Abstract:
Democracy is being challenged by low public participation in the democratic process. This has resulted in low voter turnout and is therefore called a crisis of representative democracy. As a means of increasing citizens’ engagement with their representative bodies, Information Communication Technologies (ICTs) have been implemented to encourage public online participation. In particular, the British Parliament has adopted an e-consultation system, available on its website since 2007, as an electronic tool for online participation. As e-consultation is the only system which truly enables the public to engage the legislative process, it is worth assessing this system in order to determine its effectiveness and impact. To do this, it is necessary to conduct an analysis through case studies on past econsultations. Through this analysis, an aim of this thesis is to seek new methods by which to maximise the efficiency of the e-consultation system through the consideration and application of specific research questions. Answers to specific questions were sought by means of employing empirical studies on two e-consultation cases to determine levels of public participation: the successful topic of ‘Domestic Violence’ and the less popular topic of ‘e-Petitions.’ Analysis of the two e-consultation cases reveals that e-consultation provides the public with valuable opportunity to connect with its representatives and to engage the legislative process. However, in order to maximise the efficiency of the e-consultation system, several modifications are suggested including: adequate preparation and publicity on behalf of the organising body, a high rate of MP participation, active consideration of any contributions by the committee in charge, and the establishment of a parliamentary division unit specifically for the conduction of e-consultations.

Abstract:
Telemedicine is the combination of information technology (IT) and health care and is an area that has seen increase during the last few decades due to breakthroughs in IT. Today there are still parts of the health care system that can be counterproductive with regards to the time and commitment necessary. One such part is medical consultations for private individuals where many of the simpler ones are still carried out in-person. Presented in this thesis is the implementation of a prototype telemedicine system that could be used as a substitute for in-person doctor-patient consultation. Necessary 3rd party components and their implementations into one system are described on a general use level. The result is a web application that supports login and payment via trusted sources, booking of meetings and carrying them out with video and audio. The prototype is a continuation of a telemedicine web application idea that was put on ice by a company that already provides solutions for health controls and blood analysis.
Telemedicin är kombinationen av informationsteknik (IT) och hälsovård och är ett område som har sett en ökning de senaste årtiondena på grund av genombrott inom IT. Idag finns det fortfarande delar av hälsovården som kan vara kontraproduktiva med avseende på den tid och det åtagandet som krävs. En sådan del är medicinska konsultationer för privatpersoner där många av de simplare mötena fortfarande sker i person. Presenterat i denna avhandling är implementationen av ett distribuerat telemedicinsystem som kan användas som ett substitut för de konsultationer mellan doktor och patient som sker i person. Nödvändiga tredjeparts komponenter och deras implementationer in ett system är beskrivna på en allmän användningsnivå. Resultatet är en webbapplikation som stödjer login och betalning via en pålitligt källa, bokning av möten och utförandet av dem med bild och ljud. Den presenterade prototypen är en fortsättning på en telemedicinsk webbapplikationsidé som lagts på is av ett företag som redan erbjuder lösningar för hälsokontroll och blodanalys.

The Remote Consultation and Diagnosis (RCD) in Global Picture Archiving and Communication System (Global PACS) is a unique suite of multimedia telemedicine applications developed at the University of Arizona. The applications support real-time patients' data, image files, audio and video consultation and diagnosis annotation exchanges. The RCD enables joint collaboration between pathologists, radiologists, or physicians while they are at distant geographical locations. This project provides four RCD scenarios, i.e., Case Review, Case Acquire, Store and Forward Analysis, as well as Interactive Diagnosis and Consultation. The RCD Global PACS environment consists of heterogeneous, autonomous, and legacy resources. The Common Object Request Broker Architecture (CORBA), Java Database Connectivity (JDBC), and Java language provide the capability to combine the RCD Global PACS resources into an integrated, interoperable, and scalable system. The underneath technology, including IDL, ORB, Event Service, IIOP, JDBC/ODBC, legacy system wrapping and Java implementation are explored. This distributed collaborative CORBA/JDBC based framework will challenge the advanced, medical information management requirements. It also makes the RCD Global PACS both hardware and software technologically independent. As our research and development extend, we will continue to incorporate the latest advances in computer technology. RCD Global PACS is not another new tool in telemedicine, but rather a new paradigm for the delivery of health services that requires process reengineering, cultural changes, as well as organizational changes. It is a whole new way of practicing in telemedicine. We ensure that the RCD Global PACS project has long-term, comprehensive solutions for today and tomorrow's healthcare needs.

Abstract:
This dissertation aims to use a system thinking approach to describe and evaluate the Lead-Acid Battery Recycling Program in British Columbia, compare it with other provincial regulated recycling programs and identify strategies on how it can be improved. The research is presented in the manuscript based format, comprised of four interrelated chapters. Following the introduction, chapter 2 describes a multiple regression analysis to assess how various factors identified by informed stakeholders have contributed to recycling rate in 14 transportation zones from 1995 to 2005. This study demonstrates that the existing recycling scheme ineffectively promotes recycling as it has achieved an average of 75% over the past 13 years with large fluctuations among transportation zones. The regression also shows that recycling rate of transportation zones are not highly influenced by LME lead prices and Transportation Incentive (which can be explained by the strong market power of the recycling plants responsible for setting up the price of scrap lead to which the collectors respond). Chapter 3 identifies key components that influence the performance of varied recycling systems based on a comparative analysis of provincial recycling systems informed by expert interviews. In chapter 4, comprehensive evaluation criteria for the lead-acid battery recycling program is developed based on objectives and performance measures elicited through an extensive stakeholder consultation process with various individuals and organizations. Fundamental objectives identified by stakeholders include: reduce environmental impacts, reduce occupational health impacts, reduce net costs, increase equity in resource consumption patterns and increase systematic learning. In chapter 5, we use multiple criteria decision analysis (MCDA) to design and assess effective recycling strategies to meet societal objectives previously identified in the chapter 4. Recycling strategies were compiled using the results of chapter 3. The results reveals that the optimal policy for the lead-acid battery recycling system combines a return to retailer program financed through an advanced disposal fee included in the battery price in combination with increased plant or recycling capacity domestically. This research also provides relevant contributions to the refining and application of value-focused thinking and decision analysis methodologies.

