Abstract:

7092 Background: Compliance with chemotherapy can affect treatment efficacy and patient safety. Selecting an oral chemotherapy regimen with good tolerability in appropriate patients is important. To determine general level of compliance of oral topotecan, a retrospective analysis was conducted on 3 international open-label, multicenter, 2-arm randomized, phase III studies of patients with NSCLC and SCLC. Methods: Patients were randomized to receive oral topotecan d1–5 q21d as follows: Study 387 (Ramlau et al, Proc ASCO, 2005) 2.3 mg/m2/d second-line vs IV docetaxel for NSCLC; Study 389 (Eckardt et al, Proc ASCO, 2005) 1.7 mg/m2/d first-line with cisplatin vs etoposide/cisplatin for extensive-disease SCLC; and Study 478 (O’Brien et al, Lung Cancer 2005) 2.3 mg/m2/d second-line vs best supportive care for SCLC. Oral topotecan was administered to the patient in the clinic on day 1 of each course and the patient was responsible for self-administering the oral capsules the remaining 4 days of dosing. Patient compliance was defined as 100x(sum of number of caps taken across courses)/(sum of number of caps dispensed across courses), where number of caps taken = number of caps dispensed-number of caps returned. Results: Accrual per arm was (387), 414 patients; (389) 389; (478) 71. In the treated population for all three groups, adherence to oral topotecan was high, with >90% of patients recording at least 90% overall compliance (Table). Mean age (% ≥65 years/%PS 2) was (387) 58.8 years (29.2%/14%), (389) 59.7 (32.6%/13%), and (478) 59.8 (33.8%/27%). Median oral topotecan dose intensity (% target) for each course was (387) 87%, (389) 95%, and (478) 98%. The most common reason for dose reductions or delays was hematological toxicity. Conclusions: Oral topotecan can be administered on an outpatient basis with a high degree of patient compliance in the treated population, which included those ≥65 years and those with poor PS. Abstract:

Metformin (N,N-dimethylguanylguanidine) is one of the most prescribed drugs with pleiotropic, exerted in part by not fully elucidated mechanisms of action. We developed and validated a gas chromatography–mass spectrometry (GC–MS) method for the quantitative analysis of metformin (metformin-d0) in 10-µL aliquots of human serum and urine using N,N-[dimethylo-2H6]guanylguanidine (metformin-d6) as the internal standard. The method involves evaporation of the samples to dryness, derivatization with pentafluoropropionic (PFP) anhydride in ethyl acetate (30 min, 65 °C), and extraction into toluene. The negative-ion chemical ionization GC–MS spectra of the PFP derivatives contain a single intense ion with mass-to-charge (m/z) ratios of m/z 383 for metformin-d0 and m/z 389 for metformin-d6. Our results suggest that all amine/imine groups of metformin-d0 and metformin-d6 are converted to their N,N,N-tripentafluoropropionyl derivatives, which cyclize to form a symmetric triazine derivative, of which the non-ring amine group is amidated. Quantification was performed by selected-ion monitoring (SIM) of m/z 383 and m/z 389. Upon validation, the method was applied to determine serum and urine metformin concentrations in 19 patients with Becker muscular dystrophy (BMD). Serum and urine samples were collected at baseline (Visit I), after six weeks of supplementation (Visit II) with metformin (3 × 500 mg/d; metformin group; n = 10) or l-citrulline (3 × 1500 mg/d; citrulline group; n = 9) followed by a six-week supplementation with 3 × 500 mg/d of metformin plus 3 × 1500 mg/d l-citrulline. At Visit I, the metformin concentration in the serum and urine was very low in both groups. The metformin concentrations in the serum and urine of the patients who first took metformin (MET group) were higher at Visit II and Visit III. The metformin concentration in the serum and urine samples of the patients who first took l-citrulline (CITR group) were higher at Visit III. The serum and urine concentrations of metformin were insignificantly lower in the CITR group at Visit III. The mean fractional excretion (FE) rate of metformin was 307% (Visit II) and 322% (Visit III) in the MET group, and 290% in the CITR group (Visit III). This observation suggests the accumulation of metformin in the kidney and its secretion in the urine. The GC–MS is suitable to measure reliably circulating and excretory metformin in clinical settings.

Abstract:

Plant factories using artificial light to produce vegetables have high energy costs due to the high demand for electricity for lighting. Compared to conventional light sources, light-emitting diodes (LEDs) offer the possibility of tailoring the light spectrum and regulating light intensity and are more energy-efficient in terms of energy conversion regardless of the levels of lighting intensity. Optimal light intensity and daily light integral (DLI) requirements are key factors for plant growth; however, their values vary among species and varieties. Our experiment aimed to identify the best light intensity to produce lettuce plants in controlled environment. Lettuce plants of the type Batavia cv ‘Blackhawk’ were grown in plastic pots filled with perlite and peat (20:80 v/v) for 33 days in a growth chamber under blue (B, 20%) and red (R, 80%) LED light at a photosynthetic flux density of 130 µmol m−2 s−1 (BR 130, DLI 7.49 mol m−2 d−1), 259 µmol m−2 s−1 (BR 259, DLI 14.92 mol m−2 d−1), and 389 µmol m−2 s−1 (BR 389, DLI 22.41 mol m−2 d−1). Our results showed that increasing light intensity and DLI promotes net photosynthesis, sustains the electron transport rate (ETR), and stimulates the synthesis of anthocyanins and carotenoids, with positive results for plant photoprotection. Furthermore, the decreases in vegetation indexes (photochemical reflectance index (PRI), greenness, and modified chlorophyll absorption in reflectance index (MCARI1)) also indicate changes in photosynthetic pigment content in response to plant acclimation to different DLIs. Among the three light intensities, 389 µmol m−2 s−1 (DLI 22.41 mol m−2 d−1) gave the best results for growing Batavia red lettuce cv ‘Blackhawk’, since it enhances both production and qualitative traits. These results highlight the importance of a proper light intensity to promote plant growth and qualitative traits and to reach high production targets. Hence, preliminary screening of plant performance under different light treatments is recommended to optimise plant response to artificial lighting.

