Abstract:
Dopamine produced by renal proximal tubules increases sodium excretion via a decrease in renal sodium reabsorption. Dopamine natriuresis is impaired in obese Zucker rats; however, the mechanism is not fully understood. To test the hypothesis that renal expression of one or more of the subtypes are altered in these rats, we measured whole kidney protein levels by immunoblotting of D1-like (D1R and D5R) and D2-like (D2R, D3R, and D4R) dopamine receptors in both male and female obese and lean Zucker rats. In obese males on 1% NaCl diet, D1R, D2R, D4R, and D5R were decreased, while D3R was increased, relative to lean rats. Under a 4% NaCl diet, D2R and D3R levels in obese rats were restored to lean levels. 4% NaCl diet reduced D5R in both body types, relative to 1% NaCl diet. Female rats had higher expression of D1R and D3R than did male; however, the sex difference for D1R was markedly blunted in obese rats. In obese rats, dietary candesartan (angiotensin II type 1 receptor blocker) normalized downregulated D1R and D2R, but either decreased (D3R), did not affect (D4R), or further downregulated (D5R) the other subtypes. Candesartan also decreased D4R in lean rats. In summary, reduced renal protein levels of D1R, D2R, D4R, and D5R in obese Zucker rats could induce salt sensitivity and elevate blood pressure. Increased angiotensin II type 1 receptor activity may be mechanistically involved in the decreased expression of D1R and D2R in obese rats. Finally, reduced D1R and D3R in male rats may contribute to sex differences in blood pressure.

Abstract:
297 Background: The subtype of pancreatic ductal adenocarcinoma cancer (PDAC) patients with DNA damage repair (DDR) deficiency from BRCA1/2 mutations has a favorable prognosis and is sensitive to platinum analogues and PARP inhibition. About 10-20% of PDAC patients have DDR deficiency without BRCA mutations (BRCA ness). The efficacy of olaparib in this population is unknown. Methods: Two parallel phases II trials (Israel and U.S.) are ongoing to determine the efficacy of olaparib in advanced PDAC with BRCA ness. Inclusion criteria: ≥ 1 prior systemic therapy for advanced PDAC, ECOG 0-1, germline BRCA 1/ 2 ( gBRCA) mutation negative, previously known DDR genetic aberrations (DDR-GA), family history of BRCA-associated cancers in ≥2 first-degree relatives (without DDR-GA), ATM loss by IHC (Israel only). Primary and secondary endpoints are objective response rate and PFS, respectively. With an type I error rate α of 0.1 and a β error rate of 0.2, and assuming P0 and P1 are 5% and 20%, respectively, study will enroll 24 patients at each site. Results: Thus far, 21 and 11 patients treated in Israel and U.S; respectively. DDR phenotype: Family history of BRCA cancer without DDR-GA (n = 14), DDR-GA (n = 12), ATM loss by IHC (n = 5). DDR-GAs were ATM (n = 6), PALB2 (n = 1), BRCA somatic (2), FANCB (1), PTEN (1) and CCNE1 (1). Efficacy data are in Table. Common toxicities were grade 1-2 anemia, fatigue and nausea. No responses were observed in platinum refractory cases. Conclusions: Olaparib was tolerated well in this study population. Olaparib showed encouraging initial antitumor activity in platinum-sensitive, gBRCA negative PDAC patients with DDR GA and in patients with family history of BRCA cancers (without identifiable DDR-GA). Das zweite behandelt in detaillgetreuer und sehr ausführlicher Weise die Zeit des Ersten Weltkrieges. Mit ganz besonderem Blick auf die Geschehnisse, die Verhaltensweise und die Stimmung in der Not- und Leidenszeit des ersten großen Jahrhundertkrieges in seinem Dorf, hat er ein besonderes geschichtliches Dokument geschaffen. Im Anschluss werden besondere Bedarfe herausgearbeitet, die dann zu einer genaueren Darstellung der (sprachlichen) Situation mehrsprachiger Menschen in deutschsprachigen Pflegeheimen überführen. Anhand von drei Fallbeispielen werden unterschiedliche sprachliche Konstellationen beschrieben, wobei ein besonderes Augenmerk auf die Folgen der sprachlichen Situation für die Pflegebedürftigen gelegt wird.

