Resumo: A qualidade nos serviços de saúde aumentou significativamente com a evolução da tecnologia de computadores e redes. Através da integração de informações médicas é possível realizar diagnósticos com maior rapidez e eficácia. Um dos problemas que surgiram nas instituições médicas foi o de integração dos equipamentos antigos de diagnóstico médico por imagem aos sistemas de informação da instituição visto que as informações geradas por esses equipamentos ou não são armazenadas digitalmente ou são geradas e transmitidas em um formato proprietário. Para solucionar esse problema foi desenvolvido um software denominado IDicom que, operando em um computador com Sistema Operacional Livre (Linux Debian), atua como uma interface entre o equipamento de diagnóstico médico antigo e um servidor remoto de imagens. O IDicom realiza a aquisição, conversão das imagens para o formato DICOM (Digital Imaging and Communications in Medicine), visualização e transmissão de imagens médicas para um servidor remoto padrão DICOM de um PACS (Picture Archiving and Communications System)
Abstract: The quality of health services has increased significantly with the evolution of computers and networks. Through the integration of medical information it is possible to accomplish medical diagnostics in a faster way and greater effectiveness. One of the problems that appeared in the medical institutions was the integration of old medical imaging equipment to the information system of these institutions since the information generated by those equipment is not stored digitally or is generated and transmitted in a proprietary format. To solve this problem, we have developed Software called IDicom that, running on a microcomputer with Linux Operating System (freeware), acts as an interface between the old medical imaging equipment and a remote imaging DICOM server. IDicom was designed to acquire, convert images to DICOM (Digital Imaging and Communications in Medicine) format, visualize and transmit medical images to a remote standard DICOM server in a PACS (Picture Archiving and Communications System)

Full textThe technology evolution seen in the last years provided large adoption of digital image acquisition equipments, such as: X-rays, computed tomography, magnetic resonance and ultrasound. The use of these devices in daily hospital routine generates an expressive image data volume which demands intelligent solutions for image storage, distribution and visualization. Such role is played by picture archiving and communication systems - PACS - which make extensively use of the DICOM protocol, world industry standard for image transmission and codification in medicine. Due to its digital nature, recently several medical image processing applications were developed and some programs used to process mammography and chest computed tomography images became commercial products in the last five years. These applications are usually known as CAD or computer-aided diagnosis. It\'s known that CAD solutions may improve the diagnosis efficiency and accuracy by detecting and/or classifying automatically abnormalities and/or diseases in radiologic exams. However, little effort was done in the sense of integrating the CAD tools and picture archiving and communication systems. To really assist radiologist diagnosis CAD needs to process digital images that are usually stored in the PACS server. Nowadays, specialist image processing programs like CAD still remains being used on an isolated workstation and the processing results are not integrated in a suitable fashion to other information systems such as HIS (Hospital Information System), RIS (Radiology Information System) and PACS. The work described in this dissertation presents a CAD-PACS integration solution based on free and open source programming libraries. It\'s hoped that this solution may contribute in an effective manner to increase the productivity and improve the radiology workflow, bringing benefits to the whole hospital environment.

Full textComputed tomography scan is the procedure that most deposits radiation dose in patients undergoing radiologic examinations. That is the reason why the need to record the dose received is currently discussed. The aim of this study was to implement an method to estimate effective dose received in CT scans based on well-established evidences and information contained in images. After performing dosimetry in CT equipment, mapped protocols and define correction factors, a data processing routine was created for conversion of DICOM header information in estimated effective dose, so that the information raised could be avalable in Hospital\'s radiology information system for doctors and health professionals involved.

Resumo: O armazenamento e a transmissão de grande quantidade de imagens médicas constituem um campo ativo de pesquisa. Dispositivos de imagem geram muitos dados por paciente. Além disso, deve-se prover armazenamento de longo prazo bem como uma transmissão eficiente. Esquemas de compressão atuais produzem taxas de compressão elevadas e com perda de qualidade. Uma abordagem de processamento que permita uma alta taxa de compressão e codificação de regiões de interesse ROI (Region Of Interest) com boa qualidade é assim desejável. Neste trabalho um método de codificação ROI e WT (Wavelet Transform) para compressão de imagem digital no formato padrão DICOM (Digital Imaging and Communications in Medicine) é apresentado. Essas aplicações permitem extrair informações relevantes para o armazenamento de uma imagem com um menor custo computacional e com um erro muito pequeno quando se compara a imagem original com aquela processada
Abstract: The storage and transmission of large amounts of medical images is an active field research. In general, imaging devices generates large amounts of data per patient. Besides, we need to provide long-term storage and an efficient transmission. Current compression schemes produce high compression rates if some loss of quality is affordable. However in medicine field we cannot afford any losses in diagnostically important regions. An approach that brings a high compression rate with a good quality is thus necessary. In this work a method of ROI coding e WT (Wavelet Transform) for compression of compression of digital imaging is presented. These applications allow extract relevant information from an image archiving with a lower computational cost and with a very small error when comparing the original and processed images

Full textEste trabalho apresenta a adaptação do código Geant4 para conversão de imagens DICOM (Digital Imaging and Communications in Medicine) de crânio, obtidas em tomografia convencional (CT), em um phantom antropomórfico virtual. O trabalho foi baseado no exemplo médico denominado “Código Dicom”, disponibilizado pelos desenvolvedores do código Geant4. Durante a execução do trabalho foram feitas reestruturações no exemplo “Código Dicom” para a conversão direta de imagens tomográficas em um phantom virtual. Foram retirados do código todos os passos referentes aos eventos físicos nucleares. Foi reformulado o arquivo DicomHandler.cc para não realizar a compressão dos pixels da imagem de CT. Em seguida foi realizada a conversão direta de imagens tomográficas, de um phantom físico de polietileno (PEAD) com núcleo central de acrílico e de um crânio real humano, em phantoms virtuais para o código Geant4. Os resultados demonstraram que com este código é possível a reconstrução de áreas anatômicas com geometrias complexas, partindo do uso de imagens tomográficas reais.
This work presents the adaptation of the Geant4 code for converting DICOM (Digital Imaging and Communications in Medicine) images of a skull, obtained in conventional tomography (CT), into a virtual anthropomorphic phantom. The work was based on the medical example named "Dicom Code" provided by the developers of the code Geant4. During the execution, restructurings using the "Dicom Code" example were made to achieve the direct conversion of tomographic images into a virtual phantom. All the steps referring to nuclear physical events were removed. The file DicomHandler.cc was reformulated in order to avoid the pixels compression of the CT image. The CT images of a physical polyethylene (PEAD) phantom with acrylic core and a real human skull were then converted into virtual phantoms for the code Geant4. The results showed that with this code, it may be possible the reconstruction of anatomical areas with complex geometries, based on the use of real tomographic images.

