Abstract:
The purpose of this thesis was to quantify the cost of care and treatment of pressure ulcers in Thomayer University Hospital with polyclinic (FTNsP) in Prague. Present pressure ulcer monitoring system was analyzed and a transformation of the records collected by this systém in the period 2005 - 2010 was performed in order to derive the necessary data - especially the number of patient-days, displayed by severity of the pressure ulcers. Data on expenditure of materials and pharmaceuticals was obtained by questionnaire survey, conducted in 2009 among the nursing staff. The description of the incidence of pressure ulcers in FTNsP, analysis of the effect of the use of anti-decubitus mattress on the incidence of pressure ulcers and bedsores costs per patient and for the entire hospital according to a single component of direct costs and costs of extended hospital stay are the results of the thesis.

Abstract:
Die Tiergerechtheit von Haltungssystemen für Milchkühe hat sich mit der Umstellung von Anbindeställen auf Boxenlaufställe seit den 1980er Jahren deutlich verbessert. Dennoch besteht weiterhin Handlungsbedarf und es werden verschiedene Systeme zur Beurteilung der Tiergerechtheit von Haltungssystemen entworfen. Die Basis dieser Bewertungssysteme können entweder indirekte Faktoren aus Stallbau und Management oder direkte Faktoren aus Tierverhalten und Habitus oder eine Kombination aus beiden sein. Ziel dieser Arbeit war es, den Verlauf der Entstehung von Sprunggelenksdekubitus zu beobachten und anhand der Ergebnisse zur Entwicklung eines sinnvollen Bonitursystems für die Sprunggelenks-gesundheit von Kühen beizutragen. Dieses Bonitursystem soll in Zukunft im Rahmen des aus direkten und indirekten Faktoren kombinierten Bewertungssystems „Cows and more – Was die Kühe uns sagen“ der Landwirtschaftskammer NRW Anwendung finden. Außerdem sollten Hinweise auf mögliche Ursachen für die Entstehung von Tarsalgelenks-dekubitus gefunden werden. Es wurden aus einem Bestand mit Deutschen Holsteins zwei Gruppen hochtragender Färsen nach voraussichtlichem Abkalbedatum ausgewählt und diese 53 Tiere wurden nach dem Abkalben über fünf Monate in einem zweiwöchigen Abstand bonitiert. Neben der Sprunggelenksgesundheit wurde auch der Body Condition Score (BCS) alle zwei Wochen erhoben, die Klauengesundheit wurde einmal beurteilt und bei 20 Tieren wurde über acht Wochen mittels spezieller Pedometer die Liegezeit gemessen. Bei allen Tieren wurde zu Beginn und Ende der Beobachtungszeit eine Blutprobe für ein metabolisches Übersichtsprofil genommen. Das Ergebnis dieser Studie war, dass vor allem an den seitlichen Sprunggelenksflächen Dekubitus im klassischen Sinne zu finden war. Die medialen Gelenksflächen waren so gut wie nie betroffen. Die dorsalen Fersenhöcker wiesen verletzungsbedingte Veränderungen auf, deren Ursache an anderer Stelle zu suchen ist. Als Ursache für den Dekubitus sind primär die Liegeflächen zu sehen, die aufgrund ihrer Härte, aber auch aufgrund der dünnen und wenig feuchtigkeitsbindenden Einstreu, zur Entstehung von Dekubitus beitragen. Im Rahmen von „Cows and more“ eignen sich vor allem die seitlichen Sprunggelenksflächen als Indikatoren zur Erfassung von Dekubitus-Problematiken. Der BCS könnte von Relevanz sein, die Streubreite der vorgefundenen BCS-Werte im untersuchten Bestand erlauben aber keine Aussage über die Korrelation. Andere Faktoren wie die Klauengesundheit und die Liegezeit spielen ebenfalls eine Rolle, ihre Bedeutung bedarf aber weiterer Abklärung. Insgesamt ist Dekubitus als ein multifaktorielles Geschehen zu betrachten; es kann am Ende der Beurteilung eines Bestandes durch „Cows and more“ nicht eine einzelne Antwort auf die Frage nach der Ursache für Dekubitus des Sprunggelenks stehen. Besonders wichtig ist auch die Beurteilung der Färsen und Rinder in den Jungviehställen, da das Problem „Dekubitus“ nicht erst im Milchviehstall beginnt.
