Abstract:
This chapter develops the methods for designing, executing, and analyzing large suites of computer simulations that generate stable and replicable results. It starts with a discussion of the different methods of experimental design, such as grid sweeping and Monte Carlo parameterization. Next, it demonstrates how to calculate mean estimates of output variables of interest. It does so by first discussing stochastic processes, Markov Chain representations, and model burn-in. It focuses on three stochastic process representations: nonergodic deterministic processes that converge on a single state; nondeterministic stochastic processes for which a time average provides a representative estimate of the output variables; and nondeterministic stochastic processes for which a time average does not provide a representative estimate of the output variables. The estimation strategy employed depends on which stochastic process the simulation follows. Lastly, the chapter presents a set of diagnostic checks used to establish an appropriate sample size for the estimation of the means.

Abstract:
Pharmacological stress testing plays an important role in patient management. Pharmacologic stress agents (adenosine, dipyridamole, regadenoson, dobutamine) produce coronary hyperemia, which is at least as high as that achieved by maximal exercise, by a completely different set of mechanisms than that achieved during exercise. Regadenoson, a relatively newer agent, offers ease of administration and fewer side effects than older stress agents and has become the agent of choice in many stress laboratories. The choice of the stress agent should be individualized based on its indications, contraindications and safety profile. The diagnostic accuracy of pharmacological stress testing for significant coronary artery disease detection is comparable to or exceeds that of exercise myocardial perfusion imaging.

This chapter includes essential information about common toxic exposures requiring pediatric intensive care unit care. Specific agents, grouped into categories, are reviewed, including analgesics (acetaminophen and aspirin), opiates, carbon monoxide, cardiovascular medications (calcium channel antagonists and β‎ blockers), tricyclic antidepressants, sulfonylureas, and toxic alcohols. An overview of each agent followed by clinical presentation, and appropriate diagnostic evaluation and management are provided, including alkalinization with administration of sodium bicarbonate, need for hemodialysis, and use of specific antidotes (e.g., naloxone, n-acetyl cysteine, glucagon, fomepizole).

Abstract:
Imaging is reliant upon the contrast between different body elements, which may be present naturally or may require the introduction of extrinsic contrast agents. Since the 1920s, the use of contrast agents has been refined to enhance the diagnostic potential of multiple imaging modalities. Contrast agents are a vital part of diagnosis and treatment algorithms involving image guidance. Given the wealth of contrast agents on the market, a basic understanding of the various types is critical for budding interventional radiologists who need to use such agents judiciously on a daily basis. Such contrast agents may be administered intravenously, intraarterially, intrathecally, orally, via inhalation, transrectally, or via indwelling tubes or catheters cannulating a specific viscus. In this chapter, we review the key categories, contraindications, and alternatives for such agents, with special attention to their use in an interventional radiology (IR) practice.
