously observed a link between dual usage of e-cigarettes and tobacco with increased sleep latency. We hypothesized an association between dual usage and increased sleep latency. Methods Participants were recruited to complete social media surveys. We performed three surveys: Survey 1 (n=47) in 2018, Survey 2 (n=1198) in 2019, and Survey 3 (n=564) in 2020. Surveys 1 and 2 had three sections: past and current inhalant use, the Pittsburgh Sleep Quality Index (PSQI), and the Leicester Cough Questionnaire (LCQ). Survey 3 did not include the LCQ, instead including the Hospital Anxiety and Depression Scale (HADS) and Patient Health Questionnaire (PHQ9). The adolescent data (aged 13–20 years; n=609) were isolated. Results Adolescents reported an increase in sleep duration with increasing age by one-way ANOVA. Males reported no change with increasing age, while, by Tukey’s multiple comparisons test, females got significantly more sleep at ages 19 and 20 than at age 14(p&lt;0.01). There was no significant correlation between inhalant use and sleep duration. When broken down by gender, female dual users slept more than female nonsmokers,(p=0.01; mean difference=43.8 minutes; CI=0.11 to 1.36), while there was no difference in males. We observed a significant association between inhalant use and sleep(p=0.0008), with dual use correlated with a longer sleep latency than nonsmokers (mean difference=6.27 minutes; CI=1.40 to 11.13. We saw no correlation between inhalant use and anxiety or depression, nor between inhalant use and cough severity and prevalence. Conclusion In female adolescents, we observed a peak in sleep hours at age 19 but significantly less sleep in fourteen-year olds. College-aged females may have a later wake time relative to middle-school and high-school aged females. Dual inhalant use in females was associated with a long sleep duration, raising concern for sleep disruption caused by dual use. Dual use’s association with increased sleep latency raises concern for nicotine-induced wakefulness. Further data are required in order to define public health strategies. Background. Takayasu arteritis (TA) frequently involves the coronary arteries, and restenosis is common after initial percutaneous coronary intervention (PCI). However, PCI remains a good option for patients who develop graft failure after coronary artery bypass graft surgery (CABG). Drug-coated balloons help in repeat revascularization after stent failure in TA. Case Presentation. A 31-year-old female with previous history of Takayasu arteritis (TA) and CABG with left internal mammary artery (LIMA) to left anterior descending artery (LAD) grafting in 2012, presented to us with exertional angina of 6-month duration. Her coronary angiogram showed total occlusion of the left main coronary artery (LMCA) ostium and atretic LIMA graft. Since the guiding catheter could not engage the LMCA, a retrograde approach was planned. After lesion crossing, the retrograde guidewire could not be externalized even after multiple attempts. Hence, a repeat antegrade approach was tried, and antegrade wire crossed through the channel created by the retrograde microcatheter. Then, intravascular ultrasound (IVUS) guided LMCA-LAD stenting was done. The patient was started on dual antiplatelets and prednisolone and was on regular follow-up. Three months later, the patient presented to us with non-ST elevation MI. Coronary angiogram showed critical in-stent restenosis of the LMCA stent, and optical coherence tomography (OCT) showed diffuse neointimal hyperplasia. OCT-guided PCI using sirolimus-coated balloon was done. On 8 months of follow-up, the patient remains symptom free. Conclusion. Coronary artery disease in TA may require repeated interventions due to stent/graft failure. Drug-coated balloons play a crucial role in repeat revascularization for stent failure in TA. Retrograde approach increases the technical success rate of PCI in LMCA-CTO.

