Cet article analyse les emplois schelérien et heideggérien de l’expression « logique du coeur ». Il revient sur l’origine de cette expression – forgée par Scheler, qui l’attribue à tort à Pascal en soulignant son opposition à la raison cartésienne – avant de reconstruire l’usage qu’en fait Heidegger. Trois phases sont mises au jour : Heidegger fait l’économie de cette expression dans ses jeunes années afin de prendre ses distances avec la doctrine schelérienne et pascalienne des sentiments ; il y recourt dans les années trente et quarante pour signaler que cette doctrine est déterminée par la « logique » métaphysique cartésienne, qu’il oppose à deux logiques du coeur non métaphysiques (celles du λόγος grec héraclitéen et parménidien et du Λόγος johannique) ; dans les années cinquante, Heidegger abandonne pourtant cette expression au motif que toute logique – moderne (cartésienne) comme grecque et biblique – demeure métaphysique.

Abstract:
Exposure to low temperature during germination and vegetative growth is a limiting factor to the establishment and development of rice seedlings. Higher cold tolerance of japonica than indica subspecies is well documented. However, reports of cold tolerance in indica genotypes are rare. We screened a large number of indica rice genotypes for cold tolerance during germination and initial vegetative growth. The indica genotypes IRGA 959-1-2-2F-4-1-4-A and IRGA 959-1-2-2F-4-1-4-D-1-CA-1, derived from the same cross, were characterized, respectively, as tolerant and sensitive to low temperature. Indexes of photosynthetic performance during light absorption were heavily affected by cold in both genotypes, but recovered after cold exposure only in the tolerant genotype. Activities of the antioxidant enzymes SOD and CAT (at the vegetative stage) and CAT and APX (at the germination stage) were higher in the tolerant than in the sensitive genotype. Expression of 20 genes previously related to cold response in rice was evaluated. Expression of OsLIP9 and OsWCOR413 were higher in the tolerant genotype upon or prior to cold exposure, respectively. The two sister lines show different molecular and physiological responses to low temperature stress. Further in-depth studies with these lines may help to identify new cold tolerance mechanisms in rice.

We conducted a MEDLINE search of new treatment strategies and experimental agents for treating acute ischemic stroke published from 1989–1996. Clinical trials involving thrombolytics, glutamine release inhibition, N‐methyl‐d‐aspartate receptor antagonism, opioid antagonism, calcium channel blockade, free radical scavenging, membrane stabilization, intercellular adhesion molecule‐1 antagonism, ganglioside administration, and growth factor administration were included. Basic research articles were selected based on progress of the therapeutic class toward clinical trials. Approval of tissue plasminogen activator indicates progress in new treatments for acute ischemic stroke. Experimental therapies with potential may become available soon. Recognizing signs and symptoms of stroke is crucial to ensure prompt administration of these agents. The time to diagnosis determines the therapeutic approach for acute ischemic stroke. Abstract:
Le cancer est l’une des principales causes de mortalité à travers le monde : selon une étude récente de l’Organisation Mondiale de la Santé, le cancer serait à l’origine de 7,9 millions de morts (soit 13% de la mortalité) en 2007 et ce chiffre ne cesse d’augmenter. Malgré de nombreux efforts pour le développement de nouveaux agents anticancéreux, il y a toujours un besoin flagrant de molécules alliant efficacité et sélectivité et le développement de nouveaux inhibiteurs du protéasome est une voie très prometteuse pour le traitement du cancer. C'est dans cette optique que nous nous sommes intéressés aux TMC-95A-D, inhibiteurs naturels non covalents et réversibles du protéasome, et à leur synthèse totale. Diverses études méthodologiques ont été entreprises lors de cette synthèse, ce qui a permis d'avoir accès aux différents fragments nécessaires à la préparation de ces molécules naturelles, fragments dont l'assemblage a permis d'avoir accès à un précurseur macrocyclique totalement fonctionnalisé. En parallèle de ces travaux, nous nous sommes intéressés à la réactivité des dibromoalcènes vis-à-vis de la catalyse au cuivre. Nous avons ainsi pu développer trois méthodes de synthèse différentes permettant chacune d'accéder de manière simple et efficace à des produits extrêmement interessant en chimie organique: les ynamides, les aminals de cétènes et les diynes 1,3 symétriques
Cancer is a leading cause of death worldwide. According to recent studies from the World Health Organization, it accounted for 7. 9 million deaths (13% of all deaths) in 2007 and its incidence continues to rise. Although new anticancer agents are emerging, there is still a strong need for more potent and selective drugs with fewer side effects and the development of new proteasome inhibitors is very promising for the treatment of cancer. With this goal in mind, we became interested in the proteasome inhibitor natural products TMC-95A-D, and in their total synthesis. Various processes have been developed during this synthesis, which enabled us to prepare all the fragments required for the preparation of the natural products. Fragment coupling and macrocyclization allowed us to prepare an advanced, highly functionalized, intermediate en route to TMC-95A. In parallel to this work, we investigated the reactivity of dibromoalkenes under copper catalysis. They were shown to be highly efficient and versatile reaction partners and we were able use them for the develoment of new processes for the straightforward and efficient syntheses of ynamides, ketene aminals and symmetrical diynes

