Abstract:

La senilidad no es un fenómeno nuevo. El cambio es la conceptualización de este estado de pérdida cognitiva como situación morbosa. El deterioro senil aparece, desde hace cuatro décadas, como un concepto nuevo, presente en la mirada médica y, por extensión, en la mirada social. Pero hasta hace unas décadas, la senilidad no era objeto de estudio de la medicina. Actualmente, sin embargo, goza de una gestión técnica, farmacológica y médica y su lugar natural es la clínica. El objetivo de este estudio es descubrir qué «condiciones de posibilidad» se están dando en este momento histórico para que la categoría demencia senil se esté elevando al rango de patología; para que se esté reemplazando la categoría senilidad por la de demencia senil como nuevo concepto, y para que los ancianos seniles se conviertan en enfermos. En definitiva hemos examinado cómo se ha pasado de la senilidad a la construcción de la demencia senil y del anciano demente, explorando la construcción histórica de esta nueva subjetividad vinculada a la vejez. Se ha considerado que el deterioro senil, caracterizado en tanto que síndrome patológico, es una invención que resulta de la estructura axiológica de la sociedad. El esquema de valores que conforman el marco de interacción social en el que nos movemos dificultan un encaje no problemático de la vejez en general y de la demencia en particular. La hipótesis de partida ha sido que la medicalización de la demencia senil legitima la exclusión social de los ancianos.
The aim of the present study was to determine which possibility conditions are currently taking place to uplift the category of senile dementia to the realm pathologies; and to point out why the category senility is being replaced by senile dementia as a new concept, altogether allowing the conversion from elders to patients. Briefly, we have investigated the conceptual transition from senility to senile dementia, and how this term and also the term demented have both been historically constructed, yielding, as a result of this process, a new subjectivity linked to the old age.

Abstract:

In 1976 it was proposed that senile dementia, a potential affliction of old age, be redefined as Alzheimer's disease, a rare diagnosis previously assigned to presenile dementia occurring in middle life. In response to a "public culture" generated by those caring for the afflicted, together with leaders of the biomedical reserch community, substantial financing has been allocated by the U.S. Congress to the National Institutes of Health, for investigation of senile dementia redefined as a "dread disease". This has funded studies in the neurosciences, and a range of epidemiological and high technology diagnostic investigations for which psychiatry developed a "case-finding" method. The "cognitive paradigm" for dementia was conceived by American psychiatry within a now dominant "biological" model which imputes physical causation to mental disorders and stresses "objectivity" in diagnosis. This has legitimated the use of "mental test" instruments based upon, or validated against, "intelligence tests" developed by psychologists for quantification of "intelligence" now redefined as "cognition". In a study funded by the National Institute of Mental Health, three cognitive assessment instruments were administered to a sample of individuals aged 60-93 with a broad range of educational experience across the age spectrum. Education rather than age was found to be the most significant predictor of test results for each instrument, and when the tests were repeated a marked "learning effect" was detected among those with the least education and lowest baseline scores. However, the identification of low education as a predictor, albeit less powerful than age for "cognitive impairment" indicative of dementia in other investigations has now been interpreted as a "risk factor" rather than a confounding variable and now enters into genetic mental testing models. Negative stereotyping of "old age", strongly associated with images of "senility", and "burden of ageing" economic arguments have therefore been reinforced by the dissemination of prevalence estimates from epidemiological studies conducted in communities in which there is an inverse correlation between age and education. In the meantime, basic scientists have failed to discriminate precisely between neuropathological changes indicative of "disease" and those of "normal ageing" or to establish a functional link between such changes and dementia behaviour in vivo. In consequence the legitimating rationale for public financing of the "Alzheimer's enterprise", i.e. "clinical benefit" remains elusive.

Abstract:

Excretion of biopterin and the related pteridines neo-pterin and pterin was measured in urine samples from a group of 25 male and 51 female depressed outpatients receiving lithium therapy, and compared to 61 male and female control subjects. The ratio of neopterin to biopterin excreted (N/B) was significantly higher in the patients than controls (p< 0.01), and the significant (p< 0.01) positive correlation between urinary neopterin and biopterin shown by the controls was absent in the patients, indicating disrupted biosynthesis of BH4. Urinary cortisol excretion was similar in all groups, but creatinine excretion was diminished in the patients, suggesting a nephrogenic effect of lithium. Serum folate was shown to correlate with urinary biopterin in female unipolar patients. Two groups of elderly females with senile dementia of Alzheimer type (SDAT) were examined for urinary pteridine excretion. In group I (n= 10), N/B was significantly higher than in 24 controls (p< 0.05) and the ratio B/B+ N significantly lower, indicating diminished tetrahydrobiopterin biosynthesis. A second study on 30 patients and 24 age-matched controls confirmed these findings. However, as N correlated with B in both patients and controls, the alteration in BH4 metabolism in SDAT is possibly different to that shown in depression. Lithium had no effect in vivo or in vitro on wistar rat brain or liver BH4 biosynthesis or salvage enzyme dihydropteridine reductase at a range of concentrations and duration of dosing period. In general, no significant effects were shown by the tricyclic antidepressant imipramine, the anticonvulsant sodium valproate, the vitamin folic acid or the anticholinergic agent methylparatyrosine, indicating that BH_4 may differ in man and rat. The synthetic corticosteroid dexamethasone had no effect on rat brain metabolism, nor did stimulation of natural cortisol by stress-immobilisation, although hypothalamic-pituitary-adrenal axis disturbances are know in depression.

Abstract:

A sample of 22 persons who care for relatives exhibiting initial symptoms of senile dementia were administered paper-and- pencil questionnaires to determine their level of subjective burden and psychological symptomatology. Each participant's attributional style was measured on an internal-external dimension, and their causal attributions regarding their relative's symptomatic behaviors were assessed. Results indicated that attributional style did not predict specific attributions about illness-related behaviors, but the tendency to not blame an afflicted relative for their behavior was predictive of subjective burden and symptoms of depression and anxiety. Subjective burden was found to predict feelings of hostility in caregivers.
