Abstract:
La stima del costo annuo di trattamento (CT paz) del paziente psichiatrico all'interno del DSM ha comportato (figura 6.1): a) la descrizione e quantificazione delle prestazioni ricevute nelle strutture delle UOP nel corso del 2000; b) la valorizzazione del trattamento attraverso la determinazione di costi medi standard delle prestazioni. Data l'ampiezza del campione si è optato per un modello di activity based costing, in cui la stima del costo del prodotto è ottenuta come somma dei costi di singole attività generate dall' organizzazione. Nel caso specifico l'organizzazione, il prodotto e le attività sono rappresentati rispettivamente dalla UOP, dal trattamento complessivo ricevuto dal paziente nel corso del 2000 e dalle singole prestazioni erogate dalle strutture della UOP.La prima fase (punto a) ha utilizzato il flusso informativo regionale Psiche per rilevare le prestazioni erogate nel 2000 per singolo paziente reclutato.

RIASSUNTOScopo – Negli ultimi anni si è assistito, in Italia come in altri paesi del mondo occidentale, ad un forte aumento dell'interesse, da parte dei ricercatori, per l'economia sanitaria. Anche l'assistenza psichiatrica è stata analizzata riguardo a questi aspetti e, più recentemente, l'attenzione di molti gruppi di lavoro si è rivolta a studi in grado di individuare possibili sistemi di finanziamento dei Servizi psichiatrici altemativi a quelli esistenti e di valutarne l'effetto sul funzionamento dei Servizi. Lo scopo di questo lavoro è quello di definire un nuovo tariffario basato sui costi reali delle prestazioni erogate dai Servizi psichiatrici. Metodo – Utilizzando il Registro Psichiatrico dei Casi di Verona-Sud, sono stati considerati tutti i contatti avvenuti nelle varie sedi del Servizio Psichiatrico Territoriale (SPT) di Verona-Sud in un periodo di 7 anni (1992-1998), per un totale di 125632 prestazioni, riferite a 2819 soggetti. I contatti sono stati successivamente raggruppati in 19 tipi di prestazioni. La funzione di costo è stata utilizzata per descrivere, attraverso la stima dell'elasticit?à, il comportamento dei costi nel SPT di Verona-Sud. Il costo di una prestazione psichiatrica risulta così comprensivo della spesa per gli operatori coinvolti nell'erogazione del servizio (direttamente e/o indirettamente a contatto con il paziente) e del costo delle strutture utilizzate. Risultati – Per ciascuna prestazione vengono riportati (a) i costi stimati per la prestazione effettivamente erogata, (b) il costo stimato al minuto, (c) il costo della prestazione standard erogata con caratteristiche definite (per la durata si è utilizzato il tempo modale rilevato nel campione; per gli operatori, la composizione è stata decisa tenendo conto, da una parte, della reale operatività del SPT e, dall'altra, di considerazioni su quella che potrebbe essere, date le risorse disponibili nel nostro SPT, una buona pratica clinica) ed, infine, (d) i costi relativi alle prestazioni effettuate, raggruppate nelle otto voci contemplate dal Tariffario della Regione Veneto attualmente in vigore. Conclusioni – I risultati hanno dimostrato che le definizioni delle prestazioni, da noi utilizzate, descrivono in maniera soddisfacentemente univoca i diversi tipi di attività svolti dal SPT. Il nomenclatore nazionale attualmente in vigore, invece, avrebbe permesso di descrivere solo il 28% delle prestazioni erogate (35230 su 125632). Ciò dimostra la necessità di disporre, in tempi brevi, di un nuovo nomenclatore delle prestazioni psichiatriche, accettato a livello nazionale, che sia adeguato alle modalita reali con le quali si fomisce assistenza per la salute mentale. I valori di costo da noi ottenuti dimostrano chiaramente che il finanziamento di un Servizio psichiatrico, basato sul nomenclatore nazionale attualmente in vigore, sottostima fortemente le risorse effettivamente utilizzate. La funzione di costo delle diverse prestazioni pud fornire la base per esperienze di finanziamento dei servizi fondato sulle "funzioni assistenziali" previste dal decreto 229 del 1999.