Abstract:
This thesis explores the role Li Hanjun played in the initial stage of the Communist movement in China. It describes Li’s early life, including his family background, his upbringing, his schooling and the environment he grew up in. It analyses some of Li’s early writings to demonstrate his philosophical predispositions and political orientation, as well as his character and temperament. It examines Li’s understanding of Marxism and his endeavours to disseminate it and to introduce various socialist theories into China. It describes his contacts with socialists of other countries and his cooperation with Korean socialists and Soviet agents in China, which helped open up the Communist movement in East Asia. The research focuses on Li Hanjun’s activities in establishing the Chinese Communist Party (CCP) and the opinions he expressed at the Party’s founding congress. It also deals with his ideas and actions in directing labour movements in China. Li Hanjun was a dissident within the CCP and later left the Party. This study clarifies the divergence of views between him and other Party leaders, and shows that his rejection of the Bolshevik doctrines of centralism and dictatorship and of unconditional receipt of financial aid and orders from the Communist International (Comintern) were the main causes of the conflicts and his expulsion. The thesis discusses Li’s vision of socialism, and shows that his ideal socialist society was not one in which a centralist government and the dictatorship of a Communist élite should control and intervene in everything but a collectivity of associations of free and autonomous working people organised in cooperatives. The thesis ends with a critical assessment of Li as a historical figure. It recovers historical facts that have sunk into oblivion, and thus differs from comparable studies published both in China and abroad. It fills important gaps in the history of the early Communist movement in China.

Abstract:
This thesis departs from the traditional historiographical views that portray Philby as having no political influence either while he was serving his country or after his resignation from government service when he was settled in Saudi Arabia. It also departs from picturing him as a man who tended to undermine British policy, arguing that he was loyal to his country, as was plain during his years of service. The thesis examines his background, from his early years to the work he did for his country in India, Iraq and Transjordan and takes an analytical and historical approach. It seeks to present a more comprehensive understanding of Philby’s mission to Arabia, its objectives and outcomes, focusing on his political work in Arabia and his efforts to solve problems which otherwise might have threatened British interests. This thesis deals with Philby’s efforts in the conflict between Najd and Hejaz and his actions in the border dispute between Ibn Sa‘ūd and his rival, Hussain, and assesses whether Philby played a major part in the collapse of the Hejazi kingdom. It considers Philby’s contribution to the American recognition of Saudi Arabia. It examines Philby’s method of settling political conditions after the Idrisi revolt against Ibn Sa‘ud. It asks why Philby visited Yemen and if it would somehow have enhanced Ibn Sa‘ūd’s position. It focuses on Philby’s role in the Buraimi crisis between Britain and Saudi Arabia and illustrates the persuasive arguments by which Philby upheld the Saudi opposition to Britain’s demands over Buraimi.