ZusammenfassungDas in Kapitel 4 beschriebene methodische Vorgehen ist in diesem Kapitel auf drei exemplarische Metalle – Kupfer, Tantal und Kobalt – angewandt. Die dabei generierten Ergebnisse liefern Antworten zu den Forschungsfragen (1), (2) und (3). Die drei Beispielmetalle sind in den Abschnitten 5.2– 5.4 abgehandelt. Jeder dieser Abschnitte erläutert zu Beginn die Bedeutung des jeweiligen Metalls und gibt eine kurze Beschreibung über die Ermittlung dessen sekundärer Quellen. Danach ist eine kurze Beschreibung der wichtigsten Recyclingverfahren und der darauf aufbauenden Recyclingmodelle gegeben. Die Auswertungen der Recyclingmodelle sind nach dem Status Quo sowie nach Parametervariationen vorgenommen und diskutiert. Ebenso die sind die Massenbilanzen und die Ergebnisse der energetischen Optimierung der KEA beschrieben. Auf eine detaillierte Erläuterung der Ermittlung der Datengrundlage der Modelle und Massenbilanzen ist in diesem Kapitel verzichtet. Diese kann dem Appendix II des elektronischen Zusatzmaterials entnommen werden.

Introdução: Os Métodos Não Farmacológicos (MNFs) para alívio da dor utilizados durante o trabalho de parto são de cuidado não invasivo que promovem benefícios para a parturiente, proporcionando sua autonomia e participação ativa durante o parto e nascimento. Objetivos: Objetivou-se identificar quais são os MNFs para alívio da dor mais utilizados em gestantes de uma maternidade de alto risco localizada ao noroeste do Espírito Santo e seus benefícios. Material e Métodos: Tratou-se de um estudo descritivo, de abordagem quantiqualitativa, realizado em um hospital de ensino. Os dados foram obtidos através da aplicação de um formulário semiestruturado, realizou-se a gravação das entrevistas e posteriormente foram transcritas na íntegra para análises semânticas das informações. Para avaliação da dor foi utilizada a escala visual analógica (EVA) de dor, sendo esta aplicada antes e após o uso dos MNFs de escolha. Resultados: Os MNFs para alívio da dor utilizados no estudo, foram: banho morno de aspersão, deambulação, musicoterapia, bola suíça, massagem e aromaterapia, sendo o banho morno de aspersão o método de preferência das participantes (89%). Das 100 participantes entrevistadas, 79% evoluíram para parto normal e 29% para cesariana. Das pacientes que tiveram desfecho de parto normal, todas utilizaram algum MNF para alívio da dor, 69% dessas utilizaram de três a seis MNF e 10% utilizaram de um a dois MNF. O segundo MNF mais utilizado foi a deambulação representando 75%. A musicoterapia, massagem e aromaterapia foram utilizadas respectivamente em 63%, 62% e 49% das participantes. A bola suíça foi utilizada por 59% das participantes. A maioria das entrevistadas (53%) teve redução do limiar de dor após a utilização dos MNFs, entretanto 23% das participantes relataram que a dor se manteve da mesma forma antes e após a aplicação do MNF e 24% referiram piora da dor ao utilizar o MNF. Conclusão: Resta admitir que a relevância da utilização dos MNFs e sua importância na atuação da equipe multiprofissional, assim como na assistência qualificada durante o trabalho de parto e parto, podendo mudar a realidade da prática cotidiana, oferecendo uma assistência de qualidade para as mulheres contribuindo assim, no modo de parir e nascer.