Lo scopo di questo studio è valutare la qualità di vita percepita dalle persone sottoposte a terapia dialitica in rapporto al costoutilità; I pazienti oggetto dello studio a Tivoli sono stati 28, mentre a Guidonia sono stati 82. In base alla ricerca bibliografica abbiamo scelto di utilizzare l'ACU (analisi costo/utilità), tecnica che massimizza il rapporto utilità/costo, due alternative (interventi tecnici) in cui l'utilità è espressa in termini fisici e, in particolare, in termini di QALYs (quality adjusted life years) cioè anni di vita guadagnati ponderati per la qualità della vita per due diverse tecniche dialitiche: emodialisi standard HD ed emodiafiltrazione HDF. Osservando i dati emersi dallo studio ci siamo resi conto che i due gruppi percepiscono la qualità della loro vita in maniera molto simile, nonostante i parametri dialitici ed ematologici, oggetto di studio, siano significativamente migliori nel gruppo in trattamento dialitico con la metodica deU'Emodiafiltrazione.

RIASSUNTOScopo – L'analisi del rapporto tra costi di trattamento e gravità dei pazienti psichiatrici, raggruppati in classi omogenee. Disegno – Rilevazione prospettica dei costi e della gravità di 1371 pazienti psichiatrici adulti, in carico presso 2 Unità Operative di Psichiatria, osservati per un periodo medio di circa 9 mesi. I dati raccolti riguardano tutti i servizi psichiatrici delle Unità Operative, che comprendono quelli ambulatoriali, quelli residenziali e semi-residenziali e i Servizi Psichiatrici di Diagnosi e Cura degli ospedali per acuti. Setting – Le Unità Operative Psichiatriche di Magenta (MI) e di Desio (MI). Metodo – La gravità di ogni paziente è stata misurata attraverso la scheda HoNOS. La compilazione di questa scheda è stata effettuata al momento del reclutamento e in seguito con una cadenza mediamente trimestrale. Oltre alle schede trimestrali sono state previste compilazioni al momento dell'ammissione e della dimissione dai reparti ospedalieri di psichiatria, dai Centri Residenziali di Terapie psichiatriche e di risocializzazione e dalle Comunità Protette. Tutti i pazienti osservati sono stati raggruppati in base alla diagnosi principale (ICD-10) e al livello massimo di gravità dei problemi presentati durante l'intero periodo di osservazione. I costi presi in considerazione sono i soli costi diretti sostenuti dai servizi psichiatrici pubblici. La loro attribuzione ai singoli pazienti è avvenuta sulla base di costi standard o di tariffe (procedure diagnostiche), applicati alle quantità rilevate prospetticamente attraverso il Registro regionale, integrato con apposite schede. Risultati – Il costo complessivo realizzato dai 1371 pazienti osservati è risultato pari a 9771.1 milioni di lire, con un costo medio per paziente di 7127000 lire (ds 19499400). Il costo totale per giornat'a di osservazione è risultato pari a 27172 lire (ds 68358) e non risulta correlato alia durata del periodo di osservazione. La gravità media dei pazienti risulta pari a 4.26 punti (ds 3.73) al momento dell'arruolamento e pari a 3.19 punti (ds 3.26) al termine dello studio. Quella media, misurata attraverso i valori massimi della HoNOS osservati sull'intero periodo, risulta pari a 6.00 punti (ds 4.64). Gravità e costo del trattamento per singolo caso risultano direttamente correlati (r = 0.626, p = 0.0001). II raggruppamento dei pazienti ha portato a definire delle classi che presentano un punteggio medio di gravità complessivamente diverso (p = 0.0001). Diversi nel loro insieme risultano anche i costi medi di trattamento per classe (p = 0.0001). Conclusioni Tutti i pazienti psichiatrici adulti possono essere raggruppati in classi relativamente omogenee rispetto alia gravità e con costi di trattamento almeno in parte diversi. Uno studio più ampio forse potrebbe migliorare i risultati ottenuti e fornire la base per una diversa modalità di finanziamento dei servizi psichiatrici.

