Abstract:
This study helps to understand what methods are used by counseling teachers in overcoming school skipping students. This research is descriptive qualitative with case study research. The data source of this study were two counseling teachers, namely Sartini Cut and Novrida Yanti and one class VIII student at SMPN 3 Lengayang West Sumatra. Data collection is done by interview method, and collection. The results of the study showed that the individual counseling method could overcome school absenteeism in the eighth grade students of SMPN 3 Lengayang West Sumatra by the directive, non directive and eclectic method. This third method is given twice to students and begins with directive counseling which is important is the guidance of counseling teachers, after that counseling is non-directive, namely students who are more suitable in discussing problems and eclectic counseling, while teachers with students are equally active in solving problems. With this method, the counseling teacher can make a diagnosis regarding the problems experienced by the student. Keywords: Individual Counseling, School Ballos

Abstract:
In the People’s Republic of China, telephone psychological counseling is a new phenomenon that appeared in the late 1980s, following a social transition toward a market economy. Such counseling by paraprofessionals often adopts a directive style in that the counselor gives direct advice pertaining to what the client should do on a particular issue. After a brief description of its features, this article examines factors of cultural traditions and transitions underlying the directive counseling and the interactive relationship between counseling and culture. Analysis is made regarding traditional and transitional values, social networks, and personality types. It is further argued that research and training in cross-cultural counseling should go beyond static cultural relativism and adopt an emic view of cultural change.

This study aims to determine the methods used by counseling teachers in dealing with student friendship conflicts. This research is descriptive qualitative with case study research. The data source of this research is counselor guidance and counseling is Yuni Heru Kusumawardani, guidance and counseling teacher Mr. Ruba'i and two students of class X MAN 2 Sleman. Data collection was done by interview method, documentation and observation. The results showed that the method of individual counseling to cope with conflicts friendship between students of class X MAN 2 Sleman is a directive method and eclectic method. The directive method is given to the PPA at the first meeting until the third, whereas APE is given at the first and second meeting, the directive method is used when the counseling teacher searches the data or digs the information about the problem that is being experienced by the student, in this directive method the counseling guidance teacher can make a diagnosis related to the problems experienced by the students at that time. Furthermore, the eclective method is given to the PPA at the fourth to sixth meetings, whereas the APE is given at the third and fourth meetings, the eclective method is used when the counseling teacher checks by hearing what statements and decisions or commitments the student will make on the method This counseling teacher's eclectic counseling can conduct joint evaluations with the students.Keywords: Individual Counseling, Conflict Friendship

The data collecting method were done using interview and observation. The data analysis was done using path model from Kemmis and Taggart.The research concludes that: 1) the directive supervision to improve the classroom teachers ability in providing the the guidance and counseling was done as follows: (a) the supervisor socialize the supervision program to the teachers; (b) the supervisor arranged the supervision schedule; (c) the supervisor provide the supervision materials; (d) the supervisor provide the guidance and counseling simulation; and (e) the supervisor asses the teachers’ ability in providing the guidance and counseling; and 2) The directive supervision is effective in improving the classroom teachers ability in providing the the guidance and counseling. It is demonstrated with the improvement in the teachers’ scores in each cycles.

Abstract This study examined navigational barriers to advance care planning (ACP) and advance directive (AD) completion among older Latinos with chronic illness. Older Latinos over the age of 50 were randomized to education only versus education plus counseling (N=61; education only n=31, education/counseling n=30). Exploratory factor analyses and logistic regression show relationships between navigational barriers, depression, anxiety and AD documentation. Barriers to ACP included difficulty understanding the doctor, fears about hearing bad news, worries about illness getting worse, and difficulties due to language, family conflict, and limited family discussions about dying. Treatment group was most significant contributing factor to AD completion (OR=4.213, CI=1.126 – 15.760, p=<.01). Findings indicate the importance of examining a variety of factors that could influence AD documentation. This study contributes to the literature by identifying the need for targeted and culturally relevant interventions that promote ACP education, counseling, communication, support, and AD documentation among older Latinos.

Counseling interventions with paranoid schizophrenics can be daunting. While chemical, directive, and behavioral controls often are considered important, nondirective counseling techniques used by the therapeutic staff may help schizophrenic patients explore their thoughts and feelings. Several nondirective concepts pioneered by Carl Rogers are examined. These methods, which represent basic concepts of the person-centered approach, are empathy, unconditional positive regard, and congruence. A brief illustration of an interaction with a patient diagnosed as paranoid schizophrenic is presented to suggest the effectiveness of Rogerian counseling.

