Abstract:
To examine the pediatric dentists’ perspective on level of care to children 0-3 years old by general dentists, an electronic survey was sent out to 5185 AAPD members and 769 responded (response rate: 14.8%). Forty-six percent agreed with a general dentist providing a dental home to children 0-3 years old. Only 24% agreed that general dentists could perform complex restorative and behavior management procedures for 0-3 years old. Younger respondents, pediatric dentists who practice in rural areas or teaching pediatric dentistry, and those who perceived adequate exposure during dental school were more likely to agree on general dentists proving a dental home (p values ranged from <0.0001 to 0.022). This study finding can help guide what level of care general dentists may provide and when a referral to a pediatric dentist is recommended and the appropriate competency level of entry-level general dentists to care for 0-3 years old.

Abstract:
Purpose: The purpose of this study was to examine the knowledge, attitudes, and experiences related to infant oral health for both dental and medical providers.Methods: A survey of infant oral health care was sent to 300 randomly selected general dentists, 300 randomly selected pediatricians, and all pediatric dentists in Virginia. The survey contained questions regarding the providers' routine infant oral health care regimen. In addition demographic data from the provider was collected. Responses to the questionnaire were tabulated and percent frequency distributions for responses to each item computed. Percents for all items were based on the total number of respondents in each of the three practitioner groups.Results: The response rate of the survey was 48%. While 100% of pediatricians treat the infant population, only 5% refer for the first dental visit by age one. In addition compared to dentists fewer pediatricians examine for dental decay or give oral hygiene instructions. Less than half of general dentists surveyed treat the infant population and only 12% refer for the first dental visit by age one. Compared to pediatricians and pediatric dentists general dentists were less likely to discuss the first dental visit or nutritional counseling. Amongst the three practitioner types, pediatric dentists provide the most thorough dental exam, however only 25% actually treat infants by one year of age. Access to dental care remains a significant factor in early childhood caries.Conclusion: Results from this research suggest that the majority of pediatricians and general dentists are not advising patients to see the dentist by one year of age. Concurrently the majority of dentists are not treating patients at one year of age, resulting in a critical problem with access to care. There is a need for increased infant oral health care education in the medical and dental communities to appropriately handle this infant population.

Abstract:
Attracting and retaining strong faculty members in dental schools have long been challenges in the United States. Faced with an emerging crisis in the availability and quality of dental educators, many researchers have focused on analyzing data and trends related to this subject. Even though there are substantial studies that provide a picture of why domestic dentists choose to become dental educators, there are no dedicated studies investigating why foreign-educated dentists want to become dental educators. This distinct area is of particular importance as we witness increasing diversity of patients and an increased need for a more diverse health care workforce. The purpose of this study was to analyze the reasons why foreign-educated dentists became dental educators in United States and furthermore, at University of the Pacific Arthur A. Dugoni School of Dentistry in San Francisco, California. The research data was collected by means of surveys and in-depth interviews of foreign-trained faculty and IDS graduates on staff at Pacific. The data analysis was performed using descriptive statistics for the survey and a process of thematic analysis for interviews. The surveys' analysis yielded the following six factors that most positively influenced foreign-educated dentists in deciding to embrace a career as an academic educator: (1) Intellectual challenges and stimulation; (2) Opportunity to always be on the cutting edge; (3) Interest in science, new discovery, exploration; (4) Opportunity for regular interaction with other faculty dentists; (5) Desire to be a teacher. (6) Collegial environment of the university. There were two most negative factors identified, as follows: (1) Income differential compared to private practice; (2) Pressure to generate income for the university. Corroborated by analysis of the interviews, the following themes emerged: (1) Drive for research; (2) Desire to be on cutting edge. (3) Desire to share knowledge through teaching; (4) Social and personal factors and perceptions with emphasis on mentors' influence; (5) Negative influence of salary difference with respect to private practitioners. The findings of this study were similar to the available studies on foreign-trained dentists and to most of the studies already done on domestically trained dentists. The major factors found were comparable with the up-to-date literature. The elevated research drive, the intellectual challenges, the work environment, the desire to teach, and the mentors' influence were among those which mirrored almost perfectly the other studies. Some fine differences were found for foreign-trained dentists, such as a lighter financial burden caused by smaller student debt and the irrelevance of military practice experience. The study provides a number of suggestions for enhancing the recruiting and retaining process for dental academia: (1) Support and enhance the research capacity of dental schools; (2) Create structures to develop mentors; (3) Invest to build prestige; (4) Find creative ways to offset lower salaries; (5) Foster a pleasant academic working environment; (6) Use international activities to recruit international dentists. The study revealed factors that have been influential in participants' decisions to choose an academic career, in general and at Pacific. It is hoped that this study will be a useful reference in the increasingly difficult endeavor of adding and retaining world-class dental educators.

Abstract:
Over the last 40 years, there has been a consistent increase in the proportion of women in the dentistry profession. In this study, we intended to gain insight into the issues and concerns that influence the work characteristics of female dentists. A number of studies have shown that gender differences exist in choice of specialisation, practice patterns, and professional attitudes; some have revealed that women are attracted to primary care management ahead of other dental specialties. It is also evident that women work fewer hours, work part-time and see fewer patients than men, and are less likely to own their practice than their male counterparts. There is also a net income discrepancy between male and female dentists. We used a qualitative description methodology to guide this pilot study, for which we recruited a purposive sample of six female dentists from the dental clinics of Montreal General Hospital, in the greater region of Montreal in Quebec, Canada. These dentists were at different stages of their practice (i.e., general practitioners and specialists). All interviews were fully transcribed, with the data managed with NVivo vr.9 software to facilitate the analysis. The findings suggest that female dentists consider it a challenge to balance work and childcare responsibilities. Taken together, the increased number of women in dental practice, and the additional demands of household activities, suggests that the relationship between work status and family happiness is an important matter to explicitly consider. Therefore, in order for female dentists to achieve balance between these two domains of life, a deeper understanding of the effect of family and society, and the work place is required.
Au cours des 40 dernières années, il y à eu une augmentation constante de la proportion de femmes qui occupent la profession de dentiste. Dans cette étude, j'ai l'intention de mieux comprendre les questions et les préoccupations qui influencent les caractéristiques du travail des femmes dentistes. Un certain nombre d'études ont montré que les différences existent entre les sexes dans le choix de spécialisation, les modes de pratique et les attitudes professionnelles. Ces études ont révélé que les femmes sont plus attirées par les soins dentaires primaires que par d'autres spécialités dentaires. Il est également évident que les femmes travaillent moins d'heures; travaillent à temps partiel et de voir moins de patients que les hommes et sont moins susceptibles de posséder leur propre pratique. Il y a aussi une différence de revenu net entre les dentistes masculins et féminins. Une description qualitative a été utilisée comme méthodologie, pour guider cette étude pilote. Un échantillon composé de six femmes dentistes, à différents stades de leur carrière (i.e., les médecins généralistes et spécialistes) fut recruté dans la grande région de Montréal au Canada. Les participantes proviennent soit des cliniques dentaires de l'Hôpital Général de Montréal ou de cliniques dentaires privées. Toutes les entrevues ont été entièrement retranscrites avec les données gérées parle logiciel NVivo vr.9 pour faciliter l'analyse. Les résultats suggèrent que les femmes dentistes estiment qu'il est difficile de balancer travail et résponsabilitées familiales. Dans l'ensemble, l'augmentation du nombre de femmes dans les cabinets dentaires aux prises avec les exigences supplémentaires des activités ménagère, suggère que la relation entre le statut professionnel et le bonheur familial est une question importante à considérer par chacune d'elles. Par conséquent, pour que les femmes dentistes puissent atteindre un équilibre entre ces deux domaines de la vie, une meilleure compréhension est nécessaire entre l'effet de la famille, de la société et le lieu de travail.

