Abstract:
Purpose The purpose of this paper is to explore the relationship between organizations and their PR firms during crisis situations. It contributes to the field by identifying the role of communication consultants in contemporary organizational crises, tasks they are entrusted by their clients, as well as providing their perspective on current crisis communication practice and its future development. Design/methodology/approach The research is based on 13 semi-structured in-depth interviews with two groups of senior staff in 11 Croatian PR agencies: CEOs, directors, managing partners and senior consultants, all in charge of their clients’ crisis communication projects. Findings The research results suggest that PR firms define crisis differently than their clients, who tend to consider every risk a crisis, which causes a significantly broadened scope of work for their PR firms. The findings also suggest crises to be periods when new PR firm–client relations are often established and caution PR firms to balance between openly expressing their opinion when unfavorable for their clients and providing the best advice possible to achieve a sustainable business model with the clients that they consult. Originality/value This research provides rare insight into crisis communication consulting practice, especially consultant–client relations during crises. Methodologically, it includes a representative group of senior communication practitioners acting as consultants and can provide the management of PR firms and scholars valuable insight into the current and future trends of the crisis communication field in Croatia.

Aims and MethodThe working relationship between consultant psychiatrists and crisis resolution/home treatment (CRHT) teams varies quite widely. Data from the national survey have been used to investigate the effects of consultant psychiatrist intput upon functions of the CRHT team. Logistic regression was employed to consider the effects of team size, team maturity and consultant input upon gate-keeping and fidelity to model (how many of six criteria teams' activities included).ResultsThere were statistically significant effects of size and maturity upon fidelity, and of maturity and consultant input upon gate-keeping.Clinical ImplicationsThe relationship between the consultant psychiatrist and other elements of the acute care pathway is an important determinant of how it functions. Depending upon how they relate to them, consultants can assist or hinder a team's capacity to fulfill their intended purposes.

Aims and MethodThe UK Government is promoting three types of specialist team in psychiatry: assertive outreach, crisis resolution and early intervention in psychosis. Policy guidance suggests that psychiatrists be recruited to work exclusively within these teams, but little is known about the views of psychiatrists regarding their development. A postal survey was undertaken to seek the views of consultant psychiatrists in the North West.ResultsSeventy per cent of psychiatrists responded to the questionnaire. Equal numbers agreed and disagreed with the development of specialist roles. Few services had been able to recruit to extra consultant sessions within the new teams and only a third of consultants believed the resources so far available to be reasonable. Overall views of the new teams were positive (mean scores 6.36, 6.51 and 6.03 on a 1–10 visual analogue scale for assertive outreach, crisis resolution and early onset psychosis teams). Consultants are particularly likely to believe that the new teams will increase patient satisfaction and provide a welcome change in role for some psychiatrists. A total of 64% of consultants believe that crisis resolution services could reduce hospital admissions, compared with 41% for assertive outreach and 31% for early onset psychosis teams. The concern most often voiced was that new services are being developed at the expense of existing teams.Clinical ImplicationsConsultants perceive benefits associated with the new teams but are concerned about their impact on the rest of the organisation. If resource and recruitment issues can be addressed, consultants could prove to be supportive of these new models of service.

ObjectiveAcademic pathology is facing a crisis; an ongoing decline in academic pathology posts, a paucity of academic pathologist’s in-training and unfilled posts at a time when cellular pathology departments are challenged to deliver increasing numbers of molecular tests. The National Cancer Research Institute initiative in Cellular & Molecular Pathology commissioned a survey to assess attitudes of cellular pathology consultants towards research in order to understand barriers and identify possible solutions to improve this situation. As cellular pathology is encompassing an increasing number of diagnostic molecular tests, we also surveyed the current approach to and extent of training in molecular pathology.MethodsThe survey was distributed to all UK-based consultant pathologists via the Pathological Society of Great Britain & Ireland and Royal College of Pathologist networks. Heads of Department were contacted separately to obtain figures for number of academic training and consultant posts.Results302 cellular pathologists completed the survey which represents approximately 21% of the total cellular histopathology workforce. Most respondents (89%) had been involved in research at some point; currently, 22% were undertaking research formally, and 41% on an informal basis. Of those previously involved in research, 57% stopped early in their consultant career. The majority of substantive academic posts were Professors of which 60% had been in post for >20 years. Most respondents (84%) used molecular pathology in diagnostic work, independent of where they worked or the length of time in post. Notably, 53% of consultants had not received molecular pathology training, particularly more senior consultants and consultants in district general hospitals.ConclusionsThe survey reveals that the academic workforce is skewed towards senior individuals, many of whom are approaching retirement, with a missing cohort of ‘junior consultant’ academic pathologists to replace them. Most pathologists stop formal research activity at the beginning of a consultant career. While molecular pathology is an increasing part of a pathologist’s workload, the majority of consultant cellular pathologists have not received any formal molecular training.