Interessada nas facilidades oferecidas por um sistema de arquivamento de imagens, a direção do Hospital Universitário de Brasília (HUB) decidiu implementar um servidor PACS que permitisse o recebimento e armazenamento de imagens por meio do protocolo DICOM de forma a tornar possível a visualização de imagens e confecção de laudos médicos através de uma interface Web. Como solução para este problema, foi utilizado o servidor PACS proposto pelo INCOR-SP conhecido como miniWebPACS. Em agosto de 2007, o autor dessa dissertação foi convidado pela orientadora para avaliar a segurança da aplicação miniWebPACS em uso no Hospital Universitário de Brasília. Após avaliar o código fonte da aplicação, foram encontradas duas falhas de segurança. A primeira falha permite que um invasor mal intencionado possa, através de ferramentas de captura de tráfego de rede, identificar os parâmetros de autenticação utilizados pelo protocolo DICOM e transmitir imagens médicas de um paciente, se passando pelo equipamento médico. A segunda refere-se à possibilidade de remontar a imagem médica capturando todos os pacotes de rede transmitidos entre o equipamento médico e o servidor, destruindo, desta forma, o sigilo das informações do paciente. As falhas de segurança eram extremamente graves e, se exploradas, poderiam denegrir a imagem do HUB e de toda sua equipe médica. A simples troca do servidor miniWebPACS por uma outra versão era inviável. O HUB já havia desenvolvido várias soluções para manipulação de dados médicos utilizando o atual servidor. O projeto hub2pacs nasceu com o objetivo de desenvolver um novo servidor PACS para a transmissão de imagens médicas de forma segura, utilizando como referência a biblioteca livre dcm4che e respeitando o modelo de dados em uso no Hospital Universitário de Brasília. O hub2pacs resolveu, definitivamente, o problema de compatibilidade das aplicações antigas já desenvolvidas além de oferecer garantia do sigilo do canal de comunicação e a autenticação das partes, porém, a análise do ambiente demonstrou a necessidade de permitir que equipamentos médicos sem suporte ao DICOM/TLS pudessem transferir imagens de forma segura. Para resolver este problema foi desenvolvido um novo projeto chamado hub2conversor. _________________________________________________________________________________________ ABSTRACT
With the facilities that an image archiving system offers, the HUB Hospital Universitário de Brasília (Brasilia University Hospital) decided to implement a PACS server that allowed it to receive and store images using the DICOM protocol. This, in turn, allowed it to view images and make medical diagnoses using a Web interface. As a solution to this problem, a PACS server suggested by INCOR-SP named miniWebPACS was used. In August 2007, the author of this dissertation was invited by his Thesis Orientation Professor to evaluate the security of the miniWebPACS application at use at the HUB. After evaluating the source code of this application, security failures were found. The first failure permitted an illmeaning invader to, through network traffic capture tools, identify the parameters through which the DICOM protocol authenticates, thus permitting illicit medical images to be sent in the name of a licit networked piece of medical equipment. The second failure is the possibility of reassembling the medical image by capturing all the network packets transmitted between the medical equipment and the server, therefore destroying the patients informations confidentiality. The security failures were extremely serious, and, if exploited, could damage the HUBs and its doctors images. The simple substitution of the miniWebPACS server for another, more secure, server was not viable, given that the HUB had already developed several solutions for manipulating medical data using the current server. Thus, the hub2pacs project was born with the objective of developing a PACS server for the transmission of medical images in a more secure manner, using as a reference the dcm4che free development software library, and preserving the data model at use at the HUB. The Hub2pacs solved, for once and for all, not only the compatibility issues with the old applications that had already been developed, but also offered a guarantee of confidentiality in the communications channel and the authentication of all the parts involved. However, an environmental analysis pointed out the necessity of allowing medical equipment without support for DICOM/TLS to transfer images securely too. To solve this problem a new project was started called hub2conversor.

Este trabalho foi realizado no escopo do projeto Cyclops, que é um Projeto de cooperação Brasil-Alemanha direcionado ao desenvolvimento inteligente para suporte de diagnósticos médicos. Desta maneira, o software DicomEditor tem a liberdade de optar pelo uso de um software gratuito, que acompanha o protocolo DICOM que é o CTN ou o uso do banco de dados Oracle. Este trabalho acrescenta a conexão do banco de dados Oracle ao software DicomEditor, implementado em Smalltalk. Este software tem a funcionalidade de ser um cliente de rede DICOM. DICOM é um protocolo de armazenamento e recuperação de imagens médicas utilizadas em larga escala por hospitais e grandes clinicas médicas. Através deste trabalho apresentamos os procedimentos de operação das classes de conexão do DicomEditor ao banco de dados Oracle, juntamente com o modelo relacional criado no banco de dados para armazenar as informações.

According to the World Health Organization, about 17.5 million people die each year from cardiovascular disease. The early diagnosis and treatment is crucial to reduce this high number of deaths from cardiovascular diseases. Due to the importance of the electrocardiogram (ECG) analysis, which represents one of the main tests for the detection of these cardiovascular diseases, in the present dissertation is proposed a remote system for automatic ECGs analysis based on webservices that can substantially assist the doctor, using the extracted ECGs information, with the purpose to generate reports. Hence, a Middleware is proposed to abstract the complexities and to facilitate the construction of tools that allow the ECGs automatic analysis. The proposed architecture consists on a server which provides the computational algorithms to identify the main waves present in ECGs such as QRS complex, P and T waves, and support the open standards of HL7 aECG and DICOM-ECG. In order to validate the proposed Middleware and contribute to the medical community, especially the Walter Cantídio University Hospital, a Web application for remotely accessible ECG report was developed. The proposed solution was tested using the database MITDB to simulate the patients of the system and produce the results presented in this dissertation. The proposed solution met the established requirements which makes the proposed solution viable financially and technically.
De acordo com a Organização Mundial da Saúde (OMS), cerca de 17,5 milhões de pessoas morrem todos os anos vítimas de doenças cardiovasculares. O diagnóstico e tratamento precoce às pessoas com doenças cardiovasculares é fundamental para reduzir este número elevado de óbitos. Devido à importância da análise de Eletrocardiograma (ECG), que representa um dos principais exames para detecção destas doenças cardiovasculares, na presente dissertação é proposto um sistema remoto para análise automática de Eletrocardiograma (ECG)s baseado em Web services que pode auxiliar substancialmente o médico especialista, a partir de informações extraídas de ECGs, na emissão de laudos. Neste sentido, propõe-se um Middleware para abstrair as complexidades e facilitar a construção de ferramentas que permitam a análise automática de ECGs. Na arquitetura proposta, um servidor disponibiliza os algoritmos computacionais para identificação das principais ondas presentes nos ECGs tais como complexo QRS, ondas P e T, e suporte aos padrões abertos de exames HL7 aECG e DICOM-ECG. Para validar o Middleware proposto e contribuir com a comunidade médica, em especial do Hospital Universitário Walter Cantídio, desenvolve-se uma aplicação Web para geração de laudos de ECGs acessível remotamente. Testes foram realizados com a solução proposta, utilizando-se a base de dados MIT-BIH Arrhythmia Database (MITDB) para simular os pacientes do sistema e produzir os resultados apresentados nesta dissertação. A solução proposta atendeu aos requisitos estabelecidos e representa uma solução viável, financeiramente e tecnicamente.

Images are from magnetic resonance nuclear and computadorized tomography and are given using DICOM 3.0 protocol. The system has been developed considering web technologies with friendly interfaces to retrieval of information. It is composed by three integrated modules: the image server module, the query text module and query by similarity module. Results show that images are managed and archived exactly, retrieval time of images is clinically satisfactory, considering both the text query as well as the query by similarity. The evaluation of the retrieval by similarity shows the chosen extractor can be considerated relevant to separate the images by anatomic region.