Abstract:
In the United State today, many people do not have access to adequate dental treatment, while tooth decay is the single most common chronic childhood disease-five times more common than asthma and seven times more common than hay fever. At the same time, studies show that 80% of dental illness takes place in 25% of the population, which is comprised mostly of lower socioeconomic groups. A study undertaken by the National Health and Nutrition Examination Survey in 2000, concludes that poor children suffer five times as much tooth decay as their more affluent peers. Furthermore, in poor children with tooth decay, almost 80% remain untreated. Poor dental health there-fore is a major health concern for under privileged families.The goal of this project is to enhance and facilitate the delivery of better dental treatment by designing a new prototype for a portable dental office that will be more flexible and more usable in a variety of situations and locations. For the purpose of this study, this prototype will be considered or tested within the context of three different types of spaces: the single closed room; a large open public space; and within mobile and nonpermanent structures such as tents. Several aspects of working conditions specific to the dental office will be taken into account when developing this design including transportability, degree of flexibility, storability, hygiene, and the creation of a relaxing atmosphere all of which greatly impact both the dentists and patients experience and are essential to the creation of a successful portable dental office.

Purpose: The purpose of this study was to asses fluoride varnish use by dental practitioners in Virginia. Methods: Using a cross sectional survey design, all dentists in Virginia who are members of the Virginia Dental Association (VDA) were sent an online survey about usage and knowledge of fluoride varnish. Results: The majority of the respondents were general dentists (79%) followed by pediatric dentists (12%). Fluoride varnish use increased with year of graduation from dental school. Dentists who thought fluoride varnish was more effective and less time consuming use it more than other topical fluorides. Dentists who thought their patients prefer fluoride varnish use it more than other topical fluorides.Conclusion: The majority of dentists are not aware of the advantages of fluoride varnish. However, those who are, choose to use it as opposed to foams and gels. Recent graduates, with more exposure to fluoride varnish, use it more frequently.

Professor, Department of Orthodontics Virginia Commonwealth University Richmond, Virginia May 2017 The practice of dentistry is physically demanding due to static and dynamic postures sustained daily throughout careers. Previous literature suggests that work-related musculoskeletal disorders (WMSD) are not solely the result of work habits, but also due to the individual, his or her physical makeup, genetics, and personal lifestyle. A 33-question survey was distributed to 1000 general dentists and 2300 orthodontists. The overall prevalence of work-related musculoskeletal disorders was greater among dentists and most often reported as self-limiting. Dentists were three times more likely than orthodontists to report WMSD; females were twice as likely to report WMSD than males; those who sought alternative medical remedies were two times more likely to have WMSD; and practitioners 6-10 years in practice were least likely to report WMSD. Dentists reported sitting in static positions longer than orthodontists; and those with WMSD indicated exercising, stretching, and seeking alternative health remedies more than dentists without WMSD.

Introduction: During the last 20 years, Sweden and other countries have been adjusting their models of welfare to a changed economic environment. Rationalization, influenced by New Public Management, has been implemented in public dentistry in order to improve efficiency and to streamline activities. This has involved transferring some of dentists’ tasks to dental hygienists and dental nurses. The goal is to achieve a more efficient mix of skills and more interaction between professional groups, in order to utilize all skills better in a more efficient work organization. Organizational changes may have an effect on the work environment both with regard to physical and to psychosocial work conditions and affect health and well-being. In many cases these changes have a profound negative effect on musculoskeletal and mental health, and corresponding risk factors, by reducing the number of natural breaks and thus reducing the efficacy of targeted ergonomic interventions. Dentists in Jönköping County in Sweden perceive high precision demands and poor working postures in their work. The five studies in this thesis describe organizational changes and analyse the risk of illness among dentists in the public sector in Jönköping County. Aim: The main aim is to study dentists’ physical and psychosocial work conditions and investigate associations with musculoskeletal disorders, work ability and sick leave during a period of extensive rationalizations; secondly, to assess the risk of illness as a basis for recommending preventive measures. Methods: The present thesis was designed with four cross-sectional studies (Paper I-IV) and one prospective longitudinal study (Paper V). In Paper I, a questionnaire concerning physical and psychosocial work conditions and health was sent out to all employees working in public dental care in Jönköping County in Sweden. To obtain more information on the difficult physical work situation for dentists (Paper I), an observation study with Portable Ergonomic Observation (Paper II) and an sEMG study (Paper III) was then conducted. Paper IV deals with psychosocial issues (using the same survey as in Paper I) and questions in the Eysenk Personality Questionnaire (EPQ) and the Marlowe-Crown scale SD (MCSD), to analyse their impact on perceived physical load. In Paper V, data about physical and psychosocial conditions and health from a survey, as well as production data (number of adult treatments per year per dentist) from computerized patient records (T4), are analysed with regard to changes and associations during a period of extensive rationalizations (2003 – 2008). Results: In Paper I, dentists reported the poorest physical work conditions of all occupational groups and high prevalence of musculoskeletal disorders. However, relatively low intensity of pain was reported and only a small proportion thought that work was affected. Paper II and Paper III confirmed that dentists’ work is physically demanding, with sitting postures and head bent forward, as well as prolonged low muscle loading. Paper IV shows that physical load is mainly influenced by psychosocial demands and to some extent by loss of work control. The results in Paper V show that during the period of extensive rationalizations between 2003 and 2008, dentists perceive improved precision demands and fewer uncomfortable work postures, but still a high level of physical load. The number of adults treated per dentist also improved, but there was a slight deterioration in work control and leadership. Conclusions: The results in this thesis show a consistent picture of high perceived physical load due to high precision demands and uncomfortable work postures, supported by observation of body movements (Portable Ergonomic Observation) and sEMG signs during psychosocially demanding circumstances. The rationalizations implemented in Jönköping County during the period 2003-2008 have not resulted in a deterioration of the physical environment, in spite of the fact that dentists produce more treatments of adult patients than before. This result may indicate that rationalizations do not always lead to increased health risks; it depends how they are implemented. Dentists may have changed the way they work for the better, and due to task delegation and SMS reminders a smoother patient flow has probably resulting in a reduction of workload and perceived stress regarding financial loss.