Introdução: A apendicite aguda é considerada um dos principais fatores que levam a cirurgias abdominais no mundo, podendo acarretar complicações que agravam o quadro clínico do paciente em um curto intervalo de tempo. Um ponto relevante para o seu diagnóstico é a obstrução do lúmen do apêndice vermiforme. Sinais clínicos como: dor abdominal difusa em região periumbilical, algia intensa na fossa ilíaca direita, fácies de dor, febre ou enjoo são comuns, por isso é importante que profissionais da saúde tenham uma boa base acerca do protocolo de rápida identificação do quadro do paciente para boas condutas. Objetivo: Ampliar a compreensão sobre a apendicite aguda através da análise de conduta em um caso clínico, bem como destacar a tutoria como estratégia eficiente para a formação profissional. Metodologia: Em tutoria, estudantes de Medicina discutiram o caso de uma paciente do sexo feminino, idosa, que deu entrada em um hospital do interior do Rio Grande do Norte após sentir dor abdominal difusa em região periumbilical com intensidade de uma escala de 9/10. Resultados: A paciente apresentava o abdome globoso, tenso, distendido, fígado não palpável, sem visceromegalias, com dor à palpação abdominal superficial e profunda em fossa ilíaca direita. Prontamente o profissional realizou o teste específico para apendicite aguda, identificando sinal de Blumberg e Rovsing positivo. Sabe-se que a dor causada pela inflamação do apêndice é difusa, dificultando localizar especificamente o local da dor. Para confirmação do diagnóstico, o rápido raciocínio clínico somado a boa execução do exame físico é fundamental para identificação do quadro agudo de apendicite, além do descarte de outras afecções, uma vez que, em mulheres, é comum que dores no quadrante inferior do abdômen se a semelhem as afecções do trato ginecológico como ovário policístico ou mesmo a Salpingite. Conclusão: O manejo profissional frente a suspeita de apendicite fora adequada, auxiliando na rápida condução do tratamento, visto que a demora do diagnóstico poderia levar a complicações. Assim, percebe-se a importância da atenção aos relatos de dor no quadrante inferior direito do abdômen, principalmente nas mulheres, atenção esta que pode ser melhor desenvolvida/treinada na vivência da tutoria.

Introdução: O câncer constitui-se de células que se multiplicam e determinam a formação de tumores que podem invadir outros tecidos e órgãos por disseminação direta e/ou pelas vias linfáticas e sanguíneas. Apesar do paciente com diagnóstico de câncer apresentar vários sintomas, a dor é o mais temido. Pesquisas realizadas pela OMS, na década de 1980, elegeram a dor, associada às neoplasias, como uma emergência médica mundial. Objetivo: Avaliar os métodos do tratamento da dor em pacientes oncológicos, proporcionado ao paciente conforto. Método e materiais: Foi realizada uma revisão bibliográfica descritiva, utilizando como fontes artigos em bases de dados do Scielo e Google Acadêmico publicados entre 2012 a 2019 e no livro Câncer Estadiamento e Tratamento de João Carlos Simões. Palavras-chaves: dor oncológica, métodos medicamentosos, tratamento. Resultados: É imprescindível considerar o relato da experiência dolorosa do paciente para que os profissionais consigam identificar à origem, intensidade e localização; indiquem o melhor tratamento. Os seis principais métodos de controle da dor nesses pacientes são: pela boca, pelo relógio (administrados em intervalos fixos), pela escada analgésica, para o individuo, o uso de adjuvantes (para aumentar a analgesia e controlar efeitos adversos), atenção aos detalhes (observar os benefícios e malefícios). Existem métodos para o tratamento como os físicos (como TENS), mecânicos (massagens) e cognitivos (procedimentos psicológicos). Na escala analgésica observam-se não opiáceos, onde o aumento de dose não produz maior efeito analgésico (anti-inflamatórios não hormonais e analgésicos simples); os analgésicos opiáceos que incluem todas as drogas que tem ação morphinelike nos receptores opiáceos endógenos, divididos em: fracos (codeína, tramadol) e fortes (morfina, metadona, fentanil, oxicodona). Conclusão: O controle da dor no âmbito da oncologia é de extrema importância, pois repercute no estado geral, principalmente, na qualidade de vida dos pacientes. Nesse processo, os fármacos são fundamentais no tratamento de suporte a efetuar nesses pacientes.

