Abstract:
Many complaints were received from the residential area around the Shatin Sewage Treatment Works (STSTW) about the presence of odour. The dominant odour marker H2S was selected to be studied. A review of the odour measurement and sampling methods, together with the specification of the standard limits and health effects were made. The formation process of H2S was investigated and the abatement and control measures in terms of physical, chemical and biological techniques were compared. The monthly rate of H2S emission in the atmosphere was calculated from the soluble sulfide and the measured wastewater flow at the inlet works, primary sedimentation tanks, aeration tanks, and sludge digestive and storage tanks, and the H2S mapping was carried out at the same places. Meteorological data including wind speed, wind direction, temperature and pressure were collected. The effect of temperature and pH on H2S generation is shown, and the effectiveness of odour control and deodourization processes was studied. A Gaussian dispersion model of Industrial Source Complex version 3 (ISC3) was applied in predicting the ground H2S level at various air sensitive receivers in the vicinity of STSTW. None of the places exceeded the recommended H2S concentration of World Health Organization (WHO) and the odour is a localized problem in STSTW of 35% area having H2S ranging in 0.01--‐0.023ppm. Recommendations are suggested in both short-term improvement in operation and maintenance practice and long-term design and construction of sewage treatment facilities.

Wetland are valuable habitat in Hong Kong. These area have been threatened by rapid urban development in Hong Kong for the past 50 years. Since they handover of Hong Kong from the British ruling back to China, the frontier land have been left untouched for many years. And this land is precious and shall be treated with care in order to preserve the ecological valuable area for our future generation to enjoy. The design thesis would response to issues concerning with drainage channel design in the rural Man Kam To district and current ecological stream park/wetland compensation mitigation in Hong Kong. A study area in Shenzhen River, near Man Kam To Border Control Point is selected as demonstration site for drainage edge treatment together with storm-water treatment to restore the natural edge of the area. In addition, an algae bioreactor treatment design will be incorporated as a vision to use new technology to provide renewable energy and clean air for a self sustainable design. It aims to establish an innovative landscape solution the current over engineered river channel treatment and to balance between developing the economic growth from the Shenzhen Area while preserving ecological valuable area as a greenbelt in Hong Kong for future sustainable development use.

Abstract:
There are a number of existing staging systems for patients with hepatocellular carcinoma (HCC). Yet, Barcelona Clinic Liver Cancer (BCLC) staging classification is the only one which suggests treatment guidance. Although BCLC staging is widely used in Western countries, it may not fit in the management of HCC patients in Hong Kong as they mostly have different etiologies and have more aggressive treatment strategy when compared with their counterparts in Western countries. It is aimed in this thesis to develop a new prognostic staging system in conjunction with treatment guidelines for HCC patients in Hong Kong. Three thousand eight hundred and fifty six adult HCC patients presented to the Department of Surgery, Queen Mary Hospital between January 1995 and December 2008 were included. The patient data were randomly separated into a training set and a test set for scheme development and performance assessment respectively. Four established prognostic factors which have determinative roles in treatment, namely Eastern Cooperative Oncology Group performance status, Child-Pugh grade, tumor status, and presence of extrahepatic vascular invasion/metastasis, were selected in building the scheme. Cox proportional hazards regression on overall survival was used to derive a relative coefficient for each category of these four factors. Clinical knowledge in addition to the relative coefficients was involved in the proposal of the prognostic stages. Then a classification and regression tree analysis was performed to elicit a set of simple clinical decision rules given the factors. This tree-structured classifier was adjusted with clinical judgment and reconciled with the proposed prognostic staging system for treatment guidelines. This Hong Kong Combined Liver Cancer (HKCLC) prognostic classification scheme stratifies patients to stages I to V with distinct overall survival outcomes. Its performance was compared to BCLC scheme for their discriminatory ability as staging systems and effectiveness of treatment guidelines. The former used receiver operating characteristics (ROC) analysis and concordance index as measures of the ability to distinguish patients with different prognosis for overall survival. HKCLC staging had significantly larger 1-year, 3-year and 5-year area under ROC curve values and higher concordance index vis-a-vis BCLC staging. The latter compared the overall survival of patients who received different treatments. The overall survival of patients with the same BCLC stage and the same HKCLC stage but received HKCLC recommended treatments were compared with those received BCLC recommended treatments by Kaplan-Meier plots and log-rank test. HKCLC treatment guidelines had wider indications for more aggressive treatments than the BCLC treatment schedule, and demonstrated significant survival benefit in our patients.