ENGLISH ABSTRACT: It can safely be assumed that work is a central part of people's daily activities in modem times, and dentists and other health workers are no exception to this rule. The following benefits have been ascribed to having a job, namely that of being a source of money, activity, variety, temporal structure, social contacts, status and identity in society. To what extent does the job as dentist in the civil service provide fulfilment of these basic requirements of having a satisfying job? Considering the number of resignations in recent years of state employed dentists and the well known and often reported "brain drain" of South African professionals, it is important to assess the status of job satisfaction amongst our health professionals, especially those within the state employ. These dentists are responsible for the health care delivery to the impoverished masses who make up a large proportion of the South African population. This study set out to determine the level of job satisfaction among state dentists and how this is perceived by them and reflected in their work performance. It was the aim of the researcher to provide some insight into the working lives of a very important sector of the primary health care providers employed by the Department of Health of the Western Cape Provincial Government. The outcome of the study could be used as a basis for further research in this area. A survey was conducted among all state employed dentists in the Western Cape, excluding dentists attached to the military or academic institutions. The survey instrument consisted of a structured, quantitative questionnaire as well as an open-ended section for comment on specific issues. Furthermore, an indepth semi-structured interview was conducted with one dentist in order to write a case report on the work experience of a state employed dentist.The major fmdings of this study are the following: i) The main determinants of job satisfaction revolve around the work environment, and dentists in state employ enjoy staff support, feel good about what they do for patients, and the standard of work done, but are unhappy about income, lack of respect by patients, senior management and colleagues in private practice. Limited treatment options, poor procurement systems and frequent breakdown of equipment affect their sense of job satisfaction negatively. ii) The level of dissatisfaction is not of such a proportion to persuade them to leave the service, as most dentists would like to remain in their posts. However, most dentists agree that there are no incentives for improving work performance. iii) Dentists responded to their sense of job satisfaction in different ways. While some admitted that their performance is directly linked to how well it is going at work, a large percentage of respondents reported that they do not allow their work performance to be influenced by their perceived job dissatisfaction. iv) Ifwe look at the case study, it is evident that perceived satisfaction or dissatisfaction depends on the individual and how he or she views the world. In this particular case the dentist was very realistic about his ambitions as a person and as a dentist, and set goals for himself within these boundaries. The result is that he remains motivated and satisfied.
AFRIKAANSE OPSOMMING: Dit kan geredelik aanvaar word dat werk die kern uitmaak van mense se daaglikse aktiwiteite, en tandartse en ander gesondheidswerkers is geen uitsondering nie. Die volgende voordele word aan werk toegeskryf: bron van inkomste, aktiwiteit, afwisseling, tydsindeling, sosiale omgang, status en posisie in die gemeenskap. Tot watter mate voldoen die werk van 'n tandarts in die staatsdiens aan die basiese vereistes van 'n bevredigende werk? As mens die getal bedankings van tandartse uit die staatsdiens oor die afgelope jare in ag neem, asook die welbekende en gereeld vermelde "brein trein" van professionele mense in Suid-Afrika, voel mens dis belangrik om vas te stel watter werksbevrediging heers onder gesondheidspersoneel, veral diegene in die staatsdiens. Tandartse in die staatsdiens is verantwoordelik vir gesondheidsdienslewering aan die arm massas, wat die oorgrote meerderheid uitmaak van die Suid-Afrikaanse bevolking. Hierdie studie was daarop gemik om die werksbevredigingsvlak van staatstandartse vas te stel, asook hoe hulle dit ervaar en demonstreer in hul werkverrigting. Die navorser het dit ten doel gehad om insig te bring in die werksbestaan van 'n baie belangrike komponent van gesondheidswerkers in diens van die Departement van Gesondheid in die Wes-Kaapse Provinsiale Regering. Die bevindinge van die studie kan dien as basis vir verdere navorsing op hierdie gebied. Die opname-instrument het bestaan uit 'n gestruktureerde kwantitatiewe vraelys saam met 'n afdeling vir kommentaar oor spesifieke knelpunte. Verder was daar 'n semi-gestruktureerde diepte-onderhoud gevoer met een tandarts om verslag te doen oor 'n gevallestudie rakende die werksondervinding van 'n staatstandarts. Die kembevindinge van die studie was: i) Die belangrikste determinante van werksbevrediging betrek die werksomstandighede. Terwyl tandartse ondersteuning geniet van hul mede-personeel, goed voeloor hul diens aan pasiente, en die standaard van hul werk, voel hulle ongelukkig oor inkomste, gebrek aan respek van pasiente, senior bestuur en kollegas in die privaat sektor. Beperkte behandelingskeuses, swak voorsieningsisteme en knaende gebrek aan toerusting affekteer hul werksbevrediging negatief. ii) Die mate van werksbevrediging is nie van so 'n aard dat dit hulle motiveer om die diens te verlaat nie, want die meeste tandartse wil hul poste behou. Die meeste tandartse voel egter dat daar nie enige aanmoediging bestaan vir verbeterde werkverrigting nie. iii) Tandartse reageer verskillend op hul menmg omtrent werksbevrediging. Terwyl sommige erken dat hul werkverrigting direk gekoppel is aan hoe goed dit by die werk gaan, het 'n groot persentasie van deelnemers aan die opname aangedui dat hulle nie toelaat dat hul werkverrigting deur hulonbevredigende werksomstandighede geaffekteer word nie iv) As ons die gevallestudie beskou, is dit duidelik dat of bevrediging ondervind word, al dan nie, van die individu afhang, en sy of haar lewensbeskouing. In hierdie spesifieke geval, was die tandarts baie realisties omtrent sy ambisie as persoon en as tandarts, en het sy doelwitte binne hierdie perke gestel. Die uitslag is dat hy gemotiveerd en bevredig bly.

Strategic management is concerned with the understanding, as well as choosing and implementing, the strategy or strategies that an organisation follows. It is a complex process which can be considered from a number of different perspectives. There is no single approach, model or theory to the management of strategic change. Pressures to change are always present in the form of opportunities and threats and managers must establish what approaches and decisions are likely to prove the most effective. The study was designed to help dental laboratory managers develop strategic awareness, to determine the issues and questions which must be addressed if changes in strategy are to be formulated and implemented effectively. The managerial and behavioural processes which take place within the laboratory in order that they can understand how changes come about are also discussed. Case studies of companies and dental laboratories are used jointly with the relevant literature to evaluate the theories and concepts of strategic management and implementation of change models provide a framework for evaluating how managers can reach their decisions and reach their objectives. The study has shown that the strategic ideas and methods used by private sector companies and discussed in the relevant literature can be adapted to provide a framework for managing the process of strategic change in a hospital dental laboratory.