Full textThis work presents the study and implementation of a system aiming the indexing and visualization of the images of nuclear magnetic resonance and computerized tomography from a radiology information system (RIS), allowing the retrieval and availability of the exams (results and images), through an ethernet net for visualization beginning with an interface developed to a browser. The exams may be recovered either through their register number (RGHC) or the patient\'s name. The images employed in the work follow the DICOM 3.0 (Digital Imaging and Communications in Medicine) and JPEG (Joint Photographic Experts Group) patterns. For the indexing of the exams that present images in JPEG, an interface was developed to include the information required to guarantee the consistency of the process. For the exams presenting the DICOM 3.0 format, information was extracted automatically from the heading and filed in the database. The system has a friendly interface for the user, it may be rapidly incorporated to the project of an PACS (Picture Archiving and Communication System). The implementation was idealized to serve the Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto, of the Universidade de São Paulo (HCFMRP/USP), based on its radiology information system (RIS). The results demonstrated that the time of retrieval of the images is satisfactory and considered good by evaluation qualified of the doctors.

It aims interoperation of heterogeneous systems, which have the capacity to adapt themselves to the specificities of medical environment, via Web, providing this way, proper environments to the health area. Nowadays, in order to develop environments for visualizing images on the Web, it is necessary to use patterns of images, metadata concepts and adaptations of technologies for this purpose. Throughout the use of standard format of medical images DICOM, this work proposes the construction of a Framework to visualize and exchange information via Web. Among its applications are the telemedicine and the image integration to the information systems in the health area.

Administrative Abläufe, wie die Erstellung von Nutzungsplänen für die verfügbaren technischen, räumlichen und personellen Ressourcen, die Rechnungsstellung und - in zunehmendem Maße - die Verwaltung und Archivierung von Patientendaten werden mit Hilfe von digitalen Informationssystemen rationell und effizient durchgeführt. Innerhalb der Krankenhausinformationssysteme (KIS, oder englisch HIS) stehen für die speziellen Bedürfnisse der einzelnen Fachabteilungen oft spezifische Informationssysteme zur Verfügung. Chirurgieinformationssysteme (CIS, oder englisch SIS) decken hierbei vor allen Dingen die Bereiche Operationsplanung sowie Materialwirtschaft für spezifisch chirurgische Verbrauchsmaterialien ab. Während die genannten HIS und SIS vornehmlich der Optimierung administrativer Aufgaben dienen, stehen die Systeme der Computerassistierten Chirugie (CAS) wesentlich direkter im Dienste der eigentlichen chirugischen Behandlungsplanung und Therapie. Die CAS verwendet Methoden der Robotik, digitalen Bild- und Signalverarbeitung, künstlichen Intelligenz, numerischen Simulation, um nur einige zu nennen, zur patientenspezifischen Behandlungsplanung und zur intraoperativen Unterstützung des OP-Teams, allen voran des Chirurgen. Vor allen Dingen Fortschritte in der räumlichen Verfolgung von Werkzeugen und Patienten ("Tracking"), die Verfügbarkeit dreidimensionaler radiologischer Aufnahmen (CT, MRT, ...) und der Einsatz verschiedener Robotersysteme haben in den vergangenen Jahrzehnten den Einzug des Computers in den Operationssaal - medienwirksam - ermöglicht. Weniger prominent, jedoch keinesfalls von untergeordnetem praktischen Nutzen, sind Beispiele zur automatisierten Überwachung klinischer Messwerte, wie etwa Blutdruck oder Sauerstoffsättigung. Im Gegensatz zu den meist hochgradig verteilten und gut miteinander verwobenen Informationssystemen für die Krankenhausadministration und Patientendatenverwaltung, sind die Systeme der CAS heutzutage meist wenig oder überhaupt nicht miteinander und mit Hintergrundsdatenspeichern vernetzt. Eine Reihe wissenschaftlicher Publikationen und interdisziplinärer Workshops hat sich in den vergangen ein bis zwei Jahrzehnten mit den Problemen des Alltagseinsatzes von CAS Systemen befasst. Mit steigender Intensität wurde hierbei auf den Mangel an infrastrukturiellen Grundlagen für die Vernetzung intraoperativ eingesetzter CAS Systeme miteinander und mit den perioperativ eingesetzten Planungs-, Dokumentations- und Archivierungssystemen hingewiesen. Die sich daraus ergebenden negativen Einflüsse auf die Effizienz perioperativer Abläufe - jedes Gerät muss manuell in Betrieb genommen und mit den spezifischen Daten des nächsten Patienten gefüttert werden - sowie die zunehmende Aufmerksamkeit, welche der Operateur und sein Team auf die Überwachung und dem Betrieb der einzelnen Geräte verwenden muss, werden als eine der "Kinderkrankheiten" dieser relativ jungen Technologie betrachtet und stehen einer Verbreitung über die Grenzen einer engagierten technophilen Nutzergruppe hinaus im Wege. Die vorliegende Arbeit zeigt zwei parallel von einander (jedoch, im Sinne der Schnittstellenkompatibilität, nicht gänzlich unabhängig voneinander) zu betreibende Ansätze zur Integration von CAS Systemen. - Für den perioperativen Datenaustausch wird die Spezifikation zusätzlicher Datenstrukturen zum Transfer chirurgischer Planungsdaten im Rahmen des in radiologischen Bildverarbeitungssystemen weit verbreiteten DICOM Standards vorgeschlagen und an zwei Beispielen vorgeführt. Die Erweiterung des DICOM Standards für den perioperativen Einsatz ermöglicht hierbei die nahtlose Integration chirurgischer Planungssysteme in existierende "Picture Archiving and Communication Systems" (PACS), welche in den meisten Fällen auf dem DICOM Standard basieren oder zumindest damit kompatibel sind. Dadurch ist einerseits der Tatsache Rechnung getragen, dass die patientenspezifische OP-Planung in hohem Masse auf radiologischen Bildern basiert und andererseits sicher gestellt, dass die Planungsergebnisse entsprechend der geltenden Bestimmungen langfristig archiviert und gegen unbefugten Zugriff geschützt sind - PACS Server liefern hier bereits wohlerprobte Lösungen. - Für die integration intraoperativer CAS Systeme, wie etwa Navigationssysteme, Videobildquellen oder Sensoren zur Überwachung der Vitalparameter, wird das Konzept einer "chirurgischen Middleware" vorgestellt. Unter dem Namen TiCoLi wurde eine c++ Klassenbibliothek entwickelt, auf deren Grundlage die Konfiguration von ad-hoc Netzwerken während der OP-Vorbereitung mittels plug-and-play Mechanismen erleichtert wird. Nach erfolgter Konfiguration ermöglicht die TiCoLi den Austausch kontinuierlicher Datenströme sowie einzelner Datenpakete und Kommandos zwischen den Modulen einer verteilten CAS Anwendung durch ein Ethernet-basiertes Netzwerk. Die TiCoLi ist die erste frei verfügbare Klassenbibliothek welche diese Funktionalitäten dediziert für einen Einsatz im chirurgischen Umfeld vereinigt. Zum Nachweis der Tauglichkeit der gezeigten Spezifikationen und deren Implementierungen, werden zwei modulare CAS Anwendungen präsentiert, welche die vorgeschlagenen DICOM Erweiterungen zum perioperativen Austausch von Planungsergebnissen sowie die TiCoLi zum intraoperativen Datenaustausch von Messdaten unter echzeitnahen Anforderungen verwenden

