Full textThe present work proposes a novel heuristic model for studying Interpersonal Trust. Building upon previous conceptualizations of trust, we recommend considering trust as an iterative, circular, and multi-step process. Hence, we stress the importance of considering trust expressions as a dynamic process in continuous evolution rather than a static personality disposition. In the present work, our main aim is to shed light on the processes underlying the lower propensity of individuals with Borderline Personality Disorder features to trust others (i.e., untrustworthiness bias). Throughout the thesis, we exemplify the use of the proposed model for exploring the well-established untrustworthiness bias of individuals with Borderline Personality Disorder (BPD) features. Chapter 1 provides a detailed illustration of the model mentioned above and a systematic review of the previous literature on trust impairments among individuals with BPD. In the following chapters, we present five empirical studies that elucidate the peculiar functioning of individuals with BPD features in some of the stages suggested by the model. In Chapters 2, 3, and 4, we investigate the influence of some individual differences on BPD’s untrustworthiness bias. We respectively focus on the exploration of Rejection Sensitivity, Justice Sensitivity, and Suspiciousness. In these chapters, we empirically prove that different trust-related personal dispositions have a typical influence on the interpersonal trust dispositions of individuals with BPD features. In Chapter 5, we explore the effects of diverse (i.e., direct or indirect) cues on the untrustworthiness bias of individuals with BPD features. Our findings remarkably suggest that the association between BPD features and untrustworthiness bias is not stable rather variable. Finally, in Chapter 6, we focus on the influence of Covid-19 circumstances on the interpersonal trust dispositions of individuals with BPD features. Surprisingly, we found similar interpersonal trust dispositions among individuals with different BPD features’ levels in a large community sample recruited during confinement. In conclusion, we discuss our findings and the implications of using the suggested model to study Interpersonal Trust both from an empirical and clinical perspective.

Full textABSTRACT: PERSONALITY AND PSYCHIATRIC REHABILITATION INTRODUCTION: psychiatric rehabilitation in Therapeutic Communities is aimed to let in care patients regain an adequate relational and occupational functionality, allowing their social reintegration. However, there are no reports in the literature concerning the predisposition to participate to the activities that have been previously agreed upon, according to patients’ individual care plans. Therefore, the relationship between personality traits of borderline inpatients of the Therapeutic Community “Villa Ratti”, assessed using MMPI-2, and percent of interventions performed, is investigated. METHODS: 62 patients (27 Men, 35 Women, mean age=28, sd=6,6) have performed a total of 14540 interventions, comprising 24 categories, between January 2009 and December 2010. MMPI-2 scores and school degree were available only for 50 patients (20 M, 30 W). 4 women of them were clearly outliers because of their extremely low scores in all scales. For the remaining 46 patients (20 M, 26 W), correlations and multiple regressions were analyzed, in order to evaluate predictive values of mmpi 2 scores on the percentiles of interventions. RESULTS: some mmpi-2 scales were significantly correlated with 8 out of 24 percent interventions. Moreover, some scales were significantly predictive, with R2 values spanning the range 0,1 - 0,4. However, not all scales resulted to be predictive: Lie (L), Hypochondria (Hs) and Psychopathic Deviate (Pd) do not exert significant effects onto the attendance to activities. CONCLUSIONS: In this study, significant relationships between some of the mmpi-2 scores and percentiles of interventions performed, were found. These results give an indication of which aspects of personality can affect the tendency of patients to partecipate in specific activities, independently of what had been previously decided in the Therapeuthic-Rehabilitative Project. The limits of this approach and the future potential developments are discussed.