Objective: To describe the ethical concept of the fetus as a patient and identify its implications for the deliberative practice of perinatal medicine.Methods: We describe secular medical ethics and its two fundamental principles, beneficence and respect for autonomy. We articulate the ethical concept of the fetus as a patient on the basis of the ethical principle of beneficence.Results: In the deliberative practice of perinatal medicine guided by the ethical concept of the fetus as a patient, the perinatologist should always identify and balance beneficence-based obligations to the fetal patient and beneficence-based and autonomy-based obligations to the pregnant patient. Directive counseling is appropriate when the fetus is a patient. Non-directive counseling is appropriate when the fetus is not a patient.Conclusion: Counseling pregnant women about the clinical management of their pregnancies should always identify and balance beneficence-based obligations to the fetal patient and beneficence-based and autonomy-based obligations to the pregnant patient.

Comprehensive guidance and counseling programs are based on achievement of developmental tasks, potential development, and alleviation of counselee problems. Developmental tasks are formulated as competency standards to be achieved by counselors, so this approach is also called standard-based guidance and counseling. Standards of student competence refers to Standards Independence Competence Learners (SKKPD) which is divided into 11 aspects of development. Successful implementation of comprehensive guidance and counseling program is the fulfillment of six characteristics, namely holistic, systematic, balanced, proactive, integration in school curriculum, and reflection. Procedures in the preparation of comprehensive guidance and counseling programs are planning (planning), organizing (organizing), implementation (penging), and evaluation (evaluation). Implementation of guidance and counseling programs is supported by the availability of skilled human resources. Teacher guidance and counseling / counselors are expected a set of knowledge and skills that support the implementation of the program. A full knowledge of developmental theory. Counselor's skills are to coordinate with various parties in implementing guidance and counseling program. The counselor has a strong leadership in running the guidance program, and has a proportional time management skills that is 80% of the time counseling and guidance services are directive in the learners while 20% of the service time is for the activity of the program management and system support

The ArgumentA survey of 2901 genetics professionals in 36 nations suggests that eugenic thought underlies their perceptions of the goals of genetics and that directiveness in counseling after prenatal diagnosis leads to individual decisions based on pessimistically biaed information, especially in developing nations of Asia and Eastern Europe. The “non-directive counseling” found in English-speaking nations is an aberration from the rest of the world. Most geneticists, except in China, rejected government involvement in premarital testing or sterilization, but most also held a pessimistic view of persons with genetic disabilities. Individual, but not state-coerced, eugenics survives in much modern genetic practice.

Penelitian ini bertujuan untuk mengetahui hubungan penerapan model non directive teaching terhadap motivasi belajar bahasa Arab siswa SMK Muhammadiyah Prambanan. Penelitian ini menggunakan metode kuantitatif dengan desain korelasi. Instrumen penelitian yang digunakan adalah wawancara, observasi, dokumentasi, dan kuesioner yang disebarkan kepada 104 siswa dari jumlah seluruh populasi sebesar 245 siswa kelas XI yang selanjutnya dilakukan uji validitas, reliabilitas, normalitas, dan linearitas. Untuk mengetahui besar nilai signifikansi hubungan dan pengaruh model non directive teaching dengan motivasi belajar dilakukan uji korelasi dan uji regresi. Dari perhitungan menggunakan rumus koefisien korelasi product moment, diketahui terdapat hubungan yang positif dan sangat signifikan antara penerapan model non directive teaching dengan motivasi belajar bahasa Arab siswa kelas XI di SMK Muhmmadiyah Prambanan sebesar rxy 0,428 dan sumbangan efektif penerapan model non directive teaching terhadap motivasi belajar bahasa Arab hanya sebesar 18,3% (R square 0,183) sedangkan sebesar 71,3 % motivasi belajar bahasa Arab siswa dipengaruhi oleh faktor lain. Oleh karena itu, untuk perbaikan kualitas pembelajaran, penerapan model non directive teaching perlu dikemas dengan berbagai metode pembelajaran yang mendukung, baik untuk proses non directive counseling dan maupun tujuan pembelajaran yang diinginkan. Hal ini dikarenakan motivasi belajar siswa adalah variabel yang dipengaruhi oleh berbagai factor.