Abstract:
Protecting and promoting patients’ and families’ sense of dignity at end-of-life is an indispensable goal, one that palliative care professionals have pursued unceasingly in the modern era. While researchers have made several attempts to uncover the intricate meaning and constitution of dignity and dignified end-of-life care in recent years, their works are solely based upon a Western individualistic paradigm that may not be applicable in Eastern collective societies. This limitation underscores that the existing body of knowledge is missing an important articulation on the Chinese experience. Thus, to fill this important knowledge gap, this study has applied ‘micro’, ‘meso’ and ‘macro’ levels of inquiries to attain a holistic understanding of dignity and dignified end-of-life care in the Hong Kong Chinese context. Through meaning-oriented interviews with 18 Chinese palliative terminal cancer patients and 18 of their family caregivers, as well as four interpretive-systemic focus groups with 30 key informants involved in a novel end-of-life care pathway programme (N=66), a total of 31 themes that elucidate dignity and dignified end-of-life care have been generated. These 31 themes are carefully organized into 9 categories that reflect the Individual, Familial and Institutional dimensions of dignity at end-of-life. First, the Individual dimension includes: 1) “Personal Autonomy”, which consists of Regain Control, Self-Sufficiency, Informed Care Decisions and Future Planning; 2) “Family Connectedness”, which consists of Express Appreciation, Achieve Reconciliation, Fulfill Family Obligations and Strengthen Family Bond; and 3) “Spiritual Plasticity”, which consists of Enduring pain, Spiritual Surrender, Moral Transcendence and Transgenerational Unity. Second, the Familial dimension involves: 1) “Social Agency”, which comprises of Caregiving Resources, Caregiver Assertiveness and Communicative Action; 2) “Family Integrity”, which comprises of Mutual Support, Kinship Involvement and Family Adaptability; and 3) “Filial Compassion”, which comprises of Compassionate Duty, Reciprocal Relationship and Emotional Connection. Third, the Institutional dimension entails: 1) “Regulatory Empowerment”, which encompasses Interdisciplinary Teamwork, Resource Allocation, Culture Building and Collaborative Policymaking; 2) “Family-Centered Care”, which encompasses Continuity of Care, Family Conference and Care Partnership; and 3) “Collective Compassion”, which encompasses Devotion in Care, Empathic Understanding, and Compassionate Action. These 9 categories and their respected 31 themes are clearly interrelated and embedded within the political, cultural, and spiritual contexts of society, highlighting the intricate interplay of systemic structure and social discourse for promoting dignity at end-of-life. These findings have further led to the development of the ‘Patient-Family Model of Dignified Care’ and the ‘Interpretive-Systemic Framework of Dignity at End-of-Life’. The former provides a new clinical protocol for identifying and evaluating the strengths and weaknesses within the personal, interpersonal and transpersonal functioning of the patient-family dyads in Chinese end-of-life care. The latter offers a public health roadmap for social change that accentuates the necessity for a collective consciousness of compassion in pushing forth the ultimate ideal of living and dying with dignity in Hong Kong. Based on this body of work, recommendations for patient-family care at end-of-life, expansion of community-based palliative care, and development of palliative long-term-care for ensuring quality and equality in the care of dying patients and their families are discussed.

Abstract:
Background Non-adherence at psychiatric out-patients clinics has serious impact on clinical and economic outcomes. It is costly to both health care systems and the patients. This study aimed to examine the predictors of treatment adherence in a group of patients with first episode psychosis in Hong Kong. Methods Thirty five patients with psychosis underwent assessment for prospective memory, insight and social support at baseline. Clinical symptoms were also measured. Linear regression was used to identify predictors of treatment adherence at baseline. Actual medication adherence was assessed three months later and multiple linear regression models were used to identify predictors of non-adherence. Results The effects of independent variables age, gender, number of psychiatric hospitalization, and prospective memory were no statistically significant to treatment adherence. But educational level, insight and social support were showed positive correlation with treatment adherence. Conclusions The study demonstrates that treatment adherence is associated with level of education, insight and social support system. These factors are important to assess when trying to estimate the degree of medication adherence and its relationship to clinical presentation.
