Abstract:
ZusammenfassungUm Schlaganfall-Patienten eine effiziente Sekundärprophylaxe anbieten zu können, ist eine strukturierte Diagnostik hinsichtlich der vermuteten Schlaganfallätiologie auf einer Stroke Unit unabdingbar. Bei den ätiologisch „kryptogenen” Schlaganfällen wird zunehmend ein paroxysmales Vorhofflimmern (pVHF) als mögliche auslösende Ursache gefunden. Lässt sich das permanente Vorhofflimmern (VHF) einfach durch ein 12-Kanal-EKG detektieren, kann das häufig klinisch asymptomatisch und in Clustern auftretende pVHF diagnostische Schwierigkeiten bereiten. Aktuelle Studien konnten belegen, dass ein prolongiertes EKG-Monitoring über die derzeit empfohlenen 24 Stunden hinweg häufiger zur Detektion von pVHF führt. Dies hat direkte sekundärprophylaktische Konsequenzen, da in den meisten Fällen dann eine orale Antikoagulation indiziert ist. Sowohl permanentes als auch pVHF sind mit einem erhöhten Re-Schlaganfallrisiko vergesellschaftet. Insbesondere vor dem Hintergrund der Effektivität direkter oraler Antikoagulantien, kommt damit der Diagnostik von pVHF eine besondere Bedeutung zu. In der vorliegenden Übersicht werden die Möglichkeiten einer optimierten pVHF Diagnostik diskutiert.

ABSTRAK Pendahuluan: Acute ischemic stroke (AIS) merupakan jenis stroke dengan insidensi tertinggi utamanya pada kelompok usia lanjut. Saat ini, neuroimaging menjadi modalitas utama dalam penegakan diagnosis stroke, akan tetapi masih memiliki beberapa kelemahan, seperti ketersediaan yang terbatas, biaya yang mahal, waktu yang cukup lama untuk prosedurnya, serta pada beberapa kasus ditemukan hasil interpretasi gambar yang sering tidak konsisten pada stroke fase awal. Sehingga diperlukan alat diagnostik yang cepat, tepat, dan sederhana untuk diagnosis AIS. Saat ini, tipe tertentu dari circulating microRNAs (miRNAs) ditemukan dapat menjadi biomarker untuk diagnostik AIS. Tujuan kajian ini untuk melakukan profiling sekaligus mengevaluasi nilai diagnostik circulating miRNA pada pasien AIS dibandingkan dengan pasien hemorrhagic stroke (HS) dan individu sehat. Metode: Tinjauan sistematis ini mengumpulkan literatur dari PubMed, Science Direct, Clinical Trial.gov, Directory of Open Access Journal (DOAJ), dan Epistemonikos menggunakan metode standar dengan pedoman cochrane untuk tinjauan sistematis guna mengevaluasi nilai diagnostik dari circulating miRNA sebagai biomarker untuk AIS berdasarkan sensitivitas, spesifitas, dan nilai Area Under the Curve (AUC). Pembahasan: Dari 870 studi, 15 studi sesuai dengan kriteria inklusi. Dari studi yang ditinjau menunjukkan bahwa hsa-miR-106b-5P dan kombinasi miR-125a-5p + miR-125b-5p + miR-143-3p sebagai jenis miRNA yang paling efektif menjadi biomarker dalam mendiagnosis AIS dan membedakannya dengan individu sehat. Sedangkan miR-124-3p merupakan jenis miRNA yang paling efektif menjadi biomarker untuk membedakan AIS dan HS. Simpulan: Circulating miRNA yang diisolasi dari plasma, serum, dan darah manusia dapat digunakan sebagai biomarker diagnostik untuk AIS. Abstract:
There is a large unmet need for fast and reliable diagnostics in several diseases. One such disease is stroke, where the efficacy of modern reperfusion therapies is highly time-dependent. Diagnosis of stroke and treatment initiation should be performed as soon as possible, and preferably before arrival at the stroke center. In recent years, several potential blood biomarkers for stroke have been evaluated, but without success. In this review, we will go into detail on the possibility of utilizing extracellular vesicles (EVs) released into the blood as novel biomarkers for stroke diagnostics. EVs are known to reflect the immediate state of the secreting cells and to be able to cross the blood–brain barrier, thus making them attractive as diagnostic biomarkers of brain diseases. Indeed, several studies have reported EV markers that enable differentiation between stroke patients and controls and, to a lesser extent, the ability to correctly classify the different stroke types. Most of the studies rely on the use of sophisticated and time-consuming methods to quantify specific subpopulations of the nanosized EVs. As these methods cannot be easily implemented in a rapid point of care (POC) test, technical developments followed by prospective clinical studies are needed.

