Abstract:
Health care resources are often insufficient to meet the demands placed upon them. Managing scarcity and assessing the merits of competing priorities are therefore key responsibilities for health care decision-makers. In April 2006, the new General Dental services (nGDS) contract introduced local commissioning to NHS dentistry for the first time. Primary Care Trusts (PCTs) were charged with managing devolved and finite resources, but they have since drawn criticism as a consequence of variable quality commissioning. Economics may offer some solutions to assist NHS commissioners. Programme Budgeting and Marginal Analysis (PBMA) is a pragmatic economics-based approach which may inform and assist decision-makers to prioritise scarce resources. The research was divided into three phases: a national qualitative survey of NHS decision-makers' roles in managing scarce dental resources; a PCT -based qualitative case-study involving patients, dentists and PCT staff; and a PBMA- based dental priority setting exercise. Phases 1, 2 and 3 collectively used semi- structured interviews, focus groups, an action research approach and mixed methods. The research highlighted that dental decision-makers were constrained in their ability to commission dental services as a consequence of restrictions imposed by the national dental contract. Local priority setting and resource allocation generally lacked transparency and decision-makers typically failed to include patients and clinicians in these processes. With respect to health economics, stakeholders found the pictorial representation of the cost-value ratio (CVR) a useful starting point for further deliberation, but the full application of a traditional PBMA exercise was hindered locally by organisational change and severe PCT staffing pressures. The concerns identified by stakeholders are timely considering the Government's proposals to abolish PCTs and commission NHS dental services through the NHS Commissioning Board alongside a new national dental contract in 2014.

Abstract:
The purpose of this study was to determine the awareness and attitudes of dental hygienists in Ohio regarding the Advanced Dental Hygiene Practitioner (ADHP) as an occupation proposed by the American Dental Hygienists’ Association and to determine whether the ADHP could be a viable career option for Ohio dental hygienists. I developed a survey to assess dental hygienists’ awareness, attitudes, and their views regarding the new mid-level dental provider. The study was limited to licensed dental hygienists in Ohio. The study included a simple random sample of 400 of the 4100 dental hygienists in Ohio obtained by a systematic approach, a random start and selection of subjects at a constant interval. The method of data collection was electronic surveys. Fifty-four dental hygienists (13% of the sample) participated in the study. The results show that most of those responding were aware of the ADHP. Also, their attitude regarding the new mid-level dental provider was positive. Attitudes varied more when the respondents provided data on the adoption of legislation for the new provider. There was no consensus among respondents regarding the ADHP as a viable career option. More robust research is needed on the viability of the ADHP in Ohio.

Abstract:
Background: More than 10% of UK adults suffer extreme dental anxiety. Effective verbal and non-verbal communication (VNVC) in the dental dyad following handover of a completed Modified Dental Anxiety Scale (MDAS) had been hypothesised to reduce dental state anxiety. This study aims to consider whether: (1) ‘MDAS completion and handover effects long term dental trait anxiety, and (2) ‘what VNVC elements influence patient anxiety reduction?’ Trial Design: Mixed methods: (i) a randomised cross-over study and (ii) an observational study. Methods: Six NHS Highland Salaried Dental Practices were randomised to start either with the experimental arm, where dentally anxious patients (MDAS score = 19 or 5 in any one question) complete and handover MDAS to the dentist, or the control arm where no handover occurred. Within the observational study, all participants had their dental treatment appointment videoed. The primary outcome was dental trait anxiety as measured by MDAS, pre-treatment and at 3 month follow-up. Secondary outcome was dental state anxiety, measured pre and post treatment using both the Spielberger State-Trait Anxiety Inventory (STAI-S) and Heart rate.The observational study analysed the first 2 minutes of video and related this data to the behavioural and affective outcomes. Results: 54 patients agreed to take part in the study with 47 completing 3 month follow-up. The randomised cross-over study found no difference in state or trait dental anxiety however observational outcomes showed dental state anxiety remained high when handover was blocked. A significant lack in decline in dental trait anxiety was shown when dentists blocked patient expressed cues and concerns; this occurred irrespective of patient gender, age or number of appointments received in a 3 month period (F[155.06]=7.51, P=0.009). Conclusions: VNVC flow is fundamental to dyadic balance even within the first two minutes of the patient-dentist interaction. The balanced dyad is key to dental anxiety reduction.