This paper surveys perception bias regarding the timing of proactive consultant engagement. It aims to improve efficiency and effectiveness of consultant engagement. Research is based on the primary dataset from South Eastern Europe and secondary dataset which is used for comparison as the real lifecycle phase of consultant engagement. Adizes lifecycle model provided our theoretical framework. After data were checked for normality t-test was used for hypotheses confirmation/rejection. T-test confirmed existence of negative bias in early lifecycle phases, one­sample t(118) =-16,232, p = 0.000. Significant differences were found for bias of early phases in organisations founded less than 25 years ago (M= -0.301, SD=0.22) and organisations founded more than 25 years ago (M= -0.396, SD=0.24); t(110)= 2.072, p=0.041; also, absolute bias in organisations founded less than 25 years ago(M=1.32, SD=0.27) and organisations founded more than 25 years ago (M=1.20, SD=0.31) is significantly different t(110)= -2.204, p=0.030. Significant differences were not found between early lifecycle stages bias of consultants (M= -0.374, SD=0.26) and users of consulting services (M -0.368, SD=0.22); t(117)= 0.475, p=0.636 but absolute bias in organisations with less than 25 years of chronological age (M=1.32, SD=0.27) and organisations with more than 25 years of chronological age (M=1.20, SD=0.31) is significantly different t(117)=-1.348, p=0.180. This research finds strong bias in estimation of lifecycle phase as time for engagement of proactive consulting services. The timing for consultant engagement is not often correctly estimated by the users of the service, so those services can be engaged as preventive before issues arise in organisation and become crisis at the tip of the organisational iceberg. Main limitations of our research are that we do not have true random sample and that data is from South Eastern Europe which limits wider generalization.

The main segmental areas of activity of a consultant, invited by the head of the company for a fundamental correction of the organization of management in the context of the transition to digital format have been analyzed. The previous experience in management consulting has been rethought for application in modern, digital conditions for improving organizational management. This issue is extremely relevant for the entire consulting community, which is currently experiencing a serious crisis. The article has been addressed, first of all, to colleagues from the National Guild of professional consultants, who are looking for new methodological approaches, developing packages of new consult-competencies and new options for establishing contacts with potential customers. The previous developments have been adapted, namely: visual digital models for clearer visual support of the negotiation processes in business communication with the Customer. In the first digital model a general consulting approach to understanding management in a crisis and post-crisis period of organizing management in the process of mastering digital features of management relations has been introduced. In the second digital model, the positions of transition from a non-digital to digital format have been specified. In the third digital model the meaningful positions of the digital format in management consulting have been demonstrated. In the fourth digital model, the main contradiction in the organization of management has been identified, which must be submitted to the Customer for a final understanding of the situations, that arise. All four digital models are interrelated and help deeply understand the problems of the transition of the company from one state to another. These models “visualize” the negotiation process with the Customer, helping to quickly establish business contacts, as well as significantly reducing the time of the negotiation process.

Reactions to trauma/crisis in the workplace may have far reaching repercussions on the emotional as well as financial status of an organization. It is imperative to address these issues and situations as they occur. Human resource, employee assistance, and occupational health professionals should: seek out and receive training in the area of managing trauma/crisis in the workplace; take responsibility for formulating policy and procedures with management; promote training programs that cover this topic for the organization; and use resources and other professionals that may have more/other experience in this area. If individuals feel that their training is limited, they may want to contact an outside consultant to conduct the debriefmgs. However, the in-house staff will need to remain active and involved long after the outside consultant has left. Resolution of a crisis/trauma/disaster is not an event, but rather an ongoing process.