Background Studies show that combining nicotine replacement therapy (NRT) with tobacco treatment counseling is most effective for smoking cessation. However, tobacco treatment counseling has been underutilized across the nation. A secure email message sent to patients already taking NRT was hypothesized to increase the utilization of tobacco treatment counseling among Veterans in New Jersey. Secure messaging for communication between patients and providers was implemented through a web-based password-protected, secure messaging account, where Veterans get notified through their personal email when they have a message awaiting them. Objective The main objective of this project was to determine if there was a significant increase in adoption of tobacco treatment counseling among Veterans who received a secure message describing the options for tobacco treatment counseling available to them. Secondary objectives were to demographically characterize Veterans who were and were not enrolled in secure messaging, as well as those who opened or did not open a message. Finally, because the language and content of the messages were changed across project phases, this project also sought to determine (by analysis of response rates) the type of language that was most effective at eliciting a response. Methods Over two phases, messages were sent to two samples of Veterans prescribed NRT within the prior 90 days of each phase. In phase 1, one message was sent in December 2015 (message 1). In phase 2, one message was sent in July 2016 (message 2) and the same message (message 3) was resent in August 2016 to persons who did not open message 2. Messages 2 and 3 were more directive than message 1. Response rates to message 1 versus message 2 were compared. A logistic regression analysis determined effect of age and gender on enrollment in secure messaging across both phases. The effectiveness of each phase at increasing tobacco treatment counseling was analyzed using a McNemar test. Results Message 2, sent to 423 Veterans, had a significantly higher response rate than message 1, sent to 348 Veterans (18%, 17/93 vs 8%, 6/78, P=.04). Phase 2 (ie, messages 2 and 3) significantly increased utilization of tobacco treatment counseling (net increase of six tobacco treatment counseling adopters, P=.04), whereas phase 1 (ie, message 1) did not (net increase of two tobacco treatment counseling adopters, P=.48). Women (odds ratio [OR] 1.6, 95% CI 1.1-2.3) and those aged 30 to 49 years (compared to other age groups) were more likely to be enrolled in secure messaging. Gender and age were not significant predictors of opening or replying to either message. Conclusions Although the effect was small, secure messaging was a useful modality to increase tobacco treatment counseling. Directive content with a follow-up message appeared useful. Female Veterans and/or Veterans aged between 30 and 49 years are more likely to use secure messaging.

AbstractIf the worries about the coronavirus disease 2019 (COVID-19) pandemic are not already enough, some pregnant women have been questioning whether the hospital is a safe or safe enough place to deliver their babies and therefore whether they should deliver out-of-hospital during the pandemic. In the United States, planned out-of-hospital births are associated with significantly increased risks of neonatal morbidity and death. In addition, there are obstetric emergencies during out-of-hospital births that can lead to adverse outcomes, partly because of the delay in transporting the woman to the hospital. In other countries with well-integrated obstetric services and well-trained midwives, the differences in outcomes of planned hospital birth and planned home birth are smaller. Women are empowered to make informed decisions when the obstetrician makes ethically justified recommendations, which is known as directive counseling. Recommendations are ethically justified when the outcomes of one form of management is clinically superior to another. The outcomes of morbidity and mortality and of infection control and prevention of planned hospital birth are clinically superior to those of out-of-hospital birth. The obstetrician therefore should recommend planned hospital birth and recommend against planned out-of-hospital birth during the COVID-19 pandemic. The COVID-19 pandemic has increased stress levels for all patients and even more so for pregnant patients and their families. The response in this difficult time should be to mitigate this stress and empower women to make informed decisions by routinely providing counseling that is evidence-based and directive.

This study attempted to characterize the barriers and stimuli for cancer GC among unaffected women at high risk for breast cancer. Methods: IRB approval was obtained for this qualitative interview study. 54 unaffected patients who received a cancer genetics referral from a high-risk breast clinic were invited to participate. 18 subjects completed written questionnaires and 60-min telephone interviews, which were audiotaped and transcribed. A detailed coding system was developed and N*Vivo software was used in order to identify themes in the factors affecting intent to pursue GC. Results: We identified four themes in the factors affecting uptake of GC: 1)patient understanding of GC, 2)patient awareness of increased cancer risk, 3) family-tied life events (e.g. reproduction, caretaking, death), and 4) support for GC from physicians and family. The most common theme was patient understanding of GC. Subjects did not seem to differentiate between GC and GT, and they deferred GC until they were prepared to undergo GT. Conclusion: We identified barriers and stimuli for GC that were similar to those previously described for GT. The misconception that GC automatically leads to GT, and the subsequent low uptake of GC referrals, may be influenced by the current practice of non-directive referrals. Providing more directive referrals for GC, without being directive about GT, may reduce barriers to GC. No significant financial relationships to disclose.