The problem of distortion due to welding has been the subject of continuous research since the early days of ship construction. The control of distortion in welded structures through preventive and corrective procedures is generally well established. However, there are rules of thumb knowledge, or heuristics, which streamline the design process and enhance the performance of the final product. Those rules of thumb are usually known to a few experts who, through their own personal involvement and experience over the years, have accumulated considerable knowledge in their field. This thesis proposes to demonstrate the feasibility of implementing such knowledge in a knowledge-based environment. The preliminary design of such a system for the minimization of distortion in welded structures is presented through its development phases. The specific areas of out of plane and bucking distortions are considered. The report consists of three major parts. One describes the distortion problem, the selected tools for its analysis and the current procedures for its prevention and correction; Two introduces the concepts inherent in knowledge-based systems and their applications in the fields of structural design and welding; and Three presents the preliminary design of DISCON (for DIStortion CONtrol) which includes the derivation of functional specifications as well as a suitable architecture for implementation. Abstract:
The contribution of consultation among the policy-makers and with their clients to the development of educational policy for the City of Hull is assessed between 1977 and 1985. In the period under review Humberside moved from a gradualist approach to dealing with falling enrolments, which entailed the retention of transfer to secondary education at 13, to a radical re-appraisal of the school system as a whole and a decision to return to transfer at 11 and the establishment of sixth form colleges. The series of consultations which accompanied the development of that policy, within the context of central government advice, is examined, together with the parallel consultations with the Church of England and among the Roman Catholic community. The principal objectives of the study are to determine the influence of client consultation on the development of policy aims and on the determination of particular policy decisions, in addition to an appraisal of the participants' perceptions of the objectives and the outcomes of the consultations studied. The methodology employed involved a document and literature search, non-participant observation, interviews with a sample of participants, and a questionnaire survey. The series of consultative episodes is subjected to examination against models of local authority decision-making derived from the literature with the purpose of generating hypotheses about the consultative process and decision-making at local education authority level. From the cases reviewed it is concluded that the role of client consultation can be both strategic and tactical, but that it is principally constrained by the political leadership's assessment of its potential role and the focus they provide for it. In the case of Humberside client consultation contributed to a change in the direction of policy in so far as the politicians used its outcomes to assess the extent of change necessary, but, when strategy had been politically determined, its potential for effecting change became tactical and was limited to modifying the application of that strategy. Consultation also made some contribution to greater public understanding of the Authority's policy and to making that policy more acceptable to those affected by it.

Abstract:
A computer interview has been found to be valuable in eliciting information. This thesis describes the use of a CIS in two different clinical settings (GP surgery and chiropractic clinic), with the aims of aiding recognition of patients with anxiety or depression and enhancing communication between patient and clinician. The Hospital Anxiety and Depression Scale (HADS) was included in the computer interview. 60 patients used the CIS in both settings, 6 GPs and 3 chiropractors were involved in the study. A high level of acceptance of the CIS was found in patients and clinicians in both settings. 99% of patients rated the system as “easy” or “extremely easy to use”. In the GP surgery, 80% of patients felt that they were “possibly” (68.33%) or “definitely” (11.67%) more focussed for the consultation. In the chiropractic clinic, 41.7% of patients said they disclosed new information and 33.3% felt better prepared for the consultation. The CIS aided the recognition of some individuals with anxiety or depression, more so within the chiropractic clinic than in the GP surgery. The information in the interview transcript was considered more useful by the chiropractors than the GPs (85% v 21.67%); this could be partially attributable to the fact that the chiropractors added 15 reassessment questions to the question set, whilst the GPs only added 4, more general, questions. The CIS also helped to highlight communication issues and show trends within the patient populations. Although the CIS was found to be of benefit in both settings, the GPs felt that it was more appropriate for use with specific patient groups. In the chiropractic clinic, the CIS was an effective addition to the periodic patient reassessment process. It would be possible to deliver other, individualised screening interviews using the CIS, delivery of which could be enhanced using handheld devices.