There are many factors that affect the hours worked by various professions. The literature discusses some of these factors in professions such as management and various fields of medicine. However, no information has been gathered on factors that affect the practice of dentists. A survey was disseminated to all Iowa dentists inquiring about various factors that may affect the number of hours they spend at work. The survey inquired about demographics, educational indebtedness, caregiving responsibilities of both children and dependent adults, contribution to household income, relationship status, domestic responsibilities, busyness and type of practice. The data was collected and analyzed. It was hypothesized that there was no difference in the number of hours worked between male and female Iowa dentists and there was no difference in the number of hours worked between female Iowa dentists with minor children (18 years old or younger) and female dentists without minor children. When hypotheses were statistically analyzed, the data showed there was no difference between male and female dentists' working hours in the bivariate analysis. In the multiple logistic regression model, while controlling for various factors such as age and busyness, gender was statistically significant in the number of hours worked. When evaluating the hours worked between female dentists with and without minors, there was a statistically significant difference in the number of hours worked between women with minor children and women without minors. However, in the multiple logistic regression model, presence of minors was not statistically significant. This lack of statistical significance is likely attributed to the small sample size of women dentists. As such, there was not enough power to have more than 2 variables and minor children was not included. The data suggests that young male dentists, who are responsible for 61% or more of their household income and are solo practitioners are the most likely to work full-time (32+ hours/week).

Avhandlingen består av två studier som utgår från projektet ”Det goda arbetet”. Det överordnade syftet med projektet Det Goda Arbetet var att använda tandvård som ett exempel på ett arbete där relationerna med patienterna utgör arbetets kärna. Denna typ av arbete (även kallat människovårdande arbete) har speciella psykosociala arbetsmiljövillkor och känslomässiga krav som måste tas hänsyn till vid organisering av arbetet. Syftet med den första studien var att beskriva bakgrunden och utvecklingen av frågeformuläret ’Svenska och Danska tandläkares uppfattning av ’Det Goda Arbetet’ och att skapa ett mått för generell arbetstillfredsställelse, applicerat på fyra organisatoriska miljöer. Syftet med den andra studien var att introducera konceptet emotionellt arbete i tandvård genom att ge en teoretisk överblick av de emotionella aspekterna av arbetet, villkoren under vilka arbetet utförs och de potentiella effekterna på tandläkarnas välbefinnande. I kappan har kompletterande resultat från projektet Det Goda Arbetet inkluderats i syfte att ge en empirisk illustration av hur tandläkare upplever de emotionella faktorer som relaterar till patient-interaktionen och deras arbetsglädje. Data från 1226 danska och svenska verksamma tandläkare samlades in i November 2008 med en svarsprocent på 68 %. Ett additivt index skapades för att mäta generell arbetstillfredsställelse, och resultaten visade statistiska skillnader i tandläkarnas uppfattning mellan de olika organisatoriska miljöerna (Svenska offentliga/privata och Danska offentliga/privata). De danska offentliga tandläkarna hade den högsta graden av generell arbetstillfredsställelse medan de svenska offentliga hade den lägsta graden. En möjlig förklaring till detta kan vara att danska offentliga tandläkare skiljer sig från de andra tre grupperna i karakteristika vad gäller både tandläkare och patienter. Den låga graden av generell arbetstillfredsställelse hos de offentliga svenska tandläkarna kan möjligtvis vara en effekt av New Public Management-tänkande i sättet att organisera tandvård. Tilläggsresultaten visade att de svenska offentliga tandläkarna hade mycket mindre energi till sina privatliv i jämförelse med de andra tre grupperna och bara hälften av dem förväntade sig att fortsätta arbeta som nu fram till pensionen. Att arbeta med eller på människor handlar mycket om att skapa goda interaktioner och relationer mellan vårdgivaren och patienten. Goda patientrelationer kan vara ett primärt- och/eller sekundärt mål för att göra andra saker, som t.ex. den kliniska behandlingen, lättare. För många vårdgivare är relationerna med patienterna en arena där de kan leva ut sin potential som människor och kan upplevas som en bestående inre glädje av arbetet, kallat eudaimonia. I patientrelationen utför tandläkaren emotionellt arbete som ett sätt att intervenera med patienten för att vägleda denne i en bestämd riktning. Tandläkare har uttalade emotionella arbetsuppgifter i sina interaktioner med patienterna, emellertid har dessa emotionella aspekter av arbetet hitintills varit ett försummat forskningsområde inom odontologin. De emotionella arbetsuppgifterna är betingade eftersom att tandläkarens incitament inte är endimensionella och därför kräver de en hel del emotionell flexibilitet, uppmärksamhet och reflektion av tandläkaren. Påverkan från marknadskrafter och managerialism på de professionella värdena inom tandvård kan av tandläkaren uppfattas som motstridande och utmana villkoren för emotionellt arbete och tandläkarnas välbefinnande. Denna forskning syftar till att starka och uppmuntra olika nivåer av tandvård till att ytterligare undersöka, förstå och stötta dynamiken i de emotionella aspekterna av arbetet för att skapa en hållbar arbetsmiljö där värden och logik kan uppfattas som kompatibla med tandvårdens professionella värden.
The thesis consists of two papers which are based on a research project called ‘Good Work’. The overall aim of the Good Work project was to use dentistry as an example of work which has close relations with patients at its core. This kind of work (also called human service work) has special psycho-social work environment considerations and emotional requirements, which need to be considered when organizing work. The aims of the first study were to describe the background and development of the questionnaire ‘Swedish and Danish Dentists’ Perceptions of Good Work’ and to create a measure of overall job satisfaction, applying the measure in four organizational settings. The aim of the second study was to introduce the concept of emotion work in dentistry by giving a theoretical overview of the emotional aspects of work, the conditions under which it is performed and the potential effects on the dentist’s wellbeing. Additional results from the Good Work project have been included in the thesis with the purpose of giving an empirical illustration of how dentists experience the emotional factors related to patient interaction and their job satisfaction. Data from 1226 Danish and Swedish practising dentists was collected in November 2008, with a 68% response rate. An additive index was created to measure overall job satisfaction showing statistical difference in the dentists’ experience according to affiliation (Swedish public/private, Danish public/private). The Danish public dentists had the highest degree of overall job satisfaction and the Swedish public dentists had the lowest. A reason for this difference might be that Danish public dentistry differs from the other three groups in the characteristics of both dentists and patients. However, the lower job satisfaction for the Swedish public dentists could be an effect of New Public Management thinking in organizing dentistry. The additional results showed that Swedish public dentists had substantially less energy left for their private lives compared with the other three groups and only half of them expected to continue working as they do now until retirement. Working directly with or on people is very much about creating good interactions and relations between the health professional and the patient. Good patient relations can be a primary aim and/or a secondary aim, to make other things, e.g. the clinical treatment, easier. To many health professionals their relations with the patients is an arena in which to activate their human potentials and can be experienced as a lasting intrinsic joy from work, called eudaimonia. In the relation with the patient the dentist performs emotion work as an intervention toolkit to direct the patient in a specific direction. Dentists have extensive emotional work tasks in their patient interactions, however this emotional part of dentists’ work is, so far, a neglected research area of odontology. The emotion work tasks are conditioned because the dentists’ incentives are not one-dimensional and require a great deal of emotional flexibility, attentiveness and reflection by the dentist. The influence of the market and managerialism on the professional values of dentistry may challenge the conditions for these tasks in the patient interaction and the wellbeing of the dentist if they are experienced as contradictory. This research aims to encourage and empower different levels of dentistry to further investigate, understand and support the dynamics of the emotional aspects of work with the aim to constitute a sustainable work environment where values and logics can be experienced as compatible with professional values.