Abstract:

Abstract Prior research found depressive symptoms and subjective memory to be associated with objective memory (OM) performance. One key factor of subjective memory, as measured by the Memory Functioning Questionnaire (MFQ), is General Frequency of Forgetting (GFF). However, few studies focused on identifying specific factors of depressive symptoms when examining associations between depressive symptoms, subjective memory and OM. Using structural equation modeling, cross-sectional associations of factors in the CES-D (depressive symptoms) to the MFQ (subjective memory) and OM were investigated in the Seattle Longitudinal Study (mean Age=72.39; SD=8.28; mean Education=15.12; SD=2.76; 58.4% female). Differential associations of the CES-D factors with the MFQ factors [n=389; RMSEA=.031; CFI=.973; TLI=.966] and the GFF subscales [n=389; RMSEA=.033; CFI=.971; TLI=.964] resulted. Only the CES-D somatic complaints factor was significantly associated with the GFF factor (β= -.45, p &lt;.001), suggesting that people with more somatic complaints reported more memory concerns. The CES-D somatic complaints factor was negatively associated with the frequency of forgetting in daily life (β=-.36, p&lt; .001) and forgetting while reading subscales (β= -.33, p &lt; .001), indicating individuals that reported more somatic complaints experienced more frequent memory failures when performing daily activities and reading. Overall, a key CES-D factor, somatic complaints, emerged as influential to subjective memory, whereas there was no relation to OM. Further study of the longitudinal associations between the CES-D factors and subjective and objective memory is essential to determine the potential impact on memory deficits.

Abstract:

4074 Background: In the Phase 3 HIMALAYA study (NCT03298451) of patients (pts) receiving first-line treatment for unresectable hepatocellular carcinoma (uHCC), a single priming dose of tremelimumab (T; anti-CTLA-4) plus durvalumab (D; anti-PD-L1) in the STRIDE regimen significantly improved overall survival (OS) vs sorafenib (S), and D monotherapy was noninferior to S for OS (Abou-Alfa et al. J Clin Oncol 2022;40[suppl 4]. Abs 379). Methods: A pre-planned secondary objective of HIMALAYA was to assess pt-reported outcomes (PROs) in pts receiving STRIDE (T 300 mg plus D 1500 mg [one dose] plus D 1500 mg once every 4 weeks [Q4W]; N=393) or D (1500 mg Q4W; N=389) vs S (400 mg twice daily; N=389). The European Organisation for Research and Treatment of Cancer (EORTC) 30-item Quality of Life (QoL) Questionnaire and the EORTC 18-item HCC QoL questionnaire were used to assess disease-related symptoms, physical functioning (PF), and Global Health Status (GHS)/QoL. Time to deterioration (TTD), defined as time from randomization to first clinically meaningful deterioration (worsening ≥10 points) confirmed at a subsequent visit or death, was assessed in pts with baseline scores ≤90 for symptoms or ≥10 for PF and GHS/QoL. Results: Across treatment arms, compliance rates for PROs were >77% at baseline and >70% overall. Baseline scores were comparable across treatment arms. TTD in fatigue, appetite loss, abdominal pain, PF, and GHS/QoL were significantly longer for both STRIDE and D vs S (Table). TTD in nausea and abdominal swelling were significantly longer for STRIDE vs S. Conclusions: The positive OS outcomes for STRIDE and D in pts receiving first-line treatment for uHCC in HIMALAYA were associated with clinically meaningful, pt-centered benefits, demonstrated by delayed worsening of disease-related symptoms, PF, and GHS/QoL vs S. Abstract:

L’analisi del movimento umano ha come obiettivo la descrizione del movimento assoluto e relativo dei segmenti ossei del soggetto e, ove richiesto, dei relativi tessuti molli durante l’esecuzione di esercizi fisici. La bioingegneria mette a disposizione dell’analisi del movimento gli strumenti ed i metodi necessari per una valutazione quantitativa di efficacia, funzione e/o qualità del movimento umano, consentendo al clinico l’analisi di aspetti non individuabili con gli esami tradizionali. Tali valutazioni possono essere di ausilio all’analisi clinica di pazienti e, specialmente con riferimento a problemi ortopedici, richiedono una elevata accuratezza e precisione perché il loro uso sia valido. Il miglioramento della affidabilità dell’analisi del movimento ha quindi un impatto positivo sia sulla metodologia utilizzata, sia sulle ricadute cliniche della stessa. Per perseguire gli obiettivi scientifici descritti, è necessario effettuare una stima precisa ed accurata della posizione e orientamento nello spazio dei segmenti ossei in esame durante l’esecuzione di un qualsiasi atto motorio. Tale descrizione può essere ottenuta mediante la definizione di un modello della porzione del corpo sotto analisi e la misura di due tipi di informazione: una relativa al movimento ed una alla morfologia. L’obiettivo è quindi stimare il vettore posizione e la matrice di orientamento necessari a descrivere la collocazione nello spazio virtuale 3D di un osso utilizzando le posizioni di punti, definiti sulla superficie cutanea ottenute attraverso la stereofotogrammetria. Le traiettorie dei marker, così ottenute, vengono utilizzate per la ricostruzione della posizione e dell’orientamento istantaneo di un sistema di assi solidale con il segmento sotto esame (sistema tecnico) (Cappozzo et al. 2005). Tali traiettorie e conseguentemente i sistemi tecnici, sono affetti da due tipi di errore, uno associato allo strumento di misura e l’altro associato alla presenza di tessuti molli interposti tra osso e cute. La propagazione di quest’ultimo ai risultati finali è molto più distruttiva rispetto a quella dell’errore strumentale che è facilmente minimizzabile attraverso semplici tecniche di filtraggio (Chiari et al. 2005). In letteratura è stato evidenziato che l’errore dovuto alla deformabilità dei tessuti molli durante l’analisi del movimento umano provoca inaccuratezze tali da mettere a rischio l’utilizzabilità dei risultati. A tal proposito Andriacchi scrive: “attualmente, uno dei fattori critici che rallentano il progresso negli studi del movimento umano è la misura del movimento scheletrico partendo dai marcatori posti sulla cute” (Andriacchi et al. 2000). Relativamente alla morfologia, essa può essere acquisita, ad esempio, attraverso l’utilizzazione di tecniche per bioimmagini. Queste vengono fornite con riferimento a sistemi di assi locali in generale diversi dai sistemi tecnici. Per integrare i dati relativi al movimento con i dati morfologici occorre determinare l’operatore che consente la trasformazione tra questi due sistemi di assi (matrice di registrazione) e di conseguenza è fondamentale l’individuazione di particolari terne di riferimento, dette terne anatomiche. L’identificazione di queste terne richiede la localizzazione sul segmento osseo di particolari punti notevoli, detti repere anatomici, rispetto ad un sistema di riferimento solidale con l’osso sotto esame. Tale operazione prende il nome di calibrazione anatomica. Nella maggior parte dei laboratori di analisi del movimento viene implementata una calibrazione anatomica a “bassa risoluzione” che prevede la descrizione della morfologia dell’osso a partire dall’informazione relativa alla posizione di alcuni repere corrispondenti a prominenze ossee individuabili tramite palpazione. Attraverso la stereofotogrammetria è quindi possibile registrare la posizione di questi repere rispetto ad un sistema tecnico. Un diverso approccio di calibrazione anatomica può essere realizzato avvalendosi delle tecniche ad “alta risoluzione”, ovvero attraverso l’uso di bioimmagini. In questo caso è necessario disporre di una rappresentazione digitale dell’osso in un sistema di riferimento morfologico e localizzare i repere d’interesse attraverso palpazione in ambiente virtuale (Benedetti et al. 1994 ; Van Sint Jan et al. 2002; Van Sint Jan et al. 2003). Un simile approccio è difficilmente applicabile nella maggior parte dei laboratori di analisi del movimento, in quanto normalmente non si dispone della strumentazione necessaria per ottenere le bioimmagini; inoltre è noto che tale strumentazione in alcuni casi può essere invasiva. Per entrambe le calibrazioni anatomiche rimane da tenere in considerazione che, generalmente, i repere anatomici sono dei punti definiti arbitrariamente all’interno di un’area più vasta e irregolare che i manuali di anatomia definiscono essere il repere anatomico. L’identificazione dei repere attraverso una loro descrizione verbale è quindi povera in precisione e la difficoltà nella loro identificazione tramite palpazione manuale, a causa della presenza dei tessuti molli interposti, genera errori sia in precisione che in accuratezza. Tali errori si propagano alla stima della cinematica e della dinamica articolare (Ramakrishnan et al. 1991; Della Croce et al. 1999). Della Croce (Della Croce et al. 1999) ha inoltre evidenziato che gli errori che influenzano la collocazione nello spazio delle terne anatomiche non dipendono soltanto dalla precisione con cui vengono identificati i repere anatomici, ma anche dalle regole che si utilizzano per definire le terne. E’ infine necessario evidenziare che la palpazione manuale richiede tempo e può essere effettuata esclusivamente da personale altamente specializzato, risultando quindi molto onerosa (Simon 2004). La presente tesi prende lo spunto dai problemi sopra elencati e ha come obiettivo quello di migliorare la qualità delle informazioni necessarie alla ricostruzione della cinematica 3D dei segmenti ossei in esame affrontando i problemi posti dall’artefatto di tessuto molle e le limitazioni intrinseche nelle attuali procedure di calibrazione anatomica. I problemi sono stati affrontati sia mediante procedure di elaborazione dei dati, sia apportando modifiche ai protocolli sperimentali che consentano di conseguire tale obiettivo. Per quanto riguarda l’artefatto da tessuto molle, si è affrontato l’obiettivo di sviluppare un metodo di stima che fosse specifico per il soggetto e per l’atto motorio in esame e, conseguentemente, di elaborare un metodo che ne consentisse la minimizzazione. Il metodo di stima è non invasivo, non impone restrizione al movimento dei tessuti molli, utilizza la sola misura stereofotogrammetrica ed è basato sul principio della media correlata. Le prestazioni del metodo sono state valutate su dati ottenuti mediante una misura 3D stereofotogrammetrica e fluoroscopica sincrona (Stagni et al. 2005), (Stagni et al. 2005). La coerenza dei risultati raggiunti attraverso i due differenti metodi permette di considerare ragionevoli le stime dell’artefatto ottenute con il nuovo metodo. Tale metodo fornisce informazioni sull’artefatto di pelle in differenti porzioni della coscia del soggetto e durante diversi compiti motori, può quindi essere utilizzato come base per un piazzamento ottimo dei marcatori. Lo si è quindi utilizzato come punto di partenza per elaborare un metodo di compensazione dell’errore dovuto all’artefatto di pelle che lo modella come combinazione lineare degli angoli articolari di anca e ginocchio. Il metodo di compensazione è stato validato attraverso una procedura di simulazione sviluppata ad-hoc. Relativamente alla calibrazione anatomica si è ritenuto prioritario affrontare il problema associato all’identificazione dei repere anatomici perseguendo i seguenti obiettivi: 1. migliorare la precisione nell’identificazione dei repere e, di conseguenza, la ripetibilità dell’identificazione delle terne anatomiche e della cinematica articolare, 2. diminuire il tempo richiesto, 3. permettere che la procedura di identificazione possa essere eseguita anche da personale non specializzato. Il perseguimento di tali obiettivi ha portato alla implementazione dei seguenti metodi: • Inizialmente è stata sviluppata una procedura di palpazione virtuale automatica. Dato un osso digitale, la procedura identifica automaticamente i punti di repere più significativi, nella maniera più precisa possibile e senza l'ausilio di un operatore esperto, sulla base delle informazioni ricavabili da un osso digitale di riferimento (template), preliminarmente palpato manualmente. • E’ stato poi condotto uno studio volto ad indagare i fattori metodologici che influenzano le prestazioni del metodo funzionale nell’individuazione del centro articolare d’anca, come prerequisito fondamentale per migliorare la procedura di calibrazione anatomica. A tale scopo sono stati confrontati diversi algoritmi, diversi cluster di marcatori ed è stata valutata la prestazione del metodo in presenza di compensazione dell’artefatto di pelle. • E’stato infine proposto un metodo alternativo di calibrazione anatomica basato sull’individuazione di un insieme di punti non etichettati, giacenti sulla superficie dell’osso e ricostruiti rispetto ad un TF (UP-CAST). A partire dalla posizione di questi punti, misurati su pelvi coscia e gamba, la morfologia del relativo segmento osseo è stata stimata senza identificare i repere, bensì effettuando un’operazione di matching dei punti misurati con un modello digitale dell’osso in esame. La procedura di individuazione dei punti è stata eseguita da personale non specializzato nell’individuazione dei repere anatomici. Ai soggetti in esame è stato richiesto di effettuare dei cicli di cammino in modo tale da poter indagare gli effetti della nuova procedura di calibrazione anatomica sulla determinazione della cinematica articolare. I risultati ottenuti hanno mostrato, per quel che riguarda la identificazione dei repere, che il metodo proposto migliora sia la precisione inter- che intraoperatore, rispetto alla palpazione convenzionale (Della Croce et al. 1999). E’ stato inoltre riscontrato un notevole miglioramento, rispetto ad altri protocolli (Charlton et al. 2004; Schwartz et al. 2004), nella ripetibilità della cinematica 3D di anca e ginocchio. Bisogna inoltre evidenziare che il protocollo è stato applicato da operatori non specializzati nell’identificazione dei repere anatomici. Grazie a questo miglioramento, la presenza di diversi operatori nel laboratorio non genera una riduzione di ripetibilità. Infine, il tempo richiesto per la procedura è drasticamente diminuito. Per una analisi che include la pelvi e i due arti inferiori, ad esempio, l’identificazione dei 16 repere caratteristici usando la calibrazione convenzionale richiede circa 15 minuti, mentre col nuovo metodo tra i 5 e i 10 minuti.

