Abstract:
Doctor attributes contribute significantly to the quality of the doctor–patient relationship, consultation, patient satisfaction, and treatment outcomes. However, there is a paucity of research on this topic in many settings in developing countries, including Botswana, where accessibility and availability of care itself are a challenge. The study examined doctor attributes that patients in Botswana desire from the perspectives of doctors and patients in selected public clinics located in four health districts of Botswana. We used a qualitative design and conducted face-to-face interviews with 32 adult patients and 17 doctors selected through the purposive sampling technique. Interviews were audio-recorded and transcribed. Data analysis followed the six steps of qualitative thematic data analysis. We found both discordance and congruence between the doctors and patients on key attributes that patients desire in a doctor during consultation. Both agreed that effective communication and listening skills were key desirable doctor attributes that improve the doctor–patient relationship. Conducting the consultation in the language of the patients enhances effective communication. Doctors cited clinical expertise and competence as key desirable doctor attributes, whereas patients cited interpersonal and social attributes including kindness, empathy, and respect as key doctor attributes that increase trust in the doctor. However, patients expected the doctor to have clinical knowledge, which they perceived as essential to improve doctor–patient interaction and health outcomes. The findings highlight a need to enhance the interpersonal and communication skills of doctors to improve the quality of doctor–patient interactions. To optimise and enhance the consultation, continuing professional development should be adopted as a strategy to improve the communication and interpersonal skills of doctors.

Burke, Sarah Elizabeth. "The doctor-patient relationship : an exploration of trainee doctors’ views." Thesis, University of Birmingham, 2008. http://etheses.bham.ac.uk//id/eprint/125/.

Full text
Abstract:
Greater understanding of the ways in which medical trainees perceive the doctor-patient relationship could inform future developments in educational provision. A qualitative study was conducted, using a case study approach to explore the perceptions of postgraduate trainees in two medical specialties, general practice (GP) and otolaryngology (ear, nose and throat surgery, ENT), in the West Midlands region of the United Kingdom. Following a scoping exercise in 2002, interviews with 20 trainees (10 GP and 10 ENT) in 2004 and questionnaires from 16 ENT and 89 GP trainees in 2007 explored trainees’ views of the doctor-patient relationship, including perceptions of the nature of that relationship and how they had learnt to develop relationships with patients. Five conceptual frameworks that participants drew upon when talking about the doctor-patient relationship were identified: paternalism; guided decision-making; partnership; clinical and consumerism. Trainees described a fluid doctor-patient relationship which adapts to differing contexts, taking different forms in different situations and influenced by factors outside the doctor’s control, including time and the patient’s personality. Personal experience and observing senior colleagues were considered to have had the greatest impact on learning. Higher Specialist Training which acknowledges the complexity of the doctor-patient relationship and encourages reflective practice is recommended.

Abstract:

Television is a widespread, easily accessible component of popular culture that we invite into the most intimate of environments: our homes. Like other forms of popular culture, it is not only influenced by religious belief, but also has the power to transmit both traditional and subversive religious ideas to viewers. This thesis draws upon methods used in the study of religion and popular culture to argue for the potential of television to influence religious belief. Television can transform contemporary thought and renegotiate ideas of identity as well as reposition social debate and conflict in both secular and religious environments. Focusing on the British television show Doctor Who, this thesis closely analyzes recent episodes of the series to illustrate the ways in which current show-runner Steven Moffat's story arc puts forth a Freudian critique institutional religion. This thesis demonstrates that Doctor Who has developed in such a way as to reveal the deception of organized religion and its destabilization of modern notions of rationality. Rather than banishing religion to the trash heap of history, however, Moffat's Doctor Who encourages believers to reevaluate traditional religious belief and practice.

Abstract:
This thesis has used the methodology of an ideological criticism on the long-running science-fiction television series, Doctor Who. Argued within, is that an ideological paradigm shift occurred during the fifty years of scripted storylines. To discern evidence supporting the shift, multiple episodes were viewed from each of the first eleven Doctors. During the viewing process all aspects presented within the show, such as dialogue, movements, tactics, and traits, were explored for ideological agency. Once found, the discourse was further analyzed to understand how these ideologies were enforced. From the findings, proof was examined in order to show by the end of the eleventh Doctor, Doctor Who had moved from portraying the viewpoints of individualism to collectivism.