SummaryPsychiatry recruitment in the UK is in crisis. In this paper we review reasons and solutions for the current predicament, focusing on the UK situation. We assert that there are specific national issues over and above more general and well-established ones, such as stigma and bad-mouthing, which need to be considered. These include factors that are an unintended consequence of recent changes in postgraduate training, as well as the organisation of the National Health Service. We conclude with some suggestions for psychiatrists, whether trainee or consultant, to help address the situation.

Purpose The purpose of this paper is to contribute to the literature on management and communication consulting in general, and crisis consulting in particular, by investigating how public relations and communications firms understand crises, crisis management, and crisis communication; how they see themselves practicing crisis consulting; and how they envisage the future of this field of expertise. Design/methodology/approach The study is based on 12 semi-structured in-depth expert interviews with CEOs, board members, senior consultants, and/or partners representing national and international public relations and communications firms operating in Denmark who offer crisis consulting as one of their areas of expertise. Findings Findings demonstrate that crisis consulting is a field in transition, moving away from the traditional focus on image crises in the media handled by former journalists, toward a new focus on issue crises involving other types of stakeholders. Findings also demonstrate that crisis consultants generate important insights into aspects of crisis management and crisis communication hitherto neglected by academic scholars. Practical implications The insights into the professional “world view” of crisis consultants will contribute to the professionalization of the field. Originality/value This is the first major study of crisis consulting as a specific field of expertise within the growing industry of management and communication consulting.

As the risk society has evolved, crisis management has become significant in order to survive a crisis. Crisis consultants have an important role in the risk society and organizations expects a whole lot from the profession. There is a gap in the research about the perceptions of crisis consultant’s role and expectancies in the risk society. Additionally, there is no research connected to the impact of the ongoing Covid-19 pandemic on the profession of crisis consulting. The study aims to explore the crisis consultants view of and their role in the risk society. It also seeks to investigate how the crisis consultants have adapted to the Covid-19 pandemic. The study has been conducted through a qualitative case study consisting of various crisis consultants from diverse consultancies. The primary data consisted of eight semi-structured in- depth interviews. Crises in the risk society has become more complex due to the developing technology which has had an effect on the dispersion of information and personal exposures. Crisis consultants are expected to act as change agents who provides an outside perspective and pushes the client forward during crises. The Covid-19 pandemic has had no effect on the types of crisis and neither on the processes used by the crisis consultants. The shift lies within tools and certain procedures where social distancing and digitalization has become the new standard. Although the alterations have been challenging in some situations, the crisis consultants have still managed to fulfill their tasks and increase their workload.

Problem  Crisis management has never been as important as it is today. Considering that criticism is being spread in a fast pace through social media, the reputation of an organisation can quickly be damaged. Several organisations however lack internal knowledge about crisis management. The majority of the previous research about crisis management has been conducted from the organisations’ perspective. There is however relatively little research done from an external actor’s role in supporting an organisation in crisis. Purpose  This study aims to contribute in the crisis management and communication field by investigating how an external actor can support an organisation in crisis. Method  The study was conducted through a qualitative single case study of crisis management support provided to firms by one external actor, a PR-agency. Primary data for the case was collected from six semi-structured interviews. Conclusion  External actors are a suitable helping hand in crises because they possess experience and knowledge in media and crisis communication and can view the situation objectively. Crisis consultants support their clients by providing a response strategy that is built on being honest and open. Proactive and post work are not prioritised by clients but are highly important for crisis preparedness and building a strong reputation. Social media is a standard part of today’s crisis communication but is also a demanding channel that creates incentives for hiring external support.