Introduction - Quel est le modèle de suivi à long terme adapté aux attentes et besoins des jeunes adultes guéris d’un cancer dans l’enfance ? Méthode - Cette étude a pour objectif d’évaluer l’impact sur la santé de jeunes adultes guéris d’un cancer pédiatrique d’un modèle de suivi associant auto-questionnaire et consultation médicale. De jeunes adultes ayant eu un cancer entre 1987 et 1992 en Rhône-Alpes-Auvergne ont été vus en consultation médicale et psychologique de Suivi A Long Terme (SALTO), précédée et suivie d’un auto-questionnaire. De plus, la capacité d’adaptation de leur système nerveux autonome (SNA) a été évaluée afin de déterminer les caractéristiques des sujets dont le SNA était altéré. Résultats - 207 jeunes adultes ont rempli un questionnaire pré-consultation et 197 ont déclaré avoir de nombreuses séquelles, en moyenne 3,4 séquelles, ce qui a été confirmé par l’observation médicale faite chez les 150 consultants (2,6 séquelles observées/sujet). Après la consultation, près de 70% des sujets ont modifié leur nombre de séquelles déclarées. Aucune caractéristique particulière n’a pu être clairement mise en évidence quant aux personnes ayant une capacité de régulation du SNA altérée mais une relation significative a été établie entre SNA altéré et observation d’une séquelle cardiovasculaire (P=0,049). Une enquête de satisfaction a montré une opinion très favorable à la consultation SALTO. Environ 80% des sujets présentant un trouble psychiatrique au moment de la consultation ne bénéficiaient d’aucun suivi psychologique.Conclusion - Un suivi à long terme des jeunes adultes guéris d’un cancer dans l’enfance est nécessaire, faisable et répond à une demande réelle
Introduction - What is the model of long-term follow-up adapted to the expectations and needs of youngadults cured of a childhood cancer? Method – The aim of this study was to estimate the impact of a long-term follow-up care (LTFU) associated self-questionnaire and medical consultation, on the health of young adult survivors of childhood cancer. Young adults with childhood cancer between 1987 and 1992 in Rhône-Alpes-Auvergne were seen during a LTFU medical and psychological consultation (SALTO), which was preceded and followed by a self-questionnaire. Furthermore, the capacity of adaptation of their autonomous nervous system (ANS) was estimated to determine the characteristics of the subjects whose ANS was altered. Results - 207 young adults completed a questionnaire before consultation and 197 of them declared having sequalae, on average each self-reported 3.4 late effects, which was confirmed by a medical observation made at 150 consultations (mean 2.6 observed late effects/subject). After the consultation, about 70 % of the subjects modified their number of self-reported late effects. No particular characteristic was clearly highlighted in people with an altered capacity of ANS regulation but a significant relationship was established between altered ANS and cardiovascular complication (P=0,049). A satisfaction survey showed a very favorable opinion about the SALTO consultation. Approximately 80% of the subjects presenting a psychiatric disorder at the time of the consultation had received no psychological follow-up at this time. Conclusion - Long-term follow-up of young adult survivors of a childhood cancer is necessary, feasible and answers a real need

Since the creation of the Unified Health System, through the promulgation of Brazilian Federal Constitution in 1988, the building process of regionalization and prioritization of health states needs to create mechanisms to ensure coordination between health services in order to establish it as a flow of users in a flexible and dynamic process, called the reference and counter reference. This study aimed to evaluate the implementation of the reference and counter-reference in specialized clinics of the Municipal Health System at Alfenas-MG, from the experience of the practice. This is a qualitative and quantitative descriptive study of the epidemiological type of cross-design. Initially, a review was conducted on the historical process of building the Unified Health System - SUS and the meaning of the reference system and counter-reference in this context. Next step was performed to collect data which used the chips to the routing specialist appointments used by the Municipal Health Department of Alfenas MG and the questionnaires to medical professionals. The results allowed to infer that, although under the organizing principles of SUS, the system of reference and counter reference, is still immature. However, the majority of the medical professionals agree to the necessity of its implantation by means of a computerized system for the information socialization of the attendance given to the user on different attention points and to speed the flow and reverse-flow of the user in the health attention net.
Desde a criação do Sistema Único de Saúde (SUS) através da promulgação da Constituição Federal de 1988, o processo de construção da regionalização e da hierarquização da saúde estabelece a necessidade de se criarem mecanismos que garantam a articulação entre os serviços de saúde, visando a estabelecer nele um fluxo de usuários, num processo dinâmico e flexível, denominado de referência e de contrarreferência. Este estudo teve como objetivo avaliar a implantação de um mecanismo formal da referência e de contrarreferência em consultas especializadas no Sistema Municipal de Saúde de Alfenas- MG, a partir da experiência vivenciada na prática. Trata-se de um estudo quali-quantitativo, descritivo e exploratório, do tipo epidemiológico de delineamento transversal. Num primeiro momento, foi realizada uma revisão histórica sobre o processo de construção do Sistema Único de Saúde SUS - e o significado do sistema de referência e de contrarreferência nesse contexto. Em seguida, foi realizada a etapa de coleta de dados, em que se utilizou a ficha de encaminhamento para consultas especializadas proposta pela Secretaria Municipal de Saúde de Alfenas MG e a aplicação de questionários aos profissionais médicos. Os resultados obtidos permitem inferir que, sob os princípios organizativos do SUS, o sistema de referência e de contrarreferência de consultas especializadas é ainda incipiente. Abstract:
Background: Osteoarthritis (OA) is the commonest long term condition in primary care. Current guidance suggests that much can be done to improve outcomes but existing research suggests doctors and patients are pessimistic about OA treatment. An important question concerns the role of the primary care consultation in this incongruity. This study used a combination of video recorded consultations and post consultation interviews using video-stimulated recall (VSR), to uncover what happens when patients discuss OA with their general practitioners (GPs). Methods: With ethical approval, GP consultations with 190 consenting patients aged ≥ 45 were recorded. Twenty consultations contained reference to OA, and 17 of these patients and their GPs (n=13) participated in post consultation interviews. Analysis involved thematic analysis of videotapes and comparisons of patient and GP interviews with the consultation findings. Results: Osteoarthritis arises in the consultation in complex contexts of multi-morbidity, multiple and varied patient agendas which are often not explicit, and against a background of clinician agendas including time pressures, multiple guidelines and service requirements. Dissonance between doctors and patients was observed and was often underpinned by patient perception of lack of empathy and symptom validation. Doctors and patients often adopt a ‘lay’ construct of OA where joint pain is seen as a normal part of life; this influences doctor and patient behaviour and acts as a significant barrier to formal recognition and hence treatment of the condition. Conclusions: The design of interventions to improve outcomes of patients with OA must take account of the complexity and heterogeneity of presentations in primary care. Osteoarthritis appears to be experiencing an identity crisis, with doctors and patients uncertain of what constitutes OA and when to use the term ‘osteoarthritis’. Further work is needed to identify effective ways of translating best evidence about OA management into effective primary care strategies.