Abstract:
The purpose of this paper is to show how to obtain a qualified system for teleradiology using with already available resources in the diagnostic room, such as a workstation used to post-process CT and MR images. Thank to a personal computer (PC), a software translating workstation CT and MR images compatible with PC standard, software working along with the File Transfer Protocol (FTP), and finally a connection with world wide web by modem and Internet provider, it was possible to send images from CT and MR devices to the workstation and from there to the PC. The images were formatted in JPG files and then compressed in ZIP files to reduce those files and to have a faster transmission. The files were sent by modem to the e-mail box of the doctor whose opinion was sought and he could return his diagnosis in the same way. Basically, the costs were represented by the PC, the modem, the yearly subscription to an internet provider and time/telephone charge.

Abstract:
Introduction: Acute Bacterial Skin and Skin Structure Infections (ABSSSIs) include all complicated skin and soft tissue infections. The aim of this study was to conduct a cost-utility analysis to compare dalbavancin with standard antibiotic therapies for the management of non-severe ABSSSIs from the National Health Service (NHS) perspective. Methods: A probabilistic decision tree model was developed considering a 30-days follow-up to simulate the therapeutic pathway of a patient treated with dalbavancin or Standard of Care (SoC). The model considered three mutually exclusive health states: a) discharge of patients from the emergency department, b) discharge of patients after one night from admission, c) discharge after 24 or 36 hours from admission. A one-way deterministic sensitivity analysis and a probabilistic sensitivity analysis were conducted. Results: The analysis showed that the use of dalbavancin in patients with non-severe ABSSSI compared to SoC could generate a reduction in costs (– € 291.6 per patient treated) and an increase in QALYs (+0.0018 per patient treated). In 99.7% of the simulations carried out, dalbavancin was dominant compared to the SoC. Considering a threshold for the willingness to pay of € 30,000 for QALY gained, the minimum level of efficacy of dalbavancin so that the treatment can be considered cost-effective compared to the SoC was equal to 69.4%. Conclusions: The analysis showed that dalbavancin may represent a cost-effective option compared to SoC for the treatment of patients with non-severe ABSSSI.

L’apnea ostruttiva del sonno (OSAS) è una malattia cronica eccessivamente sotto-diagnosticata con un’alta prevalenza negli adulti. L’OSAS sta diventando un problema sociale significativo perché associata ad un peggioramento della qualità della vita ed un aumento della mortalità. Il rapporto costo-efficacia nella gestione diagnostica e terapeutica dell’OSAS è un problema strategico per contrastare la crescente domanda di test oggettivi. I pazienti OSAS che non presentano comorbilità clinicamente evidenti devono essere studiati utilizzando un sistema semplificato e poco costoso, come l’Home Sleep Testing (HST). Al contrario, la Sleep Laboratory Polisomnography (PSG) rimane il gold standard per la gestione dei pazienti con OSAS in presenza di comorbidità. Occorre sottolineare che l’uso di HST potrebbe portare ad una diagnosi errata in soggetti OSAS non ben selezionati. Questa breve rassegna si propone di offrire argomenti di riflessione sulla corretta diagnosi e trattamento dell’OSAS, in rapporto ai dati di prevalenza e alle ricadute sui costi/benefici sociali della malattia. Attualmente non può essere solo il rapporto costo/efficacia a definire il modello organizzativo adottato per la gestione dell’OSAS, in quanto si rendono necessari ulteriori studi prospettici a lungo termine, volti a validare in maniera definitiva tale rapporto nonché il confronto tra il trattamento con modelli di gestione ospedaliera versus l’assistenza domiciliare.