Abstract:
Many physical changes occur as one ages, including changes associated with the oral cavity. A review of the literature suggests that the provision of dental care to institutionalized elderly patients presents problems due to a variety of factors. The purpose of this study is to assess the level or dental care provided to residents of long-term care facilities located in Warren County, Kentucky. In addition, secondary objectives Include the ascertainment of who provides dental care to residents and the amount or in-service dental training made available to staff members of the facility. Each administrator of the long term care facilities located in Warren County participated in an *interview conducted by the author. During the interview, information was obtained for a 21 item questionnaire concerning the facility, the number and age range or the residents, and types of dental services provided within the facility. Results obtained from the questionnaire indicated that 77 percent or the residents in long-tern care facilities in Warren County are 70 years of age or older. No significant differences were noted in the types or dental services provided to residents. However, the dental services provided ranged from those that were obtained in a private dental office via transportation or the resident to outside dental facilities to routine oral hygiene measures carried out by staff members employed by the facility. The findings revealed significant differences in the dental status of the MRDD residents when compared to the nursing home residents. Other findings indicated that none of the long-term care facilities had dental operatories or dental radiographic equipment on the premises. Additional research would be required in order to address uncertainties discovered in the study. A followup to the questionnaire Interview with the consulting dentists may be included to determine to what capacity and to what extent they are utilized by the facilities. Other recommendations include the utilization of entrance dental examinations to determine if services offered do meet the needs of the residents and periodic dental examinations to aid in detection and thus reduce the prevalence of dental diseases in this population.

Abstract:
Managed care has become a prominent mechanism for insuring dental care. Empirical research suggests that managed dental plans provide lower quality care to patients. However, few studies have specifically addressed the effects of managed care on the quality of dental hygiene care. Thus, in this study the researcher examines whether dental hygienists deliver a lower level of treatment to managed care patients than to those who are not subject to managed care. Questionnaire data were gathered from 193 members of the American Dental Hygienists' Association residing in the Chicago area. The primary independent variable, managed care, was measured with an item that asked the respondents to indicate the percentage of patients they treat that are insured by a managed dental plan. The questionnaire also contained items that measured the frequency in which the respondents perform 23 tasks that are indicators of quality of dental hygiene care. Principal components factor analysis of these 23 items yielded the study's two dependent variables: periodontal procedures and appointment time. Regression analysis of the data revealed a significant negative relationship between managed care and appointment time. This relationship may be attributable to an economic incentive on the part of dentist-employers who control the amount of time scheduled for dental hygienists' patients. Dentist-employers may reduce the time available for managed care patients in order to allow longer appointments for more profitable fee-for-service patients. The study results did not support the notion that managed care affects the extent to which dental hygienists perform periodontal procedures. These mixed results suggest that future research should examine the relationships between managed care and other aspects of quality of dental hygiene care not addressed in the current study.

