Abstract:
Enzootic bovine leukosis (EBL) is a notifiable infectious disease with sporadic fre-quency in Albania. EBL is not a zoonotic disease, but it has an important impact on the cattle trade. Diagnosis of this disease can be performed by many diagnostic proce-dures, including agar gel immunodiffusion (AGID), enzyme-linked immunosorbent assay (ELISA), and polymerase chain reaction (PCR) tests. In some cases, where persis-tent lymphocytosis is present, cell count can turn useful. This study aimed to evaluate serological and hematological tests' suitability for the diagnosis of EBL and assess the manual and automatic hematological count technique diagnostic parameters com-pared to the serological test. Results suggest that strategic use of hematological and AGID tests in parallel can increase the sensitivity of diagnosis and facilitate the detec-tion of infected animals in different disease stages. Moreover, our results indicate that this approach is feasible in small scale cattle herds size, which properly fits Albanian circumstances

Abstract:
The paper presents the methodology of testing common rail fuel injectors, which consisted in extending the standard diagnostic proce-dures with the analysis of the resulting fuel delivery fields. The calculations were carried out in a popular spreadsheet, using Gauss's formulas (the so-called shoelace formula). In this way, the necessity to modify the test bench software was eliminated, as the analytical process took place after the end of the active experiment phase. It has been shown that the proposed solution should be particularly useful in problematic situations where clear-cut verification and assessment of the technical condition of the fuel injectors is sometimes difficult, as shown in the example. In addition, implementation in a digital environment allows the presented algorithms to be reused in research with a similar profile.

Abstract:
We report a case of a 33 years old female patient anxious for issues since 2 years. Patient was having Bad Obstetric History (BOH) in her previous 2 pregnancies with history of Ectopic pregnancy in her last preg-nancy for which linear salpingostomy was done. Patient underwent diagnostic investigations and proce-dures to rule out the cause. She was found to have Bilateral Hydrosalpinx in her HSG findings. So, the treatment was planned accordingly, and she was treated with Ayurvedic regimen consisting of Shodhana and Shamana therapy. HSG was repeated 6 months after treatment which was found to be normal with Bi-lateral patent tubes and she was managed to conceive successfully after treatment. Though she was a K/C/O BOH also, so she was provided all the necessary advice and examinations in her Antenatal period including Masanumasika Garbhini Paricharya and she delivered healthy female child of 2.8 kgs on 21.11.2020. So, implementation of Ayurvedic approach with X ray HSG resulted in successfully treating this case without tubal corrective surgery.

From Research to Clinical Application Multi-Parameter Testing: Marker Panels for the Early Detection of Complex DiseasesMulti Parameter Analysis can open novel diagnostic opportunities for the early diagnosis and screening of multimodal diseases like cancer. Single proteins have so far failed to describe such complex diseases. Being able to screen with a set of analytes is one promising way to overcome the present limitations. Various marker identification tools including proteomics approaches have been successfully applied to identify new screening markers for early detection of colorectal cancer (CRC). CRC is one of the most incident cancers worldwide and early detection is clearly a key factor in reducing mortality from CRC. Several screening methods are recommended, including colonoscopy, fecal occult blood testing (FOBT) and fecal DNA analysis. Of these annual stool testing with the guaiac based FOBT is most often applied, in spite of limitations such as low sensitivity and dietary influences. Though proce dures with improved performance eg. immunolo gical FOBT are available, a screening assay for CRC in serum that could easily be integrated in any health check-up would be highly welcome. A positive result of such a test would trigger a follow-up colonoscopy for an exact diagnosis. In this review we will cover aspects of marker identification strategies and describe a well structured marker validation process that is based on clinically characterized sample materials. Finally the value of analytical multi-parameter platforms enabling the combination of multi markers in routine diagnostics settings is outlined. An appropriate multi-parameter immunochemistry platform concept, currently developed under the working name »IMPACT« will be introduced.

This paper describes the current commercially available US probe types, lists some of their clinical applications and briefly explains the technologies that are responsible for recent enhancements in image quality and ergonomics. Our intention is to summarize information that will allow healthcare professionals to select the appropriate probe for the intended use and the desired performance-price ratio.

