Abstract:
This study examines aspects of how dental therapy began and developed, since it was introduced as one of the first public health occupations in New Zealand, in 1921. Dental therapy began as dental nursing, and was introduced by visionary dentists in order to treat widespread dental disease within the New Zealand population. The occupation gained international recognition. Dental nurses performed their tasks under the direct supervision and direction of a public health dentist and the occupation was restricted to females and child-patients. Investigating issues that have influenced the development of dental therapy is critical in today�s climate. Background research in the thesis reveals an interplay of issues relating to gender, professional development and measures of how successful the occupation has been in relation to oral health need. The latter is particularly questionable for our indigenous people in New Zealand. How has the role of the dental therapist in New Zealand been utilised? Has the delivery of care been based on sound knowledge and dental need? Has the role progressed in order to provide effective and appropriate care within a publicly funded system? These issues are important issues, particularly when New Zealand�s dental therapy profession becomes one of the many health professions that will be affected by the new Health Practitioners Competence Assurance Act when it is implented over the following year. In order to improve oral health for the population, it is vital that the dental therapist is appropriately and effectively utilised. How oral health workers perceive the past role and future role in achieving oral health gain is investigated in this study by using interviews and postal surveys, and the results are discussed. The findings show that the dental therapist has not always been utilised and developed using sound epidemiological evidence. Elements of professional protection by the dental profession coupled with depleted health funding rather than dental need have appeared as driving factors. Furthermore, the dental therapy profession has remained in a sub-ordinate role to the dental profession. The findings of this study show that a large number of the current dental therapy workforce do not feel that they are ready to provide dental care autonomously. Information in the thesis argues that past legislation and subordination to the dental profession has largely affected the development of dental therapy, and whether this has always assisted in improving oral health for the New Zealand population is questioned in this work. With a depleted number in the workforce, the role has become focussed on a reparative form of care, not one that promotes and improves oral health. This is not acceptable in a publicly funded system. Implementing changes to the dental therapy role must be undertaken, but undertaken with caution and based on progressive health-promoting ideology.

The purpose of this study was to determine whether the changes currently evolving in the delivery of dental care are affecting the way in which dental assistants are being utilized and, if so, whether the training offered in accredited dental assisting programs is meeting the needs and demands of the system. It was intended that the results of this study would be useful in determining future directions for dental assisting as a career, and in invoking changes in dental assisting education to improve satisfaction of both dentists and dental assistants. The research was conducted via survey questionnaires which were mailed to 300 randomly selected dentists in Grand Rapids, Michigan and Boston, Massachusetts. The survey items focused on the dentists’ backgrounds and attitudes toward dental assistant utilization; numbers and credentials of dental assistants employed; and current utilization patterns of dental assistants. The results were summarized with descriptive statistics, frequencies and distributions, and analyzed using chi-square and tests of correlation. It appeared that changes in dental education and in modes of delivery of dental services had little effect on patterns of dental assistant utilization and, moreover, that the emphasis of the A.D.A. accreditation standards for dental assisting education programs was congruent with the demands of the surveyed dental practitioners. It was further apparent that although dentists had positive attitudes toward dental assistant utilization, they appeared to be unaware of the status of state regulations governing utilization of dental auxiliaries, and generally underutilize the skills of trained assistants. This paper addresses these and other issues of concern to dental assistants, dental assisting educators and dentists. The results of this study provide a foundation for many areas of future research.