The welfare of dairy cows has improved dramatically with the change from tie-stalls to free-stalls in the 1980s. Nevertheless there is still much need for improvement, therefore many different systems for the assessment of animal welfare are designed. Some of these use factors of management and building design, while others concentrate on the animal behaviour itself and the animals’ well-being and still others take management into account as well as animal based measurements. The main issue of this study was to record the development of tarsal joint lesions, i.e. skin injuries from broken hair to hair loss to ulceration as well as swelling of the tarsal joint or distal limb, and afterwards design a system to assess the tarsal joint health, which can be used as part of the “Cows and more” assessment tool of the Landwirtschaftskammer NRW. A further intention was to find out about possible risk factors or reasons for the development of decubitus at the tarsal joints in dairy cows. Therefore a group of 53 heifers, selected after estimated calving date, was visited every second week after calving for five months. Aside from the tarsal joint health, the body condition score (BCS) was assessed every two weeks as well, the claws were checked once and 20 animals wore riscounters measuring the lying time for a period of eight weeks. A blood sample was collected from every cow at the beginning and the end of the assessment period. The results of this study were that especially the lateral tarsal joint showed classical decubital lesions while the lesions of the tuber calcis seemed to derive from a different pathogenesis and the medial surfaces showed few to no lesions. The main cause for lesions on the lateral surfaces is primarily the lying surface, being too hard and bedded too thin with sawdust that has only a limited capacity to bind humidity. For evaluation tools like “Cows and more” especially the lateral surfaces of the tarsal joints seem sufficient to assess the tarsal joint health in regard to decubitus. The BCS may be of some relevance for the development of decubital sores, but within the limit of this study no verified statement can be given, for the variance of scores among the assessed group was not large enough. Other factors like lying time and claw health seem to be of some importance as well, their extent of influence on the development of decubitus at the tarsal joint needs further covering. There cannot be only one reason or answer as to the cause for decubitus of the tarsal joint, it has to be seen as a multifactorial event. It also seems to be of major importance to assess the heifers and young animals as well, since the problem does not suddenly commence after calving.

Introduction: Pressure ulcers are one of the most monitored indicators of nursing care. Thanks to a well-functioning interdisciplinary team, pressure ulcers can be effectively prevented. Main aims of the thesis: The aim of this work is to map the role of nurse in an interdisciplinary team, to describe how the interdisciplinary teams currently work and what the effectiveness of their work is in relation to the prevention of pressure ulcers and their healing. Methodology: A qualitative research survey was used to collect the data in the form of semi-structured interviews wtih nurses. The selection criterion was the fact that they encounter patients suffering from pressure sores. The informants were getting method Snow ball. The resulting set of informants consisted of 15 persons. The results of the interviews were processed using the open-coding technique of pencil and paper and then divided into six categories. Results of the thesis: Research shows that the nurse plays a key role in the interdisciplinary team in the care of a patient with pressure ulcers. Her role is to work with other team members, direct patient care, preventive work and family contact. The results show that the nurse is a full member of the team. The overall care effect of the interdisciplinary team is ineffective in some departments, because, as it happens that individual workers do not work together. In our research, we mainly come across the absence of a nutritional therapist. Also, nurses would welcome more cooperation from lower medical staff. Furthermore, nurses perceive the use of scales as a certain obligation, and in some cases report their incorrect evaluation. Last but not least, we are confronted with ignorance and non-compliance with the standards of pressure ulcers treatment. Conclusion: We would like to convey the results achieved by the necessity of cooperation of all workers who care for a patient with pressure ulcers. The provision of complex care is essential in this area, because pressure ulcers are multifactorial. The use of interdisciplinary teams in the care of a patient with pressure ulcers brings the possibility to use people with different expertise, knowledge and experience. Together, they effectively work towards the early recovery of the patient, shortening the hospitalization period and saving the cost of hospitalization.

