Abstract:
In the United States, an estimated 74,000 new cases of bladder cancer and 16,000 deaths occur annually. Because of favorable survival, approximately 578,000 persons are alive after diagnosis. Recurrence after treatment is common, necessitating lifelong surveillance and care, making bladder cancer the most costly malignancy from diagnosis to death. The median age at diagnosis has increased from 67 years in 1980 to 73 years in 2008–2012. Incidence rates rise dramatically with age, suggesting that as the US population ages, the burden of bladder cancer in the United States will continue to grow. Worldwide over 1.3 million cases are estimated to occur annually. Cigarette smoking is the principal known cause of bladder cancer, along with various chemical exposures. Recent studies suggest that, over the past two decades, the risk for current smokers has increased from three times the risk of nonsmokers to about four to five times the nonsmoker risk.

This chapter addresses the diagnosis and management of bladder and renal cancers. In the UK, bladder cancer is the fourth most common cancer in men, and the eighth most common cancer in women. Bladder cancer arises from the bladder urothelium, and is typically a papillary transitional cell carcinoma. Chronic infection with the parasite Schistosoma haematobium is associated with squamous cell carcinoma of the bladder, and is most prevalent in Egypt and sub-Saharan Africa. Renal cancer accounts for 3% of cancers in adults in the UK and, in most cases, is a renal cell carcinoma arising from proximal renal tubule epithelium. A further 5%–10% of renal cancers are transitional cell (urothelial) carcinomas of the renal pelvis. Benign kidney tumours, such as cysts, are also common.

Bladder cancer (BC) is one of the most common urological malignancies and ranks ninth among all cancers worldwide. While screening has the potential to detect early cases of BC and reduce disease specific mortality, to date there are no routine screening programmes of asymptomatic individuals conducted anywhere in the world. There are however, a range of tests and procedures available for the detection and subsequent diagnosis of BC for select individuals presenting with urological symptoms and who are at increased risk of the disease.This chapter provides an overview of the traditional screening tools used for the detection of BC, such as urinalysis for haematuria and urinary cytology, as well as a brief description of follow-up procedures including cystoscopy, imaging, and treatment modalities.

Abstract:
Bladder cancer (BC) is a common malignancy and is one of the most expensive to manage. This tumour typically affects middle-aged and elderly patients and is more common in men. These demographics reflect the aetiological links between BC and tobacco smoking and occupational exposure to carcinogens. While the presenting symptom of BC is usually painless visible haematuria, around one-quarter of tumours present with irritative voiding symptoms, recurrent or antibiotic-refractory urinary tract infections, non-visible haematuria, or are found by chance during the investigation of unrelated symptoms. As such, these urological symptoms require urgent investigation in patients at risk of BC. The diagnosis of BC is confirmed by endoscopic transurethral resection. This procedure requires meticulous tumour excision, sampling of the underlying detrusor muscle and the flat urothelium or prostatic urethra in patients with high-grade cancers. The prognosis of BC depends upon tumour grade, stage, and other features.

Abstract:
Urinary bladder cancer (UBC) ranks ninth in worldwide cancer incidence. The most common histological type in Western countries is transitional cell carcinoma (TCC), while in Africa, a substantial proportion of squamous cell carcinomas (SCC) are observed related to the prevalence of infection with Schistosoma haematobium (bilharziasis). UBC has the highest per-patient lifetime cost for cancer in terms of healthcare expenditure compared to all other types of cancer. It is more frequent in men than in women and age is now widely accepted as the greatest single risk factor for developing UBC. The median age at diagnosis is 70 years. Cigarette smoking and specific occupational exposures, such as carcinogenic dyes for painters, are the main known causes of UBC.