Introducción: La agresión sexual es un acto de violencia, está tipificada como un delito y su origen se halla en una estructura social basada en la noción de dominación masculina. La Asamblea Mundial de la Salud, en 1996, declaró la violencia uno de los principales problemas de salud pública en todo el mundo. Con esta premisa, junto a una serie de estrategias de coordinación entre las instituciones y servicios públicos, como el sanitario, el social, el policial, el educativo y el judicial, se pretende el abordaje de la violencia de género en Barcelona. El resultado de tal estrategia es la centralización de la asistencia sanitaria a mujeres que han sufrido una agresión sexual en un hospital concreto de dicha ciudad, de forma urgente y ambulatoria. Todo ello lleva a la comprensión de la agresión sexual como fenómeno social, y como tal, susceptible de ser analizado desde la perspectiva antropológica. Objetivos: Pretendemos establecer si se produce un proceso de medicalización de la agresión sexual por el hecho de atender en un centro de salud (mental) a mujeres que han sido agredidas sexualmente, indagar en la conceptualización de las informantes sobre la agresión sexual y determinar si el tipo de relación profesional-paciente, entendida como una forma de interacción social, tiene repercusiones morales, emocionales y/o terapéuticas en las mujeres. Metodología: hemos utilizado una metodología cualitativa, principalmente la observación etnográfica, la observación participante, la observación enfocada dentro de la consulta individual y en la terapia grupal y la realización de entrevistas en profundidad a pacientes y profesionales. La toma de notas de campo y el uso de grabadora para las entrevistas en profundidad han dado soporte a la observación. Resultados: se establece un proceso de medicalización de la agresión sexual a través de los dispositivos asistenciales de salud. Simultáneamente, se produce un grado de medicalización/desmedicalización a nivel de la interacción profesional-paciente definido por una doble conceptualización de la agresión, un problema social y de salud, y de la noción en la que basan su relación, la confianza, entendida como una intersección entre el tipo de confianza propia de los sistemas expertos y de la relación entre individuos. Esta forma de interacción contribuye a la reasignación de la culpa desde las mujeres víctimas de agresión sexual a los agresores. Finalmente, y en relación a la conceptualización de las mujeres sobre la agresión, destaca la relevancia de la sexualidad en su identidad. Su invasión supone una rotura de sus vidas.
INTRODUCTION: Sexual assault is an act of violence and it is typified as a crime. The roots of this form of violence can be tracked back to a social structure which is based on masculine domination. In 1996, the World Health Assembly declared violence as one of the main public health problems worldwide. Based on that premise, a whole range of coordination strategies have been implemented in Barcelona between several institutions from the health, social, police, education and judicial services. As a result, women who have suffered any form of sexual assault can be attended to in a centralized way from a single hospital, both in the emergency and outpatient services. The above described strategy leads to an understanding of sexual assault as a social phenomenon which is, as such, susceptible to be analysed from an anthropological perspective. OBJECTIVES: This thesis has three main aims. First, it seeks todiscern whether sexual assault is being medicalized or not, as women are being visited in a mental health centre. Second, it aims to find a deeper understanding of the women’s own conceptualisations of the assault. Third, it looks at the moral, emotional and/or therapeutic outcomes of the professional-patient relationship, which is understood as a form of social interaction. METHODOLOGY: In order to achieve the research aims, a qualitative approach has been used, based on ethnographic and participant observation during both the patients’ individual consultations and in their group therapy sessions. Furthermore, the conduction of in-depth interviews with patients and professionals has been undertaken. Extensive note-taking during the fieldwork period and the use of a tape-recorder for the in-depth interviews have been crucial to support the observations. RESULTS: A medicalization process of the sexual assault has been established through the healthcare delivery facilities. Concurrently, there is a degree of medicalization/demedicalization at the level of the professional-patient interaction. This is defined by a double conceptualisation of the assault as both a social and health problem, as well as by the notion of trust, on which the professional-patient interaction is based. Trustin this setting is understood to be an intersection between the type of trust of the expert systems and that which belongs to the relationship between individuals. This particular form of interaction contributes to the re-assignation of the guilt from the victims of the assault towards its perpetuators. Finally, and related to the women’s conceptualisation of the assault, the significance of sexuality in their identity becomes crucial. The invasion of this area, therefore, means a rupture of their lives.