Abstract:
The purpose of this research was to proce or disprove the widely held beliefs that X-ray radiation used in medical or dental applications may affect the normal oral flora, and may also have effects on the micro-organisms existing in the oral cavity of every person. Such beliefs might be due to the common knowledge that radiation therapy has been utilized for the long time in the cancer patients to destroy the cancerous cells, and that radiation are also widely used in agriculture to prolong the shelf life of the farm products by destroying the decay-causing microorganisms existing in the agricultural products such as meats, produce, etc. Since very few research has been conducted in this concerned area, and not much pertinent information in the scientific literature could be found in this regard, a research experiment was set up to find out if these common beliefs have any merits. The research experiment involved selecting BDORT(By-digital O-Ring Test) positive dishes to incubate human saliva collected from a volunteer patient. Once dish served as control which was not subjected X-ray irradiation. Other dishes were subjected X-ray irradiation with various doses of irradiation strength at various time interval such as 1, 3, 5, 7, and 9 seconds to find out if any significant change taken place in the bacteria colonies. The bacterial colonies in the irradiated dishes then compared with that of the control dish so as to draw a conclusion if radiation for medical and dental X-ray machines would actually have any bactericidal or/and bacteriostatic effects on the oral micro-organisms flora. Finally, dishes were brought to a local hospital Radiology Department to use high dose of irradaition (used for cancer therapy) on the dishes to find out to what extent if such a high dose X-ray irradiation emitted from therapeutic cancer therapy machine would affect the number of bacterial colonies in the dishes as when compared with when dishes were under low doses of X-ray irradiation from dental or medical diagnostic X-ray machines. During the experiment, dishes, when irradiated, were sent to a certified medical technologist in the hospital laboratory to count the number of the bacterial colonies, and each change in the bacterial colony number were recorded in to data for biostatic analysis. The data were compared with BDORT negativity scores collected each time before and after dishes were irradiated, thus to find out if any corresponding confirmity between traditional laboratory findings and the score changes from BDORT findings. The results of this experiment revealed the fact that radiation from regular diagnostic X-ray machines possess little effect on bacteria, and that they have little effect on both cells and bacteria present in the human body. Neverthless, dramatically increasing the high radiation dosages does have potential to both inhibit and destroy bacteria. Therefore,using irradiation techniques to inhibit bacterial growth is only useful in agriculture practices, since tens of thousands of irradaiation dosages are necessary for bacteria inhibition. in human, over 500 RADs (radiation absorption dose) is lethal. Therefore, bacterial inhibition could not be practical in humans since it requires tens of thousands of RADs to be useful in the aspect.

Abstract:
A collective Thomson scattering (CTS) diagnostic with a ±3 GHz band around a 77 GHz gyrotron probe beam was developed to measure the velocity distribution of bulk and fast ions in high-temperature plasmas. We propose a new in situ calibration method for a CTS diagnostic system combined with a raytracing code. The method is applied in two situations for electron cyclotron emission in plasmas and in a CTS diagnostic with a modulated probe beam. Experimental results highlight the importance of refraction correction in probe and receive beams. The CTS spectrum is measured with the in situ calibrated CTS receiver and responds to fast ions originating from a tangential neutral beam with an energy of 170 keV and from a perpendicular beam with an energy of 60 keV, both in the large helical device. From a velocity space analysis model, the results elucidate the measured anisotropic CTS spectrum caused by fast ions. The calibration methods and analyses demonstrated here are essential for CTS, millimeter-wave diagnostics, and electron cyclotron heating required under fusion reactor conditions.