Abstract:
La tesi si sviluppa in tre parti. In una prima parte viene esposto il concetto di Corporate Social Responsibility e la sua sempre maggiore rilevanza nel contesto competitivo attuale: l’evoluzione, le definizioni istituzionali e in dottrina. In una seconda parte viene analizzato il rapporto intercorrente tra performance sociale d’impresa e performance economico-finanziaria. Lo scopo è quello di esporre l’importanza che riveste il fatto che vi sia conciliazione tra i due tipi di performance perché l’impresa crei valore, in un equilibrio tendente alla massimizzazione degli interessi in gioco dei diversi stakeholder. Non sempre le imprese socialmente responsabili sono anche le imprese che ottengono performance economiche migliori. I rischi, così come i benefici ottenibili dall’essere eticamente responsabili sono numerosi; ed è necessario monitorarli costantemente. Tra le diverse metodologie di misurazione della CSP, viene rivolta l’attenzione al rating etico, e nel particolare al rating attribuito dall’Agenzia Standard Ethics. Se le imprese giudicate socialmente responsabili, e quindi in possesso di un giudizio di rating positivo, sono anche quelle che ottengono una performance finanziaria migliore rispetto alle aziende meno attente alla problematica della RSI (i benefici superano i costi), allora gli stakeholder - e nello specifico le banche che erogano il credito - dovrebbero riconoscere questo valore e la durabilità di queste aziende, in ipotesi semplificata, applicando loro un minor costo del debito. Le aziende che hanno una Corporate Social Performance migliore, ottengono una migliore performance economica? Il costo del debito è correlato al giudizio di rating etico (utilizzato come misura della RSI) attribuito all’impresa? Lo scopo dell’ultimo capitolo è quello di verificare con un’indagine empirica se vi è correlazione tra il rating attribuito dall’Agenzia Standard Ethics ad un campione di società, la performance economica ed il costo del debito.

The determination of the essential levels of the services, provided under article 117 Const., co. 2, lett. m) and the full implementation of the art.119 Const., are the two points of reference for our doctoral thesis, which, although introduced by a reform - that concerning the title V of the Constitution - which dates back more than a decade ago, have not yet been able to deploy the desired effects. The matter that makes today critical and problematic the application of the two rules is twofold: the cost of services deeply diversified in the various Regions and that of the inefficiency of some areas; issue that could find a solution both with the passage from the criterion of historic expenditure to the standard cost, and with the transit to a system of guaranteed funding for only the essential levels of services. The research reconstructs the development and recognition of social rights which took place during the time thanks to the work of the legislator and to the intervention of the Constitutional Court; and it also reconstructs the need, appeared in the legal order , to ensure more stringent budgetary constraints, which was translated into constitutional reform of the art.81. Facing this scenery, the analysis is directed to the actions that should be taken in order to ensure the financing of social benefits more effectively and more efficiently: on the one hand, the rationalization of public spending; and, on the other hand, the identification of a basket of social rights, tied up to the concept of need and to be guaranteed regardless of the resources, distinguishing them from other rights defined simple, financially conditioned, which could be satisfied by the cost-sharing of the user.