Paranasal sinus are important objects of study to rhinosinusitis diagnostic, having some papers related incidence between asthma and allergic rhinitis.Many applications can calculate to various parts of the human body, getting a CT scan or MRI input, and returning information about the region of interest observed as volume and area. The accumulated mucus in the sinuses is one of the areas of interest that have not yet been implemented methods for the calculation of volume and area. In the present scenario, the patient monitoring is done visually, depending largely on perception of the evaluator. Therefore, we seek to implement more accurate metrics to facilitate medical care to the patient and it can help prevent the worsening of rhinitis in a given patient, developing mechanisms of visual and numerical comparison, where it is possible observe the progress of treatment. This work contains a detailed study of how certain existing techniques, combined into one methodology can segment and calculate the accumulated mucus in the maxillary sinus. In addition to techniques such as Thresholding, Gaussian filter, Mathematical Morphology, Metallic Artifacts Reduction during processing and segmentation, MUNC and DTA to calculate the volume and area, and visualization techniques as the Marching Cubes, it was also necessary some adjustments in the algorithm for limit the region of interest where the thresholding combined with the gaussian filter has not been effective of retaining edges. The application will use two open source platforms, one for processing, ITK, and another for visualization, VTK. The results demonstrated that it is possible to perform segmentation and the calculation with the use of platforms as well as the methodology used is adequate to solve this problem.
Os seios paranasais são importantes objetos de estudo para o diagnóstico de rinossinusites, tendo alguns estudos relacionado a incidência de asma na fase adulta a quadros de rinite alérgica na infância. Muitas aplicações atendem a diversas partes do corpo humano, obtendo de entrada uma tomografia computadorizada ou ressonância magnética, e devolvendo, muitas vezes, números que dizem respeito ao objeto de interesse observado, como volume e área. O muco acumulado nos seios paranasais é uma das regiões de interesse que ainda não tiveram métodos implementados para o cálculo do volume e área. No cenário atual, o acompanhamento do paciente é feito de forma visual, dependendo muito da percepção do avaliador. Portanto, busca-se a implementação de métricas mais precisas para facilitar o acompanhamento médico ao paciente e ajudar na prevenção do agravamento de um quadro de rinite em um determinado paciente, criando mecanismos de comparação visual e numérica, onde é possível observar a evolução do tratamento. Este trabalho contém um estudo detalhado de como determinadas técnicas existentes, combinadas em uma metodologia, podem segmentar e calcular o muco acumulado nos seios paranasais maxilares. Além de técnicas como a Binarizacão, Filtro Gaussiano, Morfologia Matemática, Redução de Ruídos Metálico durante o processamento e segmentação, MUNC e DTA para o cálculo do volume e área, e técnicas de visualização como o Marching Cubes, foram necessários também ajustes no algoritmo para limitar a área segmentada onde a binarizacão combinada ao filtro não foi capaz de manter as bordas da região de interesse. A aplicação fará uso de duas plataformas de código livre, sendo uma para o processamento, ITK, e outra para visualização de imagens, VTK. Os resultados demonstraram que é possível realizar a segmentação e o cálculo com o uso das plataformas, bem como a metodologia empregada é adequada a resolução deste problema.

Imagens médicas são usadas diariamente para apoio ao diagnóstico em diferentes áreas da Radiologia no mundo todo. Essas imagens seguem uma padronização internacional definida pela ISO 12052, conhecida como padrão DICOM (Digital Imaging and Communications in Medicine). Cada instituição que reivindica conformidade com esta norma, possui seus próprios serviços de armazenamento, sistemas de visualização e processamento específicos para esses dados. No entanto, há uma grande necessidade de que essas imagens sejam analisadas por diferentes especialistas, a fim de que cada caso possa ser discutido de forma ampla, na busca do melhor tratamento para cada patologia. A indisponibilidade de dados em tempo real para a avaliação médica especializada impacta direta e profundamente no sucesso terapêutico. O modelo de computação em nuvem tem as características necessárias para garantir que estas imagens se encontrem ao alcance dos profissionais mais recomendados para cada caso, aptos a oferecer o melhor atendimento. A grande quantidade de recursos disponíveis em nuvem, para lidar com esses dados de forma escalável, facilita a criação de uma infraestrutura para apoio ao diagnóstico à distância através de recursos de Telemedicina. Tomando como base o paradigma computacional Publicar/Assinar, podemos estabelecer comunicação em tempo real para solucionar situações no campo da saúde, como comunicação entre hospitais ou clínicas e entre médicos, enfermeiros e especialistas envolvidos no diagnóstico. Em ambientes clínicos que lidam com transmissão massiva de imagens em alta resolução no padrão DICOM, bem como em ambientes com problemas de desempenho de rede, transmitir essas imagens em tempo hábil, armazenar e disponibilizar de forma segura é um problema sem solução espontânea. Dessa forma esse trabalho propõe uma arquitetura baseada em nuvem computacional, para coletar, comprimir, armazenar e recuperar dados utilizando o paradigma Publicar/Assinar e dois níveis de escalabilidade. O modelo PS2DICOM é estabelecido como um middleware que oferece recursos de infraestrutura na camada IaaS (Infrastructure as a Service), apoiando as tarefas de transmissão e armazenamento de arquivos dentro do padrão DICOM. O sistema oferece compactação dos dados com diferentes intensidades, conforme largura de banda disponível. O modelo PS2DICOM conta ainda com dois níveis de balanceamento de carga e com a elasticidade reativa oferecida pela infraestrutura. A pesquisa contribui ao apresentar uma arquitetura eficaz para otimizar tarefas de rede, capaz de ser adotada como solução ao desenvolver aplicações voltadas para nuvens computacionais aplicadas a saúde em futuras situações reais. A arquitetura foi testada utilizando um protótipo com módulos distintos, desenvolvidos para cada serviço específico oferecido e mostrou-se eficiente como solução para os problemas em questão. Seus detalhes são descritos nos capítulos seguintes, bem como sua implementação, que corrobora a viabilidade do modelo.
Medical images are used daily to support diagnosis in different areas of Radiology throughout the world. These images follow an international standardization defined by ISO 12052, known as DICOM (Digital Imaging and Communications in Medicine) standard. Each institution that claims compliance with this standard has its own storage services, visualization and processing systems specific to that data. However, there is a great need for these images to be analyzed by different specialists, so that each case can be discussed in a broad way, in the search for the best treatment for each pathology. The unavailability of real-time data for specialized medical evaluation has a direct and profound impact on therapeutic success. The cloud computing model has the necessary characteristics to ensure that these images are within the reach of the professionals most recommended for each case, able to offer the best service. The large amount of resources available in the cloud to handle this data in a scalable way facilitates the creation of an infrastructure to support remote diagnosis through Telemedicine resources. Based on the computational paradigm Publish/Subscribe, we can establish real-time communication to solve situations in the field of health, such as communication between hospitals or clinics and between doctors, nurses and experts involved in the diagnosis. In clinical environments that deal with massive transmission of high resolution images in the DICOM standard, as well as in environments with network performance problems, transmitting these images in a timely manner, storing and making them available securely is a problem without a spontaneous solution. In this way, this work proposes a computational cloud-based architecture to collect, compress, store and retrieve data using the Publish/Subscribe paradigm and two levels of scalability. The PS2DICOM model is established as a middleware that provides infrastructure resources in the IaaS (Infrastructure as a Service) layer, supporting the tasks of transmitting and storing files within the DICOM standard. The system offers data compression with different intensities, depending on available bandwidth. The PS2DICOM model also has two levels of load balancing and the reactive elasticity offered by the infrastructure. The research contributes to presenting an efficient architecture to optimize network tasks, capable of being adopted as a solution when developing applications focused on computational clouds applied to health in future real situations. The architecture was tested using a prototype with distinct modules, developed for each specific service offered and proved to be efficient as a solution to the problems in question. Its details are described in the following chapters, as well as its implementation, which corroborates the viability of the model.