The oral flora is one of the most ecologically diverse microbial populiations known to man. It has been proved that any dental intervention: dental preparation, professional oral hygiene, extraction of teeth, regular prophylactic examination of patient‘s mouth is related with a possible spread of infection. Therefore, one of the most important tasks for the dental care professionals is to prevent the spread of infection and create safe environment for a patient, the dentist himself and other dental staff The aim of the present study is to identify and evaluate the attitudes of Lithuanian dental health care professionals towards infection control and carried out infection control procedures, and to determine vaccination frequency against hepatitis B among dental health care professionals. The study highlighteningt the dental health care professionals‘ attitude towards infection control and infection control procedures as well as incompliance of infection control procedures with the valid normative documentation was conducted at the Institute of Odontology, Faculty of Medicine, Vilnius University in 2008-2010. Burna yra viena labiausiai infekuotų žmogaus organizmo sričių. Įrodyta, kad bet kuri odontologinė intervencija – dantų preparavimas, profesionali burnos higiena, dantų šalinimas, įprastinė profilaktinė paciento burnos apžiūra – yra susijusi su galimu infekcijų plitimu. Todėl vienas iš svarbiausių odontologine praktika besiverčiančių asmenų uždavinių yra užkirsti kelią infekcijų plitimui ir sudaryti saugią aplinką pacientui, gydytojui odontologui ir kitam personalui. Abstract:
Foi realizada uma pesquisa com cirurgiões-dentistas (CDs) e auxiliares odontológicos (AOs) das redes pública e privada, no município de Ribeirão Preto,SP – Brasil, com os objetivos de: 1. avaliar a cobertura vacinal e a efetividade da vacinação contra a hepatite B; 2. verificar o uso de equipamentos de proteção individual (EPIs) na rotina de trabalho; 3. verificar a prevalência de marcadores sorológicos do HBV; 4. analisar possíveis associações entre marcadores sorológicos do HBV e alguns fatores de risco de infecção pelo vírus. A pesquisa contou com 338 participantes, de consultórios e clínicas odontológicas – 201 CDs(26 da rede pública e 175 da rede privada) e 137 AOs (23 da rede pública e 114 da rede privada). Foi aplicado um questionário individual para obter informações referentes a características dos participantes, vacinação contra a hepatite B e fatores de risco relacionados à infecção pelo HBV. Simultaneamente, coletou-se uma amostra de 8 ml de sangue para a pesquisa dos seguintes marcadores sorológicos do HBV: HBsAg, anti-HBc e anti-HBs. Todos os exames foram feitos, através de técnicas imunoenzimáticas, no Laboratório de Sorologia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto - USP. Estavam vacinados com três doses ou mais da vacina contra a hepatite B 80,6% dos CDs (80,8% da rede pública e 80,6% da rede privada) e 24,8% dos AOS(82,6% da rede pública e 13,2% da rede privada). A prevalência de anti-HBs entre os vacinados com três doses ou mais da vacina foi de 84,3% - 84,6% entre CDs e 82,8% entre AOs. A prevalência dos marcadores sorológicos do HBV ficou assim distribuída: HBsAg, 0,5% entre CDs, não encontrado entre AOs; anti-HBc, 9,5% entre CDs (IC 95%: 5,4% - 13,5%) e 2,9% entre AOs (IC 95%: 0,1% - 5,7%);anti-HBs, 78,1% entre CDs (IC 95%: 72,3% - 83,8%) e 29,9% entre AOs (IC 95%: 22,2 - 37,6%). Os EPIs mais utilizados pelos CDs eram as luvas, com percentual de 98,0%, e a máscara, com 96,5%, no atendimento de todos os pacientes. Os CDs da rede privada utilizavam os seguintes EPIs: luvas, 97,7%; máscara, 96,0%; óculos de proteção, 70,3%; avental, 55,4%; gorro 32,6%. Os CDs da rede pública usavam os seguintes EPIs: luvas e máscaras, 100,0%; avental, 88,5%; óculos de proteção, 65,4%; gorro, 30,8%. O EPI mais utilizado pelos AOs eram as luvas, com percentual de 97,1%. Os AOs da rede pública utilizavam: luvas, 100,0%; avental, 95,7%; máscara, 78,3%; óculos de proteção; 34,8%; gorro, 17,4%. Os AOs da rede privada utilizavam: luvas, 96,5%; máscara, 37,7%; avental, 28,1%; óculos de proteção, 11,4%; gorro, 10,5%. Dos AOs da rede privada, 3,5% referiram não usar nenhum EPI. O estudo mostrou que a prevalência dos marcadores sorológicos do HBV entre CDs e AOs é semelhante à da população de pacientes de Unidades de Saúde de Ribeirão Preto e que parte dos profissionais, especialmente os AOs, negligenciam a proteção pela imunização ativa e uso de EPIs.
A survey was carried out among dentists (D) and dental assistants (DA) of the public and the private sectors in the city of Ribeirão Preto, São Paulo, Brazil, in order to a) evaluate the vaccine coverage as well as the effectiveness of the vaccination against hepatitis B; b) check for the presence of serological markers of the hepatitis B virus(HBV)infection; assess the use of individual protection equipments (IPE). The survey,involving 338 participants working in dentist private offices or in dental clinics, comprised 201 D (26 of the public sector and 175 of the private sector) and 137 DA(23 of the public sector and 114 of private practices). Individual survey questionnaires were used to gather information regarding the participants, the vaccination against hepatitis B, and the risk factors related to the HBV infection. Blood samples (8 ml) were collected to assess the following serological markers for the HBV infections: HBsAg, Anti-HBc, and Anti-HBs. All essays were done with the immunoenzimatic technique by the Serological Laboratory of the Hospital das Clinicas of the University of São Paulo Medical School, at Ribeirão Preto. A higher proportion of the D compared to the DA were vaccinated with three or more doses of the hepatitis B: 80.6% of the D (80.8% of the public sector and 80.6% of the private sector) but only 24,8% of the DA (82.6% of the public sector and 13,2% of the private sector). Among those vaccinated with three or more doses 84.3% (84.6% for D and 82.8% for DA) presented positive tests for the anti-HBs marker. Considering the whole population evaluated, the occurrence of serological markers for HBV infection was distributed as follows: HBsAg, 0.5% in D and zero in DA; anti-HBc, 9.5% in D (95% CI: 5.4% to 13.5%) and 2.9% in DA (95% CI: 0.1% to 5.7%); and anti-HBs, 78.1% in D (95% CI: 72.3% to 83.8%) and 29.9% in DA (95% CI: 22.2% to 37.6%). Most dentists make use of some IPE when treating patients: 98.0% wore gloves and 96.5% face masks. There was little difference in use of IPE between dentists working in the private and the public sectors: for the former 97.7% wore gloves, 96.0% face masks, 70.3% protective eyewear, 55.4% protective uniform, and 32.6% caps; for the later, 100% wore gloves and face masks, 88.5% protective uniform, 65.4% protective eyewear, and 30.8% caps. A high percentage of the DA also wore IPEs, although the numbers were lower for those of the private sector. Among the former group 100% wore gloves, 95.7% protective uniform, 78.3% face masks, 34.8% protective eyewear, and 17.4% caps. The corresponding figures for the DA of the private dental offices and clinics were: 96.5% wore gloves, 37.7% face masks, 28.1% protective uniform, 11.4% protective eyewear, and 10.5% caps. A small percentage (3.5%) of the DA working in the private sector did not report the use of any IPE. The study showed that the occurrence of serological markers for infection with HBV in dental professionals is similar to that of the population as a whole, and that part of the professionals, especially the DAs, neglect to secure protection by active immunization and use of IPEs.

At the present time in Australia (1992) there exists no undergraduate training for dental students in auxiliary utilisation. The result of this is that new dental graduates have no understanding of the concept of practicing sit-down four-handed dentistry in a comfortable and efficient manner. This compares with the United States of America (USA) where, since 1961, every dental student receives formal instruction in Dental Assistant Utilisation (DAU) and, since 1973, Training in Expanded Auxiliary Management. In this treatise the history and development of “four-handed dentistry” is presented along with a literature review of the fundamental principles of DAU. The intial pilot programmes and the subsequent federally funded national DAU programme in the USA are analysed. Four undergraduate DAU programmes from the USA and the current graduate continuing education DAU programme at Westmead Hospital Dental Clinical School are examined. Finally, guidelines for the establishment and operation of a DAU programme are outlined. The aim of this treatise therefore, is to provide the necessary information for a thorough understanding of the fundamentals and philosophy of Dental Assistant Utilisation. This information is presented in the hope that at some stage in the future training in assistant utilisation would become an integral and important part of the undergraduate dental curriculum in Australia. The purpose of this would be to fill a major void that currently exists in the undergraduate training of dental students in auxiliary utilisation.