Abstract:
Essay 1: Individuals with higher socioeconomic status (SES) also tend to enjoy better health. Evidence from the economics literature suggests that a potential mechanism behind this “social health gradient” is that human capabilities, that form SES, also facilitate health-promoting behaviors. This essay empirically investigates the significance of socioeconomic differences in health behaviors, using dental care consumption as an operationalization of health investments. I focus on adults at an age where lifetime trajectories for SES can be taken as given and use lifetime income to capture SES. I estimate the impact of lifetime income on dental care consumption and find robust evidence that the social gradient in dental care consumption steepens dramatically over the life-cycle. Considering that dental care consumption only reflects a small part of individuals' health investments the results suggest that lifetime effects of SES on health behaviors could be substantial in other dimensions. Essay 2: This essay studies the effect of competition on prices on a health care market where prices are market determined, namely the Swedish market for dental care. The empirical strategy exploits that the effect of competition differs across services, depending on the characteristics of the service. Price competition is theoretically more intense for services such as examinations and diagnostics (first-stage services), compared to more complicated and unusual treatments (follow-on services). By exploiting this difference, I identify a relative effect of competition on prices. The results suggest small but statistically significant negative short-term effects on prices for first-stage services relative to follow-on services. The results provide evidence that price-setting among dental care clinics responds to changes in the market environment and substantial effects of competition on prices over time cannot be ruled out. Essay 3: The Swedish dental care insurance subsidizes dental care costs above a threshold and becomes more generous as dental care consumption increases. On average, higher-income individuals consume more dental care and have better oral health than low-income individuals. Therefore, the redistributional effects of the Swedish dental care insurance are ambiguous a priori. I find that the dental care insurance adds to the progressive redistribution taking place through other parts of the Swedish social insurance (SI) for individuals aged 35-59 years whereas it reduces the progressivity in the SI for those aged 60-89 years. While the result for the oldest individuals is problematic from an equity point of view, the insurance seems to strengthen the progressitivy of the Swedish social insurance for the vast majority of patients.