A telemedicina é uma alternativa importante no cuidado do paciente e vem sendo utilizada na prestação de assistência e na educação na área médica. Uma análise comparativa entre os métodos mais utilizados na telemedicina e aqueles utilizados na tele-educação evidencia que potencialidades como a utilização de ambientes virtuais para realização de trabalhos colaborativos a distância ainda não são bem exploradas pela área da saúde. Dentre as técnicas de diagnóstico, o diagnóstico por imagem vem se destacando como uma excelente ferramenta para a descoberta de doenças. As imagens médicas possuem a capacidade de fornecer muitas informações relativas à saúde do paciente, documentando o que está acontecendo em seu corpo, reduzindo a subjetividade dos diagnósticos e oferecendo maior precisão aos tratamentos invasivos. Assim, é possível concluir que um ambiente virtual com finalidade de dar suporte ao diagnóstico médico, deve ter entre seus componentes básicos, recursos para armazenamento, manipulação e transferência de imagens. Com o objetivo de oferecer ao sistema de saúde recursos para realização de atividades multiprofissionais para o diagnóstico remoto, foi desenvolvido um ambiente virtual colaborativo integrado a ferramentas de manipulação de imagens. O sistema consiste em um ambiente virtual para interação entre pessoas para viabilizar meios eficientes para o estudo de casos clínicos e o diagnóstico médico. Assim, o sistema: i) permite que dados sobre o paciente e suas eventuais doenças sejam persistidos na base de dados para que profissionais possam acessá-los e usá-los para fundamentar os laudos, cujos resultados podem ser inseridos no próprio ambiente; ii) disponibiliza o atendimento virtual de pacientes através de recursos como chat e envio de mensagens; iii) viabiliza recursos para a definição de diagnósticos através da interação de multiprofissionais em uma rede de trabalho cooperativa; e iv) habilita o acesso de seus usuários ao banco de imagens de exames, para fundamentar o diagnóstico ou o treinamento de profissionais. Foram desenvolvidos e propostos métodos de implementação para persistência e manipulações de imagens no padrão DICOM 3.0, protocolo utilizado mundialmente para armazenamento e transmissão de imagens médicas. Também, foram integrados ao ambiente, recursos para o processamento de imagens para que essas pudessem ser trabalhadas para atenderem análises diagnósticas específicas. A disponibilização de recursos que permitem a definição de diagnósticos através da colaboração de multiprofissionais é uma importante contribuição deste trabalho, pois os sistemas de informação que trabalham com imagens médicas, em geral, não disponibilizam tais recursos. As ferramentas que permitem que discussões clínicas sejam realizadas estimulam o compartilhamento entre diferentes profissionais, localizados remotamente, expandindo os limites da colaboração. Os recursos para o trabalho colaborativo que compõem o sistema podem ser utilizados para que os profissionais possam solucionar problemas cognitivos de forma qualitativamente diferente do que teriam realizado individualmente. A ferramenta para processamento de imagens integrada ao AV permitiu a expansão de suas funcionalidades, permitindo que técnicas de realce de imagens pudessem ser utilizadas no processo de diagnóstico.

Full textThis abstract describes a degree project in computer engineering at Linnaeus University. The Department of Radiology at SUS, Skåne University Hospital in Lund, is using a lightweight portable x-ray machine that weights about 90 kg, for radiography services in nursing homes for elders. The images are transferred to hospital PACS when the machine returns to the hospital, this is done with Dragon connecting to hospital network via wire. The goal is to improve the currently too slow and unreliable wireless communication, so the images can be transferred to hospital PACS directly after examination. This report describes what alternatives can be used and investigates whether the mobile broadband services currently available in Skåne County are good enough to be relied on for the task. The conclusion is that a 3G HSDPA/HSUPA subscription can be used reliably enough, if extra redundancy is added via dual SIM cards using separate mobile networks and the signal is strengthen by an antenna placed outdoors on the transport vehicle. The transport vehicle is used as a bridge between Wifi and mobile networks. The mobile x-ray machine communicates with the transport vehicle via Wifi. In those cases Wifi does not work due to difficult building environments, a backup solution was worked out to be used. I also investigated and compared the DICOM protocol against FTP. This was to find out if DICOM has an overhead that would motivate using FTP instead for the transmission of the images, the result of the comparison showed that DICOM has an 5-10 % overhead compared with FTP.

Este trabalho tem por objetivo apresentar a implementação de um Servidor de Impressão que seja compatível com o Padrão DICOM, o qual foi idealizado em 1983 com o intuito de padronizar o armazenamento e a transmissão de arquivos gerados por equipamentos geradores de imagens médicas, a fim de que estes arquivos pudessem ser intercambiáveis. Ele faz parte de um projeto maior de implementação de um Sistema de Arquivamento e Comunicação de Imagens (Picture Archiving and Communication Sytems - PACS) no Hospital Universitário de Brasília (HUB). Atualmente, o projeto do HUB já permite o armazenamento e o acesso a imagens médicas resultantes de exames realizados por pacientes nas clínicas do hospital, em um servidor de arquivos. Este trabalho visa acrescentar a funcionalidade de impressão ao projeto da Universidade de Brasília (UnB), implementando um Servidor, que possibilite a impressão das imagens em impressoras comuns, recebendo as solicitações dos clientes e gerenciando a impressão. A arquitetura e a implementação do servidor de impressão utilizou como base o MiniWebPACS do INCOR e a biblioteca dcm4che, permitindo desta maneira a integração desta nova funcionalidade ao PACS do HUB. A funcionalidade de um Servidor de Impressão consiste na implementação das Classes SOP , isto é, consiste na implementação da criação das instâncias SOP: Basic Film Session, Basic Film Box e Basic Image Box, cada uma delas representando uma entidade do mundo real, que vai gerar arquivos raw, bmp e jpg, e sua posterior impressão. Os resultados sugerem que seja utilizado o arquivo jpg, pois ele possui um tamanho muito menor que o arquivo bmp, devido ao processo de compressão utilizado na sua criação. ________________________________________________________________________________________ ABSTRACT
This work presents the implementation of a Print Server that is compatible with the DICOM Standard. It was developed in order to standardize the storage and the transmission of files generated by medical imaging equipments, such that these files could be interchangeable. It is part of a bigger project of implementation of a Picture Archiving and Communication System – PACS, in the Hospital of the University of Brasilia (HUB). Currently, the PACS supplies the necessary structure for the storage in a file server and communication of the different types of medical images generated during all kinds of medical process. This work seeks to add a new functionality to the project of the University of Brasilia (UnB), implementing a Print Server, that enables the medical images to be printed in common printers, receiving the requests from the clients and managing the print jobs. The architecture and the implementation of the print server was based on the INCOR´s MiniWebPACS, and the library dcm4che, allowing the integration of this functionality to the PACS of the HUB. The functionality of a Print Server consists of the implementation of the SOP Classes, that means, it consists of the implementation of the following SOP instances: Basic Film Session, Basic Film Box and Basic Image Box, each one representing an entity of the real world. Files of the types raw, bmp and jpg will are generated to be printed. The results suggest the use of a jpeg file, because its size is smaller than the bmp and raw files, due to the compression used in its creation.

