Abstract:
Descemet-stripping automated endothelial keratoplasty (DSAEK) has become the procedure of choice to treat corneal endothelial dysfunction. The technique involves replacing the diseased host endothelium with a graft consisting of a thin layer of posterior stroma, Descemet membrane, and endothelium. In comparison to penetrating keratoplasty (PK), DSAEK confers quicker visual and structural recovery with absence of corneal surface incisions or sutures, and limits astigmatism. DSAEK has been proved to successfully achieve favorable visual acuity and graft clarity in bullous keratopathy, posterior polymorphous dystrophy, and failed PK grafts. This article discusses various DSAEK surgical techniques, short- and longterm post-surgical results, complications, and comparisons with other types of keratoplasty. With the advent of Descemet membrane endothelial keratoplasty (DMEK), in which only Descemet membrane is transplanted, visual rehabilitation may be attained sooner.

ABSTRACT Purpose To develop a method of producing the transplant for Descemet stripping endothelial keratoplasty using the PocketMaker device (Dioptex, Austria). Materials and methods A new application of the PocketMaker microkeratome (Dioptex) is described for preparing thin grafts for endothelial keratoplasty. We report a case of Descemet's membrane detachment treated with a PocketMaker-prepared graft 110 mK thick. Results Visual acuity increased from 0.05 to 0.6. The 106 µm thick (in the center) graft remained attached and clear after Descemet stripping PocketMaker endothelial keratoplasty (DSPEK). Fuchs endothelial corneal dystrophy (FED) is one of the leading indications for corneal transplantation. Over the last two decades we have seen a significant shift in transplantation procedures for endothelial disease away from penetrating keratoplasty to small incision procedures such as Descemet stripping automated endothelial keratoplasty and Descemet membrane endothelial keratoplasty. More recently, even newer concepts have come to light to explore options of regenerative therapy for FED, e.g. Descemet stripping without endothelial keratoplasty, Descemet membrane transplantation and the use of adjuncts such as Rho kinase inhibitors. Furthermore, alternative strategies involving tissue engineering approaches with the use of cell therapy are also reaching early clinical trial stage. With more information on the underlying genetic abnormalities in FED, gene therapy may be a possible future treatment.

Abstract:
Purpose. To describe the clinical cases and surgery results in patients with concomitant of keratoconus and Fuchs' corneal endothelial dystrophy. Material and methods. Retrospective analysis of 3 patients using different surgical techniques. All patients were female, they underwent diagnostic procedures including: keratotopography, optical coherence tomography of the anterior segment of the eye, Scheimpflug keratotomography, endothelial microscopy. First patient underwent penetrating keratoplasty (PKP), the second one – descemet stripping automated endothelial keratoplasty (DSEK), and the third one – descemet membrane endothelial keratoplasty (DMEK). Results. In all cases the combination of keratoconus and fuchs endothelial corneal dystrophy occurred in one eye, while cornea guttata was observed in both eyes. The diagnosis of keratoconus was complicated by the presence of corneal edema due to endothelial dysfunction. As a result, visual rehabilitation was achieved in 2 patients after PKP and DSEK, and in the third case (DMEK), the presence of pronounced opacities of the stroma in the central zone did not result in increase of visual acuity, despite the good function of the graft endothelium, which required the PKP. Conclusion. Surgical tactics should be based primary on the replacement of the pathologically altered endothelial monolayer (DSEK, DMEK). As for the simultaneous replacement of the corneal stroma (PKP), the decision is based on the progression of keratoconus, the degree of corneal thinning, and the presence of its surface irregularity. Descemet membrane endothelial keratoplasty (DMEK) is the most recent step forward in the evolution of endothelial keratoplasty toward thinner grafts and more natural, anatomic corneal restoration. Offering unprecedented visual results and requiring no special or expensive equipment, DMEK has the potential to become the first line treatment for corneal endothelial disorders. The surgery’s perceived shortcomings (primarily technical difficulty) have mostly been addressed by new ‘no-touch’ procedures for both graft preparation and graft unfolding in the recipient eye. And as a result, DMEK has been gaining traction with ophthalmologists the world over. Now, in its most recent formulation, DMEK is ready for the typical corneal surgeon, in any clinical setting, and at low cost.