En la presente disertación han sido tratados el problema de la hipersensibilidad dental y los procesos remin/demin. También han sido desarrolladas nuevas herramientas científicas para la evaluación de la eficacia de los productos dirigidos a la hipresensibilidad y a la prevención de la creación de caries dentales. Además, se han diseñado y llevado a cabo varios experimentos para investigar el estado del tejido dental mediante espectroscopia FTIR sin y con la fuente de sincrotrón. Con el objetivo de desarrollar el material oclusivo adecuado y determinar su eficacia; la porosidad de la dentina debe ser determinada. En la presente tesis doctoral ha sido desarrollada una nueva metodología para la evaluación de la eficacia del tratamiento de la hipersensibilidad dental. Este estudio se ha presentado en la primera parte del trabajo y se basa en la metodología de los discos dentinarios (dentin disc model). Los especímenes tratados con los agentes desensibilizantes han sido observados mediante la Espectroscopia Electrónica de Barrido (SEM). Las imágenes obtenidas han sido analizadas por un algoritmo automático de segmentación, desarrollado para ser usado mediante el software de MATLAB, el cual calcula el número y el área de los túbulos abiertos. Dicho método es una herramienta adecuada para que tanto científicos, como compañías privadas puedan llevar a cabo la supervisión y asistencia de los procesos remineralizantes de los tejidos dentales, o bien para el desarrollo de los nuevos productos focalizados en el problema de la hipersensibilidad dental. Las siguientes partes de la disertación se han enfocado en el problema de los procesos desmineralizantes del tejido dental. En la segunda parte de la tesis doctoral se presenta la implementación de una nueva técnica, la de Hyper Spectral Imaging (HSI) para la evaluación de la eficacia del tratamiento remineralizante. Para el análisis de los dientes humanos antes y después de la remineralización, se ha utilizado principalmente el Hyper Spectral Imaging que implementa dos tipos de cámaras de Infrarrojo Cercano (NIR) (161 y 256 canales); y como método complementario la Microscopía Confocal de Raman (CRM). Los resultados obtenidos han sido detalladamente analizados mediante una rutina de MATLAB desarrollada especialmente para analizar ese tipo de imágenes. El coeficiente de correlación y la técnica de MCR (Multivariate Curve Resolution) han sido utilizados para recabar la información necesaria de los espectros adquiridos. Gracias a su simplicidad, el método puede convertirse en una poderosa herramienta para obtener información sobre los efectos del tratamiento remineralizante aplicado sobre el tejido dental. Durante el proceso de evaluación del nuevo producto dental, ese tipo de información puede ayudar a decidir si el producto es eficaz y compararlo con otros agentes del mismo tipo. En la tercera parte de la presente tesis doctoral la Elipsometría de Infrarrojo (IRSE) y la Elipsometría con la radiación de Sincrotrón han sido escogidos para la determinación de las constantes ópticas del tejido dental humano. Los especímenes dentales han sido tratados con el producto remineralizante y analizados mediante la Elipsometría IR con la radiación de Sincrotrón. Posteriormente, una serie de experimentos y simulaciones han sido llevados a cabo para obtener una muestra de referencia adecuada para la interpretación de los datos obtenidos en el sincrotrón. En la última parte, se han estudiado los cambios de la estructura del esmalte humano y de la dentina producidos por el proceso de la remineralización. Los espectros de varios puntos de los especímenes dentales han sido comparados mediante la técnica FTIR (Fourier Transform Infrared Spectroscopy). Los datos obtenidos han sido interpretados aplicando el Análisis de Componentes Principales (PCA) y la deconvolución de los espectros (curve-fitting). Los resultados han sido comparados con las muestras de referencia de hidroxiapatita, fluorapatita y las consideradas en la bibliografía científica.
In presented dissertation a problem of hypersensitivity and remineralization/demineralization problem is treated. A novel, scientific tools for the evaluation of the efficacy of products targeting the hypersensitivity or caries creation were proposed. Moreover, experiments implementing FTIR spectroscopy as well as synchrotron techniques for the dental tissue investigation were performed. In order to design a proper sealing material and to investigate its efficacy, the porosity of the dentine has to be determined. In the present dissertation a novel methodology for the evaluation of the efficacy of the hypersensitivity treatment has been developed. It is presented in the first part of the work and is based on the dentin disc model methodology. Specimens treated with a desensitizing agent were observed under Scanning Electron Microscope (SEM) and the resulting images were analyzed by the MATLAB automatic segmentation algorithm developed to calculate the number and area of the open tubuli. The present method is a suitable scientific tool that will allow scientists and private companies the in vivo monitoring and assistance on the development of new remineralising agents. The comparison of three commercial toothpastes presented in this work showed a successful application of the developed methodology and proved it to be suitable for that kind of comparative studies. The proceeding parts of the presented dissertation were focused on the problem of the demineralization and opposite-remineralisation processes of teeth. In the second part of presented PhD thesis a novel approach implementing the Hyper Spectral Imaging (HSI) technique for the evaluation of the efficacy of the remineralising treatment is presented. Hyper Spectral Imaging (HSI) with two types of NIR cameras (161 and 256 channels) and confocal Raman microscopy (CRM) as a complementary technique were applied for analysis of teeth before and after application of different remineralizing agents. Obtained results were carefully analyzed and compared by the implementation of a MATLAB routine that used the correlation coefficient and MCR data analysis in order to obtain necessary information from the acquired spectra of dental samples. Taking into consideration the simplicity of the presented method it can be a powerful tool to obtain basic information about the performed treatment. In the process of the evaluation of a new dental product, this kind of information can lead to the decision whether to continue the development of the product or to proceed with another, more promising approach. In the third part of the presented dissertation the Infrared Spectroscopic Ellipsometry (IRSE) and synchrotron ellipsometry is proposed for the determination of the optical constants of human dental tissues. Human teeth were treated with the remineralising agent and analyzed with the synchrotron IR ellipsometer. Furtherly, a series of experiments and simulations were performed in order to obtain the most suitable reference sample for the interpretation of the obtained synchrotron data. In the last – fourth part, structural changes in the human enamel and dentin induced by the remineralization process were investigated. The spectra from various points proceeding from different locations of the tooth were compared and studied by the FTIR technique. Obtained data were interpreted applying Principal Component Analysis (PCA) and curve-fitting procedures. Obtained results were compared to the reference hydroxyl and fluorapatite powders and scientific literature.

