Abstract:
This article reviews the contribution of transcranial magnetic stimulation (TMS) research to the understanding of attention, eye movements, visual search, and neglect. It considers how TMS studies have confirmed, refined, or challenged prevailing ideas about the neural basis of higher visual cognition. It shows that TMS has enhanced the understanding of the location, timing, and functional roles of visual cognitive processes in the human brain. The main focus is on studies of posterior parietal cortex (PPC), with reference to recent work on the frontal eye fields (FEFs). TMS offers many advantages to complement neuropsychological patient studies to enhance the understanding of how the fronto-parietal cortical nerves function. The visuo-spatial neglect- and extinction-like deficits incurred by parietal damage have been modelled successfully using TMS. Future work might be directed at teasing apart the distinct functional roles of nodes within this frontoparietal network in different sensorimotor contexts.

Older workers are often shielded from job loss by high tenure, yet are struck particularly harshly when seeking reemployment after job loss. This article combines earlier research on coping with job loss and job search with insights on employability for older workers. We outline the situation of older workers, highlighting their vulnerability to possible job-loss and to stereotypes that may lower their perceived employability. Then we outline how this may place older workers in precarious situations regarding (a) the threat of losing their jobs, (b) suffering from loss of nonmonetary benefits (or latent functions) associated with work, (c) having different and fewer coping options than younger job-seekers, and (d) facing fewer chances of finding reemployment. Older workers face an uphill battle when searching for reemployment, which is partially explained by retirement as an alternative coping reaction to age-related stereotypes, discrimination that undermines older workers’ employability, and other factors.

This book is about decisions in military conflict and the psychological conflict they cause. By drawing on interviews conducted with real soldiers about real situations they faced, the book explores how soldiers choose between seemingly impossible options for which all outcomes appear horrendous. Their vivid and sometimes upsetting stories were part of the experience of war and became part of their identity. This book describes the processes that make choice selection so difficult; the psychology of decisional conflict; and the immediate, short-term, and longer term behavioral consequences of these actions. Special attention is directed to the concept of “decision inertia”—one of several kinds of “failures to act” in which decision-makers are unable to calculate and/or commit to a least-worst course of action. By referring to many real cases, readers are invited to consider their own responses to choosing between difficult options in order to help them understand what it might feel like when faced with making a least-worst decision in war.

In the past, detection of brain metastases signaled the conclusion of aggressive systemic treatment and shifted the focus of care toward palliation. The median survival for patients with single brain metastasis without brain-directed treatment is about a month. Whole brain radiation therapy was the traditional palliative treatment utilized, offering an additional 2 to 5 months. More recently, in addition to whole brain irradiation, the roles of surgery, stereotactic radiosurgery, chemotherapy, and targeted therapies in the definitive management of brain metastases have been investigated in numerous studies. In selected patients, the use of aggressive local therapies can be associated with long survival and good quality of life. This chapter discusses the current state of the art therapeutic options for brain metastases.