Full textThis research is an effort to explore, from a functional analytic perspective, the feelings that dealing with borderline clients causes in therapists. First, the therapeutic relationship is described as an instrument that can promote changes in the client s repertoires. This approach considers that if an individual behaves in relation to a therapist in the same way as he behaves in his daily life in relation to other people, these settings are functionally identical. So, changes in one context, will affect the others. Besides, the client s behaviors cause feelings in the therapist (a phenomenon known in psychotherapy literature as counter-transference) that can be used as cues about how other people feel in the client s daily life. So, the therapeutic relationship would be a context that makes in-vivo learning possible for the client, since the behavior can be immediately reinforced or weakened after its occurrence. Four behavioral therapists, who worked with borderline clients, took part in this research. The sessions were conducted weekly and lasted one hour each. Reports by the therapists concerning their feelings were recorded during semistructured interviews and during monthly supervisions with each therapist separately. All recordings were transcribed and explored according the methods of grounded theory. The results indicate that the therapists noted the occurrence of clinically relevant behaviors and countertransference in the therapeutic relationship. During supervision it was tried to lead the participants to use their own feelings (about the client) as cues to promote changes in clients behaviors. They reported that their clients punished many of their new behaviors they were coerced to continue reacting in ways that were complementary to the client s behaviors. The therapists what went back to behave in a way that was safe for them reinforced the problem behaviors of their clients. This counter-therapeutic behavior seems related to experiential avoidance. The results suggest that particular attention to interpersonal interactions during the session is important. It could avoid that the therapeutic relationship maintains the client s problems and can make it possible to take better advantage of the therapist s private responses.
O presente trabalho é um esforço em explorar sentimentos que o atendimento ao cliente borderline provoca em terapeutas, a partir de uma visão analítico-funcional. Primeiro, a relação terapêutica é descrita como um instrumento de mudança que pode promover mudanças no repertório do cliente. Esta abordagem parte do pressuposto de que se o indivíduo se comporta em relação ao terapeuta da mesma maneira que o faz no seu dia a dia com outras pessoas, esses ambientes são funcionalmente idênticos, e que, portanto, se um deles for mudado, esta alteração afetará os demais. Além disso, os sentimentos que os comportamentos do cliente provocam no terapeuta (fenômeno conhecido na literatura psicoterápica como contra-transferência) podem servir como dicas sobre o que outras pessoas do seu cotidiano sentem. Desta forma, a relação terapêutica seria um contexto que possibilita o aprendizado ao vivo para o indivíduo, já que o comportamento pode ser reforçado ou enfraquecido imediatamente após a sua ocorrência. Participaram deste trabalho, quatro terapeutas comportamentais que atendiam clientes borderlines. Elas realizavam os atendimentos semanalmente com sessões de uma hora de duração. Relatos de sentimentos dos terapeutas foram gravados durante entrevistas semiestruturadas e durante supervisões mensais com cada terapeuta separadamente. Todas as gravações foram transcritas e exploradas de acordo com os métodos da teoria fundamentada nos dados. Os resultados apontaram que os terapeutas percebem a ocorrência de comportamentos clinicamente relevantes e de contra-transferência na relação terapêutica. Tentou-se, nas supervisões, levar as participantes a utilizar dos seus próprios sentimentos em relação ao cliente, como dicas para atuar de modo a promover mudanças no repertório do cliente. Elas relataram que muitos dos novos comportamentos adotados por elas, foram punidos pelos seus clientes sendo isto coerção para continuar reagir de forma complementar aos seus comportamentos. As terapeutas, que voltavam a se comportar de modo mais seguro para si reforçaram os comportamentos problemáticos dos clientes. Tal comportamento contra-terapêutico parece relacionado com a esquiva experiencial. Os resultados sugerem que atenção particular para os padrões interpessoais durante a sessão poderia evitar que a relação terapêutica chegasse a manter os problemas do cliente e que um melhor proveito das respostas encobertas do terapeuta para este fim é possível.