Resumo: Os desejos e expectativas dos pacientes devem ser mais explorados pelos dentistas na consulta odontológica, visando tratamentos mais precisos e eficazes. Assim, o estado de saúde integral será beneficiado, pois depende da saúde bucal. Entretanto, as oportunidades de tratamento não são iguais para todos, o que aumenta a procura por atendimento odontológico gratuito. O objetivo desta pesquisa foi identificar os motivos, os anseios, as expectativas e dificuldades, dos pacientes atendidos na Faculdade de Odontologia de Araçatuba, que freqüentaram as salas de espera da Clínica Integrada, no período de março a junho de 2005. Uma pesquisa de pré-abordagem, pois procurou focar os sentimentos do paciente e não a avaliação dele em relação aos procedimentos clínicos, em fases pós ou trans- operatórias. A metodologia utilizada nesta pesquisa de representação social foi quali - quantitativa: Discurso do Sujeito Coletivo, técnica criada para fazer uma coletividade falar, como se fosse um só indivíduo. Abstract: For more specific and successful treatments, patients' longings and expectations should be more explored by the dentists during the deontological assistance. So the general health condition would profit, once it depends on the oral health. However the opportunities of getting a treatment are not the same for everybody, which increases the search for free deontological assistance. The objective of this research was to identify the reasons, the longings, the expectations and the patients' difficulties assisted at School of Dentistry of Araçatuba, who visited the waiting rooms of the Integrated Clinic from March to June 2005. It was a pre research, once it tried to focus the patients' feelings and not their evaluation towards the clinical procedures, in the post or trans chirurgical phases. The methodology used at this social representative research was quali-quantitative: Speeches of the Collective Subject, which is a technique developed to make the collectivity speak as a unique individual. All patients assisted on the first semester of 2005 at the Integrated Clinic of the School of Dentistry of Araçatuba - FOA - UNESP, totalizing 105 patients, were interviewed with the questions: How have you known about the assistance at FOA? Are you used to go to the dentist? How often? Why? Do you have any problems on your teeth? That's why you came here for treatment? Which is this problem? The elderly population in Sweden is growing and as a result of that, society is facing different types of challenges. Oral hygiene is important for preventing oral disease but also for the general health (GH). A goal for the Swedish dental society is that every inhabitant should have an equal right to dental-care and oral health (OH) under the same conditions. This is not only a challenge for dental staff but also for medical and healthcare personnel. Our hypothesis is that nursing staff do not possess the education needed regarding oral hygiene, especially not in relation to GH. The opinions regarding support for nursing staff differ between nursing staff and managers. This study was conducted through questionnaire interviews with managers and nursing staff at geriatric homes in three municipalities in the county of Västerbotten. Questions were asked about education and routines, performance of oral care, and OH linked to GH. Response frequencies were compared between municipalities, professions, and response options. A significant lower part (p <0.001) of nursing staff found that non-existing oral care could cause suffering in the form of general disease than in the form of oral disease. The opinion differs between managers and staff in questions about established routines (p <0.05). This study indicates that knowledge about relationships between oral and general health might be inadequate and that education is both wanted and needed among nursing staff. More extensive studies, and studies about how dental professions could support nursing staff in a better way is desirable.

Os desejos e expectativas dos pacientes devem ser mais explorados pelos dentistas na consulta odontológica, visando tratamentos mais precisos e eficazes. Assim, o estado de saúde integral será beneficiado, pois depende da saúde bucal. Entretanto, as oportunidades de tratamento não são iguais para todos, o que aumenta a procura por atendimento odontológico gratuito. O objetivo desta pesquisa foi identificar os motivos, os anseios, as expectativas e dificuldades, dos pacientes atendidos na Faculdade de Odontologia de Araçatuba, que freqüentaram as salas de espera da Clínica Integrada, no período de março a junho de 2005. A organização do processo de trabalho em Saúde Bucal (SB) tem como componente principal a equipe composta pelo Cirurgião-Dentista - CD, Técnico em Saúde Bucal - TSB e Auxiliar em Saúde Bucal ASB. O trabalho em equipe, com a delegação de tarefas para o pessoal auxiliar (ASB e TSB) busca uma maior racionalidade, aumento da qualidade, da resolutividade e cobertura da atenção em saúde. O setor público tem sido o grande responsável, atualmente, pela inserção da equipe de Saúde Bucal nos serviços. O objetivo desta pesquisa, de cunho qualitativo, foi conhecer a percepção dos dentistas quanto à inserção e trabalho do TSB na equipe. O campo de pesquisa foi o município de Embu e a técnica utilizada para a coleta de dados foi o grupo focal sendo os participantes, dentistas da rede pública deste município. Os resultados apontam, segundo os entrevistados, para a importância do TSB no trabalho em equipe, principalmente, no serviço público, onde há uma grande demanda de assistência. Os participantes, ainda, ressaltam que esta experiência não lhes foi apresentada em sua formação acadêmica, vindo a conhecê-la somente no serviço público. A importância de uma formação técnica adequada e dúvidas acerca da regulamentação profissional, também surgiram como destaques no grupo focal. A partir desses resultados, conclui-se que uma melhor ordenação do processo de trabalho em equipe de Saúde Bucal é necessária, além de um aperfeiçoamento na formação dos profissionais da equipe, vinculadas às reais necessidades do SUS. Ainda que avanços tenham ocorrido nos últimos anos com a atual política nacional de saúde bucal, incentivando o trabalho em equipe, este processo requer um maior estímulo por parte de todos os envolvidos, localmente, no planejamento das ações.
The main core of the organization of the work process in Oral Health consists in the team composition which is the Dentist, Dental Therapist (DT) and Dental Assistant (DA). Teamwork with the delegation of tasks to auxiliary personnel seeks a more rational system of assistance, an increase of quality, resolution and coverage of care. Actually, the Public Health System has been greatly responsible for the integration of DT in the oral health team. The objective of this qualitative research was to acknowledge the dentist´s perception about the integration of the DT in the oral health team. The field research was at the city of Embu, State of Sao Paulo, Brazil, and the technique used for data collection was focus group with dentists working in the public health system of this city as participants. The results indicate, according to participants, the importance of the role played by the DT in the oral health team, especially in public service, where there is great demand for assistance besides lessening dentists stress. Respondents also emphasize that this practice based on teamwork was not discussed during their undergraduation and they experienced this only after beginning the public service assistance. The importance of an adequate technical training as well as doubts about the professional regulation also emerged in the focus groups. From the results it is concluded that training of managers for the better ordering of the process of oral health teamwork is required in addition to an improvement in training of dental therapists, linked to the real needs of the Brazilian Public Health System. Although advances have occurred in recent years with the current national policy on oral health, encouraging teamwork, this process requires a greater stimulus.