Abstract:
The Nursing Coordinators (NCs) at the University of Ottawa Heart Institute (UOHI) fields phone calls from patients who have been discharged and are undergoing home care procedures at a daily basis. The project described in this thesis aims to provide tools for the Personal Digital Assistant (PDA) that the NCs can use during the phone calls. The Cognitive Work Analysis (CWA) and Ecological Interface Design (EID) approach are used to identify the information requirements to design the system. Major challenges of the telephone consultation process that are additionally identified by literature review and interviewing the NCs included visibility of patients, individual differences, and lack of standardized procedures. A combination of decision trees and visualization techniques is proposed to aid the process. Implementation of decision trees would help unload mental workload especially accesses to "knowledge in the head" as well as facilitate expert knowledge transfer to less experienced nurses. Visualization tools display integration of multiple-cues from patients in an abstract nature and can be accessed by users at any point of decision process.

Preliminary experiment with static images showed that visualization tools helped the decision makers more when the judgement tasks were more complex. The effects of different types of decision support on the cardiac nurses in simulated telephone consultation processes were examined. The system improved the performance of the decision makers and induced different types of strategic behaviours: a standardized checklist, OLDCAR, induced more through assessment, the decision algorithms induced efficient and more detailed recommendation, and the semantic network symptom map induced information gathering more relevant to diagnosis.

The research also explored methodologies to examine multi-layered decision process, where many decision makers with varying expertise are involved in modeling the strategic behaviours. This type of process can be applicable when the primary decision makers do not monitor the work domain, but can be alerted when something goes wrong.

Objetivo: Analisar a rede regional de atenção à saúde da gestante no SUS. Método: Trata-se de pesquisa de caráter transversal, quanti-qualitativa, tipo inquérito. Foram realizadas entrevistas com gestores de 28 municípios, do Ambulatório de Especialidades Médicas (AME) e do Departamento Regional de Saúde (DRS) II do estado de SP, totalizando 86 entrevistados. As variáveis pesquisadas foram: realização de atividade educativa no pré-natal, existência de protocolo de atendimento e encaminhamento, organização da atenção à saúde bucal e geral à gestante, pactuação das referências e funcionalidade do sistema de referência e contrarreferência. Realizou-se análise de conteúdo para as questões discursivas e triangulação entre as respostas obtidas nos municípios, AME e DRS. Resultado: Observou-se que 78,57% dos municípios realizam atividade educativa com gestantes; 42,86% não possuíam protocolo de atendimento implantado e em 57,14% não havia critérios de encaminhamento estabelecidos. Do total de gestores municipais, 53,57% afirmaram realizar testes rápidos para detecção de HIV, Sífilis e Hepatite B e C; 46,43% teste rápido de gravidez e 39,28% relataram avaliar a situação vacinal. Abstract: Introduction: The maternal and child health care network was established in 2011 with the purpose of promoting improvements in the efficiency of service management, but still presents a great challenge for managers. Objective: To analyze the regional health care network of pregnant women in SUS. Method: This is a cross-sectional, quantitative-qualitative research type survey. Interviews were carried out with managers from 28 municipalities, from the Ambulatory of Medical Specialties (AME) and from the Regional Department of Health (DRS) II of the state of São Paulo, totaling 86 interviewees. The variables studied were: performance of prenatal educational activity, existence of a referral protocol, organization of oral and general health care for the pregnant woman, agreement of references and functionality of the referral and counter-referral system. Content analysis was performed for the discursive questions and triangulation between the responses obtained in the municipalities, AME and DRS. Result: It was observed that 78.57% of the municipalities carry out educational activities with pregnant women; 42.86% did not have an implanted care protocol and in 57.14% there were no established referral criteria. Background People with osteoarthritis (OA) frequently consult general practitioners (GPs) for the problem but the care they receive in these consultations is often sub-optimal. The aim of the studies described in this thesis was to enhance GP clinical practice for OA by developing and implementing a model OA consultation for the initial contact between a GP and an older patient presenting with peripheral joint pain. Methods A consensus exercise was undertaken to develop the model OA consultation. This was followed by the development, and delivery in a series of workshops, of a behaviour change intervention to implement the model in practice. Impact of workshops was assessed by before-and-after methods on directly observed GP use of the model OA consultation in video-recorded consultations with simulated patients, and by self-report measures (at baseline, and one and five months after). Learner reactions and delivery in day-to-day practice were assessed. Results The model OA consultation consisted of 25 tasks for assessment and initial management. A four workshop series was developed and delivered to 24 GPs and included didactic, interactive and skills training (with simulated patients) sessions. The workshops addressed barriers and facilitators for change identified in the development of the behaviour change intervention. GP use of the model OA consultation, by 15 GPs whose video-recorded consultations were assessed, was enhanced after workshops compared with before, evidenced by increased use of 14 tasks from a median of 7 tasks before to 11 after. Impact on self-report measures was inconclusive. Learner reactions were positive but delivery in day-to-day practice was limited. Conclusion A before and after study has demonstrated that GP use of a model OA consultation in a simulated setting can be enhanced. Further research and quality improvement initiatives will be needed to enhance use of the model OA consultation in day-to-day practice.

The aim of the study was to address the issues discussed by scholars that refer to the potential and the challenges of the use of online participatory methods, and contextualize them in a case study of a web platform for public consultation. To accomplish this, first a framework was set up by conducting a literature review. This study contrasts some of the theories described in the academic literature with the empirical results of this study. Using a mixed methods approach, an online survey (N=118) and phone interviews (N=23) were conducted. The survey and interview addressed issues of satisfaction with the current experience of participating online, trade-off between transparency and privacy, governance involvement and familiarity with technology in the context of online public consultation, in the hope to learn more regarding the concerns that users have about potentially replacing more traditional methods of consultation with online methods. Based on the experience of the participants of this study, we were able to identify some of the elements that increase and decrease satisfaction when participating in public consultation online, which seems to be critical in guiding the efforts of increasing online engagement. It was also found that participants prefer features such as summary tables, graphs and pictures when presented data related to the topic consulted, and that they value having options to providing feedback on a participatory web platforms. Finally, we discuss the possibility of further research with the aims of providing participants the opportunities they look for to engage in a more nuanced way. Although this research sheds light on online behavior, it also demonstrates that this is still a field that constantly requires exploration as the interaction of internet users with participatory platforms evolves. We think that understanding how theory and practice coming into play in an empirical scenario of a public consultation web platform could help online platforms and mechanisms to not only improve online participants’ experience, but most importantly, to focus on ways to achieve the larger goals of public consultation via the Internet.