The dissertation theme "Nursing care for patients with pressure ulcers in the JIP, ARO". Thesis entitled "Nursing care of patients with pressure ulcers in JIP, ARO, deals with the prevention and treatment of pressure ulcers at selected sites. The thesis is divided into theoretical and practical. In the theoretical part of the work focuses on basic knowledge of anatomy, physiology, skin and subcutaneous tissue, the definition of pressure sore formation mechanism, formation factors, risk assessment, pressure ulcers, pressure sores and classification of knowledge about prevention and treatments, from historical sources to present a modern treatment . The aim of this thesis was to identify and map the knowledge and practice nurses in the prevention and treatment of pressure sores. The practical part describes the processing of the results obtained from a questionnaire survey. The questionnaire was distributed to five health facilities in Prague - a total of 19 sites JIP resuscitation. Data collection was conducted between November 2010 - January 2011. Pressure ulcers are one of the quality indicators of nursing care, their presence reduces the quality of life of the patient, leads to a prolongation of hospitalization, increases costs of care and affects the budget of a health care facility. European Pressure Ulcer Advisory Panel or the European Advisory Board on issues of bedsores (EPUAP) leads and supports all European countries in their efforts to prevent and treat pressure sores. EPUAP working group has developed guidelines for the prevention and treatment of pressure ulcers. One of the important factors in providing the highest quality of nursing care is the ability of nurses to practically use the research results in the nursing process. This thesis deals with the effective use of the latest advances in the prevention of pressure sores and their treatment in the intensive care in Czech Republic. Nutritional interventions as a prevention and treatment of pressure ulcers in geriatric clients living in social services facilities Introduction: Pressure ulcers are chronic wounds caused by pressure and friction. This is localized area of cell damage caused by a microcirculation disorder. Prevention is essential especially for immobile clients who are easily creating and developing these chronic wounds. Clinical practice shows that client positioning, use of anti-decubital aids and good nutritional support is necessary. The quality of nursing care is becoming an increasingly important concept in contemporary nursing. The most effective means of achieving quality of nursing care and the means of proof is the accreditation of medical facilities. It is a significant appreciation of professionalism, the work of health professionals and an important signal for the patients. Quality of care, security of supply and preventing potential risks for health care priority. The main aim of the thesis "The influence of accreditation on quality of care in selected health facilities" was to map the set of quality management and impact monitoring and evaluation of quality indicators in hospitals accredited by the Czech Republic. To determine whether accreditation has become an instrument to ensure comparable quality, and time has changed from an unpopular instrument into an instrument for continuous quality improvement. The survey took place from November 2010 to March 2011 in the form of quantitative data collection using an anonymous questionnaire survey in selected hospitals accredited by the Czech Republic. In the research study were asked managers of nursing care, main, upper station and sisters. The thesis has been established 6 goals. The first objective was to determine what indicators of quality nursing care in accredited hospitals pursue. Interested in us as quality indicators that were studied before accreditation, and quality indicators to setting up medical facilities in preparation for accreditation. The second objective was determine how frequently watched indicator of quality is monitored in hospital. The third objective was to determine whether there was in accredited hospitals to reduce adverse events - falls. The fourth objective was to determine whether there was in accredited hospitals to reduce the incidence of nosocomial infections in complying with barrier nursing regime. The fifth objective was to identify the biggest problems arose during the implementation of audits, completeness and comprehensiveness of nursing documentation management into practice. " The sixth objective was to determine whether