Resumo: O posicionamento e a postura do cirurgião-dentista no ambiente de trabalho apresentam riscos de dores e desconfortos músculo-esqueléticos na região da cintura escapular. À avaliação da repercussão da postura ocupacional, sobre a atividade muscular pela eletromiografia, tem sido utilizada em estudos ergonômicos. O objetivo desta pesquisa foi analisar a atividade elétrica dos músculos Deltóide (porção média) e Trapézio (porção superior) em abdução do braço e em 50% da sua contração máxima voluntária que foi obtida através da utilização de uma célula de carga da marca krattos por meio de eletromiografia de superfície em cirurgiões-dentistas com mais de 50 anos, que ainda estão trabalhando, relacionando esta análise eletromiográfica com os dados coletados de outro grupo que seriam cirurgiões-dentistas que estão trabalhando entre um período de 5 a 10 anos. Participaram das análises eletromiográficas 14 cirurgiões-dentistas com mais de 50 anos sendo 9 homens e 5 mulheres com média de idade entre 54,64 anos e média de tempo de trabalho de 30,78 anos e 14 cirurgiões-dentistas com tempo de trabalho de 5 a 10 anos, sendo 11 homens e 3 mulheres com média de idade de 30,64 anos e média de tempo de trabalho de 6,42 anos. Os dados eletromiográficos foram normalizados pelo pico da CVM. A análise final foi à comparação do RMS bruto encontrado nos dois grupos, sendo encontrado o RMS significantemente mais baixo (p<0,01) no grupo de CDs com mais de 50 anos para ambos os músculos, e apesar do teste do qui-quadrado não significativo, provavelmente pelo tamanho da amostra, há fortes indícios que a pratica de atividade física regularmente e tempo de trabalho (horas/dia) mais reduzido no ambiente de trabalho podem diminuir os riscos de dores e desconfortos músculo-esqueléticos na região dos ombros
Abstract: The posture of the Surgeon Dentist, in the work environment, may cause the risk of skeletal muscle pain and discomfort in the scapular region. The recognition of the value of the repercussion of the occupational posture, regarding the use of electromyography, has been practiced in ergonomic studies. The objective of this search was to analyze the electric activity of the muscle deltoid (medium portion) and trapeze (upper portion). To reach such objective a selection of Surgeon Dentists older than 50 and still practicing, abducted their arms in 50% of the maximum voluntary contraction obtained through the utilization of a Krattos charging cell and surface electromyography. A comparison was done, of this analysis, with the data obtained from another group composed of Surgeon Dentists who are still working for 5 to 10 years. This comparison provided an interesting information. 14 Surgeons Dentists with more than 50 years old participate in these analyses, 9 men and 5 women with an average age of 54,64 years and work time average 30,78 years and 14 Surgeons Dentists with work time 5 to 10 years, being 11 men and 3 women with an age average of 30,64 and work time 6,42 years. The electromyographic data was normalized with the high point of CVM. The final analysis was a comparison of the gross RMS found in both groups. However, the lowest gross RMS (p<0,01) was found in the SDs group older than 50 years for both muscles, even thought, the test Chi-Square was found nonmeaningful, probably because the size of the evidence. There are indications that the practice of regular physical activity and work time (hours/days) when reduced in the work environment can diminish the risks of skeletal muscle pain and discomfort in the shoulder region

O objetivo deste trabalho foi investigar quais os procedimentos clínicos e as dificuldades do ortodontista no pós-tratamento ortodôntico (contenção e pós-contenção), verificando se as condutas adotadas satisfazem possíveis reclames de pacientes que já terminaram o tratamento corretivo, com base nas determinações do Código Civil e do Código de Defesa do Consumidor quanto ao relacionamento profissional/paciente. A literatura ortodôntica é controversa no que diz respeito a técnicas, tempo, estabilidade e dificuldades inerentes ao tratamento. Um questionário foi enviado pelo correio a todos os especialistas inscritos no CRO/PR; 95 deles enviaram respostas. A análise percentual das respostas obtidas demonstrou que 73,6% dos profissionais não têm consciência plena do tempo para reclamos à Justiça em relação ao tratamento odontológico. Não conhecem o Art.177 do CCB (83,1%). Adotam contrato (67%), fazem ressalvas (48%), porém não estão cientes da validade das mesmas (59%). A maioria (60%) considera a Responsabilidade Civil do ortodontista como de resultado. Nos casos de recidiva pós-contenção, 70% propõem retratamento ortodôntico. Perante a insatisfação do paciente com o resultado do tratamento ortodôntico, 55% dos profissionais responderam que procurariam, de qualquer forma, evitar que o mesmo impetrasse ação de ordem cível. O desconhecimento da Lei e a existência de controvérsias no pós-tratamento ortodôntico, podem estar levando o ortodontista a responder civilmente pelas movimentações dentárias e alterações neuromusculares que ocorrerem nessa fase.