The goal of the thesis was applying multiparametric diagnostics to determine the state and wear of a high-speed milling machine. In the theoretical section, selected areas of multiparametric diagnostics have been described as well as a significant focus on vibrodiagnostics. The chosen methods for the practical section were incorporated into the group “metrological diagnostics”. Several different types of measurements have been performed on the designed test piece. The measurements were analyzed and lead to these specific results: a drawing of the test piece, a structured methodology for evaluating the test piece, recommendations for diagnostics of the specific machine, and proposals for the future development of the field of metrological diagnostics.

Abstract:
The main idea is focused on the diagnostics of a specific hydraulic system, i.e. sensing the physical quantities of the hydraulic circuit with a tank and a centrifugal pump driven by an asynchronous motor. It is a system of pipes connected to the pump, where due to its work it creates a water flow and a pressure increase. In practice, this issue is also addressed in the energy and nuclear industries. Primary circuits in some cases cannot be designed or modified to be able measure locally the pressure value. It is necessary to measure this quantity indirectly - from the motor currents. The main idea of the work is to diagnose the system by an indirect method - specifically to detect the state of the hydraulic circuit (pressure, flow) from the values that we are able to measure and detect damage in advance. In the second part of the thesis is the application of the parts of a specific hydraulic system in the simulation environment MATLAB Simulink. The model of the hydraulic circuit contains mathematical-physical relations that simulate the course of the mentioned experiment. The results of the simulation are compared with the results of the experiment. The model also investigates the simulation of a fault condition, when we supply pressure pulsations to the hydraulic circuit. It is these changes in the hydraulic part that affect the characteristics of the pump and the asynchronous motor, so we are able to diagnose this system.

The next part describes the paper machine, diagnostic system and the measuring points are classified into databases. The practical part of this work consists of a proposal for the methodology of vibration diagnostics for a paper machine, the procedure for measuring and evaluating the measured values. Part of the work is also a practical example of the measurement of the central cylinder, on which the procedure is tested, and the compilation of new limit values.

The first part of the thesis deals with problems theoretically, there are described the machine tools, maintenance, methods of technical diagnostics, specifically online vibrodiagnostic systems and an overview of profile and area parameters. In the practical part, two test samples created under ideal and degraded conditions. In the practical part, two signal processing processes under ideal and degraded conditions are compared using signal analysis from an online vibrodiagnostic system and surface texture analysis. Part of the work is the evaluation of the performed analyzes and recommendations for practice.

The thesis Diagnostics of rotating electric machines deals with the theory of diagnostics of rotating machines, focusing especially on the diagnosis of asynchronous motors of high power. Due to the importance of maintaining the trouble-free condition of these machines with regard to serviceability, the importance of their diagnosis is growing. Due to inappropriate working conditions or for many other reasons, different types of malfunctions may occur in the machine. The list and the way of occurrence of possible failures are included and explained in this thesis. With the growing age of the machine, it is important to check the insulation status through insulation testing. These tests are performed in the practical part of the thesis and the results are presented. In the case of asynchronous machines, each anomaly leaves a signature in the form of an electrical, magnetic or vibrational signal. These signals are analyzed and evaluated using a specific measuring equipment with the intention of deciding on the current state of the machine. Motor current signature analysis (MCSA) is considered to be the main diagnostic method of asynchronous machines in this thesis. This method is described in detail in this paper and is being applied in electrical engineering.

It tries to apply all the information about mentioned features to concrete branch of machines ¨C highŠ\voltage electrical rotary machines. Practical part of this thesis consists of statistical analysis of measured parameters by two hydroelectric generators. Main task of this thesis is to transpose theoretical knowledge of safety and technical systems to the branch of highŠ\voltage electrical rotary machines. This thesis also tries to find appropriate and more efficient evaluation method for analyzing the parameters of mentioned machines. Another benefit of this thesis should be bringing few new ideas about evaluating safety and technical status of highŠ\voltage electrical rotary machines. Abstract:
A clinical trial of the diagnostic kit for analysis of the genetic predisposition to ischemic stroke was carried out on an 767 independent blood samples. Genetic risk’s values of ischemic stroke were determined in the groups of patients and control, and its distribution between 6 groups of SNP combinations was analyzed in accordance with the suggested model. The differences by genetic risk values between the patient’s samples and the control group were determined as statistically significant (p<0.01) for two groups of loci associated with an increased risk of atherothrombotic and cardioembolic ischemic stroke subtypes in the corresponding patient’s subgroups. Registration certificate No. RZN 2019/8874 dated 05.09.2019 was issued according to the results of the clinical trial of the diagnostic kit for determining the genetic risk of ischemic stroke «AmpliSens ® Genoscreen Stroke SNP-FL».
