Abstract:
Abstract Substance misuse is a global phenomenon. However, little is known about substance misuse issues in Islamic nations or about the provision of preventative and rehabilitative services in such nations. This thesis explores the legal context of such services in the Maldives and pays particular attention to tensions between the formal policies of the National Narcotics Control Bureau and clinical practice. Findings are drawn from a review of government and service policy documents, five semi-structured individual interviews with clinical practitioners and senior administrative staff from rehabilitative services, and a three day focus group workshop with clinical staff. Findings show the lack of awareness of the legal and policy contexts for service provision and the ways in which existing policy frameworks often detract from the forging of therapeutic alliances. The primary concern raised by the analysis is the lack of involvement of clinical staff in policy formation and revision. This contributes to series of tensions and contradictions between official aims for services and the actual provision of these services. Further a range of ethical issues arose as a result of inadequate professional monitoring, training, and peer review. Recommendations are made regarding how these issues should be addressed in order to enhance the Maldivian response to increasing substance misuse.

Abstract:
ABSTRACT The aim of this thesis is to explore the impact of treatment on male addicts who attend Narcotic Anonymous (NA) meeting between the ages of 45 to 60 years that have been free from drug addiction without relapse in 10 years. The interest is to really understand how the addicts have been impacted and what factors have contributed to their being able to remain free from drug abuse without relapse. The research is a qualitative study. Semi structured interviews based on interview guide that are made up of six open ended questions was used to generate information (Primary data) from eight interviewees. The data generated was analyzed using qualitative content analysis in hermeneutic perspective. Also, Maslow’s need hierarchy theory and early research was used to analyze the data that were generated. It was found that the former addicts experienced improvement in their psychological health identity and social situation. Furthermore, they were impacted by different treatments like cognitive behavioral therapy, environmental therapy and the NA 12-step program. The findings of the study also indicates that the interviewees gained job, education and driving license because of the drug abuse treatment they have undergone. In addition, having a partner, stable family and regular attendance of NA meeting have helped the interviewees to remain free from drug use in 10 years. Furthermore, from the findings, improved psychological health, improved changes in identity and social situation are themes found from the coding and categorization from qualitative content analysis method. The main theme found is getting new ways of gaining control of addiction in relation to improved psychological health, improved changes in identity and improved social situation.

Este estudo tem como objetivo investigar a visão do dependente de drogas sobre o lugar do pai na família. Para tanto, foram realizadas entrevistas com dez internos, do sexo masculino, de uma clínica para tratamento da adicção em Niterói, no Rio de Janeiro. O lugar do pai, de acordo com os entrevistados, foi marcado pela pouca intimidade e pelos extremos da omissão ou autoritarismo. O lugar da mãe, em contraste, foi extremamente valorizado e idealizado pelos entrevistados. Identificamos, na fala dos sujeitos, quatro perfis de pais: autoritário; omisso; amigo e dependente de álcool e/ou outras drogas. Relacionamos, ademais, os desafios da paternidade ao difícil processo de desenvolvimento da identidade masculina. A discussão dos dados é embasada no referencial sistêmico; sendo investigado, portanto, o lugar do pai como interligado aos lugares dos outros integrantes da teia familiar. Concluímos ser muito importante, para a funcionalidade familiar, que o pai possa ocupar um lugar mais próximo dos filhos, tornando mais flexível e equilibrado o posicionamento paterno e materno na família.
This study investigates the role of the father in a drug addicts family. To this end interviews were conducted with ten male patients under treatment in a clinic for drug addicts in Niterói, Rio de Janeiro. These interviews revealed several commonalities regarding the fathers in these families: a lack of intimacy, physical absence and authoritarianism. The mother´s role, on the other hand, was highly valued and idealized. Through responses given, we were able to identify four profiles or types of father: the authoritarian father, the absent father, the friend-father and the alcohol/drug-dependent father. Furthermore, we related the challenges of fatherhood to the difficult process of the development of the male identity. The discussion of data is based on systemic theory; therefore, what is being investigated is the role of the father as it relates to the role of other family members. We concluded that, for the family structure to work, it is very important that the father have close contact with the children, contributing to better balance as concerns the paternal and maternal roles in the family.