O cirurgião dentista é constantemente exposto a um elevado número de doenças ocupacionais. Alterações no sistema musculoesquelético são freqüentes, principalmente nos membros superiores e coluna vertebral, que podem interferir negativamente, na capacidade funcional desses profissionais. O objetivo deste trabalho é contribuir para o entendimento das doenças ocupacionais associadas ao sistema musculoesquelético relacionado à coluna vertebral dos odontólogos, durante sua atividade laboral, bem como as questões psicossociais associadas. Este documento contempla uma sequência de dois artigos sobre as condições de trabalho e desordens musculoesqueléticas relacionadas à coluna vertebral em dentistas. O primeiro apresenta uma revisão teórica sobre a atividade laboral dos dentistas, evidenciando os problemas relacionados à coluna vertebral, à carga de trabalho, aos riscos biomecânicos, às principais patologias e fatores psicossociais associados. Desta maneira, se evidenciou que estes profissionais estão sujeitos a uma alta carga de trabalho e desenvolvendo posturas que comprometem a integridade da coluna vertebral gerando dor e incapacidade para o trabalho. O segundo apresenta um método de análise e um estudo aplicado para investigar as desordens musculoesqueléticas relacionadas à coluna vertebral e fatores associados às condições ocupacionais de dentistas. Este artigo explicita as regiões da coluna vertebral mais acometida por queixas musculoesqueléticas e fatores associados. O método investigativo proposto mostrou que os problemas concernentes ao trabalho dos dentistas estão provocando dores e alterações musculoesqueléticas na região da coluna vertebral e são devidos principalmente à postura de trabalho, o esforço físico, os movimentos repetitivos e carga horária de trabalho.
The dental surgeon is constantly exposed to a large number of occupational illnesses. Alterations in the musculoskeletal system are frequent, especially in the upper limbs and spine, which can interfere negatively in the functional capacity of these professionals. The aim of this study is to contribute to the understanding of occupational diseases associated to the musculoskeletal system. Mainly those related to the spine of dentists during their work activities as well as psychosocial issues. This document includes a sequence of two articles about the working conditions and musculoskeletal disorders related to the spine of dentists. The first one presents a theoretical review about labor activity of dentists, highlighting the problems related to the spine, the workload, the biomechanical risks, the major pathologies and psychosocial factors associated. In this way, it is evidenced that these professionals are subject to a high workload and developing postures that compromise the integrity of the spine causing pain and inability to work. The second presents a method of analysis and an applied study to investigate musculoskeletal disorders related to the spine and occupational factors associated with conditions of dentists. This article explains the spinal regions most affected by musculoskeletal complaints and associated factors. The investigative method proposed, showed that problems regarding to the work of dentists are causing pain and musculoskeletal alterations in the spinal region and are due mainly to the working posture, physical effort, repetitive movements and workload.

The problems associated with a global increase in antibiotic use and prescription opioid abuse have been well documented. The aim of this study was to assess the prescribing decisions of general dentists and endodontists in British Columbia regarding analgesics and antibiotics in clinical scenarios that involve endodontic disease. An additional aim was to determine whether gender, clinical experience, or the practice location of the clinician impacted their decision-making. A four-page survey was developed and distributed to half of the general dentists in Vancouver, British Columbia (n=284) and all of the endodontists in British Columbia (n=51). The survey began with some basic demographical questions, continued with seven different clinical scenarios, and concluded with a table that listed a number of different endodontic diagnoses. The participants were asked questions regarding their decisions to recommend or prescribe analgesics or antibiotics. The response rate to the survey was 49.2%. 72.0% of endodontists responded, as compared to 44.8% of general dentists. A substantial proportion of clinicians prescribed opioid analgesics and antibiotics in the various clinical scenarios. The prescription of opioid analgesics ranged from 3.6% - 46.8% over the various scenarios. The prescription of antibiotics ranged from 4.8% - 88.4%. General dentists reported prescribing opioid analgesics and antibiotics more often than endodontists. However, the respondents to our survey prescribed antibiotics less frequently than those of a 1996 survey conducted in the USA. Gender, clinical experience, and practice location did not seem to impact the decision to prescribe opioid analgesics or antibiotics.

BackgroundWithin dentistry, a limited body of literature exists regarding the referral relationships between general practitioners (GPs) and specialists. The purpose of this study was to investigate the referral relationship between GPs and periodontists within the state of Virginia. MethodsA survey focused on the demographic variables in the referral relationship between GPs and periodontists was developed. The survey was mailed to 800 dentists throughout the state of Virginia. Descriptive statistics was completed along with multivariate logistic regression analysis comparing the responses with the number of patients referred per month to a periodontist. ResultsFemale respondents were more likely to refer three or more patients per month to a periodontist than a male respondent (pConclusion This study indicates that four demographic variables have a statistical influence on the number of referrals per month from a GP to a periodontist. These variables are: female gender, practicing with one other dentist, employing two or more hygienists, and being greater than five miles away from the nearest periodontist.

Background: Patient-centred care (PCC) has been linked to many positive outcomes in medicine. Dentists currently have no available tool for practicing this approach as part of a daily routine in a dental setting. The latest UK General Dental Council (GDC) standards for dental teams has amplified the need for such a tool (GDC, 2013). Aim: The overall aim of this thesis is to develop and validate a tool for dentists in order to encourage them to practice PCC in dental settings. Methods: This research employs a mixed method research design. This thesis is divided into three studies. Study 1 includes semi-structured interviews that were conducted with 20 practicing dentists who had clinical teaching responsibilities at King’s College London. These interviews explored their views and opinions on PCC in general as well as on a published model of PCC and how applicable this model would be in a dental setting. Study 2 builds on study 1 findings by examining the ability of dental students to assess the communication skills of another dentist. This cross-sectional survey exposed students to a stage consultation via a YouTube video clip and then asked them to use the tool to evaluate the performance of the dentist in the clip. Study 3 entails validation of the newly developed PCC self-reflection tool by assessing its face, content, and criterion validity and test-retest reliability. To determine content validity, experienced dentists who were involved in education and practice evaluated the items based on relevance, clarity and representativeness of the construct as well as the ease of response. The criterion validity was determined in parallel with study 2 using a survey. For the test-retest reliability part, the modified tool was assessed twice within a 10-day period to rate a dental consultation clip by dental students (N=25). Results: The interview analysis highlighted five themes: understanding PCC, the role and influence of patients on the delivery of PCC, the role and influence of dentists on the delivery of PCC, the importance of context and dentists’ views on a hierarchy of PCC. Each of these themes included a number of sub-themes that covered the different issues identified via interview transcript analysis. The themes showed that dentists have a basic and simplistic understanding of the concept of PCC. Dentists identified a number of patient, dentist and contextrelated barriers that could hinder the adoption of PCC. Interviewees thought the hierarchy of PCC they discussed would be useful to aid understanding of PCC but more so for less experienced dentists and dental students. The analysis of the cross-sectional survey study revealed a broad understanding of the basic functions of the communication process, although students’ judgments of the quality of such communication was exaggerated. The development and validation of the tool, led to a final version of the tool containing six items. The tool showed good validity and reliability. Conclusion: A new, practical, PCC self-reflection tool was developed for dentists based on a review of the literature and interviews with dentists. The tool exhibits good content and criterion validity as well as acceptable test-retest reliability. Dentists may use this tool in dental settings to become more aware of PCC as a concept by focusing on two specific aspects: information provision and choice giving.