O avanço técnico-científico das últimas décadas fez com que as Ciências da Saúde, incluindo a Odontologia, se desenvolvessem rapidamente, em prol da saúde do paciente. Por outro lado, o progresso vertiginoso fez com que os profissionais da área da saúde voltassem sua atenção a este desenvolvimento, despersonalizando a relação com o paciente. O ser humano necessita ser olhado e visto na sua totalidade, atitudes que devem ser desenvolvidas pelos profissionais que lidam com pessoas, principalmente pessoas vulnerabilizadas pela dor e pelo sofrimento, como é o caso da Odontologia. Neste sentido, o presente trabalho avaliou, através da técnica hermenêutica, o Código de Ética Odontológica vigente, ou seja, a Resolução 42/2003, em relação à promoção da humanização das relações assistenciais. A interpretação dessa normativa permitiu concluir que o documento que deu origem ao referido código, elaborado na III CONEO preocupou-se com o zelo à saúde do paciente e com o respeito à dignidade do mesmo, preceitos fundamentais da humanização da assistência. E ainda que o Código poderia ser menos repetitivo em algumas questões e ter um objetivo mais definido com ênfase ao conceito de pessoa humana.
The technical and scientific progress made over the last decades promoted a rapid development of Health Sciences, including Odontology for the benefit of the patients’ health. On the other hand, this tremendous progress drew the attention of health professionals to the developments made and turned their relationship with their patients more impersonal. People need to be looked upon and seen as a whole, and so professionals must develop their interpersonal skills so as to behave accordingly, especially when faced with people more vulnerable by their pains and sufferings, as it is very often the case in the field of Odontology. For that purpose, this work used a hermeneutic method to assess the Code of Dental Ethics currently in force - i.e., the Resolution 42/2003 - concerning the promotion of more humane assistance care services. The interpretation of this norm enabled to conclude that the draft of the code in reference, which was elaborated at the 3 rd Brazilian National Conference on Dental Ethics [“III CONEO”] was concerned with the care given to the patients’ health and with the respect held for their dignity, both of which are fundamental precepts for the humanization of assistance care services. Finally, the work concluded that the Code would, nevertheless, gain in being less repetitive on some issues and in having a more defined purpose, while emphasizing on the concept of the human being.

The purpose of this study was to determine the extent to which academically successful first-generation college students, compared to academically successful non-first-generation college students, used academic support programs provided by UNT and to measure their perception of the benefits of these programs. Differences were examined using information gathered from a Graduate Student Survey administered to students graduating in fall 2006 from the University of North Texas. Analysis of the data from the survey indicated that there was no statistical significance between the use and perception of benefit of academic support programs between the two groups. Overall, students that used academic support programs provided by the university believed they benefited from the programs they utilized. Both groups indicated that they believed the Math Lab provided the most benefit. The Graduating Student Survey also examined input, environment and output factors of academically successful first-generation and academically successful non-first-generation students. Again, both groups indicated similar responses to the questions asked. First-generation college students in this study were successful in graduating from the University of North Texas and utilized some of the resources provided by the university to do so.

L'objectif de cette recherche a été de montrer comment l'impensé institutionnel de la maltraitance dérive d'une modalité spécifique du pacte dénégatif nommée pacte de désaveu, mettant en échec la déontologie et précipitant ainsi les sujets de l'institution dans une mise en acte répétitive du phénomène.Notre thèse s'articula autour de deux axes majeurs : un axe central autour du désaveu et un axe transversal centré sur la répétition. Celle-ci envisagée comme le corrélat du désaveu. L'examen de l'action de cet opérateur, à travers l'inquiétante étrangeté et la construction métaphoro-métonymique du Double, a permis de démontrer l'existence d'un pacte de désaveu, et l'enjeu de son utilisation lorsque son objet est la maltraitance dans une institution gériatrique : dissimuler l'existence de la maltraitance ; écarter la responsabilité de l'acte ; couvrir la participation des soignants à la maltraitance institutionnelle.L'existence d'un double désaveu a pu être mise en évidence : un désaveu de la parole du résident lié par circularité à un désaveu de l'acte de maltraitance. Ces deux désaveux se sont révélés comme secondaires, dans le sens de témoins d'un désaveu originel : le désaveu de la loi. Ce désaveu originel se basant sur une réalité fondamentale : la perte d'autonomie pouvant aller jusqu'à la grande dépendance.Ce premier axe de notre thèse a conduit nécessairement à prendre en compte un champ supplémentaire : la déontologie professionnelle en évaluant les effets et conséquences de ce désaveu dont nous avons fait mention. L'examen de la déontologie a conduit à formuler l'hypothèse d'une mise en échec de celle-ci, corrélée à cette opération de désaveu. Le second axe de recherche, transversal quand à notre problématique et dont le phénomène de répétition de l'acte de maltraitance constitue le corrélat, a permis de mettre en évidence avec quelle force contraignante, l'institution précipite ses sujets dans la répétition sous l'action du désaveu qui, en entravant la construction du souvenir, entraîne une impossible mise au passé de la maltraitance.