Abstract:
Between incurably degenerative illness and the graffiti which ignited the Syrian Civil War, YOUR TURN, DOCTOR complicates hope. When myths of revolution, of wellness, no longer console—love as measured in anything but loss. Within a multidisciplinary project how an increasingly painful embodiment intersects the material excess of capitalism is explored. Can objects function as a political demand, necessitating changes in the way the world is ordered? Who for? To understand one kind of oppression in necessary sterility and another in marginalization so profound blindness can result. That is to ask, how long must one be told they do not see a thing they see before they don’t, before transgressions become norms? A list of "Indulgences" modeled loosely after Martin Luther’s Ninety-Five Theses outlines content. Five sections reference the five pillars of Islam— with each containing nineteen individual proposals. Nineteen serves as the common denominator for the mathematical structure of much of the text of the Quran.

Abstract:
Evidence demonstrates the health benefits of early breastfeeding for mothers as well as infants. The World Health Organization states, "Breast milk should be given within the first hour of life." This purpose of this project was to develop a breastfeeding educational toolkit to promote breastfeeding in the hospital environment. The question that guided this project was, "Will the development of a breastfeeding education toolkit, designed to serve as a learning tool to support the breastfeeding mothers, be found to be useful and valid by professional staff and lay end-users?" The goals of the project were to provide an educational toolkit aimed at supporting new mothers, and to validate the toolkit with stakeholders from the Mother/Baby unit. The benefits of breastfeeding and breastfeeding research were established through an evidence-based search of peer-reviewed journals. The developed toolkit was reviewed by10 stakeholders from the Mother/Baby unit: 1 physician, 3- lactation consultants, 5 nursing professionals, and 1 layperson of childbearing age. To assess the quality of the guidelines, the Logic Model and the 4 domains of the AGREE II Instrument were used. The descriptive data analysis from the AGREE II instrument led to the recommendations for the toolkit. All reviewers acknowledged the need for the toolkit as a support for the mothers and their babies. The stakeholders provided their support for the developed toolkit. The breastfeeding education toolkit has the potential to support positive social change by improving the health of the mothers and babies through improved breast-feeding knowledge.

Abstract:
A doctor needs to know the patients’ medical history to investigate the root cause of symptoms. Traditionally it is done by a questionnaire at the clinic’s reception desk, or the doctor asks a series of questions to obtain a relevant medical history. An incomplete or wrong medical history affects the doctor’s decision. The challenge for a busy doctor is to obtain all the relevant medical history in a short period of time accurately and without missing any important history. Affordable clinics employ an assistant or junior doctor to sort out all the paper-based medical history and mark the important points. It increases doctors’ work performance by saving doctors time and making room for consulting more patients. But not all the small clinics can afford such assistants. This paper lists the technical requirements to develop a personal medical history visualization tool to increase doctors’ productivity.

Abstract:
This paper reports on the three-dimensional loads measured during chiropractic treatments delivered during low back spinal procedures. A three dimensional force transducer was placed between the doctors hand and the patient while delivering the treatment. Two doctors have delivered treatments to a total of five subjects on two consecutive days. A laptop computer was used to collect data using labview software and Keithly instruments PCMCIA card. The data was analyzed using Mathcad software to determine the magnitudes of the forces delivered. The results show three-dimensional loads are applied by the doctor at the doctor’s hand-patient interface. The compressive forces reached as high as 450 Newtons, and the shear forces have reached as high as 150 Newtons. The moments have reached as high as 4Nm. This information is valuable in further understanding the effects of these loads on the spine.