Método: Foram incluídos no estudo 240 cirurgiões-dentistas, com atuação profissional na cidade de Anápolis (GO), que responderam a um questionário auto-aplicável, com perguntas abertas e fechadas, previamente testado, contendo questões demográficas e relacionadas ao conhecimento e utilização da técnica de citologia esfoliativa. Após o preenchimento do questionário, foi entregue ao profissional um manual com informações sobre a técnica a fim de esclarecer quaisquer dúvidas sobre este importante exame complementar de diagnóstico. Em seguida os dados foram analisados estatisticamente (Qui-quadrado, Kruskal Wallis ao nível de 5%). Resultados e Conclusão: a) Cirurgiões-dentistas da cidade de Anápolis (GO) possuem conhecimento insuficiente sobre a técnica da citologia esfoliativa, mas reconhecem de maneira satisfatória as patologias em que a citologia pode ser aplicada; b) Indivíduos que realizam cirurgia e implantodontia relataram mais conhecimento de citologia que aqueles que realizam ortodontia; c) Indivíduos com cursos de atualização ou especialização relataram tanto conhecimento sobre citologia quanto aqueles sem pós-graduação, e mostraram menor conhecimento quanto ao fixador utilizado; d) Divulgação da técnica de citologia, assim como de métodos de prevenção e de diagnóstico precoce de doenças bucais, são necessários.
Objective: To evaluate the profile and knowledge that dentists in the city of Anápolis (GO), Brazil, have of exfoliative cytology and of the applicability of the method for the diagnosis of various diseases that affect the oral cavity, as well as to disseminate information regarding exfoliative cytology. Method: A total of 240 dentists who worked in Anápolis (GO), Brazil, were included in the present study. The dentists completed a previously tested self-administered questionnaire containing open and closed questions regarding demographic data, the knowledge of exfoliative cytology, and the knowledge of the use of this technique. After completing the questionnaire, the dentists received a manual containing information about exfoliative cytology in order to clarify doubts regarding this important complementary test. The data obtained were statistically analyzed by means of the chi-square or Kruskal Wallis test, at a significance level of 5%. Results and conclusion: a) Dentists in Anápolis (GO), Brazil, have little knowledge of exfoliative cytology. However, they can satisfactorily recognize the cases in which the technique can be used; b) Dentists who specialize in surgery and implants reported greater knowledge of the technique than did those who specialize in orthodontics; c) Dentists who had a specialist degree reported as much knowledge of the technique as did those who had only an undergraduate degree; in addition, the latter had greater knowledge of the fixing agent used; and d) Information regarding exfoliative cytology and the methods for the prevention or early diagnosis of oral diseases must be disseminated.

Resumo: O Brasil concentra uma grande quantidade de Faculdades de Odontologia, formando assim um número de cirurgiões-dentistas maior do que o mercado de trabalho é capaz de absorver. A região administrativa de Campinas é uma das mais desenvolvidas economicamente no Estado de São Paulo, sendo composta por sete regiões de governo, abrangendo 90 dos 645 municípios paulistas. O objetivo deste estudo foi analisar a distribuição geográfica de profissionais de Odontologia nesta região nos anos de 1970, 1980, 1990, 2000 e 2005. O número de faculdades, de cirurgiões-dentistas e de pessoal auxiliar por município foi obtido no Conselho Federal de Odontologia. Os dados populacionais e administrativos no Instituto Brasileiro de Geografia e Estatística e na Fundação Sistema Estadual de Análise de Dados. Os resultados foram analisados por estatística descritiva e por regressão quadrática. A relação de cirurgião-dentista/habitante observada foi 1:9.985 em 1970, 1:2.019 em 1980, 1:1.229 em 1990, 1:773 em 2000, e, 1:669 em 2005. O aumento do número de Faculdades de Odontologia e de vagas oferecidas proporcionou o aumento do número de dentistas neste período. A relação de cirurgião-dentista/habitante e a de cirurgião-dentista/pessoal auxiliar observadas mostraram-se discrepantes. Estes resultados demonstram que o mercado de trabalho nesta região está em desequilíbrio, pois notou-se que há uma concentração maior de profissionais em grandes centros, enquanto que em pequenos municípios o número destes profissionais foi menor ou até mesmo inexistente. Contudo, sabe-se que o grande número de profissionais não implica em melhorias nas condições de saúde bucal da população. Políticas futuras de educação voltadas aos Cursos de Odontologia, visando à redução do número de faculdades; e também políticas de incentivo profissional para que cirurgiões-dentistas possam trabalhar em regiões menos desenvolvidas deste país devem ser implantadas
Abstract: Brazil concentrates a great amount of dental schools, forming a higher number of dentists than the work market is capable to absorb. The administrative region of Campinas is one of the most economically developed in the State of São Paulo, being composed by seven regions of government, enclosing 90 from the 645 São Paulo cities. The aim of this study was to assess the geographic distribution of dental professionals in this region in the years of 1970, 1980, 1990, 2000, and 2005. The number of dental schools, dentists, and dental auxiliars for city was obtained at the Dental Council of Brazil and the populational and administrative data at the Brazilian Institute of Geography and statistics and the State System of Data Analysis Foundation. The results had been gotten by Internet and analyzed by descriptive statistics and quadratic regretion. The relation of dentists/habitant had been 1:9.985, in 1970, 1:2.019, in 1980, 1:1.229, in 1990, 1:773, in 2000, and, 1:669, in 2005. The increasing number of dental schools and of offered vacancies provided this growth on the number of dentists in these periods. Dentists/habitant and dentists/dental auxiliars relations showed to be discrepant between itself. These results demonstrate that the work-force in this region is unbalanced due to the greater concentration of professionals in great urban centers, while in small cities there is a lower or inexistent number of this kind of professionals. However, it is known that the great number of professionals does not imply in improvements in the oral health status of the population. Further politics of education, aiming the reduction of the number of dental schools and offered vacancies, and also professional incentive for work in less saturated regions of this country must be implanted.

A recent survey of the oral health of the institutionalized elderly in Greater Vancouver revealed that despite a number of dental problems amongst the chronically ill, few had been seen by a dentist since their admission to the facilities. A major issue in establishing dental services is the willingness of dentists to provide services in the long term care facilities. Three theoretical models were developed to predict dentists' interest in long term care dental practice. The socio-cultural model, which emphasizes cultural prejudices towards the elderly, was based on a number of attitudinal studies of the profession(eg.the belief that the elderly are uncommunicative and ungrateful or the aversion to depressing atmospheres of facilities). The professional model focused on professional training and socialization and the ideological focus of dentistry on reparative therapy(eg.undergraduate training with the disabled, or the perception of limited treatment options for the chronically ill). The economic model predicted that opportunity costs, that is, private practice and leisure time, would be critical in the decision to treat the institutionalized elderly. Questions relating to each of these models were included in a survey questionnaire. The survey, mailed to 603 dentists in the greater Vancouver area, asked dentists about their experience and interest in working with both the well and disabled elderly. Over half the population responded to the survey, and the results were analyzed to determine which model best predicted interest in institutional dental delivery. In the findings, 37% of the dentists indicated definite interest in practising at a facility if called upon by the administrator, and 19% of the survey respondents had practised at a facility previously.lt was concluded from a multiple classification analysis that both professional and economic, but not socio-cultural, forces guide the servicing decisions of dentists with respect to long term care service. Professional satisfaction in treating the elderly was the most significant predictor of interest. This satisfaction was linked to several enabling factors, such as the availability of a dental operatory at an institution, and experience and training with medically compromised patients. Those dentists who had extensive institutional experience, and young dentists who were beginning a practice and who were not busy, were the dentists most likely to be interested in extending their practice to long term care facilities. Conversely, those dentists who were busy in their private practice, and who had little training in treating the chronically ill, were the least likely to show interest in this area. The findings indicate that in order to extend dental services to those elderly in nursing homes and hospitals, an appropriate education, both didactic and clinical, and an acceptable financial arrangement for such specialized services should be introduced into the dental community.