This thesis focuses on an under-researched area of tourism -the multi stakeholder, inter organisational business to business Tourism IT domain which exhibits a marked rate of failure. A critical review of B2B case studies reveals that this failure is in large part due to the primacy afforded to technical problem solving approaches over human centred ones. The main purpose of the research is therefore stated as: "how do we ensure that, as technological solutions are implemented within this domain, due consideration is given to human-centred issues?" In order to tackle this research problem an interdisciplinary approach is taken and a communicative model for stakeholder consultation is developed. At the centre of the model lies an innovative method for deconstructing and reconstructing stakeholder discourse. A Co-operative Inquiry research methodology was used and a significant number of stakeholders were engaged in an Open Space event sponsored by two major Tourism IT companies who wanted to investigate the issues and opportunities connected with travel distribution and technology. This was followed up with face to face interviews and live discussions over the internet. In addition stakeholder discourse was captured via the Travelmole tourism discussion site. The discourse between stakeholders was reconstructed and the normative and objective claims analysed in depth. The presentation of these reconstructions in textual, tabular and diagrammatic formats captures the complexity of stakeholder interactions, revealing that although IT is an important tool, what really lies at the core of multi stakeholder projects are the normative positions to which participants subscribe. The model provided a practical means for critiquing stakeholder discourse, helping to identify stakeholders both involved and affected by the issue; juxtaposing the 'is' against the 'ought'; and enabling critical reflection on the coercive use of power. The review of the tourism literature revealed that these issues are as important in general B2B tourism partnerships as in Tourism IT and in this respect the model provides a practical tool for critique and for enabling the formation of a shared normative infrastructure on which multi stakeholder projects can proceed. In addition, while borrowing from Management Science, this thesis also makes a contribution to it, specifically in the area of boundary critique, through the way in which Habermas' ideal speech criteria arc practically implemented.

Asynchronous data sources such as those associated with Space Based Radar create a unique problem for Time Division Multiplexed (TDM) Pulse Code Modulation (PCM) frame formats. The problem consists of data arrival based on external occurrences such as target tracking, and not due to sampling polls from internal sequencers. Reserved time slots for asynchronous data must be provided within the synchronous TDM telemetry stream. This increases the required bandwidth to transfer collected data to ground sites proportional to the worst case arrival rate of asynchronous data and the maximum latency allowed for the application. Asynchronous data is readily handled by the Consultative Committee for Space Data Systems (CCSDS) recommended formats without the need to increase the bandwidth disproportionately. The recommendation maintains the ability to provide synchronous telemetry data collection and transmission provided by the TDM PCM frame formats. This paper provides an implementation of CCSDS recommendations and addresses the methodology of merging asynchronous and synchronous data sources without the prerequisite increase in bandwidth associated with purely synchronous TDM approaches. Additional implementation details are provided for the implementation of a Telemetry Operation Procedure (TOP) to downlink error free telemetry frames. The TOP is not currently supported within the CCSDS recommendation. The implementation is provided through the Micro Packaged Data Acquisition and Control Systems developed by SCI Technology in Huntsville, Alabama.

This study aims to illustrate, explore and argue for school systems consultation as a dimension of educational psychology service deli very which is appropriate in the South African context. It connects with the principles which guide policy development in education, and particularly, education support services in the country. A qualitative inquiry approach was adopted, in which case study methodology was employed to illuminate systems analysis and intervention at a special school in Cape Town. The participants included all adults who were employed in the school, with focus on the management, teachers and teacher-assistants. A variety of methods were used including interviews, observation, role-play and facilitation of group process. A qualitative, thematic approach was employed in the analysis and interpretation of data, extrapolating patterns, themes and relations around communication, beliefs, change and power.

The Consultative Committee for Space Data Systems (CCSDS) is developing a set of communications protocols for Advanced Orbiting Systems (AOS). The National Aeronautics and Space Administration (NASA) and the European Space Agency (ESA) are cooperating in an effort to extensively validate these AOS protocols. This paper describes the techniques and facilities being used to perform this validation. Validation of the AOS protocols consists of (1) developing a formal specification of the protocols using a standard formal definition technique (FDT), (2) developing implementations of the protocols, and (3) remote testing of the implementations. From the FDT specifications, each agency is developing independent implementations which are consistent with the FDT specifications of the AOS protocols. Errors, omissions, or discrepancies detected during the development of the FDT specification and the implementation will be reported to the CCSDS and changes to the main specification will be suggested. The independent implementations will be extensively tested locally by the developing agency and then remotely tested through a cooperative test setup between the agencies. The implementations will interact to communicate between the agencies thus providing proof that the FDT specifications are sufficiently specific to be interpreted by everyone in the same way. Significant variations in the interpretations will result in feedback to the CCSDS and any needed changes to the main specification will be suggested. The AOS protocols are divided into four categories: Path, Space Link ARQ Protocol (SLAP), Space Link (SL), and Management. Each agency has agreed to be either the leader or support agency for each of the categories. NASA has agreed to be leader for the validation of the SLAP and SLS categories while ESA has agreed to lead in the validation of the Path and Management categories. Testbeds at the European Space Research and Technology Centre (ESTEC) in Noordwijk, Holland and at the MITRE Corporation in McLean, Virginia have been constructed for the development of FDT specifications and AOS protocol implementations. Communications facilities are being obtained which will connect these testbeds. This paper describes these testbeds, the AOS FDT specifications, the protocol implementations being developed, and the results expected from the tests performed.