Nella parte teorica della Tesi è stato esposto il tema della produzione su commessa descrivendone i criteri di classificazione. Si è confrontato la produzione su commessa con quella in serie. Inoltre, il ciclo di vita delle commesse ha evidenziato la necessità di un controllo di tipo feed-forward rispetto a quello feed-back. Si è presentato uno degli strumenti chiave per il Project Management, la WBS ed è stata affrontata la tematica relativa ai preventivi di commessa e alla gestione di tempi e costi. Infine, si sono definiti il concetto di Costo e la sua determinazione per un prodotto facendo riferimento a diverse teorie diffuse in letteratura approfondendo quella del Cost Plus Pricing. Poi è stato esposto il lavoro pratico. Le attività svolte all’interno del progetto di tirocinio sono state le seguenti: raccolta dei dati storici relativi ai costi e alle caratteristiche parametrizzabili dei MAST, attraverso l’uso di SAP e di Enovia, e creazione di database di costo in base alla funzione tecnica sia dei MAST sia dei componenti; analisi ABC sui costi di un sistema di movimentazione per capire quali fossero gli assiemi che incidessero di più sul costo del prodotto; scomposizione degli assiemi più significativi nei loro componenti principali per la creazione di diversi database in base al tipo di assieme e di componente; elaborazione dei dati raccolti nel database tramite l’uso di Excel per determinare i principali cost driver dei componenti attraverso l’utilizzo della regressione multipla; determinazione di funzioni di costo per mostrare la forza del legame tra il costo di ogni singolo componente e le caratteristiche tecniche parametrizzabili; analisi degli scostamenti tra l’output previsto dalle funzioni di costo e quello osservato; creazione di un preventivo di costo standard configurabile in base alle caratteristiche tecniche richieste dal cliente con la finalità di formulare rapidamente un’offerta al cliente tramite compromesso con l’esperienza diretta sul campo.

il seguente lavoro di tesi è incentrato sullo sviluppo di un interfaccia grafica "user-friendly" in Python per la configurazione, acquisizione e gestione dei dati da sensore indossabile MetaMotionR, della mbient.inc. L'applicazione sviluppata è stata quindi utilizzata per andare ad effettuare test tecnici di caratterizzazione delle performance, messi a confronto con uno standard commerciale rappresentato dal device Opal, Apdm. Nello specifico, sono stati effettuati test per valutare: la reale frequenza di acquisizione, bias sul segnale in differenti configurazioni di fondoscala, ortogonalità degli assi. inoltre, sono stati effettuati test di performance sui task specifici di Normal walking e Tandem Walking, andando a valutare l'errore nell'identificazione di Heel Strike e Toe Off, e dei parametri temporali descrittori del ciclo del passo, considerando differenti casistiche rilevanti.

Dopo una prima parte in cui viene introdotto il Controllo di Gestione dandone una definizione e dimostrando la crescente importanza che sta acquisendo negli anni, questo elaborato si propone di descrivere la metodologia dei costi standard. Essi sono uno strumento di notevole importanza per un’azienda che ha necessità di analizzare e tenere sotto controllo i propri costi in quanto permette di svolgere dei confronti tra l’obiettivo che ci si era preposti (lo standard) ed il dato effettivamente ottenuto. In questo modo si può poi procedere con la ricerca delle cause che hanno fatto sorgere tali devianze e mettere in atto le opportune azioni correttive per riportare la situazione in linea con quanto previsto o per raggiungere dei nuovi obiettivi. I costi standard, inoltre, se opportunamente calcolati e periodicamente aggiornati, possono venire anche utilizzati per compiere delle valorizzazioni di alcune voci di conto economico e per valutare le performance dei manager ed il livello di efficienza della produzione. Abstract:
Based on a variety of archival sources, this paper presents estimations for cost of living and living standards for Lima, Peru during the 19th century. During this century Peru experienced deep swings in economic activity marked by the independence wars, the War of the Pacific, and a commodity boom. These new series show that a sizable inflationary period during the guano age had dampeningeffects on the living standards of the popular class. While living standards peaked by mid 1850s, GDP per capita did not do so until two decades later. These results suggest that the guano bonanza failed to lift working-class living standards above subsistence levels. Even though living standards climbed steadily, almost reaching those of England, all these gains were lost by the end of the century.
Basado en diversas fuentes primarias, este artículo presenta estimaciones del costo de vida y de los estándares de vida para Lima, Perú durante el siglo XIX. Durante ese siglo, dicho país experimentó profundas fluctuaciones en la actividad económica debido a las guerras de independencia, la guerra del Pacífico y al auge monoexportador. Estos factores alimentaron un importante proceso inflacionario durante la era del guano que tuvo efectos adversos en los estándares de vida de las clases populares.Es de notar que los estándares de vida llegaron a su punto máximo a mediados de la década de 1850 mientras que el PBI per cápita lo hizo dos décadas más tarde. Estos resultados sugieren que la bonanza del guano no logró mejorar el bienestar económico de las clases populares de manera sustentable. A pesar de alcanzar niveles similares a Inglaterra, estos beneficios se evaporaron hacia el fin de siglo.