Abstract Nowadays, the locus of innovations is in research and development (R&D) networks formed by companies, research agencies, universities, and governmental agencies. Innovation is seen as a source of success of corporations, nations and communities, and therefore there is a long tradition for R&D research and innovation research. Networking is the answer for the changed environment caused by globalisation, fragmentation of the knowledge base and specification of the research into several fields that cause increased financial resource and capability demand for the actors developing new offerings. Collaboration in R&D networks is necessary because resources, expertise, and capabilities are not possessed and capable of being managed by single actors. R&D networks may emerge from among willing actors or intentionally by active actors or based on existing social or strategic relationships of actors. This study focuses on intentional R&D nets and managing in these R&D nets. This study describes and understands, firstly how managing in net is conducted during R&D net formation and collaboration, secondly what benefits are achieved, and thirdly what challenges are noted in relation to managing in R&D nets. Managing in R&D nets is seen to consist of managerial processes and roles for managing. Managing in R&D nets is studied in two phases, firstly empirically in four (4) peer-reviewed papers. The papers stem from a single longitudinal case study of managing in an R&D net. The case study includes both a historical and a follow-up time perspective. Secondly, as the studies have been published between 2005 and 2008, their results needed to be compared to more novel and contemporary findings to show the contribution. Hence, in the second phase, a systematic literature review covering studies on R&D and innovation network management published between 2004–14 is conducted. Thereafter and based on the comparison to contemporary research, the findings of this study that are supported, contradicted and/or still novel are discussed. The novel findings of this study are the following. Firstly, this study extends the time frame of managing in R&D nets. Secondly, it proposes a process model for understanding R&D net formation and collaboration, based on overlapping, simultaneous and iterative activities in R&D net. Thirdly, as a novel finding, this study creates a conceptual framework for depicting and typologising roles for managing in R&D nets. Finally, this study extends the understanding of benefits and challenges of managing in R&D nets. This study provides recommendations both for managers as well as for research of managing in R&D nets Tiivistelmä Innovaatiot syntyvät yritysten, yliopistojen, tutkimuslaitosten ja julkisten toimijoiden verkostoissa. Innovaatiot ovat yritysten, yhteisöjen ja kansojen menestyksen lähde, joten niiden tutkimuksella on pitkät perinteet. Abstract:

This thesis aims to illuminate the economic and social world of the Glasgow-West India merchants, planters and the temporary economic migrants who travelled across the Atlantic during the period, 1776-1846. The city of Glasgow and her satellite ports was the premier Scottish transatlantic hub with connections across the British Atlantic world. This thesis has focused on the period after the American War of Independence ended the city of Glasgow’s tobacco monopoly. Thus, the rise to prominence of the city’s West India elite is assessed as well as the social, political, financial and commercial networks that underpinned their rise. This thesis offers new insights on religious affiliations of the merchants of Glasgow and traces the exportation of Presbyterianism to Jamaica in 1814. This thesis has implications for other aspects of the incipient Scottish-Atlantic historiography. In particular it contributes to T.M. Devine’s recent view that Caribbean slavery made Scotia great. However, this thesis is deliberately placed into a British-Atlantic context. Although this research demonstrates how a distinctly Caledonian operation promoted the flow of capital to Scotland, the ‘Glasgow West India interest’ themselves were part of a wider international network which in turn dictates the scope of this thesis and the historiography with which it engages. Specifically, this body of research traces direct investments of capital by West India merchants into Scottish industry and land, thus providing qualified support for Eric Williams’ main thesis in Capitalism and Slavery. However, this work goes significantly beyond the work of Williams to trace the connections between commerce and banking institutions in Scotland and the plantations of the West Indies. This thesis has examined in some detail the political activities of the Glasgow West India Association from inception in 1807 up to 1834. The Association’s sophisticated operations at a national and regional level supported the exploitative activities of the Glasgow-West India elite. Indeed, this research demonstrates that the members of the Association collected the bulk of the compensation awarded to individuals resident in Glasgow on the emancipation of slavery in 1834. This thesis has adopted a transatlantic approach that connects Scotland and the West Indies. In particular, these connections are illuminated through the prism of the careers of the young Scotsmen who sojourned to Jamaica and Grenada in particular. This thesis suggests there were increasing levels of emigration to the West Indies in this period and the skilled and educated young men sought economic opportunities not available at home. By examining wealth repatriation in life and post-mortem property transmission strategies, this thesis offers a revision on the view that such young men struggled to repatriate colonial profits. This has implications for the work of Alan Karras and others. The transatlantic approach is developed in case study examinations of Glasgow-West India merchant houses. This connects Scottish banks, commerce and industry with the British Parliament and the planters of the West Indies. The world of Scottish planters, merchants and sojourners is now becoming increasingly well known. The life, wealth and legacy of the Glasgow West India elite traced here provide innovative insights into their living conditions and material culture. It is further argued that a West India career could propel even those of modest means into the British super-wealthy. Finally, this thesis recognises the contribution of enslaved peoples to the economic development of Scotland which will hopefully stimulate further research in a Scottish-Atlantic context.

Abstract:

The use of various biodegradable polymers for the improvement of different controlled and long-lasting drug release systems is an active research area in recent years. The application of different metal prostheses, especially titanium based ones, to the human body is also very common. A most important disadvantage of these prostheses is the risk of infection at the application areas that necessitates the removing of the prosthesis with a second surgical operation and reapplication of it after recovery. One of the best ways to solve this problem is to render metal prostheses infection free with controlled and sustainable drug (antibiotic) release systems. The long term sustained release of relevant antibiotics from the various biodegradable polymer coated metal implants is studied in this thesis. Virtual fatigue analysis and drug loading capacities of titanium and stainless steel samples with different surface pattern and modifications were studied. Various biodegradable polymer and drug combinations were examined and used for coating of metal prosthesis. The aim is to design polymer-drug coated metal implants that are capable of releasing a feasible amount of drug up to a period of at least 1 month. Various coating techniques and surface modifications were also employed to improve the adhesional properties of the drug containing polymers. Their adhesion abilities on the metal substrates were tested by Lap-shear and T-peel tests. Polymer degradation kinetics was followed by viscosity studies. Calibration lines for different drugs were obtained and drug releases on different systems were followed by using UV spectroscopy and microbial antibiotic sensitivity tests. Among the techniques applied to prevent fast release of drugs initially, the coatings of Vancomycin absorbed &amp #946 -TCP (&amp #946 -tricalcium phosphate) homogeneously distributed in poly(D,L-lactide-co-glycolide) solution in chloroform followed by an inert coating with poly(L-lactide) system proved to be feasible. By this technique, initial burst release was minimized and drug release from implants lasted nearly 2 months. Multiple coatings on polymer plus drug coating layer also gave promising results. In vivo studies on dorsal muscles of native rabbits with antibiotic loaded implants gave no negative effect on the surrounding tissues with high compatibility free of infection.

