Abstract:
Thoracic duct injury is potential complication of neck surgeries, especially surgeries that include neck dissection. It can present as a lymphatic drainage at the wound site, chylous fistula, chylothorax, chylomediastinum, chylopericardium, lymphocele, persistent lymphorrhea, or secondary lymphedema. This complication is managed with intraoperative repair of the injury if recognized, conservative measures of dietary changes and octeriotide, thoracic duct embolization by interventional radiology, or surgical re-exploration of the wound with repair of the injury. This chapter describes a technique to selectively embolize the injured lymphatic branch of the thoracic duct utilizing coils and Onyx instead of embolizing the main duct. This technique eliminates the need for nodal or pedal lymphogram and thus saves time, effort, and reduces expense.

Abstract:
Digital breast tomosynthesis (DBT) is a relatively new X-ray technique that allows quasi–three-dimensional imaging of the breast to overcome limitations of conventional 2-D digital mammography (DM). Several early screening studies have shown that DBT reduces the number of false-positive recalls while simultaneously improving the cancer detection rate. Cost-effectiveness studies have shown that incorporating DBT in screening has the potential to save health care dollars due to lower recall rates as well as reduced treatment costs resulting from the earlier detection of breast cancer. In the diagnostic setting, DBT imaging may allow a more efficient work-up of breast lesions due to improved lesion conspicuity and the ability to better localize lesions within the breast.

Lower extremity deep venous thrombosis (DVT) may be complicated by pulmonary embolism, post-thrombotic syndrome, and phlegmasia cerulea dolens. Due to these complications, the American Venous Forum now recommends thrombus removal for large or symptomatic thrombus burden. The AngioJet Solent Proxy and Omni thrombectomy sets are indicated for use in iliofemoral and lower extremity veins with a diameter ≥3 mm. The device has quickly become a preferred device among the available mechanical thrombectomy options. The AngioJet system has been demonstrated as both efficacious and safe as a method of thrombectomy in lower extremity DVT. This chapter discusses two techniques to utilize the AngioJet device in iliofemoral DVT.

The transcranial application of weak direct current (DC) to the brain is an effective neuromodulation technique that has had more than a century of experimental and therapeutic use. Focal DC brain polarization is now undergoing renewed interest, because of the wide acceptance of TMS as a research tool and candidate treatment for brain disorders. The effects of static electrical fields on cortical neurons in vivo have been known since the advent of intracellular recording. These effects are highly selective for neurons oriented longitudinally in the plane of the electric field. DC can enhance cognitive processes occurring in the treated area. The earliest clinical application of DC polarization was in the field of mood disorders. However, due to lack of temporal and spatial resolution, this technique does not appear particularly useful for exploring neurophysiological mechanisms.

Placement of inferior vena cava (IVC) filters is among the most common medical procedures, with more than 265,000 placed annually. Absolute indications for their placement include acute proximal deep venous thrombosis (DVT) and pulmonary embolism (PE) in patients who have an absolute contraindication to anticoagulation and patients with recurrent thromboembolism despite adequate anticoagulation. Although the survival benefit is unknown, it has been shown that filters decrease the incidence of PE in the short term. Unfortunately, this comes at the expense of known complications, the most important being DVT. This chapter discusses simple techniques to prevent conical filter tilting and enhance retrieval.

For the treatment of borderline personality disorder, a meta-analysis found statistically significant effects indicating a beneficial effect of dialectical behavior therapy (DBT) over treatment as usual for reduction in anger, parasuicidality, and mental health. All of these effects were moderate to large. However, DBT is not uniquely effective for borderline personality disorder. Two type 1 randomized controlled trials found equal effects for versions of psychodynamic therapy. Single type 1 studies have also supported the efficacy, compared to treatment as usual, of cognitive therapy, schema therapy, and Systems Training for Emotional Predictability and Problem Solving (STEPPS, a therapy that combines cognitive-behavioral techniques and skills training with a systems component) as treatments for borderline personality disorder.

Ductography or galactography is a technique used in the diagnostic workup of unilateral, single duct, spontaneous, and bloody or clear nipple discharge. Mammographic imaging with magnification may reveal the source of the discharge. The technique is challenging, yet it can be mastered with attention to detail, a well thought-out protocol, and practice. Imaging findings can be non-specific, yet may guide biopsy or surgical excision. Now with the wide use of breast magnetic resonance imaging (MRI), which has been shown to be more sensitive for lesion detection, ductography is used less frequently. This chapter, appearing in the section on interventions and surgical changes, reviews the indications, procedure protocols, and pitfalls, as well as the management and imaging follow-up for ductography or galactography.

Nuclear medicine-based imaging techniques can provide an estimation of nigrostriatal tract denervation based on radionucleotide uptake in the distal presynaptic terminals of dopaminergic neurons. Although unhelpful in differentiating between differing etiologies of denervation in varied neurodegenerative disorders associated with parkinsonism, this imaging is justified in situations in which parkinsonism is believed to be drug-induced or functional or in cases in which subclinical parkinsonism is suspected. The most common clinical situation in which dopamine transporter imaging is helpful is in the patient on neuroleptic therapy that cannot be stopped who has developed parkinsonism. Dopamine transporter imaging should be normal in drug-induced tremor.