In an effort to gain more insight into the subjective experience of recovery, and provide a platform for their voices to be heard, this research endeavour focuses on the storied-experiences of recovering addicts as conveyed through life narratives. Through the process of narrative reconstruction, the study will elucidate shifts in meaning-making. The study employed a postmodern social constructionist epistemological lens. Three recovery addicts were recruited from an Ibogaine-assisted rehabilitation centre. Narrative inquiry methodologically navigated this study. A qualitative methodological design was employed. Data was collected using written autobiographical narratives, and semi-structured interviews. Data was analysed using narrative-thematic analysis, which gave rise to six main themes and eight sub-themes. Recovery involved a reconstruction of identity which was informed by shifts in meaning related to love, acceptance, forgiveness, and the acquisition of and identification with new roles. Shifts within their relationship with drugs and the meaning they attached to the accompanying lifestyle, appeared to be vital reinterpretations in their journey. Communal and familial support was identified as a key source of recovery capital. The narratives reflected growth and reparation following addictive stagnation and destruction. Ibogaine was identified as a vital catalyst in the reconstruction of their life narratives. A renegotiation of power and control also informed the transition from addict to recovering addict. Volitional-change was an important feature. Faith and altruism served as important facets in constructing a purposeful recovery narrative. The study allowed for new insights into the experience and meaning of recovery from drug addiction, conveyed through rich idiosyncratic accounts.

Abstract:
This study is about the use of alcohol and its influence during the recuperation of drug addict patients undergoing treatment. Levels of the use of illegal drugs and alcohol were compared before, during and after the treatment of 82 patients assessed over a one-and-a-half year follow-up study. The results show that after the intervention there is an increase of alcohol use by 50% of the addicts. The 10-13% exhibit excessive levels. The higher alcohol use coincide withthe end of the treatment. This tendency changes later (18 months) into similar levels as the initialones. These results do not allow to consider the aforementioned increase as a confirmationof the substitution hypothesis, that is the alcohol use-redirection towards other more socially tolerated substances.
Se estudia la influencia del consumo de alcohol en la recuperación de drogadictos sometidos a tratamiento. Se relacionan los niveles de consumo de drogas ilegales y de alcohol de antes, durantey después del tratamiento en una muestra de 82 pacientes seguidos durante un año y medio. Se evidencia que tras la intervención aumenta el consumo de alcohol en el 50% de adictosy llega a niveles excesivos en un 10-13%. El mayor consumo coincide con la finalización del tratamiento. Posteriormente (18 meses) baja a niveles próximos a los iniciales. Estos resultados no permiten interpretar die ho aumento mediante la hipótesis de la sustitución, es decir, el desvío del consumo hacia sustancias más toleradas socialmente.

Abstract:
The rising numbers of young drug abusers in Hong Kong have attracted much concern by the government. Unfortunately, there are few established studies that have been conducted to evaluate the effectiveness of the existing rehabilitation and treatment programs. The dearth of research in this area can compromise the potential for the improvement of current services and hinder the development of new intervention strategies. The present study aimed to help filling in this gap in the literature through the evaluation of Christian Zheng Sheng College, a school-based rehabilitation program. Across a year of observation, we documented the students’ improvements on a range of outcomes (i.e. self-efficacy, school engagement, civic responsibility, perceived social support and religious coping). We also examined whether the perceived quality of natural mentoring relationships established within Zheng Sheng were related to these changes. Across the four waves, the overall population displayed significant linear growths in all measured outcomes apart from civic responsibility and perceived social support. Results also revealed that the perceived closeness with an identified mentor in Zheng Sheng accounted for differential impact toward youth’s self-efficacy, civic responsibility and perceived social support. Implications of the findings were discussed in particular reference to other drug rehabilitation programs in Hong Kong.