Full textThe digital radiography image acquired by the photostimulable storage phosphor plates system normally is showed in the computer screen as a DICOM format, characterized for the high resolution of the images. As this format uses a lot of memory space for storage, the digital images are submitted to a compression, which optimizes the capacity of space on the computers and reduces the time of transmission for the Internet. The aim of this study was to evaluate the effect of TIFF and JPEG compression in the intraobserver and interobserver reproducibility of the cephalometric landmarks identification on lateral headfilms compared with DICOM format. The sample consisted of 90 digital lateral cephalometric images obtained from 30 individuals, in DICOM format. These images were converted for formats JPEG, with Quality Factor 80 and TIFF. After blind and random the sample, three observers, orthodontists, were calibrated and ask to identify 15 cephalometric landmarks in each image using x and y coordinates system. The results had shown that the cephalometric landmarks identification had presented agreement of intraobserver and interobserver reproducibility, except for the points Go, Po, Or, B and Pog'. The statistic analyze showed no difference between the file formats for each point and axis. JPEG and TIFF compressions studied had not effect in the intraobserver and interobserver reproducibility of the cephalometric landmarks identification on digital lateral headfilm.(AU)
A imagem radiográfica digital adquirida pelo sistema de placa de fósforo foto ativada é normalmente visualizada no monitor do computador no formato DICOM, caracterizado pela alta resolução das imagens. Como este formato ocupa muito espaço para armazenamento, as imagens digitais são submetidas a uma compressão, que otimiza a capacidade de espaço dos computadores e reduz o tempo de transmissão pela Internet. O objetivo desse estudo foi avaliar a influência da compressão TIFF e JPEG na reprodutibilidade intra e interexaminador da marcação de pontos cefalométricos em imagens de telerradiografias em norma lateral comparadas com o formato DICOM. A amostra consistiu de 90 imagens de telerradiografias obtidas a partir de 30 indivíduos, dos quais se obteve uma radiografia digital exibida no formato DICOM. Estas imagens foram convertidas para os formatos JPEG, com Fator de Qualidade 80 e TIFF. Após o cegamento e randomização da amostra, três ortodontistas calibrados marcaram a localização de 15 pontos cefalométricos em cada imagem utilizando o sistema de coordenadas x e y. Os resultados mostraram que as marcações dos pontos cefalométricos apresentaram concordância de reprodutibilidade tanto intra como interexaminador, exceto para os pontos Go, Po, Or, B e Pog . Os diferentes formatos de arquivo mostraram resultados estatisticamente semelhantes para cada ponto e eixo aferido. As compressões JPEG e TIFF estudadas não tiveram efeito, em imagens de telerradiografias em norma lateral, na reprodutibilidade intra e interexaminadores da marcação dos pontos cefalométricos testados.(AU)

Full textImage quality assessment in mammography represents one of the greatest challenges that health services face in order to detect breast cancer early. The world health organization recommends quality assurance guidelines, which consist of acceptance tests for digital mammography (DM) and digital breast tomosynthesis (DBT), to standardize the radiological services. However, these guidelines are based on test objects widely used to simulate standard breasts in terms of composition and thickness, which often do not match with the demographic profiles in the breast care centers. In this thesis work, an automated system for tracking and data management was developed to extract the metadata (DICOM header) from the image acquisition process and to store it into a custom DICOM Service Class Provider (SPC). The developed system correlates the exposure factors\' info in order to evaluate the references for patient radiation dose provided by the manufacturers. As result of the data correlations, the reference required by the international guidelines for standard breasts are not a feasible representation for the demographic profile in the breast care center evaluated in terms of breast density and thickness. While the standard for quality assurance testing consists of breasts with 50 mm of thickness and 50% of density, the demographic population presented compressed breast thickness of approximately 60 mm and low fibroglandular tissue (less than 25%). Therefore, using the developed system should guide the quality assurance in digital mammography to specific breast profiles. This guidance may assist in determining optimal radiation doses to improve the image quality, producing useful benchmarks for the quality of radiological services.

Full textIn the year of 2017 the Swedish social office reported the most common cancer related death amongst women was lung cancer and the second most common amongst men. A way to find out if a patient has lung cancer is for a doctor to study a computed tomography scan of a patients lungs. This introduces the chance for human error and could lead to fatal consequences. To prevent mistakes from happening it is possible to use computers and advanced algorithms for training a network model to detect details and deviations in the scans. This technique is called deep structural learning. It is both time consuming and highly challenging to create such a model. This discloses the importance of decorous training, and a lot of studies cover this subject. What these studies fail to emphasize is the significance of the preprocessing technique called lung segmentation. Therefore we investigated how is the accuracy and loss of a convolutional network model affected when lung segmentation is applied to the model’s training and test data? In this study a number of models were trained and evaluated on data where lung segmentation was applied, in relation to when it was not. The final conclusion of this report shows that the technique counteracts overfitting of a model and we allege that this study can ease further research within the same area of study.