The impact of AIDS and the dread of acquiring HIV infection from patients have led to the resurgence in infection-control practices among health care workers. Recent reports of blood-borne pathogen transmission in health care settings, including oral health, have caused considerable public health concern. Transmission has been reported from patient to patient, patient to health care workers, but rarely from health care worker to patient. The risks of dental clinicians acquiring serious infections have been well documented but the risk to dental assistants has received less attention, especially in South Africa. Aim: To assess infection-control practices of dental assistants and their level of adherence to universal precautions in public health care facilities in Limpopo Province. Objectives: To establish the prevalence and the type of occupational exposures among dental assistants working in public health care facilities in Limpopo Province. Methods: A cross-sectional survey was conducted among dental assistants in Limpopo Province in 2005. The study population comprised all 73 employees who performed the functions of a dental assistant in public dental facilities. A self-administered questionnaire was used to collect information regarding work experiences and training, infection-control practice and knowledge, and the nature, incidences and reporting of any occupational exposures they had experienced. A follow-up telephone call was made to these dental assistants, after they had received the questionnaire, to re-iterate the importance of the survey and to request them to complete and return the questionnaire in the prepaid envelope they had been given. The facilities were clustered according to the six districts in Limpopo Province. Ethical approval was given by the University of the Witwatersrand and the Department of Health and Welfare in Limpopo Province. Results: Fifty-nine dental assistants returned the completed questionnaire, giving a response rate of 80.8%. Epi Info Version 3.3.2 programme was used to analyze the data. The majority of respondents were female (95%), with a mean age of 40.2 years (age range 23-54). More than 90% of the respondents had no formal training for their occupation, half (49.1%) did not have any health training, 22% were auxiliary nurses, 18.6% were “correspondence-trained” assistants who had been trained via distance learning and had no practical clinical training and only 10.2% of the respondents had received training at a technikon or university . The majority of the dental facilities (57.6%) had one dental assistant working alone, followed by those with two or three assistants (39.5%). The number of respondents assisting more than two oral clinicians in a day was 93.3%. The mean number of clinicians assisted per day was 3.8. The total numbers of dental assistants who experienced occupational exposures while working at the various dental clinics were 26 (44.1%), with 11.5% experiencing multiple injuries within the preceding six months. Auxiliary nurses and trained assistants were significantly more likely than untrained assistants to be aware of universal precautions, their protective effects, needle stick protocols, and of the need for personal protective equipments to be worn for all procedures (p=0.001). Compliance with infection-control practices was low overall. More than twothirds of the assistants routinely wore gloves during procedures. The lowest compliance reported was the use of protective eye shields, whilst more than 62.7% were not vaccinated against hepatitis B virus. More than two-thirds of the assistants were injured in the process of removing and or cleaning instruments; 65.3% of the injuries were direct punctures. Twenty-three percent did not report the injury. The risk of injury for the untrained assistants was 9.9 times higher than that for auxiliary nurses, p=0.008. A small percentage (23.8%) of those with sharp injuries was placed on antiretroviral drugs. Surprisingly, a significant high percentage of respondents were given wound cleaning only as treatment of their occupational exposures (78.4%) and sharp injuries (83%). Conclusion and recommendation More than 90% of the respondents had no formal training for their occupation. Dental assistants were understaffed and had increased workload. The greatest incidence of injury was associated with the handling of sharp objects, and this included recapping used needles. Occupational exposures to infectious material were found to be relatively high whilst compliance to some basic infection-control guidelines was low among dental assistants. The training of dental assistants should be regulated. More suitably qualified dental assistants should be appointed and existing ones should be given inservice training on the importance of infection-control practices and compliance with universal precautions.

The public’s demands for medication have increased with the changing of society; consequently, questions surrounding the quality of medical services have come into sharp focus. Recently, a shortage of nursing staffs in Taiwan has led to high turnover intention in medical fields, especially in dentistry. Dental assistants have an important role in the provision of dental services: necessary for the clinical operations and diagnostic quality; yet, without official supplies, acknowledged systems and proper training programs, the current strain on human resources and medical properties will be further exacerbated. An uneven distribution in the quality of clinical operations and dental assistants will be more widespread, affecting the overall quality of medical treatment. Thus, knowing the reasons for the turnover inclination amongst dental assistants, and finding out the factors for the high resignation rate have become a major subject for the operators in dentistry. Our research aims to discuss the relative factors for the high turnover intention amongst dental assistants, by using a range of criteria: job satisfaction, working pressure and the misery index as explanatory variables. Through data from questionnaires, we find that job satisfaction has negative impact, while working pressure and misery index have positive impacts on turnover inclination. In conclusion, our research will help to realize the working status quo of dental assistants and be regarded as a reference for clinic operators to rectify the current problems about the nursing shortage, reduce the resignation rate and improve the quality of medication.

The dental assistants had already taken an important role in the dental clinic since several years ago because of the four-handed dentistry. In America, there are lots of training courses for dental assistants. However, the education and formal trainings are very insufficient in Taiwan. Furthermore, the dental assistants maybe engaged as many different kinds of occupations before the dental clinics employ them. So there could be some troubles for dentists to communicate with new assistants. In order to solve these problems, we have developed an interactive multimedia e-learning system for dental clinic assistants. They can learn the basic knowledge from the system such as the Chinese and English names of the instruments, the pronunciation of English name of the instruments, the appearance of the instruments, and the brief introduction of the instruments. After a short time training, the new dental clinic assistants will have the ability to communicate with the dentists very easily.