Objectives: The intent of publicly financed dental insurance programs is to provide to children in deprived circumstances the coverage they need to remove the financial barrier to obtaining dental treatment. Recent research has revealed that publicly financed programs available to adults have failed to overcome financial barriers to dental care for many, yet surprisingly there has been no research on this issue regarding access to treatment for children. Assessing how well publicly financed dental programs are meeting the needs of children is a significant gap in the literature and this study was undertaken in an attempt to begin to fill that gap. This study is limited to the Healthy Kids Dental Program of British Columbia. Methods: Qualitative description was the method used for this study. Little is known about this issue of children accessing dental care under publicly financed dental programs, therefore this research was exploratory. Because the research was intended to uncover the range of experiences for a specific population, the sampling technique was purposeful and sought maximum variation. Data collection was via 16 semi-structured interviews, primarily from residents in the North and Central Okanagan (14), and two from the Kootenay Boundary area. The data were analyzed using the method of content analysis. This research sought to describe phenomena by uncovering the voice of those affected through stories of their experiences. Results: The Healthy Kids Dental Program did work very well for some families. Parents greatly appreciated the financial help that was provided. However, over half of participants interviewed did not have the financial help they needed from the program to obtain dental care. The results show that for many people the publicly financed dental program for children in BC is not doing what it was developed to do, i.e., help parents overcome financial barriers to obtaining dental treatment for their children. Conclusion: The Healthy Kids Dental Program is valued, the program is serving some participants well but failing others, and there exists a discrepancy between what the dentist bills and what the government funds. This is creating financial barriers to dental care that some families cannot overcome.

Dental anxiety and the avoidance of dental treatment serve as substantial barriers to dental care and are associated with a range of maladaptive behaviors and outcomes. However, many important psychological constructs that may increase our understanding of anxiety and avoidance in relation to dental care and hence our ability to properly intervene have been poorly examined or neglected in the literature. The present study examined the prevalence of dental anxiety, specific phobia of dental procedures, and avoidance of dental care among dental patients presenting for routine (n = 78) and emergency (n = 42) care using self-report questionnaires, clinician-administered interview, and 6-month follow-up dental appointment data. We investigated correlates of dental anxiety, dental phobia, avoidance of dental care, and oral-health related quality of life (OHRQoL). It was expected that emergency patients would exhibit significantly greater impairment that regular care patients across several variables, with a similar pattern expected for patients with a specific phobia relative to those without the diagnosis. Further, it was hypothesized that emotion regulation, distress tolerance, experiential avoidance, and mindfulness would moderate the anxiety-avoidance and anxiety-OHRQoL relationships. Additional models tested the moderating roles of pain, optimism, and re-experiencing symptoms related to a prior traumatic dental event on the relationship between social appearance anxiety and avoidance of dental care. Dental anxiety, severity of phobia, and OHRQoL correlated with social appearance anxiety, pain experienced at the last dental appointment, re-experiencing symptoms, and blood-injection-injury fears, among other variables; however, there was no evidence that they were related to emotion regulation, and limited relationships emerged with experiential avoidance, distress tolerance, and mindfulness. Avoidance of dental care was unrelated to dental anxiety, other forms of anxiety, OHRQoL, pain, pessimism, and several other constructs related to emotion dysregulation, and difficulties with mindfulness. Our hypotheses that emergency patients would have a higher prevalence of dental phobia, longer avoidance, and greater pain at the last dental appointment than regular care patients was supported, although the groups did not differ on dental anxiety or remaining variables. As expected, patients with specific phobia (n = 26) scored higher than those without a diagnosis (n = 94) on dental anxiety, OHRQoL, social appearance anxiety, re-experiencing symptoms, and pain, but these groups did not differ on avoidance or other constructs. Our moderation hypotheses had minimal support: Dental anxiety was most strongly associated with avoidance of a dental appointment among those reporting the greatest experiential avoidance and the least mindful awareness, and dental anxiety was most strongly associated with OHRQoL among those demonstrating the greatest mindful observing. Implications of these findings are addressed, followed by a discussion of study limitations and directions for future research.