Full textA monitorização sistemática da exposição dos cidadãos à radiação ionizante associada aos procedimentos imagiológicos é fundamental para garantir a qualidade dos serviços clínicos. Esta atividade é importante no controlo de desempenho, na optimização de protocolos e na rápida rectificação das práticas erradas. Em teoria, os episódios de radiodiagnóstico devem sempre manter a exposição à radiação tão baixa quanto razoavelmente possível (princípio ALARA), preservando a qualidade de diagnóstico. Os sistemas de monitorização de dose automáticos podem ser úteis em todas as fases de procedimentos radiológicos, ajudando os profissionais de saúde a melhorar os seus comportamentos de dosimetria. Mais ainda, a exposição aplicada nos procedimentos deve ser planeada individualmente, o que significa que a monitorização da dose também deverá ser. Além disso, o acesso integrado à história imagiológica do paciente pode ser útil para efetuar um melhor tratamento. No entanto, muitos dos atuais sistemas de informação não permitem efetuar análise de dose e a sua monitorização contínua é rara. Neste contexto, o contributo desta dissertação é o Dose Center, uma ferramenta centrada no paciente que permite monitorizar e analisar a dose de radiação. Ela tem capacidade para extrair informação proveniente de diferentes fontes e permite uma visualização integrada de toda a informação relativa aos pacientes, quais os estudos realizados, a dose efetiva e cumulativa de radiação. A ferramenta permite ainda sinalizar os casos que excedam os limites pré-definidos de radiação, uma inequívoca contribuição para a melhoria da segurança do paciente.
Systematic monitoring of radiation dose exposure is a key factor to increase the quality of radiological services. This activity may lead to performance control, protocol optimization and rapid rectification of wrong practices. Moreover, dose monitoring can help the healthcare professionals to improve their dosimetric behaviors. In theory, radiodiagnostic episodes should always keep the radiation exposure as low as reasonably achievable (ALARA), while preserving the quality of diagnosis. Hence, the applied exposure in the radiology departments shall be individually planned, which means that the dose monitoring should be performed individually to ensure an appropriate dose usage. Automatic dose monitoring systems may be helpful during all the phases of radiologic procedures and the integrated access to imagiologic history may be helpful to do a better patient treatment. However, many of actual healthcare information systems do not allow dose analysis and its continuous monitoring is rare. In this context, this document proposes the Dose Center, a software platform that provides a patient-centric radiation dose analysis and a monitoring system that was designed to automatically extract and analyze dose reports captured from distinct data sources. It provides several data analytics views like, for instance, by modality or patient, including the studies effective and cumulative dose radiation. Cases exceeding the radiation thresholds are signalizing, contributing this way to improve the patient safety.

Full textO Cancro da Mama constitui o tipo de cancro que afeta o maior número de mulheres europeias e a sua deteção precoce, diagnóstico e tratamento eficazes são fundamentais. Os Structured Reports (SR) surgiram como alternativa aos Relatórios Free Text (FT) e a norma Digital Imaging and Communication in Medicine (DICOM) estendeu a sua aplicação aos SR, com o suplemento DICOM Structured Reports (DICOM SR). A presente dissertação, teve como objetivos principais a conceção de um Repositório DICOM SR e a avaliação da aceitação dos Radiologistas e não Radiologistas para Relatórios FT versus SR na comunicação de resultados em Imagiologia Mamária. A investigação compreendeu duas fases, sendo que na primeira conceptualizou-se o MamoCatalogue para a estruturação dos Relatórios de acordo com a terminologia DICOM que, à posteriori, foram convertidos em DICOM SR e integrados com o Dicoogle - Picture Archiving and Communication System (PACS) open source. Na segunda fase do estudo, foram selecionados 7 Relatórios de Mamografia e/ou Ecografia Mamária, que posteriormente foram avaliados por um grupo de 25 Médicos, Radiologistas e não Radiologistas (Ginecologia/Obstetrícia e Medicina Geral e Familiar - MGF). Cada médico apreciou os 7 Relatórios, em FT e SR, com uma distância temporal de pelo menos 3 meses e avaliou os seguintes aspetos: Estrutura do Relatório, Clareza e assertividade da linguagem, Diagnóstico e recomendações, Facilidade de leitura, Leitura do Relatório na totalidade, Leitura parcial do Relatório com avanço para a classificação BI-RADS e Existência de aspetos ambíguos. As primeiras quatro variáveis foram cotadas na escala de 5 pontos de Likert, de Nada satisfeito (1) a Totalemte satisfeito (5) e, as restantes respondidas afirmativa ou negativamente. Para o tratamento estatístico foram utilizados os softwares The Statistical Package for Social Sciences® (IBM SPSS statistics v24) e Microsoft Excel 2016® e, empregues os testes t de amostras emparelhadas, Wilcoxon, McNemar e t de amostras independentes. A integração dos documentos DICOM SR com o Dicoogle permitiu a conceção do Repositório DICOM SR, onde é possível armazenar imagens e Relatórios e, pesquisar e recuperar dados relativos aos Relatórios. A avaliação da aceitação dos Relatórios FT vs. SR revelou que, sem distinção entre as especialidades médicas, existe uma tendência global favorecendo os FT. Considerando os Médicos de Ginecologia/Obstetrícia e de MGF, estes não demonstraram diferenças significativas FT vs. SR (valor p > 0,05). Os Radiologistas apresentaram-se em média (Dp) 3,86 (0,91) satisfeitos com a Estrutura do Relatório FT e 2,84 (0,59) com a do SR, a Clareza e assertividade da linguagem utilizada apresentou uma média (Dp) de 3,92 (0,90) para os FT e de 2,94 (0,50) para os SR e, o Diagnóstico e Recomendações satisfez, em média (Dp), 4,16 (0,71) nos FT e 3,22 (0,47) nos SR. Concluiu-se que a constituição de um Repositório DICOM SR possibilita o armazenamento de Imagens e Relatórios de forma conjunta e permite a pesquisa, recuperação e consulta de dados referentes aos Relatórios, tarefas impraticáveis com os sistemas de Relato FT. Foi possível, também concluir, que os Médicos de Ginecologia/Obstetrícia e MGF aceitam, de forma semelhante, a comunicação de resultados em Imagiologia Mamária por Relatórios FT e SR. O mesmo não acontece com os Radiologistas que consideram os Relatórios FT mais claros e assertivos na comunicação de resultados e no Diagnóstico e recomendações. Assim, melhorias aos SR devem ser implementadas e estratégias que estimulem a sua aceitação pelos Radiologistas devem ser delineadas, impulsionando a comunicação global na comunidade médica e otimizando o cuidado aos pacientes.
Breast Cancer is the most common type of cancer affecting european women, therefore the early detection, diagnosis and treatment efficiency are crucial. The Structured Reports (SR) emerge as an alternative of Free Text (FT) and the standard Digital Imaging and Communication in Medicine (DICOM) extend his application to SR with DICOM Structured Reports (DICOM SR). These dissertation main goals are the creation of a DICOM SR repository and the comparison of acceptance of FT versus SR by Radiologists and nonRadiologists, regarding the communication of Breast Imaging findings. The study embraces two phases. The first one, concerns the creation of MamoCatalogue that allowed structuring the Mamography and Breast Ultrassound Reports, according DICOM terminology, after converted into DICOM SR and integrated with Dicoogle - Picture Archiving and Communication System (PACS) open source. In the second phase seven representative Mamography and/or Ultrassound Breast Imaging Reports were selected and evaluated by a group of 25 physicians, Radiologists and nonRadiologists (Gynecologists and General Practitioners - GP). Each physician evaluated the seven Reports in FT and SR formats with a 3 months time-lag. A Questionnaire was designed addressing the following issues: Structure, Clarity and assertiveness, Diagnostic/Recommendations, Easiness of reading, Full Reading, Partially Reading with BIRADS focus and Ambiguity. Physicians rated the first four variables, on a 5-point Likert scale, from 1 (very dissatisfied) to 5 (totally satisfied) and the others with yes or no. The Statistical Package for Social Sciences® (SPSS vv24) and the Microsoft Excel 2016® were used for the statistical analysis, using paired t test, Wilcoxon, McNemar and t test for independent samples. A DICOM SR repository was created and integrated with Dicoogle, where search, query and retrieve operations are possible. The DICOM SR repository, also allows that Breast Images and related Reports are stored and displayed together. The statistical analysis showed a global trend favoring FT. Although, for Gynecologists and GP the differences between FT and SR weren’t significant (p value > 0.05), unlike what happens with Radiologists. The mean and standard deviation acceptance metrics for the Radiologists were as follows: 3.86 (0.91) in FT Structure and 2.84 (0.59) in SR, 3.92 (0.90) in FT Clarity and assertiveness and 2.94 (0.50) in SR and 4.16 (0.71) in FT Diagnosis/Recommendation and 3.22 (0,39) in SR. So, it is concluded that DICOM SR repository allows the storage of images and Reports, and let’s search, retrieval and query operations related to Reports data, impracticable task with FT systems. It follows, also, that Gynecologists and GP similarly accept the communication of results in Breast Imaging by FT and SR. Nevertheless, Radiologists considered FT Reports clearer and more assertive than SR in communication of Breast Imaging findings and Diagnostic/Recommendations. There are still improvements to perform in SR and its acceptance, by Radiologists, must be reinforced upon a global communication on clinical community and improvement of patient health care.