But there are no stipulations or clauses related to dental hygienists in Taiwan now. It is an urgent issue about how to raise the quality of dental medical service by utilizing and controlling the manpower efficiently. In addition there is insufficient literature mentioning the influence of dental efficiency during medical treatment in the dental assistants’ auxiliary intervening act. Consequently, we considered that it is necessary to investigate and analyze the clinical actual work of dental assistants. Purpose: The purpose of this study was to evaluate the related impact factors, including the duration, quality and economical efficiency of dental medical work influenced by the dental assistants’ auxiliary intervening act in dental treatment procedures, and we hope to provide information and suggestions to the related authorities. Methods: This investigation was utilized by a questionnaire completed by dentists and dental assistants during the period from November, 2005 to August 2006 in a teaching hospital and a periodontal clinic. The questionnaire content included:patient''s fundamental data record、treatment items、periodontol status and its severity、their treatment time from the start to the finish in which assistants had worked。The study included 526 questionnaires and one hundred and eighty-four questionnaires were collected sampling from the teaching hospital and the periodontal clinics。Purposive database was designed using Microsoft Office Excel 2003 and the data were analyzed using JMP(p<0.05 was statistically significant). Results: In this study,males occupied 60.08% of the hospital samples,females occupied 60.33% of the periodontal clinic samples. The majority of patients were between 41-50 years old(40.68%), and the lowest proportion was in patients below 40(14.26%) and over 61 years old(19.39%). From the survey of periodontal treatment items, the order from highest to lowest proportion was full-mouth scaling(24.72%), hand scaling and root planing(20.91%), and oral hygiene instruction(18.25%). Based on the classification of work experience of the dentists, the attending dentists spent less treatment time, whereas the residents spent more time. The relationship between the time of oral hygiene instruction and the qualification of the dentists revealed no statistical difference (p= 0.1318). Less time was used during charting (21.15±7.47mins)with the aid of the dental assistants, and there was a statistically significant difference in comparison to treatment without the dental assistants’ aid (p=0.0011). Less time was used during recording PI (4.19±1.70mins)with the aid of the dental assistants, and there was a statistically significant difference in comparison to treatment without the dental assistants’ aid (p= 0.0003). From the survey of periodontal disease severity, less time was used in patients with mild periodontitis, and more time was used in patients with severe periodontitis. From the aspect of amount of dental calculus, the treatment time in the teaching hospital was less than that in the clinic. More time was used during full-mouth scaling in the teaching hospital(11.30±4.72mins) compared with less time used in the dental clinic(10.79±2.69mins) when aided by an assistant (1 person). Conclusion and Suggestions: Less time was used for the periodontal therapy with the aid of a dental assistant. More time was used during full-mouth scaling in the teaching hospital compared with less time used in the dental clinic when aided by an assistant. There are few dental assistants working in the teaching hospital, and there is more than one dental assistant working in the periodontal clinics. This may be caused by the differences existing in the environment and operation and the difference between the hospital and periodontal clinics. Moreover, in order to promote the operation efficiency ,treatment quality and tactical analysis,the imbalance of the number between the dental hygienists 、assistants and dentists should be discussed and managed. This study aims to explore the human resource management of dental assistants in Taipei City medical centers and its related issues. Through in-depth interviews and comparative research, this study is conducted by interviews with supervisors in dentistry departments of A, B, C, D medical centers of different properties. The content of the interviews includes working conditions, the practice of human resource management and the future development of dental assistants and its striving direction.   With the literature research, the in-depth interviews and the comparative analysis, this study finds: the main source of the dental assistants in those medical centers comes from nurses; diploma of Oral Hygiene department is not required when recruiting; job titles are not unified; dental assistants who work in dentistry departments are managed by different departments; the human resource can not be flexibly utilized for the lack of job rotation; the allocation of human resource is below the standard. The practice of human resource management in the observed medical centers differs for the varied properties, especially in the education, the salary, the achievement and the assessment. Consequently, this study recommends: I.To include “diploma of Oral Hygiene department” as recruiting condition II.Before the implement of certification system, licensed nurses are the best source of dental assistants. III.To cultivate dental assistants’ whole conception of dentistry departments IV.To unify the human resource management of dental assistants V.To define the title of dental assistants VI.Hospitals work with communities to implement oral hygiene education by dental hygienists, being able to effectively control medical expenditures under global budget system. Abstract Background:There has been significant change in the oral health-care system in Taiwan, including Oral Health Act practice, the Department of Health of the Executive Yuan established a Committee on Dental Medicine, and the set-up of the dental hygiene schools. It’s reveal that the style of oral care is stepping in a form of dental teamwork. Therefore, informational databases of dental auxiliaries are needed to build up the related law and the educational system. In addition, there are no investigative studies that deal with work stress, job satisfaction and burnout among dental assistants. It is necessary to collect and analyze such data related to the work of dental assistants. Study object:The purpose of this study is to investigate the distribution, work content, the extent of work stress, job satisfaction and burnout of assistants, and related factors in order to provide a reference and suggestions. Methods:The survey was performed by mailing questionnaires to dental assistants in Kaohsiung, Pingtung and Penghu areas during Oct. 2003 to Feb. 2004. These dental assistants were contacted by phone prior to getting the survey. Results:From all the subjects of the 890 dental offices, a total of 1778 questionnaires were distributed and 710 were returned. The response rate was 39.93%. There were 1.44 dental assistants in average worked for one dentist. There were fourteen percent dentists in the survey that didn’t hire any assistants, and twenty-four percent only hired one assistant. A large amount of work included items such as:taking impressions, gingival retractions, application over ulcerations, wound dressing, removing sutures, temporary prosthesis fabrication, pit and fissure sealants, re-cementing crowns, applying fluoride, placing or removing rubber dams and orthodontic bands, taking X-rays, removing calculus, removing temporary restorations, and temporary fillings, which were also the duty of dental hygienists in other countries who were certified and trained under the law in their countries. The factors significantly associated with higher work stress by stepwise regression were the higher education, dental league jobs, the unclear and undefined work duties, taken in-service study, work-hours differentiation per week, the unavailability of dental related magazines, the association with part-time dentists, difference in age group, and full-time staff sector differentiation. The factors significantly associated with higher job satisfaction were the shorter period of study, salary differentiation, the on-job education, well defined and clear work duties, and the habit of reading dental related magazines. The factors significantly associated with higher burnout were unclear and undefined work duties, the unavailability of dental related magazines, younger age group, dental league jobs, salary differentiation, and more work-hours per week. The linear regression analysis showed that higher work stress leaded to higher levels of burnout and higher satisfaction leaded to lower levels of burnout. Conclusion:The ratio of dentist to assistant in this study is lower than America, Canada and Japan. Work stress and job satisfaction among dental assistants are slightly above average, and the burnout is slightly below average. This study reveals that the administration of the dental department, including arrangement of working schedules, holidays and working hours, a regulated salary, the conduct of continued education, well defined and clear work duties, the availability of dental related magazines etc. can significantly affect work stress, job satisfaction and burnout of dental assistants. Currently, the duties of dental assistants in Taiwan are similar to the dental hygienists in other countries, but there are no rules or policy to confirm the duties of dental assistants. It’s important for our government and dentists to pay attention and more serious on it.