O surgimento e crescente uso das redes sociais online fez com que as organizações perdessem o monopólio das mensagens difundidas sobre si. Os utilizadores veem nestas plataformas uma oportunidade de iniciarem diálogos com as organizações, que podem resultar de experiências negativas com produtos ou serviços. As organizações procuram saber o que é dito sobre si online, visando mitigar assuntos que podem evoluir para situações de crise. Com a pergunta de partida: “qual a importância atribuída pelos consultores de comunicação em Portugal à monitorização de redes sociais online na prevenção de situações de crise nas organizações?” esta investigação visa compreender a monitorização de redes sociais online como ferramenta de prevenção de situações de crise nas organizações, na perspetiva dos consultores de comunicação em Portugal. Recorreu-se ao método indutivo, realizando-se sete entrevistas semiestruturadas em profundidade a consultores de comunicação em Portugal e uma pesquisa e análise documental a conteúdos escritos, visuais, e multimédia de websites da especialidade, seguindo-se a análise interpretativa dos resultados. Através das técnicas de recolha e técnica de análise foram identificados e analisados 23 softwares de monitorização de redes sociais online. Concluiu-se que os consultores de comunicação em Portugal veem a monitorização de redes sociais online como uma importante ferramenta de prevenção de situações de crise nas organizações, ao considerarem imperativo que as organizações estejam atentas ao que é dito sobre si online.
The rise and increasing use of online social networks has caused organizations to lose the monopoly on the messages spread about them. On these platforms, users see an opportunity to initiate dialogues with organizations, which may result from negative experiences with products or services. Organizations seek to know what is said about themselves online, in order to mitigate issues that can evolve into crisis situations. With the starting question: “what is the importance attributed by communication consultants in Portugal to the monitoring of online social networks in preventing crisis situations in organizations?” this investigation aims to understand the relationship of online social networks monitoring as a tool for preventing crisis situations in organizations, from the perspective of communication consultants in Portugal. The inductive method was used, conducting seven semi-structured in-depth interviews with communication consultants in Portugal and a documentary research and analysis of written, visual, and multimedia content from specialized websites, followed by an interpretative analysis of the results. Through both the collection and the analysis techniques, 23 online social networks monitoring softwares were identified and analyzed. We concluded that communication consultants in Portugal see online social networks monitoring as an important tool for preventing crisis situations in organizations, as they consider it imperative that organizations pay attention to what is said about themselves online.

(in English) The aim of this work is to perform an in-depth analysis of the workplace of the children's crisis line - Linka bezpečí, focusing on the work of consultants with repeat callers of this line. The chosen subject was processed by a quantitative method (finding the number of calls with long-term clients on the crisis line during the six-month period, the duration of these calls, the topics that repeat callers bring to the calls on the line, etc.) and the qualitative method - organization a focus group with Linka bezpečí line's staff to find out their potential to work with long-term clients. The main result of the thesis is a summary of the findings, which make the work with the long-term calling clients easy for the workers and which, on the contrary, make it difficult, along with practical tips for practice.

The thesis Safety line workers mental hygiene deals about psychohygiene of consultants, who provide crisis intervention to children and adolescents over the phone. Topics such as mental hygiene and phone crisis intervention are elaborated in the first part of the thesis and the Safety line organization is discussed in detail as well. An empirical part of the thesis deals about types of psychohygiene which are used by the Safety line consultants. It has been discovered, that the Safety line workers use forms of psychohygiene, which the organization has to offer. A chance to discuss specific problems with shift team lead is mostly appreciated by them as well as the fact, that there are teams at the Safety line therefore consultants are not alone for solving issues. Hypotheses used for discovering the connection between socio-demografic entries and psychohygiene haven't been rejected except one. The only hypothesis, which could be rejected, was related to the lengths of practice at the Safety line and level of mental balance. The goal of this thesis was to chart methods of mental hygiene of the Safety line workers and thus to show them ways, how to nurture themselves and how to reach mental equilibrium while working for the Safety line.

As the financial crisis hit the Portuguese banks, the profitability of the whole sector squeezed to historical minimums. Reinventing the banking business model in the post-crisis context is an overriding issue to achieve sustainable profits and a low cost-to-income ratio. We propose banks to adopt a true multichannel approach, proceeding to a branch network reformatting and a relocation of services to online channels; adopt a true customer-centric approach and reduce the product and services portfolio complexity; finally, industrialize operations and rationalize the structure. With these measures in place, Portuguese banks will become leaner and more efficient, aspiring to a cost-to-income ratio below 45%.

Business failure crisis is not a new phenomenon. The competitive dynamism to which businesses are susceptible exposes free economic agents to the vagaries of the marketplace, encompassing problems of growth, sustainability, and paradoxically, failure. However, the problems are more exacerbated in small-to-medium sized enterprises (SMEs) than large businesses, which is why they have continued to experience disproportionately high failure rates. Set against this background, this chapter focuses on the role of early consultancy intervention in securing sustainable development of SMEs. It does this by analysing SME growth strategies with respect to the interface with consultants. Using the ‘embedded case study’ approach and leaning significantly on the author’s consultancy undertakings with SMEs in the UK, the chapter provides refreshing perspectives on the nature of consultancy, strategic options open to SMEs, entrepreneurial competences and characteristics, the need for consultants, and an assessment of role effectiveness in the SME sector.