This thesis compares the consultation conducted by the Law Commission ('LC') and the Australian Law Reform Commission ('ALRC'). Its first goal is to describe the process in detail, which begins with the purposes of consultation. Next, the process of consultation is described with a discussion of each of the techniques employed by the Commissions. Although there is much overlap in how the LC and the ALRC consult, they do approach the exercise differently and these differences are discussed. The description of the Commissions' consultation concludes by examining its impact -- A second goal is to compare the two Commissions' approach to consultation and this comparison is aided by the development of two models: the English Commission's expert model of consultation and the Australian Commission's more inclusive model. Underpinning the comparison between the two Commissions and these different models is the intended target of the consultation exercise. It is argued that the LC's decisions are motivated by the goal of securing expertise, more than is the case at the ALRC. By contrast, the Australian Commission is influenced more than is its English counterpart by a desire to include as many consultees as possible. An important part of this comparative study is to explain why the two Commissions consult differently. The most significant reasons are the history of two Commissions, especially the role of the founding Chairmen, and the types of projects that the Commissions undertake -- A third goal, albeit only a tentative one, is to suggest ways in which the Commissions could improve their consultation. These comments are scattered throughout the thesis, but one theme that emerged was that there seems to be insufficient thought given to a number of important stages in the consultation process.

This study is aimed at empirically examining health seeking behaviours in terms of illness response on household level at South Africa using 2007 General Household Survey and other
relevant secondary sources. It provides an assessment of health seeking behaviours at the household level using individuals as unit of analysis by exploring the type of health care provider sought, the reason for delay in health seeking and the cause for not consulting. This study also assesses the extent of dissatisfaction among households using medical centres and this factor in health care utilisation is considered as the main reason for not consulting health care services. All the demographic and health seeking variables utilised in this study are controlled for medical aid cover because it is a critical variable in health care seeking. Therefore this study makes distinction on illness reporting and they type of health care consulted by medical aid holders and non medical aid holders. Statistical analyses are conducted to explore and predict the way in which demographic variables and socio economic variables affect health care seeking behaviours.

Educational Psychology has been defined in the past as an interventionist profession, mainly focused on the alleviation of educational problems that interfere with the educational process. Currently a paradigm shift seems to herald new opportunities for the profession to develop whole school approaches and devote less of its time to individual case work. Educational psychologists world-wide foresee a broadening in the focus of their work to include a larger commitment to consultancy and school systems development. The benefits are seen in terms of the more efficient use of time, eliciting wider ranging change and the opportunity for offering a preventative approach to service delivery. School systems consultation represents such an approach. This study explores the practice of school systems consultation using an action research approach. It is argued that action research methodology provides a framework to contain and guide the complex developments which emerge once the consultation gets under way. Central concepts in an action research approach such as collaboration, participation and reflection are analysed and reflected upon in terms of the opportunities and constraints they presented to school systems consultation as a service delivery option for the educational psychologist. The role of the educational psychologist in South Africa has been conservative and the shift towards a more socially accountable one seems inevitable if such services are to be regarded as appropriate. The role of the educational psychologist as change agent in the transformation process in the South African education system is explored in this study.

During the current decade, international cooperation in space projects has become more and more popular and this trend is increasing. Initially, this involved only single missions with agencies flying payloads on other agencies' spacecraft. Later, this trend continued with international ventures, involving different agencies. In the immediate future, even more challenging scenarios are foreseen. The best known example and prime driver for such sophisticated missions will be the Space Station Freedom and its participating partners' spacecraft. Some of the international missions (ESA missions) are described briefly in this paper, in order to set the scene for a better understanding of the complex needs for standards within advanced orbiting systems. These ventures call for efficient means for cooperation and interoperability. Part of these requirements can be met by following international standards for space communications and space data systems. The Consultative Committee for Space Data Systems (CCSDS) undertook the task of integrating the space data systems requirements and developing appropriate recommendations for data systems standards for these Advanced Orbiting Systems (AOS). All international partners in the Space Station Freedom Program participated in the definition, development, and review of the AOS recommendations. The need for better cooperation in space communications via data relay satellite prompted the formation of a three party international panel called the Space Network Interoperability Panel (SNIP). An important aspect is the need for verification and validation of the concept and of the detailed technical recommendations. For the immediate future, special compatibility campaigns, involving the international agencies are planned in order to ensure the smooth application and functioning of the AOS recommendations.

In a national context of transferring their competences to intercommunal institutions, reducing their resources and weakening of their regulatory prerogatives, local elected representatives in small cities, and their technical services, are faced with issues that often exceed their capacity for action. The implementation of a participatory urban project requires a paradigm shift in the political and professional cultures they have inherited. It clearly makes it necessary to rethink their roles and modalities of intervention.Based on the experience of an architectural and urban planning agency, this thesis analyses the place and the modalities of action of the elected representative as "Occasional Contracting Authority", for urban projects organized with significant and unusual participatory ambitions for the community. On the basis of three case studies, it describes and interprets the difficulties encountered by the small city and its accompanying structure. Relying on an approach of "observant participation" and on the Theory of Social Regulation, this research examines the conflicts and negotiations within the system of actors implied in the urban project in which the local elected is involved