Métodos de planejamento cirúrgico baseado em imagens de tomografia computadorizada de feixe cônico são utilizados para aprimorar a terapêutica em Implantodontia. Esta tecnologia possibilita a manipulação das imagens pelo Cirurgião-Dentista. Para isto, é necessário que a imagem enviada pela clínica de radiologia tenha sido reformatada/convertida, para um programa de planejamento cirúrgico virtual; necessário para tornar o exame compatível com a capacidade de processamento dos computadores domésticos. O objetivo desta pesquisa foi comparar a nitidez e a medida da altura de 55 exames tomográficos em um corte coronal da região mandibular posterior inferior entre os arquivos DICOM reformatados em 3 (3X-RF) e 5 vezes (5X-RF) e o não reformatado (S-RF). Em valores absolutos o grupo S-RF apresentou imagens mais nítidas que os demais. Foi significativa a análise da variância das fontes de variação, inerentes ao processo de medição (p≤0,05). Pelo Estudo de Gage R&R concluiu-se que os critérios de medições da altura do lado direito (MID) e do lado esquerdo (MIE), não foram alterados nos grupos S-RF, 3X-RF e 5X-RF.
Methods of surgical planning based on images from cone beam computed tomography are used to improve therapy in Implantology. This technology allows the manipulation of images by the Dental Surgeon. For this, it is necessary that the image sent for clinical radiology has been resized / converted to a program of virtual surgical planning, necessary to make the examination compatible with the processing power of home computers. The aim of this study was to compare the sharpness and height measurement of coronal section of the mandibular posterior region in 55 CT scans between DICOM files reformatted in 3 (RF-3X) and 5 (RF-5X) times, and not reformatted (S-RF). The S-RF group showed sharper images than the RF-3X and 5X-RF groups. The variance analysis of the sources of variation inherent in the measurement process was statistically significant between groups (p ≤ 0.05). By Gage R & R study concluded that measures of height of coronal mandibular right and left, did not differ in groups S-RF, RF-3X and 5X-RF.

Full textThe digital image on DICOM format request a large store size capability, making difficult the transmission through internet, usually requesting the image compression via JPEG file format. The aim of this study was to evaluate the influence of DICOM and JPEG file formats on Quality Factors 100, 80 and 60, in the reproducibility intra and interexaminer at cephalometrics landmarks identification at digital Frontal Teleradiographies. The sample consists of 120 digital images of Frontal Teleradiographies, obtained from 30 individuals. The 30 original images in DICOM format was converted into a JPEG format on Quality Factors 100, 80 and 60. After blind and random the sample, three calibrated orthodontist marked 18 cephalometric points on each image using a computerized cephalometric program which has in account cephametric points values in a X and Y cartesians coordinates system. At the results, the statistical tests at intraclasses correlations and variant analysis (ANOVA) presents a reproducibility agreement of the cephalometric points on digital Frontal Teleradiographies on intra such as interexaminer, except for points ZL, ZR, AZ, JR, NC, CN on coordinate Y and A6 on coordinate X, independently of file formats. As a conclusion, the file formats DICOM and JPEG on Quality Factors 100, 80 and 60 do not affected the reproducibility intra and interexaminers on a cephalometric landmarks identification.(AU)
A imagem digital no formato DICOM requer grande espaço para armazenamento, dificultando o arquivamento e transmissão da imagem via internet, sendo necessária, muitas vezes, a compressão das imagens por meio de formatos de arquivo como o JPEG. O objetivo neste estudo foi avaliar a influência dos formatos DICOM e JPEG, nos Fatores de Qualidade 100, 80 e 60, na reprodutibilidade intra e interexaminador na marcação de pontos cefalométricos em Telerradiografias digitais em Norma Frontal. A amostra consistiu de 120 imagens digitais de Telerradiografias em Norma Frontal, obtidas de 30 indivíduos. As 30 imagens originais, em formato DICOM, posteriormente, foram convertidas para o formato JPEG, nos Fatores de Qualidade 100, 80 e 60. Após cegar e randomizar a amostra, três ortodontistas calibrados marcaram os 18 pontos cefalométricos em cada imagem utilizando um programa de cefalometria computadorizada, que registra as medidas dos pontos cefalométricos em um sistema de coordenadas cartesianas X e Y. Nos resultados, os testes estatísticos de correlações intraclasses e análise de variância (ANOVA) apresentaram concordância de reprodutibilidade dos pontos cefalométricos em Telerradiografias digitais em Norma Frontal, tanto intra como interexaminador, com exceção dos pontos ZL, ZR, AZ, JR, NC, CN na coordenada Y e A6 na coordenada X, independentemente dos formatos de arquivo. Em conclusão, os formatos de arquivo DICOM e JPEG, nos Fatores de Qualidade 100, 80 e 60, não afetaram a reprodutibilidade intra e interexaminador na marcação dos pontos cefalométricos.(AU)

Full textThe use of quantitative analysis in medical radiology has been of great value in the detection of not accessible alterations in the simple visual analysis, said qualitative, for being very subtle, or for not being present in conventional magnetic resonance image techniques. However, certain types of quantification demand the acquisition of high cost softwares and computational platforms, beyond specialized workmanship, with technical knowledge in computation, to operate in non intuitive environments. In this scenery the objective of this work was the implementation of a software for analysis of transference of magneti zation in nuclear magnetic resonance images that works in IBM-PC platform and free operational systems as GNU/Linux. So, an algorithm for reading of standard DICOM 3.0 codified images was elaborated, an algorithm for the construction of Magnetization Transfer Ratio maps of acquired volume, and a visualizer with friendly interface for segmentation and analysis of the results. Finally the software made the opening of DICOM image possible. It also generated in efficient way the maps of percentage difference among the images without and with the pulse of magnetization transfer (MT), also making devices of movement corrections possible, when they are not very intense. It allowed the delineation of regions of irregular interest, with good visibility of the results. As standard control, the results were compared with the set of tools of the McGill University (Brain Imaging Center, McGuill University, Montreal, Quebec, Canada), widely tested in published articles. The elaborated program took care of the considered objectives.