The importance of the Hittites derives from the fact that they were an organized central power extending over a large territory within which a number of societies maintained their language, culture and traditions. The archaeological findings of Hattusha, the Hittite capital reveal that the city had reached its maximum limits during the Great Kingdom period and the most magnificient and monumental buildings of the city are dated to this period. Yazilikaya, the open air sanctuary which reached to its final form during the Great Kingdom period too, is an outstanding example of the many temples constructed in Hattusha that belong to different periods and the dimensions of which are quite dfferent from each other. Among the archaeological findings of Hattusha, the royal archives are other important material evidence as they transmit us the Hittites'
perception of the cosmos. However, understanding the antique world which is at a "
distance"
both historaiclly and conceptually is not an easy task. In this context, rethinking and reinterpreting the meaning attributed to Yazilikaya, th eopen air sanctuary, can only be achieved by considering the political, architectural and religious aspects together. This study is an attempt to reinterpret the material knowledge by drawing the appropriate limits of this knowledge with a contextual approach.

Aesthetic appreciation, like any ordinary phenomenon of human life, triggers affective and cognitive processes that can provide the subject with sensations of hedonic pleasure and cognitive self-reward (Leder H, Belke B, Oeberst A, Augustin D. Br J Psychol 95(4):489–508, 2004). Although humans share several neuropsychological processes, the experience of aesthetic appreciation is undeniably idiosyncratic, and sometimes it is not that simple to find beauty where we were supposed to find it, and more often the same object can elicit different reactions amongst observers.

Reflecting on the research work conducted in Hotel House, the chapter explores four main aspects: (a) the ethical and practical implications of entering other people’s homes, focusing the analysis on the field access and the researcher’s positionality; (b) the daily material and affective construction of multisensory atmospheres and landscapes in the domestic spaces of Hotel House; (c) the ambivalence of the practices of sociability and mutual aid that take place in the domestic spaces of Hotel House, with particular regard to Senegalese families; (d) the multiscalarity of homemaking and its potential emergence outside of the boundaries of domestic spaces.

Abstract Low Temperature Co-fired Ceramic (LTCC) tapes (DuPont, 951 series), originally developed for monolithic packaging of interconnects and hybrid microelectronic circuitry, have been used in the last four years to develop a meso-scale integrated fluidic technology. The LTCC fluidic technology is shown to be versatile, inexpensive, fast, and free of packaging problems. The manufacturing basis for this technology is patterning of individual tapes, which are laminated and co-fired to create a layered 3-D structure. In this paper, we present another attractive facet of this technology by creating 3-D shell structures with a single layer of LTCC tape. The processing technique is illustrated with hemispherical and cylindrical shells that have internal 3-D conduits. A novel three degree-of-freedom spherical stepper motor is also presented as an application of an active device using magneto-statically actuated curved LTCC shells.

We have studied fibrin degradation products, D-dimer, in citrated plasma with an ELISA assay using monoclonal antibodies (Asserachrom D-Di Stago).35 patients with DVT< 7 days were studied at day O (DO) that is upon admission before treatment with streptokinase (n=6), heparin (n=5) or LMWH CY222 Choay (n=24). Phlebographies were scored by Marder’s index (MI).At DO, MI = 24.28 ± 10.85 (m ± SD), the mean (± SD) of D-dimer was : 4056 ± 5545 ng/ml (range 950-19000) (normal values < 500 ng/ml). No correlation was found between D-dimer and MI and between D-dimer and clinical delay of DVT.The evolution of D-dimer was studied during 7 days in patients treated with heparin or LMWH (n=29). Blood sampling was undertaken at DO and at 8 h AM, 1 hour before injection at Dl, D3, D5, D7. The evolution of thrombolysis was evaluated by comparing MI at DO and D7. Among patients with MI at DO < 10 (n=25) two groups were compared : - successful group : 8 patients with decrease of MI at D7 < 80% ; - failure group : 11 patients with decrease of MI at D7< 20% : D-dimer at DO were higher in successful group (p< 0.05) ; these results could be explained by better endogenous thrombolysis in this group. The percentages of the decrease of D-dimer at D3 and at D7 compared to DO were higher in the successful group : Successful group versus Failure group (m ± SD) = % decrease at D3 : 46.4 ± 29.2/ 13.4 ± 11 (p< 0.05) ; % decrease at D7 : 52.3 ± 31.5/ 24.2 ± 26.3 (p<0.05) (in agreement with Soria and al, -IX th International Congress of Phlebology, September 1986, Kyoto) ; these results could be explained by a smaller clot mass to be lysed in successful group.In conclusion the level of D-dimer at DO and the intensity of the decrease of D-dimer during 7 days (as from D3) may have a predictive value for in vivo thrombolysis.