Introdução: Fibromialgia é definida com dor musculoesquelética crônica, com acometimento difuso, associada a desordens psicossomáticas. A prevalência média global na população é de 2,7%. A patogênese envolve alterações em neurotransmissores, fatores neuroendócrinos, e neurossensoriais - com amplificação central álgica ou antinocicepção reduzida. Assim, por tratar-se de uma doença multifatorial, requer tratamento holístico, reforçando as medidas não farmacológicas e a participação ativa do paciente. Objetivo: Evidenciar a importância do exercício físico na terapêutica da fibromialgia. Materiais e Métodos: Revisão de literatura integrativa, realizada nos portais eletrônicos PUBMED e SCIELO. Utilizou-se como estratégia de busca os descritores: "fibromyalgia" "physical exercise", combinado pelo operador booleano AND. Como critério de inclusão, elegeram-se artigos publicados de 2016 a abril de 2021, em trabalhos restritos a modelos humanos, sem restrição linguística. Enquanto como critério de exclusão descartaram-se duplicatas e estudos que não abrangeram o recorte de análise. As pesquisas retornaram respectivamente 27 e 4 resultados. Após a interpretação dos títulos e resumos, selecionaram-se 10 trabalhos. Resultados: Desde os primeiros estudos clínicos randomizados foi demonstrado benefícios do exercício físico sobre a fibromialgia, assim como resultados ao longo da realização sustentada, ainda que o paciente relate intolerância nos estágios iniciais após a prática. Dentre esses, a atividade aeróbia melhora a capacidade cardiopulmonar, reduzindo a intensidade da escala da dor e a percepção de fadiga, além de promover qualidade de vida. Os resistivos associaram-se à melhora do bem-estar global, à função física. Os combinados (aeróbios e resistidos ou de flexibilidade) produzem efeitos sobre a depressão, melhoria no limiar da dor e a habilidade motora. Já os exercícios na água (hidroginástica e hidroterapia) melhoram a dor, função física, força muscular, como também possuem resultados positivos sobre a saúde mental. Conclusão: Intervenção da atividade física para pacientes com fibromialgia proporciona ao mesmo tempo melhor condicionamento físico e redução sintomática. O exercício aeróbico reduz a dor e quadros depressivos; treinos resistidos, atenuam a magnitude dos sintomas; em modalidades na água - evidenciam efeitos positivos, contudo não são superiores aos aeróbios. Logo, protocolos indicam que o estímulo à atividade física e tratamento das comorbidades atreladas são essenciais na terapêutica da síndrome fibromialgica.

Abstract:
Introdução: O leiomioma uterino é uma neoplasia benigna das células musculares lisas do miométrio responsiva aos hormônios ovarianos, podendo ser classificada de acordo com a sua posição. Segundo o Ministério da Saúde este tumor acomete 20 a 30% das mulheres em idade fértil, destas, 50% são assintomáticas e, dentre as sintomáticas pode-se ressaltar sangramento genital aumentado, dor pélvica, anemia, infertilidade, aborto de repetição, aumento de volume abdominal. O diagnóstico é clínico complementado por exames de imagem, seguido de estudo anatomopatológico. O tratamento pode ser expectante ou cirúrgico, sendo necessária avaliação individualizada. Relato de Caso: Paciente feminina, 37 anos, G0P0A0, com diagnóstico de leiomioma, medindo 27,66x9,23x24,76 cm em ultrassonografia após 3 anos de evolução. A paciente apresentava pirose, dificuldade em evacuação e distensão abdominal leve. Foi submetida a um procedimento cirúrgico eletivo com incisão inicial em T invertida. Identificado leiomioma de corpo uterino intramural pesando 3712,43 gramas, classificação FIGO 5, aderido ao intestino grosso e ao apêndice, sendo necessária a realização de uma apendicectomia devido à uma apendicite aguda edematosa, e extração em bloco do mioma, útero, ovário direito e trompas congestionadas. A paciente evolui satisfatoriamente no pós-operatório. Discussão: Os leiomiomas ocorrem com maior frequência em mulheres na pré-menopausa, sendo que estas podem ser assintomáticas e diagnosticadas por meio de exames de rotina como a ultrassonografia. Podem gerar sintomas como o aumento do fluxo sanguíneo e duração do período menstrual, bem como metrorragia, dor lombar, dor ou dificuldade miccional ou fecal devido a compressão gerada nos órgãos adjacentes ao útero. Conclusão: Diante do exposto, analisando a origem do leiomioma acerca de sua benignidade e bom prognóstico, torna-se necessária a avaliação e análise entre médico-paciente com o intuito de buscar o melhor tratamento. Podendo este ser através de uma terapêutica cirúrgica, como a histerectomia total, ou através de uma conduta expectante.

