The metaphor is defined as «an expression which describes a person or an object, by referring to something that is supposed to have similar characteristics» (Cambridge Academic Content Dictionary, s.v. “Metaphor,” 2017). Since the 90s, metaphors were studied in General Practice (GP), as strategies that impact on the quality of healthcare (Mabeck & Olesen, 1997). In Palliative and Primary Care, metaphors are used to educate students and patients, and to ameliorate clinical and organizational issues (Rodriguez & Bélanger, 2014). Research explored more metaphors expressed by patients, than those of clinicians. There is no research on the reflection and meta-reflection of clinicians about their deliberate use of metaphors in clinical encounters. The state of the art provides a dualism on metaphors, as useful or dangerous tools. Methodology. The research question “what is the representation of metaphor provided by professionals who work in a GP group, on the use of metaphors?” arose from a gap in the literature. The Interpretative Phenomenological Analysis (IPA)(Smith et al., 2009) was selected as methodology, because it suggests a preliminary bibliographic search, differently from the pure phenomenological approach (Giorgi,1985). The design is qualitative, with an idiographic focus (Mantovani, 1998). The research aims to understand the representations of physicians, nurses, and administrative assistants in a GP group, which is a situation where it is possible to share contexts and therapeutic goals. Sample. We hypothesized that a group was more opened to talk about these issues than a context in which a physician works alone. We recruited a group of 15 participants (10 physicians, 3 administrative assistants, 2 nurses) (10 females, 5 males) with a purposeful sampling. Tools. We conducted 15 semistructured interviews on the use of metaphors by participants and patients, and on the use of metaphors in their professional education. After a month, we conducted 15 indepth interviews (Sità, 2012), to saturate data. Analysis. The IPA requires a careful read of the interviews, the selection and the organization of the contents into categories. A set of generalizations is created to saturate data, by respecting the differences among participants. A theoretical construct is formulated according to the literature. The results report follows the case study strategy (Mortari & Zannini, 2017). Results. The representation of metaphors changes, if these expressions are used by clinicians or by patients, in clinical and educative contexts, and differs for each participant. Strengths and limitations are provided on the clinical use of metaphors. Sometimes, metaphors are used unconsciously by participants, to express some concepts, to talk about patient education, the effects of clinical language, or their professional identity. Conclusion. The hypothesis on the sample is verified. To overcome the dualism in the literature, this study suggests that a metaphor is an epistemological, a relational, and an ontological phenomenon. Therefore, specific curricula could be designed to enhance awareness on this issue, to help students/professionals to reflect on metaphoric language as a communicative strategy in clinical encounters, that also impact on patient education.

Full textI Sistemi Sanitari Europei sono sottoposti da alcuni anni a penalizzanti tensioni economico/finanziarie dovute sia agli elevati debiti pubblici sia al ciclo economico non favorevole. Tale situazione assume maggior rilevanza nei Paesi in cui i Sistemi Sanitari sono pubblicamente finanziati. Nonostante le suddette condizioni i Sistemi Sanitari sono stati e sono impegnati a perseguire gli obiettivi propri di miglioramento dello stato di salute della popolazione, di garanzia ad un accesso equo ai servizi sanitari, di un uso efficiente ed economico delle risorse ad essi assegnate, di perseguimento di un sistema di finanziamento più equo, di miglioramento della qualità delle cure e appropriatezza dei trattamenti sanitari. Tra le condizioni economico/finanziarie non agevoli e l'impegno al perseguimento degli obbiettivi sopra indicati i Sistemi Sanitari Europei sono stati oggetto di numerose e radicali riforme, e a questo non ha fatto eccezione il Sistema Sanitario Italiano, soprattutto a partire dagli