Abstract:
Background This report responds to a need to update the available information about incompatible living-donor kidney transplantation (LDKT) previously published in 2014 which, based on 14 case series and 1 cohort study, concluded that this type of transplants could be a therapeutic option with survival, graft and patient outcomes, adequate and similar to a compatible living-donor kidney transplant, however, these report only included ABO-incompatible information. Objective The purpose of the report is to provide updated evidence on effectiveness and safety in terms of graft survival and survival of patients undergoing incompatible LDKT. Method To answer the question a systematic review of the literature was carried out, by updating the structured searches of the previous existing report. The selection of the references was carried out first by title and abstract. Next, the full-text papers were selected by applying the inclusion and exclusion criteria, defined a priori, by a single researcher. In the same way, we proceeded to extract the data from the articles finally selected, and their synthesis in tables similar to those of the previous report, with special attention to the HLA incompatibility LDKT data, since no information was collected in the previous report. Results From 232 localized references, 35 papers on incompatible LDKT have finally been included, 16 with information on HLAi transplant patients and 19 with ABO incompatible transplant patients. In all cases, case series with or without a control group and a very limited number of patients were treated, only 1 study registered more than 1000 patients undergoing incompatible LDKT in 22 centers (Orandi et al. 2014) in the case of HLAi. The information on graft survival in patients undergoing HLA-incompatible LDKT at 1 year is between 90 % (Laftavi et al. 2011) and 100 % (Blumberg et al. 2013; Yamanaga et al. 2013), while the data recorded over five years survival were lower, from 69 % (Couzi et al. 2015) to 94.7 % (Jakson et al. 2015). The 1-year patient survival registered was found between 90.5 % by Sharif et al. 2014 and 100 % (Blumberg et al. 2013 and Laftavi et al. 2011). The 5-year patient survival recorded is in the range of 59.2 % (in one of the subgroups described by Orandi et al. ) when the other HLAi subgroup does offer similar figures to the rest of the studies, around 86 % survival and the 5-year value provided by Kim et al. which registered 95.8 %. In general, for LDKT with ABO incompatibility, the results of the previous review from 2012 are maintained, with a 1-year graft survival in ABOi-type living donor kidney transplants recorded in up to 8 of the 19 included studies and one 84 % minimum (Bachmann et al. 2018). For patient survival at 1 year, it is 100 % or very close in all the studies on ABOi and figures are high, but somewhat lower, for patient survival at five years (between 92 % of Melexopoulus et al. and 97.7 % from Subramanian et al.). Conclusions There is great variability in the information presented by the studies, so that it make difficult to group together. The quality of the evidence is very limited, as these are case series studies with a high risk of bias, many without a control group, and others with comparative cohort results (historical retrospectives). However, the results shown are consistent and the claims of the previous 2012 report are maintained. Graft survival and patient survival for patients undergoing HLA-incompatible LDKT are high and comparable to values offered by ABOi transplants and ABO compatible transplants. In the studies on LDKT with ABOi data, the results collected on both survival variables maintain the statements of the previous report, remaining at high values. A single localized study about economic efficiency aspects was carried out in the United States, the authors conclude that the LDKT can be an efficient option in terms of cost per QALY, although this conclusion is not directly transferable to our National Health System.

This chapter focuses on internet-based pain self-management (IPSM) training for adults with chronic pain. Due to space limitations, it does not address programs directed toward children or adolescents or the burgeoning research on mobile technologies. The chapter discusses various definitions of self-management (SM) and proposes an organizing framework for the concept of SM. It examines barriers to traditional face-to-face pain SM training and the role of Internet-based training as a partial solution to the lack of care options for many. It does not reiterate the numerous excellent reviews of the efficacy of online pain SM programs. Those reviews provide support for the continued development and testing of such programs. The chapter’s focus is on the identification of strengths and weaknesses of extant technologies with an eye toward future improvements. The review of 27 IPSM programs reveals a number of important substantive and methodological issues.

Chronic knee pain affects 27 million people in the United States and is a leading cause of disability. Seventy percent of the population 65 years or older will have knee pain with radiographic evidence of osteoarthritis, and 12% will have clinical symptoms of osteoarthritis. Chronic knee pain after total knee replacement ranges from 10% to 20%. Patellofemoral pain syndrome (PFPS) refers to anterior knee pain exacerbated with knee joint loading activities (squatting, kneeling, prolonged sitting, ascending/descending stairs). PFPS is a clinical diagnosis, and treatment is directed toward pain alleviation and restoration of proper biomechanics. Pes anserine syndrome is common in runners, athletes, and individuals with osteoarthritis of the knee. Other risk factors include: female sex and a history of diabetes mellitus, obesity, or arthritis. Knowledge of the common knee pain etiologies, as well as key clinical manifestations, physical exam findings, differential diagnosis, and treatment options for each is important for pain specialists.

Abstract:
Normal agents in the actual world are limited: they cannot think about all the options that are available to them—or even about all options that are available to them according to their evidence. Moreover, agents cannot choose an option unless they have thought about that option. Such agents can be irrational in two ways: either by making their choice too quickly, without canvassing enough options, or by wasting time canvassing ever more options when they have already thought of enough options. This chapter proposes an account of this kind of rationality for limited agents. To be rational, such agents need a kind of background sensitivity to whether or not continuing to search for more options has greater expected value than deciding right away. According to this proposal, it is the unavoidable predicament of choice that all rational decision-making depends on the unconscious operation of mental dispositions.