Introduction: It is known that breast cancer is the type of cancer that mostly affects women in the world, both in the developing and developed countries, with about 2.3 million new cases in 2020, comprising 25% of all cancers diagnosed in women. Vitamin D concentration has been studied as a risk and prognostic factor in women with breast cancer; its deficiency is common in women with postmenopausal breast cancer, and some evidence suggests that low vitamin D status increases the risk for disease development. The impact of vitamin D at the time of diagnosis on the outcome of patients with breast cancer is less well understood. In view of the increasing number of breast cancer survivors and the high prevalence of vitamin D deficiency among patients with breast cancer, an evaluation of the role of vitamin D in prognosis and survival among patients with breast cancer is essential. Objective: The aim of this study was to evaluate the association between serum vitamin D (VD) levels at diagnosis and overall survival (OS), disease-free survival (DFS), and cancer-specific survival (CSS) in postmenopausal women treated for breast cancer. Methods: This is a single-center prospective cohort. The study included patients newly diagnosed with invasive breast cancer between 2014 and 2016, aged ≥45 years, and in amenorrhea for ≥12 months, and VD assessment at the time of diagnosis, before any cancer treatment. Patients were classified into three groups according to serum levels of 25-hydroxyvitamin-D [25(OH)D]: sufficient (≥30 ng/mL), insufficient (between 20 and 29 ng/mL), and deficient (<20 ng/mL). Clinical and anatomopathological data were collected. The primary outcome was OS and secondary outcomes were DFS and CSS. Kaplan-Meier curve and Cox regression model were used to assess the association between 25(OH)D levels and OS, DFS, and CSS. Differences in survival were evaluated by hazard ratios (HRs). The study was approved by the Ethics Committee (CAAE: 71399117.2.0000.5411). Results: The study included 192 women with a mean age of 61.3±9.6 years at diagnosis, mean 25(OH)D levels of 25.8 ng/mL (ranging from 12.0 to 59.2 ng/mL), and follow-up period between 54 and 78 months. Sufficient VD levels were detected in 65 patients (33.9%), insufficient in 92 (47.9%), and deficient in 35 (18.2%). Patients with 25(OH)D insufficiency and deficiency had a larger proportion of high-grade tumors, locally advanced and with distant metastasis, positive axillary lymph nodes, negative estrogen receptors (ER), and progesterone receptors (PR), and higher Ki67 index (p<0.05 ). The mean OS time was 54.4±20.2 months (range 9–78 months), and 51 patients (26.6%) died during the study period. Patients with VD deficiency and insufficiency at diagnosis had significantly lower OS, DFS, and CSS compared to patients with sufficient values (p <0.0001). After the adjustment for clinical and tumoral prognostic factors, patients with serum 25(OH)D levels considered deficient at the time of diagnosis had a significantly higher risk of global death (HR=4.65, 95%CI 1.65–13.12), higher risk of disease recurrence (HR=6.87, 95%CI 2.35–21.18), and higher risk of death from the disease (HR=5.91, 95%CI 1.98–17.60) than the group with sufficient 25(OH)D levels.