The current pandemic has created new scenarios and problems regarding religious freedom. To combat the spread of the coronavirus, governments have ordered social distance and total closure of numerous activities including the celebration of sacred rites without consulting religious authorities. Religions have accepted the restrictions with a sense of responsibility, but the sacrifice of religious freedom for the faithful has been great. In addition, the effects of the pandemic together with the negative effects of globalization will continue over time, generating economic and social damage. In addition to prayer, religions have invited the faithful to a social commitment to reduce the critical issues of the crisis and specially to combat poverty. It is therefore necessary to analyze some topics: critical issues relating to the limitation of the right to religious freedom; what problems arise in the relations between powers (civil and religious); what problems arise in relations between state and religions; how the constitutional rights of the faithful and citizens are protected; what are the legal problems internal to the different religions, considering that the judgment on the validity of online rites is different; what is the role of religions in the face of the economic crisis. For the first time since the beginning of the human rights era, there has been a serious conflict between human rights, especially for the greater protection given to the right to health. The right to religious freedom also suffered, but it must be considered that the protection of the right to religious freedom also contributes to the recovery of a „good” economy, which can counteract the negative effects of the pandemic and globalization. We must build a personalist humanism, which the alliance between religions can promote. A humanism that respects the rights and dignity of man, against the logic of profit, and that rewrites the ethical rules of the economy. Looking at the post-pandemic, religions can be the soul of the ethical and moral rules that must guide the „good economy” in society to overcome social and economic differences.

Estas ideas aparecen en marco de una lucha interna con modelo previo moral e insatisfacción vital. Como antedecentes de SM, una consulta una semana antes donde acude para deshabituación y manejo de ansiedad en relación al craving. No alergias, enfermedades somáticas o antecedentes de intervenciones quirúrgicas. EXPLORACIÓN PSICOPATOLÓGICA: Consciente, orientado, colaborador y abordable. Lenguaje coherente y organizado. Ánimo bajo con disminución de las capacidades hedónicas e intenso craving. Ideas autolíticas en este contexto sin plan estructurado, pero riesgo de paso al acto con poca contención familiar. No fenómenos de orden psicótico. No alteraciones groseras en sueño o alimentación. Juicio de realidad y capacidades volitivas conservadas. Test de tóxicos positivo a BZD y AD tricíclicos. TRATAMIENTO: Se decide ingreso breve en Unidad de Hospitalización para contención.Se inicia AD, ansiolíticos y oxcarbazepina para manejo de impulsos. EVOLUCIÓN Durante su estancia en la planta (de tres días de duración) se mitigan estas ideas y se trabaja en relación a la autosuficiencia y autodeterminaciones vitales y la confrontación de vida autodestructiva como evasión. Permeable a ello y consciente de necesidad de manejo alternativo. Solicita ayuda específica para deshabituación y restablecimiento de rutina ajustada. JUICIO CLÍNICO: -Ideación autolítica, ya resuelta. -Trastorno adaptativo. -Consumo perjudicial de tóxicos (mefedrona). PLAN TERAPÉUTICO: Se deriva a centro privado de deshabituación y seguimiento posterior en Salud Mental.

A interoperabilidade de dados e informações entre diferentes Sistemas de Informação em qualquer ecossistema moderno de tecnologia é um item necessário para consultas e validações em bases de dados distintas. Essa troca de dados em ambientes governamentais se faz necessária na entrega rápida e eficaz de serviços públicos digitais para a população. Este trabalho propõe a utilização de um barramento que auxilia a interoperabilidade visando um impulso tecnológico em Governos Eletrônicos. Tal proposta de barramento, chamado IO Data Bus, se mostrou promissor através de simulações práticas utilizando na prova de conceito o barramento de troca de dados X-Road, elemento esse que proporcionou a base tecnológica da Estônia Digital. Como embasamento no nosso experimento, utilizamos o exemplo do pagamento do Auxílio Emergencial no Brasil, durante o período de crise causada pela pandemia do novo coronavírus.

