The prevalence of mental health and learning disorders in school-age children is a growing concern. Yet, a significant delay exists between the onset of symptoms and referral for intervention, contributing to long-term challenges for affected children. The current mental health system is fragmented, with teachers possessing valuable insights into their students' well-being but limited knowledge of mental health, while clinicians often only encounter more severe cases. Inconsistent implementation of existing screening programs in schools, mainly due to resource constraints, suggests the need for more effective solutions. This thesis presents two novel approaches for improvement of mental health and learning outcomes of children and adolescents. The first approach uses data-driven methods, leveraging the Healthy Brain Network dataset which contains item-level responses from over 50 assessments, consensus diagnoses, and cognitive task scores from thousands of children. Using machine learning techniques, item subsets were identified to predict common mental health and learning disability diagnoses. The approach demonstrated promising performance, offering potential utility for both mental health and learning disability detection. Furthermore, our approach provides an easy-to-use starting point for researchers to apply our method to new datasets. The second approach is a framework aimed at improving the mental health and learning outcomes of children by addressing the challenges faced by teachers in heterogeneous classrooms. This framework enables teachers to create tailored teaching strategies based on identified needs of individual students, and when necessary, suggest referral to clinical care. The first step of the framework is an instrument designed to assess each student's well-being and learning profile. FACETS is a 60-item scale built through partnerships with teachers and clinicians. Teacher acceptance and psychometric properties of FACETS are investigated. Preliminary pilot study demonstrated overall acceptance of FACETS among teachers. In conclusion, this thesis presents a framework to bridge the gap in detection and support of mental health and learning disorders in school-age children. Future studies will further validate and refine our tools, offering more timely and effective interventions to improve the well-being and learning outcomes of children in diverse educational settings

Human immunodeficiency virus (HIV) is one of the leading causes of death in low- and middle-income countries.On June 8, 2021, the United Nations adopted the Political Declaration on Ending Inequality and Ending AIDS by 2030. Nevertheless, the need for prevention and treatment in many of the most affected countries far exceed their ability to pay. In addition, AIDS mainly affects most-at-risk population groups. This thesis shows that the benefits of this massive investment by the international community to end the AIDS epidemic by 2030 exceed 6 for every 1 invested. While this is an attractive investment, achieving the global 95-95-95 HIV target is far from certain, especially for sexual and gender minorities who are up to 26 times more likely to acquire HIV.The economic models used in this research identify three main factors that impede the achievement of the targets. First, stigma and discrimination are the main barrier to accessing HIV services. Second, too little emphasis is placed on the role of mental health in vulnerability to HIV infection. Finally, it demonstrates that inequalities undermine HIV response efforts by reducing the uptake of HIV testing.The findings of this thesis encourage policy makers to consider the importance to address inequality, mental health and homophobia in order to ending AIDS

Youth offenders have a high prevalence of mental disorders compared to adolescents in the general population. It seems that a large proportion of them do not receive the appropriate services and treatments for their mental conditions, often due to a lack of screening. The Massachusetts Youth Screening Instrument – second version (MAYSI-2) appears to be a key screening tool for youth offenders in the United States and around the world. A version adapted for Francophones in Quebec has been in clinical use since 2016 but has not been the subject of any validation so far. The objective of our study is to validate the factorial structure of the French- version of the MAYSI-2 for French speaking adolescent in Quebec based on data collected from a sample of adolescent housed in units for youth offenders (N=962). To do this, we analysed the internal consistency of the instrument and its various sub-scales. We then carried out a confirmatory factor analysis to test the structure of the dimensions and a multigroup confirmatory factor analysis to examine the robustness of the solutions according to gender (boy: N= 880 -girl: N= 82) and legal status (LSJPA, N = 741 and Intensive Supervision, N = 207). Our results indicate that the internal consistency of the scales is reliable for five of the seven scales (Alpha between 0.704 and 0.805), satisfactory for the scale of traumatic experiences (Alpha of 0,603) and below the standards for the scale of thought disturbance (Alpha of 0,480). Confirmatory factor analysis results indicate that the fit of the seven-factor model is beyond the satisfaction criteria (root mean square error of approximation (RMSEA) = 0.041, Tucker and Lewis Index (TLI) = 0.911 and Comparative Fit Index (CFI) = 0,905). Our measures of the invariance of the factor structure between subgroups indicates that the seven-factor model is a satisfactory solution for all subgroups in our study. Taken as a whole, our results confirm the factorial validity of the French-version of the MAYSI-2 for French speaking adolescent in Quebec. These results support the relevance of implementing the French version of the MAYSI-2 tool more widely as a screening tool to be used systematically with French speaking young offenders in Quebec.