The oral flora is one of the most ecologically diverse microbial populiations known to man. It has been proved that any dental intervention: dental preparation, professional oral hygiene, extraction of teeth, regular prophylactic examination of patient‘s mouth is related with a possible spread of infection. Therefore, one of the most important tasks for the dental care professionals is to prevent the spread of infection and create safe environment for a patient, the dentist himself and other dental staff The aim of the present study is to identify and evaluate the attitudes of Lithuanian dental health care professionals towards infection control and carried out infection control procedures, and to determine vaccination frequency against hepatitis B among dental health care professionals. The study highlighteningt the dental health care professionals‘ attitude towards infection control and infection control procedures as well as incompliance of infection control procedures with the valid normative documentation was conducted at the Institute of Odontology, Faculty of Medicine, Vilnius University in 2008-2010. Purpose: The specific aim of this study is to evaluate the trends in dental health care for individuals with ectodermal dysplasia. Methods: This was a cross sectional analysis of subjects recruited through the National Foundation of Ectodermal Dysplasia (NFED). From 1997 to 2000, individuals with ectodermal dysplasia or their caregiver (if the individuals were too young to selfreport) voluntarily completed questionnaires. The questionnaire consisted of 37 items consisting of demographics, ectodermal dysplasia diagnosis, access to dental care, level of dental utilization, and type of dental services received. Descriptive statistics were used in addition to ANOVA analyses to evaluate the changing trends in oral health care for individuals with ectodermal dysplasia. Results: Preliminary results indicate: 1) individuals with ectodermal dysplasia are being diagnosed earlier than in the past, 2) physicians are primary source of the initial diagnosis of ectodermal dysplasia, 3) children with ectodermal dysplasia are receiving prostheses earlier than in the past, and 4) access to care is problematic. Conclusion: Diagnosis and recognition of treatment needs are occurring at an earlier age and that an access to dental care for individuals with ectodermal dysplasia continues to be an issue.

Person-centred care (PCC) is recognised as one of the key domains in measuring quality within health care, with patient feedback playing an increasingly important role in assessing the level of service delivered. This has been developed within the general medical services in the UK through the Quality Outcomes Framework and a similar model within general dental practice is currently being piloted. Measurement of PCC as an indicator of quality is likely to be highly relevant within the new Dental Quality Outcomes Framework (DQOF) and the Care Quality Commission key lines of enquiry toolkit. It is important that we are able to understand what we aim to measure before we can consider implementing a tool with which to measure it. This research project aims to understand what is meant by the term "person-centred care” in relation to general dental practice, with the intention that the findings may subsequently be used to inform development of a suitable tool to accurately measure PCC in the future. The research strategy was based on a systematic review and the use of qualitative methods to explore the views of patients and dentists towards PCC in dentistry. The systematic review identified a limited body of research associated with PCC in dentistry, with no studies involving the views of patients. This limited information was used in conjunction with the findings from my qualitative research to identify the key features which are relevant to the delivery of PCC in general dental practice. These features were categorised as relational and functional aspects of care. Relational aspects of care were considered to be closely aligned to the provision of PCC and viewed as an integral feature of its successful delivery. Four dimensions of relational aspects of care were identified: • Connection • Caring attitude • Communication • Control Two sub-categories of functional aspects of care were identified as physical environment and healthcare system, and these were considered to be influencing factors in the delivery of PCC. A provisional model of PCC was developed based on fhe research findings from the patient interviews and introduced during the dentist interviews. It is suggested that future research should include testing of this model to allow refinement and validation.