Bakgrund och problem: Det som fokuseras är bedömningen av vad som är utgifter för utveckling, om dessa ska aktiveras som en tillgång i balansräkningen eller kostnadsföras löpande, och vad som påverkar redovisningen. Syfte: Syftet med studien är att öka kunskapen om och förståelsen för hur företag redovisar utgifter för FoU. Ett delsyfte är att också förklara varför de redovisar som de gör. Ett annat delsyfte med studien är att undersöka om redovisningen och bedömningen av vad som är FoU påverkas av subjektivitet. Metod: För att uppfylla studiens syfte har en kvalitativ metod med fallstudier som huvudsaklig undersökningsdesign använts. Fyra intervjuer har genomförts, varav tre med för studien intressanta företag som har möjlighet att aktivera utgifter för utveckling i balansräkningen. För att få en kompletterande bild och ett annat perspektiv genomfördes även en intervju med en revisor. Empiri och slutsats: Den främsta orsaken till hur företagen i studien redovisar är vad standarden, IFRS, säger. Med en tillräcklig och övertygande argumentation, så är det ändå möjligt att göra företagsspecifika tolkningar och tillämpningar av principerna i IFRS. Detta genom att exempelvis tillämpa försiktighetsprincipen och väsentlighetsprincipen vid bedömningar av om FoU uppfyller kriterierna för aktivering i IAS 38. Endel företag som redovisar enligt IFRS tillämpar företagsspecifika väsentlighetskriterier för materialitetsgränser som anger ett lägsta belopp för utvecklingskostnader som kan aktiveras. Dessa undre gränser lämnar trots allt ett relativt stort utrymme att inte ta upp utvecklingsprojekt i balansräkningen utan istället kostnadsföra utgifter för utveckling om det är något som företaget önskar. Ingen av de intervjuade personerna i studien anser att språkliga hinder och bristande kommunikation mellan tekniker och ekonomer, har någon inverkan på bedömningen av vad som är utvecklingskostnader eller inte och för värderingen av FoU. Background and problem: The focus lies on the decision and judgment of how to account for R & D in the entities, if it is an intangible asset or not. Aim: The aim with the study is to increase the knowledge of how entities account for R&D. One part of the aim is to also explain why they are doing it in that way. Another aim of the study is to explore if the accounting of the R&D is influenced by subjectivity. Method: A qualitative research method where case studies as the main design has been used. The empirical material has been collected through four interviews. Three of these interviews were made with persons in for the study interesting companies which had the possibility to capitalize development cost in the balance sheet. To have another picture and perspective on the situation one interview was made with an authorized public accountant. Result and conclusion: The foremost reason to how the entities in the study account for R&D is what the standard, IFRS, tells. It is possible to convince with argument to do company specific interpretations and applications of the principles in IFRS. For example this could be done thrue application of the principle of carefulness or of the principle of essential in the judgment and decision if the R&D fulfills the criteria for capitalizing, especially IAS 38, point 57 d. Some entities who account according to IFRS use company specific criterion of essential for materiality which state a lowest amount for development cost to be activated. These lower boundaries for materiality leave a relative big space to the entities not to activate development cost in the balance sheet if that is what the company wish. None of the interviewed persons in the study think that scarce communication between economist and engineers affect the judgment and decision of R&D and if the development cost is an intangible asset.

Detta kan innebära ett problem då det finns risk för variation mellan företag i tolkning och tillämpning av kriterierna. Det kan i sin tur påverka jämförbarheten mellan företagen i redovisningen av FoU. Syfte: Syftet med denna studie är att utifrån ett informationsasymmetriproblem studera jämförbarheten mellan företag i redovisning av FoU. Vi ämnar kartlägga tillämpningen av kriterierna i IAS 38 med avseende på variation mellan företag. En eventuell variation ämnar vi förklara utifrån hur företag förhåller sig till försiktighets- och matchningsprincipen. Metod: Metodvalet är en kvalitativ undersökning med djupgående intervjuer med fem företag i IT-branschen. Under intervjuerna har vi behandlat beslutsprocessen för aktivering av FoU samt deras inställning till avvägningsproblematiken mellan försiktighets- och matchningsprincipen. För att utreda studiens syfte har vi använt oss av en analysmodell som illustrerar konflikten mellan försiktighets- och matchningsprincipen med hjälp av en skala. Företagens placering på skalan gällande redovisningen av FoU har utgjort grunden för analysen. Empiri och slutsats: Utifrån det empiriska underlaget har vi funnit att företagens unika förutsättningar såsom FoU-projektens utformning, kännedom om produktefterfrågan samt projektredovisningssystem påverkar hur företagen tillämpar kriterierna i IAS 38. Företagens unika förutsättningar har lett till att företagen har ett principbeslut gällande aktivering av FoU. På så sätt påverkar principbeslutet tillämpningen av kriterierna. Att företagens tillämpning av försiktighets- och matchningsprincipen kan förklara principbeslutet har vi sett tendenser på. De är dock svaga och därför kan vi inte med säkerhet dra slutsatsen att redovisningsprinciper har en påverkan på tillämpningen av kriterierna. Resultatet från denna studie tyder på att redovisningen av FoU mellan företag inte är jämförbar på grund av variation i principbesluten. Background and problem: We have identified a room for discretion and interpretation in IAS 38 regarding the criteria for capitalization of R&D expenses. This can be a problem if there is a risk for variation in interpretation and use of the criteria, which can in turn affect the comparability between companies in the accounting for R&D. Aim: The aim with this study is to analyze the comparability between companies in the accounting for R&D from an information asymmetry perspective. We intend to illustrate the use of the criteria in IAS 38 regarding variation between companies. Our purpose is to use the prudence and matching principle to explain a possible variation. Method: The choice of method is a qualitative research through deep interviews with five companies in the IT-industry. During the interviews we have treated the decision-making process for capitalization of R&D and the companies’ attitude to the trade off between prudence and matching principle. In the study we have used an analysis model that illustrate the conflict between the prudence and matching principle as a scale. The companies’ placement on the scale regarding their accounting for R&D makes the foundation for our analysis. Result and conclusion: From our empirical results we have found that the companies’ unique conditions, like the structure of R&D-projects, knowledge of the product demand and project accounting system affect the usage of the criteria in IAS 38. The different conditions in all the companies have resulted in different policy decisions regarding capitalization. Therefore the policy decision affects the usage of the criteria. We have only seen weak trends that the prudence and matching principle can explain the policy decision in the companies. Therefore we cannot for certainty conclude that accounting principles affect the usage of the criteria in IAS 38. The results indicate that the accounting for R&D is not comparable because of the variation in policy decision.