Much has been researched and written about road pricing, and especially how it affects traffic flow, congestion, the environment, and social welfare in general. This doctoral thesis analyses the implementation of road user charges rather than its effects. The dissertation consists of five separate papers, each addressing a subset of the topic. The first paper sheds light on the costs to introduce congestion taxes in Stockholm, and describes how public opinion and political risk lead to a system design that in all likelihood was more expensive than necessary. In the second paper the European regulation on interoperability between road charging system is evaluated from a benefit/cost perspective, which is found to be overly ambitious. Paper three builds upon the second, and suggests a general model for how costs and benefits interact in the case of road charging interoperability. Inpaper four a potential solution is presented for how the practical problems related to enforcement of road charging in an international context or in countries with weak institutions. The fifth and final paper analyses the public opinion of road charges, and especially urban congestion charges, using a survey conducted simultaneously in three European cities.
Mycket har forskats och skrivits kring vägavgifter, och framför allt hur de påverkar trafikströmmar, trängsel, miljö och samhällsnytta. Denna sammanläggningsavhandling analyserar införande av vägavgifter snarare än dess effekter. Avhandlingen består av fem uppsatser som var och en tar sig an en delmängd av området. Den första uppsatsen belyser kostnaderna för införande av trängselskatter i Stockholm, och beskriver hur opinion och politisk risk ledde till en systemdesign som troligen var mer kostsam än nödvändigt. I den andra uppsatsen utvärderas den europeiska lagstiftningen för interoperabilitet mellan vägavgiftssystem ifrån ett nytto/kostnads-perspektiv, och finner den alltför ambitiös. Uppsats tre bygger vidare på den andra och föreslår en generell modell för hur kostnader och nyttor samverkar avseende interoperabilitet mellan vägavgiftssystem. I uppsats fyra presenteras en möjlig lösning på hur de praktiska problemen med beivrande av vägavgifter i ett internationellt sammanhang eller i länder med svaga institutioner kan lösas. Den avslutande femte uppsatsenanalyserar allmänhetens attityder till vägavgifter, framför allt urbana trängselavgifter, med hjälp av en enkätundersökning som genomförts samtidigt i tre europeiska städer.

Organizations that depend on the use of IT in their business models must maintain their systems and keep their systems current to survive (Filipek, 2008; Kulkarni, Kumar, Mookerjee, & Sethi, 2009; Unterkalmsteiner et al., 2012). As most IT departments allocate as much as 80% of their budget to maintain stability while leaving only the other 20% to allow improvements (Telea et al., 2010), high cost of stability may be a reason many IT organizations cannot afford efficient staffing and even jeopardize the existence of the organization (Filipek, 2008; Talib, Abdullah, Atan, & Murad, 2010). The purpose of this exploratory mixed methods study was to discover the IT programming standards used in IT departments that predict a decrease in project maintenance costs. This study employed an exploratory mixed methods data collection and analysis to develop and test a collection of universal programming standards. The qualitative portion of the study resulted in a list of IT programming standards from the Fortune 20 companies of 2011. Surveyed from IT departments in the Fortune 500 companies of 2011, the quantitative portion of this study correlate the degree of enforcement of each IT programming standard to a decrease in average project maintenance costs using a backward stepwise regression. Using a 95% confidence interval and a 5% margin of error (α = .05), the backward stepwise regression discarded 18 of the 22 IT programming standards. The remaining correlations give evidence that a) the more the department enforces waiting for feedback the higher the maintenance costs, b) the more the department enforces having the architectural team develop coding guidelines the lower the maintenance costs, and c) the more the IT department enforces the following of change management procedures, the higher the maintenance costs.