Abstract:
ЦЕЛЬ: оценить эффективность приема препаратов кальция и витамина D в течение 1 года в профилактике постменопаузального остеопороза у женщин старше 50 лет. МАТЕРИАЛЫ И МЕТОДЫ: исследование включило в себя ретроспективный анализ 213 амбулаторных медицинских карт, из которых были отобраны 30 пациенток с установленным диагнозом постменопаузальный остеопороз. Проведен анализ показателей Т-критерия до и через 1 год после приема препаратов кальция в дозировке 1000 мг и витамина D в дозировке 400 МЕ в сутки. Результаты: Средний возраст пациенток составил 59,57 ± 4.84 лет. Средний уровень показателей Т-критерия до терапии составил -1.877 ± 0.262 (95% ДИ -1,78 – -1,97) ед.SD. Средний уровень показателей Т-критерия через 1 год после терапии препаратами кальция и витамина D составил -1.763 ± 0.323 (95% ДИ -1,65 – -1,88) ед.SD. Статичестически значимого изменения Т-критерия выявлено не было (р=0, 006). ВЫВОДЫ: несмотря на то, что статически значимого повышения уровня минеральной плотности костной ткани не было получено, стоит отметить тенденцию к улучшение показателей Т-критерия у большинства пациенток, что дает возможность говорить о необходимости профилактического приема препаратов кальция и витамина D пациентками в постменопаузе. Обеспечение достаточного потребления кальция требуется также и при проведении терапии остеопороза. Это обусловлено прежде всего гипокальциемическим действием большинства антирезорбтивных препаратов (кальцитонин, бисфосфонаты, иприфлавон), а с другой – возможным нарушением минерализации костной ткани при использовании фторидов, бисфосфонатов I поколения. Кальций в суточной дозе 1000-1500 мг и витамин D в дозе 400-1200 МЕ должны быть обязательной частью стратегии лечения остеопороза всеми имеющимися антирезорбтивными препаратами (бисфосфонатами, деносумабом, заместительной гормональной терапией, селективными модуляторами эстрогеновых рецепторов) при отсутствии гиперкальциемии.

An increased plasma level of D-dimer has been suggested as an indicator of postoperative thromboembolism (TE). We studied the D-dimer level in patients undergoing major elective abdominal surgery and in healthy volunteers to evaluate a possible value of D-dimer as a screening test for TE. 18 patients and 5 healthy volunteers were studied. The patients received low molecular weight heparin (Logiparin TM) subcutaneously once . daily for 7 days. The 125 I-fibrinogen uptake test (FUT) was done in all patients. Blood samples were taken preoperatively, postoperatively and on the postoperative day 1, 3, 4, 5 and 6. The volunteers had blood samples taken before and 4 hours after subcutaneous Logiparin injection. D-dimer was assayed by enzyme-immunoassay (Boehringer Mannheim, Elisa D-dimer, Cat.no. 998117). Results are given as median with 95% confidence limits in brackets.One patient developed deep venous thrombosis verified by phlebography. Two patients had abnormal FUT but normal phlebography. The plasma level for D-dimer in the 15 patients with normal FUT was 500 ng/ml (300-800) preoperatively. The D-dimer level increased postoperatively to 1700 ng/ml (900-3300) (p < 0.001). The following days the D-dimer level increased steadily to 4800 ng/ml (2600-6800) 6 days postoperatively. The plasma level of D-dimer in the volunteers were less than 110 ng/ml both before and after injection of Logiparin. The plasma level of D-dimer in the patient with deep venous thrombosis and the 2 patients with abnormal FUT were within the range of the patients with normal FUT.The postoperative D-dimer level was as high as that reported in patients with diagnosed thromboembolism.In conclusion, the D-dimer test does not seem to be a potential screening test for postoperative thromboembolism.