anni '90. Gli obiettivi principali delle riforme sanitarie Italiane, ma non solo in Italia le riforme hanno avuto luogo, sono stati l'istituzione di un Servizio Sanitario Nazionale (SSN) pubblico, più anteriormente, nonché la creazione di "quasi mercati", ovvero la separazione tra acquirenti e fornitori di servizi, la razionalizzazione della rete ospedaliera, l'introduzione di sistemi di finanziamento ospedaliero diversi dai sistemi precedentemente adottati sia per le strutture pubbliche che private accreditate, l'istituto dell'accreditamento, il proseguimento del processo federalista, soprattutto di natura fiscale, che sta attribuendo alle Regioni Italiane sempre maggiore autonomia sia dal lato della spesa sia dal lato delle fonti di finanziamento in sanità, per citarne alcuni dagli anni '90 in poi... Full textAntimicrobial resistance (AMR) is a global concern. Klebsiella pneumoniae and Acinetobacter baumannii responsible of hospital infections and non-typhoidal Salmonella enterica relevant in the food chain are among pathogens showing high multi-drug resistance (MDR). Aim of this study was to describe the epidemiological insights related to AMR characterizing by different molecular approaches K.pneumoniae and A.baumannii collected within the ‘Alert organism’ surveillance system in the ‘A. Cardarelli’ hospital (Molise Region, central Italy), and non-typhoidal S. enterica collected during the inspection activities carried out by Public Health England, London, United Kingdom. Twenty-six K.pneumoniae and 24 A.baumannii were analyzed to evaluate resistance profile (resistotype) assessing Minimal Inhibitory Concentration for different antibiotics, and prevalence of AMR determinants (KPC/GES/VIM/IMP,NDM,OXA-48,CTX-M/TEM/SHV; GIM/AmpC, OXA-23/24/51/58 only in A.baumannii, such as adeB/J/G/S/M, soxR and craA coding for efflux systems) by PCR. In colistin-resistant K.pneumoniae, detection of mcr1-8variants trough PCR and mutations in mgrB by sequencing was also performed. For molecular typing, Pulsed field gel electrophoresis (PFGE) (using XbaI for K.pneumoniae and ApaI/AscI for A.baumannii) and Multilocus sequence typing (MLST) according to Pasteur and Oxford schemes for K.pneumoniae and A.baumannii, respectively, were used. Whole genome sequencing (WGS) data of non-typhoidal S.enterica collected between 2013-18 were analyzed, including 74,958 samples from different sources to evaluate prevalence and serovars, and AMR determinants analysis focused on strains from United Kingdom, Thailand and Brazil. KPC was confirmed as the endemic carbapenemase in MDR K.pneumoniae, together with a high prevalence of VIM, TEM and SHV, while colistin resistance(70%) was attributed to point mutations and sequence insertions (IS5like, ISKpn14) in mgrB. PFGE and MLST allowed identifying an epidemic cluster occurred in the hospital intensive care unit and high prevalence of sequence type (ST) 512 followed by ST101, in line with national data. OXA-23 and OXA-51 were observed in all MDR A.baumannii, according to other Italian studies and VIM carbapenemase was found in 30% isolates, while is typically found in K. pneumoniae. The genes coding for the efflux pumps were identified in all strains, while abeM and craA were absent. PFGE grouped the strains in clusters, while ST1720 was the prevalent clone, although not found in other Italian studies. Prevalence non-typhoid S.enterica was of 1.3%, and was high in chicken and betel leaves, and S. Heidelberg was the serovar mostly identified. Analysis of AMR determinants covered 384 strains including those from Brazil, Thailand and the United Kingdom. The isolates from Brazil showed a high MDR (to more than 3 classes of antibiotics), due to the presence of aac(6')-Iy(100%), parC(97.2%), tet(A)-1(94.6%) sul-2(94,2%) and CMY-2(75%), associated with resistance to aminoglycosides, fluoroquinolones, tetracyclines, sulfamethoxazole and β-lactams, respectively. Strains from Thailand also showed high AMR levels, showing parC(59%) and aac(6')-Iy(56%). Among isolates from UK, AMR was found at lower level compared to the others, and parC(70%) and aac(6')-Iy(68.8%) prevailed. The different prevalence of resistance genes is likely associated with diverse strategies to fight AMR in the United Kingdom and Thailand, while are still absent in Brazil.