La negligenza spaziale unilaterale (NSU) è un disturbo neuropsicologico dovuto a eventi cerebrovascolari (CVA), tumori cerebrali e lesioni cerebrali acquisite. I pazienti con NSU falliscono nell’orientarsi verso, rispondere e riportare eventi sensoriali che si verificano nel lato dello spazio e del corpo controlaterale al lato della lesione (tipicamente, il lato sinistro in pazienti con danno cerebrale destro), e nell’esplorare queste porzioni di spazio. La valutazione della NSU si basa soprattutto sulla valutazione delle prestazioni dei pazienti a test carta-e-matita. Tuttavia, sono ora disponibili evidenze emergenti che le tecnologie che si basano sui computer possano migliorare le procedure diagnostiche. I test computerizzati possono offrire più sensibilità e flessibilità, registrando molte più informazioni (ad es., l’accuratezza e i tempi di reazione simultaneamente). Queste caratteristiche riducono le possibilità di errore umano e permettono misurazioni quantitative e continue nei singoli pazienti, includendo un monitoraggio individuale e sensibile dei cambiamenti prestazionali lungo il corso di ripetute valutazioni. L’obiettivo principale della presente tesi di dottorato era quello di validare preliminarmente dei test digitali appositamente creati, comparandoli a test carta-e-matita tradizionalmente usati nella pratica clinica per la diagnosi di NSU. Tutti questi test sono stati creati in un ambiente digitale facile da usare e basato sull’utilizzo di uno schermo touch e di una penna digitale, così da simulare il tradizionale atto di scrittura. All’interno di tre studi, i partecipanti sani e i pazienti con danno cerebrale destro, con e senza NSU, sono stati sottoposti sia ai testi digitali che ai test carta-e-matita. Le analisi dei dati hanno mostrato risultati promettenti, suggerendo che i test digitali potrebbero condurre a una valutazione neuropsicologica più sensibile e precisa, smascherando inoltre casi di neglect residuale. Infine, i risultati presenti incoraggiano la raccolta di dati normativi per poter adottare i test digitali nella pratica clinica.
Unilateral Spatial Neglect (USN) is a neuropsychological disorder due to cerebrovascular accidents (CVA), cerebral tumours and brain injuries. USN patients fail to orient towards, respond to and report sensory events occurring in the side of space and the body contralateral to the side of the lesion (typically the left side in the right-brain-damaged patients), and to explore these portions of space. The assessment of USN has relied mainly on evaluating patients’ performances at paper-and-pencil tests. Nevertheless, emerging evidence that computer-based technologies may improve the assessment procedure is now available. More sensitivity and flexibility may indeed be offered by computerised tests, which typically record much more information (i.e., accuracy and reaction time measures simultaneously). These features reduce the chances of human error and allow for quantitative, continuous measures and even significance levels in single patients, including sensitive individual monitoring of performance changes through repeated assessments. The main aim of the present PhD thesis was to preliminarily validate purposely created digital tests, compared to similar classic paper-and-pencil tests traditionally used in the clinical setting for USN assessment. All these tasks were built up in a user-friendly digital environment based on touch screens and digital pens, simulating the act of filling in a paper-and-pencil set-up with the advantages of the touch system. Within three studies, healthy participants and right-brain-damaged patients with and without USN performed both the digital and the paper-and-pencil versions of tests. Data analyses showed promising results, suggesting that digital tests could lead to a more sensitive and precise neuropsychological evaluation, also unmasking cases of covert USN. Lastly, the present findings encourage the collection of normative data in order to adopt digital tests in clinical practice.

L’imagerie proton est étudiée comme alternative à la tomodensitométrie X pour la planification de traitement en hadronthérapie. En obtenant directement les pouvoirs d’arrêt relatifs des tissus, l’incertitude sur le parcours des particules pourrait être réduite. Un scanner à protons est constitué d’un calorimètre ou d’un détecteur de parcours afin d’obtenir l’information sur l’énergie déposée par chaque proton dans l’objet imagé et de deux ensembles de trajectographes enregistrant la position et direction de chaque particule en amont et en aval de l’objet. Ce travail concerne l’étude des données d’un scanner à protons et l’utilisation possible de toutes les informations enregistrées. Une étude de reconstruction d’image a permis de montrer que les informations sur le taux de transmission et sur la déviation de chaque particule peuvent être utilisées pour produire des images aux propriétés visuelles intéressantes pour le diagnostic. La preuve de concept de la possibilité d’une imagerie quantitative utilisant ces informations est présentée. Ces résultats sont une première étape vers l’imagerie proton utilisant toutes les données enregistrées
Proton computed tomography is being studied as an alternative to X-ray CT imaging for charged particle therapy treatment planning. By directly mapping the relative stopping power of the tissues, the uncertainty on the range of the particles could be reduced. A proton scanner consists in a calorimeter or range-meter to obtain the information on the energy lost by each proton in the object, as well as two sets of tracking planes to record the position and direction of each particle upstream and downstream from the object. This work concerns the study of the outputs of a proton scanner and the possible use of all the recorded information. A reconstruction study made it possible to show that the information on the transmission rate and on the scattering of each particle can be used to produce images with visual properties that could be of interest for diagnostics. The proof of concept of the possibility of quantitative imaging using this information is also put forward. These results are the first step towards a clinical use of proton imaging with all the recorded data