Calibration is typically performed with large, expensive standards with multiple modes of operation. These standards can be expensive to ship and difficult to move around. This makes their use in field calibration both difficult and expensive. Modular instrumentation based on standard computer buses, such as PXIe, is now available with specifications which meet or exceed those needed for many calibrations. These modular instruments are typically smaller, more flexible, less expensive, and faster than the equivalent, standalone standard. This paper will analyze several common field service use cases and show the relative merits of both traditional and modular instruments as field calibration standards.

Abstract Standard system has been developed for Nuclear Power Plants in China during the last ten years, which covers the entire lifespan of pressurized water reactors. It is mainly composed of the following parts, including general & foundation, preliminary work, designing, equipment, construction, commissioning, operation and decommissioning. Engineering economics is an important part of general & foundation section. Standards related to cost and cost management constitute engineering economics standard system, which is one of the most important subsystems in the standard system for Chinese nuclear power plants. In general, engineering economics standard system is composed of 39 nuclear industry standards, with 4 management standards, 6 specifications of bill of quantities and 29 cost quota standards. Management standard specifies the engineering management and engineering cost evaluation for the construction of nuclear power plants. For the technical part of engineering economics standard system, two kinds of pricing methods for construction projects in china should be considered: fixed quota pricing and bill of quantities, and those standards should cover the overall construction content and construction phase of nuclear power plants. This paper presents the establishment of engineering economics standard system framework and major elements in this system. The study in this paper shows a good example of the establishment of the standard system in engineering economics field for nuclear power plants in China.

This paper describes a risk-based approach for corrosion management of offshore floating structures. The objective of this approach is to reduce the risk of corrosion related failures, reduce the associated downtime, while improving the cost-effectiveness of corrosion inspection and maintenance. Corrosion is increasingly a significant challenge to the offshore industry and attributed to: an aging worldwide offshore fleet; assets being kept in operation for prolonged periods of time; units operating beyond their original “design basis”; and newer larger vessels in deeper, harsher environments with less opportunity for ship yard repair. Corrosion is particularly detrimental to the integrity of the unit, and if not managed properly will increase maintenance costs and downtime costs, possibly reducing the useful operational life of the unit. Although, there are several existing offshore corrosion design standards, experience still reveals a number of assets in poor and critical condition due to corrosion. Clearly there is a need for a holistic approach on corrosion management during the full operational life of the asset. The presented methodology is based on the principles of ISO 31000 (Risk Management - Principles and Guidelines) to provide a solid consistent framework for corrosion management. The risk-based corrosion management process for offshore structures described in this paper consists of five (5) basic steps: Pre Assessment; Screening and Risk Ranking; Detailed Examination; Remediation and Repair; and Life Cycle Management. Adopting the described risk based corrosion methodology will provide confidence to the operators and demonstrable evidence to key stakeholders that corrosion is being managed on their assets. It will account for life extension, reduce the risk of corrosion failure, and lower the cost of inspection and maintenance. Paper published with permission.

This paper describes an optimization procedure of the heat exchanger design, which is based on cost effectiveness. In the procedure, a genetic optimization algorithm varies the heat exchanger geometry and operating conditions. The influence of the heat exchanger design on its costs, heat transfer and friction performance has been evaluated economically by determining investment and operation costs of the system. For this purpose, a specific approach for setting up of cost functions of the heat exchanger, fans and pumps is presented as well. It has been shown on a non-standard application, that the heat transfer costs of the optimized heat exchanger design is 30% lower than that of the reference configuration, which was designed by a heat exchanger producer.