Through the experience of the Sabaot of western Kenya, this chapter highlights the complex dialectic that engages both missionaries and the missionized in ongoing cultural exchange. While early missionaries may have introduced Western hymnody (which did not follow the tonal contours of the Sabaot language), many current musical projects are directed toward the search for a “traditional mix” that assigns Christian lyrics and meanings to songs in a local cultural mode. This discussion draws attention to a surprising coincidence between identity politics, privileging the “local” and Protestant ideas on religious truth: because religious truth hinges on rightly interpreted and comprehensible texts, local melodies and instruments are important both to those who wish to promote a “Sabaot” identity and to missionaries who are concerned that local churches encounter textual “truth.”

This chapter explores shifting patterns of intelligence surveillance in the United States. The Fourth Amendment protects Americans from unreasonable search and seizure without a warrant, but foreign spying is subject to few constraints. During the Cold War, surveillance power was abused for political purposes. Operating in a culture of secrecy, American intelligence agencies engaged in extensive illegal domestic spying. The intelligence scandals of the 1970s revealed these abuses, prompting new laws, notably the Foreign Intelligence Surveillance Act. Fearing further recrimination, the national security establishment increasingly demanded legal cover. After 9/11, Congress expanded lawful surveillance powers with the PATRIOT Act. Meanwhile, the Bush administration directed the National Security Agency to conduct warrantless domestic wiretapping. To justify this program, officials sought to redefine unconstrained foreign surveillance to subsume previously protected communications. The Obama administration continued to authorize mass surveillance and data mining programs and legally rationalize bulk collection of Americans’ data.

While the previous chapter considered diversity from the perspective of vote correlation, this chapter explores the positive effects of diverse search strategies and dividing epistemic labour. First, we consider searches on ‘rugged landscapes’ to point out the value of diverse localized search. The second section is premised on the assumption that voter competence declines the more options voters face. We show several strategies to address this. One is to consider fewer options at a time, another to let subgroups propose and the whole group dispose, yet another to let experts reduce the agenda. A different way to respond is to disaggregate the problem along several dimensions and vote on each dimension separately.

The larval stage of the parasite Taenia solium can encyst in the central nervous system causing neurocysticercosis, which is the main cause of acquired epilepsy in the countries in which the parasite is endemic. Endemic areas are those with the presence (or likely presence) of the full life cycle of Taenia solium. The parasite is most prevalent in poor and vulnerable communities in which pigs roam free, open defecation is practiced, basic sanitation is deficient, and health education is absent or limited. Several tools are available for the control of Taenia solium. Preventive chemotherapy for Taenia solium taeniasis, which is directed at the adult tapeworm, is one of them. Other tools focus on pig management, pig vaccination and treatment, sanitation and hygiene, and community education. Three potential drugs—niclosamide, praziquantel, and albendazole—have been considered for use for preventive chemotherapy in Taenia solium taeniasis control programs through mass drug administration or targeted chemotherapy. In this Guideline, we provide recommendations for preventive chemotherapy in Taenia solium-endemic areas using niclosamide, praziquantel, or albendazole, including at which dose and in which population groups. The development of this Guideline is based on the latest standard World Health Organization methods for guideline development, including the use of systematic search strategies, synthesis, quality assessment of the available evidence to support the recommendations, and participation of experts and stakeholders in the Guideline Development Group and External Review Group. The recommendations are intended for a wide audience, including policymakers and their expert advisers, and technical and program staff at governmental institutions and organizations involved in the planning, implementation, monitoring, and evaluation of preventive chemotherapy programs for the control of Taenia solium.

Post-amputation pain (PAP) has challenged clinicians for centuries. The first written record of this perplexing condition came from the 16th-century French military surgeon Ambrose Paré. The term phantom limb pain (PLP) was coined by Silas Weir Mitchell, who provided a comprehensive description of the condition during the 19th century. Since that time, the understanding of PLP has greatly expanded; however, our knowledge of the exact mechanisms underlying it is still very deficient. Amputation of a body part can result in one sequela or more than one neurologic sequelae occurring concurrently: phantom sensation, phantom pain, and stump pain. The incidence and prevalence vary across the spectrum of these syndromes. A myriad of treatment modalities are employed in an attempt to terminate PLP, including pharmacotherapy, injections, alternative therapy, surgical interventions, and neuromodulation. Despite an extensive search for effective therapeutic options, PLP remains a highly challenging and debilitating condition.