Upon return to service from a refueling outage, the 1150 MW SONGS Unit 3 was at 39% reactor power when a circuit breaker fault resulted in a fire, a partial loss of AC off-site power, and a reactor shutdown. The failure of a circuit breaker to function properly resulted in the unavailability of the DC driven turbine emergency lubricating oil pump, causing extensive damage to the turbine generator. The power supply crisis in California at this time highlighted the need for an urgent understanding of the seriousness of the damage to the turbine generator, the consequences for the repair and timely return of the unit to service. Initial inspections indicated that all rotor line bearings had suffered from loss of white metal and most of the housings were distorted. Rotor journals were scored from solid contact and subject to excessive heat damage. Rotor steam seals had suffered rubbing damage. In view of the component sizes associated with such a large machine, and the need to minimize the outage duration, a large proportion of the repair work was undertaken on site. This enabled SCE to project manage the work themselves ‘in-house’. It was decided that the HP and the three LP turbine rotors would remain on site, with only the Generator rotor and the complete Exciter being shipped to the OEM’s workshops for repair. Specialist sub-contractors were employed for site machining and heat treatment. Consultants were hired for specialist technical matters, primarily related to metallurgical issues, to complement the in-house expertise of both SCE and ALSTOM. The serious nature of the damage required close cooperation between SCE and ALSTOM the OEM. It was important to understand the design margins and therefore the scope for repair, and long term integrity of the solutions. The detailed repair instructions were prepared and controlled jointly between ALSTOM and SCE engineers on site, with extensive technical back-up provided from the OEM engineering design departments. Similar close co-operation on the re-design of components and fast track supply of spare parts was crucial to the success of the project.

Introdução: A Síndrome de Ehlers-Danlos (SED) é um distúrbio hereditário do tecido conjuntivo, e sua prevalência na gravidez é estimada em 1:150.000 gestantes. A doença em geral é caracterizada pela presença de hiperextensibilidade da pele, hipermobilidade articular, luxações, fragilidade do tecido, alterações cardíacas, ruptura arterial, como ruptura espontânea, dissecção, aneurisma dissecante e até mesmo alterações oftalmológicas, como ceratocone e descolamento de retina. Por ser uma condição rara, não há ampla literatura sobre a doença e o ciclo gravídico-puerperal. Objetivo: Descrever o caso de uma paciente com SED com intercorrências de dores articulares durante o pré-natal, porém com evoluções materna e neonatal favoráveis. Material e Métodos: D.S.R., feminina, 24 anos, G2P1A0, natural do Rio de Janeiro. Apresenta diagnóstico de SED há nove anos, com hipermobilidade articular e dor, porém sem acompanhamento regular com reumatologista. Iniciou seguimento de pré-natal em um serviço de alto risco, onde realizou seis consultas. As intercorrências na gestação se deram em função das várias crises de dor articular, que cessavam parcialmente com analgesia venosa. Sem intercorrências obstétricas durante o pré-natal e o parto. Paciente foi submetida à cesariana eletiva, com 38 semanas e 5 dias. Nascimento de recém-nascido (RN) do sexo feminino, pesando 3435 g, Apgar no 1 minuto 9 e no 5 minuto 9. Puerpério fisiológico, sem intercorrências. Resultados: A SED envolve alterações hereditárias em genes que afetam a síntese e o processamento de diferentes formas de colágeno, que são importantes na estrutura de muitos tecidos e órgãos, incluindo a pele, os tendões, os ligamentos, os vasos, o esqueleto e os olhos. A base genética para a maioria dos tipos de SED foi definida, exceto para o tipo hipermóvel (que é provavelmente geneticamente heterogênea), e o teste genético pode ser útil para diagnosticar vários desses distúrbios. A paciente descrita, embora não tenha o teste genético confirmatório, provavelmente possui a SED tipo III, pelas características de hipermobilidade articular, dor articular e luxações. A escolha pelo parto cesáreo foi por conta das crises de dor articular apresentadas pela paciente durante o pré-natal. Complicações obstétricas podem ocorrer, como ruptura prematura de membranas ovulares e prematuridade. A gravidez pode ser o primeiro evento com risco de complicações nessas mulheres, principalmente a rotura uterina, as rupturas intestinais e, com mais gravidade, as rupturas vasculares. Conclusão: As pacientes com SED devem ser encaminhadas para unidade de pré-natal de alto risco, preferencialmente em hospital terciário com suporte multidisciplinar em razão do risco potencial de complicações graves. O aconselhamento genético tem papel fundamental por permitir melhor caracterização do tipo de SED e orientação quanto aos riscos de perpetuação da doença na futura prole. É de grande valia a identificação do tipo de SED para manejo obstétrico seguro e adequado.