Hundreds of millions of people worldwide are exposed to arsenic-contaminated drinking water, leading to significant health complications, and social and economic losses. Currently, a wide range of technologies exists to remove arsenic from water. However, despite ongoing research on such technologies, their widespread application remains limited. To bridge this gap, this review aims to compare the effectiveness and costs of various arsenic remediation technologies while considering their practical applicability. A search conducted using the Medline and Embase databases yielded 31 relevant articles published from 1996 to 2018, which were categorized into laboratory and field studies. Data on the effectiveness of technologies in removing arsenic and associated costs were extracted and standardized for comparison as much as was possible, given the diversity of ways that studies report their key results. The twenty-three (23) technologies tested in laboratory settings demonstrated efficiencies ranging from 50% to ~100%, with the majority reaching relatively high removal efficiencies (>90%). Approximately half achieved the WHO standard of 10 µg/L. Laboratory studies used groundwater samples from nine (9) different countries – Argentina, Bangladesh, Cambodia, China, Guatemala, India, Thailand, the United States, and Vietnam. The fourteen (14) technologies tested in the field achieved removal efficiency levels ranging between 60% and ~99%, with ten (10) attaining above 90% removal efficiency. Of these, only five (5) reached established the WHO standard. Some of the technologies under-performed when their influent water contained excessive concentrations of arsenic. Only six (6) countries (Argentina, Bangladesh, Chile, China, India, and Nicaragua) were represented among the studies that implemented and tested technologies in the field, either at household or community level. For technologies tested in the laboratory, the cost of treating one cubic meter of water ranged from near-zero to ~USD 93, except for one technology which cost USD 299/m³. For studies conducted in the field, the cost of treating one cubic meter of water ranged from near-zero to ~USD 70. Key factors influencing the removal efficiencies and their costs include the arsenic concentration of the influent water, pH of the influent water, materials used, the energy required, absorption capacity, labour used, regeneration period and geographical location. Technologies that demonstrate high removal efficiencies when treating moderately arsenic-contaminated water may not be as efficient when treating highly contaminated water. Also, the lifetime of the removal agents is a significant factor in determining their efficiency. It is suggested that remediation technologies that demonstrate high arsenic removal efficiencies in a laboratory setting need to be further assessed for their suitability for larger-scale application, considering their high production and operational costs. Costs can be reduced by using locally available materials and natural adsorbents, which provide near zero-cost options and can have high arsenic removal efficiencies. A notable feature of many arsenic removal approaches is that some countries with resource constraints or certain environmental circumstances – like typically high arsenic concentrations in groundwater –aim to reach resultant arsenic concentrations that are much higher than WHO’s recommended standard of 10 µg/L. This report maintains that – while this may be a pragmatic approach that helps progressively mitigate the arsenic-related health risks – it is unfortunately not a sustainable solution. Continuing exposure to higher levels of arsenic ingestion remains harmful for humans. Hence arsenic-removal technology should only be seen efficient if it can bring the water to the WHO standard. A less radical approach effectively shifts the attention from the origin of the problem in addressing the impacts and postpones achieving the best possible outcome for populations. The quantitative summary of costs and effectiveness of arsenic remediation technologies reviewed in this report can serve as a preliminary guideline for selecting the most cost-effective option. It may also be used as an initial guideline (minimum standard) for summarising the results of future studies describing arsenic remediation approaches. Looking ahead, this study identifies four priority areas that may assist in commercializing wide-scale implementation of arsenic removal technologies. These include: i) focusing efforts on determining market viability of technologies, ii) overcoming practical limitations of technologies, iii) determining technology contextual appropriateness and iv) concerted effort to increase knowledge sharing in and across regions to accelerate the implementation of research on the ground. Overall, the current science and knowledge on arsenic remediation technologies may be mature enough already to help significantly reduce the global numbers of affected populations. The missing link for today’s arsenic removal challenge is the ability to translate research evidence and laboratory-level successes into quantifiable and sustainable impacts on the ground. Achieving this requires a concerted and sustained effort from policymakers, engineers, healthcare providers, donors, and community leaders.