Abstract:
Treatment success often depends on the availability of accurate and reliable diagnostic assays to guide clinical practitioners in their treatment choices. An optimal test must excel in specificity and sensitivity, and depending on the application area time, low-cost and simplicity are equally important. For instance, time is essential in infectious diagnostics but this is less important in non-invasive prenatal testing (NIPT). In NIPT, specificity and sensitivity are the most important parameters. In this thesis I describe the development of four different methods, all based on padlock probes and rolling circle amplification, intended for molecular diagnostics. Application areas range from infectious disease diagnostics to NIPT and oncology. The methods described have in common that they overcome certain limitations of currently available assays. This thesis includes two new assays targeting infectious agents: one assay specifically detecting a highly variable double stranded RNA virus and the second assay demonstrating a new format of antibiotic susceptibility testing, which is rapid and generally applicable to different pathogens. Furthermore, I describe the development of a method that uses methylation markers to enrich fetal DNA, accurately quantify chromosome ratios and thus, detecting trisomy 21 and 18. The fourth method described in this thesis uses gap-fill ligation of padlock probes to detect diagnostic relevant point mutations with high specificity in situ. The assays presented have the potential, after automation and successful validation and verification studies, to be implemented into clinical practice. Furthermore, these assays demonstrate the wide applicability of padlock probes which, due to their properties in regard to specificity and multiplexity, are useful tools for nucleic acid detection in vitro as well as in situ.

NASAâ s VAriable Specific Impulse Magnetoplasma Rocket Engine (VASIMR) will provide a highly efficient propulsion source that can dramatically reduce Martian transit times, provide for more abort contingencies, and protect astronauts from space radiation with its highly radiation-absorbent hydrogen fuel. The VASIMR is still in its developmental infancy and requires many years of research before its initial operational capability. Much is still unknown about the complex plasma interactions in the exhaust. A Langmuir probe was designed, constructed, and operated to determine current density radial profiles and total particle flux at various stages in the exhaust of VASIMR. The Langmuir probe results proved that the exhaustâ s radial profile is Gaussian, experimentally validated predictions of magnetic field line dragging, and verified the ionization efficiency of VASIMR.

Abstract:
The ionosphere is the atmosphere layer characterized by its high concentration of ionized plasma. It has a great impact on radio communications with satellites, causing disturbances and disruptions. Therefore, it is important to understand and predict the ionosphere characteristics. The Sweeping Impedance Probe (SIP) is an instrument for characterizing the ionosphere used for many decades with great success. In this thesis, a new SIP architecture design is presented using the latest techniques and components available. The design is detailed and analyses have been performed to ensure the required performances. The new SIP will be flown in the Auroral Spatial Structures Probe (ASSP) sounding rocket mission in early 2015, and it is expected it will make the most accurate measurements to date. Lastly, the conclusions of this project are presented and future work is outlined for what will become the next generation of SIP instruments.

L'objectif de cette these est la detection de points de fuite electromagnetiques sur des structures metalliques. L'approche consiste en une transformation champ proche / champ tres proche, en utilisant les courants induits sur l'objet comme donnee intermediaire. La structure testee est consideree comme une antenne a l'emission dont on mesure le champ en zone proche en coordonnees spheriques. Ce champ proche sert de base a l'inversion pour reconstruire les courants induits sur l'objet rayonnant. On suppose connues la forme de l'objet et sa position dans l'espace. Une fois le courant sur l'objet rayonnant connu, le champ peut etre calcule en n'importe quel point de l'espace et, en particulier, en zone tres proche. Il s'agit d'un probleme de diffraction inverse de type source, tridimensionnel, vectoriel, lineaire, mais mal pose. L'equation de base est l'efie (electric field integral equation), la methode de regularisation est de type tikhonov associee a la methode de validation croisee generalisee pour choisir le parametre de regularisation. Une premiere phase de validation est realisee sur des structures filaires. Le code de calcul nec est utilise pour obtenir des resultats de reference. Cette etape demontre la viabilite de l'algorithme et permet son evaluation vis-a-vis d'erreurs aleatoires et systematiques. La methode est ensuite appliquee a des objets complexes a partir de mesures reelles de champ proche. Les tests sont conduits sur des objets de 50 cm, entre 600 mhz et 3700 mhz. Les resultats attestent des capacites de la procedure de diagnostic a localiser les points de fuite et/ou a discriminer parmi plusieurs zones potentielles de rayonnement