Abstract:
По данным ВОЗ пожилые люди, а также больные с сопутствующими, в том числе эндокринными за- болеваниями подвержены наибольшему риску тяжелого течения коронавирусной инфекции (COVID-19). Ученые и клиницисты всего мира активно изучают особенности патогенеза COVID-19 и его влияние на эн- докринную патологию. ЦЕЛЬ: изучить структуру эндокринной патологии, а также особенности минерального обмена у паци- ентов, госпитализированных с COVID-19. МАТЕРИАЛЫ И МЕТОДЫ: проведено одноцентровое ретроспективное исследование 193 пациентов. В первый день госпитализации до назначения упреждающей терапии было выполнено расширенное ла- бораторное обследование. Сравнение независимых групп по качественным признакам выполнено с по- мощью критерия Хи-квадрат Пирсона (χ2). Исходный критический уровень значимости (p) при проверке статистических гипотез принимался равным 0,05. РЕЗУЛЬТАТЫ: среди обследованных пациентов выявлены следующие эндокринопатии: СД (17,6%), ожирение (43,5%), узловой зоб (5,1%), ПГПТ (0,5%), ВГПТ (38,9%). Дефицит/недостаточность витамина D диагностированы в 96,8%. Значимых отклонений кальцитонина не зафиксировано (медиана 1 [1; 1] пг/мл). Медиана 25(ОН)D составила 12,10 [6,98; 17,00] нг/мл, ПТГ - 43,02 [29,51; 58,53] пг/мл, альбумин-скорректиро- ванного кальция 2,18 [2,11; 2,26] ммоль/л. Гипокальциемия зафиксирована в 38,9% случаев, в то время как гиперкальциемия в 1% (n=2). ПТГ имел тенденцию к умеренной отрицательной корреляции с лабораторно подтвержденным COVID-19 (r= -0,211, p=0,003); отмечена умеренная положительная корреляция альбу- мин-скорректированного кальция с лейкоцитами (r=0,297, p<0,001), с тромбоцитами (r=0,307, p<0,001). Гипокалиемия имела умеренную отрицательную связь с высоким уровнем СРБ (r= -0,211, р=0,003) и ЛДГ (r= -0,213, р=0,003). В подгруппе тяжелой степени тяжести были ниже уровни 25(ОН)D, общего кальция и альбумина (р<0,05). При пошаговом логистическом регрессионном анализе была выявлена ассоцииация гипоальбуминемии с более высокой степенью поражения легких по МСКТ (ОШ 0,815, 95% ДИ 0,736 —0,903), а также с более высокими значениями D-димера (ОШ 0,864, 95% ДИ 0,773-0,967). Гипокалиемия коррелировала с повы- шенными значениями ЛДГ (ОШ 0,175, 95% ДИ 0,056—0,545) и D-димера (ОШ 0,101, 95% ДИ 0,022-0,457). При сравнении подгруппы с эндокринными заболеваниями (n=139) с подгруппой без эндокринных заболеваний (n=54) в первой отмечены более низкие уровни альбумина (р=0,018, Σ2), магния (л, р=0,056, Σ2), рСКФ (р=0,0003, Σ2), а уровни ФРФ-23 и D-димера были более высокими (р=0,009 и р=0,055, соответ- ственно, Σ2). Также, у них чаще развивался «цитокиновый шторм» (48,1% против 30,4%, р=0,021, Σ2). Для пациентов с СД (n=34) были характерны более высокие маркеры воспаления, статистически значимые для СРБ, ИЛ-6, ИЛ-1β (p<0,05), и более низкий уровень IgG (р=0,034). Кроме того, в подгруппе с СД чаще встречались гипоальбуминемия, гипомагниемия и недостаточность витамина D (р<0,05) и была характерна высокая частота «цитокинового шторма» (51,3% против 31,4%, р=0,0203, Σ2) с летальным исходом (15,4% против 5,8%, р=0,047, Σ2). ВЫВОДЫ: высокая частота эндокринной патологии и ее взаимосвязь с тяжелым течением COVID-19 диктует необходимость расширенного обследования во время госпитализации. Своевременная диагно- стика и коррекция выявленных нарушений нужна для улучшения прогноза пациентов.

