Computerized tomographic pulmonary angiography is the current gold standard tool for the diagnosis of pulmonary embolism. Mortality risk stratification based on clinical, imaging, and biochemical indices dictates the treatment strategy employed in an individual patient from outpatient anticoagulation to surgical or percutaneous embolectomy for severe, life-threatening cases.Presentation of acute pulmonary embolism ranges from a chance diagnostic finding in an otherwise asymptomatic patient through to shock and cardiac arrest. The degree of obstruction and the ability (or otherwise) of the patient to compensate for any cardiorespiratory derangement will dictate the symptomatology, clinical findings, and eventual outcome. The scale of medical intervention—ranging from anticoagulation through to thrombolysis and, on occasion, embolectomy—can be based on a variety of clinical, biochemical and imaging risk indices.

Psychiatric symptoms are not restricted to primary psychiatric disorders and are relatively frequent in medical and neurological disorders. They may represent the clinical manifestations of these disorders, of a comorbid psychiatric disorder, or of iatrogenic complications of pharmacological and/or surgical therapies. Clearly, proper diagnosis is of the essence to provide the correct treatment. Electroencephalographic (EEG) studies are used on a regular basis to identify a potential organic cause of psychiatric symptomatology. This chapter reviews the diagnostic yield of EEG recordings in psychiatric symptomatology associated with primary psychiatric disorders, with neurological and medical conditions, and in particular with epilepsy, and provides suggestions on the optimal use of the different types of EEG recordings in clinical practice.

Anticholinergic toxidrome can be a side effect of medications or herbal therapies or can result from intentional or inadvertent overdose of prescribed medications or abused substances. The diagnosis of anticholinergic toxicity involves symptomatology, including tachycardia, hyperthermia, central nervous system dysfunction including delirium, and urinary retention. The diagnosis can be challenging, as it is based on clinical symptoms with presentation mimicking other etiologies. Early diagnosis and intervention with treatment are key with the specific identification of the agent involved and other confounding substances. Discontinuation of the agent(s) involved as early as possible and providing supportive care are early interventions, with additional therapeutic options determined by the severity of the toxicity and the agent(s) involved.

Autism spectrum disorder (ASD) is an early onset and persistent condition defined by alterations in social communication and social interation alongside repetitive, restricted stereotypic behaviours and interests causing disabilities. Until recently, research on the co-occurrence of ADHD with ASD has been limited by DSM-IV criteria, allowing no dual diagnosis of these two neurodevelopmental disorders. Since the DSM-5 permits a double diagnosis of ADHD plus ASD, research on their comorbidity has substantially increased. In addition to shared and distinct aetiological factors, studies have revealed a high clinical impact of the combined symptomatology on individual outcomes. This chapter provides a selective overview of behavioural, cognitive, and biological findings as well as intervention strategies in combined ADHD/ASD phenotypes.

Allergic disorders are on the increase. This chapter covers a range of common allergic problems encountered in patients with severe asthma which may contribute to symptomatology and exacerbations, including genetic and environmental factors, affecting the airways, and the diagnosis and management of atopic asthma in association with seasonal and perennial allergic rhinitis. The account elucidates the basic principles behind simple investigations, such as skin prick testing and in vitro allergen-specific IgE assays, as well as relatively novel tests, including component resolved diagnosis. Also covered are recent advances in managing patients with seasonal and perennial rhinosinusitis, with an explanation of the indications, and the protocols available for desensitization to pollens, house dust mites, and animal dander. Overall, this chapter should provide the reader with adequate knowledge to deal with day-to-day management dilemmas in a specialist asthma clinic.