there was during the preparation of an increase in hospital nursing staff. The thesis then was examined six hypotheses. First hypothesis Accredited hospitals monitor the quality of care provided by at least ten quality indicators - were confirmed. Hypothesis 2 The most frequently monitored indicator of the quality of nursing care, the prevalence of pressure ulcers - was confirmed. All respondents said they monitor the prevalence of pressure ulcers as an indicator of quality nursing care. Hypothesis 3 gaining accreditation to reduce the incidence of falls - was confirmed. Hypothesis 4 obtaining accreditation decreased nosocomial infections - was confirmed. Hypothesis 5 The biggest problem in implementing the audit, management complexity and completeness of nursing documentation in practice "has been an increase in paperwork for nursing staff - were confirmed. Hypothesis 6 During the preparation for accreditation has been an increase in nursing staff - has not been confirmed. Accreditation is perceived by respondents as beneficial for the hospital. Due to accreditation standards and developing quality indicators were identified risk areas in the provision of hospital care and detail can be paid to the prevention of adverse events, nosocomial infections and other risks. Abstract:
The issue of pressure ulcers is related to limited mobility of P/Cs in bed, and is well known at present. With regard to the permanent increase of surviving P/Cs in critical conditions the prevention of pressure ulcers is a current nationwide topic. Nevertheless, statistical information admits the occurrence of pressure ulcers. The occurrence of pressure ulcers is a rapidly developing nursing problem. This problem has to be taken into account and its prevention has to start from the very beginning of the limited mobility of a P/C in bed.The theoretical part of the thesis is divided into three areas. The first one describes the quality of nursing care in general. The second area is directly focused on pressure ulcers. A chapter dealing particularly with the prevention of pressure ulcers at an ARD extends the last area of the theoretical part. Two goals were set for this thesis: 1. To map the roles of nurses (medical rescue workers) in the prevention of pressure ulcers at an ARD. 2. To map the factors affecting the occurrence of pressure ulcers at an ARD. Four research questions were formulated to achieve the goals: 1. What role does a nurse (a medical rescue worker) play in the prevention of pressure ulcers at an ARD? 2. What factors influence the occurrence of pressure ulcers at an ARD? 3. What information do nurses (medical rescue workers) working at an ARD miss? 4. What problems do nurses (medical rescue workers) working at an ARD have? The empiric part of the thesis was processed by a qualitative method in the forms of the semi-structured interview and involved observation. On the basis of the obtained data the research was extended by a chapter "The Occurrence of Pressure Ulcers", where the data obtained from the head nurses of the involved workplaces was processed and described to summarize the research. The elaboration and the processing of the research brought answers to all four research questions. The results of the research have revealed a number of roles that nurses play. The roles are: the provision of the prevention as such, detection of the first stage of a pressure ulcer, the care of a pressure ulcer already occurred and the prevention of complications. The internal and external factors influencing the occurrence of pressure ulcers are also describer here. The information on the issues of pressure ulcers prevention among nurses and medical rescue workers, the interest in the issues, where the results are surprising in the negative sense, where two thirds of the respondents showed disinterest. We have found that the respondents miss basic information like the classification of pressure ulcers, the information on modern prevention materials and on the healing and treatment of pressure ulcers. Problems in the prevention of pressure ulcers based on two sides were also found out, namely on the side of the management and on the side of the nursing care. On the basis of the circumstances ascertained within the research head nurses from the involved workplaces of the "X" region have been invited to participate in the organization of a seminar for nurses and medical rescue workers working at ARDs, focused on the topic: The issues of the prevention of pressure ulcers at an anaesthesiology and resuscitation department.