Background: Post-menopausal women are at high risk for breast cancer. The association between serum 25-hydroxyvitamin D [25(OH)D] concentration and breast cancer in post-menopausal women is not well understood. Objectives: The aim of this study was to investigate the association between serum 25(OH)D and breast cancer using nationally representative sample surveys. Methodology: In this cross-sectional study, we used seven cycles of National Health and Nutrition Examination Surveys from 2001 through 2014. Participants were non-institutionalized post-menopausal women (n=8100). Logistic regression was performed to determine the association between serum 25(OH)D concentrations and breast cancer prevalence. A restricted cubic spline method was used to assess the non-linear association. Results: The prevalence of breast cancer was 3.3%, 4.0%, 4.6%, 6.4%, and 6.9% in the groups with serum 25(OH)D levels of <30, 30-<50, 50-<75, 75-<100, and ≥100 nmol/L, respectively. The risk of having breast cancer was significantly higher in the serum 25(OH)D 75-<100 nmol/L category compared to the 25(OH)D <30 nmol/L concentration [OR and 95% CI; 2.21 (1.23-3.98)]. Furthermore, a significant non-linear relationship between serum 25(OH)D concentrations (when used as a continuous variable) and breast cancer in all post-menopausal women (p for non-linear trend 0.032) was observed. Overall, the risk of breast cancer was highest (OR=1.5) between 70 nmol/L and 80 nmol/L of serum 25(OH)D concentration in all post-menopausal women. Conclusion: An adverse association was observed between serum 25(OH)D concentration and breast cancer in post-menopausal women. Further research is needed to elucidate the mechanism of vitamin D in cancer pathogenesis.

Multipole phase shift torquemeters now measure shaft power to 0.1% accuracy in R & D applications, and to 1.0% when designed for permanent installation, both at confidence levels of 95%. The total installed power to date is 2 million kW, and over 2 million hours have been logged with good reliability. They are in service at speeds up to 90,000 RPM and powers of 50MW. The paper outlines ways of ensuring credibility, and describes typical designs for R & D use, and for permanent installation. It concludes that the application problems are largely those of transmission engineering.

ABSTRACT Background: Pre-eclampsia (PE), a complication of pregnancy, remains a major cause of maternal and fetal morbidity and mortality. Research showed that vitamin D reduces the risk of pre-eclampsia in pregnant women. The recommended dose for vitamin D supplementation is 600 IU per day. This study aimed to determine the effect of vitamin D supplementationon the pre-eclampsia risk reduction in pregnant women. Subjects and Method: This was a meta-analysis and sytematic review toward vitamin supplementation and pre-eclampsia in pregnant women. Published original studies from PubMed, Science Direct, Springer Link, and Google Scholar databases, from year 2013 to 2017 were collected for this study. Keywords used “Vitamin D” AND “Decrease Risk” OR “Prevention” OR “Reduce Risk” AND “Pre-eclampsia”. The inclusion criteria were full text, in English language, using randomized controlled trial study design, and reporting odds ratio. The selected articles were analyzed using PRISMA guideline and Revman 5.3. Results: 7 studies were met inclusion criteria. This study showed that vitamin D supplementation reduced the risk of pre-eclampsia in pregnant women (OR= 0.97; 95% CI= 0.79 to 1.18; p=0.730), with (I2 = 86%; p<0.001). Limited information exists about enrollment in Part D prescription coverage by Medicare beneficiaries with cancer. Part D coverage may increase access to medicines. This study evaluated patterns of Part D uptake and costs and assessed the effects of coverage on hospitalizations and emergency department (ED) use among people with colorectal cancer (CRC). We analyzed Surveillance, Epidemiology, and End Results (SEER)–Medicare linked data on fee-for-service (FFS) Medicare beneficiaries with at least 36 months of follow-up who were diagnosed with CRC at any point from January 2007 through December 2010, and a matched cohort of beneficiaries without cancer. Dual (Medicare/Medicaid) enrollees were excluded because they are automatically enrolled in Part D. Among beneficiaries with CRC (n=12,774), 39 percent had complete Part D coverage, defined as coverage in the diagnosis year and 2 subsequent years; the rate was 38 percent in the matched comparison cohort (P=.119). Among those with complete Part D coverage, there was no significant difference in annual prescription drug costs between people with CRC ($3,157, 95% confidence interval [CI]: $3,098–$3,216) and without ($3,113, 95% CI: $3,054–$3,172). Among people with CRC, odds of ED use ranged from unchanged to marginally higher for those with no or partial Part D coverage, (adjusted odds ratio: 1.09, 95% CI: 1.00–1.18), compared with those with complete Part D coverage. Lack of continuous Part D coverage was associated with more ED use among Medicare FFS beneficiaries with CRC in 2007–2013. Among people with Part D coverage, prescription drug costs varied little between those with CRC and matched beneficiaries without cancer.