A l'aide de ces champs magnétiques externes, il est possible d'obtenir une cartographie de la densité de courant interne par résolution d'un problème inverse. Ce problème est néanmoins mal posé : la solution est non unique et est extrêmement sensible au bruit. Des techniques de régularisation ont ainsi été mises en place pour filtrer les erreurs de mesures et obtenir une solution physiquement acceptable. Afin d'augmenter la qualité de reconstruction des courants, nous avons conçu notre outil de diagnostic de manière à ce qu'il soit uniquement sensible aux défaillances de la pile (capteur de défauts). De plus, cette reconstruction se base sur un nombre extrêmement faible de mesures. Une telle approche facilite l'instrumentation du système et augmente la précision et la rapidité de celui-ci. La sensibilité de notre outil à certaines défaillances (assèchements, appauvrissement en réactifs, dégradations) est démontrée.

The transmitted power of motors can be defined by test bench examinations, in which the motor needs to be dismounted. By applying diagnostic methods only minimal disassembly, or indeed none at all, is necessary. In this paper diagnostic methods are systematized by different studies. Moreover, a new diagnostic procedure (established by the authors) is also introduced, which can be carried out by a relatively cheap measuring system design. Power measurement on a roller test bench is a well-tried technique for vehicle examination. Basically, the wheel parameters (tractive force, torque, power) are measured, but there are also procedures for motor power measurement. By applying the new procedure, the diagnostic method, (simple construction and test bench construction at a favourable price) torque and power characteristics of the mounted motor can be defined. The theoretical considerations of the new measuring technique and the implementation of the measuring system are introduced in this paper. Test bench for drive motor power and torque curve determination. In this study the diagnostical measuring systems and, for the first time, the measuring principle and technology of the new theory-based roller power test bench are presented. For its construction measurements have been carried out which make an important contribution to the research. The results of the research contribute to the development of diagnostics methods and to making them more wide-spread.

Gas turbine diagnostic techniques are often based on the recognition methods using the deviations between actual and expected thermodynamic performances. The problem is that the deviations depend on real operating conditions. However, our studies show that such a dependency can be reduced. In this paper, we propose the generalized fault classification that is independent of the operating conditions. To prove this idea, the averaged probabilities of the correct diagnosis are computed and compared for two cases: the proposed classification and the traditional one based on the fixed operating conditions. The probabilities are calculated through a stochastic modeling of the diagnostic process, in which a thermodynamic model generates deviations that are induced by the faults. Artificial neural networks recognize these faults. The proposed classification principle has been realized for both, steady state and transient operation of the gas turbine units. The results show that the acceptance of the generalized classification practically does not reduce the diagnosis trustworthiness.

Sensors installed on gas turbine gas path are used to obtain gas path measurement parameters for control and condition monitoring purpose. These sensors are prone to degradation or failure due to hostile working environment around them. Most gas path sensor diagnostic research is based on an assumption that the power setting sensor, a sensor used by engine control system to control engine power output, has no fault so engine measurement data can always be obtained at desired operating conditions. However in practice, power setting sensor may also be faulty, which may result in misleading measurement data and diagnostic results. In this paper, an artificial neural network based gas path diagnostic approach for engine power setting sensor fault detection and quantification has been introduced. Nested artificial neural networks (ANN) are used to detect power setting sensor fault and ensure prediction accuracy. Measurement noise is also considered in the training and testing samples to ensure the robustness of the diagnostic system. The developed power setting sensor diagnostic approach has been applied to a model 2-shafts industrial gas turbine engine similar to a GE LM2500+G4 engine to test the effectiveness of the approach. The selected power setting parameter is the shaft power output measured by a power setting sensor. An engine performance model is produced using Cranfield University’s gas turbine performance and diagnostics software, Pythia. Training samples with the consideration of sensor faults were simulated with the engine model assuming one of the sensors, either the power setting sensor or other gas path sensors may be faulty. In the nested neural network for sensor fault diagnostics, the system separately performs sensor fault detection, sensor fault identification and sensor fault quantification. Results show that the developed nested neural network diagnostic system is able to identify the power setting sensor fault and correctly predict the magnitude of the fault. This would allow the engine control system correct its control schedule and accommodate the power setting sensor fault.