A regulação de consultas especializadas tem se mostrado como uma das áreas mais problemáticas do Sistema Único de Saúde (SUS) no Brasil. Cabe aos gestores de saúde nos municípios, estados, e governo federal, estabelecerem mecanismos de regulação coerentes com o volume de recursos disponíveis e com o contingente populacional a atender. Diversas centrais de regulação para atendimentos especializados foram implantadas nas secretarias municipais de saúde e sistemas de informação foram criados como ferramentas para apoio a estas centrais. Seu escopo tem sido progressivamente ampliado, de maneira a incluir uma visão crítica das necessidades da população em relação à capacidade de atendimento dos prestadores de serviço. No processo de regulação de consultas especializadas, duas questões têm-se destacado: (1) para um dado caso, quais pacientes têm maior prioridade de atendimento, e (2) quais prestadores de serviço podem resolver melhor o caso? Fundamentado nestas duas questões, e a partir da consideração dos requisitos legitimados na área da assistência à saúde, este trabalho propõe um sistema para apoio à decisão de agendamento de consultas especializadas para servir às centrais de regulação. O sistema proposto integra análise de decisão multi-critério e programação linear para o agendamento das consultas, onde a alocação dos pacientes é definida em função da relevância relativa de um conjunto de critérios relacionados à noção de efetividade da assistência médica especializada e da capacidade de atendimento das unidades de assistência credenciadas. Da integração destes modelos resulta uma representação que leva em conta simultaneamente os aspectos relacionados ao diagnóstico médico e suas conseqüências na vida do paciente, os aspectos relacionados às instalações e processos disponíveis nas unidades assistenciais credenciadas, e os aspectos relacionados à dificuldade de acesso do paciente a estas unidades. O uso do sistema permite que as informações pessoais e médicas do paciente, assim como as informações sobre as unidades assistenciais, sejam incorporadas em um modelo de programação linear de maneira a maximizar a efetividade do conjunto de solicitações para cada especialidade. Os modelos foram implementados em um sistema informatizado, e aplicados em uma parcela dos serviços da Secretaria Municipal de Saúde de Porto Alegre para as especialidades de cardiologia e cirurgia vascular. O sistema e os resultados obtidos foram validados por um grupo de peritos, que confirmou a viabilidade do uso deste modelo como uma ferramenta para a otimização da alocação de recursos no atendimento especializado pelo SUS.
The regulation of specialized medical consultations has been one of the most problematic areas of the Government Unified Health System (SUS) in Brazil. It is the role of health managers from cities, states and federal government to establish coherent mechanisms of regulation with the amount of available resources and the population contingent to be assisted. Many regulation centers for specialized consultations had been created in public city health departments, and many information systems were developed to support these centers. Its target has been gradually extended, in a way to include a critical vision of the necessities of the population in relation to the capacity of attendance of the service rendering. On the specialized consultations regulatory process, two questions arise: (1) for a random situation, which patient has priority to be assisted? (2) Which health providers can better solve this problem? Based on these two questions, and from the consideration of the legitimated requirements in health care, this work considers a decision support system for the scheduling process of specialized consultations into regulation central offices. The considered system integrates multi-criteria analysis and linear programming for the scheduling process, where the allocation of the patients is defined in function of the relative relevance of a set of criteria related to the notion of effectiveness of the specialized medical assistance and the capacity of assistance of the credential service providers. By the integration of these models, a representation results that simultaneously takes in account the related aspects to the medical diagnosis and its consequences in the patient’s life, the aspects related to the installations and available processes in the credential assistance units, and the aspects related to the difficulty of access of the patient to these units. The use of the system allows that the personal and medical information of the patient, as well as the information on the assistance units, are incorporated in a model of linear programming in a way to maximize the effectiveness of the set of requests for each specialty. The models had been implemented in a decision support system, and applied in a parcel of the services of the Porto Alegre City Health Department for cardiology and vascular surgery. The information system and the outgoing results were validated by a group of experts, which confirmed the model viability for using as a tool to improve the resource distribution at the SUS's specialized assistance.

La deuxième question porte sur la complémentarité ou la substitution concernant des relations entre les médecins généralistes et les spécialistes. Le troisième projet examine la manière dont les patients ont réagi à la réforme du parcours de soins en France en termes de conformité, et un examen préalable des facteurs qui affectent cette conformité. La 2e et 3e articles examine les aspects liés à la variation géographique aussi. Dans mon quatrième article, je présente un protocole d'étude rédigé comme un suivi du premier article
This thesis examines the role of specialist referrals, as they have implications for patient outcomes and the organization and sustainability of health systems. The first project specifically examines the opinions of specialists in infectious diseases in a French tertiary hospital (hospitalized) from the point of view of the payer. The second project examines whether the relationship between General Practitioners (GPs) and specialities is complementary or substitutable. The third project examines the way in which patients reacted to the reform of the care pathway in France in terms of compliance, and a preliminary examination of the factors that affect this compliance. The 2nd and 3rd articles examine aspects related to geographic variation as well. In my fourth article, I present a study protocol written as a follow-up to the first article

Traditional management-leadership and organizational literature depicts the individual as conflated with their role and instructs them to handle their employees and the situations that arise daily and over the course of business cycles instrumentally and for the purposes of control and productivity. This more traditional and mainstream literature does not adequately address, if at all, the unconscious factors influencing people or the management situations in which they find themselves. Using a model based upon the theory of the Swiss psychologist Carl Gustav Jung and the relationship of consciousness to the unconscious, this dissertation looks at situational management cases and reveals the existence of the unconscious in the midst of our strongest claims to rationality. Present and active, the cases show the unconscious to be a significant factor in creating subjective meaning and ordering our world even in the most "rational" moments of our lives in public administration. They further describe how it is that the individual in the manager-leader role is implicated in and caused by the very situations they are attempting to manage and the way in which acknowledging and relating to the unconscious provides an additional resource for public managers.