Danburite as a member of the Luc Yen pegmatite mineral assemblage has been studied using fluid inclusion microthermometry and Raman spectroscopy. Data characterize well-preserved fluid inclusions which originate from primary large tubular inclusions as result of necking down. Same modifications underwent a second inclusion generation that evolved during healing of a later crack. Both generations of fluid inclusions show the same chemistry (H2O-CO2) characterizing 3-phase inclusions with additional solids (calcite, sassolite and danburite). Inclusions consist of pure CO2 and H2O with additional NaCl ± KCl comprising a salinity of about 4.5 mass%. Internal fluid inclusion pressures as well as bulk inclusion densities have been calculated using the fermi diad split method of pure CO2 at clathrate melting temperatures of the system and total homogenization temperatures, respectively. Mean internal pressures of ca. 4.5 MPa as well as a bulk density around 0.60 g/cm3 represent a low-dense fluid with XH2O~0.86 and XCO2~0.14 in composition that was present during formation of danburite. Abstract Background Ig light chain (LC) diseases such as AL amyloidosis and monoclonal light-chain deposition disease are caused by pathologic free LC. Treatment is aimed at eliminating LC production but success is limited. RNA interference (RNAi) can stop LC production but the diversity of LC variable region sequences poses a challenge that targeting consensus sequences in the constant region (CR) of LC mRNA may overcome (Blood 2014;123:3440). We have developed siRNA pools designed to target the κ or λ LC CR mRNA in human plasma cells and impair LC production and secretion, and have shown that the pool targeting the λ LC CR can do so, and can also trigger a terminal unfolded protein response in clones producing intact Ig due to intracellular accumulation of unpaired heavy chains (ibid). Here we report the results of continued in vitro and in vivo testing of these pools in patient specimens and in a murine xenograft model. Methods Pools of siRNA for the κ or λ LC CR (si[IGLCκCR], si[IGLCλCR]) were custom produced with a non-target control (si[-]). They were introduced in vitro into human plasma cells by an optimized streptolysin O-based method (SLO) and in a NOD.SCID xenograft flank plasmacytoma model by in vivo electroporation as per Gene Therapy 2011;18:1150. In vitro we evaluated LC gene expression, production and secretion at 24 hours in human myeloma cell lines and CD138-selected specimens from patients with plasma cell neoplasms, using real-time PCR (qPCR) for LC mRNA, flow cytometry for intracellular LC mean fluorescence intensity (MFI) and ELISA (Bethyl Laboratories) for LC secretion in 24-hour suspension cultures (106 cells/ml). In vivo we inoculated each of the flanks of NOD.SCID mice with 107 human myeloma cells (ALMC-1 or ALMC-2). When plasmacytomas were 0.5cm3 we injected si[IGLCλCR] or si[-] one time to each flank plasmacytoma respectively, allowing each mouse to serve as its own control. Two days later, the mice were sacrificed and the plasmacytomas excised for qPCR for λ LC mRNA and serum was obtained to measure human λ LC levels by ELISA. Results We have previously described results with siRNA targeting the λ LC CR in human cell lines that make λ LC (ALMC-1, ALMC-2, EJM, OPM2, MM.1S, and MM.1R) and in 16 AL λ patient specimens. We demonstrated significant decreases in LC mRNA, intracellular LC MFI, and λ LC secretion by cell lines (Blood 2014;123:3220); moreover, transcriptional profiling indicated minimal off-target effects (ibid; Supplement). We now report that in vitro secretion of λ LC by CD138-selected plasma cells from AL patients (n=3, newly diagnosed λ) treated with si[IGLCλCR] was reduced by 65% from a mean of 3.1 to 1.0µg/ml and that the residual λ LC mRNA was 49% of control. Similarly we treated κ LC secreting human myeloma cell lines with si[IGLCκCR] and si[-] (IM9, H929, JJN-3, and ARH77). By qPCR the residual κ LC mRNA was 13%, by flow cytometry the MFI was reduced by a median of 67.3% (22.5-90.8), and by ELISA mean κ LC secretion was reduced from 3.7 to 0.8µg/ml (P = 0.055, paired t test). We treated CD138-selected κ patient samples (AL 3, LCDD 1, MM 6) in the same way. By qPCR the residual κ LC mRNA was 57% control, by flow cytometry the MFI was reduced by a median of 37.5% (14-69.8), and by ELISA secretion was reduced from 9.4 to 6.5µg/ml (P = 0.02, paired t test). In the murine dual-flank xenograft model employing λ secreting cells, by qPCR there was a reduction in λ LC mRNA with si[IGLCλCR] treatment in 13 of 16 mice (ALMC-1 11/114, ALMC-2 2/2). In these 14, the median λ LC expression was 66% of control (range, 17-97). In 6/13 the average reduction in λ LC expression was 59%. Of note, measurable levels of human λ LC were found in the blood of all mice at sacrifice. Conclusion With one pool of siRNA targeting the constant region of the κ or λ LC we can significantly reduce production and secretion of LC by clonal human plasma cells, including patient cells, and also reduce the expression of LC in xenograft plasmacytomas in vivo. Two methods of siRNA delivery have been employed in this work thus far, SLO and in vivo electroporation, neither of which require endosomal escape. The specificity of the siRNA pools for plasma cell LC genes and the possible receptivity of plasma cells to RNAi are important positive aspects of this work. Further pre-clinical development of Ig LC CR RNAi employing