Abstract:
A web-based telemedicine system between a patient and a doctor is presented in this paper. Telemedicine is something people can get medical treatment from a certified doctor online. As COVID-19 hit the world, many countries took the initiative to take non-therapeutic preventive measures, and telemedicine can play an important role. More than half the population lives in rural areas in Bangladesh, and there are not enough specialized doctors for that population. People who live in rural areas either have to wait a long time to get connected with a doctor or have to travel far to get to the big cities. So, most of them have to consult quack doctors to get medical treatment. The primary focus of this research is to create a web-based telemedicine system that is cost-effective, simple to use, and portable in the event of a pandemic, and that can be utilized anywhere in Bangladesh. Anyone can find their specific doctor to get medical treatment instantly. There will be a dashboard for both patients and doctors to track everything effortlessly. There will be a profile page for both, and they can edit and update their profile page at any time. The payment gateway is also very easy to use and hassle-free. We used PHP and Java-script to develop the backend, HTML and CSS for the frontend, MySQL for database design, and, for the video API, we used videoHDK. This system was tested in a lab environment, and it produced satisfactory results at such a low cost and was user-friendly as well.

Abstract:
For the last thirty years, we, as inhabitants of the planet Earth, have been witnessing a powerful technological breakthrough – nanotechnologies, wireless means of communication, cars with autopilot – a commonplace for modern man. The human essence, the soul, and the diseases that doctors have been studying and successfully fighting with them for thousands of years remain unchanged. Almost a century has passed since the term "iatrogenic" was first proposed in medical practice. This term had a different, narrower concept in contrast to modern realities. In modern conditions, even taking into account the fact that from the position of the legislator there is no clear definition for the term "iatrogenic", but it is successfully used in the practice of doctors of forensic medical experts and forensic investigative bodies - "iatrogenic crime". The doctor, in relations with the patient, is the most unprotected person; every word of the patient (patient) is considered true, since from the position of the legislator he is a priori a victim, and the professional experience and scientifically based facts of the doctor are an excuse. At the same time, during the production of forensic medical examinations, only an assessment of the actions of the doctor is made, according to the results of the analysis of which the presence or absence of a causal relationship between the actions of the doctor, but not the patient, is established.

Abstract:
During the process of medical interpretation and diagnosis in medical image, doctors’ attention allocations are various depending on individuals and cases. The process of diagnosing in the medical image involves complex interplay between visual perception and effective information acquisition strategies coupled with medical knowledge. It is difficult for doctors to explicitly explain their strategies because the process is often implicit. To date, precisely what attention allocation patterns and cognitive strategies in medical image reading, remains unknown. This study aims to uncover the doctor’s attention allocation and transition patterns in reading chest X-ray image, elicit diagnostic strategies based on doctor’s eye movements and interviews, and find the differences of diagnostic strategies between expert and novice doctors. Finally, prospective suggestions for leading novice doctors to an effective diagnostic strategy in reading X-ray image can be presented. We simulate the scenario of annual medical checkup using four patients’ cases, and recruit participants with diverse medical experiences and specialties in Tokyo Medical and Dental University Hospital to compare the differences of attention allocations between doctors. Doctors are asked to identify the lesion and give diagnostic decision to four cases. Their eye movements are recorded in the whole process by eye tracker. After completing all four cases, participants are asked to attend an interview session in which their eye movements are used as cues to elicit their diagnostic strategies. And two questionnaires are answered at last. Fixation duration, the number of fixations in each are of interest (AOI) are used to visualize doctors’ attention allocation and fixation transition patterns. Both qualitative and quantitative analysis are used to describe doctors’ diagnostic strategies and compare the differences between expert and novice doctors. Each doctor has personal characteristics when diagnosing. Doctors have a significant preference to read to current image. Doctors tend to pay more attention to areas where physiological structures overlap and where doctors think suspicious. As for diagnostic strategies, four typical patterns of change of diagnostic strategies in timeline are found. Furthermore, the differences are found between expert and novice doctors in attention allocation and the use of historical image. The effective diagnostic strategy is that performing the inspection routine of the current image separate with comparison with the historical image to avoid distracting and missing information. The comparison should focus on important areas and suspicious areas rather than the whole image. The suggested important areas are the lung apex, mediastinum, heart, left lung hilar and the lower lung field. The proposed effective strategies could be included in the medical education and new doctor training to improve novice doctors’ ability to diagnose by multiple images.