Abstract:
Abstract Objectives/Scope The world is changing rapidly. In order to response to VUCA conditions, we started the business transformation of GPN scientific and technical center (STC) in the mid 2019, and finished shortly after COVID crisis began (in June 2020). Now we can sum up the ultimate effect: Methods, Procedures, Process GPN STC made a long way since 2007 and became a strong corporate engineering center, providing a full range of services from the basin modeling to the integrated modeling. In 2019, we started the transformation from the description of objectives, and for these purpose, the survey of different business branches was performed. After that, we defined the shortcomings of the existing model (which was actually inside the organizational and administrative area), such as: The transformation met all the challenges and made dramatic changes in the area of management and processes: We transformed STC works catalogue to product line and shift from instruments (such as geomodel) to business value (such as exploration strategy, based on geomodel). Now we focus on integrated analytical framework in support of key business decisions, rather than methodsor processes.The above shifting requires new roles and competences, thus we completely rebuild role model and staffing table. We initiated new role of account manager for every internal business customer, that is typically used in service companies, not in-house centers, and here we saw fantastic results and received positive feedback from business. The new role of discipline leader helped to increase speed of team gathering for new projects from average 7 weeks to 3-4 weeks;We removed 2 levels of hierarchy and moved on to "flattened" organizational structure;We initiated the internal business consulting team, responsible for implementation all of the conceptual decisions (which will be a new service for internal business customers in future);The single-contract principle was suggested, and the orders inside of this according to product line were initiated, where account manager is the person, responsible for implementation of this contract;Product transformation was supported by organizational agile transfrormation for cross-functional teams (Mohamed Saeed AlMarar, 2019);And a lot of another important changes (which will be described in final presentation); Results, Observations, Conclusions It is important to highlight again, this deep and ambitious transformation allowed STC to obtain sustainable target indexes/KPIs and business results: Novel/Additive Information We were the first (at least in Russian O&G industry), who shifted the entire corporate engineering center for product model, with focus on business value and now we perform strong operationalization of concept in order to push all changes into real life.

Abstract:
Introdução: A pandemia provocada pelo surgimento e expansão da COVID-19 acarretou diversos problemas na sociedade brasileira, como: prejuízos financeiros, redução da prática de atividades físicas e aspectos relacionados ao convívio social, com grande impacto no âmbito psicológico dos pacientes, amigos e familiares. Objetivo: Analisar o fenômeno do isolamento social e suas repercussões no âmbito psicológico na população brasileira. Material e métodos: Elaborou-se uma revisão bibliográfica através da consulta das bases de dados Scielo, PubMed e Google Acadêmico, conforme a disponibilidade na íntegra de artigos entre os anos de 2017 a 2021. Resultados: O isolamento social é o ato através do qual os indivíduos se mantêm afastados do convívio social, sendo necessário para evitar a propagação do novo coronavírus. Estamos presenciando uma grave crise de saúde pública, em âmbito mundial, em cujo cenário destacam-se visões estereotipadas em relação à parcelas da população, como os idosos, cujo isolamento social é necessário mas que também os coloca em risco de agravos à saúde como: depressão e ansiedade. Novas dinâmicas informacionais e um preocupante negacionismo de resultados advindos de pesquisas científicas são fatores que amplificam os sentimentos de angústia, insegurança e medo na população brasileira, intensificando os níveis de estresse e amplificando sintomas de pacientes com transtornos mentais préexistentes. Conclusão: É de grande importância a avaliação do contexto social dos pacientes afetados pela COVID-19, tendo-se em vista os fatores de não adesão à medidas preventivas, bem como o uso de medicamentos de eficácia não comprovada cientificamente. O farmacêutico deve atuar, através do estímulo ao uso racional de medicamentos, e as novas tecnologias de comunicação podem ser de grande ajuda nesta missão.