Accurate gas turbine diagnosis relies on accurate measurements from sensors. Unfortunately, sensors are prone to degradation or failure during gas turbine operations. In this paper a stack of decentralised artificial neural networks are introduced and investigated as an approach to approximate the measurement of a failed sensor once it is detected. Such a system is embedded into a nested neural network system for gas turbine diagnosis. The whole neural network diagnostic system consists of a number of feedforward neural networks for engine component diagnosis, sensor fault detection and isolation; and a stack of decentralised neural networks for sensor fault recovery. The application of the decentralised neural networks for the recovery of any failed sensor has the advantage that the configuration of the nested neural network system for engine component diagnosis is relatively simple as the system does not take into account sensor failure. When a sensor fails, the biased measurement of the failed sensor is replaced with a recovered measurement approximated with the measurements of other healthy sensors. The developed approach has been applied to an engine similar to the industrial 2-shaft engine, GE LM2500+, whose performance and training samples are simulated with an aero-thermodynamic modelling tool — Cranfield University’s TURBOMATCH computer program. Analysis shows that the use of the stack of decentralised neural networks for sensor fault recovery can effectively recover the measurement of a failed sensor. Comparison between the performance of the diagnostic system with and without the decentralised neural networks shows that the sensor recovery can improve the performance of the neural network engine diagnostic system significantly when a sensor fault is present.

Abstract In the present cost-constrained environment, it is critical that operators effectively complete their wells while minimizing capital expenditure. Optimization efforts focus on increasing recovery factor by managing landing zone, increasing the number of effective fractures, increasing the size of the fractures, and increasing the length of the lateral, while reducing the total number of stages and job size, without sacrificing efficient proppant and fluid delivery. The same pressure to reduce expenditure also impacts decision making on diagnostic evaluation, reducing operators to ‘free’ or low-cost feedback, like surface production rates and decline curves. Operators are responding to these challenges by utilizing a combination of lower cost, post-completion diagnostics like deployed fiber optics, downhole camera evaluation of perforations and radioactive tracers. These less expensive options allow for a broader scope and number of diagnostic inquiries, whereas a permanent fiber may prove to be cost-prohibitive, reducing diagnostic focus to one well, in one part of a play. Combining differing diagnostic technologies enhances the overall description of the well and reservoir behaviors and improves confidence in their interpretation of stimulation and production efficiency; furthermore, where a single diagnostic measurement may be unlikely to justify dramatic change in a completion strategy, a combination of data points from different domains can and does support design change that leads to rapid, real world performance improvements. Care is needed in the conclusions drawn when utilizing complimentary diagnostics due to the differences in depth of investigation and the non-unique interpretation of some data types. This paper discusses three post-completion diagnostic technologies, perforation evaluation by downhole camera, radioactive tracers, and distributed acoustic and temperature sensing (DAS+DTS) data and their respective physical measurements, strengths and weaknesses and how they can be combined to better understand well and reservoir behavior. It concludes with a review of completion optimization efforts from the Rockies area, where these post-completion diagnostic technologies were applied in the evaluation of eXtreme Limited Entry (XLE) trials. A statistical analysis of the RA tracer, downhole camera measurement of perforation area and deployed fiber optic acquisition of DAS+DTS reveals no correlation between diagnostic answers, indicating no one diagnostic measurement can accurately predict the other, such that it could substitute for that diagnostic and provide the same answer. Asking the right question can often enhance the value of diagnostic descriptions of the system in question. Those answers often lead to the next question and clear the path forward in advancing completion optimization. Complimentary diagnostics facilitate a more complete understanding of stimulation and production performance when compared, increasing confidence when they agree. When one or more appear to disagree, the different respective physical measurements and depths of investigation often reveal a more complete and complex understanding of stimulation and production efficiency. As an aggregate they provide clarity on the effect of efforts to create conductive pathways into the reservoir, allowing operators increased control over the resulting production.