<em>The purpose of this study is to describe (1) the form of directive speech acts in JK Rowling's Harry Potter novels year 1 and 2, (2) the form of language politeness, (3) the relevance of the Harry Potter novelsyear 1 and 2 in Indonesian language learning to students in middle school. The data collection technique used in this study was purposive sampling and data analysis used an interactive analysis model. The results of this study indicate that the form of directive speech acts and compliance with the principles of politeness contained in the Harry Potter novels year 1 and 2 by J. Rowling are as follows: (1) speech acts; requests, questions, rules, prohibitions, granting permits, and directive forms of counseling which numbered 419 data; (2) compliance with the principles of teacher and student politeness during the learning process is found as many as 118 data; (3) Harry Potter novels year 1 and 2 by J. K. Rowling are related to literary learning in junior high schools.</em>

Background: When using Motivational Interviewing (MI), once resistance or ambivalence are resolved and motivation is solidified, many practitioners struggle with how best to transition the discussion toward action planning, while still retaining the spirit and style of client centeredness, i.e., moving from the WHY phase to the HOW phase of counseling in a style that is MI-consistent. For many, there is a perception that the counseling style, skills, and strategies used to build motivation are distinct from those used in the action planning phase. The WHY to HOW transition does not, however, necessitate abandoning a client-centered style for a more overtly educational or directive style. Goal setting, action planning, provision of advice, and relapse prevention can be implemented from an autonomy supportive, MI consistent framework. Method: To this end, this article will present a new class of reflection, which we have termed “action reflections”, that can be used to help bridge the WHY-HOW gap. Action reflections (AR) allow the clinician to maintain a tone and orientation that are consistent with MI, i.e. autonomy support; guiding versus directing, during the action phase of counseling. They differ from reflecting change talk as they focus not on the WHY of change, but the HOW, WHEN, or WHERE. Action Reflections (ARs) also differ from the more common type of reflections such as those that focus on client feelings, rolling with resistance, or acknowledging ambivalence as ARs usually contain a potential concrete step that the client has directly or obliquely mentioned. Like any type of reflection, ARs represent the clinician's best guess for what the client has said or, more apropos here, where the conversation might be heading. Conclusion: This article describes the various types of ARs and provides examples of each to help clinicians incorporate them into their behavior change counseling.

Abstract Since the introduction of non-invasive prenatal testing (NIPT) in Belgium in 2013, expectant parents have had access to a new efficient and reliable test for genetic abnormalities, including trisomy 13, 18 and 21. In 2017, the Belgian government decided to cover over 95% of the cost for all women who choose to take this test. In this context, a diagnosis of trisomy 21 (Down syndrome) presents an especially challenging decision for parents: it is impossible to tell the severity of impairment prenatally, and although several medical risks are involved in the condition, many individuals with Down syndrome live a long, happy, and relatively independent life (Shakespeare, 1998; Scott et. al., 2014). Expectant parents face a very difficult situation: how to decide whether or not to terminate a pregnancy? It appears that termination is becoming the routinized outcome of the clinical encounter during which a diagnosis is delivered.The intricacy of the matter, and the fact that many actors are involved, lead to the necessity of a transdisciplinary methodology when scrutinizing the future of prenatal counseling. However, medical professionals tend to approach Down syndrome from an exclusively medical perspective (Skirton and Barr, 2010), leaving aside the economic, social and psychological dimensions of living with Down syndrome. This may result in unbalanced information; some (perhaps precipitously) refer to this phenomenon as ‘nudging practices’ (Hippman et. al., 2012; Reid et. al., 2009). In order to address this imbalance, we adopt a stakeholder approach in this project: we consulted <target target-type="page-num" id="p-39"/>experts with diverse backgrounds and fields of expertise. Through this transdisciplinary approach, we hope to bring about a more transdisciplinary perspective directly within the offices of gynecologists and GPs. More specifically, we aim to find ways to provide GPs and gynecologists with direct access to information about the aforementioned dimensions of Down syndrome.The next step is to define a strategy to achieve this goal. Given the fact that medical professionals often lack both time and easy access to concise information about Down syndrome to present a nuanced perspective (Ahmed et. al., 2007), but are still the first in the prenatal screening process, the output of this project is the idea of an online tool in a succinct Q&A format. The goal of such a website would be to provide up-to-date, easily accessible and balanced information for general practitioners and gynecologists on multiple aspects of Down syndrome. This way, in anticipation of and directing explicitly towards further counseling by a multidisciplinary team, physicians can provide expectant parents with a more balanced, transdisciplinary view of Down syndrome, thereby enhancing their capability to make informed, autonomous and hence sustainable decisions about their pregnancies.