Full textThis research focuses on Industrial Design extension as a strategic tool for industrial equipment companies. Though there is no scientific debate concerning this Design phenomenon, the empirical data analysis and our consultancy work in the last few years, underline a rising trend. Industrial design is becoming essential to automation and machine tool companies. Design discipline has broadly faced issues concerning product design process and today product designer efforts keep going toward this direction; their sensitivity in managing the fragile network of stakeholder relationships inside a project is well known and almost essential. These statements are surely true for consumer markets but quite hasty for industrial equipments B2B markets: effectiveness of Design practice within this specific sector has been scarcely investigated. The scientific objective is to analyze the inclusion of design cultures in B2B industrial enterprises, mostly alien to this culture, comparing it with similar phenomena investigated appropriately and bring out the similarities that can be recognized as a success parameter of the asset. Ultimately is proposed a definition of a best practice for the transformation of the company's innovation process, so that the research of design driven innovation becomes transferable to other local SMEs.

Full textLean has shown to deliver significant performance improvement in healthcare organizations. However, its practical implementation remains challenging and the ability to deliver in the long term is an under investigated issue. By using a three-step research protocol, the study draws on the main streams of literature on implementation science and lean management healthcare to explore: (i) the relevant organizational conditions for implementing lean in healthcare organizations, in particular in hospitals; (ii) the continuous process of reciprocal interdependence between lean as a system-wide organizational strategy and its context of application, and (iii) the influence of these mutual adaptation mechanisms on the sustainability of the implemented practices. As emerged from the empirical analysis (multiple case study), the implementation and the maintenance of lean efforts over time is the result of a continuous process of adjustments and modifications occurring in the overall change program originally adopted and in its context of application. In particular, the use of a socio-technical system framework to systematize the emerged evidence points out some key interrelated change patterns that hospital managers must place equal focus on: decentralizing responsibilities to practically work on lean; ensuring a stable guidance and sponsorship at all organizational levels; introducing an intermediate level between the top management and professionals; launching internal events; developing a continuous learning and improvement process; structuring a comprehensive approach to the optimization of hospital production processes.

Full textGiving an overview about the state of art of cultural heritage technologies, it’s clear how the borderline between different survey approaches (three-dimensinal active and passive acquisition systems) and computer vision techniques (modeling and rendering) is getting thinner and more confused in favour of an increasing integration between cross-curricular elaborations and management systems. Aware of this background, a flexible working methodology aimed at cultural heritage documentation and virtual reconstruction can’t manage without the synergy affects gained by a smart combination of different survey approaches and visualization methods. In this research an interactive working methodology for the documentation of good architectural/archaeological value is proposed, taking under special consideration Image-Based Modeling gathering from Spherical Photogrammetry to automated 3D reconstruction from a sparse photo set (Structure from Motion). The interactive Image-Based Modeling (IBM) approach developed goes through different steps (surface reconstruction, inverse mapping and texture projection, real time rendering) and feeds back until an optimized and photo-realistic reconstruction of archaeological /architectural heritage is achieved. Close range Photogrammetry (Spherical Photogrammetry and Structure from Motion) are used to obtain metric data. Beside, Computer Vision applications (Surface Reconstruction, Inverse Mapping and Real Time Rendering) allow photo-realistic results and high communicative impact. The developed methodology ensures accuracy, accessibility and a high degree of flexibility for the photo-realistic model reconstruction and, although using low-cost tools and technologies, allows to meet quality standards and professional levels. Moreover the proposed research highlights how is possible, using the same panorama dataset, to combine the advantages of the two mentioned technologies (Spherical photogrammetry and Structure from Motion) to speed up the 3D photorealistic model reconstruction without losing metric and photometric accuracy. Again, using multimedia tools, reliable results were obtained by developing a platform for the management of survey acquisition data and the provision of innovative services to the visitors and the responsibles for archeological sites: reality-based 3D surveying, modeling and visualization applied to architectures and cultural heritage.