This dissertation investigates how the settings of drug use influence the affective and neurobiological response to heroin versus cocaine in addicts. Chapter 1 reviews the neuropharmacology of heroin and cocaine and the theoretical background for drugs-settings interactions, including a detailed discussion of findings from previous studies in animals and humans that show how the same settings can influence in opposite directions the reinforcing effect of heroin and cocaine. Cocaine self-administration, for example, was greatly facilitated when rats were tested outside the home environment relative to rats test at home. The opposite pattern was found for heroin. Translational studies in humans yielded similar results. Indeed, heroin and cocaine co-abusers reported using the two drugs in distinct settings: heroin preferentially at home and cocaine preferentially outside the home. The aim of this dissertation is to determine whether the setting could also influence in opposite manner the affective and neurobiological response to heroin and cocaine in human addicts. Chapter 2 illustrates the findings of a study aimed at testing the hypothesis that the affective state experienced under cocaine or heroin is the result of an interaction between central and peripheral drug effects and the surroundings of drug use. According to this hypothesis, when cocaine is taken at home there is a mismatch between the familiar environment and the peripheral effects such as arousal, increased heart rate, increased respiratory rate, and increased muscular tension (which are usually produced in stressful situations). This mismatch dampens cocaine-rewarding effects. A mismatch would also occurs when heroin (which produces sedation and decreases heart rate, respiratory rate, and muscular tension) is used outside the home in contexts requiring vigilance. We found indeed that co-abusers subjectively experienced opposite changes in arousal, heart rate, respiratory rate, and muscular tension in response to cocaine (increase) versus heroin (decrease). Most important, using a novel two-dimensional visual test, we found that in agreement with the working hypothesis the valence of the affective state produced by heroin and cocaine shifted in opposite directions as a function of the setting of drug use: heroin was reported to be more pleasant at home than outside the home, and vice versa for cocaine. Chapter 3 illustrates the results of in which emotional imagery was combined with fMRI to investigation the neurobiological underpinnings of drug and setting interactions in addicts. Heroin and cocaine co-abusers were asked to recreate real-world settings of drug use during fMRI. In agreement with the working hypothesis, we found that heroin and cocaine imagery produced opposite changes in BOLD in the prefrontal cortex and in the striatum, regions implicated in brain reward in humans. Furthermore the same pattern of dissociation was observed in the cerebellum, suggesting that that a fronto-triatal-cerebellar network is implicated in processing drug-setting interactions. Chapter 4 includes a summary of the results, a general discussion, and suggestions for future research and implication. The major finding is that the environment surrounding drug use can influence in opposite manner the affective and neurobiological response to heroin and cocaine, suggesting that therapeutic approaches to the treatment of drug addiction should take into account the distinctive effects of different classes of drugs as well as the contexts of drug use. The consequences of drug use and benefits of methadone maintenance therapy (MMT) were investigated in a random sample of Maori and non- Maori injecting drug users in Christchurch, Aotearoa New Zealand. Eighty- five injecting drug users (IDUs) who had been on MMT for a mean time of 57 months were interviewed and followed up over an average 18 month period. Markov models were used to model cohorts of IDUs, changes in their health states and the effects of MMT and anti-viral therapy on morbidity and mortality. The savings in life from reductions in drug overdoses were used as the main outcome measure in cost-effectiveness analysis. Cost-utility and cost-benefit analysis were also used to provide additional information on the costs and outcomes of treatment. Comparisons were made between: (a) MMT alone; (b) MMT provided with conventional combination therapy for hepatitis C virus (HCV); and (c) MMT provided with anti-viral therapy with pegylated interferon. The monetary costs of drug use and benefits of MMT were similar for Maori and non-Maori. However, Markov modelling indicated that MMT is associated with greater savings in life for Maori than for non-Maori. Further, Maori IDUs identified the main personal costs of drug use as being loss of their children and loss of marriage or partners. Large reductions in use of opioids and benzodiazipines were reported at interview, compared with before starting MMT. The participants also reported large reductions in crime and stabilisation of their lifestyles. Improvements in the general health of IDUs om MMT were reported. However, 89% were positive for HCV infection, which was identified as the major physical health problem affecting IDUs in New Zealand. Few IDUs had received anti-viral therapy for HCV infections, despite having stabilised on MMT. This study investigated the benefits of providing anti-viral therapy for HCV to all patients meeting treatment criteria. The cost-effectiveness of MMT alone was estimated at $25,397 per life year saved (LYS) for non- Maori men and $25,035 for non-Maori women IDUs (costs and benefits discounted at 3%). The incremental effects of providing anti-viral therapy for HCV to all eligible patients were to save extra years of life, as well as to involve additional costs. The net effect was that anti-viral therapy could be provided, at a similar level of cost-effectiveness, to all patients who meet HCV treatment criteria. Cost-effectiveness could be improved if IDUs could be stabilised on MMT five years earlier at an average age of 26 instead of the current age of 31 years. The cost-effectiveness of treatment with pegylated interferon was similar to that for conventional combination therapy because there were incremental savings in life as well as increased treatment costs. Costs per LYS were estimated to be lower for Maori than for non-Maori, reflecting ethnic differences in mortality. Sensitivity analysis revealed that provision of MMT with anti-viral treatment remained cost-effective under varying assumptions of mortality, disease progression and compliance with treatment. the main problems that were not improved during MMT were continuing use of tobacco and cannabis, low participation in paid employment, only three participants had received specific treatment for their HCV infections. Cost-benefit analysis using a conservative approach showed a ratio of the benefits to the costs of MMT of 8:1. Benefits were demonstrated in terms of large reductions in crime. Benefit to cost ratios were similar for the different policy examined, as well as for both Maori and non-Maori IDUs.