Brain stimulation techniques allow for the search of crucial areas for a given function. Not always convergent with neuroimaging, TMS studies have targeted parietal areas critical for quantity representation, spatio-numerical links, numerical and non-numerical quantity, finger gnosis and calculation. TMS data indicate the intraparietal sulcus and surrounding areas in the left and right hemisphere as crucial for quantity processing, although left hemisphere might be dominant. Bilateral parietal loci are essential for calculation and bilateral parietal areas are behind the spatio-numerical link, which extends to frontal and posterior parietal sites. DCE studies to date have focused on the dominant hemisphere, have used calculation tasks and have found both dissociation and overlap between operations. An integrative view of positive findings and a focus on convergence and possible improvement is proposed. Although regularities are found between these techniques, more research is needed before arriving at conclusions that will have basic and clinical importance.

Neurostimulation therapies are an alternative for non-responders to pharmacological or psychotherapy management, as well as when first-line treatments are contraindicated for treatment of neuropsychiatric disorders in the elderly. Brain stimulation treatments for neuropsychiatric disorders include the following FDA approved treatments for major depressive disorder: electroconvulsive therapy (ECT), which remains one of the most effective therapies for several neuropsychiatric disorders; repetitive transcranial magnetic stimulation (rTMS); and vagus nerve stimulation (VNS). Deep brain stimulation (DBS);magnetic seizure therapy (MST); transcranial direct-current stimulation (tDCS); and direct cortical stimulation (DCS) are not currently FDA approved. These techniques are reviewed in this chapter with special attention to their application in older adults. Medicolegal issues of informed consent and substituted decisions for procedures are also discussed.

Since its original formulation in the mid-1990's, atomistic spin-dynamics has become an important tool for modelling of dynamic processes in magnetic materials. So far this book has described current methodological methods and functionalities of atomistic spin-dynamics simulations. Applications of DFT and ASD techniques to selected topics have been presented in this book, for instance methods for calculation of the microscopic Heisenberg and Gilbert parameter from first principles (Chapters 2 and 6), multiscale modelling of magnon spectra in bulk and thin film magnets (Chapter 9), and theoretical investigations of ultrafast switching dynamics in ferromagnets and ferrimagnets (Chapter 10), and of exotic dynamics of topologically protected spin textures (Chapter 11). In this closing chapter we give an outlook on recent and anticipated developments of the methodology.

Intracerebral hemorrhage (ICH) during pregnancy and the puerperuim is a rare but potentially devastating complication of pregnancy. It is a condition that requires emergent medical attention and inclusion of multiple medical and or surgical specialists. Accurate diagnosis based on clinical exam and supported by neuroimaging techniques is essential. Medical management is the mainstay in most cases including post-hemorrhage blood product consideration, reversal agents if on anti-coagulation, blood pressure control, cerebral edema management, and treatment of seizures. Circumstances also arise in which surgical intervention is needed. It is important to also discuss optimal timing of delivery. Postpartum care including close blood pressure control, deep vein thrombosis prophylaxis (DVT), and risk factor modification are important. Ethical situations sometimes arise and must also be considered with respect to the mother and child.

Many different kinds of group or individual psychotherapeutic interventions, applied to the special needs of patients with ADHD, already exist (e.g. cognitive–behavioural therapy [CBT], coaching, problem-focused therapy, meta-cognitive training, mindfulness-based interventions, psychoeducation, and dialectical behaviour therapy [DBT]-based cognitive therapy). Most of these treatments are behaviourally orientated, and are predominantly modularly structured. CBT has been investigated most frequently and is regarded as the most effective psychological treatment option. The National Institute for Health and Care Excellence guidelines recommend CBT as appropriate for adolescents with moderate impairments and for adults, i.e. if drug treatment has proved to be only partially effective or ineffective. Interventions treating adolescents with ADHD are frequently based on CBT principles, behavioural therapy approaches, and cognitive and social skills training techniques addressing the patient, parents, or teachers. Unfortunately, interventions for adolescents have been investigated on a comparatively limited scale.

Abstract:
An elucidation of ideas and insights generated by the paradigm of "early vision," presented in the form of dialogues. Renowned for his work in depth perception and pattern recognition, Bela Julesz originated the techniques—involving computer-generated random-dot stereograms, cinematograms, and textures—that resulted in the subfield of human psychology called "early vision." In this book, Julesz elucidates the ideas and insights generated by this exciting paradigm in a series of dialogues between Julesz the naif and Julesz the scientist. These playful, personal, deeply informed dialogues, though challenging, are never beyond the reach of the general scientific reader interested in brain research. Among the topics covered are stereopsis, motion perception, neurophysiology, texture and auditory perception, early vision, visual cognition, and machine vision. Julesz's work on early vision and focal attention is the basis of the discussions, but he is equally at ease recounting his debate with John Searle and describing his interactions with Salvador Dali.