There has been a call from society for both the provision of a quality education and for a quality “student” who can contribute to the economy and citizenry of South Africa. It therefore makes sense for any educational structure to develop and adopt, strategies and systems in order to meet these new calls, and to provide technologically relevant education that will meet the demands of modern society. This has led to a renewed need for consistency, conformity and quality within higher education. Concerns about students’ capabilities have been raised, and have led to questions about the accountability of higher education institutions. A quality management system framework may ensure that these higher education structures are able to deliver a quality education to their external and internal customers. This research investigated the current quality management systems used in dental assisting both locally and world-wide, and then developed a quality management system framework to suit South African conditions. An action research study, using data gathered from a purposive census sample, and data obtained by means of a mixed-method approach, was conducted in two phases. A preliminary study (Part A) was conducted amongst the four South African dental assisting education programme providers and deficiencies in the quality management systems of these programmes were identified. The preliminary work (Part B) in this study consisted of a comparative analysis of the quality management systems of a convenience sample of dental assisting education providers in twenty countries. Both good practices and deficiencies in practices were identified. A further review of the literature identified possible solutions to improve the quality of the dental assisting education programmes, and was used as the foundation for the development of the Dental Assisting Higher Education – Quality Management Framework (DAHE-QMF). The results of the preliminary study - the comparative analysis and the literature review - revealed that the development and implementation of a quality management system framework for dental assisting in South Africa could lead to an improvement of the standards of training, and thus the competence of the student. The framework will be presented to the four programme leaders at the four Universities of Technology that offer dental assisting education for their consideration of partial/total adaptation of the framework.

The longstanding debate over the safety of mercury in dentistry has latterly moved from scientific argument to public health dilemma. Mercury is a neurotoxin. Adverse psychological outcomes can result from exposure, so The Quicksilver Quest aimed to investigate mercury in dentistry from a qualitative, critical health psychology perspective, and a quantitative, neuropsychology assessment. The qualitative study used focus group methodology to explore micro-mercurialism linked to dental amalgam fillings. A random sample of people, who had been medically diagnosed with mercury poisoning, formed seven focus groups. The discussion of experiences, beliefs, and health was analysed for themes and issues. The main findings were that the participants were not a homogeneous group, as had been anticipated, but fell into categories differentiated by their symptoms, fiscal resources, and motivation. A placebo effect was rejected as an exclusive explanation for the positive health outcomes reported by those who had had amalgam removal and detoxification. The quantitative study investigated the long-term effects of occupational mercury exposure on a cohort of women in dentistry. The aim was to test the null hypothesis: that women who endured high occupational mercury exposure in the 1970s (43 participants), and matched controls (32 participants), would show no between-group differences on a general and reproductive health survey, and a nine-test neurobehavioural assessment. Results generally supported accepting the null hypothesis. Significant exceptions were current symptom experience, reproductive health, and two mood subscales. There was a suggestion of peripheral nerve damage in the exposed group. Overall, the general discussion systematically reviews tension points in the debate, in light of a proposed model of tolerance to mercury. This begins to explain how it might appear that mercury in dentistry is safe for dental personnel, as pro-amalgam debaters claim, yet unsafe for some dental patients, as anti-amalgam debaters claim. Further study is suggested for occupationally exposed women, on tremor, and to test the proposed tolerance to mercury model. Finally, as the debate has a political aspect, a recommendation is made for a shift in public health policy to dental amalgam being restricted to use only in an adult population.

In recent years, information technology and internet were rapid development. The information system has been widely used in enterprises. Many enterprises, including the medical industry, invest in considerable manpower to develop and maintain the system. The medical information system can not only improve the quality of care of the users but also integrate medical-related data. Therefore, operators of the medical institutions are quality and efficiency-oriented to enhance the competitiveness of the medical institution in the medical industry. In response to the continual amendment and adjustment of healthcare policies, the medical institutions improve the operating efficiency of the users via medical information system, thereby reducing the operating costs of the medical institutions, from which, we can see that the medical information system has become an indispensable medical device in the medical institution. Therefore, a set of evaluation modes to evaluate its efficiency and effectiveness are needed for measuring the quality and performance of the medical information system. The research is a qualitative research method. The objects of interview and case study in this paper were two doctors and three assistants from Z Dentist’s Office. The research was investigated from the aspects, such as users’ background, hospital information system, information system successful model, factors influencing the effectiveness of the information system, etc. and the satisfaction with the medical information system was probed into from the perspective of doctors and assistants. The analysis of actual benefits brought by the application of medical information system showed that the use of medical information system by the doctors and the assistants would considerably improve work efficiency, effectiveness, acceptance and satisfaction. In fact, “system quality”, “information quality” exerted positive impacts on “users’ satisfaction”. It was also found that the users holding different posts showed different attitudes towards the use of information system. However, they were inclined to hold a positive attitude towards the system. According to the research results, recommendations on the system arrangement and system quality have proposed by the researcher, expecting to provide a further reference for personnel engaging in developing medical information system in the future