AbstractExtensive submarine cold seep areas, i.e., the Beikang Basin and the Nansha Trough, were discovered on the southern continental slope of the South China Sea. Bottom-simulating reflections are widespread in these areas and show a close relationship to the cold seep system. High-resolution 2-D seismic data and multibeam bathymetry data have confirmed the existence of deep-routed conduits−mud volcanoes, diapirs, and gas chimneys. The geochemical characteristics of seep carbonates and headspace gas indicate that the fluid was mainly sourced from biogenic gas, with contributions from deep-rooted thermogenic gases. Additionally, negative pore water chloride anomalies and positive δ18O values (3.7‰ < δ18O < 5.0‰) of the seep carbonates provided indicators of hydrate water addition during carbonate precipitation. The negative δ13C excursion of planktonic foraminifera from the Nansha Trough indicated two methane release events, which occurred approximately 29–32 ka and 38–42 ka before present, and the driving mechanism for methane seepage in this area is possibly related to overpressure from the large sediment accumulation that occurred during sea level lowstands.

ZusammenfassungDie Kirchen sind mit einem radikalen Umbruch in der Religionslandschaft konfrontiert: Die Bevölkerungsanteile der römisch-katholischen und evangelisch-reformierten Kirche sanken in den letzten Jahrzehnten, während der Anteil an Konfessionslosen stark anstieg. Gleichzeitig haben die Kirchen mit Vertrauensverlusten zu kämpfen. Der vorliegende Beitrag zeichnet diesen Umbruch nach und umreißt die sich daraus ergebenden gesellschaftlichen und persönlichen Konsequenzen (Austrittsneigung). Dazu thematisiert er die Mitgliederbindung der Kirchen, die Rolle des Vertrauens und analysiert Umfragedaten aus den Jahren 1998, 2009 und 2018. Dabei zeigt sich: (a) Die Kirchen haben in der Vergangenheit Vertrauen verloren. Im Vergleich mit anderen wichtigen gesellschaftlichen Akteuren wird ihnen aktuell am wenigsten Vertrauen entgegengebracht. Insbesondere die Konfessionslosen gehen gegenüber den Kirchen zusehends auf Distanz. (b) Der Einsatz der Kirchen für sozial Benachteiligte wird anerkannt. Die Bedeutung der Kirchen im eigenen Leben dagegen ist stark von der persönlichen Religiosität abhängig. (c) Jedes dritte Mitglied der römisch-katholischen (38 %) sowie der evangelisch-reformierten Kirche (37 %) überlegt sich, aus der Kirche auszutreten (Austrittsneigung). Und schließlich (d): Es lassen sich deutliche Zusammenhänge feststellen zwischen den Grössen Austrittsneigung, Religiosität, konfessionelle Zugehörigkeit, Vertrauen in die Kirchen und (in geringerem Maße) Soziodemografie der befragten Personen. Das Vertrauen, welches den Kirchen (z. B. im diakonisch-sozialen Bereich) noch entgegengebracht wird, gilt es zu bewahren und falls möglich auszubauen. Ansonsten droht die Gefahr, dass sich die Trends fortschreiben und die Entfremdung zur Kirche weiterwächst.

Abstract:

Abstract The Earth-Moon system is constantly bombarded by meteoroids of different sizes. They are fragments of asteroids, comets, and major celestial bodies which provide information about the formation and evolution of the Solar System. Meteor showers have been studied for at least 50 years to construct Solar System meteoroid models which can be exploited to understand the spatial distribution of objects near the Earth-Moon system, to predict the degradation of spaceborne equipment, and to forecast large impact on Earth [1]. These models have been constructed by performing Earth-based observations of meteor showers and lunar flashes due to meteoroid impacts. As Earth-based observations are limited by geometrical, illumination, and meteorological constraints, a Moon-orbiting observatory could increase the quality and quantities of meteoroid impact detection to improve current meteoroid models. To answer these questions, the Lunar Meteoroid Impacts Observer (LUMIO) mission has been designed and is currently in development under ESA funding [2]. LUMIO is a CubeSat mission to a halo orbit at Earth-Moon L2 that shall observe, quantify, and characterize meteoroid impacts on the Lunar farside by detecting their flashes, complementing Earth-based observations on the Lunar nearside, to provide global information on the Lunar Meteoroid Environment and contribute to Lunar Situational Awareness [3, 4]. The LUMIO mission foresees two modes during its operative phase [5]: the Science Cycle and the Navigation & Engineering Cycle. The former lasts approximately 14 days and occurs when the Moon far side has the optimal illumination to perform flash detection (i.e., half of the Moon is not illuminated). The latter is defined as the complementary of the Science Cycle. During the Navigation & Engineering Cycle, the CubeSats cannot carry out scientific observations and performs reaction wheel desaturation, communication with the Earth, station-keeping, and technological demonstration. The technological demonstration performed by LUMIO is the Optical Navigation Experiment (ONX) [6]. The ONX aims at proving the feasibility of CubeSats to autonomously navigate in the cislunar environment without communication with the ground by exploiting images of the Moon. Indeed, the spacecraft-Moon range is computed by determining the apparent size of the Moon in the image [7, 8]. This piece of information is then provided to the navigation filter to determine the spacecraft state [6]. The computation of the spacecraft-Moon range is determined by the image processing (IP) algorithm which determines the Moon limb location in the image at subpixel precision [9] and estimates the spacecraft-Moon range with a non-iterative method [10]. To make the algorithm robust to possible outliers in the limb location points, the RANSAC algorithm is used [11]. This IP pipeline provides position measurements to the navigation filter by exploiting attitude information from the ADCS subsystem. The navigation filter is an Extended Kalman Filter (EKF) in ECI J2000 taking into account the Moon and Earth gravitational attraction and the Sun third-body perturbation. This work presents the current status of the ONX by focusing on the image processing (IP) algorithm, the navigation filter, and the expected results during the experiment. Introdução: As hepatites virais são infecções silenciosas, o que dificulta seu diagnóstico. Os principais agentes etiológicos são os vírus do tipo A, B, C ou D e sua identificação é fundamental para o tratamento e prognóstico do paciente, além de contribuir na implementação de políticas públicas de saúde coletiva que visam diminuir os óbitos que atualmente atinge cera de 1,4 milhões de pessoas no mundo. Objetivo: Identificar a prevalência de hepatite no Pará, o principal município acometido e a principal cepa viral diagnosticada nos casos notificados no período de 2016 a 2018. Material e métodos: trata-se de um estudo epidemiológico descritivo. A análise foi realizada por meio de consulta à base de dados do Sistema de Informações de Agravos de Notificação (SINAN) disponibilizada pelo Departamento de Informática do Sistema Único de Saúde (DATASUS). Resultados: foram notificados 2.448 casos de hepatite no estado do Pará. O ano de maior incidência foi 2017 (n=934). Em 2018 houve o menor número de notificações com uma redução de 36% (n=601) em relação ao ano anterior. Das 21 cidades registradas, o município de Belém liderou o número de absoluto de casos nos 3 anos analisados: 2016 37,8% (n=389), 2017 44,21% (n=419), 2018 23,30% (n=140). A faixa etária mais acometida foi de 20-39 anos, representando 38,20% dos casos. A hepatite B foi a doença mais diagnosticada pelos testes anti-HBc IgM (n=51) e HBsAg (n=881), correspondendo a 37,9% (n=932), seguida pela hepatite C, diagnosticada pelo teste anti-HCV, que representou 30,8% (n=754). Conclusão: Nos 3 anos analisados a cidade de Belém obteve o maior número de notificações de hepatites virais, principalmente hepatite B, na faixa etária de 20-39 anos. Estes dados são importantes para auxiliar a aplicação das ações de saúde pública voltadas para vigilância, prevenção e controle das hepatites virais no estado do Pará.

Limited information exists about enrollment in Part D prescription coverage by Medicare beneficiaries with cancer. Part D coverage may increase access to medicines. This study evaluated patterns of Part D uptake and costs and assessed the effects of coverage on hospitalizations and emergency department (ED) use among people with colorectal cancer (CRC). We analyzed Surveillance, Epidemiology, and End Results (SEER)–Medicare linked data on fee-for-service (FFS) Medicare beneficiaries with at least 36 months of follow-up who were diagnosed with CRC at any point from January 2007 through December 2010, and a matched cohort of beneficiaries without cancer. Dual (Medicare/Medicaid) enrollees were excluded because they are automatically enrolled in Part D. Among beneficiaries with CRC (n=12,774), 39 percent had complete Part D coverage, defined as coverage in the diagnosis year and 2 subsequent years; the rate was 38 percent in the matched comparison cohort (P=.119). Among those with complete Part D coverage, there was no significant difference in annual prescription drug costs between people with CRC ($3,157, 95% confidence interval [CI]: $3,098–$3,216) and without ($3,113, 95% CI: $3,054–$3,172). Among people with CRC, odds of ED use ranged from unchanged to marginally higher for those with no or partial Part D coverage, (adjusted odds ratio: 1.09, 95% CI: 1.00–1.18), compared with those with complete Part D coverage. Lack of continuous Part D coverage was associated with more ED use among Medicare FFS beneficiaries with CRC in 2007–2013. Among people with Part D coverage, prescription drug costs varied little between those with CRC and matched beneficiaries without cancer.