Métodos Se realizó una revisión sistemática de RS y ECAS que evaluaron el impacto del rastreo de hipovitaminosis D y la eficacia de la suplementación de vitamina D. Se realizó una búsqueda amplia en la base de datos Epistemonikos, la cuál se construye mediante búsquedas periódicas en diversas bases de datos y se complementó con una búsqueda en Pubmed desde fecha de inicio de las bases de datos hasta el 08/09/2023. De manera independiente y pareda se seleccionaron los estudios y extrajeron los datos. El riesgo de sesgo se evalúo con las herramientas RoB 2 y AMSTAR. Los resultados se metaanalizaron utilizando el método del inverso de la varianza con un modelo de efectos aleatorios. También se evaluó el efecto de subgrupo utilizando el instrumento ICEMAN. La certeza de la evidencia fue evaluada con el enfoque GRADE. Resultados A través de las estrategias de búsqueda se identificaron 2444 RS y 1588 ECAS. De estas, se incluyeron en la síntesis 2 RS y 17 ECAS reportados en 48 referencias. No se encontraron estudios aleatorizados que evaluaran la utilidad del rastreo. En relación con la efectividad de la suplementación de vitamina D, se sugiere que probablemente no tenga un impacto significativo en la mortalidad por todas las causas (riesgo relativo (RR=1.01), intervalo de confianza (IC) del 95%: 0.83 a 1.23, con moderada certeza en la evidencia), ni en la incidencia de eventos cardiovasculares (RR=1.02, IC 95%: 0.83 a 1.24, con moderada certeza en la evidencia), ni en el riesgo de fracturas (RR=0.84; IC 95%: 0.60 a 1.18, con moderada certeza en la evidencia), ni en el riesgo de caídas (RR=0.91, IC 95%: 0.73 a 1.13, con moderada certeza en la evidencia), ni en el riesgo de hospitalizaciones (RR=1.46, IC 95%: 0.70 a 3.03, con baja certeza en la evidencia), así como tampoco en el riesgo de desarrollar infecciones respiratorias (RR=0.95, IC 95%: 0.67 a 1.35, con baja certeza en la evidencia). Con respecto a la seguridad de la suplementación con vitamina D en la población con hipovitaminosis, se sugiere que podría no aumentar el riesgo de eventos adversos serios, litiasis renal o eventos adversos generales. Conclusiones Dada la ausencia de beneficios evidentes se concluye que el rastreo podría no ser útil y su implementación podría resultar en una alta tasa de sobrediagnóstico y sobretratamiento. En relación a la suplementación en individuos con hipovitaminosis, los resultados evidencian la falta de beneficios significativos en la mortalidad, eventos cardiovasculares o incidencia de cáncer. Tampoco se encontró una influencia significativa en la incidencia de fracturas o en el riesgo de hospitalizaciones, infecciones respiratorias y caídas.