Background: End-of-life (EOL) care for Latinos with chronic illness is a critically important problem. Latinos with chronic illness suffer worse health outcomes and poorer quality of care due to various issues occurring in care delivery systems. Latinos are less likely than non-Hispanic whites to prepare an advance directive (AD) for health-care decision-making that impacts treatment decisions for when EOL is near. Advance care planning (ACP) interventions tailored specifically for Latinos have rarely been implemented. Objective: The primary aim examines whether a motivational interviewing (MI) intervention increased rates of AD documentation among older Latinos. The secondary aim was to examine whether MI improved communication with providers and family members. Methods: We pilot tested a randomized controlled trial with older Latinos >50 years with one or more chronic illnesses, including cancer. Participants were randomly assigned to usual care (UC) receiving ACP education alone versus treatment (TX), which received ACP education, plus MI counseling including interactive decisional support, emotional support, and barrier navigation. Results: Results of logistic regression indicate TX group participants were significantly more likely to document an AD than UC, however were less ready to talk with health-care providers or family members. Those reporting navigational barriers for talking about dying is difficult showed a significant negative relationship for AD completion even with significant intervention effects. Conclusion: When using MI to motivate individuals toward ACP EOL conversations other factors are important to consider. Further research is needed, especially among Latinos to understand best practices for ACP education and counseling for EOL care.

During the 1950s and 1960s, the disabled child stood at the intersection of two distinct but connected developments: the emergence of more psychosocial approaches to disability, and the rise of child analysis as a distinct field. Disabled children and their families came into the focus of psychoanalysts, and became testing cases for theories about ego development, family dynamics, grief, mourning, and loss. This psychoanalytic subfield influenced other professions, not least because it played on widespread stereotypes about disability and motherhood. It was picked up, for example, by geneticists as part of a larger refashioning of their field. In a superficial distancing from the coercive, population-driven eugenics of the past, geneticists redefined genetic counseling as a non-directive psychosocial service that was to resolve parental guilt, blame, shock, and anxiety in order to facilitate ‘rational’ reproductive decisions. Psychoanalytic motives thus filled a need for establishing new forms of professional authority, and for defining ‘effective’ counseling.

Несмотря на очевидную полезность генетических рекомендаций для консультируемого и членов его семьи, а также факт оплаты им услуги консультирования, он (консультируемый) этим рекомендациям часто не следует, подвергая и свою жизнь, и жизнь близких опасности. Как понять эту странность? В качестве виртуального участия в гибридном форуме, обсуждаются два казуса из практики генетического консультирования, которые, при существенном сходстве ситуации, отличаются друг от друга наличием или отсутствие доверия пациентов к научному знанию и экспертным суждениям генетика консультанта. В целях теоретического истолкования поставленной проблемы вводится предположение о самости как коммуникативной дисфункции, которая диагностируется в различных ценностных координатах либо как некомплаентность (noncomplience) в директивных моделях консультирования, либо как отказ от сотрудничества (nonadherence) в недирективных. Если же учесть, что в основе медицины как культурной практики лежит чувство солидарности перед лицом телесного страдания, то в этом плане странное, контрпродуктивное поведение пациента может указывать на наличие глубоких проблем в основах современной социальности. Предлагается формулировка нарративного императива. The article analyzes communication dysfunctions between patients, consulting geneticists and doctors in the practice of medical genetic counseling. Despite the obvious usefulness of genetic advice for the counseled and their family members, as well as the fact that patients pay for counseling services, they (the counseled) often do not follow these recommendations, putting both their own lives and the lives of the loved ones at risk. How to understand this oddity? As a virtual participation in a hybrid forum, two cases from the practice of genetic counseling are discussed, which, with a significant similarity of the situation, differ from each other in the presence or absence of patient confidence in the scientific knowledge and expert judgments of the geneticist. In order to theoretically interpret the problem posed, an assumption is introduced about the self as a communication dysfunction, which is diagnosed in various value coordinates either as noncompliance in directive models of counseling, or as nonadherence in non-directive ones. If we consider that medicine as a cultural practice is based on a sense of solidarity in the face of bodily suffering, then, in this regard, the patient’s strange, counterproductive behavior may indicate the presence of deep problems in the foundations of modern sociality. The formulation of the narrative imperative is proposed.

