La dysplasie ectodermique (DE) comprend un groupe hétérogène de maladies héréditaires caractérisées par des troubles développement aux (dystrophie ou agénésie) de structures dérivées de l’ectoderme embryonnaire. Le diagnostic de la DE est en général relativement tardif; il est posé le plus souvent en relation avec une éruption dentaire retardée. En raison de la forte prévalence des anomalies orales liées à la DE,le médecin-dentiste est souvent mis à contribution pour poser le diagnostic. Pour permettre aux jeunes patients une intégration sociale adéquate en dépit de dents manquantes, une intervention médico-dentaire précoce est indiquée. La réhabilitation prothétique peut améliorer l’esthétique de manière déterminante, mais aussi la fonction masticatoire et la phonétique – et donc la confiance en soi. En rai- son de la complexité de ces cas, la coopération pluridisciplinaire bien coordonnée des pédiatres, orthodontistes, médecins-dentistes, spécialistes en chirurgie orale et maxillo-faciale et prothésistes est essentielle pour assurer, jusqu’à l’âge adulte, une prise en charge compétente des patients. Des exemples de restaurations prothétiques simples sont présentés dans le présent travail, sur la base de deux études de cas d’une même fratrie (un frère et une sœur).

How medical practitioners make a diagnosis is not well understood. When confronted with what is apparently a new disease, the diagnostician faces a special, perhaps revealing, challenge. In this paper I use the special circumstance of African Burkitt’s lymphoma as an apparently new disease to examine how it was recognized, characterized, and diagnosed between 1958-1963. African Burkitt’s lymphoma seemed strikingly new when it was first detected at a district hospital in Uganda. It puzzled Denis Burkitt and the pathologists who worked with him for about five years. After much study and time, they could only agree that it was a malignant tumour, likely some type of lymphoma. Eventually, the particular geographic restriction of its occurrence and its distinctive distribution in the human body were the features that led to its recognition as a specific entity able to be diagnosed. The controversy that unfolded over five years shows how clinicians proceed to diagnosis, particularly when confronted with what appears to be a new disease. RésuméOn ne comprend pas encore très bien la manière dont les médecins praticiens établissent un diagnostic. Lor-squ’il est confronté à ce qui semble être une nouvelle maladie, le diagnosticien fait face à un défi particulier, voire révélateur. Dans le présent article, l’auteur utilise la situation particulière de l’apparition du lymphome de Burkitt africain en tant que maladie apparemment nouvelle pour examiner comment il a été reconnu, caractérisé et diagnostiqué entre les années 1958 et 1963. Le lymphome de Burkitt africain semblait éton-namment nouveau lorsqu’il a été détecté pour la première fois dans un hôpital de district, en Ouganda. Il a intrigué Denis Burkitt et les pathologistes qui travaillaient avec lui pendant environ cinq ans. Après y avoir consacré beaucoup d’étude et de temps, ils n’ont pu que convenir du fait qu’il s’agissait d’une tumeur maligne, probablement une sorte de lymphome. Au bout du compte, la restriction géographique particulière de son apparition et sa répartition particulière dans le corps humain ont été les caractéristiques ayant mené à sa reconnaissance en tant qu’entité spécifique, pouvant faire l’objet d’un diagnostic. La controverse qui a duré cinq ans montre comment les cliniciens en arrivent à un diagnostic, particulièrement lorsqu’ils sont con-frontés à ce qui semble être une nouvelle maladie.