The title of the research is Sustainability Performance Evaluation Management Systems Model for Individual Organizations and Supply Chains. This research has achieved its aim to develop and demonstrate the practical implementation of a simple and objective sustainability performance evaluation management system model for individual organizations and supply chains. It has resulted in the recognition that a new concept – Network of Interested Partners – underpins the achievement of sustainability. The term acknowledges the interdependence and reflects the essential cooperation that must be achieved between business organisations, their commercially related entities and the local community if progress towards sustainability is to be achieved. It therefore encompasses and extends the concept of a supply chain as currently used. Sustainable Development is defined by the World Commission on Environment and Development as development, which meets the needs of the present without compromising the ability of future societies to meet their own needs. Organizations, as part of human activities, also have to be sustainable. The sustainability of organizations is directly linked to the continual improvement of business performance. Many organizations have found a way to improve performance through the establishment of management systems. International Organization for Standardization (ISO) standards are recognized worldwide as reliable and efficient tools for the implementation of management systems. However, they do not always result in the desired improvement in outcomes. Therefore, if the required improvement of business performance is to be achieved, improved methodologies for development and implementation of performance evaluation (PE) processes are necessary. These methodologies must take into consideration sustainability principles. They also have to be applicable to individual entities and supply chains, with or without management systems in place. Supply chains are important because it is being increasingly recognized that overall supply chain performance is a means of adding value and competitive advantage to all businesses. In the first part of the research a performance evaluation model or PE (version 1 model) was developed. This was used as criteria to compare and evaluate existing performance evaluation processes and outcomes of individual organisations and their respective supply chain within the Gladstone region, Australia. Questionnaires have also been used to identify and evaluate the needs of the interested parties in relation to the organizations’ and supply chains’ business performance and processes of performance evaluation. All the information provided in the first part of the research was used by the researcher to develop the Sustainability Performance Evaluation Management Systems model or SPEMS (version 2 model). This incorporates the concepts of Network of Interested Parties/Partners. In particular, one of the outcomes is recognition that organizations need to establish partnerships if effective supply chain performance improvement is to be achieved. Therefore the establishment of partnerships has become a key requirement for the implementation of SPEMS. The establishment of partnership among participants of a supply chain of Gladstone and implementation of the eight first steps of the SPEMS (version 2 model) in this supply chain was commenced successfully through workshops. The supply chain was formed by commercial organizations, government entities and interested parties from the community. SPEMS requires that partners all have the same level of ownership and authority in the decisions of the supply chain. Some new terms and their definitions have been created within the research to support the new SPEMS model. They include: Network of Interested Partners, sustainability for organizations, sustainability KPI and sustainability friendly organizations. All of the above are encompassed within the final SPEMS (version 3 model).

Because pathologists are rare in Taiwan, the patients’ pathological sections need to be delivered to the pathology department in the teaching or central hospitals. All pathological sections must be consulted by the pathologists and than generated the pathology reports. Base on these reports, the clinical doctors can judge whether perform a surgical operation or not. On the other hand, DICOM 3.0 standard already standardize the format of medical images as CT, MRI, Ultrasound. Since pathology images are too complicated to analyze, and the volume of one section’s all magnifying power image is very great, there is no appropriate solution up to today. In this thesis we discuss the implementation and use of a system, which can collect all of the pathological section images automatically and rapidly. For digitizing purpose, they will be saved as DICOM VL format. The System includes two parts: Automatically move and snap on LabVIEW interface, database consultation system on a WWW browser. The technologists can control the microscopy auto-stage either manual or let the program automatically snap specific magnifying power’s image, the program also provides the review of the snapping images. After collect all images, the program will save the images as DICOM VL format into the pathology database. Pathologists who in distance place can diagnose and write reports by judging from the collection of all images in the database on a WWW browser. We also discuss the pathology sections’ volume and characteristic in the thesis.

Furthermore, CPR also can present distinct phase according to different demand even though data are distributed. Additionally, users can reduce mistakes because CPR is easy to identify. After patient record is computerized, the speed of acquiring and quality of service will be improved, the space and manpower of patient record room can be saved. Therefore, in order to handle huge outpatient services and to enhance quality of medical treatment, CPR is a trend in future. The demands trend of hospitals using network to exchange data are increasing recently, therefore the development of CPR will help to access data quickly. As the technologies of network and multimedia develop vigorously, it is growing up day by day with the application of multimedia in transferring data and user interface. The hospitals hope to increase efficiency and save costs of communication. The doctors need to get a fully patient record quickly to help diagnosing, and then improve quality of medicine. The processing of data transmission needs to follow a standard protocol and its primary goal is to provide standards for medical data exchange, to simplify custom interfaces in programming, and to share the medical information. In the study, primary goal is to provide an effective electronic consultation agent system. It can provide a friendly user interface with multimedia and retrieve data about CPR and medical image. Finally, this will be recommended as a reference for relative studies.

Although our living standard and dietary intake become better and better, many people suffer from civilization diseases, for example, diabetes, hypertension, and obesity, etc. To make these patients get effective control, balanced nutrition in diet and regular exercise are important daily practices. Therefore, nutrition counseling plays an important role. Since most nutritionists are often overloaded with too many clients to make effective consultation, we develop a system to provide better analysis and supervision functions. This system can not only help nutritionist in consultation but also assist customers to record their physiological data, dietary intake, and exercise information in the system. In the proposed nutrition consultation and recommendation system we employ the data mining technology of predicting and finding suitable patterns to assist nutritionists develop personalized, comprehensive nutrition programs for clients. Decision tree analysis is used to find the matching pairs between customers’ physiological statuses and their proper diet programs. As the system decision maker, nutritionist will perform the final assessment. After the decision is approved, the dietary recommendation is given to the client. Follow-ups are tracked to ensure the effectiveness of the program execution before the consultation is complete. In order to increase the efficiency of the system performance and storage requirement, our implementation is in the environment of cloud computing. Thus, nutritionists and clients can use the system anytime in any places.

Automation is one of the most important approach that may be able to reduce skill labor requireement and improve productivity. TO assist management in making better decisions concerning automated consturction equipment selection, this research utilizes a structured system analysis approach to build proto-type system on an environment with Macintosh IIvx and 4th Dimension. his proto-type consists of a database system for managing data related to construction equipments and some application programs for using these data ffectively. Based on net present value method, an analytic framework built on top of thisdata base system is alsi illustrated to help management in evaluating andcomparing current existed equipments. This proto- type system has been tested andverified by experienced engineers from RESA. Further tuning and tailering may bemeeded implementing in a different system environment.