Introdução: Um dos eixos da Política Nacional de Atenção Integral à Saúde da Criança é o aleitamento materno e a alimentação complementar saudável. Para tanto, cabe salientar que, para que essas práticas sejam implementadas as políticas públicas foram de fundamental importância. Segundo Espírito Santo, Monteiro e Almeida (2017) o trabalho realizado nessa temática no Brasil, devem iniciar no Pré-Natal (PN), promovendo as ações referentes ao aleitamento materno (AM) e de apoio a mulher, levando em consideração os benefícios para a criança, a mãe, a família e a sociedade. Dentre as dificuldades enfrentadas pelas mães durante a amamentação, a dor e o desconforto são fatores que influenciam de forma direta no desmame precoce. Objetivo: Este estudo teve como objetivo identificar quais estratégias que o enfermeiro poderá utilizar para prevenção da dor e do desconforto causados pela amamentação, na perspectiva de prolongar por maior tempo possível a prática do aleitamento materno, orientando-a do pré-natal, ao puerpério. Metodologia: Foi realizada uma revisão da literatura exploratória por meio de busca online disponíveis na Biblioteca Virtual em Saúde, no período de 2010 a 2020. Resultado: O estudo mostrou que a participação do enfermeiro no período gravídico-puerperal é de fundamental importância, visto que, são essenciais para estimular a amamentação e prestar o cuidado, além de, realizar as correções dos possíveis erros, elevando assim, o índice de adesão ao aleitamento materno, trazendo inúmeros benefícios para ambos. E também, colaborando diretamente para diminuição dos índices de mortalidade infantil. Conclusão: As atividades de educação em saúde e promoção do aleitamento materno ofertadas pelo enfermeiro às mulheres durante o período gravídico-puerperal despontam-se como estratégias de prevenção de intercorrências e proteção desta prática. Sabendo-se que a dor é um dos principais fatores que contribuem com o tempo de oferta do AME, as informações ofertadas durante o pré-natal e, a percepção do apoio demandado pela gestante através do acolhimento feito pelo enfermeiro, podem contribuir de maneira significativa para a redução do risco de intercorrências mamárias dolorosas que inviabilizem a amamentação de maneira adequada, prazerosa e duradoura.

Abstract:
Die Nutzung verfügbarer Radverkehrsdaten auf GPS-Basis stellt eine preisgünstige Möglichkeit für Kommunen dar, einen Überblick über das Nutzungsverhalten ihrer Radfahrenden zu erhalten. Mit den vorliegenden Ergebnissen soll eine Lücke bei der Interpretation von GPS-basierten Daten geschlossen werden. Die Radfahrtypologie auf Basis des geäußerten Verhaltens kann dabei helfen, GPS-Daten auch ohne detaillierte Kenntnisse der zugrundeliegenden Nutzergruppen zielgenauer zu interpretieren. Damit können zukünftig Kommunen die Potenziale entstehender oder bereits vorhandener Angebote an GPS-Radverkehrsdaten zielführender nutzen und ihre Radverkehrsinfrastruktur besser darauf abstimmen. In einem ersten Schritt wurde auf Basis einer Befragung eine empirisch belegte und wissenschaftlich hergeleitete multidimensionale Typologisierung von Radfahrenden erstellt. Anschließend wurde eine umfangreiche heterogene Probandengruppe mit unterschiedlichen soziodemografischen Ausprägungen mit Geräten für die Aufzeichnung ihrer Radrouten ausgestattet. Das auf diesem Weg erhobene Radverkehrsverhalten wurde, gestützt durch kontinuierliche begleitende Befragungen, ausgewertet und anhand unterschiedlicher Indikatoren beschrieben. Damit wurden Präferenzen einzelner Gruppen, z. B. im Hinblick auf Geschwindigkeit, Streckenlänge, Typ der Radverkehrsinfrastruktur, Fahrtzweck oder Routenwahl identifiziert. Auf Basis einer Onlineumfrage konnten vier unterschiedliche