Abstract The diploma thesis is focused on the application of a national methodics for prevalence monitoring and comparison of pressure sores (decubitus ulcers) incidence in health care facilities in the Czech Republic. Pressure sores are a global problem and their incidence should be prevented by preventive measures and high-quality nursing care. However, despite all the provisions, we are not successful in the prevention of their incidence. It is necessary to define standardized quality indicators of the nursing care, to compare the results, and to share the information with specialists, which would result in continuous quality improvement (benchmarking), and to support good practice. In the world the quality indicators monitoring of health care is quite common, in the Czech Republic the process has been only developing. In the area of Czech nursing care only the falls in health care facilities are monitored on the national level. The methodics for the monitoring of pressure sore incidence has not been established yet on the national level. In 2003 the first comparison of four teaching hospitals was carried out in the incidence of pressure sores by means of the method of prevalence investigation. In 2005 the data collection methodics was elaborated during the solution of the grant project IGA MZ Č NO 77723-3. Six teaching hospitals were compared for the prevalence of pressure sores, with division into four specialty groups: internal medicine, surgical, intensive and follow-up care. In 2008 a project of quality and safety of the Ministry of Health was implemented, with a view to elaborate uniform methodics for prevalence investigation and comparison of a standard indicator in the area of pressure sores at the national level, regardless of the type of a health care facility. At the meeting of the Work Group for Health Care Quality of the Ministry of Health of the Czech Republic I recommended to include the indicator in the National Set of Health Care Indicators. The work aims at the verification of the applicability of the new methodics and software in practice, and at the facilitation of their implementation into practice. It is focused on the opinions of the project participants on the developed software, on its application into practice and on the challenges that should be tackled during the implementation into practice. In order to get the necessary information, we have chosen a method of qualitative investigation using a semi-structured interview. Eight respondents, direct participants in the project, were interviewed. According to the results of the interviews, health facilities pay much attention to the pressure sores monitoring. The investigations are carried out by means of an incidence and prevalence method. The monitoring of pressure sores incidence has not been included in clinical information systems yet, and it is carried out in several different ways. The interest in comparison between hospitals (benchmarking) is considerable. The results of the investigation are used above all for the purpose of improvement of measures preventing the incidence of pressure sores, of the improvement of quality of nursing care, and of reducing the workload of nurses. The extent of the economic impact of the pressure sores incidence on health facilities is not clear. If we want to facilitate the application of national methodics of monitoring the pressures sores prevalence to practice and the use of application software, we should ensure adequate awareness of health care managers, training of teams of health professionals, coordinators and editors, who will go on to work with the national methodics. The monitoring of the national quality indicator in the area of pressure sores is another shift in the implementation of goals of the Ministry of Health concerning the quality of care and safety of the patients.

This study aims to evaluate the importance of proper back support for patients with spinal damage after long-term sitting in a mechanical wheelchair. The subject of this thesis was to analyze Czech and foreign literature and offer the most accurate and still missing information when selecting and compiling the selection of mechanical wheelchair seating unit. The thesis describes the differences between right and wrong sitting posture of mechanical wheelchair users. It analyzes the available information on the possibility of objective assessment, as well as trying to justify the importance of the support of the trunk on the resulting stability and functionality during activities of daily living. It refers to the most common secondary changes and their chaining caused by poor sitting posture. A subjective evaluation of the comfort of users sitting in mechanical wheelchairs in the form of a questionnaire was chosen to assess the urgency of the theme. The research carried out confirmed the hypotheses of clinical practice, which are included in the work. Basic theoretical bases As a chronic wound is called a secondarily healing wound which despite adequate therapy does not tend to heal for a period of 6-9 weeks. The cause of the chronic wound occurrence and its transformation into an acute wound can be infection, influence of associated diseases, skin top layer microtraumatization or skin necrosis cavity. Among the most frequent types of chronic wounds there are aligned venous ulcerations, arterial rodent ulcers, decubitus ulcers and neuropathic rodent ulcers as a consequence of diabetes mellitus disease. Knowledge about wound healing has been deepened considerably in the last few years. The pharmaceutical industry introduces to the market continuously new means of phase wound healing and owing to new trends the classical treatment has nearly disappeared. Essay targets 1.To monitor nurse knowledge about new trends in chronic wound healing. 