Abstract STUDY QUESTION Could whole-exome sequencing (WES) be useful in clinical practice for men with maturation arrest (MA) after a first testicular sperm extraction (TESE)? SUMMARY ANSWER WES in combination with TESE yields substantial additional information and may potentially be added as a test to predict a negative outcome of a recurrent TESE in patients with MA. WHAT IS KNOWN ALREADY At present, the only definitive contraindications for TESE in men with non-obstructive azoospermia (NOA) are a 46,XX karyotype and microdeletions in the azoospermia factor a (AZFa) and/or AZFb regions. After a first negative TESE with MA, no test currently exists to predict a negative outcome of a recurrent TESE. STUDY DESIGN, SIZE, DURATION In a cohort study, we retrospectively included 26 patients with idiopathic NOA caused by complete MA diagnosed after a first TESE. PARTICIPANTS/MATERIALS, SETTING, METHODS Twenty-six men with MA at the spermatocyte stage in all seminiferous tubules, according to a histopathological analysis performed independently by two expert histologists, and a normal karyotype (i.e. no AZF gene microdeletions on the Y chromosome) were included. Single-nucleotide polymorphism comparative genomic hybridization array and WES were carried out. The results were validated with Sanger sequencing. For all the variants thought to influence spermatogenesis, we used immunohistochemical techniques to analyse the level of the altered protein. MAIN RESULTS AND THE ROLE OF CHANCE Deleterious homozygous variants were identified in all seven consanguineous patients and in three of the 19 non-consanguineous patients. Compound heterozygous variants were identified in another 5 of the 19 non-consanguineous patients. No recurrent variants were identified. We found new variants in genes known to be involved in azoospermia or MA [including testis expressed 11 (TEX11), meiotic double-stranded break formation protein 1 (MEI1), proteasome 26s subunit, ATPase 3 interacting protein (PSMC3IP), synaptonemal complex central element protein 1 (SYCE1) and Fanconi anaemia complementation group M (FANCM) and variants in genes not previously linked to human MA (including CCCTC-binding factor like (CTCFL), Mov10 like RISC complex RNA helicase 1 (MOV10L1), chromosome 11 open reading frame 80 (C11ORF80) and exonuclease 1 (EXO1)]. LARGE SCALE DATA Data available on request LIMITATIONS, REASONS FOR CAUTION More data are required before WES screening can be used to avoid recurrent TESE, although screening should be recommended for men with a consanguineous family background. WES is still a complex technology and can generate incidental findings. WIDER IMPLICATIONS OF THE FINDINGS Our results confirmed the genetic aetiology of MA in most patients: the proportion of individuals with at least one pathologic variant was 50% in the overall study population and 100% in the consanguineous patients. With the exception of MEI1 (compound heterozygous variants of which were identified in two cases), each variant corresponded to a specific gene—confirming the high degree of genetic heterogeneity in men with MA. Our results suggest that WES screening could help to avoid recurrent, futile TESE in men with MA in general and in consanguineous individuals in particular, but these results need to be confirmed in future studies before clinical implementation. L'analyse et le diagnostic précis à partir d'images dentaires sont un facteur essentiel de la réussite des traitements orthodontiques. De nombreux procédés de traitement d'image ont été proposés pour résoudre ce problème. Cependant, ces études fonctionnent principalement sur de petits ensembles de données de radiographies dans des conditions de laboratoire et ne sont pas vraiment applicables en tant que produits ou services complets. Dans cette thèse, nous construisons des modèles d'apprentissage profond pour diagnostiquer des problèmes dentaires tels que la gingivite et les dents chevauchées à l'aide de photos prises par de téléphones portables. Nous étudions les couches cachées de ces modèles pour trouver les forces et les limites de chaque méthode. Nous proposons un pipeline complet intégrant le prétraitement des images, l'apprentissage du modèle et le post-traitement des résultats pour créer un processus d'analyse complet prêt à être mis en production en situation réel. Afin d'améliorer la fiabilité des modèles, nous avons étudié différentes méthodes d'augmentation des données, en particulier les méthodes d'adaptation de domaine en utilisant des approche de transfert d'images, à la fois supervisée et non supervisée, et obtenons des résultats prometteurs. Les approches de transformation d'images sont également utilisés pour simplifier le choix des appareils orthodontiques par les patients en leur montrant à quoi pourraient ressembler leurs dents pendant le traitement. Nos méthodes permettent de générées des images réalistes et en haute définition. Nous proposons également un nouveau modèle de transformation d'image non supervisé qui peut manipuler les caractéristiques de l'image sans nécessiter d'annotation supplémentaire. Notre modèle surpasse les techniques de pointe sur plusieurs applications de transformation d'images et est également étendu pour les problèmes de « few-shot learning »
Accurate processing and diagnosis of dental images is an essential factor determining the success of orthodontic treatment. Many image processing methods have been proposed to address this problem. Those studies mainly work on small datasets of radiographs under laboratory conditions and are not highly applicable as complete products or services. In this thesis, we train deep learning models to diagnose dental problems such as gingivitis and crowded teeth using mobile phones' images. We study feature layers of these models to find the strengths and limitations of each method. Besides training deep learning models, we also embed each of them in a pipeline, including preprocessing and post-processing steps, to create a complete product. For the lack of training data problem, we studied a variety of methods for data augmentation, especially domain adaptation methods using image-to-image translation models, both supervised and unsupervised, and obtain promising results. Image translation networks are also used to simplifying patients' choice of orthodontic appliances by showing them how their teeth could look like during treatment. Generated images have are realistic and in high resolution. Researching further into unsupervised image translation neural networks, we propose an unsupervised imageto- image translation model which can manipulate features of objects in the image without requiring additional annotation. Our model outperforms state-of-the-art techniques on multiple image translation applications and is also extended for few-shot learning problems