OBJECTIVES: The aim of this study is to examine the association between food insecurity and untreated dental caries among U.S. children and adults and to investigate the effect of participating in the Supplemental Nutrition Assistance Program (SNAP) on dental caries among U.S. adults. METHODS: Our sample was derived from the National Health and Nutrition Examination Survey (NHANES) cycles 2011-2012 and 2013-2014. Food security was measured as overall food security status (full food secure/ food insecure) and household-level food security (full, marginal, low, and very low). SNAP participation in the past 12 months was self-reported (yes/ no). The main outcome variable was untreated dental caries (none vs. one or more). Descriptive, Bivariate and multiple logistic regression analyses were conducted to evaluate the relationship among food insecurity, SNAP participation, and untreated caries. RESULTS: Our results suggest that food insecure children were more likely to have untreated caries compared to full food secure counterparts, after controlling for potential confounders (OR: 1.38; 95% CI: 1.11-1.72). On household-level food security, children from marginal and very low food secure households had significantly higher odds of untreated caries compared to children from fully food secure households (OR: 1.48; 95% CI: 1.10-2.01 and OR: 1.59; 95% CI: 1.12-2.26 respectively). Moreover, our findings suggest significant association between overall food security status and dental caries among U.S. adults (OR: 1.35, 95% CI: 1.05-1.74). We also observed a significant association between the severity of household food insecurity and untreated caries among adults after adjusting for confounders (P=0.04). SNAP participants were more likely to have untreated caries compared to non-SNAP participants (OR: 1.5; 95% CI: 1.19-1.87). The interaction of food insecurity and SNAP participation was not significant. However, SNAP participants from all levels of food security had higher prevalence ratio of dental caries compared to non-SNAP participants, regardless of food security status. CONCLUSION: Food insecurity was negatively associated with dental caries among U.S. children and adults. In addition, SNAP participation had adverse association with dental caries among U.S. adults. Further research is needed to more comprehensively understand the impact of food insecurity and food assistance programs on oral health.

The ILO (International Labour Office, ILO) in 2014 noted that occupational stress is not just for special occasions or a personal problem, but a global trend of increasing significantly affect all occupations other staff, all workplaces and all countries. It pointed out that scientific evidence does lead to some environmental practice most people work pressure, especially the heavy workload of medical practice environment. Studies have shown that work stress affects not only the physical and mental health nurses, and with the willingness to remain in office, is closely related to quality of care and patient safety. The dental assistant personnel are often required to maintain a good working attitude and enthusiasm, the face of these work pressure caused by stress symptoms, which may affect the health and quality of care of individual employees and unwind. NLP (Neuro- Linguistic Programming, abbreviated NLP) formed in the 1970s to change the mentality of many individuals from the inner effective techniques can be used, and can effectively reduce the individual''s behavior problems. In this study, from the Department of Dental assistants and dental clinics operating characteristics perspective, Neuro-Linguistic Programming raised pressure relieving method of transition, in order to improve physical and mental health of employees, but also to promote harmonious labor relations, and thus create a healthy workplace. In this study, the dentist assistance for the study, a randomized controlled trial mining samples were divided into experimental and control groups. Before and after each test questionnaires sent 168 copies, after data processing to obtain 164 valid questionnaires. Females ratio is 97.6%, males ratio is 2.4%, the largest number in the age of "25 years of age" is (36.3%), followed by "26-30 years" (31.3%). Statistical analysis using SPSS22.0 statistical software, analytical methods including descriptive statistics, Chi-square test and Independent Sample t-Test. The research evidence shows Neuro-Linguistic Programming for assistance in "exposure to occupational hazards," "work-life conflict," on improving the "overall pressure" and "personal fatigue" and other aspects are helpful. Based on these findings, hoping to offer the dentist to understand consciousness and professional assistance for job satisfaction, as dentist-job education and training, task assignment and adjustment reference human resources, to assist the dentist managers monitor the quality of care and services improved framework targets. And by the early pressure source of management against sustained pressure accumulation of physical and mental health, dental assistant or even reduce the turnover rate; and provide other medical institutes and related management reference nurses, health care workers in the workplace to promote health and reduce health care practitioner health risks, and thus achieve the best quality of patient care.

Introdução: O Burnout é uma doença profissional de causa multifatorial, associada a profissões que executam tarefas complexas ou com contacto interpessoal direto e descrita através de três dimensões: exaustão emocional, despersonalização e baixa realização profissional. Em Portugal não foram identificados estudos sobre a prevalência e impacto da síndrome de Burnout em assistentes dentários. Materiais e Métodos: Recurso a um questionário com questões associadas ao Maslach Burnout Inventory (MBI) e questões sociodemográficas aplicadas junto de assistentes de consultórios dentários a exercer atividade em Portugal. Foi realizada análise estatística quantitativa com recurso ao SPSSv.27 para avaliação de significância estatística entre variáveis em estudo. O nível de significância estabelecido foi de p=0,05. Resultados: Dos 908 inqueridos, a prevalência de burnout encontrado entre os participantes foi de 98,4 %, dos quais 72,1% apresenta um nível moderado e 26,3% um nível elevado de Burnout. 44,3% apresentam um nível elevado de “Exaustão”, 19,4% um nível elevado de “Despersonalização” e 40,4% um nível elevado de falta de “Realização Profissional”. Conclusão: Os impactos na profissão de assistentes dentários em Portugal são significativos à semelhança do que já se havia estudado noutras profissões de saúde. É indispensável o estabelecimento de estratégias preventivas para atenuar estes impactos e também prevenir que a síndrome se desenvolva ou agrave, visando o bem-estar geral dos assistentes de medicina dentária e minimizando risco para a segurança do paciente ou impactos negativos nas organizações ou clínicas dentárias.
Introduction: Burnout is a professional disease with a multifactorial cause, associated with professions that perform complex tasks or with direct interpersonal contact, and is described through three dimensions: emotional exhaustion, depersonalization and low professional fulfillment. In Portugal, no studies were identified on the prevalence and impact of Burnout syndrome in dental assistants. Materials and Methods: Use of a questionnaire with questions associated with the Maslach Burnout Inventory (MBI) and sociodemographic characteristics applied to dental assistants working in Portugal. Quantitative statistical analysis was performed using SPSS v.27 to assess statistical significance between variables under study. The level of significance established was p=0.05. Results: Of the 908 respondents, the prevalence of burnout found was 98.4%, of which 72.1% had a moderate level and 26.3% had a high level of Burnout. 44.3% have a high level of “Exhaustion”, 19.4% a high level of “Depersonalization” and 40.4% a high level of lack of “Professional Fulfillment”. Conclusion: The impacts on the profession of dental assistants in Portugal are significant, similar to what had already been studied in other health professions. It is essential to establish preventive strategies to mitigate its impacts and prevent the syndrome from developing or worsening, aiming at the general well-being of dental assistants and minimizing risk to patient safety or negative impacts on dental organizations or clinics.