The thesis consists of two papers which are based on a researchproject called ‘Considerations on Swedish Dental Care’. The aimsof the project were to analyse the characteristics of leadership in thepublic dental service (PDS) in Sweden (Paper I) and to study anddescribe patients’ opinions of dental care provided by the PDS andin private dentistry (Paper II).The aim of the first study was to investigate how experienced chiefdental officers (CDOs) in the PDS managed to maintain a marketposition at a time of social change and increased competition froma changing private sector. The CDOs, who have held a leading positionfor at least five years, were asked to participate in the study. Anempirical study with a qualitative design was conducted. Data from16 CDOs were collected in September-October 2014, with a 75%response rate. The aim of the second study was to investigate adults’ experiencesand opinions of the dental care they have received over time frombeing 50 years old to 70 years old. Patients’ dental visiting patterns,satisfaction with care and oral health measured as numbers of teethwere compared between the two care-provision sectors, public andprivate. In addition, a follow-up was conducted among those whoclaimed to have visited only the public sector or the private sectorand those who claimed to have used both sectors during the wholestudy period. Both studies may be of wider interest when examining Swedishdentistry:a) there are no studies of the characteristics of the top leadershipin the Swedish PDS. The PDS in Sweden differs a great dealfrom the PDS in other countries, as it covers much more of themarket and accounts for almost 45% of the total oral healthcaremarket in Sweden;b) there are no previous studies in Sweden, where a comparison ofpatients’ opinions of care in the two sectors of care providers inSweden is made in this way. The findings in Paper I underscore the fact that CDOs in the PDSexert a great deal of effort to consolidate the actual market position.The PDS is also “open” to patients of all kinds, not only to children,adolescents and special needs groups, and it also offers specialist care,which is unusual in many other countries.Paper II shows that patients who visited the PDS had a slightly poorerdental status, compared with the private patients. Both groups lostteeth during a 20-year period and almost at the same level. AlthoughCDOs in the first study focused on maintaining a strong marketposition, the patients in the second study reported greater satisfactionand a more frequent visiting pattern in private dental care comparedwith the PDS. To summarise, this thesis aims to illustrate the spirit of the top managementin the Swedish PDS and to explore adult patients’ opinionsand experience of care in the two provider sectors in Swedish oralhealth care.

Dental caries is the most prevalent childhood illness and disproportionately affects children from low socioeconomic backgrounds. Dental organizations are collaborating within communities to decrease oral health disparities among children by offering free preventive oral health events. These programs face the problem of low enrollment due to lack of informed parental consent. Also, gaps in the literature indicated the need to examine oral health perceptions and dental-care-seeking practices of culturally diverse low-income parents regarding preventive care for their children. The purpose of this qualitative case study was to explore the reasons why parents are not allowing their children to participate in the aforementioned programs. This inquiry examined how perceived barriers impede parents from seeking free preventive dental care for their children. The transtheoretical model and social cognitive theory were used in this study. Open-ended questions were used to interview 20 purposefully sampled parents regarding perceptions of free preventive dental care programs until saturation. Interviews were audio recorded, and all data were transcribed verbatim, coded, and analyzed thematically. The main themes revealed through this analysis were lack of trust and cultural dissimilarities as potential barriers. Additional themes of money, fear, lack of insurance, transportation, time, and access to care were also confirmed. This study may contribute to positive social change by increasing knowledge that may inform the development of clinical and policy solutions aimed at improving parents' awareness regarding children's oral health, ultimately enabling a reduction in childhood caries and oral health disparities.

One of the most important factors in accessing dental care is having dental insurance. For people with low incomes, Medicaid is the main source of health insurance. Medi-Cal is California’s Medicaid program. Adult dental services were mostly eliminated in Medi-Cal in 2009 due to the economic downturn and partially restored in 2014. The objective of this study is to evaluate the effect of change in Medi-Cal dental coverage, specifically the partial restoration of adult dental coverage in 2014, on dental care utilization among Medi-Cal beneficiaries. The partial restoration significantly increased the utilization rates in dental clinics from 2014 to 2017 (22% in 2017 vs. 12% in 2013) for the overall population. However, the magnitude of increase differs in different age groups and ethnic groups. More statistically significant findings show greater utilization rates among beneficiaries of 19-64 than 65-74 and 75+ years old. Also, more significant findings show lower utilization among Black than White, Hispanic or Asian beneficiaries. The partial restoration significantly reduced the dental related ER visits among Medi-Cal beneficiaries from 2015 to 2017. However, the reduction is largely seen in beneficiaries of 19-64 years old in the ethnic groups of White and Black with reductions of 20 and 15 visits per 1,000 enrollees respectively in 2017 comparing to 2013. The dental related ER visits were lower for Hispanics and Asians, and remained very low among those 65 years old and above. In addition, the partial restoration resulted in increases in participation of dental care providers in the Medi-Cal program.