Advance care planning (ACP) does not readily occur in medical settings and often gets missed. Older Latinos need ACP information to encourage advance directive (AD) completion indicating preferences for end-of-life (EOL) care. Objective: To explore the experiences with counseling using motivational interviewing (MI) techniques and social workers to encourage ACP communication among older Latinos with advance chronic diseases. This study describes stages of readiness to plan for EOL care. Design: We conducted a qualitative study with older Latinos who participated in a community-based intervention in Southern New Mexico. Methods: Participants in the intervention were selected because they received ACP education plus counseling involving MI to address resistance to ACP. Motivational interviewing counseling involved the following: (1) engaging in structured dialogue about ACP, (2) using and completing AD documentation, (3) encouraging ACP communication with providers and families, and (4) applying AD information into actionable behavior. We utilized a constant comparative method and thematic analysis to explore the meaning of older Latinos’ experiences with MI counseling and stages of change. Results: Participants (n = 32) were mostly women (74.3%), half born in the United States and half from Mexico in the United States on average for 31.75 (standard deviation 16.22) years. Many had less than sixth grade education (31.3%) or had not completed high school (21.9%). Key themes indicate the following stages of change: (1) precontemplation, (2) contemplation, (3) preparation, (4) ACP action, and (5) maintenance. Conclusion: This study contributes to the literature by identifying areas for adaption to enhance understanding and increase information to ultimately achieve the completion of ACP among Latinos.

The data in this study are words, terms, phrases, sentences, and discourses containing directive speech act. The source of data is taken from educational posters found on educational websites. Data collection in this study uses a scrutiny method refer to technique of record. Data analysis uses pair method. The result shows that based on speech mode, the directive speech strategy used includes direct and indirect strategies. Six functions of speech act are found in education posters, namely (1) prohibiting function, (2) inviting function, (3) command function, (4) counseling function, (5) satire / criticizing function, and (6) ) request/appeal function. Kajian ini membahas tindak tutur direktif dalam poster pendidikan. Kajian yang dilakukan bertujuan untuk mengetahui strategi dan fungsi tindak tutur direktif dalam poster pendidi- kan. Data dalam penelitian ini berupa data yang berwujud kata, istilah, ungkapan, kalimat, wacana yang mengandung tindak tutur direktif. Adapun sumber data diambil dari poster pendidikan yang ditemukan dalam situs-situs pendidikan. Pengumpulan data dalam penelitian ini menggunakan metode simak dengan teknik catat. Analisis data menggunakan metode padan. Hasil penelitian menunjukkan bahwa berdasar modus tuturannya, strategi tindak tutur direktif yang digunakan meliputi strategi langsung dan strategi tidak langsung. 7 Background: In Colorado, there is no legal designation as to who is authorized to make healthcare decisions for an adult unable to make decisions for themselves. Completing an advance directive is increasingly associated with better end-of-life outcomes. Failure to complete an advance directive leads to fewer days on hospice and expensive, end-of-life care. It was hypothesized that patients who completed a Medical Durable Power of Attorney (MDPOA) document early may have higher adoption of advance care planning and earlier hospice at end-of-life. Methods: The Rocky Mountain Cancer Centers (RMCC) is the largest private oncology group in Colorado with multiple offices across the state. The RMCC-Pueblo clinic developed and piloted a new patient process where, at registration, all new patients were invited to complete a MDPOA while also completing other new patient forms. The Medical Durable Power of Attorney (MDPOA) is a document that appoints a healthcare decision maker (“healthcare agent”). Data on 4,441 patients from Nov. 1, 2017, to June 30, 2018, were extracted from the electronic health record (EHR), including code status, advance care planning counseling appointment, advance directive completion, and outcomes. Results: During this pilot, advance directive capture within the EHR rose from 15.4% to 21.4%. While the number of patients who elected hospice enrollment at the end of life was about the same, the length of hospice stay increased from an average of 14 to over 20 days, a 50% improvement. Conclusions: A MDPOA as a routine part of the new patient intake process is feasible and scalable. It opens the door to future ACP discussions and improved hospice utilization. Because of these findings, we are currently expanding this process to all RMCC practice sites. The results suggest that early completion of an Advance Directive lead to ongoing and healthy discussions about the patient’s end-of-life preferences for care, including appropriate hospice enrollment.