Full textIn post-modernity, the psychoanalytical practice has been facing the challenge of dealing with changes in subjectivity and new psychopathological disorders. In this scenario, it comes the Borderline patient, bringing up challenges and opportunities in psychoanalysis that are gaining more relevance in the clinical and scientific discussions of the country, requiring analysts dedication to reflect on how to handle this sort of patiens within the clinical practice. Given this reality and in search for a more clinically effective approach, the objective of this survey was to deepen the current insights on the Borderline Personality disorder, with the intention of undertanding specific aspects of transference and countertransference demand that are experienced in the psychoanalysis process. This work was accomplished through a qualitative approach based on the clinical method of psychoanalysis, following the theoretical framework of Donald Winnicott. Having reflected on how to manage a borderline case, this survey outlines specific issues concerning transference and countertransference. It was observed that the problem lies at the confluence of the existential suffering of the patient and the countertransferencial suffering of the analyst, that requires professional sensitivity so as to ensure an appropriate approach aligned with the patient needs, indeed considering its own limits. Na pós-modernidade, a clínica psicanalítica vem se deparando com o desafio de lidar com mudanças na subjetividade e novas formações psicopatológicas. Neste cenário, está o paciente borderline, cujos desafios e possibilidades no atendimento vêm ganhando cada vez mais espaço nos meios clínicos e científicos do país e exigindo dos analistas tempo para refletir sobre como manejá-los clinicamente. Diante desta realidade e em busca de uma clínica mais efetiva, o objetivo deste trabalho foi aprofundar o estudo acerca do Transtorno de Personalidade Borderline, para compreender aspectos específicos da demanda transferencial e da contratransferência experimentados nesse atendimento. Tratou-se de um trabalho de abordagem qualitativa, baseado no método clínico da psicanálise, seguindo o referencial teórico winnicottiano. Ao refletir sobre o manejo do borderline, foram sublinhadas demandas específicas assentadas nas questões da transferência e da contratransferência. Observou-se que a problemática reside na confluência do sofrimento existencial deste paciente com o sofrimento contratransferencial do analista, exigindo do profissional sensibilidade para a realização de um manejo ajustado às necessidades de cada paciente, considerando também seus próprios limites. Desta forma, a análise funcionaria como um caminho para a construção de um self mais integrado

This dissertation had the goal of presenting Winnicott s comprehension of the borderline disorder, how the defense false self in such cases is developed, and the technic for clinic handling, a valuable contribution from Winnicott to the psicoanalysis practice. Inicially, the origin of the term borderline in Psichiatry is presented as well as some contributions to the comprehension of the case from Psicoanalysis authors, emphasizingwith the innovations proposed by Winnicott s publications. For the author, the borderline disorder presents as main characteristic the false self defense, being necessary a review of general aspects of the theory of psychosis in connection with the theory of personal development to comprehend the disorder, and to clarify concepts like the environment, trauma, and unthinkable agony. It is relevant the importance that the author gives to the handling, innovative and original technique which allows for the analyst to adapt to the patient s real needs, offering the necessary holding for the patient s regression to a stage of absolute dependency, a moment in which the maturing development was interrupted because of a environmental deficiency. Finally, synthesis of borderline cases held by the author are presented, reinforcing the relevance of those patients going through the experience with the analyst, more than the use of interpretations of verbal contents during the treatment
A presente dissertação teve como objetivo apresentar a compreensão winnicottiana sobre o distúrbio borderline, como se forma a defesa falso si-mesmo nesses casos e a técnica do manejo clínico, uma contribuição valiosa de Winnicott para a prática psicanalítica. Inicialmente, é apresentada a origem do termo borderline na Psiquiatria e algumas contribuições de autores da Psicanálise para a compreensão do quadro, com destaque para as inovações propostas na obra de Winnicott. Para o autor, o distúrbio borderline tem como característica principal a defesa falso si-mesmo, sendo necessário para compreendê-la retomar aspectos gerais da teoria das psicoses em conexão com a teoria do amadurecimento pessoal, de forma a esclarecer conceitos como ambiente, trauma e agonia impensável. É destacada a importância que o autor atribui ao manejo, técnica inovadora e original que permite ao analista se adaptar às reais necessidades do paciente, oferecendo o holding necessário para que ele possa regredir ao estágio de dependência absoluta, momento em que o desenvolvimento maturacional foi interrompido devido à falha ambiental. Por fim, são apresentadas sínteses de casos borderline atendidos pelo autor, que enfatiza a importância de esses pacientes vivenciarem experiências com o analista, mais do que o uso interpretações de conteúdos verbais durante o tratamento