Typen von Radfahrenden beschrieben werden, die sich hinsichtlich der Nutzungshäufigkeit, zurückgelegter Entfernungen, Fahrverhalten, Sicherheitsempfinden, Identifikation als Radfahrerende, Wetterabhängigkeit und in motivationalen Aspekten unterscheiden. Anhand der unterschiedlichen Ausprägungen in diesen Merkmalen werden sie als die ambitionierten, die funktionellen, die pragmatischen und die passionierten Radfahrenden bezeichnet. Bezogen auf das Verkehrsverhalten steigt die Nutzungshäufigkeit von ambitionierten über passionierte und pragmatische Radfahrende an. Funktionelle Radfahrende geben die mit Abstand geringste Fahrradnutzung unter allen vier Typen an. Hinsichtlich der angegebenen Distanzen, die zurückgelegt werden, liegen passionierte, pragmatische und funktionelle Radfahrende dicht beieinander. Ambitionierte Radfahrende gaben dagegen an, deutlich größere Distanzen zurückzulegen. Die Ergebnisse aus der Umfrage zeigten sich in einer anschließenden Felduntersuchung in abgeschwächter Form. Insbesondere der ambitionierte Radfahrtyp lässt sich durch höhere Tageskilometerwerte, Geschwindigkeiten und Beschleunigungen von den anderen Typen abgrenzen. Bei den anderen Typen ist eine Unterscheidung weniger ausgeprägt. Hier zeigte sich, dass vor allem die Zugehörigkeit zu einer bestimmten Altersgruppe einen Einfluss auf das Fahrverhalten hat. In Übereinstimmung mit bisherigen Erkenntnissen zeigte sich, dass mit zunehmendem Alter tendenziell etwas langsamer und stetiger gefahren wird. Ebenso radeln auch weibliche Personen etwas langsamer und stetiger als männliche Radfahrer. In der Nutzerbefragung zeigten sich geringe Unterschiede für die Präferenz bei der Infrastrukturnutzung zwischen den Typen, z.B. bei funktionellen Radfahrenden, die eine getrennte Führung im Seitenraum bevorzugen. In der Feldstudie wurde dies ebenfalls untersucht. Auch hier zeigten sich nur geringe Unterschiede. Die Ergebnisse werden auch vor dem Hintergrund eines, eventuell durch die Versuchssituation veränderten Fahrverhaltens der teilnehmenden Radfahrenden, diskutiert. Es konnte vor allem eine hohe Nutzungsfrequenz und Häufigkeit beobachtet werden, die die angegebenen Werte aus der Typenbefragung übertrafen. Für die Nutzung von GPS-Daten für die Radverkehrsplanung wird aus den Ergebnissen abgeleitet, dass eine mögliche Skalierung beziehungsweise Wichtung von Daten entlang soziodemografischer Faktoren die größten Potenziale bietet.

This report summarizes the findings of a study carried out to evaluate the agronomic performance and sensory acceptance by small holder farmers of six biofortified orange-fleshed sweetpotato (OFSP) varieties that were first introduced from CIP’s sweetpotato breeding hub for Southern Africa in Mozambique. The study was participatory and carried out under different agroecological environments in Zimbabwe. The six OFSP varieties, namely Alisha, Victoria, Delvia, Sumaia, Namanga and Irene were planted in the 2019/20 agricultural season along with two non-biofortified white-fleshed local varieties, namely Chingova and German II, at seven DR&SS research stations (Kadoma, Marondera, Harare, Henderson, Gwebi, Makoholi and Panmure) and 120 farmer managed on-farm trial sites in 12 LFSP districts of Bindura, Gokwe North, Gokwe South, Guruve, Kwekwe, Makoni, Mazowe, Mount Darwin, Mutasa, Mutare, Shurugwi and Zvimba. At all but one of the research stations, two trials were set up, one under irrigation and the other under rain-fed conditions. On-farm trials were established following the Mother-Baby Trial approach with 2 mother trials and 8 baby trials per district. In each of the districts, one mother trial was planted under irrigation while the other was rain-fed. All the baby trials were rain-fed.