The purpose of this article was to describe the methodology and findings related to the treatment fidelity of three face-to-face motivational interviewing sessions involving middle school girls and a school nurse to help the girls increase their moderate-to-vigorous physical activity. The following four areas related to treatment fidelity were addressed: (a) study design, (b) training of interventionists, (c) intervention delivery, and (d) intervention receipt. Findings showed that 34 of 37 (91.9%) girls completed all three sessions. An initial motivational interviewing training workshop followed by evaluation of audiotaped sessions with constructive feedback can result in successful and consistent delivery by a school nurse.

Abstract:
This study examined the molecular characterization of a prenatal case with true fetal mosaicism of small supernumerary marker chromosome 16 (sSMC(16)). A 41-year-old female underwent amniocentesis at 19 weeks of gestation due to advanced maternal age. Chromosomal analysis for cultured amniocytes revealed a karyotype of 47,XY,+mar[4]/46,XY[16]. Spectral karyotyping and metaphase fluorescence in situ hybridization (FISH) demonstrated that the sSMC was derived from chromosome 16 (47,XY,+mar.ish der(16)(D16Z1+)[13/20]). Confined placental mosaicism was initially suspected because the prenatal ultrasound revealed a normal structure and the pregnancy was uneventful. However, interphase FISH of cord blood performed at 28 weeks of gestation showed 20% mosaicism of trisomy chromosome 16 (nuc ish(D16Z2×3)[40/200]). Chromosome microarray analysis further demonstrated 55% mosaicism of an 8.02 Mb segmental duplication at the subcentromeric region of 16p12.1p11.1 (arr[GRCh37] 16p12.1p11.1(27021975_35045499)×3[0.55]). The results demonstrated a true fetal mosaicism of sSMC(16) involving chromosome16p12.1p11.1 that is associated with chromosome 16p11.2 duplication syndrome (OMIM #614671). After non-directive genetic counseling, the couple opted for late termination of pregnancy. This case illustrated the use of multiple molecular cytogenetic tools to elucidate the origin and structure of sSMC, which is crucial for prenatal counseling, decision making, and clinical management.

192 Background: The newest Commission on Cancer (CoC) standards focus on patient-centered care across the cancer continuum and include palliative care (PC) and psychosocial care (PS). Little is published regarding the challenges of implementing these standards in community oncology settings. To help examine the integration of PC and PS and to identify barriers and best practices, an assessment was conducted by sites selected to take part in the National Cancer Institutes’ Community Cancer Centers Project (NCCCP). Methods: The NCCCP’s goals to improve quality of care and reduce healthcare disparities included goals for expansion of PC and PS services. Volunteers formed workgroups using monthly teleconferences to share best practices and resources. A questionnaire based on the National Quality Forum (NQF) preferred practices was completed (January-April 2012) by 22 of 29 sites. Results: All respondents have inpatient PC available, 65% in the outpatient setting; 39% described PC as an integral part of their cancer program. All have designated outpatient staff providing symptom management and rehabilitation services. Nurse practitioners and physician assistants are used at 78% of the centers. Interdisciplinary staff includes pharmacists and dietitians; integrative/complementary medicine is found in 57% of the centers. All have integrated PS services: social workers are available in 91% of sites, 30% have psychiatrists, and 30% have psychologists. Most perform psychosocial screening using the NCCN Distress Management tool; 74% have standardized processes. Spiritual support is present in 70% of programs, sexual counseling services in 26%. The majority provides bereavement support. 48% have an outpatient advanced directive program. Inpatient hospice beds are available in 52% of the institutions and 22% own a hospice. Conclusions: Challenges remain with implementation of psychosocial screening and care. Programs to address sexual counseling and advanced directives are lacking. Main concepts of a theory of psychological trauma including seeing trauma as «a double-edged sword», and a concept of posttraumatic personality growth are outlined. Some new data obtained from the most recent (2017—2018) qualitative study among young adults who survived terror act at Beslan school in 2004, are presented. Issues of potential long-term sequels of unmetabolized trauma; of a probability of a secondary traumatization of a patient under conditions of a directive psychotherapy; and probability of unconscious transmission of unmetabolized trauma to the next generations are raised. Fundamental differences between medical and psychological models of psychotherapeutic help including the differences between discourse of «impact» vs discourse of «reciprocity» are discussed. Fallacy of equating of mental suffering and illness is stressed. Psychological counseling and psychotherapy are considered as a transformation of a latent potential of suffering into strengthening of the ego.