lipid-based nanoparticle platforms is warranted in order to optimize cell-specific delivery, delivery efficiency and siRNA targeting. Abstract A 74-year-old man with a history of ischaemic heart disease (multiple percutaneous coronary interventions (PCI)), broke his ankle. Therefore, he discontinued dual antiplatelet therapy (DAPT) -acetylsalicylic acid plus clopidogrel- and started enoxaparin sodium 4000 UI/day on medical advice. A few days later, he was admitted to the emergency department for refractory chest pain, vomit and syncope. Twelve-lead electrocardiogram (ECG) showed a sinus rhythm interrupted by frequent premature ventricular contractions in a pattern of trigeminy. The transthoracic echocardiogram revealed a mildly dilated LV with normal wall thickness and marked hypokinesis/ akinesis in apical, infero-lateral and antero-lateral walls, with a 33% ejection fraction. High sensitivity cardiac troponin was increased, and a second ECG showed a diffuse ST segment depression. The patient was loaded with aspirin and emergently taken to the cardiac catheterization lab. Coronary angiography showed a very late stent thrombosis of the left circumflex artery (LCx), with occlusion of the extreme distality of the second obtuse marginal artery and a 50-60% intrastent restenosis of the ostium-proximal tract of the left anterior descending artery (LAD). Given the non-obstructive nature of the thrombus, risk of peripheral embolization, TIMI flow grade 3 and improvement of symptoms, the decision was made to adopt a conservative strategy with loading dose of ticagrelor, heparin and tirofiban infusion. Besides, an optical coherence tomography (OCT) control was scheduled for the next day. OCT of the LCx revealed a well apposed and well expanded stent with a non-obstructive residual thrombus, nevertheless not visible on angiography. Thus, the tirofiban infusion was prolonged up to 48 hours. OCT of the LAD detected a fibro-calcific disease determining a significant restenosis of the distal left main (LM)-LAD ostium. OCT-guided provisional stenting with two drug eluting stents on LM-LAD-LCx was performed successfully. The patient was discharged home on acetylsalicylic acid (lifelong), ticagrelor (for 12 months) and enoxaparin sodium 4000 UI/day (according to specialist indication). DAPT is superior to anticoagulant therapy in preventing stent thrombosis in patients undergoing PCI, and a more informed inter-specialist communication is needed. Intracoronary imaging can help in the management of stent failure and in the guidance of complex PCI, allowing a patient-tailored treatment.

Abstract Background ST segment elevation acute myocardial infarction and acute pancreatitis are diagnostic and therapeutic emergencies. The concomitance of these two pathologies produces intricate and confusing symptoms leading to late presentation. Clinical Case Description- We present the case of a 62-year-old male patient, non-smoker, admitted in the emergency department for intense, continuous epigastric pain with posterior radiation and dyspnea, started four days before. On admission we found hemodynamic stability, lung crackles in the ½ inferior of the right lung, SaO2= 94% under 3l/min O2, normal cardiac sounds, HR 102/min, mitral systolic murmur, BP 145/80mmHg, abdominal pain on palpation. Lab tests revealed elevated cardiac troponin values and detection of a fall of cTn values in dynamics (hs-cTnI 30372ng/l, TnI 73ng/ml), but also high serum transaminases, amylase and lipase (1278 U/l), with high inflammatory markers, leukocytosis and neutrophilia. ECG showed sinus rhythm, QRS axis 0 degrees, ST segment elevation in V1-V4, ST segment depression DI, aVL, negative and biphasic T waves in V5-V6, DI, DII, aVL, Q wave in DIII, aVF and poor R wave progression V1-V4. Admission echocardiography revealed dilated left cavities with severe left ventricular systolic dysfunction (EF 20%), akinesia of the anterior and inferolateral walls, moderate mitral and tricuspid regurgitation, grade III diastolic dysfunction and possible pulmonary arterial hypertension. Coronary angiography revealed distal stenosis of the left main, involving the bifurcation, 80% ostial LAD stenosis, proximal LAD occlusion, 30% ostial LCX. PCI with DES of the proximal LAD, and left main–LDA junction with overlapping was performed successfully. Abdominal echography showed increased volume of the pancreas and significant decrease in echogenicity. Abdominal CT scan assessed inflammatory changes in pancreas and peripancreatic fat (grade C Balthazar score), superior mesenteric artery partial thrombosis and alveolar consolidation in the right lung. Echocardiography reassessment four days later revealed a fresh apical thrombus of the left ventricle. He received treatment with low molecular weight heparin, dual antiplatelet therapy statin, loop diuretic, aldosterone antagonist, digoxin, antibiotics and food restriction with gradual oral realimentation. He needed positive inotropic support for the first 4 days from a total of 28 days of hospitalization. Conclusions ECG changes and prothrombotic status can occur in patients with acute pancreatitis. The diagnosis and management of STEMI concomitant with acute pancreatitis can be challenging. The multimodality imaging and interdisciplinary approach individualized to the patient’s clinical situation is important, especially when having safety concerns about the revascularization therapy, antiplatelet and anticoagulant therapy.