The purpose of this study was to evaluate what factors influence dentists in their decision to provide services in long-term care facilities within British Columbia. The secondary purpose was to determine if dentists practicing in rural areas of British Columbia are more willing to provide services in LTC compared to dentists in urban areas. Also, to assess if there were any changes in opinions of dentists (practicing in Metro-Vancouver) in providing services to patients in long-term care compared to a similar study from 1985. A questionnaire was developed to determine views and opinions of general dentists practicing in British Columbia with respect to the provision of services in long-term care. Eight hundred dentists from urban and rural areas of British Columbia were randomly selected to participate in this study. The British Columbia Dental Association mailed a package containing 3 questionnaires. The participants were to fill out one of the questionnaires based on whether they treated, never treated or stopped treating patients in long-term care. These questionnaires were faxed back to the British Columbia Dental Association. A reminder was sent out to the dentists 3 weeks after the initial mail-out. About thirty percent of those dentists surveyed responded with completed questionnaires for analysis. Dentists who treated patients in long-term care reported that it was a part of their professional responsibility to provide services. The lack of a dental operatory and lack of experience/training in geriatric dentistry were primary concerns of dentists who never provided services. Compared to 1985, dentists in 2008 showed increased awareness for a need for dental services by patients in long-term care facilities. Dentists in rural areas were more likely to be providing services to patients in long-term care facilities, compared to dentists in urban areas. Dentists who never provided services in long-term care facilities expressed interest in providing dental services.

During the past years it has been reported that musculoskeletal disorders (MSD) have increased in working life. The body region with the highest prevalence of reported pain is the neck region. Several studies show that MSD in the neck region varies between 20–68% among dentists and dental hygienists. The dental personnel work with a limited field of vision and space, which often results in an unnatural work posture. The aim of this study was to evaluate the prevalence of neck pain in dentists and dental hygienists working in the county of Västra Götaland, Sweden and to explore possible work-related and individual risk factors. The study was based on a questionnaire, which was sent out to all dentists and dental hygienists (n = 473) who worked at Folktandvården (response rate 60%). The overall prevalence of neck pain (neck pain every day the past month) was 23%. Female dentists and dental hygienists had an increased risk of neck pain compared to male dentists (OR 2.68, 95% CI 1.22 - 6.75). Dental personnel that experienced severe stress tended to have an increased risk of neck pain compared to those who perceived low stress (OR 2.08, 95% CI 0.94 - 4.53). We conclude that neck pain is a common problem among dental personnel in the county of Västra Götaland. Female dentists and female dental hygienists are at greater risk concerning the prevalence of neck pain.

The purposes of this study were to evaluate if orthodontists’ and general dentists’ perceptions of how often, under what circumstances, and by which media orthodontists communicated with general dentists were similar, and to determine if orthodontists were meeting the communication needs of general dentists. Orthodontists (N=1,000) and general dentists (N=1,000) throughout the United States were randomly selected to participate in a web-based and mailed survey, respectively. The results indicated that the orthodontists communicated with general dentists using the type of media the general dentists preferred to use. As treatment complexity increased, orthodontists shifted from one-way forms of communication to two-way forms of communication (P < .05). Both orthodontists and general dentists reported that orthodontists’ communication regarding white spot lesions was inadequate. When treating patients with missing or malformed teeth, orthodontists reported they sought input from the general dentists at higher rate than the general dentists reported (P < .005).

The purpose of this study was to identify and compare preferences and perceptions of orthodontists and general dentists when restoring peg-shaped lateral incisors. The investigation sought to summarize these preferences with regard to treatment planning, tooth preparation and interdisciplinary communication. A pair of mailed and electronic surveys was distributed to 1,500 general dentists and orthodontists, respectively. The results indicated that general dentists perceived that general dentists held the primary decision-making responsibility, while orthodontists disagreed (P<0.0001). Orthodontists prioritized the treatment goals of Class I canine relationship and overbite/overjet more significantly than general dentists, whom valued tooth proportions more highly (P<0.0001). General dentists reported receiving significantly less input than orthodontists report seeking (P<0.0001).The consensus of both groups showed that the tooth should be positioned centered mesiodistally and guided by the gingival margins incisogingivally. Both groups agree that orthodontists must improve communication to improve treatment results.

Purpose: The purpose of this study is to determine the impact that the introduction of articaine has had on local anesthetic selection by general and pediatric dentists for use in three different age groups of children.Methods: Using a cross sectional survey design, a questionnaire regarding the use of local anesthetics in children was mailed to a random sample of 500 general dentists from North Carolina, 500 general dentists from Virginia, and all 230 pediatric dentists from North Carolina and Virginia. The 16-item questionnaire included questions regarding the preferred local anesthetic to use in three different age groups, 2-3 years of age, 4-6 years of age, and 7-10 years of age. Furthermore, the questionnaire also included questions specifically on articaine use in the three different age groups and any related side effects. The association between dental practitioner type and anesthetic use was tested using chi-square or Fisher's exact test.Results: A sample of 337 dentists completed the questionnaire. There was no significant difference in preference of articaine except in older patients aged 7-10 years old where general dentists prefer articaine significantly more than do pediatric dentists (28.1% versus 15.9%). Lidocaine with epinephrine was the local anesthetic that was most preferred in all age groups by all practitioners. Pediatric dentists preferred lidocaine more often than general dentists and general dentists preferred lidocaine without epinephrine more often than pediatric dentists. Twenty-one percent of all dentists surveyed have used articaine in children under 4 years of age and 13% list articaine as the preferred local anesthetic for children under 4 years of age.Conclusion: While lidocaine with epinephrine is still the preferred local anesthetic for use in children, the use of articaine in children is very prevalent among general and pediatric dentists. Articaine use becomes more prevalent as the age of the patient increases and many pediatric and general dentists are using articaine in children under four years of age.