Method used to analyze data is qualitative, phenomenology with Interpretative Phenomenological Analysis (IPA) technique. Data are gathered with semi structured, open-ended and non-directive questions. The result of this study is the respondent going through the process of "making-confiding account" and griefing process with the passing of counseling. Theoretical understanding of the implications of the breakup on the practice of clinical psychology will also be addressed in the discussion.

AbstractSince 2007, patients with severe advanced life-limiting illnesses and high, complex symptom burdens have a right to receive specialized outpatient palliative care (SAPV). Multi-professional teams with heterogeneous organizational structures provide care in cooperation with primary care givers, not limited to cancer patients. The aim of SAPV is to foster patient’s autonomy and quality of life. SAPV can be provided as counseling of patient and care givers, coordination of care, additional supportive and full care provision. While the basis of SAPV provision is regulated by a SAPV directive, different contracts between care providers and health care insurances regulate organization, cooperation, definition of care levels, service provision and compensation. Some regions have model contracts that are binding for all SAPV teams in the area; in other regions teams negotiate e. g. compensation, individually with insurances. The article gives an overview of the regulations regarding SAPV.

147 Background: Over the past 2 years a structured advance care planning program (ACP) has been created, piloted and is evaluated monthly at 10 Texas Oncology practices. This program is a systematic process for assessing patient values, is physician supported and is delivered by advanced practice providers at each site. Patient values and quality of life are at the center of the conversations, enhancing patient-centered care and communication with their oncology providers. Targeted patient populations include metastatic lung, pancreatic and colon cancers. Methods: Over the past 14 months, 332 patients engaged in ACP in ten sites. Descriptive statistics for each site were calculated and examined to explore 1) ACP counseling sessions, 2) number of Patient Values questionnaires completed, 3) frequency of code status documentation in the health record, and 4) hospice enrollment for both the targeted and all metastatic populations. Results: A three month review revealed the ACP work within all metastatic populations at the ten sites. Targeted population totals showed continued increase in the percentages of patient ACP engagement at each site. For the practice as a whole, and the targeted population, 9-23% of patients have received an ACP counseling visit. There have been 149 completed Patient Values questionnaires. In the electronic health record, there are 2,233 (24%) patients who have a documented code status. A range of hospice enrollment from 8-28% in the targeted populations has continued to improve hospice length of stay. Conclusions: The patient is invited to explore their treatment options, their personal values and the completion of the advance directive documents. Truthful conversations are critical between patient and provider in order to create a meaningful treatment plan, and to allow the patient to gain understanding about their options. Early palliative care referrals are integrated into each session for symptom management through local partnerships with palliative care physicians. Education regarding life-sustaining interventions help the patient determine their choices for medical care. Having ACP counseling is important at any stage of health and particularly within life-limiting disease.

The University of Maryland Tinnitus & Hyperacusis Center in Baltimore was the first center in the United States dedicated to the evaluation and treatment of tinnitus and hyperacusis patients implementing an habituation-based protocol that has become known internationally as Tinnitus Retraining Therapy (TRT). A crucial feature of the model is the postulate that a number of systems in the brain are involved in the emergence of tinnitus. The cochlea and auditory periphery play only a secondary role. To facilitate the goal of habituation of the tinnitus signal, TRT implements both directive counseling to neutralize the negative emotional associations toward the tinnitus, and sound therapy to interfere with the signal. As an outgrowth of the work with tinnitus, the evaluation and treatment of hyperacusis has emerged as an increasingly important part of our program. This report describes the unique facility, staff, and services of the Center as we celebrate a decade of research and clinical management dedicated to the scientific understanding of tinnitus and hyperacusis.