2.To determine what means of draping and bandaging materials are used in chronic wound treatment in practice and what means appear to be the most effective. 3.To find out if in practical terms there are conditions for nurses to use new trends in chronic wound healing and what constraints they meet in their endeavour to apply these trends. 4.To learn if patients with a non-healing wound are informed about new trends in chronic wound healing. 5.To learn if new trends in chronic wound healing are available for affected patients and in what extent they are used by them. 6.To determine if new trends in chronic wound healing improve life quality of patients with chronic wounds. Used methods The empirical part of the diploma work was processed with a tool of qualitative research inquiry. For data acquisition I chose the technique of semi-structured interview with respondents. For research inquiry I selected two groups of respondents. The first group of respondents was comprised of 10 nurses employed in Nemocnice České Budějovice, a.s. and in the other group there were 5 patients hospitalized in this facility. The interview findings were analysed and divided into categories. For clear arrangement there were created schemes to individual categories in XMind programme. The schemes are supplied with commentary and several quotations of the respondents. Findings From interview analyses it is clear that there is a certain influence of individuality. In the first part there is monitored nurse knowledge about modern trends in chronic wound healing. The conclusion of the research is that in all sections where interviewed nurses work chronic wounds are treated by the way of wet (phase) healing. I also found out that there are several therapeutic and draping materials suitable for all phases of chronic wound healing. There are also described a few practical problems which make effective care about chronic wounds difficult. In the second part there are mentioned particular defects in patient acquaintance with modern methods in wound healing and insufficient education by medical staff. There are also described all problems which deteriorate life quality of patients with chronic wounds. Conclusion The chronic wound healing methods have changed completely in the last few years. Pharmaceutical firms introduce to the market a lot of new materials facilitating very effective and comfortable care about wounds. However, this fact requires continuous education of medical staff participating in the chronic wound treatment. This brings positive results and finally decreases economic costs of care about patients with chronic wounds.

Abstract:
Thesis objectives: The diploma thesis deals with the issue of prevention of skin lesions in perioperative care. In the theoretical part, the current state of knowledge is presented concerning the pressure ulcers identification due to the persisting real problems in differential diagnostics in clinical practice. Further, the thesis deals with the issue of wet and thermal lesions, which occur also during the perioperative care. In the empirical part of the diploma thesis, the first objective was to find the frequency of documented occurrence of decubital lesions in connection with the surgery in Jihlava Hospital for a period of one year. The second objective of the thesis was to find out whether, and what barriers exist when using specific preventive measures. The last aim of the diploma thesis was to compare the efficiency of preventive measures from the point of view of perioperative nurses and nurses from service units. Three hypotheses were formulated to meet the objectives of the thesis. Method for achieving the objectives: The set objectives were achieved through a retrospective analysis of the hospital information system data and a quantitative questionnaire survey. The study included nurses working in surgery, orthopaedics, traumatology, intensive care units and perioperative nurses in Jihlava Hospital. The total number of respondents was 140. Scientific benefits of the thesis: The research implies that the use of prophylactic aids in pre-operative preparation contributes significantly to reducing the risk of skin lesions. The addressed set of nurses did not indicate any serious barriers to their application and, on the contrary, considered it to be very effective. The results of the thesis can be used in practice, especially in a specific health facility for further care improvement. The findings and conclusions: On the research group we have verified that the declared use of preventive strategies in the area of prevention of lesions in perioperative care differs according to the type and focus of the workplace; (p = 0.000) the least frequent use was declared by the general nurse from orthopaedics, however, the methods used did not differ (p = 0,220). In addition, we found out that there was a statistically significant difference in the opinion on efficiency of prophylactic covering in lesion prevention (p = 0.001, the worst evaluation was by the respondents from orthopaedics) but only 2 respondents stated negative experience with their use.
